As the first step in image interpretation is detection, an error in perception can prematurely end the diagnostic process leading to missed diagnoses. Because perceptual errors of this sort-"failure to detect"-are the most common interpretive error (and cause of litigation) in radiology, understanding the nature of perceptual expertise is essential in decreasing radiology's long-standing error rates. In this article, we review what constitutes a perceptual error, the existing models of radiologic image perception, the development of perceptual expertise and how it can be tested, perceptual learning methods in training radiologists, and why understanding perceptual expertise is still relevant in the era of artificial intelligence. Adding targeted interventions, such as perceptual learning, to existing teaching practices, has the potential to enhance expertise and reduce medical error.

Humans perceive millions of colors along three dimensions of color space: hue, lightness, and chroma. A major gap in knowledge is where the brain represents these specific dimensions in cortex, and how they relate to each other. Previous studies have shown that brain areas V4 and the posterior inferotemporal cortex (PIT) are central to computing color dimensions. To determine the contribution of V1 to setting up these downstream processing mechanisms, we studied cortical color responses in macaques—who share color vision mechanisms with humans. We used two-photon calcium imaging at both meso- and micro-scales and found that hue and lightness are laid out in orthogonal directions on the cortical map, with chroma represented by the strength of neuronal responses, as previously shown in PIT. These findings suggest that the earliest cortical stages of vision determine the three primary dimensions of human color perception.

Commonly conflated with sleepiness, fatigue is a distinct multidimensional condition with physical and mental effects. Fatigue in health care providers and any secondary effects on patient care are an important societal concern. As medical image interpretation is highly dependent on visual input, visual fatigue is of particular interest to radiologists. Humans analyze their surroundings with rapid eye movements called saccades, and fatigue decreases saccadic velocity. Oculomotor parameters may, therefore, be an objective and reproducible metric of fatigue and eye movement analysis can provide valuable insight into the etiology of fatigue-related error.

The response dynamics of neurons in the visual pathway are driven, in part, by the dynamics of lateral inhibitory networks. Illusions of invisibility, such as the visual masking illusions, in addition to the dynamics of visibility itself, can be explained by the actions of such networks. Here we provide a descriptive model of a lateral inhibitory network in space and time. We provide physiological evidence that neurons in the early visual system of primates respond in a fashion predicted by these temporal dynamics. Furthermore, we discuss how the network predicts the existence of novel visual illusions and their physiological correlates.

We have shown that application of an inhibitor of the enzyme nitric oxide synthase (NOS) effectively suppresses the visual responses of relay cells in the dorsal lateral geniculate nucleus (dLGN) of the anaesthetized paralysed cat. Such suppression seems to result from a specific reduction in transmission via N-methyl-D-aspartic acid (NMDA) receptors, since iontophoretic application of the inhibitor of NOS selectively and in a dose-dependent manner decreased the responses to exogenously applied NMDA. Responses to other exogenously applied amino acid agonists, such as quisqualate (Quis), kainate (Kain) and alpha-amino-3-hydroxy-5-5-methyl-4-isoxazole-propionic acid (AMPA) were largely unaffected. Furthermore, the excitatory action of acetylcholine (ACh), normally co-localized with NOS in axonal terminals within the dLGN arising from the brainstem, was also unaffected. Unlike some other actions of nitric oxide (NO), this role seems not to involve an increase in production of cyclic guanosine-3',5'-mono-phosphate (cGMP), since application of the membrane permeable cGMP analogue 8-bromo-cGMP did not alter the suppressive effect of NOS inhibitors on either visual or NMDA evoked responses. We conclude that the normal function of NO at this level of the visual system is permissive, allowing full expression of NMDA mediated visually elicited information.

SummaryOur eyes move continually, even while we fixate our gaze on an object. If fixational eye movements are counteracted, our perception of stationary objects fades completely, due to neural adaptation. Some studies have suggested that fixational microsaccades refresh retinal images, thereby preventing adaptation and fading. However, other studies disagree, and so the role of microsaccades remains unclear. Here, we correlate visibility during fixation to the occurrence of microsaccades. We asked subjects to indicate when Troxler fading of a peripheral target occurs, while simultaneously recording their eye movements with high precision. We found that before a fading period, the probability, rate, and magnitude of microsaccades decreased. Before transitions toward visibility, the probability, rate, and magnitude of microsaccades increased. These results reveal a direct link between suppression of microsaccades and fading and suggest a causal relationship between microsaccade production and target visibility during fixation.
