25. Comparison of YEARS and Adjust-Unlikely D-dimer Testing for Pulmonary Embolism in the Emergency Department

Abstract

We prospectively assessed the diagnostic accuracy of YEARS and a modified age-adjusted clinical decision rule ("Adjust-Unlikely") for pulmonary embolism (PE) testing in the emergency department.This study was conducted in tertiary care Canadian emergency departments. When the D-dimer was500 ng/ml, PE was excluded. Pulmonary imaging for PE was performed when the D-dimer was ≥500 ng/ml. Patients were followed for 30 days, and PE outcomes were independently adjudicated. Physicians systematically recorded the presence or absence of YEARS items (PE most likely, hemoptysis, signs of deep venous thrombosis) prior to D-dimer testing and imaging. We analyzed the diagnostic accuracy of YEARS and the "Adjust-Unlikely" rule. Age adjustment (age x 10 in those50 years old) was applied in patients where PE was not the most likely diagnosis and 500 ng/ml threshold when PE was most likely.One thousand seven hundred three patients were included, median age 62 (50, 74), 58% female, PE prevalence 8.0%. YEARS sensitivity for PE diagnosis was 92.6% (87.0, 96.0%) and specificity 45.0% (42.5, 47.5%). Adjust-Unlikely sensitivity was 100.0% (97.2, 100.0%) and specificity 32.4% (30.1, 34.8%). Posttest probability of PE in the group of patients with PE excluded by D-dimer between 500 ng/ml and the adjusted limit was 2.8% (1.6, 5.1%) for YEARS and 0.0% (0.0, 2.6%) for the "Adjust-Unlikely" rule.The "Adjust-Unlikely" rule would modestly reduce imaging and identify all cases of PE. YEARS would substantially reduce imaging but miss 1 in 14 cases of PE.

Abstract

Avian influenza virus (AIV)is easy to cause diseases in birds and humans.It causes great economic losses to the poultry farms and leads to public health problems. Using vaccines is the main approach to control the prevalence of AIV. In our previously published article, a recombinant Lactobacillus plantarum (L. plantarum) expressing the NP-M2 peptide ofH9N2 AIV was generated, and its protective effect was evaluated in a chicken model. In this study, the protective effect was estimated in mice model. Humoral and cellular immune response parameters were measured using flow cytometry adding to body weight loss, survival rate, virus load, and histopathological changes in the lung. The obtained results elucidated that, the recombinant L. plantarum can promote the activation of dendritic cells (DC), proliferation of T and B cells adding to eliciting protective secretory IgA (sIgA) and humeral IgG level in mice model. Accordingly, it could be used as a patent vaccine to control the AIV infection.

Abstract

Collagen fibril hydrogel (CH), with controllable micro-structure, sufficient modifying sites and excellent biocompatibility, has received widely attention in the regulation of biomacromolecules. Herein, dialdehyde carboxymethyl cellulose (DCMC) in different -CHO contents and molecular weights demonstrated two types of cross-linking behaviors to CH, 'limited and long-range' or 'multiple and short range' cross-linking, corresponding to -CHO content ranged from 0 to 53 % and 53 to 90 %, respectively. In regard of structure, non-destroying effect of DCMC on collagen was supported by FT-IR and XRD analysis. CH cross-linked by DCMC (CH-DC) showed declining porosity and aggregating fibrils as -CHO content of DCMC rising. In regard of physicochemical properties, DCMC with53 % -CHO strengthened the hydrophilicity, thermal stability and degradation resistance of CH-DC. Also, there was 110 % growth on gel strength, 86 Pa enhancements on storage modulus, and 4.6 times decrease on the swelling ratio of CH-DC. Results indicated that DCMC with 79 % -CHO remarkably improved the physicochemical properties of CH via developing sufficient Schiff-base bonds with collagen fibril in a short distance. This study distinguished two patterns of DCMC cross-linking from physicochemical view. In other words, DCMC is potential to meet the requirement of protein-based materials with different expectations by adjusting its -CHO content and molecular weight.
