We performed a retrospective analysis of the patients assessed at our memory unit for whom Alzheimer disease (AD) cerebrospinal fluid biomarker results were available. We selected patients diagnosed with mild cognitive impairment due to AD (National Institute on Aging-Alzheimer’s Association clinical criteria), confirmed neuropsychological deficit, a Global Deterioration Scale score of 3, and an abnormal profile of cerebrospinal fluid biomarkers. Of the 588 cases reviewed, 110 met the inclusion criteria. During follow-up, 50 cases (45.45%) progressed to dementia due to AD. Baseline levels of total and phosphorylated tau were higher in the group of patients that progressed to dementia than in those remaining with mild cognitive impairment. After adjusting for age, sex, history of hypertension, diabetes, and educational level, a 10% increase in total tau protein values was associated with a 7.60% increase in the risk of progression to dementia (hazard ratio: 2.22; 95% confidence interval, 1.28–3.84]; P = .004). Among patients with mild cognitive impairment due to AD and abnormal cerebrospinal fluid biomarker profiles, progressively higher concentrations of total or phosphorylated tau were associated with increased risk of progression to dementia. Resumen: Realizamos un análisis retrospectivo de los pacientes evaluados en nuestra unidad de memoria en los que se realizó determinación de biomarcadores licuorales de enfermedad de Alzheimer (EA). Se seleccionaron aquellos casos con diagnóstico de deterioro cognitivo leve debido a EA según criterios clínicos (criterios NIA-AA), déficit neuropsicológico comprobado, una puntuación igual a 3 en la escala GDS y un perfil alterado de biomarcadores en líquido cefalorraquídeo. De los 588 casos revisados, 110 cumplieron los criterios de inclusión. Durante el seguimiento, 50 de estos 110 casos (45,45%) progresaron a demencia por EA. Se observaron diferencias significativas en los niveles basales de tau total y tau fosforilada entre los casos que evolucionaron a demencia y los que permanecieron estables como deterioro cognitivo leve, siendo los niveles más altos en el grupo que progresó a demencia. Después del ajuste por edad, sexo, antecedentes de hipertensión, diabetes y nivel educativo, un aumento del 10% en los valores de proteína tau total se asoció con un aumento del 7,60% en el riesgo de progresión a demencia (HR = 2,22, IC 95% [1,28, 3.84], P = .004). En pacientes con deterioro cognitivo leve debido a EA un perfil alterado de biomarcadores licuorales, concentraciones progresivamente mayores de tau-t y tau-p se asocian a un mayor riesgo de conversión a demencia.

Introduction: The diagnostic paradigm of Alzheimer's disease (AD) is changing; there is a trend toward diagnosing the disease in its early stages, even before the complete syndrome of dementia is apparent. The clinical stage at which AD is usually diagnosed in our area is unknown. Therefore, the purpose of this study is to describe the clinical stages of AD patients at time of diagnosis. Methods: Multicentre, observational and cross-sectional study. Patients with probable AD according to NINCDS-ARDRA criteria, attended in specialist clinics in Spain, were included in the study. We recorded the symptom onset to evaluation and symptom onset to diagnosis intervals and clinical status of AD (based on MMSE, NPI questionnaire, and CDR scale). Results: Participants in this study included 437 specialists representing all of Spain's autonomous communities and a total of 1707 patients, of whom 1694 were included in the analysis. Resumen: Introducción: Estamos asistiendo a un cambio en el paradigma del diagnóstico de la enfermedad de Alzheimer (EA), de modo que tiende a realizarse en fases más precoces de la evolución, incluso antes de la aparición del síndrome completo de demencia. En nuestro entorno no se conoce en qué situación clínica se está realizando el diagnóstico de la EA. Por ese motivo, se ha llevado a cabo este estudio, para describir el estadio evolutivo de los pacientes con EA en el momento del diagnóstico. Métodos: Estudio multicéntrico, observacional y transversal. Se incluyeron pacientes que cumplían criterios NINCDS-ARDRA de EA probable, atendidos en consultas de Atención Especializada en España. Se recogieron los datos sobre los tiempos asistenciales y de evolución de la EA según el MMSE, el cuestionario NPI y la escala CDR. Resultados: Participaron 437 especialistas de todas las Comunidades Autónomas, que incluyeron un total de 1.707 pacientes, de los cuales 1.694 fueron incluidos en el análisis. The relevance of museums of anatomy and pathology lies both in the conservation of anatomical specimens and their excellent reproductions and their use in education and research. The teaching of anatomy dates from ancient times, originating in the Theatrum Anatomicum and anatomical cabinets, located in the anatomy lecture rooms of European medical schools. These were followed by museums of anatomy and pathology in successive centuries. However, after a golden period in the XVIII century, there was a progressive decline which eventually led to a dramatic loss of many museums. Currently, there is a growing interest in the recovery and importance of these collections. We present an historical approach to their development and a review of the current situation in the principal anatomical museums of anatomy in Spain and the rest of Europe., (Copyright © 2018 Sociedad Española de Anatomía Patológica. Objective: To evaluate the contribution of 11 C-Methionine PET in the early differentiation between tumour recurrence and radionecrosis in patients treated for a high grade glioma., Method: The study included 30 patients with glioma (III/IV grade) treated with surgery/radiotherapy/chemotherapy (5-8 months) and with an indeterminate MRI. All patients underwent a 11 C-Methione PET (within 15 days of MRI) and studies were visually analysed (intensity and morphology of uptake), quantified (SUV max/SUV mean background), and coregistered to MRI (3D-Flair). Patient management was decided by the neuro-oncology committee to clinical and imaging follow-up, second-line treatment, or surgery., Results: There were 23 11 C-Methionine PET studies visually positive. Morphology of uptake was focal in 15, diffuse in 4, and ring-shaped in 4. Three out of the focal uptake cases underwent resection (Histopathology +). Sixteen underwent second-line therapy (11 responded; 5 progressed). The 4 cases with ring-shaped uptake were followed-up, and progression was found in 2 (true-positive), and disease-free in 2 (follow-up of 6 and 7 months, respectively) (false-positive). Seven out of 11 C-Methionine studies PET were visually negative, and all of them were disease-free (follow-up of 3-12 months). SUV lesion/background was 2.79±1.35 in tumour recurrence, and 1.53±0.39 in radionecrosis (P<.05). Taking into account a SUV lesion/background threshold of 2.35, the sensitivity and specificity values were 90.5% and 100%, respectively., Conclusion: Visual analysis, quantitative and PET/MRI coregistration of 11 C-Methionine PET showed their complementary role in patients with indeterminate MRI results, thus allowing early differentiation between tumour recurrence and radionecrosis, and helping in the individual therapy approach., (Copyright © 2016 Elsevier España, S.L.U. y SEMNIM. Objective: Demonstrate the role of the virulence factors of Helicobacter pylori, the sex, and tobacco and alcohol use in the development of peptic ulcer in patients infected by H. pylori., Patients and Methods: One hundred and seventy-four patient with dyspepsia were studied after they went consecutively for gastroscopy. The diagnosis of infection by H. pylori was carried out by culture and/or histology. Through western-blot the presence of specific anti-H. pylori antibodies was determined. The tobacco and alcohol use data were collected., Results: One hundred and twenty-five patients (32 with ulceration, 12 with duodenitis and 81 with functional dyspepsia) showed H. pylori infection with anti-bacterium IgG antibodies. The detection of the proteins CagA and VacA, male sex and heavy consumption of alcohol and tobacco were associated with the finding of peptic ulcer. In the multivariate analysis only anti-CagA antibodies (OR: 4.1; 95% CI: 1.1-15.3; p = 0.036), anti-VacA (OR: 3.9; 95% CI: 1.4-10.8; p = 0.009) and male sex (OR: 3.1; 95% CI: 1.2-8.3; p = 0.02) were associated with the ulcerative disease., Conclusions: The proteins CagA and VacA, and the male sex, contribute independent risk factors for peptic ulcer in patients infected by H. pylori.

Introduction: The aim of this study was to determine the relationship among antibodies against virulence factors (CagA and VacA), clinical status and primary resistance in dyspeptic patients with Helicobacter pylori infection., Methods: Ninety-eight adult patients with Helicobacter pylori infection who underwent gastric endoscopy for dyspepsia were studied. Specific serum IgG antibodies against CagA and VacA proteins were detected by Western-blot (Helicoblot 2.0). Minimum inhibitory concentrations of metronidazole, amoxicillin, tetracycline and clarithromycin were determined with the E-test., Results: Thirty-nine patients presented peptic ulcer disease and 59 had non-ulcer dyspepsia. CagA protein was detected in 63 patients, and VacA protein in 52 subjects, and both were significantly associated with peptic ulcers (p 5 0.034 and p 5 0.029, respectively). Susceptibility results showed 38.8% of strains resistant to metronidazole and 10.3% resistant to clarithromycin. No resistance to amoxicillin or tetracycline was found. Susceptibility to clarithromycin was more frequent in ulcer patients than in non-ulcer dyspepsia patients (p 5 0.046). CagA protein was more frequent in patients with clarithromycin-sensitive strains (p < 0.001)., Conclusion: Antibodies against CagA protein were associated with higher antibiotic susceptibility in patients with ulcers or non-ulcer dyspepsia. Thus, anti-CagA antibody detection could be a useful marker of favorable prognosis with antibiotic treatment.

Introduction: [corrected] Perioperative cardiovascular complications are an important cause of post-surgical morbility and mortality in patients undergoing major vascular surgery. Dobutamine Stress Echo is considered one of the methods of choice in the detection of coronary artery disease in this subgroup of patients., Objectives: . Our aim was to analyze if dipyridamole stress echocardiography could be used as an alternative to Dobutamine Stress Echo in the perioperative evaluation of patients in need of major vascular surgery., Patients and Method: The result of consecutives dypiridamole and dobutamine stress exams prior to vascular surgery were reviewed. We analyzed if those patients with a positive stress echo presented a higher number of cardiac events during and after surgery than those with negative stress echo. The negative and positive predictive values were calculated for both techniques., Results: 133 stress exams were analysed: 39 with dobutamine and 94 with dipyridamole. Of the 39 dobutamine studies 2 were positive, 29 negatives and 8 non conclusive. Of the 94 dypiridamole studies 13 were positive and 81 negatives. None of the patients with a positive dobutamine echo underwent surgery. The negative predictive value for dobutamine echo was 96.5%, quite similar to that of dypiridamole stress echo (97.5%)., Conclusion: Dipyridamole stress echocardiography is a valid alternative to dobutamine echocardiography in the pre-surgical evaluation of patients undergoing major vascular surgery.

Scytalidium dimidiatum dermatomycosis are usually reported in tropical and subtropical countries. Some cases in Spain have been diagnosed due to the increasing number of immigrants from these areas. We herein describe three new cases of S. dimidiatum infections detected in Madrid. Two patients were from Guinea Ecuatorial and the third from Angola. We also report the first case of S. dimidiatum infection in a HIV patient in Spain. Clinical and epidemiological characteristics of S. dimidiatum infections reported in Spain are discussed.

Background: Microsporidia are opportunistic parasites which, due to their morphologic characteristics, continue presenting diagnostic problems. Species-specific identification of microsporidia has become important because of varying levels of response to albendazole, which is the only effective treatment for some kinds of intestinal microsporidiosis. Although these parasites cause up to 50% of otherwise unexplained chronic diarrhea in HIV-positive patients, the number of reported cases is still very scarce in our country when compared to the existing HIV-positive population., Methods: Intestinal microsporidiosis in HIV-positive patients with diarrhea was investigated using the modified trichrome staining technique. Microsporidia species identification was done by indirect immunofluorescence (IIF) and polymerase chain reaction (PCR) with specific primers., Results: Six new cases of intestinal microsporidiosis caused by Enterocytozoon bieneusi were diagnosed in Madrid (Spain). All patients were in an advanced state of the HIV infection and they presented CD4+ values equal or inferior to 100 x 10(6)/I., Conclusions: Due to the number of cases that are accumulating, microsporidia must be included among the enteropathogens responsible for chronic diarrhea in HIV-positive individuals in Spain. The PCR technique using specific primers is a suitable determinator of the microsporidia species implicated in this intestinal pathology.

Background: Most of the latest clinical and epidemiologic studies indicate that microsporidiosis and above all Enterocytozoon bieneusi cause approximately 7-50% of otherwise unexplained diarrhea in HIV-infected patients. Four cases of intestinal microsporidiosis in AIDS-patients are reported., Methods: Detection of spores of microsporidiosis in stool samples from HIV-infected patients was performed by the following techniques: 1) light microscopy, using Kokoskin's modification of the Weber trichrome stain; 2) electron microscopy, for confirmation (only available in one case), and 3) indirect immunofluorescence assay (IIF), performed on all positive samples. Rabbit polyclonal antibodies anti-Encephalitozoon cuniculi, anti-E. hellem and anti-E. (Septata) intestinalis were used to eliminate the possibilities of presence of microsporidiosis other than E. bieneusi., Results: Detection of spores of microsporidiosis by trichrome stain was positive in all samples from the 4 patients. By elimination, IIF confirmed the presence of Enterocytozoon bieneusi in all 4 cases. E. bieneusi was identified by electron microscopy in the only case available. Three patients had CD4+ lymphocyte counts < 100 x 10(6)/l and one < 500 x 10(6)/l. All had AIDS and three of them were homosexual. Two patients remained positive for microsporidiosis for almost 5 months., Conclusions: Despite the very few cases described in Spain, these preliminary data lead us to believe that intestinal microsporidiosis should be considered as an possible cause of diarrhea in our country also, and mainly in acute immunodeficiency patients.

The association of celiac disease, epilepsy and occipital calcifications with initial clinical manifestations of epilepsy during the first two decades of life with an often progressive and variable course and clinical expression of malabsorption has recently been described. Two cases of celiac disease with occipital calcifications and a presentation with neurologic symptoms in adulthood are reported. The first case is that of a 40-year-old male who presented recurrent and alternating pure brachial monoparesis and later acute abdominal pain following which celiac sprue was diagnosed. The second case is that of a 53-year-old woman diagnosed with celiac sprue 20 years before, presenting permanent myoclonus in the lower limbs which were progressive in severity, ataxic march and generalized tonoclonic seizures. Both patients had bilateral occipital calcifications on CT and celiac disease was demonstrated on biopsy. The first case also showed marked signal alteration in the white matter on MRI. Celiac disease with cerebral calcifications presents also in adulthood with atypical clinical manifestations. Suspicion of celiac disease may be confirmed by non-invasive methods such as antigliadin and antiendomysium antibody determination. CT imaging is characteristic.

We have sequentially studied for one year CD3+, CD4+, CD8+, CD11b+, and CD20+ lymphocyte subpopulations in 52 brucellosis patients whose diagnosis was confirmed by hemoculture. We observe at diagnosis a decreased percentage of CD4+ lymphocytes with an increase in CD8+ and inverted CD4+/CD8+ ratio, which was greater in patients with an evolution longer than 4 weeks (p less than 0.05) returning to normal very slowly. The percentage of monocytes was also increase at diagnosis (p less than 0.01) and at their second month CD3+ an CD20+ subpopulations did not suffer any changes during the follow up period. Patients were divided into two groups according to whether one or two anti-Brucella IgG peaks were observed in ELISA throughout the evolution. Patients in whom IgG titer decreased uninterruptedly, the inversion of the ratio during the first two months was greater, while those patients who suffered a relapse showed a simultaneous new decrease in CD4/CD8 ratio (p less than 0.05).

All the cases of HIV infected patients diagnosed in our department during 1987 of Salmonella bacteremia are included in this report. Out of a total of 10 patients with both conditions, six were diagnosed of AIDS, three of AIDS related complex, and one presented an asymptomatic HIV infection. The mean age was 32.9 years (range 22 to 56). Eight of them were males, seven were intravenous drug addicts, and three were homosexual. Fever was observed in all cases and diarrhea in four of them. Salmonella enteritidis was isolated from the blood culture of nine patients and salmonella typhimurium from one. Salmonella enteritidis was isolated from the stool culture of 5 patients and Salmonella typhimurium and Campylobacter from the stool culture of one patient. Of the 10 patients 6 have died, 4 of them within the three weeks after the diagnosis of Salmonella non-typhi bacteremia, 2 have not relapsed and two patients were lost during follow up.
