3. MRI T2 mapping sequence concordance with CT-arthrography for knee cartilaginous lesions in traumatic and degenerative lesions

Abstract

Objective: To assess the concordance of MRI T2 mapping sequence with CT-arthrography for traumatic and degenerative chondral lesions. Subjects and methods: Fifteen consecutive patients were included prospectively from September2011 to April2012. They referred for CT-arthrography examination for degenerative or traumatic knee pain and had also knee 3. T MRI with T2 mapping sequences. CT-arthrography was used as benchmark diagnostic test for chondral lesion, read in a consensual way by two senior musculo-skeletal radiologists. The same, blinded to CT-arthrography results, independently reviewed standard and T2 Mapping MRI sequences and rated number of hyaline cartilage lesions, and their grade according to the International Cartilage Research Society Score (ICRS) in each anatomical compartment. Size and T2 value of the largest lesion was measured. We used Mac Nemar test to assess concordance between MRI and CT-arthrography findings, a regression model to compare CT-arthrography ICRS classification to T2 values, and Wilcoxon test for inter-reader agreement. Results: Because of artifact and loss to follow-up, 11patients were finally included. Concordance of T2 mapping MRI with CT-arthrography for reader1 and reader2 were respectively 90% and 80% in medial compartment, 50% and 70% in lateral compartment, 90% and 70% in anterior compartment. There was no significant correlation between CT-arthrography ICRS classification and T2 values (P>0.06). No significant differences between reader1 and reader2 were observed when evaluating the prevalence of knee lesions (P=0.25 for the medial compartment, P=0.62 for the lateral compartment and P=1 for the anterior zone). Conclusion: T2 mapping is a reproducible imaging technique, with a good concordance to CT-arthrography for detection and size of chondral lesions, with no correlation between CT-arthrography ICRS grade and T2values. © 2013 Elsevier Masson SAS.

Abstract

Purpose: The purpose of this study was to investigate the prognostic value of left atrioventricular coupling index (LACI) assessed by cardiac computed tomography (CT), to predict cardiovascular death in consecutive patients referred for cardiac CT with coronary analysis., Materials and Methods: Between 2010 and 2020, we conducted a single-centre study with all consecutive patients without known cardiovascular disease referred for cardiac CT. LACI was defined as the ratio of left atrial to left ventricle end-diastolic volumes. The primary outcome was cardiovascular death. Cox regressions were used to evaluate the association between LACI and primary outcome after adjustment for traditional risk factors and cardiac CT angiography findings., Results: In 1,444 patients (mean age, 70 ± 12 [standard deviation] years; 43% men), 67 (4.3%) patients experienced cardiovascular death after a median follow-up of 6.8 (Q1, Q3: 5.9, 9.1) years. After adjustment, LACI was positively associated with the occurrence of cardiovascular death (adjusted hazard ratio [HR], 1.07 [95% CI: 1.05-1.09] per 1% increment; P < 0.001), and all-cause death (adjusted HR, 1.05 [95% CI: 1.03-1.07] per 1% increment; P <0.001). After adjustment, a LACI ≥ 25% showed the best improvement in model discrimination and reclassification for predicting cardiovascular death above traditional risk factors and cardiac CT findings (C-statistic improvement: 0.27; Nnet reclassification improvement = 0.826; Integrative discrimination index =0.209, all P < 0.001; likelihood-ratio-test, P < 0.001)., Conclusion: LACI measured by cardiac CT is independently associated with cardiovascular death and all-cause death in patients without known cardiovascular disease referred for cardiac CT, with an incremental prognostic value over traditional risk factors and cardiac CT findings., Competing Interests: Declaration of Competing Interest Solenn Toupin is employee of Siemens Healthcare. Abstract

Purpose: The purpose of this study was to evaluate the relationships between the three-dimensional anatomy of operated hip in standing position using low-dose stereo-radiography imaging system and postoperative hip disability and osteoarthritis outcome score (HOOS) after total hip arthroplasty (THA)., Material and Methods: A total of 123 patients who underwent THA during a one-year period were included. There were 50 men and 73 women with a mean age of 67.3±13.6 (SD) years (range: 19-89 years). All patients underwent pre- and postoperative low-dose stereo-radiography examination and completed a HOOS form (score from 0 to 100, 100 for full satisfaction). We recorded 16 anatomical parameters before THA, and 15 after THA. After binary transformation of HOOS score using 70 as threshold value, outcome was assessed using logistic or generalised linear models., Results: A total of 103 patients (103/123; 83.7%) had a HOOS score≥70 and were considered as the satisfied group. A significant difference in pelvic incidence (the angle between a line perpendicular to the sacral plate at its midpoint and a line connecting the same point to the centre of the bicoxofemoral axis) was found between the satisfied 56.4±10.4 (SD)° (range: 31-85°) and the unsatisfied group 48.7±8.9 (SD)° (range: 40-65) (P=0.006). The relative variation of offset (distance from the centre of rotation of the femoral head to a line bisecting the long axis of the femur) compared to the contralateral hip was -7% in the satisfied group and 7.2% in the unsatisfied group (P=0.01)., Conclusion: Pelvic incidence, a parameter independent of the reconstructed anatomy, probably influences the quality of life of patients with THA, via pelvic compensatory capabilities. Abstract

Dactylitis refers to a global swelling of a finger or a toe giving it a clinical sausage-shape presentation. It is an extremely suggestive symptom as it guides the rheumatologist towards a shortlist of diagnoses. However, radiologists are less familiar with dactylitis. The aim of this review is to detail and illustrate the main causes of dactylitis using standard X-ray imaging, ultrasound, computed tomography and magnetic resonance imaging in order to make radiologists more familiar with this symptom by illustrating the various conditions that are associated with dactylitis including infection, peripheral spondyloarthritis, sarcoidosis, microcrystalline deposition, osteoid osteoma, and sickle cell disease., (Copyright © 2020 Société française de radiologie. Abstract

In the absence of obvious trauma, the calcifications/ossifications of the bone surface and soft tissues of the wrist, hand and fingers can be challenging and may not be noticed or lead to unnecessary examinations and monitoring. Although these are usually benign conditions and despite a favorable spontaneous outcome, surgical resection may be required and recurrence may occur. In practice, only paraneoplastic syndromes such as secondary hypertrophic osteoarthropathy (Pierre Marie-Bamberger syndrome) may reveal a malignant tumor, most often pulmonary. We suggest a diagnostic approach based on the initial clinical presentation (acute pain, chronic pain, growth±pain) and the radiological features., (Copyright © 2014 Éditions françaises de radiologie.
