This study aimed to know the opinion of professionals participating in an experiment to implement a pilot for molecular tests to detect Chlamydia trachomatis and Neisseria gonorrhoeae at the Brazilian Unified National Health System (SUS). The detection rate of C. trachomatis and/or N. gonorrhoeae and the factors associated with infection were determined. The strategy included laboratories belonging to the HIV and viral hepatitis viral load network. Testing targeted people who are more vulnerable to sexually transmitted infections and collected urine samples and/or vaginal, endocervical, and/or male urethral swabs. Questionnaires were sent to state managers and laboratory professionals about the implementation of the pilot. Reviews were overall positive. Weaknesses included difficulties changing work processes, lack of human resources, poorly sensitized care professionals, and absence of primary urine tubes, the only input not provided. Strengths included the centralized acquisition of tests, sharing of equipment, and storage of samples at room temperature. Of the 16,177 people who were tested, 1,004 (6.21%) were positive for C. trachomatis; 1,036 (6.4%), for N. gonorrhoeae; and 239 (1.48%), for C. trachomatis/N. gonorrhoeae . Detection of any infection occurred more frequently in young people (≤ 24 vs. > 24 years) (adjOR = 2.65; 95%CI: 2.38-2.96), men (adjOR = 1.95; 95%CI: 1.72-2.21), brown/black individuals (adjOR = 1.06; 95%CI: 1.05-1.11), those in Southeastern Brazil (adjOR = 1.08; 95%CI: 1.02-1.13), and in urethral secretion samples (adjOR = 1.46; 95%CI: 1.41-1.52). Results show the importance of making testing available nationwide, which supported the implementation of a definitive network to detection C. trachomatis/N. gonorrhoeae in SUS.

This study aimed to identify lifestyle changes and associated sociodemographic factors in women and men participating in the Brazilian Longitudinal Study for Adult Health (ELSA-Brasil) cohort during the COVID-19 pandemic. Longitudinal study with 3,776 (aged 58.8 years; SD ± 8.5) employees of public higher education institutions in the second follow-up and the wave-COVID of ELSA-Brasil. Data collected using structured questionnaires. An exploratory analysis was performed using binary and multinomial logistic regression on the dependent variables with two and three categories, respectively, by obtaining crude and adjusted odds ratio estimates in SPSS 20.0, considering a p-value < 0.05. There was a reduction in physical activity of 195.5 (SD ± 1,146.4) metabolic equivalents per week in women and 240.5 (SD ± 1,474.2) in men, and in smoking by 15.2%. There was an increase in alcohol consumption in men and women (434.2 ± 5,144.0; and 366.1 ± 4,879.0, respectively), in the food quality score (0.8 ± 3.7, women; 0.5 ± 3.7, men), sleeping time (0.4 ± 1.2, women; 0.5 ± 1.1, men), screen time (1.7 ± 2.4, women; 1.4 ± 2.3, men), and sitting time (1.7 ± 2.6, women; 1.5 ± 2.4, men) (hours/day). In total, 18.6% increased the purchase of ultra-processed foods and 36% increased the purchase of natural foods. Age and work activity contributed to increase the chance of purchasing ultra-processed foods, and age and adherence to social distancing influenced the shift to a more sedentary behavior, while income and active work favored the increase in alcoholic beverage consumption. These factors should be considered when developing public policies to avoid individual behaviors that are harmful to health during pandemics.

The aim of the study is to assess the adherence to recommended prevention measures during the COVID-19 pandemic and to investigate the factors associated with this adherence in the adult population. This study has a cross-sectional design and used data from the complementary study Brazilian Longitudinal Study of Adult Health (ELSA-Brasil) - COVID, conducted from 2020 to 2021. Data were analyzed using the chi-square test and multinomial logistic regression. The sample consisted of 5,440 participants. The preventive measure with the highest adherence was the use of a face mask (95.5%). There was greater adherence by females and lower chance of adherence by white people, by those who consume alcoholic beverages, people who were retired, as well as for those who live alone or who have family members who did not follow the recommendations to stay at home. Greater adherence to preventive behaviors was observed in only one third of the participating population, which demonstrates that there was a need for greater awareness of the risks in specific populations. The findings contribute to improving our understanding about health promotion and COVID-19 prevention.

This study aimed to critically analyze the narrative of Brazilian public health policies in obesity care based on an intersectional approach. This is a qualitative exploratory, documentary, and analytical study based on the "What's the problem represented to be?" approach (WPR). This approach constitutes a methodological instrument for critical analysis of public policies based on six guiding questions. A total of ten documents were selected, published from 2004 to 2021 by the Brazilian government. The critical analysis resulted in three categories: (i) obesity causes and the dominant narrative: what problems are represented?; (ii) dominant narrative and health care: what are the effects for people with obesity?; (iii) obesity and intersectionality: where are silences? The consumption of food and sedentary lifestyle were the dominant narrative as causes of obesity. Intersectionality, mediated by the categories of gender/sex, race/skin-color, and social class, was identified as silenced in the narrative of public health policies, not being associated as linked causes of obesity, nor effectively included in the proposed actions of the policies. The silences found in the study highlight the need to include intersectionality in the elaboration and execution of public health policies and in the care of people with obesity. Considering the intersections of gender/sex, race/skin-color, and social class and their forms of oppression in the emergence and aggravation of obesity, critical analyses of simplistic narratives in public health policies are extremely relevant to problematize gaps affect the care of users with obesity.

The Care Network for People with Disabilities (RCPCD) was implemented in 2012 as a consequence of the actions of the Viver sem Limite (Living without Limits) plan and has been the research object of recent studies. However, no published studies address the degree of implementation of this network. This study aimed to evaluate the degree of implementation of the RCPCD in eight states in the five regions of Brazil. This multiple case study performed evaluative research of the degree of implementation of the RCPCD in the states of Amazonas, Bahia, Espírito Santo, Mato Grosso do Sul, Minas Gerais, Paraíba, Rio Grande do Sul, and São Paulo. A logical model of the policy and a measurement matrix were developed. The degree of implementation of seven states was moderate, but Amazonas had an incipient degree of implementation. The evaluation of each stage of the process showed important differences, as the regional diagnosis and network adhesion stages presented moderate to advanced degrees of implementation in most states. In the service contracting stage, no state had an advanced degree of implementation, and the stage of follow-up and monitoring of the RCPCD was not even reached. The measurement matrix helps evaluate the degree of implementation of the RCPCD, and, by recognizing its results, the state steering groups and technical area recommend its use. Actions to improve this implementation, such as strengthening regionalization, establishing regional steering groups, ensuring contracting mechanisms, and defining criteria for certification of the care points, are necessary.

This article analyzed the activity of Social Health Organizations (OSS) in the Brazilian Unified National Health System (SUS) and the use of management contracts and contract addenda as instruments of privatization. The article aimed to understand the processes that legitimize privatization, based on quantification of the amounts transferred through management contracts and contract addenda to organizations that signed contracts with the Rio de Janeiro Municipal Health Department, Rio de Janeiro, Brazil, from 2009 to 2018. A descriptive, exploratory, mixed-methods study was developed to analyze the amounts transferred to the OSS. The study used secondary public data available on the Health Department's websites and other official documents. A total of 268 documents were identified, 61 of which were contracts and 207 addenda, related to 15 institutions. The funds transferred to these organizations totaled BRL 15.94 billion (USD 2.9 billion). The OSS that received the most public funds was IABAS (Institute of Primary and Advanced Health Care), with a total of BRL 4.021 billion (USD 731 million). In 2014, Rio de Janeiro's total health budget was approximately BRL 4 billion (USD 727 million), of which BRL 2.5 billion (USD 455 million) was transferred to OSS, representing 62% of the municipal health budget. The data show that management contracts and contract addenda serve as instruments for privatization of the SUS. This privatization does not employ the conventional model but a functional and fluctuating one.

The study aimed to analyze the Brazil Networks Telehealth Program in the context of primary healthcare (PHC) in Brazil and to identify related factors. This cross-sectional study used data from the second cycle of the Program for Improvement of Access and Quality of Basic Healthcare (PMAQ-AB). The sample consisted of 29,756 healthcare teams who joined the program voluntarily. Independent variables included contextual characteristics (region and population size), healthcare unit (type, telephone access, broadband, number of physicians and nurses, consultation offices, community health workers' room, meeting room, existence of telehealth) and health team characteristics (institutional support). Crude and Poisson regression-adjusted analyses assessed which variables are associated with greater use of telehealth. Prevalence of use of telehealth was 32.7% in the total sample and 73.3% among teams with the Program implemented. Tele-education was the most frequently used modality. Higher rates of prevalence of use were found in the South and Southeast of Brazil, in municipalities with up to 30,000 inhabitants, with telehealth implemented in the unit, with presence of at least one physician, and with at least one telephone available in the service. Institutional support had a 40% positive impact on prevalence of use of telehealth. Infrastructure variables such as Internet and availability of rooms with different purposes appeared not to significantly impact the use of these technologies, indicating that institutional support and the implementation of telehealth itself in the unit are more important for increasing adherence to the Program.

Following the reemergence of yellow fever in 2014/2015, Brazil recorded its largest yellow fever epidemic in recent decades, mainly affecting the country's Southeast region. Yellow fever is a hemorrhagic viral disease caused by a flavivirus transmitted by sylvatic mosquitos (Haemagogus; Sabethes). In the urban cycle, eradicated in Brazil since 1942, the virus is transmitted by Aedes aegypti. Nonhuman primates are the principal hosts of the virus and constitute "sentinels" in yellow fever surveillance. This article describes the control and prevention activities launched during the yellow fever epidemic in the State of Espírito Santo, Brazil, and the implementation of vaccination, through an ecological study with a spatial approach. The study revealed the lack of detection of epizootics in nonhuman primates by surveillance services in Espírito Santo, with simultaneous detection in humans. The study presented the evolution of vaccination activities, reaching 85% overall coverage for the state in six months, varying widely, from 59% to 122%, between municipalities (counties). Importantly, 55% of the municipalities with timely immunization, considering the interval adopted for this study, did not present human cases. The intensification of surveillance activities, communication between areas, and multidisciplinary teams in managing the epidemic optimized the detection and diagnosis of human cases and allowed control of the epidemic. The study identifies progress and points to some late measures and gaps in surveillance that require improvements.

The objective was to analyze the diffusion of cases of yellow fever in time and space in the epidemic of 2017 in the state of Espírito Santo, Brazil. An ecological observational study was performed with spatial analysis of yellow fever cases. Georeferencing of information and spatial analysis used the digital grid for the state of Espírito Santo, divided into 78 municipalities (counties), using the Arcgis software, 10.3. Geostatistical analysis was performed using the ordinary kriging function. The study found an incidence of 4.85/100,000 inhabitants of sylvatic yellow fever in Espírito Santo in 2017, with 29.74% case-fatality. Sylvatic yellow fever cases were distributed across 34 of the state's 78 municipalities, representing 43% of its territory. The temporal distribution of reported yellow fever cases in the current study occurred from the 1st to the 19th Epidemiological Weeks (EW). The geostatistical spatial analysis via ordinary kriging demonstrated spatial diffusion by yellow fever contagion among the municipalities in the state of Espírito Santo, with spatial continuity. The disease emerged in the state in the EW 1 through municipalities bordering on the state of Minas Gerais. Geoprocessing showed that yellow fever reached the state of Espírito Santo through the municipalities bordering on the state of Minas Gerais, moving eastward in the state and reaching the Atlantic coastline. There was a higher concentration of cases and persistence in the state's Central and Metropolitan regions, which have areas of Atlantic Forest, showing a pattern of diffusion continuity by contagion.

The objective of the present study was to evaluate the association between social position and anthropometric status in women and men Brazilian adult. This was a cross-sectional study that used baseline data collected from 2008 to 2010 for the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil, in Portuguese), in the six major Brazilian state capital cities. A total of 15,105 active and retired civil servants aged from 35 to 74 years. Two latent variables were defined by latent class analysis, social position and anthropometric status. Both constructs and the analyses were separately evaluated by sex. Associations were assessed using multivariate logistic regression analysis with adjustment for age, self-reported skin color/race, and marital status. Around 44% of the women and 26% of the men were classified as overweight or obese. Social position tended to be lower in women (43.2%) and higher among men (40.4%). Heavier women were more likely to be black and brown-skinned, whereas slimmer women were more likely to be white. After adjustment, women's weight increased as social position decreased (OR = 1.52; 95%CI: 1.36-1.70), whereas in men weight decreased as social position decreased (OR = 0.87; 95%CI: 0.76-0.99). Social position affected the anthropometric status of women and men differently, with body patterns also being affected by ethnicity/skin color, showing the potentiality of taking the intersectional perspective when investigating the possible social determinants of the phenomenon.

Intrauterine life is a critical period for the development of body fat and metabolic risk. This study investigated associations between birth weight and total and truncal body fat in adults. To do so, we analyzed data on 10,011 adults participating in the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil) who self-reported birth weight as < 2.5kg, 2.5-4.0kg, or > 4.0kg at baseline (2008-2010) and underwent bioimpedance in the next follow-up visit (2012-2014). Greater mean total and truncal fat mass were seen in those with high birth weight compared with adequate birth weight (p < 0.001) in both sexes (total fat: 25.2 vs. 23.1kg in men and 31.4 vs. 27.7kg in women, and truncal fat: 13.5 vs. 12.4kg in men and 15.9 vs. 14.2kg in women). U-shaped patterns were observed in restricted cubic-spline analyses in the subset of 5,212 individuals reporting exact birth weights, although statistically significant only for those with high birth weight. In the whole sample, in comparing high to adequate birth weight, the latter predicted having a large (> 85 percentile) total and truncal fat mass, respectively: OR = 1.76, 95%CI: 1.37-2.25 (men) and OR = 1.86, 95%CI: 1.42-2.44 (women); OR = 1.68, 95% CI: 1.31-2.16 (men) and OR = 1.73, 95%CI: 1.31-2.28 (women). However, low birth weight predicted having a large (> 85 percentile) % truncal fat only in women (OR = 1.40, 95%CI: 1.03-1.91). In conclusion, in these men and women born in a period in which fetal malnutrition was prevalent, birth weight showed complex, frequently non-linear associations with adult body fat, highlighting the need for interventions to prevent low and high birth weight during pregnancy.

The study aimed to estimate the prevalence of sexually transmissible infections (STIs) and associated factors in women in quilombola (maroon) communities in Brazil. This was a population-based cross-sectional study of quilombola women from March 2017 to January 2019. A questionnaire was used with sociodemographic, behavioral, and clinical information. A gynecological examination was performed for the collection of uterine cervical cells for oncotic cytology and the detection of Chlamydia trachomatis, Neisseria gonorrhoeae, Trichomonas vaginalis, and human papillomavirus (HPV) via polymerase chain reaction. Rapid tests for HIV and syphilis were performed. The main outcome was defined as infection with one or more sexually transmissible pathogens. The analysis used chi-square test and hierarchical logistic regression. From a total of 380 women, 352 (92.6%) were included in the study. Prevalence of at least one STI was 18.5% (95%CI: 14.76-22.85). The highest prevalence was for HPV, with 11.1%, followed by 6.3% for T. vaginalis and 4.3% for C. trachomatis. There were no cases of N. gonorrhoeae. Prevalence was 0.3% for HIV and 4.3% for syphilis. Cervical-vaginal cytology was altered in 7.7% of the women. Detection of one or more STIs was significantly associated with age 25 to 44 years (OR = 2.33; 95%CI: 1.05-5.18), alcohol consumption (OR = 1.96; 95%CI: 1.06-3.64), altered cervical cytology (OR = 3.96; 95%CI: 1.65-9.48), and bacterial vaginosis (OR = 3.61; 95%CI: 2.01-6.47). Quilombola women showed high prevalence of one or more STIs, emphasizing the importance of organizing prevention strategies targeted to these women.

As the COVID-19 pandemic has spread worldwide in 2020, populations, authorities, and local and global health governance institutions have been affected differently. Global Health Diplomacy and "paradiplomacy" have become relevant instruments and arenas for the challenges raised by the pandemic, especially for non-State or subnational actors. This Essay analyzes the case of the Brazilian state of Maranhão during the pandemic, which used a "wartime operation" to purchase more than a hundred mechanical ventilators on the international market, over the Federal Government's head, at a moment of fierce international competition for medical supplies and equipment. The Essay examines the principal aspects, contexts, reasons, factors, actors, and actions that contextualize the operation conducted by the state of Maranhão as an activity in paradiplomacy and Global Health Diplomacy by a subnational government in Brazil. We analyzed these concepts in light of the literature on the topic and studied the action by Maranhão based on cross-analysis of data from documents, statements, and news coverage. We conclude that the case of Maranhão illustrates the capacity of subnational bodies to respond to global emergencies, mainly in contexts of inefficacy or absence of the Federal executive, legitimizing independent action aimed at saving lives.

This study aims to analyze survival rates in acute lymphocytic leukemia in the city of São Paulo, Brazil, from 1997 to 2013, according to sex, age bracket, and administrative region. This was a longitudinal study to calculate survival rates. Data on new cases of leukemia in the age bracket 0 to 14 years of age were collected from the database in the Population-Based Cancer Registry of the city of São Paulo. Population and mortality data were obtained from the Brazilian Health Informatics Department. The sample included children 0 to 14 years of age diagnosed with leukemia from 1997 to 2013, living in the city of São Paulo. The variables were sex, age bracket, and Regional Health Divisions. The survival analysis calculated the Kaplan-Meier product limit estimator and compared the survival curves through the log-rank test, and the Cox proportional risks model was adjusted. The estimated majority of new cases were males, 0 to 4 years of age, diagnosed from 2000 to 2004, and living in the southeast regional division of the city of São Paulo. The estimated overall incidence rate was 34.7 new cases per million, and the mortality rate was 9.0 deaths per million. The overall five-year survival of children from acute lymphocytic leukemia was 68%. There was a statistically significant difference according to decade of diagnosis (p = 0.026), and specifically the survival rates decreased over the years, with the worst prognosis in the years 2005-2009.

The study aimed to assess the implementation of the More Doctors Program (PMM in Portuguese) based on a case study of the Espírito Santo State, Brazil. The first stage involved analyzing the documents that launched the PMM, allowing the creation of a logic model. In the second stage, data on the PMM in Espírito Santo State were analyzed (2013-2016) based on population brackets, creating indicators for the proportion of physicians before and after the Program with the following parameters: unacceptable (0 to 0.99 physicians/1,000 inhabitants); acceptable (1 to 1.99); and desirable (2.0 or more). Data were analyzed with the Wilcoxon and Kruskal-Wallis non-parametric tests. The model included the size, inputs, activities, outputs, results, and impact of the PMM, orienting analysis of the Program's implementation. Of all the physicians in the emergency supply to Espírito Santo State (432), the majority (63.8%) were cooperative exchange physicians (Cubans), while 31.2% were regional board-accredited Brazilians. The Brazilian and individual exchange physicians were allocated mainly in medium and large municipalities, while the Cubans were distributed more heterogeneously in municipalities of all population sizes and were thus more common in areas with greater social vulnerability and lower social capital. There was a significant increase in the number of physicians in Espírito Santo State, but the supply to the extremely poor population did not reach the desired level, presenting an unacceptable mean rate in municipalities with up to 10,000 inhabitants and an acceptable rate in municipalities in the other population brackets.

Lipid abnormalities and subclinical systemic inflammation are associated with atherosclerosis and are used as markers of cardiovascular risk. Studies have suggested a possible beneficial effect of dairy products on cardiovascular health, but the results in lipid and inflammatory markers are still controversial. This study aimed to assess the association between consumption of dairy products and their different subgroups and C-reactive protein (CRP), LDL-cholesterol (LDL-C), and triglyceride/HDL-cholesterol ratio (TG/HDL-C) in participants in the Longitudinal Study of Adult Health (ELSA-Brasil) (n = 9,372). Consumption of dairy products was assessed via a validated food frequency questionnaire and expressed as servings/day. Total consumption of dairy products was described in four categories (≤ 1 serving/day to > 4 servings/day). The associations were estimated via odds ratios (OR), using the group with the lowest consumption (≤ 1 serving/day) as the reference. The lowest ORs for TG/HDL-C in the multivariate model (0.70; 95%CI: 0.55-0.90 in men; and 0.55; 95%CI: 0.43-0.70 in women) were found in the group that consumed > 4 servings day of dairy products. These results were supported by the inverse associations between different subgroups of dairy products and the TG/HDL-C ratio. No association was found between consumption of dairy products and their subgroups and LDL-C and CRP. The results suggest a possible beneficial effect of dairy products on lipid profile, but longitudinal and intervention studies are needed to elucidate the effect mechanisms of different types of dairy products.

This study aimed to analyze the aggregation of alcohol consumption, smoking, excess weight, and short sleep in Brazilian adolescents. This was a cross-sectional multicenter study conducted with teens participating in the Study of Cardiovascular Risk Factors in Adolescents (ERICA in Portuguese). The sample consisted of adolescents that answered the complete questionnaires on sleep, tobacco, and alcoholic beverages, in addition to having their weight and height measured. Aggregation was analyzed by comparing the observed and expected prevalence of risk factors in all possible groupings, with the respective 95% confidence intervals. Analyses were performed in Stata 14 using the svy (survey) command for complex sample data. The sample included 73,624 adolescents, of whom 25.5% had excess weight and 24.2% consumed alcoholic beverages. Aggregation of the four risk factors was O/E = 5.6. Aggregation of three factors was more prevalent in those 15 to 17 years of age (P = 4.8). In the POR (prevalence odds ratio) analysis of the combination of two risk factors, those that smoked showed 11.80 higher odds of also consuming alcohol, compared to those that did not smoke, and vice versa, in private schools. In relation to age, adolescents 12 to 14 years of age that smoked showed 15.46 times higher odds of also drinking, and vice and versa. Adolescents in the sample presented the aggregate presence of four risk factors, and there was a significant relationship between tobacco and alcohol consumption.

The early neonatal period accounts for approximately half of the deaths of young children under one year of age, and the neonatal near miss can recognize factors causing this high number of deaths. Thus, the aim of this study is to determine whether advanced maternal age increases the chance of neonatal near miss, in addition is to identify which factors are associated with the neonatal near miss, stratified by parity. Data are from the 2011-2012 Birth in Brazil study, which used a national population-based sample of 15,092 newborns of women between 20-29 and 35 years of age or more (advanced maternal age). Multiple logistic regression was performed to test the association between neonatal near miss and prenatal and childbirth variables, pre-gestational diseases, obstetric history and socioeconomic characteristics, stratified by parity. Advanced maternal age was to be statistically associated with neonatal near miss in nulliparous (OR = 1.62; 95%CI: 1.05-2.50) and multiparous (OR = 1.51; 95%CI: 1.20-1.91) when compared to women 20-29 years of age. For nulliparous women, the main variables statistically associated with neonatal near miss were multiple gestation (OR = 8.91) and hypertensive disease (OR = 2.57), whereas forceps-assisted vaginal delivery (OR = 7.19) and multiple gestation (OR = 4.47) were the variables associated for multiparous women. Neonatal near miss has been shown to be connected with access to health services for childbirth, gestational complications and maternal characteristics, mainly advanced maternal age. Therefore, to properly monitor and classify maternal gestational risk, to control gestational complications during prenatal care, and to correctly refer these women to childbirth care should be priority strategies for healthcare services.

The study aimed to investigate the short-term association between air pollution and emergency treatments for respiratory diseases in children 0 to 6 years of age. This was an ecological space-time study in Greater Metropolitan Vitória, Espírito Santo State, Brazil. A Poisson regression general additive model (GAM) used the number of daily treatments for respiratory diseases as the dependent variable, and the independent variables were daily concentrations of air pollutants (PM10, SO2, NO2, O3, and CO), temperature, humidity, and precipitation. Average daily concentrations were used to make estimates for the entire metropolitan area and in loco analyses considering children residing in a 2km radius around 8 air quality monitoring stations. An increase of 10μg/m3 in the concentration of air pollutants increased the risk of emergency treatment for respiratory disease. In the overall area, for PM10, the increase was 2.43%, 2.73%, and 3.29% in the cumulative values at 5, 6, and 7 days, respectively. For SO2, the increase was 4.47% on the day of exposure, 5.26% two days later, and 6.47%, 8.8%, 8.76%, and 7.09% for the cumulative values at days 2, 3, 4, and 5 days, respectively. CO showed a significant association for residents around two stations, and O3 for only one. Even within the limits set by the World Health Organization, the pollutants PM10, SO2, NO2, and O3 are associated with increased risk of treatment for respiratory diseases in children 0 to 6 years of age, and some effects were only identified when disaggregating by neighborhood, i.e., in loco, which allows capturing greater variation in the data.

This study aimed to investigate factors associated with orthostatic hypotension in 14,833 individuals 35-74 years of age. This was a cross-sectional study of baseline data (2008-2010) from the Longitudinal Study of Adult Health (ELSA-Brasil). Postural testing was performed after 20 minutes resting in supine position and active adoption of orthostatic posture. Blood pressure was measured in supine position and at 3 minutes in orthostatic position with an oscillometer (HEM 705 CP, Omron, São Paulo, Brazil). Orthostatic hypotension was defined as a drop of ≥ 20mmHg in systolic blood pressure and/or a drop of ≥ 10mmHg in diastolic blood pressure. The target covariates were sex, age bracket, race/color, schooling, nutritional status, waist circumference, alteration in the ankle-brachial index, pulse wave velocity, systolic and diastolic blood pressure, hypertension, diabetes, use of antihypertensives, cholesterol, triglycerides, Chagas disease serology, symptoms, and heart rate variation in the postural test, self-reported heart disease, acute myocardial infarction (AMI)/revascularization, and stroke. Orthostatic hypotension was significantly associated with higher age bracket, OR = 1.83 (95%CI: 1.14-2.95); alteration in the ankle-brachial index, OR = 2.8 (95%CI: 1.13-6.88); AMI/revascularization, OR = 1.70 (95%CI: 1.01-2.87); report of heart disease, OR = 3.03 (95%CI: 1.71-5.36); increased systolic blood pressure, OR = 1.012 (95%CI: 1.006-1.019); positive Chagas disease serology, OR = 2.29 (95%CI: 1.23-4.27); and occurrence of symptoms with postural change, OR = 20.81 (95%CI: 14.81-29.24). Presence of orthostatic hypotension can be a warning sign for cardiovascular disorders and thus a useful tool for screening and prevention.

This study aimed to describe fetal, neonatal, and post-neonatal mortality and associated factors in participants of the 2015 Pelotas (Brazil) birth cohort. The child mortality sub-study followed up all deaths in the first year of life. Data were collected on intrauterine fetal deaths (weight ≥ 500g and/or gestational age ≥ 20 weeks), neonatal deaths (< 28 days of life), and post-neonatal deaths (from 28 days to the end of the first year of life). Descriptive analyses using the Pearson chi-square test and a multinomial logistic regression to estimate the risk of fetal, neonatal, and post-neonatal deaths compared to live infants in the cohort (reference group) were performed. Data from 4,329 eligible births were collected, of which 54 died during the fetal period. Of the 4,275 eligible live births, 59 died in the first year of life. An association between fetal, neonatal, and post-neonatal deaths (OR = 15.60, 7.63, and 5.51 respectively) was found, as well as less than six prenatal consultations. Compared to live infants, fetal deaths were more likely to occur in non-white mothers, and neonatal deaths were 14.09 times more likely to occur in a preterm gestational age (< 37 weeks). Compared to live infants, infants that were born in a C-section delivery had 3.71 increased odds of post-neonatal death. Additionally, neonatal deaths were 102.37 times more likely to have a low Apgar score on the fifth minute after birth. These findings show the need for early interventions during pregnancy, ensuring access to adequate prenatal care.

This study aimed to identify sociodemographic factors associated with high estimated sodium/potassium ratio. A total of 15,105 participants were assessed, from 35 to 74 years of age, from a cohort of public employees (ELSA-Brazil). Socioeconomic data were collected with a questionnaire, and 12-hour nighttime urine samples were collected to estimate sodium and potassium intake. Sodium/potassium ratio was calculated in mmol/L and divided into tertiles: 1st tertile, corresponding to "low"; 2nd tertile for "medium"; and 3rd tertile for "high" sodium/potassium ratio. The proportions and means were compared according to the classification of tertiles in the sodium/potassium ratio, using chi-square test and analysis of variance (ANOVA). Crude and adjusted multinomial logistic regression models were tested, with the 1st tertile as the reference. Factors associated with sodium/potassium ratio were: male sex (OR = 1.78; 95%CI: 1.60-1.98); age 35 to 44 years (OR = 1.71; 95%CI: 1.33-2.19); incomplete elementary schooling (OR = 2.38; 95%CI: 1.78-3.18); and lower income (OR = 1.47; 95%CI: 1.26-1.73). Men, younger individuals, and those with less schooling and income showed higher mean sodium/potassium ratio.

The study's objectives: compare self-report measures of weight and height with direct measures; assess the impact of these discrepancies on body mass index (BMI) and prevalence of overweight and obesity; and apply correction models to the self-report measures and assess the degree of improvement in the corrected measures produced with the use of these models. A cross-sectional study was performed, assessing 4,151 adults (18 to 60 years) participating in the São Paulo Megacity Epidemiological Study. Linear regression models stratified by sex were proposed for correction of self-reported measures. Agreement was assessed with the intraclass correlation coefficient for the direct measures, self-report measures, and corrected measures, and kappa coefficient for BMI classification categories. Self-reported weight and the resulting BMI were underestimated, while height was overestimated, compared to direct measures. With all the correction models, the corrected measures were closer to the direct measures. Prevalence rates for excess weight, calculated by self-report measures, were underestimated by 24% in men and by 28% in women; with corrections, the underestimation decreased to 8% and 10%, respectively. The results showed moderate agreement for self-report measures and substantial agreement for corrected measures compared to direct measures. The use of correction equations for self-report data proved to be a useful method for producing more trustworthy estimates of prevalence of overweight and obesity in the general population, usually estimated from self-report measures of weight and height in population surveys.

More than one in four Brazilians have private health insurance (PHI), even thought it covers mostly the same procedures as the Brazilian Unified National Health System (SUS). This literature review included articles and monographs published since 1990 about the utilization of SUS by privately insured individuals. Considering outpatient care and hospitalization, privately insured people in Brazil use SUS in approximately 13% of the times they receive health care, and approximately 7% of people receiving care paid by SUS are privately insured; these findings vary depending on the type of service studied and on study methods. Utilization of SUS is more frequent in less developed regions, by people with more restricted PHI plans and by people with worse health status. Privately insured people report the limitations of PHI plans as their reasons for resorting to SUS. Sometimes, beneficiaries of PHI plans owned by nonprofit hospitals (which also provide health care financed by SUS) have easier access to care than uninsured people financed by SUS. Anecdotally, privately insured people are satisfied with SUS, but not to the point of adopting SUS as their preferred source of care. In short, for privately insured people, SUS only plays a secondary role in their health care. Despite PHI taking over part of the SUS's health care demand, PHI represents a restriction of the universal, equitable character of the SUS.

This study's objectives were to estimate the prevalence of hypertriglyceridemic waist (HTW) phenotype in participants in the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil), identify associated risk factors, and compare with other cardiovascular and metabolic risk indicators. This was a cross-sectional study with baseline data from a cohort of public employees. HTW is defined as the simultaneous presence of increased waist circumference (WC) (≥ 80cm for women, ≥ 90cm for men according to the International Diabetes Federation - IDF; and ≥ 88cm for women, ≥ 102cm for men according to the U.S. National Cholesterol Education Program - NCEP) and hypertriglyceridemia. Associations between independent variables and HTW were tested with multivariate logistic regression models. HTW was also compared to other cardiovascular and metabolic risk indicators by means of correlation tests, kappa index, sensitivity, and specificity. After exclusions, 12,811 participants were analyzed. Prevalence of HTW ranged from 24.7% (IDF) to 13.3% (NCEP). HTW was associated with age, excessive alcohol consumption, former smoking, low HDL, non-high HDL, and increased C-reactive protein, independently of gender or the criterion used to define HTW. HTW was associated with cardiovascular risk indicators, especially metabolic syndrome. The high prevalence of HTW and its association with cardiovascular risk indicators, especially metabolic syndrome, supports its use as a cardiometabolic risk screening tool in clinical practice.

The objective of the study was to estimate the contribution of ultra-processed foods to total caloric intake and investigate whether it differs according to socioeconomic position. We analyzed baseline data from the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil 2008-2010; N = 14.378) and data on dietary intake using a food frequency questionnaire, assigning it into three categories: unprocessed or minimally processed foods and processed culinary ingredients, processed foods, and ultra-processed foods. We measured the associations between socioeconomic position (education, per capita household income, and occupational social class) and the percentage of caloric contribution of ultra-processed foods, using generalized linear regression models adjusted for age and sex. Unprocessed or minimally processed foods and processed culinary ingredients contributed to 65.7% of the total caloric intake, followed by ultra-processed foods (22.7%). After adjustments, the percentage of caloric contribution of ultra-processed foods was 20% lower among participants with incomplete elementary school when compared to postgraduates. Compared to individuals from upper income classes, the caloric contribution of ultra-processed foods was 10%, 15% and 20% lower among the ones from the three lowest income, respectively. The caloric contribution of ultra-processed foods was also 7%, 12%, 12%, and 17% lower among participants in the lowest occupational social class compared to those from high social classes. Results suggest that the caloric contribution of ultra-processed foods is higher among individuals from high socioeconomic positions with a dose-response relationship for the associations.

This study aimed to investigate the existence and magnitude of the association between advanced maternal age (AMA) and occurrence of placenta praevia (PP) and placental abruption (PA) among nulliparous and multiparous women, by a systematic review and meta-analysis. We searched articles published between January 1, 2005 and December 31, 2015, in any language, in the following databases: PubMed, Scopus, Web of Science, and LILACS. Women were grouped into two age categories: up to 34 years old and 35 years or older. The Newcastle-Ottawa Scale was used to evaluate the methodological quality of the studies. A meta-analysis was conducted for the PP and PA outcomes, using a meta-regression model to find possible covariates associated with heterogeneity among the studies and Egger's test to assess publication bias. The protocol of this systematic review was registered in the International Prospective Register of Systematic Reviews (PROSPERO) system (CRD42016045594). Twenty-three studies met the criteria and were included in the meta-analysis. For both outcomes, an increase in age increased the magnitude of association strength, and PP (OR = 3.16, 95%CI: 2.79-3.57) was more strongly associated with AMA than PA (OR = 1.44, 95%CI: 1.35-1.54). For parity, there was no difference between nulliparous and multiparous women considered older for the PP and PA outcomes. Our review provided very low-quality evidence for both outcomes, since it encompasses observational studies with high statistical heterogeneity, diversity of populations, no control of confounding factors in several cases, and publication bias. However, the confidence intervals were small and there is a dose-response gradient, as well as a large magnitude of effect for PP.

Very little is known about the association between objective indicators of socioeconomic position in childhood and adolescence and low subjective social status in adult life, after adjusting for adult socioeconomic position. We used baseline data (2008-2010) from the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil), a multicenter cohort study of 15,105 civil servants from six Brazilian states. Subjective social status was measured using the The MacArthur Scale of Subjective Social Status, which represents social hierarchy in the form of a 10-rung ladder with the top rung representing the highest subjective social status. Participants who chose the bottom four rungs in the ladder were assigned to the low subjective social status category. The following socioeconomic position indicators were investigated: childhood (maternal education), adolescence (occupational social class of the household head; participant's occupational social class of first job; nature of occupation of household head; participant's nature of occupation of first job), and adulthood (participant's occupational social class, nature of occupation and education). The associations between low subjective social status and socioeconomic position were determined using multiple logistic regression, after adjusting for sociodemographic factors and socioeconomic position indicators from other stages of life. After adjustments, low socioeconomic position in childhood, adolescence and adulthood remained significantly associated with low subjective social status in adulthood with dose-response gradients. The magnitude of these associations was stronger for intra-individual than for intergenerational socioeconomic positions. Results suggest that subjective social status in adulthood is the result of a complex developmental process of acquiring socioeconomic self-perception, which is intrinsic to subjective social status and includes current and past, individual and family household experiences.

Health care workers (HCW) are at increased risk of latent tuberculosis infection (LTBI) from occupational exposure to Mycobacterium tuberculosis. The objective was to determine the prevalence of and risk factors for LTBI among primary HCW in five Brazilian cities. We conducted a cross-sectional study, from 2011 to 2013, among primary HCW, using a structured questionnaire and an evaluated for LTBI using the Quantiferon-TB Gold in-tube test. The magnitude of the associations was assessed using hierarchical logistic regression models. Among 708 HCW, the LTBI prevalence was 27% (n = 196; 95%CI: 24%-31%). We found that the following factors were positively associated with LTBI in primary HCW: age > 50 years (OR = 2.94; 95%CI: 1.44-5.99), absence of a BCG scar (OR = 2.10; 95%CI: 1.28-3.43), self-reported ex-smoker status (OR = 1.80; 95%CI: 1.04-3.11), being a nurse (OR = 2.97; 95%CI: 1.13-7.83), being a nurse technician (OR = 3.10; 95%CI: 1.26-7.60), being a community health agent (OR = 2.60; 95%CI: 1.06-6.40), and irregular use of N95 masks (OR = 2.51; 95%CI: 1.11-5.98). In contrast, HCWs who do not work in health care facilities with a TB control program were less likely to have LTBI (OR = 0.66; 95%CI: 0.45-0.97). This study demonstrated a substantial occupational risk of LTBI among primary HCW in Brazil. The Brazilian TB control program, as well as local programs, need to target these high-risk HCW with education, as well as with better personal protective equipment to prevent acquisition of new TB infection.

This study was designed to assess the reasons for health insurance coverage in a population covered by the Family Health Strategy in Brazil. We describe overall health insurance coverage and according to types, and analyze its association with health-related and socio-demographic characteristics. Among the 31.3% of persons (95%CI: 23.8-39.9) who reported "health insurance" coverage, 57.0% (95%CI: 45.2-68.0) were covered only by discount cards, which do not offer any kind of coverage for medical care, but only discounts in pharmacies, clinics, and hospitals. Both for health insurance and discount cards, the most frequently cited reasons for such coverage were "to be on the safe side" and "to receive better care". Both types of coverage were associated statistically with age (+65 vs. 15-24 years: adjusted odds ratios, aOR = 2.98, 95%CI: 1.28-6.90; and aOR = 3.67; 95%CI: 2.22-6.07, respectively) and socioeconomic status (additional standard deviation: aOR = 2.25, 95%CI: 1.62-3.14; and aOR = 1.96, 95%CI: 1.34-2.97). In addition, health insurance coverage was associated with schooling (aOR = 7.59, 95%CI: 4.44-13.00) for complete University Education and aOR = 3.74 (95%CI: 1.61-8.68) for complete Secondary Education, compared to less than complete Primary Education. Meanwhile, neither health insurance nor discount card was associated with health status or number of diagnosed diseases. In conclusion, studies that aim to assess private health insurance should be planned to distinguish between discount cards and formal health insurance.

Mental disorders are associated with employment status as significant predictors and as consequences of unemployment and early retirement. This study describes the estimates and associations of 12-month DSM-IV prevalence rates of mental disorders and use of health services with employment status by gender in the São Paulo Metropolitan Area, Brazil. Data from the São Paulo Megacity Mental Health Survey was analyzed (n = 5,037). This is a population-based study assessing the prevalence and determinants of mental disorders among adults, using the Composite International Diagnostic Interview. The associations were estimated by odds ratios obtained through binomial and multinomial logistic regression. This study demonstrates that having mental disorders, especially mood disorders, is associated with being inactive or unemployed among men and inactive among women, but only having a substance use disorder is associated with being unemployed among women. Among those with mental disorders, seeking health care services is less frequent within unemployed.

The objective of this study was to compare the avoidability of infant deaths according to different classification methods. This was a descriptive comparative study from 2006 to 2013 in Espírito Santo State, Brazil, focusing on the classification of 5,316 infant deaths according to five different methods. The methods of the International Collaborative Effort on Infant Mortality (ICE) and the SEADE Foundation correctly classified the highest proportions of deaths as avoidable versus unavoidable (94.6% and 94.4% correct classification, respectively). Most deaths resulted from quality problems in prenatal, childbirth, and postpartum care, regardless of which classification method was used. There were also considerable numbers of deaths from "ill-defined" causes according to all the methods, suggesting difficulty in access or precious care in health services. Avoidability methods provide an important instrument for diagnosis of quality problems in health services performance and orientation of measures to reduce avoidable infant deaths. Thus, strengthening maternal and child care and investment in training and capacity-building for health professionals and services are priorities for public policies to reduce infant mortality.

This study's objective was to identify the cut-off point for waist-to-height ratio (WHtR) with the best sensitivity, specificity, and accuracy for the elderly Brazilian population, using body mass index (BMI) as the anthropometric reference. A representative sample of the Brazilian population consisted of 5,428 elderly individuals participating in an epidemiological survey. The variables were weight, height, and waist circumference (WC). WHtR was assessed with BMI as the gold standard, using two proposals for classification of the elderly population's nutritional status. The ideal cut-off point for WHtR simultaneously showing the highest sensitivity and specificity was determined using the receiver operating characteristic (ROC) curve. Sensitivity from 94.9% to 98.4%, specificity from 43% to 55.4%, and values for area under the ROC curve from 0.878 to 0.883 were identified with a cut-off point of 0.55. We recommend use of WHtR in clinical practice due to its simplicity and good power to detect overweight in the elderly.

The objective of this study was to identify the association between emotional distress and social support networks with quality of life in primary care patients. This was a cross-sectional study involving 1,466 patients in the cities of São Paulo and Rio de Janeiro, Brazil, in 2009/2010. The General Health Questionnaire, the Hospital Anxiety and Depression Scale and the brief version of the World Health Organization Quality of Life Instrument were used. The Social Support Network Index classified patients with the highest and lowest index as socially integrated or isolated. A bivariate analysis and four multiple linear regressions were conducted for each quality of life outcome. The means scores for the physical, psychological, social relations, and environment domains were, respectively, 64.7; 64.2; 68.5 and 49.1. In the multivariate analysis, the psychological domain was negatively associated with isolation, whereas the social relations and environment domains were positively associated with integration. Integration and isolation proved to be important factors for those in emotional distress as they minimize or maximize negative effects on quality of life.

This study aimed to assess the cascade of care in the reduction of mother-to-child HIV transmission in the states of Amazonas, Ceará, Espírito Santo, Rio de Janeiro, and Rio Grande do Sul and the Distrito Federal, Brazil, using data from the Brazilian Information System on Diseases of Notification (SINAN). From 2007 to 2012, there was an increase (from 7.3% in Distrito Federal to 46.1% in Amazonas) in intra-gestational detection of HIV in 5 states, with a 18.6% reduction in Rio de Janeiro. Fewer than 90% of the women received antiretroviral therapy during their prenatal care, including those that already knew they were HIV-positive. The elective cesarean rate was low. The AIDS detection rate in children under 5 years as a proxy for mother-to-child HIV transmission showed a reduction of 6.3% from 2007 to 2012, and was highest in Rio Grande do Sul (50%), the state with the highest rates in the period, while Espírito Santo showed the highest increase (50%). Evaluation of the cascade of HIV care in pregnant women identified flaws in all the points. A link is needed between primary care and referral centers for HIV/AIDS, organizing care for the family and better outcomes for the children.

The objective of this study was to investigate factors associated with intimate partner violence after disclosing the diagnosis of sexually transmissible diseases (STDs) in Fortaleza, Ceará State, Brazil. This cross-sectional study enrolled 221 individuals treated at STD clinics. Multivariate logistic regression was performed. A total of 28.1% of individuals committed some type of intimate partner violence after disclosure of the diagnosis. Committing intimate partner violence was associated with alcohol use (OR = 2.79; 95%CI: 1.25-6.22; p = 0.012), the partner having relations with someone else during the current relationship (OR = 4.71; 95%CI: 2.24-9.91; p = 0.000), a history of violence prior to the STD (OR = 2.87; 95%CI: 1.22-6.73; p = 0.015), and having suffered violence after diagnosis of the STD (OR = 6.53; 95%CI: 3.06-13.93; p = 0.000). Intimate partner violence following disclosure of the STD signals that health professionals should identify patients' difficulties in revealing an STD diagnosis to their partners.

The food consumption of 15,071 public employees was analyzed in six Brazilian cities participating in the baseline for Brazilian Longitudinal Study of Adult Health (ELSA-Brasil, 2008-2010) with the aim of identifying eating patterns and their relationship to socio-demographic variables. Multiple correspondence and cluster analysis were applied. Four patterns were identified, with their respective frequencies: "traditional" (48%); "fruits and vegetables" (25%); "pastry shop" (24%); and "diet/light" (5%) The "traditional" and "pastry shop" patterns were more frequent among men, younger individuals, and those with less schooling. "Fruits and vegetables" and "diet/light" were more frequent in women, older individuals, and those with more schooling. Our findings show the inclusion of new items in the "traditional" pattern and the appearance of the "low sugar/low fat" pattern among the eating habits of Brazilian workers, and signal socio-demographic and regional differences.

This article aimed to determine the prevalence of latent Mycobacterium tuberculosis infection and associated factors in community health workers (CHW) in Brazil, using two cutoff points for the tuberculin skin test (5mm and 10mm). This was a cross-sectional study with data on gender, age, BCG scar, working in a primary care unit (PCU), having a household contact with tuberculosis (TB), alcohol consumption, previous tuberculin skin test (TST), smoking, time on the job as CHW, and comorbidities. Logistic regression was used to estimate odds ratios (OR) and to control for confounding. TST was applied and read after 48-72 hours. Prevalence rates were 57.88% and 37.3%, respectively, for the 5mm and 10mm TST cutoffs. Variables that remained associated with positive TST using the 10mm cutoff were working in a primary care unit that had implemented a Tuberculosis Control Program (TCP) and history of household contact with TB. Variables associated with the 5mm cutoff were working in a primary care unit with a TCP. Necessary actions at the local level include awareness-raising and strengthening of continuing education.

This ecological study assessed mammography and Papanicolaou test (Pap smear) screening rates and their association with the Human Development Index (HDI) in Brazilian state capitals and Federal District in 2011, based on VIGITEL telephone survey data. More than 70% of women had a mammogram some time in life in all the state capitals, while the Pap smear rates any time in life and in the previous three years fell short of the 80% target in most state capitals. There was a strong positive correlation between HDI and the test rates (r = 0.52 and 0.66 for mammogram any time in life and in the previous two years and r = 0.66 and 0.71 for Pap smear any time in life and in the previous three years, respectively). Prevalence ratio (PR) for mammogram in the previous two years was 1.06 (95%CI: 1.01-1.10) and PR for Pap smear in the previous three years was 1.07 (IC95%: 1.04-1.10). The promotion of public health policies that encourage Brazil's urban economic and social development should help increase coverage for these tests.

To study mortality from liver cirrhosis in Espírito Santo State, Brazil, we reviewed death certificates (DC) from 2000-2010 and medical records of deceased patients with investigation of alcoholism and hepatitis B or C. From a total of 218,410 DC, 3,554 deaths from liver cirrhosis were retrieved. The annual mortality rate was 19.8/100,000 for men and 4.31/100,000 for women, without significant changes after correction for ICD-R98 and R99 and without a significant increase in the annual percentage change. In 49% of death certificates, the aetiology of cirrhosis was defined: of these alcoholism in 81.5% of cases and viral hepatitis in 15.7%. Aetiology was confirmed in 262 reviewed records, including alcoholism (40.5%); hepatitis B or C (26.7%); other (3.8%); and cryptogenic (10.6%). The mean annual potential years of life lost were 5,946 years and 1,739 years for men and women respectively. The mortality rate from cirrhosis in Espírito Santo State is intermediate in relationship to worldwide data; alcoholism and hepatitis B or C were the main aetiologies; probably alcoholism is overestimated and hepatitis B and C viruses are underestimated as causes of cirrhosis registered on death certificates.

This study aimed to compare the evaluation of adequate access to prenatal care according to different indices. Data to construct the indices were obtained from 1,006 patient interviews, prenatal cards, and medical charts for postpartum women who had been admitted for childbirth at maternity hospitals in Greater Metropolitan Vitória, Espírito Santo State, Brazil, from April to September 2010. The various indices for the evaluation of prenatal care were compared to the Kotelchuck index (1994) as the standard reference. Prevalence rates for adequacy were calculated, as were agreement, sensitivity, specificity, predictive values, accuracy, and likelihood ratios. The Takeda index showed the highest prevalence of adequacy (55.8%). The highest agreement was between the indices proposed by Villar et al. and Rosen et al. (adjusted kappa = 0.84). The study concludes that the Carvalho & Novaes index and the Brazilian Ministry of Health index are relevant for assessing adequate access to prenatal care.

Uterine cervical cancer is a leading cause of death from cancer in the female population worldwide. The aim of this study was to analyze survival of women with cervical cancer treated at the Santa Rita de Cássia Hospital/Women's Association for Cancer Education and Control (HSRC/AFECC) in Espírito Santo State, Brazil, from 2000 to 2005 and to describe associated prognostic factors. This was a cohort study using retrospective secondary data with a sample of 964 cases. The Kaplan-Meier curve and Cox model were used to evaluate survival and for multiple logistic analysis. There were 421 deaths (43.6%) during the minimum 5-year follow-up, with an overall 5-year survival of 58.8%. Risk factors were place of residence in the Serrana Region of the State (HR: 1.94; 95%CI: 1.09-3.45) and advanced staging. Women with stages III and IV at diagnosis showed an increased risk of 4.33 (95%CI: 3.00-6.24) and 15.40 (95%CI: 9.72-24.39), respectively, for lower survival when compared to stage I. The results show that early diagnosis and treatment are essential for reducing mortality from cervical cancer.

This study evaluated the reproducibility and relative validity of the Food Frequency Questionnaire (FFQ) used in the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil). Participants (n = 281) completed the FFQ and three food records on two occasions during a 12-month period. Energy and nutrient values from food records were disattenuated and log-transformed. Reproducibility and validity were assessed by the intra-class correlation coefficient (ICC). Agreement between the two methods was evaluated by classification in tertiles. In the evaluation of reproducibility, ICC estimated ranged from 0.55 to 0.83 for protein and vitamin E, respectively. On relative validity, ICC ranged from 0.20 to 0.72 for selenium and calcium, respectively. Exact and adjacent agreement between methods varied from 82.9% for vitamin E to 89% for lipids and calcium (mean 86%). Average disagreement was 13.6%. In conclusion, this FFQ showed satisfactory reliability for all nutrients and reasonable validity, especially for energy, macronutrients, calcium, potassium, and vitamins E and C.

This study aimed to assess the completeness of prenatal care information on the patients' prenatal care cards, according to coverage by various public health services: Family Health Strategy (FHS), Community-Based Health Workers' Program (CBHWP), and traditional Primary Care Units (PCU) in Greater Metropolitan Vitória, Espírito Santo State, Brazil. In a cross-sectional study, 1,006 prenatal cards were randomly selected from postpartum women at maternity hospitals in the metropolitan area. Completeness of the cards was assessed according to the criteria proposed by Romero & Cunha, which measure the quality on a scale from excellent (< 5% incomplete cards) to very bad (> 50% incomplete cards). In general, completion of information on the cards was bad (> 20% incomplete), but cards were filled out better in the FHS than in the CBHWP and PCU, especially for tetanus vaccination (p = 0.016) and gestational weight (p = 0.039). In conclusion, the quality of prenatal care in the public health system in Greater Metropolitan Vitória fails to meet the Brazilian national guidelines for maternal and child health.

Evaluation of health planning activities adapted to the local context is an essential part of daily work in the Family Health Strategy (FHS). This study aimed to evaluate the planning activities conducted by family health teams in Espírito Santo State, Brazil. This was an ecological study based on secondary data from 241 health teams in 46 municipalities (counties) in the State that use the Evaluation for Quality Improvement in the Family Health Strategy (AMQ-ESF) form. Criteria were set for evaluating the responses by health professionals to the quality standards in the AMQ-ESF forms, and health planning indicators were listed. The responses were stratified according to the municipality's population size and time since implementation of the FHS and population coverage. The study mostly showed low adequacy rates in the teams' work processes in relation to the AMQ quality standards, and stratification of the variables had no influence on the local planning practices. Specific measures are needed to upgrade the work processes in the FHS.

This cross-sectional study aimed to verify agreement between information given by mothers after delivery and data recorded on Pregnant Cards about antenatal care under the Brazilian Unified National Health System in the Metropolitan Region of Vitória, Espírito Santo State, Brazil. The study considered a population of 1,035 postpartum mothers interviewed in eight hospitals, where the cards were copied. The representativeness of the sample was guaranteed by stratification according to the proportion of births. Kappa and McNemar tests were carried out with the collected and processed information. Agreement levels regarding antenatal care were predominantly poor (kappa < 0.20). Mothers tend to: overestimate the number of antenatal visits (McNemar = 51.73; p-value = 0.001); affirm diseases during pregnancy, such as diabetes, anemia, hypertension and urinary infections; report the performance of laboratory tests; report the carrying out of clinical examinations. Results suggest the need to reflect on the type of data used for planning and implementing maternal and child public health polices, since data varies depending on the information source.

Leprosy may present acute/subacute inflammatory processes (leprosy reactions). The study characterized the reactional states of patients at health clinics in Vitória, Espírito Santo State, Brazil, and associated them with sociodemographic factors and clinical/nutritional variables. between January and December 2009, longitudinal follow-up of patients with leprosy continued until leprosy reactions occurred or patients completed 6 months of multidrug therapy. Of the 151 patients participating, 78 (51.7%) were females, 48 (31.8%) had 5 to 8 years schooling, 93 (61.6%) worked and earned from 1 to 3 minimum wages, and 55 (36.4 %) had leprosy reactions, but with no statistical association to socioeconomic characteristics or nutritional status. However, absence of reaction was more common in the low-weight group, suggesting a trend in this group to protection from the reaction (p = 0.0906). The study found no association between nutritional status and leprosy reaction.

The objective of this study was to compare the costs and outcomes associated with guardian-supervised directly observed treatment relative to the standard of care Directly Observed Therapy, Short Course (DOTS) provided by community health workers (CHW). New cases of culture-positive pulmonary tuberculosis (TB) treated in Vitória, Espírito Santo State, Brazil, between January 2005 and December 2006 were interviewed and chose their preferred treatment strategy. Costs incurred by providers and patients (and patients' families) were estimated, and cost-effectiveness was assessed by comparing costs per successfully treated patient. 130 patients were included in the study; 84 chose CHW-supervised DOTS and 46 chose guardian-supervised DOTS. 45 of 46 (98%) patients treated with guardian-supervised DOTS were cured or completed treatment compared to 70/84 (83%) of the CHW-supervised patients (p = 0.01). Logistic regression showed only the strategy of supervision to be a significant association with treatment outcome, with guardian-supervised care strongly protective. Cost per patient treated with guardian-supervised DOTS was US$398, compared to US$548 for CHW-supervised DOTS. The guardian-supervised DOTS is an attractive option to complement CHW-supervised DOTS.

Epidemiological studies have suggested that arterial hypertension is a chronic disease that begins in childhood, and that prematurity (birth at less than 37 weeks' gestational age) is potentially associated with the development of hypertension in childhood and adulthood. Our objective was to identify the association between prematurity and high blood pressure in children, using a systematic literature review. Original articles related to the theme and published in English, Portuguese, or Spanish from 1998 to 2009 were selected from the MEDLINE, LILACS, and SciELO databases. We excluded articles without abstracts, review articles, and articles not related to prematurity and hypertension in childhood. Nine articles were located and analyzed: 5 case-control studies, 2 cross-sectional studies, and 2 cohort studies. The majority of the studies failed to show an association between prematurity and arterial hypertension in childhood. However, the influence of prematurity should not be ruled out, given the small number of studies on this theme and the diversity of methodological approaches in the literature.
