Aiming to understand how transgender women have been served in institutions of the Unified Health System, the study adopted the qualitative design. The thematic content analysis of interviews and focus group resulted in three categories: Use of health services by trans women; Violence in health care; Lack of specialized care. The participants reported having no links with primary care, seeking care in the outpatient clinic linked to the teaching hospital and in emergency care. Cases of institutional violence, negligence, and prejudice permeate their experiences. Humanized care; accreditation of the hospital for the transsexualization process; investment in the worker’s well-being; creation of care protocols and complaint channels were suggested. Equity and integral attention require the expansion and qualification of services. Investments in permanent education are essential.

4. Prevalence and temporal trend of prematurity in Brazil before and during the COVID-19 pandemic: a historical time series analysis, 2011-2021

Objective: to measure the prevalence of prematurity according to the Brazilian macro-regions and maternal characteristics over the past 11 years; to compare the proportions during the COVID-19 pandemic (2020-2021) with those of the historical series (2011-2019). Methods: this was an ecological study, with data from the Live Birth Information System; the prevalence was calculated according to year, macro-region and maternal characteristics; time series analysis was performed using Prais-Winsten regression model. Results: the prevalence of preterm birth in 2011-2021 was 11.1%, stable; the average in the pandemic period 11.3% (95%CI 11.2;11.4%) was similar to that of the base period 11.0% (95%CI 10.6;11.5%); the North region (11.6%) showed the highest proportion between 2011 and 2021; twin pregnancy (56.3%) and pregnant women who had 4-6 prenatal care visits (16.7%) showed an increasing trend (p-value < 0.001); the highest prevalence was observed for extremes of maternal age, pregnant women of Black race/skin color, indigenous women and those with lower level of education. Conclusion: preterm birth rates were highest for socially vulnerable pregnant women, twin pregnancies and in the North; stable prevalence, with no difference between periods.

Resumo Com o objetivo de compreender o modo como mulheres transexuais têm sido atendidas em instituições do Sistema Único de Saúde, o estudo adotou o delineamento qualitativo. A análise de conteúdo temática de entrevistas e de grupo focal resultou em três categorias: utilização de serviços de saúde por mulheres trans; violência na Atenção à Saúde; falta de atendimento especializado. As participantes referiram não possuir vínculos com a Atenção Primária e buscar atendimento no ambulatório vinculado ao hospital de ensino e em pronto atendimento. Casos de violência institucional, negligência e preconceito permeiam suas experiências. Atendimento humanizado; credenciamento do hospital para o processo transexualizador; investimento no bem-estar do trabalhador; criação de protocolos de atendimento e canais de denúncia foram sugeridos. A equidade e a atenção integral requerem ampliação e qualificação dos serviços. Investimentos em Educação Permanente são essenciais.

Purpose: To compare four commercially available hydrogel formulations in the healing of partial thickness burns experimentally induced in rats. Methods: Wistar rats were used, and after the burn wound induction they were divided into the following treatment groups: G1) NaCl 0.9%; G2) 1% silver sulfadiazine; G3) Debrigel™; G4) Safgel™; G5) Dersani™; G6) Solosite™. Purpose This study aimed to elaborate a hydrogel constituted by carboxymethyl chitosan (CMC), hyaluronic acid (HA) and silver (Ag) and to evaluate its healing effect on partial-thickness burn wounds experimentally induced in rats. Methods CMC was obtained by chitosan reacting with monochloroacetic acid. The carboxymethylation was confirmed by Fourier-transform infrared spectroscopy and hydrogen nuclear magnetic resonance (NMR). Scanning electron microscopy was used to determine the morphologicalcharacteristics of chitosan and CMC. After the experimental burn wound induction, the animals (n = 126) were treated with different CMC formulations, had their occlusive dressings changed daily and were followed through 7, 14 and 30 days. Morphometric, macroscopic and microscopic aspects and collagen quantification were evaluated. Results Significative wound contraction, granulation tissue formation, inflammatory infiltration and collagen fibers deposit throughout different phases of the healing process were observed in the CMC hydrogels treated groups. Conclusions The results showed that, in the initial phase of the healing process, the most adequate product was the CMC/HA/Ag association, while in the other phases the CMC/HA association was the best one to promote the healing of burn wounds.

Resumo: Introdução: Os movimentos sociais, organizados em torno da reforma sanitária, contribuíram para a institucionalização do processo de formação em saúde no país que deve estar em consonância com as reais necessidades de saúde da população e, dessa forma, promover a inclusão e equidade na perspectiva da social accountability. Isso se torna ainda mais importante no contexto da pandemia da Covid-19, já que se vivencia uma nova prioridade de saúde. Entretanto, o que se tem observado ao longo da pandemia da Covid-19 é um despreparo profissional para um cuidado integral em saúde que considere as pessoas e comunidades historicamente invisibilizadas. Objetivo: Analisar criticamente as questões das diversidades em relação às Diretrizes Curriculares Nacionais para os Cursos de Graduação em Medicina (DCN). Desenvolvimento: Por meio de três blocos temáticos - “Análise comparativa das DCN de 2001 e 2014 na perspectiva da diversidade”,“Como podemos problematizar as questões da diversidade a partir das DCN de 2014?” “As DCN de 2014 e o que precisamos tornar mais evidente na busca pela diversidade no ensino médico” -, aprofundou-se o debate crítico e reflexivo sobre a educação médica a partir da perspectiva de diversidade. Constatou-se a necessidade de uma articulação formativa e assistencial com as demais políticas públicas em saúde, principalmente aquelas relacionadas a populações marginalizadas. Conclusão: A pandemia da Covid-19 mostra-se como uma oportunidade de a mídia e a sociedade como um todo olharem para as desigualdades sociais em saúde, considerarem a relevância do SUS e enfatizarem os múltiplos apontamentos contemporâneos que evidenciam que os projetos pedagógicos e os componentes curriculares dos cursos de Medicina precisam ser atualizados e se comprometer com a construção de uma proposta de ensino e cuidado em saúde que valorize a diversidade e diminuição das iniquidades em saúde.

The environmental scenario requires the participation of the society in the integration of individual responsibilities to the political action. The recycling pro-environmental behavior contributes to the proper disposal of solid waste, a major challenge in urban public management. The article aims to analyze the scientific production of the pro-environmental behaviors and recycling in order to weave notes that promote behavioral changes provided for in public policies. The PsycINFO, Scielo and EBSCOhost databases were consulted using the keywords: environmental attitudes, pro-environmental behavior and recycling. We selected 51 articles, among which predominated the investigation into motivation to recycle behavior, influence of social network and social norms. Environmental attitudes and behavior to recycle have shown to be related to social norms, motivation, identity, altruism and awareness; these should be considered when planning public policies and environmental awareness actions.

Avaliou-se o efeito do silício sobre o desempenho de nove cultivares de alface cultivadas em solução nutritiva no sistema hidropônico NFT. O experimento foi conduzido em ambiente protegido, durante os meses de março a maio de 2003. O delineamento experimental foi o de blocos casualizados em esquema fatorial 9x2, com três repetições, com os fatores: nove cultivares de alface (Vera, Verônica, Elisa, Regina, Aurélia, Tainá, Lucy Brown, Uberlândia-10000 e Mimosa Salad Bowl); e duas soluções nutritivas, uma contendo silício - 1,5 mmol L-1 e outra sem silício. A fonte de silício utilizada foi a sílica gel de origem japonesa, com 20% de SiO2 e 80% de H2O. As características analisadas foram: diâmetro da cabeça, massa seca e fresca de parte aérea e de raiz e, concentração de silício nas folhas e raízes. As plantas de alface que receberam silício apresentaram um menor tamanho quando comparadas às plantas cultivadas em solução sem silício, apesar de, em ambos os casos, terem alcançado tamanho comercial. As plantas tratadas com silício apresentaram menor incidência da anomalia fisiológica queima dos bordos. Dessa forma o silício pode ser uma alternativa viável para produção hidropônica de alface, principalmente quando se leva em consideração as novas tendências de mercado por produtos menores e com boa aparência. The effect of silicon was evaluated on the performance of nine lettuce cvs, cultivated in the nutritive solution in hidroponic system NFT. The experiment was conducted in protected cultivation, from March to May 2003. The randomized complete blocks design 9x2 was used, with three replications. Two factors were studied: nine lettuce cultivar (Vera, Verônica, Elisa, Regina, Aurélia, Tainá, Lucy Brown, Uberlândia-10000 and Mimosa), and presence (1,5 mmol L-1) or absence of silicon (japanese "silica gel") in the nutritive solution with 20% of SiO2 and 80% of H2O. The head diameter, weight of dry and fresh matter of head and roots, the concentration of silicon in leaves and roots were analyzed. The lettuce plants which received silicon presented a lower weight in comparison to the ones cultivated in solution without silicon, although both reached commercial weight. The lettuce treated with silicon presented lower incidence of physiological anomalies such as tip burn. Therefore, the silicon may be a reliable alternative for hydroponics production of lettuce mainly when considering the new market tendencies for products with better appearance.

OBJETIVO: avaliar os efeitos de drogas anti-retrovirais isoladas e em associação sobre as taxas de fertilidade em ratas prenhes expostas a estes fármacos, bem como o efeito perinatal nas crias. MÉTODOS: foram utilizadas ratas fêmeas prenhes adultas da raça Wistar, pesando inicialmente entre 200 e 230 g. Foram testadas a azidotimidina (AZT), lamivudina (3TC) e nelfinavir (NFV), cujas dosagens foram padronizadas em 10 vezes a dose normalmente utilizada em gestantes, proporcionalmente ao peso dos animais. No total foram avaliados sete grupos, incluindo o controle. O início da experimentação foi o dia zero da prenhez e as cesarianas foram realizadas com 21 dias de prenhez. Para o cálculo das taxas de fertilidade, avaliaram-se o número de fetos vivos e mortos, sítios de implantação e número de corpos lúteos. Utilizaram-se o teste t de Student e Mann-Whitney para a análise estatística. RESULTADOS: não houve alterações significativas nas taxas de perdas pré-implantação e da eficiência de implantação de ratas tratadas com anti-retrovirais isolados e em associação. No entanto, houve aumento significativo nas taxas de perdas pós-implantação (grupo controle: 7,6%; grupos medicamentos: de 20,2 a 26,7%), redução significativa nas taxas de viabilidade fetal (grupo controle: 92,4%; grupos medicamentos: de 73,3 a 79,8%) e redução significativa no número de fetos por ninhada (grupo controle: 14,7, grupos medicamentos: de 11,1 a 12,7). Observou-se também que houve redução do peso materno e dos fetos nos grupos tratados com 3TC, AZT + 3TC e AZT + 3TC + NFV. CONCLUSÃO: com a utilização de doses elevadas de medicamentos anti-retrovirais na presente casuística, efeitos importantes sobre a fertilidade foram encontrados, o que sinaliza para a necessidade de se pesquisarem outros fármacos anti-retrovirais com menor potencial histotóxico e que possam, com segurança, ser utilizados por gestantes portadoras da infecção pelo vírus da imunodeficiência humana. PURPOSE: to evaluate experimentally the effects of antiretroviral drugs used alone and in association upon the fertility of pregnant Wistar rats and the perinatal effects on the offspring. METHODS: adult female pregnant Wistar rats weighing 200-230 g were used. The antiretroviral drugs zidovudine (AZT), lamivudine (3TC) and nelfinavir (NFV) were used alone and in association at daily doses of ten times the dose normally used in pregnant women, proportionally to the animal's body weight. Seven groups were studied, including the control one. The experiment started on day 0 and the pregnant animals were sacrificed on day 21. The alive and dead fetuses, the total implantation sites and the total numbers of corporea lutea were used to calculate the fertility values. The statistical analysis was performed by Student's t test and by the Mann-Whitney test. RESULTS: there were no significant statistical differences regarding preimplantation loss and implantation efficiency values of the rats treated with isolated and associated antiretroviral drugs. There was a significant increase in the postimplantation loss values (control group: 7.6%; drug groups variation: 20.2-26.7%), a decrease in the fetal viability values (control group: 92.4%, drug groups variation: 73.3-79.8%), and a decreasing number of fetuses per animal (control group: 14.7; drug groups variation: 11.1-12.7). There was a significant weight reduction of the female rats and of the offspring of animals treated with 3TC, AZT + 3TC and AZT + 3TC + NFV. CONCLUSION: with the administration of high antiretroviral doses, important fertility effects could be observed, which showed that less histotoxic antiretroviral drugs must be studied in order to warrant the safety of using these medicines in pregnant HIV-1 - infected women.
