IntroductionA common concern for individuals with moderate-to-profound hearing loss who have received a cochlear implants (CIs) is difficulty following conversations in noisy environments. A possible reason is the increased use of cognitive resources (attention and working memory) to disambiguate the speech stimuli potentially causing detriments in functional aspects of life. However, this relationship is not well-documented. The overall arching goal of this study was to quantify the relationships between Patient-Reported Outcome Measures (PROMs) and cognitive ability through working memory and speech perception measures.MethodsIn the current study, 31 adult CI users and typical hearing (TH) age-matched controls were recruited. CI users completed PROMs including Speech, Spatial and Quality of hearing survey (SSQ) and versions of a quality of life (QOL) for CI users (CIQOL) and Nijmegen Cochlear Implant Questionnaire (NCIQ). Measures of cognition were assessed using internet-based tools. Outcomes were compared between groups and PROMs and were related to the cognitive tasks.ResultsReduced auditory working memory and speech perception in noise were observed in CI users compared to TH controls. Correlation analyses revealed significant domain-specific PROM relationships with cognitive tasks in CI users, but not in TH controls. The SSQ had more significant relationships compared to the other PROMS.ConclusionThese results suggest that internet-based metrics of cognitive ability are associated with the functional hearing aspects of life in CI users and that SSQ is more sensitive to investigate the cognitive related effects of QOL compared to other commonly used PROMs.

Healthy diets are unaffordable for almost 3.1 billion people worldwide, and in 2018, already 43% of the African population were urban dwellers. Our food systems are changing rapidly, coupled with rising dietary aspirations. What are healthy diets and how can healthy diets be reached and increased in an urban context was the question of project NOURICITY. Looking into different information channels to deliver information on healthy diets in a low-income area in urban Kampala, Uganda, two packages of interventions were developed based on information on the selected focus group discussions with parish representatives. The first package consisted of a flier including graphic information on healthy diets and three food groups (treatment group 1). The second included the same flier plus interactive voice responses (IVRs) as a phone call to deliver the flier information in a different format (treatment group 2). For the study, we targeted 450 randomly selected households, which were randomly assigned into three groups (control, treatment group 1, and treatment group 2). Respondents from Kanyanya (a low-income parish of Kampala city) were randomly selected. They were visited two times in March as well as in December of the year 2021, while the intervention was rolled out in the period from September to November 2021. Healthy diets are measured using the Household Dietary Diversity Score and the food variety score is based on a 7-day food consumption recall, while dietary quality is measured for a subsample of women and the minimum dietary diversity for all of the selected women. The results indicated statistically significant changes per group at household food consumption in March 2021 compared to December 2021. However, the results of the difference-in-difference method between the control and the treatment groups did not display any significant difference at the household level. However, increased dairy and meat consumption in the treatment groups was observed. Over 90% of the households in treatment group 2 listened to any type of IVR message. Although no significant treatment effects were found, almost all households receiving intervention package 2 mentioned that the IVR calls were easy to follow, while almost 80% indicated sharing the information with their neighbors and 92% enjoyed receiving the message. However, the intervention has potential but needs to be improved upon.

Water is a factor input for many food system activities such as agriculture, food processing and consumption. However, food system activities also affect water resources. Moreover, the shift in focus of food security in Low- and Middle-Income Countries (LMICs) from producing enough staple foods toward healthy diets stimulates local production of fresh food such as fruit, vegetables and fish even in water scarce regions. To secure local production, polluted water is used for food production, processing and consumption, which might jeopardize human health. However, scientific evidence is still scattered and fragmented. The aim of this study is to systematically investigate the empirical tested impacts of water quality on the food system activities and vice versa. Using a comprehensive framework, we sketch the inter-relationships between water quality and food systems based on a literature study. Food system activities included food production (crop production, livestock and aquaculture), food processing, and food consumption. Multiple contaminants were incorporated such as nitrogen, phosphorus, pesticides, pathogens, cyanotoxins, and heavy metals. Moreover, we considered different water sources such as groundwater, surface water, wastewater and coastal water. We found that food system activities contaminate water in several ways, and these differ between food system activity and type of food produced. The impact of water quality on the food system depends on the food produced, the type of contaminant and techniques of food preparation. In addition, food is contaminated in multiple ways along the food system. Irrigation with polluted water may sound familiar, but polluted water is sometimes also used in food processing (cleaning of equipment or food products), and in food preparation (at home or by street vendors). Hygiene in food consumption is crucial to prevent fecal-oral transmission. However, water, sanitation and hygiene (WASH) received little attention in relation to food consumption. If local production of fresh food is encouraged to improve food security, all aspects of water quality should be analyzed to avoid undesirable consequences.

Ablative fractional laser treatment facilitates epidermal drug delivery, which might be an interesting option to increase the topical efficacy of biological drugs in a variety of dermatological diseases. This work aims at investigating safety and tolerability of this new treatment approach in patients with plaque-type psoriasis. Eight patients with plaque-type psoriasis were enrolled in this study. All patients received (i) ablative fractional laser microporation (AFL) of a psoriatic lesion with an Er:YAG laser + etanercept (ETA; Enbrel® solution for injection) (AFL-ETA), (ii) ETA alone on another lesion, and, if feasible, (iii) AFL alone on an additional lesion. Overall, all treatment arms showed a favorable safety profile. AFL-ETA improved the lesion-specific TPSS score by 1.75 vs. baseline, whereas ETA or AFL alone showed a TPSS score improvement of 0.75 points, a difference that was not statistically significant and might be attributable to differences in baseline scores. Topical administration of ETA to psoriatic plaques via AFL-generated micropores was generally well-tolerated. No special precautions seem necessary in future studies. Clinical benefit will need assessment in sufficiently powered follow-up studies.
