The short term clinical and radiological results of a hydroxyapatite-coated hip prosthesis with bonding osteogenesis on the hydroxyapatite ceramic and bony tissue reaction had to be studied. 233 patients who had hip replacement were followed up for two to five years and assessed radiologically and by the Merle D'Aubigne rating. Primary total hip arthroplasty as well as revision arthroplasty show sufficient clinical results. Tight pain and aseptic loosening of the stem did not occur. Hydroxyapatite-coating and contour for the Furlong hip prosthesis show a reliable short term anchorage.

In einer prospektiven Studie wurden 102 Patienten mit malignen Tumoren der Mundhohle, des Oro- und Hypopharynx sowie des Larynx mit der Magnetresonanztomographie (MRT) untersucht, wobei besonders Wert auf die Bestimmung derjenigen Tumorausdehnungen gelegt wurde, die die Behandlungsstrategie beeinflussen. Die Ergebnisse wurden mit den Befunden der Palpation, Sonographie, Computertomographie und pathologisch-anatomischen Gutachten bzw. OP-Berichte korreliert. Die Ergebnisse zeigen, das mit der MRT Tumoren der Kategorien pT2 bis pT4 in den untersuchten Regionen gut abgrenzbar sind. Die Beurteilung insbesondere kleiner Lasionen des weichen Gaumens, der Tonsillen, der Sinus piriformes und der Stimmbander bereitet jedoch Schwierigkeiten. Die MRT erwies sich neben der Endoskopie als gut geeignete Methode bei der pratherapeutischen Diagnostik von Tumoren der Zunge, des Oropharynx, des Hypopharynx und des Larynx. In a prospective study 102 patients with malignant tumors of the tongue, oropharynx, hypopharynx, and larynx were staged by means of MRI. Special attention was directed at those tumour extensions that could influence the treatment strategy. The MR findings were correlated with the results of palpation, endoscopy, ultrasound, computerised tomography and histopathological findings. MRI showed a good delineation of pT2 to pT4 tumours. However, visualisation of small tumours in the soft palate, the tonsils, the pyriform sinuses and vocal cords was difficult. In conclusion our results suggest that in addition to endoscopy MRI is a valuable tool for the examination of tumours of tongue, oropharynx, hypopharynx and larynx.

A retrospective review is presented of the clinical and roentgenographic findings of 40 patients with malignant lymphomas of the stomach treated at the Medical University Hannover during the period from 1971-1983. All cases of Non-Hodgkin-Lymphoma (35 primary and 5 secondary) were classified by modified Kiel classification. All patients were staged retrospectively, using modified Ann Arbor staging. In 33 patients the primary choice of treatment were operation, in 4 chemotherapy, and in three patients primary radiation therapy. The male-female ratio was 1:1, the average age at the moment of diagnosis was 58 years. The majority of the tumors were located in the corpus and the antrum with an average diameter of 8 cm. The evaluation of the roentgenographic presentation of gastric lymphoma showed in 75% a combination of different radiologic patterns. According to the main radiologic feature the ulcerating form was found in 11, the polypoid type in 10, the disruption of the mucosal picture (area, folds) in 12, and the gastric wall infiltration was found in 7 of all patients. The 5-year survival rate of all cases with primary lymphoma of the stomach was 55%.

Pre-operatively, radiological examination is important for the diagnosis of malignant tumours in the stomach. Postoperatively, it can be used to demonstrate the state of the anastomosis and possible defects of the suture, for the early recognition of recurrences or other abnormalities, and for the diagnosis of reflux oesophagitis and the dumping syndrome. Changes in the oesophagus following various types of reconstruction were examined, both clinically and radiologically, in 40 patients who had had gastrectomies. Following jejunal interposition with a proximal end-to-side anastomosis and a long blind segment, there may be reflex into the oesophagus, but symptoms of reflux oesophagitis only occur if there is also duodeno-jejunal reflux. Patients with a Roux loop show few symptoms, while the worst radiological and clinical results were achieved with a double-barrelled jejunal loop, despite a low anastomosis.

Zusammenfassend last sich sagen, das das Praparat Bayer 1238 weitgehend sowohl die verschiedenen Vorzuge der bisher bekannten Kontrastmittelgruppen aufweist als auch die Nachteile der einzelnen Kontrastmittelarten vermissen last, wodurch es zu einem Kontrastmittel wird, dessen breitere Anwendung empfohlen werden kann. In the authors' opinion the substance Bayer 1238 possesses most of the advantages of previously known types of contrast medium without the disadvantages of these substances. its extended use can therefore be recommended. On peut dire en resume que la preparation Bayer 1238 offre non seulement tous les avantages des autres substances de contraste connues utilisees pour les bronchographies, mais ne presente encore aucun des desavantages qu'elles peuvent determiner; elle constitue ainsi une substance de contraste dont on ne peut que recommander la generalisation de l'emploi. Resumiendo puede decirse que el preparado Bayer 1238 presenta ampliamente todas las distintas ventajas de los medios de contraste conocidos hasta ahora, al mismo tiempo que carece de los inconvenientes de aquellos, por lo que reune las cualidades de una substancia de contraste recomendable en un amplio margen de aplicacion.

The authors report on 2 patients who developed dense opacities of the thinned cornea during long-term unconsciousness under intensive care, due to insufficient care of the cornea in the case of lagophthalmic keratitis. Although the keratoplasty performed was finally successful, postoperative treatment was very difficult, because the very thin host cornea made re-suturing necessary. The authors recommend using a donor graft 0.2 to 0.3 mm larger in such cases. In order to prevent a keratitis with subsequent corneal opacities. It is very important to instruct the nursing staff to take care to apply eye ointments regularly in unconscious patients with lagophthalmus.
