Abstract

ZusammenfassungDie Mammasonografie hat sich seit vielen Jahren neben der Mammografie als wichtige Methode zur Abklärung von Brustbefunden etabliert.Der Arbeitskreis Mammasonografie der DEGUM beabsichtigt mit der „Best Practice Guideline“ den senologisch tätigen Kolleginnen und Kollegen neben dem in Teil I publizierten aktuellen Dignitätskriterien- und Befundungskatalog in dem vorliegenden Teil II die additiven und fakultativen Anwendungsmodalitäten zur Abklärung von Brustbefunden zu beschreiben und dazu DEGUM-Empfehlungen zu äußern, um die Differenzialdiagnose von unklaren Läsionen zu erleichtern.Die vorliegende „Best Practice Guideline“ hat sich zum Ziel gesetzt, den Anforderungen zur Qualitätssicherung und der Gewährleistung einer qualitätskontrollierten Durchführung der Mammasonografie nachzukommen. Die wichtigsten Aspekte der Qualitätssicherung werden in diesem Teil II der Best Practice Guideline erläutert.

Abstract

The aim of the present study was to determine the accuracy of breast cancer measurement with 2 D shear wave elastography (2 D SWE), B-mode ultrasound, and mammography by comparing these methods with the actual histopathological tumor size. 135 patients with proven malignant breast lesions at two centers were included in a prospective study. The maximum lesion diameter was measured by 2 D SWE, B-mode ultrasound, and mammography. After surgery, the histopathological tumor size was measured by a pathologist. The mean difference in tumor size on 2 D SWE, B-mode ultrasound, and mammography on the one hand, and the actual tumor size on the other, was -0.03 cm, 0.16 cm and 0.10 cm, respectively. Lesion size on histopathological investigation was overestimated by 2 D SWE (p = 0.004) and underestimated by B-mode imaging (p 0.001). All three imaging methods underestimated the size of invasive lobular cancers and lesions 15 mm; 2 D SWE was most accurate in this regard. 2 D SWE predicted lesion size more precisely than B-mode ultrasound or mammography. In cases of invasive lobular carcinoma, all three imaging methods underestimated lesion size, with 2 D SWE coming closest to the actual tumor size.ZIEL: Ziel der Studie ist es, die Genauigkeit der Brustkrebsgrößenmessung mittels 2D-Scherwellenelastografie (2D-SWE), B-Bild-Ultraschall und Mammografie zu evaluieren und mit der tatsächlichen histopathologischen Tumorgröße zu vergleichen. Diese an 2 Zentren durchgeführte prospektive Studie umfasste 135 Patienten mit nachgewiesenen malignen Brusttumoren. Der maximale Durchmesser wurde mittels 2D-SWE, B-Bild-Ultraschall und Mammografie gemessen. Nach operativer Entfernung der Tumoren wurde die histopathologische Tumorgröße von den Pathologen gemessen und als Goldstandard zum Vergleich mit den bildgebenden Messungen verwendet. Die mittlere Differenz zwischen der gemessenen und der tatsächlichen Tumorgröße für 2D-SWE, B-Bild-Ultraschall und Mammografie betrug –0,03 cm, 0,16 cm bzw. 0,10 cm. Die 2D-SWE überschätzt (p = 0,004), während der B-Bild-Ultraschall (p 0,001) die Größe der histopathologischen Veränderung unterschätzt. Alle 3 bildgebenden Verfahren unterschätzen die Ausdehnung invasiv lobulärer Karzinome und Tumoren 15 mm, wobei die 2D-SWE hier am genauesten misst. Die Größenmessung maligner Brusttumoren ist mit der 2D-SWE genauer als mit dem B-Bild-Ultraschall und der Mammografie. Bei invasiv lobulären Karzinomen unterschätzen alle 3 bildgebenden Verfahren die tatsächliche Größe des Tumors, wobei die 2D-SWE der tatsächlichen Tumorgröße am nächsten kommt.

Abstract

Malignant degeneration of colon endometriosis is a very rare event. We report here on three cases. A 48-year-old woman with a 10-year history of endometriosis was treated for a rectal adenocarcinoma, a 61-year-old G1P1, who was operated at the age of 40 years for ovarian endometriosis and again at the age of 53 years for an endometriosis-associated endometroid ovarian carcinoma, presented for therapy for a lymph node recurrence of the ovarian cancer and, secondly, due to a malignantly degenerated rectum-sigmoid colon endometriosis; furthermore a 54-year old woman with a 21-year history of endometriosis was operated for malignant colon endometriosis. The tumour occurred during an adjuvant anti-oestrogen treatment with an aromatase inhibitor following surgical and radiotherapy for breast cancer. In all cases a radical cancer operation was followed by adjuvant chemotherapy and in one case with an additional radiotherapy. In the follow-up periods of 18 months, 2 and 5 years, respectively, all women remained free of recurrences. Although this is not a randomised controlled study due to the rare occurrence of such cases, a radical operation followed by individualised adjuvant therapy appears to be the treatment of choice.Die maligne Entartung einer Darmendometriose ist sehr selten. Wir berichten über 3 Fälle. Eine 48-jährige Patientin mit 10-jähriger Endometrioseanamnese wurde wegen eines Adenokarzinom des Rektums behandelt; eine 61-jährige G1P1, die erstmals im Alter von 40 Jahren wegen Ovarialendometriose und dann im Alter von 53 Jahren wegen eines Endometriose-assoziierten endometrioiden Ovrialkarzinoms operiert wurde, kam wegen Lymphknotenrezidiv des Ovarialkarzinoms und zweitens wegen einer maligne entarteten Rektum-Sigma-Endometriose zur Therapie; ferner wurde eine 54-jährige Patientin, die seit 21 Jahren an Endometriose litt, wegen maligner Darmendometriose operiert. Der Tumor war während einer adjuvanten Antiöstrogenbehandlung mit einem Aromatasehemmer nach operiertem und bestrahltem Mammakarzinom entstanden. In allen Fällen erfolgte eine radikale Karzinomoperation gefolgt von adjuvanter Chemotherapie und in einem Fall zusätzlich adjuvanter Radiatio. Im Nachuntersuchungszeitraum von 18 Monaten, 2 bzw. 5 Jahren sind alle Frauen bisher rezidivfrei. Obwohl es keine randomisierten, kontrollierten Studien wegen der Seltenheit der Fälle gibt, scheint die radikale Operation gefolgt von einer individualisierten adjuvanten Therapie die Behandlung der Wahl.
