The article discusses the guidelines for endoscopic submucosal dissection (ESD) which is designed to complete resection of neoplastic lesions with early gastric cancer (EGC) risk. The guideline is aimed to familiarize Spanish endoscopists and gastroenterologists with the general indications of the procedure, the possible complications, and the dedicated tools. ESD is recommended as the best method for esophageal superficial neoplasms, squamous carcinoma, and Barrett's esophagus.

Thoracoscopy and mediastinoscopy are common procedures with painful incisions and prominent scars. Transesophageal endoscopic mediastinoscopy could reduce pain, improve cosmesis, and provide better access to the posterior mediastinal compartment, especially in patients with a complex mediastinum because of previousmediastinoscopy or with tracheostoma.
