Introduction: home enteral nutrition (HEN) has undergone an important development; however, there is a notable lack of information with regard to its incidence and characteristics. Objectives: our aim was to assess the state of HEN in our area. Methods: an observational, prospective study, involving all patients who had initiated HEN in the Nutrition Unit during a year. Epidemiological, functional, and nutritional evolution of the patients was described and incidence of HEN was calculated. Results: HEN incidences totalled 229/100,000 inhabitants/year. The HEN population in our area was characterized by the aged and a high frequency of comorbidity and functional limitations. Neurological and oncological diseases accounted for 50% of indications. The remaining cases were malnourished patients who had received short periods of HEN after hospitalization or a hip fracture. Oral supplements (60%) with standard and hypercaloric formulas were used the most. At baseline, 75% of the patients suffered from malnutrition. During the follow-up, patients showed weight gain (1.6%), an increase in the percentage of normal weight and overweight (from 74% to 82.7%, p = 0.001) and a reduction in pressure ulcers (15.7% vs 10.3%, p < 0.001). The median duration of HEN was 8.5 months. Only a quarter of the patients experienced complications (mostly mild gastrointestinal complications); 43.1% had died at the end of the follow-up. Conclusions: in our area, the HEN incidence was much higher than those described in the literature. HEN appears to be a safe therapy with few complications that improves the nutritional status of the patients, even with short periods of administration. Resumen Introducción: la nutrición enteral domiciliaria (NED) ha experimentado un importante desarrollo, aunque aún existe una notable carencia de información acerca de su incidencia y características. Objetivos: evaluar el estado de la NED en nuestra área. Métodos: estudio observacional y prospectivo que incluyó a todos los pacientes que iniciaron NED en el periodo de un año. Describimos su evolución epidemiológica, funcional y nutricional y calculamos la incidencia de NED. Resultados: la incidencia de NED alcanzó los 229/100.000 habitantes/año. La población con NED se caracterizó por ser añosa, con una elevada frecuencia de comorbilidad y limitación funcional. Las enfermedades neurológicas y oncológicas representaron el 50% de las indicaciones. El resto fueron pacientes malnutridos que recibieron periodos cortos de NED tras una hospitalización o fractura de cadera. Los suplementos orales con fórmulas estándar o hipercalóricas fueron los más utilizados (60%). Al inicio, el 75% de los pacientes tenía malnutrición. En el seguimiento, los pacientes lograron aumento de peso (1,6%) y mayor porcentaje de normopeso o sobrepeso (de 74% a 82,7%, p = 0,001). Las úlceras por presión se redujeron (15,7% vs. 10,3%, p < 0,001). La duración mediana de NED fue 8,5 meses. Solo una cuarta parte de los pacientes experimentaron complicaciones, la mayoría de ellas gastrointestinales y leves. El 43,1% había fallecido al final del seguimiento. Conclusiones: en nuestra área, la incidencia de NED es más elevada respecto a lo descrito en la literatura. La NED es un tratamiento seguro con pocas complicaciones que mejora el estado nutricional de los pacientes, incluso con periodos cortos de administración.

NutriQoL® was reliable [ICC: 0.88 (95%CI: 0.80-0.93); Cronbach's α: 0.77 (1st visit) and 0.83 (2nd visit)], valid (significant Rho), lowly sensitive to changes (effect size 0.23), can be completed by either patients or caregivers (ICC: 0.82). The mean HRQoL (SD) with NutriQoL® was 14.98 (14.86), EQ-5D tariff: 53(0.25), EQ-5D VAS: 54.15(20.64) and COOP/WONCA charts: 23.32(5.66). HRQoL with NutriQoL® was better (p < 0.05) for oral HEN [19.54(13,23)], than nasoenteric tube [14(11.71)], ostomy [7.02 (15.48)]; administered orally [19.54 (13.23)], than by gravity [10.97 (14.46)], pump [8.5 (19.78)] or syringe bolus [7 (11.40)]; as a supplement [19.33 (13.73)] instead of sole source of nutrition [8.18 (14.23)]. Conclusions: NutriQoL® is valid, reliable, even if lowly sensitive to change, and useful to measure HRQoL in this population. More studies are needed to know HRQoL in routine practice. Introducción: la calidad de vida relacionada con la salud (CVRS) permite disponer de una visión global del estado de salud del paciente que recibe nutrición enteral domiciliaria (NED). Objetivo: evaluar la CVRS de pacientes con NED usando el cuestionario NutriQoL®, herramienta específica para pacientes con NED independientemente de la patología subyacente y vía de administración. Materiales y métodos: estudio observacional, prospectivo, multicéntrico, en el contexto de la validación y evaluación de las propiedades psicométricas del cuestionario NutriQoL®. Resultados: se incluyeron 140 individuos [patologías: oncológica (58,6%), malabsorción y otros (27,1%), neurológica (13,6%); NED: complemento a la alimentación (64,4%), única nutrición (35,7%); vía de administración: oral (54,3%), ostomía (31,4%) y sonda naso-entérica (12,1%)]. El NutriQoL® resultó fiable [CCI: 0,88 (IC95%: 0,80-0,93); α de Cronbach: 0,77 (1ª visita) y 0,83 (2ª visita)], válido (Rho significativas), aunque poco sensible a los cambios (tamaño del efecto: 0,23), pudiendo ser cumplimentado por el paciente o su cuidador (CCI: 0,82). La CVRS media (DE) con NutriQoL® fue 14,98(14,86), con la tarifa EQ-5D: 53(0,25), EVA EQ-5D: 54,15(20,64) y viñetas COOP/WONCA: 23,32(5,66). La CVRS medida con NutriQoL® fue mejor (p < 0,05) en pacientes con NED por vía oral [19,54(13,23)] que con sonda naso-entérica [14(11,71)] u ostomía [7,02(15,48)]; administrada por vía oral [19,54(13,23)] que por gravedad [10,97(14,46)], bomba [8,5(19,78)] o bolo con jeringa [7(11,40)]; como complemento [19,33(13,73)] que como única alimentación [8,18(14,23)]. Conclusiones: NutriQoL® es un cuestionario válido, fiable, aunque poco sensible a los cambios y útil para medir la CVRS en pacientes con NED. Son necesarios más estudios para conocer la CVRS de estos pacientes en la práctica habitual.

The aim of this study was to assess the state and economic impact of HAN in our health area. Methods: an observational, prospective study was designed to calculate the cost of nutritional formulas and materials in patients who initiated HAN in the nutrition unit during a year. Results: we included 573 new patients with HAN during the study period, 60% of whom were treated with oral supplements. The median daily cost of oral HAN was 3.65 (IQR 18.63) euros compared to a cost of 8.86 (IQR 20.02) euros for enteral-access HAN. The daily expenditure per 1 000 kcal of diet was higher for patients on oral HAN than for patients with tubes (5.13 vs. 4.52 euros, p < 0.001). The median cost of the complete HAN treatments during the study period was also calculated (186.60 euros and 531.99 euros for oral and tube HAN, respectively). The total estimated cost for all patients who initiated HAN in the study period was around one million euros. Conclusions: HAN represented an important economic burden in our health area, but the estimated daily cost of HAN was moderate, probably because of the high frequency of oral HAN, the adjusted treatments, and the centralized dispensation by the hospital pharmacy. Introducción: la nutrición artificial domiciliaria (NAD) es una terapia en creciente desarrollo, pero la ausencia de registros obligatorios hace difícil calcular la carga económica que implica. Nuestro objetivo es evaluar el estado y el impacto económico de la NAD en nuestra área sanitaria. Métodos: estudio observacional y prospectivo diseñado para estimar el gasto en fórmulas y materiales nutricionales en los pacientes que iniciaron NAD durante 1 año. Resultados: se incluyeron 573 pacientes que iniciaron NAD en el período de estudio. El 60% recibieron suplementos orales. La mediana de gasto diario fue de 3,65 (RIC 18,63) euros en la NAD oral y de 8,86 (RIC 20,02) euros en la NAD por acceso enteral. El gasto por 1.000 kcal de dieta fue superior en los pacientes con NAD oral respecto a los pacientes con NAD enteral (5,13 vs. 4,52 euros, p < 0.001). Asimismo se calculó la mediana de gasto completo de los tratamientos durante todo el período de estudio (186,60 euros y 531,99 euros en NAD oral y enteral, respectivamente). Considerando el gasto conjunto de todos los pacientes que iniciaron NAD en el período de estudio, el gasto estimado estaría en torno a un millón de euros. Conclusiones: la NAD representa una importante carga económica en nuestra área sanitaria. Sin embargo, el gasto diario estimado fue moderado, probablemente por la elevada proporción de NAD oral en nuestra muestra, la revisión frecuente de la indicación y la centralización de la dispensación desde el servicio de Farmacia Hospitalaria.

Objetivos: Analizar las complicaciones relacionadas con el soporte nutricional enteral en los pacientes con esclerosis lateral amiotrófica que forman parte de nuestro programa de ventilación mecánica domiciliaria, haciendo especial hincapié en las gastrointestinales. Método: Estudio retrospectivo de tipo descriptivo de enfermos que se incluyeron en nuestro Programa de Ventilación Mecánica Domiciliaria (PVMD) dirigido por médicos intensivistas, mediante la revisión sistemática de historias clínicas (procedentes de una base de datos de Microsoft Access), durante los años 2004-2011. Resultados: Entre los años 2004-2011 se siguieron 73 pacientes con diagnostico de Esclerosis lateral amiotrófica: 34 de ellos (46,6%) rechazaron el aporte nutricional a través de gastrostomía o de sonda nasogástrica, mientras que 39 (53,4%) aceptaron su colocación. De los 39 pacientes en los que se inició la NED: 20 eran mujeres mujeres (51,3%). La edad media de los pacienes fue de 60,6 + 13,4 años (IC 95% 56,464,8). Los diagnósticos al ingreso en el PVMD fueron: ELA, 21 casos (53,8%), y ELA con afectación bulbar, 18 (43,1%). Se alimentaron a través de GEP 34 pacientes (87,2%), con gastrostomía quirúrgica 3 (7,7%) y mediante sonda nasogástrica 3 (7,7%). La gastrostomía percutánea endoscópica se realizó tras la inclusión de los pacientes en el programa, con una media días de 222,7 + 356,6 (IC 95% 110,8-334,7). En pacientes con ELA la media fue de 271,4 + 449,5 días (IC 95% 130,3-412,1), con ELA y afectación bulbar de 126,4 + 131,3 días (IC 95% 90-172,6). El recambio de sonda fue de 7,3 + 4,8 meses (IC 95% 4-10,6). La nutrición enteral tuvo una duración media de 578,6 + 872,9 días (IC 95% 304,7-852,6). Se hallaron complicaciones en 35 pacientes (89,7%), y solamente en 4, no se encontró ninguna (10,3%). Ver tabla II. El estreñimiento se manifestó, después del inicio de la NE, en 30 pacientes (76,9%); sin embargo, ya existía previamente en 18 de ellos (60%). El resto de complicaciones gastrointestinales evidenciadas fueron: distensión abdominal, en 9 pacientes (23,1%); dolor abdominal, en 6 (15,4%); sensación nauseosa y vómitos, en 5 (12,8%); y diarrea, en 3 (7,7%). Conclusiones: Las complicaciones gastrointestinales son las más frecuentes; destaca el estreñimiento como problema fundamental en pacientes con ELA y NED. Sin embargo, no se puede considerar una complicación exclusiva debida al soporte nutricional ya que también forma parte de la evolución de la enfermedad. La aparición de granulomas es también muy frecuente. Objectives: To analyze the complications related to enteral nutritional support in patients with lateral amyotrophic sclerosis included in our home-based mechanical ventilation program (HMVP), with a special emphasis on gastrointestinal complications. Method: retrospective, descriptive study of the patients included in our HMVP directed by intensive care physicians, by means of systematic review of the medical records (included in a Microsoft Access database) during the period 2004-2011. Results: in the period 2004-2011, 73 patients with a diagnosis of lateral amyotrophic sclerosis were followed: 34 (46.6%) refused nutritional support through gastrostomy or nasogastric tube, whereas 39 (53.4%) accepted. Twenty (51.3%) of the 39 patients with (home-based nutritional support) were females. The mean age of the patients was 60.6 + 13.4 years (95% CI 56.4-64.8). Diagnoses at the time of inclusion in the HMVP were: LAS, 21 cases (53.8%), and LAS with bulbar impairment 18 (43.1%). 34 patients (87.2%) were fed through percutaneous endoscopic gastrostomy (PEG), 3 (7.7%) through surgical gastrostomy, and 3 (7.7%). PEG was performed at the time of inclusion of the patients in the program, with a mean duration of 222.7 + 356.6 days (95% CI: 110.8-334.7). In patients with LAS, the mean duration was 271.4 + 449.5 days (95% CI: 130.3-412.1), and with LAS and bulbar impairment of 126.4 + 131.3 days (95% CI: 90-172.6). The mean duration of the nasogastric tube was 7.3 + 4.8 months (95% CI: 4-10.6). The mean duration of enteral nutrition was 578.6 + 872.9 days (95% CI: 304.7-852.6). There were complications in 35 patients (89.7%), and no complications occurred in only 4 patients (10.3%) (See table 2). Constipation occurred after initiating EN in 30 patients (76.9%); however, it was already present in 18 of them (60%). The remaining gastrointestinal complications observed were: abdominal bloating (9 patients; 23.1%); abdominal pain (6; 15,.4%); nausea and vomiting (5; 12.8%); diarrhea (3; 7.7%). Conclusions: gastrointestinal complications are the most common ones; constipation stands out as the main problem in patients with LAS and HEN. However it may not be considered as a complication exclusively due to nutritional support since it is also a manifestation in the disease course. The occurrence of granulomas is also common.

Aims: To assess the degree of satisfaction of caregivers of children with gastrostomy tube (GT) feeding through the structured questionnaire SAGA-8. Secondly, to evaluate if the parental satisfaction degree was related to several independent variables. Methods: A cross-sectional observational study was performed in 92 caregivers of children with GT feeding and chronic diseases. The following data was obtained: caregiver satisfaction with GT feeding (SAGA-8), age at GT placement, anthropometric data, length and mode of nutritional support, family demographic characteristics and caregiver psychological status. Results: All primary caregivers were mothers. High satisfaction with GT feeding was expressed by 82.6% of families. The simplicity of the system was emphasized by 87%, and 85.9% were very satisfied with the support received from the hospital staff. 73.9% of mothers acknowledged their child's nutritional status had improved and 89.1% rated the enhancement family's overall situation. Moreover, 75% of mothers reduced feeding-time, and 68.5% reported less respiratory infections. Finally, 71.7% of mothers recognized that they would have implemented this technique earlier. Caregiver satisfaction was positively correlated with age at GT placement and length of treatment, and both variables explained the 19.4% of the satisfaction variance. No correlation with anthropometric data, nutrition support mode, family demographic characteristics or caregiver psychological status was observed. Conclusions: The SAGA-8 questionnaire is a simple, specific, straight-forward tool to evaluate parental/caregiver degree of satisfaction with GT feeding and facilitates effective monitoring of the intervention. Lengths of HEN and precocious age at GT placement are responsible for most of parental satisfaction. Objetivos: Valorar el grado de satisfacción de los cuidadores de niños con alimentación mediante gastrostomía aplicando el cuestionario estructurado SAGA-8. Segundo, evaluar si la satisfacción parental guarda relación con diversas variables independientes. Métodos: Se realizó un estudio observacional, transversal en 92 cuidadores de niños con enfermedades crónicas que recibían alimentación mediante gastrostomía. Se analizaron los siguientes datos: satisfacción de los cuidadores con la alimentación por gastrostomía (SAGA-8), edad de implantación de la gastrostomía, parámetros antropométricos, tiempo de soporte nutricional, características demográficas familiares y estado psicológico del cuidador. Resultados: Todos los cuidadores eran madres. La mayoría (82,6%) manifestaron una elevada satisfacción con la alimentación mediante gastrostomía. El 85,9% resaltó la sencillez de la técnica sintiéndose el 85,9% muy satisfecha con el apoyo ofrecido por el centro. Un 73,9% de las madres percibió mejoría nutricional en su hijo, señalando el 89,1% un mayor bienestar familiar. Además, el 75% de las madres comunicaron una disminución en el tiempo de alimentación y el 68,5% una reducción en la morbilidad respiratoria. Finalmente, el 71,7% de las madres reconoció que hubieran aceptado antes el procedimiento. La satisfacción se correlacionó significativamente con la edad de implantación de la gastrostomía y con la duración del soporte nutricional, explicando ambos factores el 19,4% de la varianza de la satisfacción. No se encontró correlación con datos antropométricos, tipo de soporte nutricional, características demográficas familiares ni con el estado psicológico del cuidador. Conclusiones: El cuestionario SAGA-8 es sencillo, específico y fácil de aplicar para evaluar el grado de satisfacción de los padres/cuidadores con la alimentación mediante gastrostomía contribuyendo a su monitorización. La duración del soporte nutricional y la precocidad en la implantación de la gastrostomía son los responsables principales de la satisfacción de los cuidadores.

La nutrición enteral domiciliaria (NED) surge de la necesidad de continuar con el soporte nutricional especializado (SNE) en pacientes cuya situación clínica ya no justifica su hospitalización. En el registro del grupo NADYA del año 2010 sobre NED en España la vía de acceso más frecuente era la sonda nasogástrica (51%), seguida por la gastrostomía (27%), vía oral (10%), y por último la yeyunostomía (3%). La yeyunostomía en catéter de aguja es una técnica recomendada en pacientes sometidos a cirugía abdominal mayor candidatos a nutrición enteral en el postoperatorio, pero son escasas las publicaciones relativas a su uso como vía de acceso para NED a largo plazo. Presentamos una serie de casos en que ésta fue la vía seleccionada. En este momento suponen el 1,14% de los episodios de NED abiertos en nuestra unidad, con un tiempo de permanencia media del catéter de 210 ± 222 días. Las complicaciones fueron frecuentes, pero todas ellas poco relevantes. Home enteral nutrition (HEN) is the choice for patients who can not mantain oral intake but have a functioning gastrointestinal tract when it isn't justify keeping the patient in the hospital. The results of the HEN registry of the NADYA group in 2010 shows information related to the enteral acces route: 51% of the cases used nasogastric tubes, 27% gastrostomies, 10% oral route and 3% jejunostomies. Placement of a needle catheter jejunostomy is recommended for candidates for enteral nutrition undergoing major abdominal surgery, but publications about long-term use of this acces are scarce. We report same cases of patients whom the selected enteral acces was surgical jejunostomy. They represents the 1,14% of our patients with HEN at this moment, with a mean dwelling time of 210 ± 222 days. There has been frequent complications, but they were sligh.

Background & aims: Home enteral nutrition is used increasingly in pediatric populations. Our objective was to describe the profile of pediatric patients requiring this treatment. Material and methods: All patients under 18 years old requiring treatment with home enteral nutrition between January 1995 and December 2004 were analyzed retrospectively. Results: 304 patients were studied (157 boys). The mean age at the start of treatment was 4.02 ± 4.09 years, median of 2.5 years; 28% of all patients were under 1 year. The main indications were oncological disease in 91 patients (29.9%) and digestive diseases in 84 (27.6%). There were significant differences depending on the clinical diagnosis for the start age, type of access, infusion regime and formula prescribed. Nutrients were delivered by nasogastric tube in 218 patients (71.7%). Overnight enteral nutrition was the preferred infusion regime in 155 patients (51%). Adult or pediatric polymeric formulas were mostly prescribed in 190 patients (62.5%). The mean treatment duration was 306 ± 544 days. Conclusion: In our series, enteral support usually begins at an early age. Its characteristics varied depending on patient pathology. Knowledge of the pediatric patient profile is important to design the most effective strategy for home enteral nutrition. Nuestro objetivo ha sido describir el perfil de un grupo de pacientes pediátricos que precisaron este tratamiento. Material y métodos: Se analizaron retrospectivamente todos los pacientes menores de 18 años que precisaron tratamiento con nutrición enteral domiciliaria entre enero 1995 y diciembre 2004. Resultados: Se estudiaron 304 pacientes (157 niños). La edad media al inicio del tratamiento fue de 4,02 ± 4,09 años, con una mediana de 2,5 años; un 28% de los pacientes eran menores de 1 año. Las indicaciones principales fueron la enfermedad oncológica en 91 pacientes (29,9%) y la digestiva en 84 (27,6%). Se encontraron diferencias significativas en función del diagnóstico clínico para la edad de inicio, el tipo de acceso, el modo de administración y la fórmula prescrita. El tipo de acceso más utilizado fue la sonda nasogástrica en 218 pacientes (71,7%). El régimen de infusión más utilizado fue la nutrición enteral nocturna en 155 pacientes (51%). Se prescribió principalmente fórmula polimérica de adultos o pediátrica en 190 pacientes (62,5%). La duración media del tratamiento fue de 306 ± 544 días. Conclusión: En nuestra serie, el soporte enteral suele iniciarse a edades muy tempranas. Sus características variaron en función de la patología del paciente. Conocer el perfil del paciente pediátrico es importante para diseñar la estrategia más eficaz en el uso de la nutrición enteral domiciliaria.

Objetivos: Describir los resultados del registro de nutrición enteral domiciliaria (NED) del grupo NADYA-SENPE del año 2010. Material y métodos: Se recopilaron los datos introducidos en el registro desde el 1 de enero al 31 de diciembre de 2010. Resultados: Se registraron 6.591 pacientes (51% varones) con 6.688 episodios de NED, procedentes de 32 hospitales. La edad media en los menores de 14 años (4%) fue de 1 ± 2 años (m ± DS) y de 69,9 ± 17,8 en los mayores de 14 años. El 76% de los pacientes recibieron la NED por un tiempo superior a 2 años. La patología más prevalente fue la neurológica 42%, seguida de la neoplasia 28% (en su mayoría cáncer de cabeza y cuello 18%). La información referente a la vía de acceso sólo se recogió en 626 casos (9,4%), el 51% de los pacientes utilizaron sonda nasogástrica, 27% gastrostomías, 10% vía oral y 3% yeyunostomías. Sólo 251 episodios finalizaron a lo largo del año, siendo el motivo más frecuente el fallecimiento del paciente en el 57% de los casos y el paso a la alimentación oral en el 14%. El 29% de los pacientes presentaban una actividad limitada y el 39% estaba confinado en cama/sillón. El 68% de los adultos requerían ayuda total o parcial. El suministro del producto se realizó desde el hospital o la farmacia de referencia en el 63% y 34%, respectivamente. El suministro del material fungible se realizó desde el hospital o atención primaria en el 83% y 16%, respectivamente. Conclusiones: Los resultados obtenidos en el registro de NED del año 2010 muestran características muy similares a las recogidas en los años previos en cuanto al número y características de los pacientes registrados. Seguimos encontrando problemas en la recogida de datos relativos a la vía de acceso y finalización de los episodios. Objective: To describe the results of the home enteral nutrition (HEN) registry of the NADYA-SENPE group in 2010. Material and methods: We retrieved the data of the patients recorded from January 1st to December 31st 2010. Results: We registered 6,591 patients (51% males) with 6,688 episodes of HEN, from 32 hospitals. Mean age in those younger than 14 yr (4%) was 1 ± 2 yrs (m ± SD) and 69,9 ± 17,8 yrs in those older than 14 yr. The length of HEN was longer than 2 yrs in 76% of the patients. The most frequent underlying disease was neurological disorders 42%, followed by cancer 28% (mostly head and neck cancer 18%). We had information related to the enteral access route in only 626 cases (9,4%), 51% of them used nasogastric tubes, 27% gastrostomies, 10% oral route and 3% jejunostomies. Only 251 episodes were closed during the year, mostly due to patient death 57% and progress to oral diet 14%. The activity level was limited in 29% of the patients and 39% of them were bed- or chairridden. Total or partial help was needed by 68% of the patients. The hospitals and the private pharmacies delivered the enteral formula in 63% and 34% of the cases, respectively. The hospitals and the primary care centres delivered the disposables in 83% and 16% of the cases, respectively. Conclusions: The results of the 2010 HEN registry are similar to those published in previous years regarding the number and characteristics of the patients. We continue finding problems in the entrance of data referred to the enteral access route and the closing of the episodes.

Whenever possible, enteral nutrition (EN) is used to promote intestinal rehabilitation and reduce PN dependency. Our aim is to verify whether EN + oral intake (OI) in severe short bowel syndrome (SBS) surgical adult patients can maintain adequate nutritional status in the long term. Methods: This longitudinal retrospective study included 10 patients followed for 7 post-operative years. Body mass index (BMI), percentage of involuntary loss of usual body weight (UWL), free fat mass (FFM), and fat mass (FM) composition assessed by bioelectric impedance, and laboratory tests were evaluated at 6, 12, 24, 36, 48, 60, 72, and 84 months after surgery. Energy and protein offered in HPN and at long term by HEN+ oral intake (OI), was evaluated at the same periods. The statistical model of generalized estimating equations with p < 0,05 was used. Results: With long term EN + OI there was a progressive increase in the UWL, a decrease in BMI, FFM, and FM (p < 0,05). PN weaning was possible in eight patients. Infection due to central venous catheter (CVC) contamination was the most common complication (1.2 episodes CVC/patient/year). There was an increase in energy and protein intake supply provided by HEN+OI (p < 0.05). All patients survived for at least 2 years, seven for 5 years and six for 7 years of follow-up. Conclusions: In the long term SBS surgical adult patients fed with HEN+OI couldn't maintain adequate nutritional status with loss of FM and FFM. Antecedentes: La nutrición parenteral (NP) se emplea para controlar el estado nutricional después de resecciones intestinales extensas. Siempre que sea posible, se empleará la nutrición enteral (NE) para favorecer la rehabilitación intestinal y reducir la dependencia de la NP. Nuestro propósito fue verificar si la NE + ingesta oral (IO) en el síndrome del intestino corto (SIC) grave en pacientes adultos quirúrgicos puede mantener un estado nutricional adecuado a largo plazo. Métodos: Este estudio longitudinal retrospectivo incluyó 10 pacientes seguidos durante 7 años tras la intervención quirúrgica. Se evaluaron el índice de masa corporal (IMC), el porcentaje de pérdida involuntaria del peso corporal habitual (PCH), la masa grasa libre (MGL) y la composición de la masa grasa (MG) mediante impedancia bioeléctrica, así como los datos de laboratorio a los 6, 12, 24, 36, 48, 60, 72 y 84 meses tras la cirugía. Se evaluaron en los mismos periodos la energía y las proteínas aportadas con la NPD y a largo plazo con la NED + ingesta oral (IO). Se utilizó un modelo estadístico de ecuaciones estimativas generalizadas con una p < 0,05. Resultados: Con la NE + IO a largo plazo hubo un aumento progresivo del PCH, una descenso del IMC, la MGL y la MG (p < 0,05). La retirada de la NP fue posible en ocho pacientes. La complicación más frecuente fue la infección por contaminación del catéter venoso central (CVC) (1,2 episodios CVC/paciente/año). Hubo un aumento en el consumo de energía y proteínas proporcionadas por la NED + IO (p < 0,05). Todos los pacientes sobrevivieron al menos dos años, siete durante 5 años y seis durante los 7 años de seguimiento. Conclusiones: los pacientes adultos con SIC quirúrgico nutridos a largo plazo con NED + IO no pudieron mantener un adecuado estado nutricional con una pérdida de MG y de MGL.

Home enteral nutrition (HEN) is a type of enteral nutrition (EN) which is becoming progressively more widespread in pediatrics due to the benefits it affords to patients, their families and to reducing hospital costs. However, the true extent of its use is unknown in Spain as the data-base set up for this purpose is still underused (Registro de Nutrición Enteral Pediátrica Ambulatoria y Domiciliaria -NEPAD-). More thorough registration of patients in the NEPAD online register will provide information about the characteristics of HEN in Spain: prevalence, diagnosis, the population sector being administered HEN, complications and developments. Likewise, forecast and planning of the necessary resources could be made while those in use could be analysed. La nutrición enteral domiciliaria (NED) es una modalidad de nutrición enteral (NE) con una progresiva extensión en pediatría por los beneficios que supone para el paciente, sus familiares y la disminución de los costes hospitalarios. Sin embargo, se desconoce su verdadero alcance en España ya que el registro creado a tal efecto está aún infrautilizado (Registro de Nutrición Enteral Pediátrica Ambulatoria y Domiciliaria -NEPAD-). La inclusión exhaustiva de pacientes en NEPAD permitiría conocer las características de la NED en España: prevalencia, indicaciones, población a la que se le está administrando, complicaciones y evolución. Así mismo se podría realizar una previsión y planificación de los recursos que se precisan y analizar los que se están utilizando.

Objetivo: Conocer las características y la evolución de los pacientes diagnosticados de neoplasia de cabeza y cuello (NCC) que precisaron recibir tratamiento de radioterapia y que fueron atendidos en un programa de nutrición enteral por sonda a domicilio (NESD). Análisis del peso en función de administrar la nutrición enteral antes o después del inicio de la radioterapia. Material y métodos: Estudio observacional de pacientes con NCC incluidos en el programa de NESD de nuestro hospital durante 2 años. Variables analizadas: sexo, edad, índice de masa corporal (IMC), índice de Karnofsky (IK), motivo del inicio de soporte nutricional, tipo de sonda para la administración de la nutrición, tipo de fórmula nutricional y aporte calórico prescrito, necesidad de cambio de vía y días de NESD. Resultados: Se incluyeron 62 pacientes (77,4% varones y 22,6% hembras) con una edad media de 64 años ± 10,1 (rango entre 39 y 90). La disfagia fue el motivo principal de colocación de sonda en estos pacientes. Predominio de sonda nasogástrica (67,7%). La fórmula más utilizada fue la polimérica hipercalórica con un aporte calórico de 1.629 ± 267,09 kcal/día. Globalmente, se observó una pérdida de peso en todos los pacientes durante el período de estudio. Sin embargo, aquellos en los que se inició la NESD antes de la radioterapia el IMC no disminuyó. Conclusión: Los pacientes que iniciaron nutrición por sonda con anterioridad al tratamiento oncológico no perdieron peso durante el periodo de estudio. Objective: To know characteristics and the patients' evolution with head and neck cancer who received radiotherapy treatment and they were included at a home enteral nutrition (HEN) by feeding tube programme. To analyse the weight evolution according to the start of HEN before or after radiotherapy. Methods: Observational study of tube feeding patients with head and neck cancer who were included in HEN programme in our hospital for two years. Variables analysed: gender, age, Body Mass Index (BMI), Karnofsky Index (KI), reason for nutritional support, type of feeding tube, formula used and prescribed caloric contribution, necessity to change access device and HEN days. Results: 62 patients were studied (77.4% men; 22.6% women). Age 64 ± 10.1 years (rang: 39-90). The dysphagia was the main cause to begin enteral nutrition by feeding tube in these patients. Naso-gastric tube was prevalence (67.7%). The most used formula was polymeric hypercaloric diet with a mean of caloric contribution of 1,629 ± 267.09 kcal/day. Overall, there was a weight loss in all patients during the study period time. However, patients who began the HEN by feeding tube before the radiotherapy treatment, the BMI did not decrease. Conclusion: All patients who began feeding tube before oncological treatment didn't lose weight for the period of study.

El índice de Karnofsky (IK) es una escala funcional desarrollada en oncología y ampliamente utilizada, que ha mostrado ser útil para predecir la evolución en pacientes oncológicos y geriátricos. Teóricamente, como herramienta de valoración funcional podría predecir la mortalidad en pacientes con nutrición enteral domiciliaria (NED). Objetivos del estudio: Conocer el IK inicial y su evolución a 6 meses en pacientes en régimen de NED y estudiar la relación entre la tasa de mortalidad de los mismos con dicho índice. Material y métodos: Estudio observacional y prospectivo realizado durante 2002 y 2003 con pacientes neurológicos y oncológicos con nutrición enteral por sonda seguidos durante 10 meses desde su inclusión en el programa de NED. Resultados: Se estudiaron 201 pacientes, 131 (65,2%) neurológicos y 70 (34,8%) oncológicos, sin existir entre ambos grupos diferencias significativas en la edad, días con NE y mortalidad al final del periodo de estudio (35,1% en pacientes neurológicos y 40% en oncológicos). A los 10 meses un mayor porcentaje de pacientes oncológicos había recuperado la capacidad de ingesta oral (27,1% frente a 10,7% en los neurológicos, p < 0,05). Los valores del IK fueron mayores en los pacientes oncológicos que en los neurológicos en las tres determinaciones (inicial, a los 3 y a los 6 meses, p < 0,001). Dicho índice no se modificó significativamente en ninguno de los dos grupos durante su evolución. Analizando el conjunto de los pacientes se observó una relación significativa entre los valores iniciales del IK y la media de supervivencia a los 10 meses (p < 0,001), y una relación inversa entre los valores iniciales del índice y el porcentaje de mortalidad (p < 0,001). Conclusión: El IK es una herramienta útil para predecir la mortalidad en pacientes oncológicos y neurológicos en programa de NED. Karnofsky Index (KI) is a widely used functional scale developed for oncology patients. It has proved useful as outcome predictor with cancer and geriatric patients. Theoretically, KI could be used to predict mortality in patients with home enteral nutrition (HEN). Study objectives: To determine baseline KI and its 6-month evolution in HEN patients, and to assess its relation with the mortality rate. Methodology: Observational and prospective study carried out during 2002 and 2003 with tube feeding neurologic and cancer patients followed during 10 months since their HEN programme inclusion. Results: 201 patients were included, 131 (65.2%) with neurological diseases and 70 (34.8%) with neoplasm. There were not significant differences between groups in age, days with HEN and mortality rate at the end of the study period (35.1% in neurologic patients and 40% in cancer ones). 27.1% of cancer patients had resumed full oral nutrition after ten months from the beginning of the study, whereas only 10.7% of neurologic patients did (p < 0.05). In the three measurement phases (initial, past-3 and past-6 months) KI values were higher for cancer patients than for neurologic ones (p < 0.001). In both groups we didn't found statistically significant differences in KI along the three measurements. A significant relation was observed overall between initial KI values and average survival after 10 months (p < 0.001), and an inverse relation was found between the former and mortality rate (p < 0.001). Conclusion: KI is a useful tool to predict mortality rate in cancer and neurologic patients under HEN.

La Hospitalización a Domicilio (HAD) es una opción asistencial que posibilita proporcionar cuidados propios de un hospital en el domicilio del paciente. El soporte nutricional (SN) -enteral por sonda o parenteral- suele indicarse en pacientes de edad avanzada y/o severamente discapacitados por lo que la nutrición artificial cobra gran interés en la atención domiciliaria. Objetivos: Analizar las características de los pacientes, atendidos en el domicilio, por cuestiones específicas del SN que reciben. Material y métodos: Estudio descriptivo, observacional y retrospectivo de los pacientes atendidos por la Unidad de Soporte Nutricional (SN), en el ámbito de la HAD, desde el 1 de Septiembre del 2006 hasta el 31 de Agosto del 2007. Resultados: En el domicilio del paciente el procedimiento realizado fue: recambio de sonda de ostomia en 158 ocasiones; modificación de la pauta de nutrición enteral (NE) o nutrición parenteral (NP) en 53 casos; adiestramiento de la técnica de nutrición artificial en 14 casos. Se realizaron 39 visitas urgentes por complicaciones -fundamentalmente por infección o incontinencia del estoma y por obstrucción de la sonda-. Sólo en 3 pacientes (7,7%) la asistencia domiciliaria indicó el traslado del paciente al Servicio de Urgencias. Conclusión: En nuestro centro, la infraestructura de la HAD ha permitido dar respuesta a las necesidades de los pacientes que reciben SN domiciliario en nuestra área de influencia. The Hospital at Home (HAD) is a choice of care that enables own care in a hospital at home patient. Moreover, the nutritional support (NS) -enteral or parenteral nutrition- is usually indicated in patients with serious underlying disease, and/or frequently remain severely disabled. Aims: To analyze the characteristics of the patients, attended at home for specific questions of the NS that receive. Material and methods: descriptive and retrospective study of the patients attended by the Nutritional Support Unit (NSU), in the area of the HAD, from September 1, 2006 until August 31, 2007. Results: At home, the realized procedure was: refill of gastrostomia or jejunostomia feeding tube in 158 cases; modification of the guideline of enteral nutrition (EN) or parenteral nutrition (PN) in 53 cases; training of the skill of artificial nutrition in 14 cases. 39 visits were realized by complications -by infection or lead throught the estoma and by obstruction of the feeding tube-. Only in 3 patients (7.7%) the domiciliary assistance indicated the movement of the patient to the Emergency Unit. Conclusion: In our center, the infrastructure of the HAD has allowed to give answer to the needs of the patients who receive NS at home in our area of influence.

Background: There are few data on trends in home enteral nutrition (HEN) practice in different countries. NADYA is the Spanish home artificial nutrition (HAN) group, and is responsible for the Spanish HAN registry. Method: We performed a 16-year retrospective study (1992-2007) of the Spanish HEN registry by retrieving data from the NADYA database and from publications of the working group. People receiving more than 1000 kcal/d with an enteral formula were included regardless of the access route (oral/tube feeding). Results: The number of patients registered increased more than 8 times during the study period: the current prevalence is 113 patients/10(6) inhabitants (oral and tube feeding), or 41 patients/10(6) inhabitants (tube feeding). The distribution of the patients was not uniform, and most came from six autonomous communities (Catalonia, Galicia, Castilla-León, Madrid, Andalusia and Extremadura). Gender distribution was nearly 1:1. The number of paediatric patients was very low, representing less than 10% of the total. Mean age in adults was above 65 years in most of the reports. We observed an increase in the age of the patients over the years. The most common underlying diseases were neurological disorders, followed by cancer. We observed an increase in the use of the oral route, from 5.8% in 1992 to 64% in 2007, with a parallel decrease in the use of nasogastric tubes. Gastrostomy tubes were used in 15-20% of the patients. The number of complications was low (less than one complication/patient/year), the most frequent being change of tube, followed by gastrointestinal complications. The principal reasons for discontinuing treatment were death related to the underlying disease (40-50%) and switch to oral diet (30-40%). Most of the patients (75%) were followed by the hospital nutrition unit. Provision of the enteral formula and disposables varied according to the autonomous community. Most of the patients had limited physical activity or were chair- or bed-bound, requiring partial or total help in their daily life. Conclusions: The use of HEN has increased dramatically in the last 16 years in Spain. Most of the patients registered were elderly people with neurological disorders and limited physical activity. The oral route was the most frequently used. The number of complications was low. The mortality rate in these patients was highly related to the underlying disease.

La nutrición enteral a domicilio es un tratamiento cada vez más utilizado por las ventajas que comporta no sólo al paciente y familiares o cuidadores (menos riesgo de infecciones nosocomiales y mejor integración socio-familiar) sino también a la administración en el sentido de representar un ahorro en coste económico además de disponer de mas camas de hospitalización. Objetivo: Analizar el grado de satisfacción de los pacientes y familiares o cuidadores en relación a un servicio de Nutrición Enteral Domiciliaria (NED) controlado desde la Unidad de Nutrición del Hospital. Metodología: Un cuestionario para los pacientes y otro para los familiares/cuidadores fue remitido a los enfermos que recibiendo nutrición enteral por sonda en su domicilio quisieron voluntariamente colaborar. Cuestionarios similares se remitieron a pacientes ingresados en el hospital tratados con nutrición enteral así como a sus posibles cuidadores en el caso de que recibieran este tratamiento a domicilio. Resultados: De acuerdo con los resultados, en general los pacientes y cuidadores consideran bueno o muy bueno el servicio recibido. Si este tratamiento se ofrece en el domicilio lo habitual es que quieran seguir con esta modalidad y no volver al hospital para recibirlo. Finalmente, la posibilidad de disponer de contacto telefónico seguido de visita si es necesaria, es la preferencia de elección. Conclusiones: Los resultados de las encuestas realizadas en nuestros pacientes/familiares permiten concluir que la nutrición enteral domiciliaría dentro de un programa de atención y seguimiento a domicilio controlado desde la Unidad de Nutrición del hospital de referencia es un tratamiento muy bien aceptado tanto por pacientes como por familiares de los mismos o cuidadores. Home enteral nutrition is a treatment carried out frequently due to advantages for patients and caregivers (lower risk of nosocomial infections and better integration in socio-familiar media) but also for the health administration because of the lower economic cost and the release of hospital beds. Objetive: To investigate patients' and caregivers' satisfaction related to tube enteral nutrition administered at home by a Service controlled and followed by the Nutritional Support Unit from the reference hospital. Methods: One questionnaire for the patients and another for the caregivers were delivered to the patients with home enteral nutrition asking for voluntary response. Similar questionnaires were sent to hospitalized patients and their relatives or possible caregivers. Results: According to the results of the questionnaires, the majority of patients and relatives were satisfied with the service received. In the cases in which the service was offered at home, the preference was to continue there and not to return to the hospital. Finally, the possibility of telephone contact and home visits were the most important factors that influenced patient satisfaction. Conclusion: Our results let us conclude that home enteral nutrition controlled and followed up by the Nutritional Support Unit form the hospital is a treatment well accepted both by patients and relatives.

Los registros son bases de datos que proporcionan información sobre una actuación o patología concreta en una población determinada. En el caso de la nutrición artificial domiciliaria (NAD) existen registros de pacientes en diferentes países europeos, EE.UU. y Japón con datos de prevalencia, indicaciones, seguimiento y complicaciones de estos tratamientos. En 1992 se creó en España el grupo NADYA, formado por profesionales con dedicación a la nutrición artificial. Una de sus inquietudes fue la creación de un registro de pacientes con NAD. Se trata de un registro voluntario al que se accede a través de la página web www.nadya-senpe.com. En el año 2005, se introdujeron algunas modificaciones para adaptarlo a la Ley de Protección de Datos. Gracias al trabajo de todos los miembros del grupo NADYA disponemos de muchos datos sobre la NAD en nuestro país. A partir de 1994, con excepción del período 97-98, se han publicado periódicamente registros anuales de nutrición parenteral domiciliaria (NPD) y nutrición enteral domiciliaria (NED), observándose un incremento en el número de pacientes y de centros colaboradores, que nos acercan a la práctica real de la NAD en nuestro país. The registries are databases including information about a treatment or a disease in a definite population. Concerning home artificial nutrition there are registries of patients in different European countries, USA and Japan, collecting data such as the prevalence, indication, follow-up and complications of these treatments. The Spanish group of Home Artificial Nutrition (HAN), NADYA, was established in 1992 with professionals devoted to the practise of artificial nutrition. Since then, one of its wills was the development of the Spanish registry of HAN. This is a voluntary registry accessible through the web page www.nadya-senpe.com. In 2005, this group decided to update the registry according to the Data Protection Law and including other improvements. Thank to the work of all the members of this group we have many data on the practise of HAN in our country. Since 1994, except the years 97-98, the annual registries of patients on home parenteral nutrition and home enteral nutrition have been published periodically, observing an increase in both, the number of patients and collaborating centres, presenting to us the actual practise of HAN in our country.

La Nutrición Enteral es una de las disciplinas más desarrolladas en la medicina moderna. Los avances tecnológicos, el mejor conocimiento de la fisiopatología de la desnutrición, y su participación en la evolución de diferentes entidades clínicas han hecho posible mejorar el cuidado nutricional de nuestros pacientes. La utilización de la NE se ha extendido como una práctica de primera elección en pacientes desnutridos o en riesgo de desnutrición que tienen un intestino minimamente funcionante y no son capaces de cubrir con la alimentación natural o suplementación, el total de sus requerimientos calórico-proteicos. Términos como "alimento-medicamento" o "nutrición órgano o sistema-específica o nutrición patología-específica" han revolucionado el campo de la NE en los últimos 20 años con la eclosión de fórmulas específicamente definidas .La NE ha demostrado ser coste-efectiva en los pacientes desnutridos cuando se establece la indicación con precocidad. La Nutrición Artificial Domiciliaria y Ambulatoria es una técnica regulada administrativamente muy extendida en nuestro país que permite reducir el coste de algunos procesos. En ocasiones la utilización o la retirada de la NE puede constituir una situación de conflicto ético que debe ser evitado desde el respeto, el acompañamiento y la información compartida entre el equipo sanitario, los pacientes y sus sustitutos. Enteral Nutrition is among the most developed disciplines in modern Medicine. Technological advances, a better knowledge of malnutrition physiopathology and its involvement in the evolution of several clinical entities have made it possible to improve the nutritional attention paid to our patients. The use of EN has expanded as a first choice practice in patients with undernutrition or at risk of undernutrition that have a minimally functional intestine and are unable to cover their total calorie and protein requirements with natural or supplemented diets. Terms like "medical food" or "organ- or system-specific nutrition" or "pathology-specific nutrition" have revolutionized the EN field in the last 20 years with the emergence of specifically defined formulations. EN has been shown to be cost-effective in patients with malnutrition when the indication is established early. Home and Ambulatory Artificial Nutrition techniques are administratively regulated and, as they are widespread in our country, they allow some cost reductions in certain processes. Occasionally, the introduction, use or withdrawal of EN may constitute a situation of ethical conflict that should be avoided by making use of respect, companionship and shared information between the health team, patients and their substitutes.

Objetivo: El consumo de productos de nutrición enteral domiciliaria financiados por el Sistema Nacional de Salud ha experimentado un notable incremento. El objetivo del trabajo es conocer su evolución durante los últimos cuatro años (1998-2001), las diferencias de precio de facturación de un mismo producto y la ocupación de mercado de los distintos preparados. Ámbito: Analizando las recetas de la Seguridad Social que fueron dispensadas por farmacias de Madrid, se ha realizado un estudio atendiendo a tipo de producto, número de envases, importe y empresas fabricantes. Al ser productos sujetos a la Ley de Defensa de la Competencia, los datos de importe se han tomado individualmente de cada receta. Resultados: Son importantes los aumentos entre noviembre de 1998 y mayo de 2000 del consumo de envases (66,6%) e importe (supera el 100%). El consumo se reparte de manera desigual entre tres empresas. El precio medio aumenta un 25,9%, disminuyendo la utilización de productos estándar a favor de productos modificados más específicos. La diferencia de precio de facturación del mismo producto, dependiendo de la farmacia que lo dispense, llega a un 120%. Conclusiones: El consumo en envases observa un crecimiento tendencial mantenido hasta el año 2000, ralentizándose en el 2001, no así en importe que mantiene su tendencia ascendente. Se incrementan los preparados envasados para administración con sistemas de infusión continua. Objective: The comsumption of products of home enteral nutrition financed by the national system of health, has experimented a remarkable increment. The purpose of this present paper is to evaluate the changes during the last four years (1998-2001), the differences between the costs in which a product is checked and the market occupation of each product. Scope: Analysing the official social security forms which were dispensed by pharmacies of Madrid, a study was realized attending to type of product, number of units, cost of each and manufactures industries. As these products are covered by the Spanish Defence of Competition, the data related to the costs has been taken individually of each social security form. Results: The increment between november of 1998 and may of 2000 in packings (66,6%) and costs (over 100%). Comsumption shared unequaly by three manufactures. The average cost increases 25,9%, whereas the utilization of standard products dicreases in favor of more specific modificated products. The difference between the cost in which a product is checked, depending on the pharmacy which dispenses it, reaches a 120%. Conclusions: The trend of increased comsumption in packings mantaines stable until the year 2000, slowing down in 2001, whereas the costs mantain this trend to increase. There is a remarkable increment in products in which administration involves a continuous infusion.

Objetivo: A pesar del número creciente de nutrición enteral domiciliaria (NED), sólo algunos artículos recogen la frecuencia de sus complicaciones. Este estudio multicéntrico analiza estas complicaciones en relación a la vía de acceso y la duración del soporte nutricional. Método: Seleccionamos aleatoriamente 92 pacientes con NED de 8 hospitales y se distribuyeron en relación a la duración del soporte y a la vía de acceso: sonda nasogástrica (SNG) y gastrostomías percutánea o quirúrgica. Tras un programa educativo, los pacientes completaron un cuestionario inicial que se repitió en los días 15 y 30. Recibieron una media de 1.650 kcal de fórmula enteral. Se analizaron un total de 2.760 días prospectivos de NED. Resultados: En la fase prospectiva, el 42% de los pacientes tuvo alguna complicación (112 episodios). Los más frecuentes fueron digestivos (55%) y mecánicos (29%); se registraron 0,16 complicaciones por paciente-año. Las complicaciones más comunes fueron: extracción (15%), estreñimiento (13%), vómitos (12%) y diarrea (10%). El grupo de gastrostomía tuvo más complicaciones digestivas. En el análisis retrospectivo, el grupo de gastrostomía percutánea tuvo menos complicaciones y el de SNG requirió más recambios de sondas (4 frente a 2) y tuvieron 1,96 complicaciones/paciente (grupo de gastrostomía percutánea 1,85 y quirúrgica 3,1). Conclusión: La NED es segura y presenta una baja incidencia de complicaciones. Un programa educativo adecuado es muy útil y esperamos, en el futuro, contribuir a un mejor sistema de atención domiciliaria. Aim: In spite of the increasing number of home enteral nutrition (HEN) patients, only few articles had reported the frequency of complications related to this treatment. Our multicentric study analyzes the HEN complications in relation to access device and time of treatment. Method: 92 HEN patients from 8 hospitals were randomly selected. Patients were distributed in relation to the time of treatment and access device (nasogastric tube and percutaneous or surgical gastrostomies). After an educational program, they were filled in an initial questionnaire and repeated it the days 15 and 30. They received a mean of 1650 Kcal of enteral solution. A total of 2760 HEN prospective days were analyzed. Results: In prospective study 42% of patients had some complication (112 episodes). The most frequent were gastrointestinal (55%) and mechanical (29%); 0.16 complications of patient-year were registered. The most common complications were: extraction (15%), constipation (13%), vomiting (12%) and diarrhoea (10%). The gastrostomy group had more gastrointestinal complications. In retrospective evaluation, percutaneous gastrostomy group had the lowest ratio of complications and nasogastric tube group required more tube replacements (4 vs 2) and had 1.96 episodes/patient (percutaneous group 1.85 and surgical gastrostomy 3.1 episodes/patient). Conclusion: HEN is safe with low incidence of complications. An adequate educational program is very important and we expect, in the future, to establish an proper National Home Care System.
