Abstract: Sustained response off-treatment (SROT) after thrombopoietin receptor agonist (TPO-RA) discontinuation has been reported in ITP. This prospective multicenter interventional study enrolled adults with persistent or chronic primary ITP and complete response on TPO-RAs. The primary endpoint was the proportion of patients achieving SROT (platelet count > 30x109/L and no bleeding) at W24 with no other ITP-specific medications. Secondary endpoints included the proportion of sustained complete response off-treatment (SCROT, platelet count > 100x109/L and no bleeding) and SROT at W52, bleeding events, and pattern of response to a new course of TPO-RAs. We included 48 patients with median (IQR) age 58.5 years (41-73.5); 30/48 (63%) had chronic ITP at TPO-RA initiation. In the intention-to-treat analysis, 27/48 (56.2%, 95% CI, 41.2-70.5) achieved SROT; 15/48 (31.3%; 95% CI, 18.9-44.5) achieved SCROT at W24, and 25/48 (52.1%; 95% CI, 37.2-66.7) achieved respectively SROT and 14/48 (29.2%; 95% CI, 17.2-42.3) SCROT at W52. No severe bleeding episode occurred in patients who relapsed. Among patients re-challenged with TPO-RA, 11/12 achieved CR. We found no significant clinical predictors of SROT at W24. Single-cell RNA-seq revealed enrichment of a "TNFα signaling via NF-κB" signature in CD8+ T cells of patients with no sustained response after TPO-RA discontinuation, which was further confirmed by a significant overexpression of CD69 on CD8+ T cells at baseline in these patients as compared with those achieving SCROT/SROT. Our results strongly support a strategy based of progressive tapering and discontinuation of TPO-RAs for patients with chronic ITP who achieved a stable CR on treatment. Abstract: Objective To describe the main features at diagnosis and evolution over time of patients with localized granulomatosis with polyangiitis (L-GPA) compared with those of systemic GPA (S-GPA). Methods EULAR definitions of L-GPA, i.e. upper and/or lower respiratory tract involvement, and S-GPA were applied to patients from the French Vasculitis Study Group Registry. L-GPA and S-GPA patients’ characteristics at diagnosis and long-term outcomes were analysed and compared. Results Among the 795 Registry patients, 79 (10%) had L-GPA. Their main clinical manifestations were rhinitis, lung nodules, sinusitis and otitis. L-GPA vs S-GPA patients at diagnosis, respectively, were younger, more frequently had saddle nose deformity or subglottic stenosis and were less often PR3-ANCA–positive. L-GPA vs S-GPA induction therapy less frequently included CYC but more often a combination of MTX and glucocorticoids; 64% of MTX-treated patients experienced disease progression within 18 months post-diagnosis. L- and S-GPA patients’ estimated relapse-free–survival probabilities, relapse rates and refractory disease rates at each time point were comparable, but L-GPA patients had more frequent ENT and lung relapses, and higher overall survival rates (P Conclusions The relapse risks of L-GPA and S-GPA were similar, but relapse patterns differed and L-GPA overall survival rate was higher. About one-quarter of L-GPA patients developed S-GPA over time, but without end-stage organ involvement.

Abstract: ObjectiveTo investigate whether antineutrophil cytoplasm antibody (ANCA)-negative and myeloperoxidase (MPO)-ANCA–positive granulomatosis with polyangiitis (GPA) differ from proteinase-3 (PR3)-ANCA–positive GPA.MethodsDiagnostic characteristics and outcomes of newly diagnosed French Vasculitis Study Group Registry patients with ANCA-negative, MPO-ANCA–positive or PR3-ANCA–positive GPA satisfying American College of Rheumatology criteria and/or Chapel Hill Conference Consensus Nomenclature were compared.ResultsAmong 727 GPA, 62 (8.5%) were ANCA-negative, 119 (16.4%) MPO-ANCA–positive and 546 (75.1%) PR3-ANCA–positive. ANCA-negative patients had significantly (pConclusionsBased on this large cohort, ANCA-negative versus ANCA-positive patients more frequently had limited disease but similar RFS and OS. MPO-ANCA–positive patients had similar RFS but lower OS due to their older age. PR3-ANCA–positive GPA patients’ RFS was lower than those of the two other subsets combined but that difference did not persist when comparing only PR3 versus MPO-ANCA–positive patients.

Abstract: Objectives To describe the characteristics, treatment and outcome of patients with immune thrombocytopenia with clinical significance (ITPCS) associated with SLE. Methods This retrospective multicentre study included SLE patients who experienced ≥1 ITPCS (defined as ITP with attributable bleeding disorders and/or a platelet count 8 and/or WHO score >2. Results A total of 90 patients were included, the median (range) follow-up duration was 80 (6–446) months. ITP was diagnosed before SLE in 25 patients. They presented a high rate of autoimmune haemolytic anaemia (15%), antiphospholipid antibody (62%) and antiphospholipid syndrome (19%). The 25 (28%) patients who experienced MBG had significantly more bleedings at ITP diagnosis and higher bleeding scores, and serositis and thrombosis during follow-up. They required significantly more treatment lines, transfusions and hospitalizations. The 11 (12%) patients who experienced no bleeding event presented a significantly more restricted SLE phenotype (cutaneous and/or articular). Patients received a mean (range) of 4.2 (1–11) treatment lines. Corticosteroids and HCQ allowed ITPCS overall response in one-third of patients. The median relapse-free survival of rituximab (n = 34), AZA (n = 19), MMF (n = 8), thrombopoietin-receptor agonists (n = 16) and splenectomy (n = 19) were 53, 31.5, 61, 24.5 and 78 months, respectively. Four patients experienced thrombotic events after splenectomy and one occurred under thrombopoietin-receptor agonist treatment. Conclusion SLE-ITCS patients displayed a high rate of haematological abnormalities and MBG patients exhibited higher morbidity. Management of thrombocytopenia was highly heterogeneous and many options seem viable.

Abstract: Erdheim–Chester disease is a rare histiocytosis characterized by iconic features associated with compatible histology. Most patients have somatic mutations in the MAP-kinase pathway gene, and the mutations occur in CD14+ monocytes. Differentiation of the myeloid lineage plays a central role in the pathogenesis of histiocytosis. Monocytes are myeloid-derived white blood cells, divided into three subsets, but only the CD14++CD16− “classical monocyte” can differentiate into dendritic cells and tissue macrophages. Since most mutations occur in CD14+ cells and since ECD patients have a particular monocytic phenotype resembling CMML, we studied the correlation between disease activity and monocytic subset distribution during the course of a severe vascular form of ECD requiring liver transplantation. During early follow-up, increased CD14++CD16− “classical monocyte” associated with decreased CD14lowCD16++ “non-classical monocyte” correlated with disease activity. Further studies are needed to confirm the use of monocyte as a marker of disease activity in patients with ECD.

Abstract: Objectives An increased risk of haematological malignancies (HM) has been reported in GCA patients. Our study aimed to investigate the incidence and the type of HM occurring in GCA. Methods All patients with GCA and HM living in Côte d’Or (France) were identified by crossing data from the RHEMCO (Registre des Hémopathies Malignes de Côte d’Or) and those having a positive temporal artery biopsy between 1 January 2001 and 31 December 2018. Results Among 276 biopsy-proven GCA patients, 14 HM were identified in 12 patients (4.3%). In comparison with the general population aged >50 y, the incidence of myeloid HM and myeloproliferative syndromes were increased in GCA patients [standardized incidence ratios (SIR) = 2.71 and 5.16, respectively], with a specific increase in men with GCA (SIR = 4.82 and 9.04, respectively) but not in women. In addition, the study of SIR depending on the chronology between GCA and HM diagnoses suggests that there was an increased risk of developing GCA in men but not in women, after a diagnosis of myeloid HM (SIR = 9.56), especially if it was a MPS (SIR = 17.56). Conclusions Our study shows a particular epidemiology of HM in GCA patients, which is characterized by an increased incidence of myeloid HM, especially MPS, in male GCA patients. The chronology of the diagnoses of GCA and HM raises the hypothesis that clonal hematopoiesis may be implicated in some cases of GCA.

Abstract: Objectives Clinical significance of anti-Ro52 antibodies in connective tissue diseases (CTD) is controversial. Anti-Ro52 antibodies might be associated with a more severe CTD phenotype, especially interstitial lung disease (ILD). The aims of this study were to evaluate ILD prevalence and severity, the prevalence of micro- or macroangiopathy and CTD-associated cancers in CTD with anti-Ro52 antibodies. Methods CTD patients with anti-Ro52 antibody screening by immunoblot at diagnosis were enrolled. Two groups were retrospectively formed according to the presence of anti-Ro52 antibodies with an unbiased 1:1 matching on CTD types. Unsupervised multiple correspondence analysis and hierarchical clustering analysis were used to aggregate anti-Ro52 positive patients in subgroups. Results 408 CTD patients were included. Anti-Ro52 antibodies were detected in 33 % of CTD patients. Anti-Ro52 antibodies were associated with ILD at CTD diagnosis (47.8% vs. 23.0%, OR 3.3 95% IC 1.4 to 8.0, p = 0.008), even after adjusting for the presence of anti-Ro60 antibodies, especially in patients with antisynthetase syndrome, primary Sjogren syndrome and systemic sclerosis. Micro- or macroangiopathy was more frequent in anti-Ro52 positive CTD patients (18.6% vs. 9.7%, p = 0.02) and CTD patients with anti-Ro52 antibodies experienced more frequent relapses and required more immunosuppressive drugs. Clusters 4 and 5 identified anti-Ro52 positive CTD patients with severe ILD and with clinical features of systemic sclerosis or antisynthetase syndrome respectively. Conclusions We found that anti-Ro52 antibodies were independently associated with ILD in CTD patients irrespective of CTD type. Anti-Ro52 antibodies could be associated with severity and a more relapsing disease course in CTD patients.

Abstract: International audience; This study aimed to assess the implication of mucosal-associated invariant T (MAIT) cells in GCA. Blood samples were obtained from 34 GCA patients (before and after 3 months of treatment with glucocorticoids (GC) alone) and compared with 20 controls aged >50 years. MAIT cells, defined by a CD3(+)CD4(-)TCRγδ(-)TCRVα7.2(+)CD161(+) phenotype, were analyzed by flow cytometry. After sorting, we assessed the ability of MAIT cells to proliferate and produce cytokines after stimulation with anti CD3/CD28 microbeads or IL-12 and IL-18. MAIT were stained in temporal artery biopsies (TAB) by confocal microscopy. MAIT cells were found in the arterial wall of positive TABs but was absent in negative TAB. MAIT frequency among total αβ-T cells was similar in the blood of patients and controls (0.52 vs. 0.57%; P = 0.43) and not modified after GC treatment (P = 0.82). Expression of IFN-γ was increased in MAIT cells from GCA patients compared to controls (44.49 vs. 32.9%; P = 0.029), and not modified after 3 months of GC therapy (P = 0.82). When they were stimulated with IL-12 and IL-18, MAIT from GCA patients produced very high levels of IFN-γ and displayed a stronger proliferation compared with MAIT from controls (proliferation index 3.39 vs. 1.4; P = 0.032). In GCA, the functional characteristics of MAIT cells are modified toward a pro-inflammatory phenotype and a stronger proliferation capability in response to IL-12 and IL-18, suggesting that MAIT might play a role in GCA pathogenesis. Our results support the use of treatments targeting IL-12/IL-18 to inhibit the IFN-γ pathway in GCA.

Abstract: International audience; Immune thrombocytopenia (ITP) is a rare autoimmune disease due to an immune peripheral destruction of platelets and an inappropriate platelet production. The pathogenesis of ITP is now better understood: it involves a humoral immune response which dependents on the stimulation of B cells by specific T cells called T follicular helper cells, leading to their differentiation into plasma cells that produce antiplatelet antibodies thus promoting the phagocytosis of platelets mainly by splenic macrophages. The deciphering of ITP pathogenesis has led to a better understanding of the inefficiency of treatments such as rituximab, although it has not provided yet the determination of biological predictive factor of response to treatments. Moreover, new therapeutic perspectives have been opened in the last few years with the development of molecules targeting Fcγ receptor signalling such as Syk inhibitor, or molecules increasing the clearance of pathogenic autoantibodies such as inhibitors of the neonatal Fc receptor (FcRn).

Abstract: Resume L’arterite a cellules geantes (ACG) est la plus frequente des vascularites de l’adulte. Il s’agit d’une vascularite granulomateuse, affectant les vaisseaux de gros calibre et survenant chez les patients de plus de 50 ans. La pierre angulaire du traitement de l’ACG reste a l’heure actuelle la corticotherapie, debutee entre 0,7 et 1 mg/kg/j en fonction de l’existence de complication(s) ischemique(s). Ce traitement est tres efficace, mais son sevrage est difficile, car associe a la survenue de rechutes qui contribuent a le poursuivre et a augmenter le risque de complications iatrogenes. Ainsi, des strategies d’epargne en corticoides doivent etre developpees. Des progres majeurs ont ete realises dans la comprehension de la physiopathologie de l’ACG ces dernieres annees. Le facteur declenchant reste a ce jour inconnu, mais les mecanismes conduisant a l’inflammation granulomateuse de la paroi vasculaire et a son remodelage, source des complications ischemiques de l’ACG, sont de mieux en mieux connus et ont permis l’emergence de nouvelles cibles therapeutiques, en particulier le blocage de l’activation du lymphocyte T, l’inhibition de la signalisation de l’interleukine-6 (IL-6), de l’IL-12/23 ou de l’IL-1β et dans un futur proche un meilleur controle du remodelage vasculaire.

Abstract: Points essentiels En 2019, la corticotherapie reste le traitement de reference de l’arterite a cellules geantes (ACG) mais a condition de respecter des valeurs seuils concernant la decroissance des doses : 15 a 20 mg/j a 3 mois, 10 mg/j a 6 mois, 5 mg/j a 9–12 mois et sevrage en 12 a 18 mois. En cas de corticodependance a plus de 7,5 mg/j de prednisone ou d’intolerance de la corticotherapie, un traitement d’epargne en corticoides doit etre instaure, en premiere intention, methotrexate ou tocilizumab. Le choix d’un traitement d’epargne en corticoides doit prendre en consideration les caracteristiques individuelles du patient, les donnees d’efficacite disponibles concernant ce traitement, son cout et les modalites de suivi des patients. C’est pour cela que, sauf exception, nous utilisons d’abord le methotrexate et ensuite le tocilizumab. La prevention des evenements cardio-vasculaires est un element essentiel de la prise en charge des patients atteints d’ACG. Nous recommandons la prescription d’aspirine (75–100 mg/j) au cours du 1er mois de traitement et au long cours en cas de complication ischemique. Tout patient atteint d’ACG doit recevoir un traitement preventif de l’osteoporose selon les recommandations habituelles.

Raynaud’s phenomenon (RP) in systemic sclerosis (SSc) can be severe with active digital ulceration (ADU) and gangrene. RP physiopathology includes early endothelial cell injury, vascular dysfunction and sometimes microvascular thrombosis.1 Antiphospholipid antibodies (aPL) activate endothelial cells and platelets by complexes of beta-2 glycoprotein 1 (β2GP1) and anti-β2GP12 and could therefore contribute to the initiation of and/or aggravate SSc-related RP. aPL prevalence seems increased in patients with SSc compared with controls,3 but aPL-associated clinical features are often contradictory. The aims of this study were to assess aPL and antiphospholipid syndrome (APS) prevalence in patients with SSc and their association with ADU. Consecutive patients aged more than 18 years, fulfilling the American College of Rheumatology/EULAR classification criteria for SSc4 and followed in seven French expert centres for autoimmune diseases labelled by the French national network for autoimmune disease care, were enrolled prospectively during 8 months. Abstract: Introduction L’arterite a cellules geantes (ACG) est une vascularite granulomateuse affectant les arteres de gros calibre et touchant les patients de plus de 50 ans. Les causes de l’ACG sont inconnues a ce jour ; cependant comme les MAIT (Mucosal associated invariant T lymphocytes), lymphocytes T caracterises par l’expression d’un TCR semi-invariant (TCRVα7.2) dont l’interaction est restreinte a MR1 (MHC related protein 1) [1] , semblent impliques au cours d’autres vascularites comme les vascularites associees aux ANCA [2] , nous avons analyse leurs caracteristiques quantitatives et fonctionnelles au cours de l’ACG. Materiels et methodes Apres signature d’un consentement eclaire, 27 patients atteints d’ACG au diagnostic et avant tout traitement (≥ 3/5 criteres ACR avec preuve histologique ou par imagerie de vascularite) et 16 sujets sains (absence de pathologie auto-immune, infectieuse, neoplasique et CRP ≤ 5 mg/L) ont ete inclus dans cette etude. Apres 3 mois de traitement par corticoides, un prelevement supplementaire a ete obtenu chez 20/27 patients. Les MAIT, definis par le phenotype CD3 + CD4-TCRγd-TCRVα7.2 + CD161 +, ont ete quantifies et phenotypes par cytometrie en flux. Pour l’etude de l’expression de l’IL-17 et l’IFN-γ, les cellules ont ete stimulees par PMA et Ionomycine en presence de brefeldine A (4 heures). Les donnees ont ete acquises sur un LSR II (BD Bioscience) et analysees avec Flowjo v10®. Les MAIT et les lymphocytes T (LT) CD161 + TCRVα7.2- ont ete isoles par Fluorescent Assisted Cell Sorting (FACS) puis cultives dans differentes conditions : aucune stimulation, billes anti-CD3/CD28, IL-12 et IL-18 (n = 4 patients et 5 temoins). Apres 7 jours, la proliferation cellulaire a ete mesuree par cytometrie en flux et l’index de proliferation calcule avec le logiciel ModFit®. Les coupes de biopsie d’arteres temporales positives ont ete etudiees par microscopie confocale (Leica TCS SP8). Les resultats sont exprimes en mediane [intervalle interquartile] et P est le resultat de tests de Mann–Whitney ou de Wilcoxon. Resultats Dans le sang, la frequence des MAIT parmi les LTαβ totaux etait comparable entre patients et temoins (0,48 [0,15–1,01] vs 0,47 [0,28–1,125] % ; p = 0,63) et n’etait pas modifiee par le traitement de l’ACG (0,48 [0,15–0,76] vs 0,43 [0,14–0,88] % ; p = 0,51). Le phenotype des MAIT etait modifie chez les patients : diminution de l’expression de CXCR3 (6,08 [1,57–13,96] vs 8,33 [6,13–36,66] % ; p = 0,048), augmentation de l’expression d’INF-γ (47,7 [31,8–66,7] vs 31,0 % [16,6–38,7] % ; p = 0.027). Le traitement par corticoides ne permettait pas de corriger ces anomalies phenotypiques des MAIT : respectivement (8,31 [3,90–16,3] vs 4,36 [1,64–17,47] % ; p = 0.96) et (39,8 [23,7–61,27] vs 42,4 [23,5–63,3,] % ; p = 0.85). A la difference des LT CD161 + TCRVα7.2-, les MAIT etaient capables de proliferer en presence d’IL-12 et d’IL-18 sans activation de leur TCR et a un niveau plus important chez les patients que chez les temoins : index de proliferation a 3,39 [2,36–7,15] vs 1,4 [1,2–2,42] ; (p = 0,032). Des MAIT, definies par le phenotype CD3 + IL-18R + TCRVα7.2 + ont ete identifies au sein des zones d’infiltrats en LT dans les arteres temporales de patients atteints d’ACG (n = 3). Discussion Meme si la frequence des MAIT n’est pas modifiee dans le sang des patients atteints d’ACG, leurs caracteristiques fonctionnelles sont modifiees vers un profil pro-inflammatoire cours de l’ACG : plus forte expression d’IFN-γ et plus forte proliferation en presence d’IL-12 et IL-18 sans activation de leur TCR. L’IL-12 et l’IL-18 etant 2 cytokines fortement exprimees dans les lesions d’ACG, nos resultats suggerent que les MAIT pourraient, en etant activees de facon independante de leur TCR, jouer un role dans l’amplification de la reponse inflammatoire au cours de l’ACG, ce d’autant que la corticotherapie ne permet pas de corriger le phenotype des MAIT. La diminution de l’expression de CXCR3 par les MAIT des patients atteints d’ACG pourrait etre liee au recrutement de MAIT CXCR3 + au sein des lesions de vascularite mais nos resultats ne permettent pas encore de le confirmer. Conclusion La modification du phenotype et des caracteristiques fonctionnelles des MAIT associes a leur presence dans les lesions de vascularite suggerent fortement que qu’ils jouent un role dans la physiopathologie de l’ACG.

Abstract: Introduction Les dermatomyosites a anticorps anti-MDA5 (DM MDA5+) ont des atteintes extramusculaires au premier plan (lesions cutanees severes, arthralgies/arthrite, fievre) pouvant engager le pronostic vital (atteinte pulmonaire), alors que l’atteinte musculaire est peu marquee. Leur pronostic est sombre (25 % de mortalite) et il est important de definir des facteurs de risque de mortalite pour mieux definir la strategie therapeutique. L’objectif de cette etude est d’identifier des facteurs pronostiques chez les DM MDA5+. Materiels et methodes Nous avons realise une etude nationale multicentrique retrospective grâce au Reseau francais des myosites pour les DM MDA5+. Les criteres d’inclusion etaient : – un diagnostic de DM (definie selon les criteres de l’ENMC en 2004 : un debut progressif subaigu, une faiblesse musculaire plutot proximale, une atteinte cutanee typique comme un erytheme lilace des paupieres, des papules de Gottron ou un erytheme du decollete et une atrophie perifasciculaire a la biopsie musculaire) ; – la positivite des anticorps anti-MDA5. Les recueils des donnees cliniques, paracliniques et evolutives ont ete realises a partir des dossiers des patients. Resultats Quarante et un patients ont ete identifies. L’âge moyen etait de 44 ans (± 16,5) avec un sex-ratio femme/homme de 1,4. Au diagnostic 92 % patients presentaient une atteinte pulmonaire et 87 % une atteinte cutanee. Un tiers (31 %) des patients ont presente une atteinte pulmonaire rapidement progressive (RPILD definie par une aggravation des lesions au scanner thoracique ou une aggravation des EFR pendant une duree inferieure ou egale a 3 mois). Les atteintes articulaires et musculaires (deficit moteur ou elevation des CK) etaient observees respectivement dans 75 % et 51 % des cas. La duree moyenne de suivi etait de 41,2 mois (± 61,3). La plupart des patients ont beneficie de corticosteroides (98 %) et d’immunosuppresseurs (83 %, dont 24 % du cyclophosphamide et 15 % des inhibiteurs des calcineurines) ou de rituximab (7 %), plus ou moins associes a des echanges plasmatiques (20 %) et/ou des immunoglobulines intraveineuses (15 %) selon leur severite. Le taux de mortalite etait de 22 %, avec des 44 % des deces survenus dans les 3 mois suivants le diagnostic. Les deces etaient dus a l’atteinte respiratoire dans 67 % des cas. Parmi les patients vivants, 52 % ont presente au moins une rechute, avec le plus frequemment une rechute pulmonaire (45 % des patients rechuteurs). L’analyse univariee identifie 3 variables liees au deces. Comme attendu les signes de gravite immediate respiratoire ont ete identifies avec une hypoxie severe (paO2 Conclusion Comme attendu, la severite initiale de l’atteinte pulmonaire (RPILD et hypoxie profonde) est un facteur pronostique pejoratif. Cette etude suggere pour la premiere fois que le sexe feminin et l’existence d’une atteinte articulaire seraient des elements de bon pronostic a prendre en compte dans la strategie therapeutique chez les patients sans signe de gravite pulmonaire immediate.

Abstract: International audience; Thrombotic manifestations are a hallmark of many auto-immune diseases (AID), specially of warm autoimmune hemolytic anemia (wAIHA), as 15 to 33% of adults with wAIHA experience venous thromboembolic events (VTE). However, beyond the presence of positive antiphospholipid antibodies and splenectomy, risk factors for developing a VTE during wAIHA have not been clearly identified. The aim of this retrospective study was to characterize VTEs during wAIHA and to identify risk factors for VTE. Forty-eight patients with wAIHA were included, among whom 26 (54%) had secondary wAIHA. Eleven (23%) patients presented at least one VTE, that occurred during an active phase of the disease for 10/11 patients (90%). The frequency of VTE was not different between primary and secondary AIHA (23.7 vs. 19.2%; p = 0.5). The Padua prediction score based on traditional risk factors was not different between patients with and without VTE. On multivariate analysis, total bilirubin ≥ 40 μmol/L [odds ratio (OR) = 7.4; p = 0.02] and leucocyte count above 7x109/L (OR = 15.7; p = 0.02) were significantly associated with a higher risk of thrombosis. Antiphospholipid antibodies were screened in 9 out the 11 patients who presented a VTE and were negative. Thus, the frequency of VTE is high (23%) during wAIHA and VTE preferentially occur within the first weeks of diagnosis. As no clinically relevant predictive factors of VTE could be identified, the systematic use of a prophylactic anticoagulation should be recommended in case of active hemolysis and its maintenance after hospital discharge should be considered. The benefit of a systematic screening for VTE and its procedure remain to be determined.

Abstract: IF 7.607; International audience; Innate lymphoid cells (ILCs) have been characterized as innate immune cells capable to modulate the immune response in the mucosae. Human ILCs have been rarely described in secondary lymphoid organs except in tonsils. Moreover, their function and phenotype in human secondary lymphoid organs during autoimmune diseases have never been studied. We took advantage of splenectomy as a treatment of immune thrombocytopenia (ITP) to describe and compare splenic ILC from 18 ITP patients to 11 controls. We first confirmed that ILC3 represented the most abundant ILC subset in human non-inflamed spleens, accounting for 90% of total ILC, and that they were mostly constituted of NKp44- cells. On the contrary, proportions of ILC1 and ILC2 in spleens were lower than in blood. Splenic IL-2- and IFN-γ-producing ILC1 were increased in ITP. While the frequencies of total splenic ILC3 were similar in the two groups, splenic GM-CSF-producing ILC3 were increased in ITP. This is the first description of human ILC in a major secondary lymphoid organ during an autoimmune disease, ITP. We observed an expansion of splenic ILC1 that could participate to the Th1 skewing, while the increased production of GM-CSF by splenic ILC3 could stimulate splenic macrophages which play a key role in ITP pathophysiology.

Abstract: International audience; ObjectivesLarge-vessel involvement (LVI) can occur in giant-cell arteritis (GCA) and may represent a distinct disease subgroup with a higher risk for aortic dilation. This study aimed to better characterize the presentation and evolution of LVI in patients with GCA.Patients and methodsA retrospective multicenter study enrolled 248 GCA patients with LVI and 301 GCA patients without LVI on imaging. Factors associated with aortic dilation were identified in a multivariable model.ResultsThe patients with LVI were younger (p < 0.0001), more likely to be women (p = 0.01), and showed fewer cephalic symptoms (p < 0.0001) and polymyalgia rheumatica (p = 0.001) but more extracranial vascular symptoms (p = 0.05) than the patients without LVI. Glucocorticoids (GC) management did not differ between the two groups, but the GC discontinuation rate was lower in the patients with LVI (p = 0.0003). Repeated aortic imaging procedures were performed at 19 months [range: 5–162 months] and 17 months [range: 6–168 months] after diagnosis in 154 patients with LVI and 123 patients without LVI, respectively, of whom 21% and 7%, respectively, presented new aortic dilations (p = 0.0008). In the patients with LVI, aortic dilation occurred on an aorta segment shown to be inflammatory on previous imaging in 94% of patients. In the multivariate analysis, LVI was the strongest predictor of aortic dilation (hazard ratio: 3.16 [range: 1.34–7.48], p = 0.009).ConclusionsLVI represents a distinct disease pattern of GCA with an increased risk of aortic dilation. Control of the aortic morphology during follow-up is required.

Abstract: International audience; BACKGROUND: Immunosuppressed patients are at risk of severe viral infections-related complications. National and international vaccination guidelines have been developed to decrease the mortality risk associated with these infections. However, a summary of these guidelines and the value of immunisation in this population is missing. OBJECTIVES: To summarize specific guidelines regarding vaccination in immunosuppressed patients. METHODS: We performed a literature search based on last update vaccine guidelines in immunosuppressed adult patients published between 1/1/2005-1/31/2016 in English or French language using PubMed, Cochrane and Embase, as well as relevant medical society websites. RESULTS: Of the 389 citations identified, 12 guidelines were selected Three additional guidelines were selected by searching on the websites from medical societies of each specialty. 15 guidelines were included, involving 19 medical societies issued from the US (n~=~6), international collaboration (n~=~3), UK (n~=~2), Canada (n~=~1), Australia (n~=~1), France (n~=~1), and Germany (n~=~1). These guidelines provide recommendations on vaccination in asplenic patients (n~=~5), cancer patients (n~=~4), HIV patients (n~=~5), hematopoietic stem cell recipients (n~=~4), inflammatory bowel diseases patients (n~=~5), psoriasis patients (n~=~4), primary immunocompromised patients (n~=~3), inflammatory rheumatic diseases patients (n~=~6), and solid organ transplant recipients (n~=~5). All guidelines recommended pneumococcal and injectable influenza vaccines. Other inactivated vaccines were recommended only in high risk patients. Live vaccines were usually contraindicated in patients under immunosuppressive therapy and/or in HIV patients with a CD4 count under 200/mm3. CONCLUSION: Pneumococcal and injectable influenza are the two essential vaccines recommended in all immunocompromised patients. Other inactivated vaccines are only indicated in high risk patients. Live vaccines are usually contraindicated.

Abstract: Summary Splenic macrophages play a key role in immune thrombocytopenia (ITP) pathogenesis by clearing opsonized platelets. Fcγ receptors (FcγR) participate in this phenomenon, but their expression on splenic macrophages and their modulation by treatment have scarcely been studied in human ITP. We aimed to compare the phenotype and function of splenic macrophages between six controls and 24 ITP patients and between ITP patients according to the treatments they received prior to splenectomy. CD86, human leucocyte antigen D-related (HLA-DR) and FcγR expression were measured by flow cytometry on splenic macrophages. The major FcγR polymorphisms were determined and splenic macrophage function was assessed by a phagocytosis assay. The expression of the activation markers CD86 and HLA-DR was higher on splenic macrophages during ITP compared to controls. While the expression of FcγR was not different between ITP and controls, the phagocytic function of splenic macrophages was reduced in ITP patients treated with intravenous immunoglobulin (IVIg) within the 2 weeks prior to splenectomy. The FCGR3A (158V/F) polymorphism, known to increase the affinity of FcγRIII to IgG, was over-represented in ITP patients. Thus, these are the first results arguing for the fact that the therapeutic use of IVIg during human chronic ITP does not modulate FcγR expression on splenic macrophages but decreases their phagocytic capabilities.

Abstract: International audience; Objectives: Acute pneumonia (AP) induces an excess of mortality among the elderly. We evaluated the value of a new predictive biomarker index compared to usual prognosis scores for predicting in-hospital and 1-year mortalities in elderly inpatients with AP.Design: Retrospective study in 6 clinical departments of a university hospital.Setting: Burgundy university hospital (France).Participants: All patients aged 75 and over with AP and hospitalized between January 1 and June 30, 2013, in the departments of medicine (5) and intensive care (1) of our university hospital.Measurements: A new index, which we named UBMo, was created by multiplying the uremia (U in the formula) by the N-terminal-pro-brain natriuretic peptide (NT-proBNP) plasmatic rate (B), divided by the monocyte count (Mo).Results: Among the 217 patients included, there were 138 community-acquired pneumonia, 56 nursing home-acquired pneumonia, and 23 hospital-acquired pneumonia. In-hospital and 1-year mortality rates were respectively 19.8% and 43.8%. In multivariate analysis, Pneumonia Severity Index (PSI), unlike CURB-65 (confusion, urea >7 mmol/L, respiratory rate >= 30 breaths/min, blood pressure = 65) score, was associated with in-hospital and 1-year mortalities. UBMo index performed better than PSI and CURB-65 scores in predicting both in-hospital and 1-year mortalities. For in-hospital mortality, the areas under the receiver operating characteristic curves (AUCs) were 0.89 (95% CI = 0.84-0.94), 0.72 (95% CI = 0.65-0.80), and 0.63 (95% CI = 0.54-0.72), respectively, for the 3 scores. For 1-year mortality, the AUCs were 0.93 (95% CI = 0.89-0.98), 0.66 (95% CI = 0.59-0.74), and 0.58 (95% CI = 0.50-0.66), respectively, for the 3 scores. The cut point for the UBMo index of 20,000 x 10(-9) ng.mmol/L had a sensitivity of 93.1% and 80.9% and a specificity of 76.3% and 95.8%, respectively, for in-hospital and 1-year mortalities.Conclusion: If confirmed by prospective studies, the UBMo index appears very efficient in identifying patients at high risk of in-hospital and 1-year mortalities after an AP. Abstract: International audience; Background: Post-thrombotic syndrome (PTS) is a frequent complication of deep vein thrombosis (DVT) despite adequate treatment. Venous angioplasty and stent placement has been progressively used to restore and maintain venous patency in PTS patients. This study reports our single-center experience with the use of endovascular treatment for chronic post-thrombotic symptomatic ilio-femoral venous obstructive lesions. Methods: A prospective mono-centric observational cohort study of PTS patients with chronic symptomatic ilio-femoral venous obstructive lesions referred for endovascular treatment was conducted from March 2012 to April 2016. Procedure consisted in recanalization, pre-dilation and self-expandable stenting of stenotic or occluded iliac and/or femoral veins. Severity of PTS, quality-of-life and treatment outcomes were assessed using Villalta scale and Chronic Venous Insufficiency Questionnaire (CIVIQ-20) at baseline and 3 months after the procedure. Imaging follow-up was based on duplex ultrasound (US) and computed tomography (CT). Results: Twenty-one patients (11 females, 10 males; median age, 41 years; range, 32-60) were included. Recanalization and stenting was successfully accomplished in all prime procedures, 4 patients benefitted from an additional procedure. Immediate technical success rate was 96% considering 25 procedures, performed without any complications. Median follow-up was 18 months (range, 6-30 months) with a 90.5% stent patency rate. Abstract: for the PGRx-ITP Study Group; International audience; This prospective observational cohort study aimed to explore the clinical features of incident immune thrombocytopenia in adults and predictors of outcome, while determining if a family history of autoimmune disorder is a risk factor for immune thrombocytopenia. All adults, 18 years of age or older, recently diagnosed with immune thrombocytopenia were consecutively recruited across 21 hospital centers in France. Data were collected at diagnosis and after 12 months. Predictors of chronicity at 12 months were explored using logistic regression models. The association between family history of autoimmune disorder and the risk of developing immune thrombocytopenia was explored using a conditional logistic regression model after matching each case to 10 controls. One hundred and forty-three patients were included: 63% female, mean age 48 years old (Standard Deviation=19), and 84% presented with bleeding symptoms. Median platelet count was 10×109/L. Initial treatment was required in 82% of patients. After 12 months, only 37% of patients not subject to disease-modifying interventions achieved cure. The sole possible predictor of chronicity at 12 months was a higher platelet count at baseline [Odds Ratio 1.03; 95%CI: 1.00, 1.06]. No association was found between outcome and any of the following features: age, sex, presence of either bleeding symptoms or antinuclear antibodies at diagnosis. Likewise, family history of autoimmune disorder was not associated with incident immune thrombocytopenia. Immune thrombocytopenia in adults has been shown to progress to a chronic form in the majority of patients. A lower platelet count could be indicative of a more favorable outcome.

Abstract: International audience; Primary warm autoimmune hemolytic anemia (wAIHA) is a rare autoimmune disease in which red blood cells are eliminated by IgG autoantibodies. We analyzed the antibody-secreting cells in the spleen and the peripheral blood of wAIHA patients in various contexts of treatment. Plasmablasts were observed in peripheral blood of newly diagnosed wAIHA patients and, accordingly, active germinal center reactions were present in the spleen of patients receiving short-term corticosteroid therapy. Long-term corticosteroid regimens markedly reduced this response while splenic plasma cells were able to persist, a fraction of them secreting anti-red blood cell IgG in vitro. In wAIHA patients treated by rituximab and who underwent splenectomy because of treatment failure, plasma cells were still present in the spleen, some of them being autoreactive. By using a set of diagnostic genes that allowed us to assess the plasma cell maturation stage, we observed that these cells displayed a long-lived program, differing from the one of plasma cells from healthy donors or from wAIHA patients with various immunosuppressant treatments, and more similar to the one of normal long-lived bone-marrow plasma cells. Interestingly, an increased level of B-cell activating factor (BAFF) was observed in the supernatant of spleen cell cultures from such rituximab-treated wAIHA patients. These results suggest, in line with our previous report on primary immune thrombocytopenia, that the B-cell depletion induced by rituximab promoted a suitable environment for the maturation and survival of auto-immune long-lived plasma cells in the spleen

Abstract: BACKGROUND The combination of cyclophosphamide and glucocorticoids leads to remission in most patients with antineutrophil cytoplasm antibody (ANCA)–associated vasculitides. However, even when patients receive maintenance treatment with azathioprine or methotrexate, the relapse rate remains high. Rituximab may help to maintain remission. METHODS Patients with newly diagnosed or relapsing granulomatosis with polyangiitis, microscopic polyangiitis, or renal-limited ANCA-associated vasculitis in complete remission after a cyclophosphamide–glucocorticoid regimen were randomly assigned to receive either 500 mg of rituximab on days 0 and 14 and at months 6, 12, and 18 after study entry or daily azathioprine until month 22. The primary end point at month 28 was the rate of major relapse (the reappearance of disease activity or worsening, with a Birmingham Vasculitis Activity Score >0, and involvement of one or more major organs, disease-related life-threatening events, or both). RESULTS The 115 enrolled patients (87 with granulomatosis with polyangiitis, 23 with microscopic polyangiitis, and 5 with renal-limited ANCA-associated vasculitis) received azathioprine (58 patients) or rituximab (57 patients). At month 28, major relapse had occurred in 17 patients in the azathioprine group (29%) and in 3 patients in the rituximab group (5%) (hazard ratio for relapse, 6.61; 95% confidence interval, 1.56 to 27.96; P = 0.002). The frequencies of severe adverse events were similar in the two groups. Twenty-five patients in each group (P = 0.92) had severe adverse events; there were 44 events in the azathioprine group and 45 in the rituximab group. Eight patients in the azathioprine group and 11 in the rituximab group had severe infections, and cancer developed in 2 patients in the azathioprine group and 1 in the rituximab group. Two patients in the azathioprine group died (1 from sepsis and 1 from pancreatic cancer). CONCLUSIONS More patients with ANCA-associated vasculitides had sustained remission at month 28 with rituximab than with azathioprine. Abstract: International audience; OBJECTIVE: Giant cell arteritis (GCA) is the most frequently occurring vasculitis in elderly individuals, and its pathogenesis is not fully understood. The objective of this study was to decipher the role of the major CD4+ T cell subsets in GCA and its rheumatologic form, polymyalgia rheumatica (PMR). METHODS: A prospective study of the phenotype and the function of major CD4+ T cell subsets (Th1, Th17, and Treg cells) was performed in 34 untreated patients with GCA or PMR, in comparison with 31 healthy control subjects and with the 27 treated patients who remained after the 7 others withdrew. RESULTS: Compared with control subjects, patients with GCA and patients with PMR had a decreased frequency of Treg cells and Th1 cells, whereas the percentage of Th17 cells was significantly increased. Furthermore, an analysis of temporal artery biopsy specimens obtained from patients affected by GCA for whom biopsy results were positive demonstrated massive infiltration by Th17 and Th1 lymphocytes without any Treg cells. After glucocorticoid treatment, the percentages of circulating Th1 and Th17 cells decreased, whereas no change in the Treg cell frequency was observed. The frequency of CD161+CD4+ T cells, which are considered to be Th17 cell precursors, was similar in patients and control subjects. However, these cells highly infiltrated GCA temporal artery biopsy specimens, and their ability to produce interleukin-17 in vitro was significantly enhanced in patients with GCA and patients with PMR and was correlated with a decrease in the phosphorylated form of STAT-1. CONCLUSION: This study is the first to demonstrate that the frequency of Treg cells is decreased in patients with GCA and patients with PMR, and that CD161+CD4+ T lymphocytes, differentiated into Th1 cells and Th17 cells, are involved in the pathogenesis of GCA and PMR.

Abstract: International audience; OBJECTIVE: To cross-culturally adapt the LupusQoL into French, to test its measurement properties and to further investigate its domain structure. METHODS: The cultural adaptation process according to guidelines and pre-testing resulted in the LupusQoL-FR. SLE patients completed the LupusQoL-FR at baseline, 15 days, 3 months and 6 months. Validity was studied through content and construct validity (factorial and Rasch analysis for structural validity, Spearman's correlation and Mann-Whitney tests for external validity). Cronbach's α and intra-class correlation coefficients were computed for reliability. The standardized response mean was computed to evaluate responsiveness. RESULTS: In all, 182 patients, age 39.6 (10.6) years, mostly outpatients [mean SELENA-SLEDAI 2.6 (3.5)] were recruited. Factor analysis with eight imposed factors was very close to the original LupusQoL. A screeplot with parallel analysis showed that LupusQoL domains could be aggregated in two physical and mental scales. Both eight- and two-factor structures showed a good Rasch fit, internal consistency (Cronbach's α: 0.85-0.95), and test-retest reliability (intra-class correlation coefficient 0.79-0.95). External convergent (correlation with SF-36, r=0.59-0.78) and divergent validity (according to SELENA-SLEDAI) were also satisfactory. CONCLUSION: The LupusQoL-FR is valid to assess quality of life in SLE patients. A two-factor structure of physical and mental aggregated scales is a valid alternative to the original eight-domain structure.

Abstract: Known for years as professional APCs, dendritic cells (DCs) are also endowed with tumoricidal activity. This dual role of DC as killers and messengers may have important implications for tumor immunotherapy. However, the tumoricidal activity of DCs has mainly been investigated in animal models. Cancer cells inhibit antitumor immune responses using numerous mechanisms, including the induction of immunosuppressive/ tolerogenic DCs that have lost their ability to present Ags in an immunogenic manner. In this study, we evaluated the possibility of generating tumor killer DCs from patients with advanced-stage cancers. We demonstrate that human monocyte-derived DCs are endowed with significant cytotoxic activity against tumor cells following activation with LPS. The mechanism of DC-mediated tumor cell killing primarily involves peroxynitrites. This observed cytotoxic activity is restricted to immature DCs. Additionally, after killing, these cytotoxic DCs are able to activate tumor Ag-specific T cells. These observations may open important new perspectives for the use of autologous cytotoxic DCs in cancer immunotherapy strategies.

Abstract: International audience; Common variable immunodeficiency (CVID) is the most frequent clinically expressed primary immunodeficiency in adults and is characterized by primary defective immunoglobulin production. Besides recurrent infectious manifestations, up to 20% of CVID patients develop autoimmune complications. In this study, we took advantages of the French DEFI database to investigate possible correlations between peripheral lymphocyte subpopulations and autoimmune clinical expression in CVID adult patients. In order to analyse homogeneous populations of patients with precise clinical phenotypes, we first focused on patients with autoimmune cytopenia because they represent prototypic autoantibody mediated diseases. In a secondary analysis, we have tested our conclusions including all "autoimmune" CVID patients. We describe one of the largest European studies with 311 CVID patients, including 55 patients with autoimmune cytopenia and 61 patients with clinical or serologic autoimmune expression, excluding autoimmune cytopenia. We clarify previous reports and we confirm a very significant correlation between an increased proportion of CD21(low) B cells and CVID associated autoimmune cytopenia, but independently of the presence of other autoimmune disorders or of splenomegaly. Moreover, in CVID associated autoimmune cytopenia, T cells display an activated phenotype with an increase of HLA-DR and CD95 expression and a decrease in the naïve T cell numbers. Patients with other autoimmune manifestations do not harbour this "T and B cells phenotypic picture". In view of recent findings on CD21(low) B cells in CVID and RA, we suggest that both a restricted subset of B cells and a T cell help are required for a breakdown of B cell tolerance against membrane auto antigens in CVID.

Abstract: SummaryWe determined the number and functional status of CD4+CD25high regulatory T cells (Treg) in blood samples from patients with metastatic carcinoma, and evaluated their sensitivity to a single intravenous infusion of cyclophosphamide. Treg numbers were significantly higher in 49 patients with metastatic cancer (9·2% of CD4+ T cells) compared to 24 healthy donors (7·1%). These cells expressed the transcription factor forkhead box P3 (FoxP3), glucocorticoid-induced tumour necrosis factor receptor family-related protein (GITR) and intracellular CD152, and demonstrated a suppressive activity in vitro against CD4+CD25– autologous proliferation. At a single intravenous infusion, cyclophosphamide failed, in association with a non-specific immunotherapy by intratumoral bacille Calmette–Guérin (BCG), to modulate significantly Treg numbers or function. Metastatic cancer is associated with an expansion of peripheral blood CD4+CD25highFoxP3+GITR+CD152+Treg cells whose immunosuppressive properties do not differ from those of healthy subjects. Moreover, cyclophosphamide administration may not represent an optimal therapy to eliminate Treg, which further underlines the need to identify specific agents that would selectively deplete these cells.

Abstract: Background Data on survival and prognosis factors in incident cohorts are scarce in systemic sclerosis (SStc). To describe survival, standardized mortality ratio (SMR), and prognosis factors in systemic sclerosis (SSc), we analyzed a multicenter French cohort of incident patients and performed a systematic review of the literature and meta-analysis. Methods A multicenter, French cohort study was conducted between January 1, 2000, and December 31, 2013. Patients were followed-up until July 1, 2016. A systematic review of the literature was carried out in MEDLINE and EMBASE up to July 2017. Meta-analysis was performed using all available data on SMR and hazard ratios of prognosis factors. Results A total of 625 patients (493 females, 446 lcSSc) were included. During the study period, 104 deaths (16.6%) were recorded and 133 patients were lost to follow-up. Overall survival rates at 1, 3, 5, and 10 years from diagnosis were 98.0%, 92.5%, 85.9%, and 71.7% respectively in the French cohort. Overall SMR was 5.73 (95% CI 4.68–6.94). Age at diagnosis > 60 years, diffuse cutaneous SSc, scleroderma renal crisis, dyspnea, 6-min walking distance (6MWD), forced vital capacity 8 mg/l were associated with a poorer survival after adjustment. Eighteen studies (11,719 patients) were included in the SMR meta-analysis and 36 studies (26,187 patients) in the prognosis factor analysis. Pooled SMR was 3.45 (95%CI 3.03–3.94). Age at disease onset, male sex, African origin, diffuse cutaneous SSc, anti-Scl70 antibodies, cardiac and renal involvement, interstitial lung disease, PH, and malignancy were significantly associated with a worse prognosis. Anti-centromere antibodies were associated with a better survival. Conclusions Overall, our study highlights a high mortality rate in SSc patients and confirms previously described prognosis factors related to skin extension and organ involvement while identifying additional prognosis factors such as autoantibody status, telangiectasia, 6MWD, and valvular disease. Abstract: Known for years as professional antigen presenting cells (APC), dendritic cells (DC) play a crucial role in immune response regulation. Their state of activation and maturation conditions the antitumor immune response. Thus, at an immature stage, the DC are unable to activate the immune response. Our team demonstrated that besides being ineffective, intra-tumoral DC acquire immunosuppressive properties and inhibit T cell activation. Conversely, activated and mature DC can be used in cancer immunotherapy. Previous reports from our research team deciphered that under certain conditions DC can become tumoricidal. These DC combine a cytotoxic activity against tumor cells and the capacity to activate tumor specific T cells. Prior launching the possibility of the application of such cytotoxic DC (referred as KDC for Killer DC) in clinic, we explored the interactions of these cells with regulatory T cells (Treg), -major immunosuppressive cells promoted by tumor microenvironment. Our study revealed that KDC, generated from the blood of cancer patients, can inhibit Treg generation and deviate naive T cell polarization into T helper 1 cells, key players in the anti-tumor immune response. We have further established that the inhibition of Treg generation by the KDC requires cell to cell contact, though implies neither IL-6 nor NO, but the mechanism still to be deciphered. Spleen, a major lymphoid organ, drives foremost mechanisms in immune tolerance during cancer and autoimmune diseases (AID). A better understanding of the immune response specifications during these 2 types of diseases in the spleen is therefore essential goal for therapeutic developments. Nevertheless, there is negligible volume of studies on human splenic immune cells, in particular DC. Our research on splenic DC, derived from patients with various cancers and AID, display distinct DC subset distribution respective to the disease. Although limited by the availability of human splenic samples, we also determined the activation profile of DC subsets after stimulation by different TLR ligands. Given the key role of DC in initiating specific immune response, this novel findings could be implemented to target certain DC subset activation or inhibition in these pathologies. These investigations provide important knowledge for more refine exploitation of DC in immunotherapeutic strategies as treatment tools or as targets.; Les cellules dendritiques (DC), principales cellules présentatrices d’antigène, jouent un rôle crucial dans la régulation de la réponse immunitaire. Leur état d’activation et de maturation conditionne notamment la réponse immunitaire anti-tumorale. Ainsi à un stade immature, les DC sont incapables d’activer la réponse immunitaire. Notre équipe a montré qu’en plus d’être inefficaces, les DC intra-tumorales acquièrent des propriétés immunosuppressives et inhibent l’activation lymphocytaire. A l’inverse, activées et matures, les DC peuvent être utilisées en immunothérapie des cancers. Notre équipe a montré que les DC peuvent dans certaines conditions devenir tumoricides et que ces DC tumoricides combinent une activité cytotoxique contre les cellules tumorales et la capacité d’activation des lymphocytes T spécifiques de la tumeur. Avant d’utiliser ces DC tumoricides en phase clinique, nous avons voulu étudier les interactions de ces cellules avec des cellules immunosuppressives générées par les tumeurs, les lymphocytes T régulateurs (Treg). Notre étude montre que les DC cytotoxiques (KDC), générées à partir du sang de patients atteints de tumeurs, sont capables d’inhiber la génération des lymphocytes T régulateurs, impliqués dans l’immunosuppression induite par les tumeurs. Elles sont également à l’origine de la polarisation des lymphocytes T naïfs en des lymphocytes T helper 1, principaux acteurs de la réponse immunitaire anti-tumorale. Sans parvenir à identifier les molécules impliquées, nous avons établi que l’inhibition de la génération des Treg par les KDC n’implique ni l’IL-6, ni le NO, mais est partiellement dépendante d’un contact cellulaire. La rate joue un rôle central dans les mécanismes de tolérance immunitaire, lors des cancers, mais également lors des maladies auto-immunes (MAI). Une meilleure compréhension de la réponse immunitaire au cours de ces 2 types de pathologies au niveau d’un organe lymphoïde majeur tel que la rate est indispensable pour adapter au mieux les thérapeutiques. Cependant, il n’existe que très peu d’études portant sur les DC spléniques humaines. Nos travaux sur les DC au sein de rates provenant de patients atteints de divers cancers et MAI mettent en évidence une répartition des sous populations de DC qui diffère selon les pathologies. Bien que limité par le nombre d’échantillons humains, nous avons également déterminé le profil d’activation de ces sous populations de DC après stimulation par différents TLR. Etant donné le rôle clé des DC dans l’initiation de la réponse immunitaire spécifique, ces nouvelles connaissances pourraient permettre de cibler certains sous-types de DC afin de les activer ou de les inhiber lors de ces différentes pathologies. Ces observations offrent d’importantes perspectives pour la future utilisation des DC dans les stratégies d'immunothérapie.

Abstract: Immune thrombocytopenia (ITP) is an autoimmune disease responsible for a peripheral immune destruction of platelets associated with an inappropriate bone marrow production. In this work, we first review the mechanisms involved in the pathogenesis of ITP. We also focus on the T cell immune response, highlighting the key role of regulatory T cells (Treg) in peripheral tolerance. The implication of the spleen in the immune response and the effects of rituximab, a B cell depleting therapy, are discussed. Then, our results obtained from 40 ITP patients are reported. Despite the fact that CD4+CD25HighFoxp3+ circulating Treg levels are similar between patients and controls, a significant increase is observed in responder patients. In the spleen, the rate of Treg is lower in ITP patients. Analyses of the spleens also reveal an increase in the level of marginal zone B cells in ITP. Rituximab is responsible for a complete depletion of both circulating and splenic B cells, which is not sufficient to achieve a response. Moreover, plasma cells are still observed after treatment. An increase in the Th1/Treg ratio in the spleen of non responder patients after rituximab infusion could trigger an escape to this therapy. The involvement of CD8+ T cells in the pathogenesis of ITP is highlighted by the increase in the CD8+/CD4+ ratio in the spleen after rituximab. New fields in the understanding of the pathogenesis of ITP are opened with these results, particularly by showing a quantitative deficiency in splenic Treg and the possible involvement of marginal zone B cells. Regarding rituximab effect on the immune response, we demonstrate on the one hand that complete circulating and splenic B cell depletion is not sufficient to achieve remission, and on the other hand that Th1 response and increase in CD8+ T cells level may represent an escape to this treatment., La thrombopénie immunologique ou purpura thrombopénique immunologique (PTI) est une maladie auto-immune rare responsable d’une destruction périphérique immunologique des plaquettes associée à une production médullaire inadaptée. Dans la première partie de ce travail, nous exposons les connaissances actuelles de sa physiopathologie ainsi que certaines données concernant la réponse immunitaire T, le rôle des lymphocytes T régulateurs (Treg), l’implication de la rate dans la réponse immunitaire ainsi que les modes d’action d’une thérapeutique anti-lymphocytaire B, le rituximab. Dans une seconde partie, nous rapportons les résultats obtenus chez 40 patients atteints de PTI. Nous avons montré que le taux des Treg circulants CD4+CD25HighFoxp3+ est similaire chez les patients et les témoins, avec une élévation de leur taux chez les sujets répondeurs aux traitements. A l’inverse, il existe un déficit quantitatif en Treg au sein de la rate des patients. L’analyse des sous-populations lymphocytaires B spléniques a montré une augmentation du taux de lymphocytes B de la zone marginale chez les patients. Concernant les mécanismes d’action du rituximab, nous avons montré qu’une déplétion lymphocytaire B sanguine et splénique n’est pas suffisante pour obtenir une rémission, et que les plasmocytes ne sont pas sensibles à cette thérapeutique. Par ailleurs, nous proposons un mécanisme d’échappement à ce traitement. En effet, nous avons montré que les patients résistants au RTX présentent une élévation du ratio Th1/Treg spléniques. Chez ces sujets non répondeurs, nous avons également observé une élévation du ratio lymphocytes T CD8+/CD4+, au sein de la rate, suggérant une participation des lymphocytes T cytotoxiques dans la physiopathologie du PTI. Ces résultats ouvrent donc de nouvelles perspectives dans la compréhension de la physiopathologie du PTI, notamment la possible implication des lymphocytes B de la zone marginale et le défaut de contrôle de la réponse immunitaire splénique par les Treg. Concernant le rituximab, son action sur la réponse immunitaire ne semble pas se limiter à une déplétion lymphocytaire B qui n’est pas suffisante pour obtenir une rémission. Un mécanisme d’échappement ou de résistance à cette thérapeutique passe par une orientation Th1 et une probable implication des lymphocytes T CD8+.

Abstract: During my thesis, I studied the cytotoxic function of dendritic cells (DC) from cancer patients and compared it to DC from healthy donors. Our results indicate that human monocyte-derived DC can acquire strong cytotoxic activity toward tumor cells after activation with low dose of LPS. The cytotoxic potential of DC derived from cancer patients was almost the same as the one generated from healthy donors. We identified the tumor cell killing mechanism which involves peroxynitrite release. After killing of cancer cells, DC are capable of engulfing dead tumor cell fragments and overexpress the costimulatory molecules necessary for T cell proliferation. A second study consisted in an analysis of inflammatory macrophages and their significance in atherosclerosis. Macrophages produce CETP (cholesteryl ester transfert protein), a target for LXR (liver X receptor) receptors. Our results show that LXR-mediated induction of CETP expression is lost in inflammatory macrophages. Our study suggests that inflammatory macrophages may not increase the circulating CETP pool on LXR agonist treatment., Au cours de ma thèse, j’ai étudié la fonction cytotoxique des cellules dendritiques (DC) de patients cancéreux comparée à celle de sujets sains. Nous avons montré que les DC générées à partir des monocytes du sang périphérique peuvent acquérir des capacités cytotoxiques importantes après activation par des faibles doses de LPS. Le potentiel cytotoxique des DC générées à partir de patients cancéreux est comparable à celui de sujets sains. Nous avons identifié le mécanisme de cytotoxicité qui fait intervenir la production de peroxynitrite. Après avoir tué les cellules tumorales, les DC phagocytent des fragments tumoraux, surexpriment des molécules de costimulation et induisent la prolifération des lymphocytes. Un deuxième axe de recherche au cours de ma thèse a consisté en l’étude des macrophages inflammatoires et de leur implication dans l’athérosclérose. Les macrophages sécrètent la CETP (cholesteryl ester transfert protein), protéine-cible des récepteurs LXR (liver X receptor). Nous avons montré que l’expression de la CETP, en réponse aux agonistes LXR, n’est pas augmentée dans les macrophages inflammatoires. Ceci suggère que les macrophages inflammatoires ne participeraient pas à l’augmentation du pool plasmatique de CETP en cas de traitement par des agonistes LXR.

Abstract: Immune thrombocytopenia (ITP) is an autoimmune disease responsible for a peripheral immune destruction of platelets associated with an inappropriate bone marrow production. In this work, we first review the mechanisms involved in the pathogenesis of ITP. We also focus on the T cell immune response, highlighting the key role of regulatory T cells (Treg) in peripheral tolerance. The implication of the spleen in the immune response and the effects of rituximab, a B cell depleting therapy, are discussed. Then, our results obtained from 40 ITP patients are reported. Despite the fact that CD4+CD25HighFoxp3+ circulating Treg levels are similar between patients and controls, a significant increase is observed in responder patients. In the spleen, the rate of Treg is lower in ITP patients. Analyses of the spleens also reveal an increase in the level of marginal zone B cells in ITP. Rituximab is responsible for a complete depletion of both circulating and splenic B cells, which is not sufficient to achieve a response. Moreover, plasma cells are still observed after treatment. An increase in the Th1/Treg ratio in the spleen of non responder patients after rituximab infusion could trigger an escape to this therapy. The involvement of CD8+ T cells in the pathogenesis of ITP is highlighted by the increase in the CD8+/CD4+ ratio in the spleen after rituximab. New fields in the understanding of the pathogenesis of ITP are opened with these results, particularly by showing a quantitative deficiency in splenic Treg and the possible involvement of marginal zone B cells. Regarding rituximab effect on the immune response, we demonstrate on the one hand that complete circulating and splenic B cell depletion is not sufficient to achieve remission, and on the other hand that Th1 response and increase in CD8+ T cells level may represent an escape to this treatment., La thrombopénie immunologique ou purpura thrombopénique immunologique (PTI) est une maladie auto-immune rare responsable d’une destruction périphérique immunologique des plaquettes associée à une production médullaire inadaptée. Dans la première partie de ce travail, nous exposons les connaissances actuelles de sa physiopathologie ainsi que certaines données concernant la réponse immunitaire T, le rôle des lymphocytes T régulateurs (Treg), l’implication de la rate dans la réponse immunitaire ainsi que les modes d’action d’une thérapeutique anti-lymphocytaire B, le rituximab. Dans une seconde partie, nous rapportons les résultats obtenus chez 40 patients atteints de PTI. Nous avons montré que le taux des Treg circulants CD4+CD25HighFoxp3+ est similaire chez les patients et les témoins, avec une élévation de leur taux chez les sujets répondeurs aux traitements. A l’inverse, il existe un déficit quantitatif en Treg au sein de la rate des patients. L’analyse des sous-populations lymphocytaires B spléniques a montré une augmentation du taux de lymphocytes B de la zone marginale chez les patients. Concernant les mécanismes d’action du rituximab, nous avons montré qu’une déplétion lymphocytaire B sanguine et splénique n’est pas suffisante pour obtenir une rémission, et que les plasmocytes ne sont pas sensibles à cette thérapeutique. Par ailleurs, nous proposons un mécanisme d’échappement à ce traitement. En effet, nous avons montré que les patients résistants au RTX présentent une élévation du ratio Th1/Treg spléniques. Chez ces sujets non répondeurs, nous avons également observé une élévation du ratio lymphocytes T CD8+/CD4+, au sein de la rate, suggérant une participation des lymphocytes T cytotoxiques dans la physiopathologie du PTI. Ces résultats ouvrent donc de nouvelles perspectives dans la compréhension de la physiopathologie du PTI, notamment la possible implication des lymphocytes B de la zone marginale et le défaut de contrôle de la réponse immunitaire splénique par les Treg. Concernant le rituximab, son action sur la réponse immunitaire ne semble pas se limiter à une déplétion lymphocytaire B qui n’est pas suffisante pour obtenir une rémission. Un mécanisme d’échappement ou de résistance à cette thérapeutique passe par une orientation Th1 et une probable implication des lymphocytes T CD8+.
