Abstract

International audience; Introduction (contexte de la recherche) :L’exposition répétée des patients asthmatiques aux allergènes intradomiciliaires contribue aux exacerbations de l’asthme. Plusieurs stratégies d’intervention sont proposées, parmi lesquelles, la visite domiciliaire de conseillers en environnement intérieur (CEI). Les CEI établissent un état des lieu au domicile du patient et proposent des mesures de remédiation afin d’améliorer les fonctions respiratoires. Plusieurs études monocentriques ont montré l’intérêt d’une telle démarche.ObjectifL’étude ECENVIR est un essai clinique, multicentrique, prospectif et randomisé qui vise à évaluer l’impact des CEI sur le niveau de contrôle de l’asthme.Méthodes :Au total, 104 patients ont été inclus dont 85 patients ont été suivis médicalement et randomisés dans 2 groupes : le Groupe Intervention (GI) a bénéficié d’une visite de CEI avec prélèvements pour mesures d’allergènes et conseils à l’inclusion et après 12 mois de suivi ; et le groupe témoin (GT) a bénéficié d’une même visite de contrôle uniquement à M12.Résultats :Le critère de jugement principal a été analysé chez 71/85 patients : 34 dans le GI et 37 dans le GT. A M12, une amélioration du contrôle de l’asthme était notée dans le GI avec une diminution du nombre d’asthme sévère et une augmentation d’asthme intermittent (p = 0,05) comparativement au GT (p = 0,24). En comparaison intergroupe, 50 % (17/34) des patients du GI ont montré une amélioration clinique comparativement au GT (p = 0,43). Bien que non statistiquement significative probablement dû à un manque de puissance, les patients du GI avaient 1,5 fois plus de chance de s’améliorer cliniquement en analyse multivariée (OR 1,47 CI95 % [0,57–3,75]. Les analyses secondaires montrent que : (i) dans la population des asthmes sévères, 12/16 patients (75 %) du GI se sont améliorés contre 7/16 du GT(44 %)(p = 0,06) ; (ii) la consommation des corticoïdes oraux et le taux d’hospitalisation sont significativement diminués dans le groupe ayant bénéficié de conseils (p = 0,029 et p = 0,042, respectivement).Conclusions :L’intervention des CEI est bénéfique cliniquement sur le niveau du contrôle de l’asthme et permet sur le plan économique une diminution de la consommation de soins, en cours d’investigation via les données d’Assurance maladie.

Abstract

International audience; Background: Scant data are currently available about a potential link between comorbid chronic lung diseases (CLD) and the risk and severity of the coronavirus disease 2019 (COVID-19) infection.Methods: To describe the clinical characteristics of and outcomes for patients with COVID-19 infection, including patients with comorbid respiratory diseases, who have been primarily hospitalized in the pulmonology department of Strasbourg University Hospital, France. In this retrospective, single-center study, we included all confirmed cases of COVID-19 from March 3 to April 15, 2020. We then compared the symptoms, biological and radiological findings, and outcomes for patients with and without CLD.Results: Of the 124 patients that were enrolled, the median age was 62 years, and 75 patients (60%) were male. Overall, 40% of patients (n=50) had preexisting CLD, including chronic obstructive pulmonary disease (COPD) (n=15, 12%) and asthma (n=19, 15%). Twenty-eight patients were transferred to the intensive care unit (ICU), and six patients died in our unit. Abstract

Allergic conjunctivitis affects 15 to 20% of the general population. It is currently evaluated by the Conjunctival Provocation Test (CPT), which is considered as the gold standard. In the investigation of allergic rhinitis and asthma, environmental exposure chambers (EEC) are increasingly utilised. For allergic conjunctivitis, EEC might be a valid alternative to the CPT. However, evaluation of the allergen response in individual provocation tests or in EECs is still in discussion due to the multiplicity of symptom scores. Indeed, there are many scores used to evaluate allergic conjunctivitis. The main criteria used were described by Abelson in 1990 and include redness, itching, tearing, and swelling. In clinical studies, the specifically ocular score most used is the Total Ocular Symptom Score (TOSS). Few treatments have been evaluated by EEC, including cold compresses, epinastine and N-acetyl aspartyl glutamic acid. Moreover, early data shows good correlation between ocular symptoms induced in an EEC and those assessed during natural exposure. EEC might be a valid alternative to CPT and correlate with natural seasonal allergen exposure. Finally, EEC might be useful in other fields as well, such as in the study of dry eye disease.

Abstract

Objective To describe the clinical and economic burden of severe asthma in France over 12 months. Methods Data were retrieved from the observational, prospective “Cohorte Obstruction Bronchique et Asthme” (COBRA) cohort, which has enrolled nearly 1000 asthma patients since 2007 from throughout France. Patients undergoing treatment with GINA step-4 or 5 medications uninterruptedly for 12 months (thus defining “severe asthma”) were identified and their clinical data used to describe the clinical burden of asthma (exacerbations, symptoms outside exacerbations, and level of asthma control). Patients’ utilization of healthcare resources was described and used to estimate the direct medical costs incurred to treat severe asthma. Results 155 patients were included in the present study. Over the 12-month period of interest, 128 (83%) patients experienced at least one asthma exacerbation, 22 (14%) patients were hospitalized for asthma, 133 (86%) patients experienced continuous symptoms outside exacerbations, and 77 (50%) patients experienced important limitations in daily life activities. The median number of asthma-related drugs used was 4. The mean estimated annual asthma-related cost was 8,222 euros (standard deviation, SD = 11,886), including 7,229 euros (SD = 11,703) for controller medications. Conclusion Symptoms outside exacerbation periods are highly prevalent in severe asthma patients, for whom the main driver of medical costs is controller medication.

Abstract

Formaldehyde is a common air pollutant of the indoor environment. It has a potential effect on the respiratory function of people and so studies have been performed to assess this using exposure methods with high concentrations of formaldehyde. However, epidemiology studies have shown its impact on allergic diseases at lower domestic levels. The aim of this study was to develop an exposure chamber to study effects of low controlled concentrations of formaldehyde. Concentrations in the chamber were quantified by a simple DNHP-derivatisation method followed by liquid chromatography coupled to UV detection. Experiments to assess formaldehyde generation yield and reproducibility of formaldehyde levels in the chamber were performed in an empty chamber. The temporal decay of formaldehyde concentration in the exposure chamber was also investigated for more than 2h under different experimental conditions, for example in darkness or with somebody inside the chamber. Actual measured levels of formaldehyde were found to be 78-91% of the calculated value expected. The study of the temporal decay showed that subject’s breathing accounted for the fate of approximately 50% of the formaldehyde lost. Leaks, wall deposition, photolysis and adsorption on clothes were processes identified to explain the remaining 50%. However, this study has shown that both homogeneity and stability of formaldehyde concentration in the gas phase can be maintained acceptably to carry out future human exposure experiments.

Abstract

Background Pre-clinical research has shown β2-adrenoceptors to be essential for the antiallodynic action of antidepressant drugs in murine models of neuropathic pain and that sustained treatment with β2-agonists has an antiallodynic action. Here, we clinically investigated whether chronic β2-agonist treatments may influence the incidence of post-thoracotomy chronic pain, defined as pain that recurs or persists along a thoracotomy scar more than 2 months after surgery, either neuropathic or non-neuropathic. Methods We conducted an epidemiological study on patients operated by thoracotomy. Demographic data, medical history and treatments concomitant to the surgery were recorded at a follow-up visit. Information on perioperative treatments was collected from the anaesthesia records and confirmed by the patients. In patients with pain at the surgery level, post-thoracotomy chronic pain was assessed by clinical examination and numeric scale. Physical examination and DN4 questionnaire were used to discriminate neuropathic and non-neuropathic chronic pain at scar level. Results One hundred and eighty-nine patients were included. Eighty-one patients reported persisting thoracic pain, with neuropathic characteristics in 58 of them (30% of the 189 patients). The most common chronic drugs during the perioperative period were inhaled β2-agonists (28.6%). The chronic use of β2-agonists was an independent predictor of thoracic neuropathic pain (but not of non-neuropathic pain) and was associated with a five-fold decrease in the relative incidence of neuropathic pain [OR = 0.19 (0.06–0.45)]. Conclusions These data suggest a possible influence of chronic β2-agonist treatments on neuropathic pain secondary to thoracotomy. This apparent preventive effect of β2-agonist treatments should warrant controlled clinical trials.

Abstract

International audience; Hypersensitivity pneumonitis (HP) occurred after organic antigens inhalation at home is rare and the diagnosis is very often difficult. We report the case of a 55-year male patient with allergic asthma since childhood, well controlled with inhaled corticosteroids, twice hospitalized for respiratory distresses. The patient presented fever (39°C), dry cough, rapidly progressive dyspnea, chest pain and crackles. Blood gas analysis found a hypoxemia of 52 mmHg, and CT-scan showed ground glass images in the upper lobes. Respiratory function tests showed severe obstructive syndrome and a decrease of diffusion test. HP was suspected because the symptoms were triggered by domestic environmental. The Medical Indoor Environment Councelor (MIEC) visited the patient's house and camper and performed air and dust samples. Moldy walnuts were found in the camper. The identification of microorganisms present in the air and on the surfaces in the camper was used for serum precipitins research by double diffusion (DD) and electrosyneresis (E) methods. From the 14 antigens tested, serological tests were considered significant for mesophilic Streptomyces (five arcs DD, six arcs E) and Penicillium chrysogenum (one arc DD, four arcs E). After removal from the camper of the objects suspected to be contaminated, the patient's symptoms regressed. This is a typical case of domestic HP to mesophilic Streptomyces and P. chrysogenum. The MIEC's intervention was useful in both diagnosis and treatment.

Abstract

Background The management of severe occupational asthma (OA) remains problematic and new alternative treatments providing better disease control are required, ideally enabling affected individuals to remain in their job. Methods Ten patients with severe uncontrolled OA were treated with the monoclonal anti-IgE antibody omalizumab. In six cases the causative agent was a high molecular weight (HMW) compound and in four cases it was a low molecular weight (LMW) chemical. All of the patients had well documented OA despite workplace adjustments. Results During treatment, nine patients exhibited a lower rate of asthma exacerbations and used less oral or inhaled corticosteroids. Seven patients were able to continue working at the same workplace as before treatment. Conclusion We have demonstrated that omalizumab is a potential treatment for severe uncontrolled OA and enabled seven of the ten patients in the study to remain in their job.

Abstract

Rural dairy farming is associated with high exposure to indoor endotoxins as compared to rural nonfarming houses and urban houses. The time spent on the mattress (7 h for an adult) and of the proximity of the contaminated source should be taken into account with the other causes of exposure. Studies in European children from a farming background have shown that these children have a reduced risk of asthma and atopic sensitization compared to their urban counterparts. It has been suggested that this might be due to exposure to high levels of endotoxin in the farming environment. The aim of this study was to compare indoor endotoxin concentrations in air and dust samples from randomly selected urban and rural dwellings. In the rural area, endotoxins were analysed in farmhouses and nonfarmhouses as well as housing characteristics, lifestyle factors and agricultural practices likely to influence air and dust endotoxin levels. Endotoxin levels were significantly higher in floor (6600 ± 6100 vs 3600 ± 5600 and 3800 ± 17,000 ng g⁻¹; P < 0·001) and mattress dust (2900 ± 4100 vs 1100 ± 2400 and 800 ± 2600 ng g⁻¹; P < 0·001) from farmhouses compared to other rural and urban homes. However, no difference was observed between endotoxin concentrations in the air of urban and rural houses, and airborne endotoxin levels did not correlate to dust levels. Lack of ventilation and direct entry into the house were correlated with an increase in dust endotoxin levels. These results confirm that dairy farming is associated with high exposure to endotoxins in indoor dust samples. No difference was observed between indoor airborne concentrations between urban and rural houses. These results suggest that measuring endotoxin in dust is the most relevant method to assess endotoxin exposure.

Abstract

International audience; BACKGROUND: Allergic sensitization to Alternaria has been related to asthma in various studies, but its association with allergic rhinitis is still controversial. OBJECTIVES: The aim of this study was to assess at the population level the relationships in childhood between Alternaria sensitization and 'past-year rhinoconjunctivitis' (PYRC), 'ever hay fever' (EHF) and 'ever allergic rhinitis caused by allergens other than pollens' (EAR) according to the presence or the absence of asthma. METHODS: This study is part of the Six Cities Study, the French contribution to the International Study of Asthma and Allergies in Childhood (ISAAC) Phase II. Children underwent skin prick test (SPT) to Alternaria and parents filled a standardized medical questionnaire. RESULTS: Some 6726 children with a mean age of 10 years were examined. The overall prevalence of Alternaria sensitization was 2.8%, 0.8% for monosensitization. Prevalences of symptoms in sensitized children were 27.7% for PYRC, 27.0% for EHF and 30.4% for EAR. Adjusted Odds Ratios (OR) between Alternaria sensitization and allergic rhinitis phenotypes were 2.34 (95% confidence interval: 1.51-3.63) for PYRC, 2.40 (1.65-3.50) for EHF and 2.95 (2.05-4.23) for EAR. The relationship still remained in the case of monosensitization to Alternaria for both PYRC and EAR when excluding the asthmatic children [OR = 3.87 (1.54-9.78) and 2.88 (1.10-7.55) respectively]. CONCLUSION: In our population-based sample of children, we found a link between Alternaria sensitization and allergic rhinitis, independently of asthma, which is compatible with the mechanisms of deposition of Alternaria in the upper airways.

Abstract

International audience; BACKGROUND: Masitinib is a tyrosine kinase inhibitor targeting stem cell factor receptor (c-kit) and platelet-derived growth factor (PDGF) receptor, which are expressed on several cell types including mast cells and bronchial structural cells, respectively. We hypothesized that c-kit and PDGF receptor inhibition may decrease bronchial inflammation and interfere with airway remodeling, which are crucial features of severe asthma. OBJECTIVES: The primary endpoint was the percent change from baseline in oral corticosteroids after 16 weeks of treatment. Change in asthma control (asthma control questionnaire), exacerbation rate, pulmonary function tests, rescue medication requirement and safety were secondary endpoints. METHODS: A 16-week randomized, dose-ranging (3, 4.5, and 6 mg/kg/day), placebo-controlled study was undertaken in 44 patients with severe corticosteroid-dependent asthma who remained poorly controlled despite optimal asthma management. RESULTS: At 16 weeks of treatment, a comparable reduction in oral corticosteroids was achieved with masitinib and placebo (median reduction of -78% and -57% in the masitinib and placebo arms, respectively). Despite this similar reduction, the Asthma Control Questionnaire score was significantly better in the masitinib arm as compared to placebo with a reduction by 0.99 unit at week 16 (P < 0.001) vs 0.43 unit in the placebo arm. Masitinib therapy was associated with more transient skin rash and edema. CONCLUSIONS: Masitinib, a c-kit and PDGF-receptor tyrosine kinase inhibitor, may represent an innovative avenue of treatment in corticosteroid-dependent asthma. These preliminary results warrant further long-term clinical studies in severe asthma

Abstract

Aldehydes concentrations were measured in 162 homes in the Strasbourg area (East of France) in the context of a case/control study pairing asthmatic and non-asthmatic people. The surveyed people have completed a questionnaire aiming to characterize the indoor homes and the people life practices. Gaseous aldehyde levels were quantified by a conventional DNHP-derivatization method followed by HPLC/UV. Formaldehyde, acetaldehyde and hexanal were the main encountered aldehydes with mean concentrations of 32.2±14.6, 14.3±9.7 and 8.6±8.1 μg m −3 , respectively, while propionaldehyde and benzaldehyde concentrations were usually −3 . The aldehydes concentrations simultaneously measured in both bedroom and living room were not significantly different except for formaldehyde and were correlated between them meaning that indoor air was quite homogenous in homes. Combination of information collected in our questionnaires and statistical analysis was used to investigate indoor aldehydes determinants. Even if formaldehyde sources are theoretically well identified, they are multiple so that it was difficult to determine the main parameters influencing its concentrations in domestic environment. Higher hexanal concentrations were related to new coatings such as painting, wallpapers and laminate floorings. Hexanal concentration decreased with both coating and furniture ages so that this compound may be considered as a tracer of these emissions.

Abstract

Resume Le formaldehyde est un polluant chimique du milieu interieur tres ubiquitaire. Les expositions a de fortes concentrations en milieu professionnel ont revele son caractere irritant et allergisant. Cependant, les taux d’exposition plus faibles en milieu domestique pourraient egalement avoir un effet sur la sante respiratoire, de facon non specifique puisque retrouve egalement pour d’autres polluants. Une potentialisation de la reponse a l’allergene a ete observee sur la sensibilisation chez l’animal et chez des enfants. Cette action a egalement ete retrouvee sur les symptomes respiratoires avec un risque d’asthme augmente de 39 % pour une exposition au domicile a plus de 60 μg m−3. Nous avons recemment pu montrer dans une etude chez des asthmatiques sensibilises aux acariens que la reponse a l’allergene lors de tests de provocation bronchique etait aggravee apres une exposition de 30 minutes a 100 μg m−3 de formaldehyde. Toutes les donnees montrent que la faible exposition au formaldehyde dans l’habitat est suffisante pour provoquer une sensibilisation mais egalement une aggravation des symptomes chez les sujets presentant un asthme allergique. Compte tenu des arguments publies, il conviendrait que soient connues les concentrations de formaldehyde dans les produits utilises au sein de l’habitat afin de permettre d’ameliorer la qualite de l’air interieur.
