Distalization is a conservative method that is utilized in orthodontics to gain space by moving posterior teeth distally. It may be combined with other space gaining strategies, such as expansion, or can be used alone. Many methods have been used for distalization. These methods differ significantly in their place, whether to be extraoral or intraoral, site of action in upper and/or lower arch, and cooperation needed by the patient if it is removable or fixed. This review illustrates some of the most commonly used methods for distalization with a brief presentation of three cases that incorporated successful distalization techniques.

This study focuses on the synthesis and characterization of a magnetic nanomaterial used in magnetic hyperthermia. Cerium gadolinium zinc-iron (CeGdZnFe) magnetic nanoparticles were synthesized by a coprecipitation method for application as magnetic hyperthermia agents. Determination of phase purity and their identification was achieved by X-ray diffraction studies using a Phillips powder diffractometer with Cu Kα radiation. Typical TEM micrographs of the dispersion of CeGdZn-ferrite nanoparticles and CeGdZn-ferrite PEG-encapsulated nanoparticles in ethanol deposited over a Cu grid were taken as part of the characterization techniques to be used for newly developed materials. It was then fitted by a Gaussian distribution with mean diameter dm ±1.0 nm. The investigation of magnetic properties showed that adjusting, Gd and Zn contributes to the nanoparticles added to the adjustment of all magnetic properties of CeGdZnFe.

Purpose. The purpose of our study is to evaluate the level of evidence (LOE) of spine surgery publications in the Arab countries and compare it with standard international literature in spine surgery and to determine the stand of the Arab nations academic production with that of the global one. Methods. An online search using “PubMed” and “Google Scholar” was carried out, using search terms related to spine surgery such as “Spine surgery,” “Scoliosis,” “Herniated disc.” Each article was reviewed and graded by two reviewers using Oxford Centre for Evidence-Based Medicine (OCEBM) Levels of Evidence scale. Results. We have identified 434 articles that met the inclusion criteria; 56% were level IV studies. The most common study design was case reports (42%). The number of Arab countries with publications in spine surgery was 18 countries. The country with the highest rate of publications was Egypt (26%). The quantity of the published studies increased from 151 in (2000–2008) to 283 in (2009–2015). There is statistical significance between high and low LOE articles (p=0.0007). Conclusion. We have observed that LOE has not changed significantly over the period of 15 years and that much of the publications are of a low LOE (levels III and IV). We, herein, emphasize the need for spine surgeons in the Arab countries to conduct studies of higher LOE.

Skull metastasis from lung cancer is relatively common, yet the first presentation for this malignant disease is a rare occurrence. We herein report a case of a 54-year-old female, who had a good outcome following Whipple procedure for periampullary adenocarcinoma five years before her current presentation. During a routine follow-up, she was found to have a slowly progressive painless right parietal swelling. The systemic screening workup revealed no abdominal disease, but a solitary pulmonary nodule was identified. The presence of these two lesions raised the diagnosis of metastases from a previously treated pancreatic adenocarcinoma. The patient underwent complete excision of the skull lesion and subsequent lung biopsy, both of which proved on histopathological examination to be consistent with a primary lung cancer. This case emphasizes the importance of imaging and histopathological correlation in the diagnosis of solitary skull metastases and their effect on the subsequent management.

Children with epileptic encephalopathy often have global impairment of brain function and frequent intractable seizures, which contribute further to their developmental disability. Many of these children have identifiable brain lesion on neurological imaging. In such cases, epilepsy surgery may be considered as a treatment option despite the lack of localized epileptic pattern on electroencephalogram (EEG). In this paper, we summarize the clinical features of epileptic encephalopathy syndromes and review the reported literature on the surgical approach to some of these disorders.

Temporal lobe epilepsy (TLE) is the most common form of medically intractable epilepsy. Advances in electrophysiology and neuroimaging have led to a more precise localization of the epileptogenic zone within the temporal lobe. Resective surgery is the most effective treatment for TLE. Despite the variability in surgical techniques and in the extent of resection, the overall outcomes of different TLE surgeries are similar. Here, we review different surgical interventions for the management of TLE.

Transorbital slow-penetrating injury is an uncommon type of head injury that is seen more often in the pediatric age group. This type of injury can be occult, which is often associated with serious complications. We report on a 4-year-old female who presented with orbital swelling after an unwitnessed right orbital injury following a fall on her face at her school. Three days after injury, the presence of a foreign body was discovered on imaging study when she presented with orbital swelling and purulent discharges. She recovered well after surgical and medical management. This paper sheds light on the importance of high suspicion for the presence and early surgical management of a penetrating foreign body.

The purpose of our study was to assess the role of a 64-slice multidetector CT (MDCT) scanner in the characterization of different solid renal masses, using a simplified approach to correct the postenhancement attenuation values. The study included 96 consecutive adults (58 men, 38 women) with renal masses; 93 with unilateral and three with bilateral masses. All of our patients underwent multiphasic CT study including pre- and postcontrast corticomedullary (CM) and nephrographic phases. We analyzed the images and corrected the postcontrast attenuation values at the CM phase. The postbiopsy or -surgical data were used as reference standard. There were 53 masses at the right kidney, 40 at the left kidney, and three bilateral. The final diagnosis of the 96 solid parenchymal masses were 28 clear-type renal cell carcinoma (RCC), 22 papillary-type RCC, 21 chromophobe-type RCC, six XP 11.2 chromosomal translocation–type RCC, 15 angiomyolipoma (AML), and seven oncocytoma. All the AML had fat, with attenuation values less than -40 HU at the nonenhanced scan. There is no difference in the precontrast attenuation values for the different types other than AML. At the postcontrast CM phase after the correction of the attenuation values, the clear cell type could be separated easily, with attenuation values >20 with specificity, sensitivity, and overall accuracy of 92, 84, and 93%, respectively. The 64-slice MDCT scanner with application of enhancement values correction allows diagnosis of clear cell carcinoma. Also, AML could be identified easily with fat inside at the precontrast scan.

We report a 9-year-old boy with acute lymphoblastic leukemia (ALL) in high-risk group who suffered from left sided focal seizures and ipsilateral hemiparesis during his induction with Asparaginase chemotherapy. Superior sagittal sinus thrombosis and right frontal hemorrhage were demonstrated on brain magnetic resonance imaging (MRI) scans . Anticoagulation was initiated with unfractionated heparin and switched to low molecular weight heparin after 3 weeks and continued for 6 months. At one-year followup, he had complete response to chemotherapy for ALL, with residual mild left hemiparesis, and his MRI scans revealed recanalized venous sinuses. The case highlights the importance of considering cerebral venous thrombosis as a complication of Asparaginase therapy.
