Objectives: The dimensional approach to psychopathology has been proposed to reliably evaluate suicidality. Potential gender modulation of psychopathological dimensions associated to suicide attempts was investigated., Methods: 91 subjects who committed a near-lethal suicide (SA group) and 374 who did not (nSA group) were recruited in a Psychiatric Intensive Care Unit regardless of their categorical diagnosis. The Hamilton Depression Rating Scale (HAM-D), the Brief Psychiatric Rating Scale - Expanded version (BPRS-E) and the Scale for Rapid Dimensional Evaluation (SVARAD) were administered at the admission. A Factorial Multivariate ANOVA was conducted according to General Linear Model: Sex and Suicidality were input as fixed factors, HAM-D, BPRS-E and SVARAD scores as dependent variables., Results: Men with SA (MSA) displayed significant lower scores in SVARAD Activation dimension compared to women with SA (FSA) (p=0.049), men without SA (MnSA) (p<0.001) and women without SA (FnSA) (p<0.001). Both SA groups displayed significant higher scores compared to nSA groups in regard of Depression item (BPRS-E) (p<0,001). The MSA group displayed significant lower scores in Psychomotor agitation (HAM-D) compared to FSA (p=0,044), MnSA (p<0,001) and FnSA (p<0,001)., Conclusions: By means of multifactorial statistics sex resulted a moderator of the relation between activation/agitation and suicidality, despite categorical diagnosis.

This narrative review addresses the interconnections among stress, mental disorders and migraine with a specific focus on non-pharmacological interventions that may be effective in improving both migraine and the psychiatric comorbidity. Migraine is often comorbid with depression, anxiety, personality disorders, and sleep disorders. Subjective stress and stressors are common triggers for migraine attacks and are risk factors for chronification, whilst mental disorders and stress responses are closely linked in a bidirectional relation. Recent studies show that psychiatric comorbidity is associated with migraine severity, worse outcomes, increased disability and reduce quality of life. Numerous studies on non-pharmacological interventions for migraine were published and behavioural treatments included biofeedback, cognitive-behavioural therapy, relaxation training, stress management and brief psychodynamic psychotherapy. Taken together, psychological interventions proved to be effective in migraine treatment and a combination of pharmacological and psychological treatment appear to be more effective than either medication or psychotherapy alone. Non-pharmacological interventions effectiveness should be due to the improvement of migraine, stress-related vulnerability and mental disorders together and the combined treatment could prevent the chronification circuit of migraine. Well-designed long-term studies are needed to clarify comparative effectiveness of non-pharmacological techniques in the treatment and the prevention of migraine.

Purpose: One of the most consistent models investigating the relationship between premorbid personality and depression was described by Tellenbach. According to this model, concern with orderliness, conscientiousness, hyper/hetereonomia and intolerance of ambiguity are the core features of Typus Melancholicus (TM). Previous studies showed a relationship between unipolar depression and TM. The primary hypothesis of this study was that patients affected by unipolar depressive disorders bare more of TM-related features of concern with orderliness and conscientiousness than healthy subjects., Methods: In this single center cross-sectional study we recruited 74 patients affected by unipolar depressive disorders according to DSM-5 criteria and 60 healthy controls. In order to assess the presence of TM's orderliness and conscientiousness we adopted the factors of Kasahara's Inventory for the Melancholic Type Personality (KIMTP) named "harmony in personal relationships" and "social norms". Stress-Related Vulnerability Scale (SVS) was administered to explore the relationship between measures of TM and perceived stress., Results: Independent sample t test disclosed significantly higher scores on KIMTP "harmony in personal relationships" (orderliness) but not on "social norms" (conscientiousness) in the clinical sample (p<0.001) compared to the non-clinical sample. Pearson's correlation disclosed a significant positive correlation between stress scores and KIMTP'S "harmony in personal relationships" subscale., Discussion and Conclusions: KIMTP's "harmony in personal relationships" (orderliness) was higher in patients with unipolar depression than in non-clinical subjects, and positively associated with perceived stress, while KIMTP's "social norms" (conscientiousness) was (i) similarly represented across the two groups, and (ii) not correlated with perceived stress. This raises the possibility that, among the core features of TM, concern with orderliness can be considered as a relevant and potentially premorbid personality factor in relation to unipolar depressive disorders.

Free will has often been considered central to criminal responsibility. Yet, the concept of free will is also difficult to define and operationalize, and, moreover, it is intensely debated. In particular, the very existence of free will has been denied based on recent neuroscience findings. This debate has significant implications on those fields in which the link between free will and behaviour is the main focus of interest, such as forensic psychiatry. In fact, a tension is often experienced between the centrality of the notion of free will on the one hand, and its controversial status on the other. This tension needs to be addressed, especially in forensic psychiatry, since it is relevant for actual assessments of legal insanity. In the present paper we will try to operationalize “free will” using a fourpartite decision-making capacity model, which can be used in forensic assessment of insanity. We will describe its advantages and application to guide mental insanity assessments. Whereas free will is often considered problematic from a neuroscience perspective, this model, we argue, is compatible with neuroscience; moreover, evaluations using this model can also be informed and strengthened by neuroscientific findings, for example regarding inhibitory control.

Migration is an important risk factor for the development of common and severe mental disorders. Nevertheless, in Europe, immigrants are less likely to access community mental health care and to adhere to treatments, with consequent emergency referrals, involuntary admissions, and traumatic coercive measures. At a clinical level, changes in practices and cross-cultural skills of mental health professionals might be crucial in addressing this challenge.

Unlabelled: Suicide attempts (SA) have been recognized among the most important predictors of suicide., Aim: The aim of this study was to assess suicidal ideation, socio-demographic and clinical characteristics in patients admitted to the emergency room for a SA and to identify factors associated with clinical outcome. Admission to a Psychiatric Intensive Care Unit and further SA within one year were considered., Materials and Methods: Patients that agreed to participate in the study were evaluated in the emergency room with a clinical interview and they were administered the Intent Score Scale. Patients were then contacted and interviewed by telephone 1, 6, and 12 months after the SA. Clinical outcomes and the occurrence of new SA were investigated., Results: Seventy-four patients were enrolled and 49, 24, and 20 patients were reassessed at the 1, 6, and 12 month follow-up respectively. Eight patients reported at least one new SA within one year. Six SA occurred within the first month. Unemployment and underemployment were significantly associated with further SA. The time interval before a new SA was longer for patients admitted at the Psychiatric Intensive Care Unit and shorter for those with high suicidal intent at the first SA., Conclusions: Although with limitations, our study might suggest that future research should focus on the role of hospitalization in the management of SA.

Aims: To assess in a sample of people with mental disorders: 1) fear of crime and perceived insecurity; 2) the association between fear of crime and insecurity; 3) the incidence of crimes., Methods: Twenty-four Italian mental health centres have been invited to participate in the study from the network of the Early Career Psychiatrists' Committee of the Italian Psychiatric Association. In each participating centre, the first 20 patients consecutively accessing the mental health centre between February and April 2011 have been recruited. All patients have been assessed using validated assessment tools., Results: The final sample consists of 426 patients. They are mostly female (70.1%), with a mean age of 45 years (± 13.5), and with a good level of education. Fifty-two percent of patients have a diagnosis of mood disorders, and 37.8% on anxiety spectrum disorders. About half of the sample declares that the most prominent feeling toward life is uncertainty. Almost all patients report to have at least one big fear, with the most frequently report being: 1) loss or death of a loved one (41.2%); 2) financial constraint (28.4%); 3) physical or mental health problems (26.5%)., Discussion and Conclusions: Our results show the presence of a common sense of uncertainty among patients, probably as a result of the historical moment we are facing. It is reasonable that this attitude toward life can have a detrimental impact on patients' psychological and physical wellbeing, contributing to high levels of distress. Further studies are needed in order to clarify the possible relationship between fears, uncertainty and mental disorders.

Background: Most depression rating scales are characterized by negatively-phrased questions, exploring the presence of various symptoms. Questions such as those regarding suicidal ideation or painful experiences may reduce accept-ability or even lead the reader to withdraw participation in the study. Although positively-worded items may be useful, it should be acknowledged that without formal testing they cannot be assumed to be equivalent to negatively-worded ones. The aim of the present study was to test the reliability and validity of a depression rating scale including only positively-phrased items., Methods: Two groups were enrolled in the study: the first comprised 104 adult psychiatric outpatients, the second 88 undergraduate students. All participants completed the depression scale of the Patient Health Questionnaire, the Zung Self-Rating Depression Scale, and the Positively-phrased Depression Scale (PDS), a 10-item self-report instrument in which the items are phrased in a positive way to reflect the absence of symptoms. Psychiatric outpatients also were rated by their clinician on the Hamilton Depression Rating Scale., Results: The internal consistency of the PDS was satisfactory. The correlations between scores on the PDS and on the other depression scales were moderate to high. Mean PDS scores of patients with a diagnosis of depressive disorder were significantly higher than those of patients with other mental disorders., Conclusions: Despite some limitations, this study suggests that the PDS a valid and reliable instrument which might prove particularly useful for the assessment of depressive symptoms in studies where issues of acceptability are important, such as studies on non-clinical populations, occupational samples, and patients drawn from non-psychiatric settings.

Studies have shown that psychological hardiness is an important stress resilience resource for individuals. The 15-items Dispositional Resilience Scale (DRS-15) is a short, reliable and valid self-report instrument to measure hardiness that is not available in Italian. The present study was undertaken to create an Italian version of the DRS-15, and evaluate its psychometric properties and validity in the Italian context. An Italian version was produced using multiple independent bilingual translators. This version was administered to a non-clinical sample of adults (N=150), along with measure o psychological well-being (PWB-18) and health. A sub-sample (N = 66) completed the DRS-15 again one month later. Results showed good reliability in terms of internal consistency and test-retest stability. With regard to the subscale, stability was high for all three subscales, whereas two subscales (Commitment and Control) showed marginal internal consistency. DRS-15 total and subscales scores showed a theoretically meaningful pattern of correlations with PWB-18 subscales, supporting the validity of the Italian DRS. Also, multiple regression analysis revealed a correlation between DRS-15 scores and self-rated general health, even after controlling for age and sex. The new Italian DRS-15 provides a valid, reliable and easy to use tool fr assessing stress resilience in clinical and research settings.

Aims: A robust psychopathological and psychotherapeutic tradition underscores the importance of the clinician's feelings in the assessment and therapeutic process. Our aim is to develop an instrument to evaluate psychiatrist' experience induced by each patient. This paper describes the development and preliminary validation of this instrument., Methods: Based on the relevant literature and our clinical experience, we developed a self-completed questionnaire consisting of 65 items covering various aspects of the clinician' emotional resonance. Ten psychiatrists completed our questionnaire immediately after first clinical interviews involving a total of 125 patients with various psychiatric diagnoses. Also, they completed the Profile of Mood State (POMS) immediately before and after these interviews. The factor structure and convergent validity of the questionnaire was examined., Results: We identified a six-factor structure. The factors showed acceptable internal consistency and were interpretable as different aspects of the clinician-patient interaction as experienced by the clinician. There was a consistent pattern of correlations between factor scores and changes in POMS scale scores during the clinical interviews., Discussion: The study results, though preliminary, suggest that clinicians' subjective reactions during their interactions with patients can be validly and reliably measured. Quantitative measurement of clinician's subjectivity is potentially useful in both clinical and research settings.

Introduction: People seeking for psychotherapeutic help present with a variety of conditions including stress, depression, anxiety, and maladaptive individual difference factors. This paper reviews the studies investigating the link between these conditions and immunity., Findings: The relationship between stress-related conditions and immunity varies depending on the nature of the stressor. Acute time-limited stressors are associated with an upregulation of natural immunity, brief naturalistic stressors with a shift from cellular to humoral immunity, while long-term and chronic stressors with a decrease in most functional immune measures. Depression is associated with changes in both enumerative and functional immune measures. The most consistent association is with decreased cellular immunity. There is considerable heterogeneity in study findings that may be accounted for by clinical and demographic factors. Only limited evidence has been presented for an association between anxiety and changes in immune function. The results are somewhat inconsistent and no firm conclusions can be drawn from these studies. Relatively few studies investigated the relationship between maladaptive individual difference factors and immunity. Replicated findings include an association between alexithymia and impaired cellular immunity and altered cytokine balance with a shift towards proinflammatory mediators, and a link between Type C and Type C-related coping and HIV progression and impaired immunity., Conclusions: Several conditions leading people to psychotherapy are associated with changes in immune function. However, several findings are preliminary and demand replication. Also, most of the evidence is correlational and further studies allowing for causal inferences are needed. Future psychoimmunology studies should better clarify the clinical relevance of the findings.

United States cinema motion pictures from the beginning of 20th century to the present are characterized by massive use of sterotypes to represent psychiatrist's image, as well as psychiatric treatment and inpatients psychiatric facilities. Representation tends to undergo considerable changes between psychiatric different historical periods. Psychiatric disorders also are commonly depicted in movies, often in a not realistic way. The images of psychiatrist and mental disorders shown in movies are likely to impact on the beliefs and attitudes of people towards psychiatry.
