We have developed a novel rapid test strip for detecting pancreatic amylase in urine and prospectively evaluated its accuracy in screening for acute pancreatitis (AP). The test strip is based on the immunochromatography principle and uses two monoclonal antibodies specific for pancreatic amylase. Urine samples were collected from 500 consecutive patients with acute abdominal disease (52 with AP) and prospectively tested with the strip. The accuracy of the test strip was compared with that of two quantitative urine amylase determinations and a urinary dipstick test for amylase (Rapignost). Sensitivity of the test was 69% and specificity was 97% in differentiating patients with AP from those with acute abdominal extrapancreatic disease at admission. The negative predictive value was 0.986. The test showed moderate agreement both with an assay measuring total amylase activity and with another measuring pancreatic amylase immunoreactivity. At similar high specificity (97%), quantitative determination of total amylase activity (cut-off 3960 U/L) and pancreatic amylase (cut-off 2180 micrograms/L) showed lower sensitivity (54% and 41%) than the test strip (69%). The test is specific and rapid to perform, and it rules out AP with high probability. It could therefore be useful in an emergency setting without laboratory facilities in the differential diagnosis of acute abdominal pain.

Micronutrient antioxidants interact with glutathione in tissues to facilitate the disposal of reactive oxygen species and xenobiotic metabolites derived via cytochromes P450. Published evidence linking cytochrome P450I induction with chronic pancreatitis therefore led us to compare antioxidant status in patients at Manchester in the northwest of England and at Madras in the southeast of India. Serum studies in healthy volunteers showed that the biologic availabilities of selenium and alpha-tocopherol were equally high in the two zones but that the availabilities of beta-carotene and ascorbic acid were lower in the tropical area (p < 0.001), where the ratio of ascorbic acid to total vitamin C concentration in serum was substantially reduced (p < 0.001). The serum antioxidant profiles of the chronic pancreatitis groups reflected these indigenous differences: a decrement in selenium and alpha-tocopherol was evident in both zones, whereas beta-carotene and ascorbic acid values were subnormal only in the Manchester group. The concentration of inorganic sulphate in urine--an index of long-term intake of sulphur amino acids for synthesis of glutathione and other detoxifiers--was similar in controls and patients from Manchester, but levels were lower than in their Madras counterparts (p < 0.02, p < 0.01, respectively). The results suggest that culinary practices that erode the biologic availabilities of ascorbic acid and beta-carotene may predispose to pancreatic oxidative stress and thereby to the changes leading to chronic pancreatitis at an early age in south India. These findings have implications for treatment and prophylaxis.

The urinary excretion of phenol, p-cresol, and indican was determined in 7 patients with the stagnant loop syndrome, 26 patients with coeliac disease, chronic pancreatitis, and partial gastrectomy, and 18 control patients. The mean excretion of the compounds in the patients with the stagnant loop syndrome and in the control patients, respectively, was 77 and 2.3 mg/24 h of phenol (p less than 0.05), 164 and 39.5 mg/24 h of p-cresol (n.s.), and 369 and 41.5 mg/24 h of indican (p less than 0.01). When applied as diagnostic tests for the stagnant loop syndrome, the phenol excretion showed 2 false negative results, the p-cresol excretion 3 false negative and 2 false positive results, and the indican excretion 6 false positive results. The combined use of phenol and indican determinations eliminated the number of false positive results with the indican test, and was found most useful as screening procedure. Determination of phenol and indican in a 24-hour urine sample is likely to provide a simple method for selecting patients with signs of abnormal bacterial colonization in the small intestine for more detailed investigations.

Thyrotropin-releasing hormone (TRH) has been shown to be present and have actions in the human gastrointestinal tract. We have studied urine TRH immunoreactivity (TRH-ir) levels in healthy subjects and patients with acute pancreatitis, gallstones, ulcerative colitis, or acute gastritis. The urine samples were prepurified by SP-Sephadex-C-25 cation exchange chromatography, subjected to reverse-phase high-pressure liquid chromatography, and assayed in our TRH radioimmunoassay. The mean urine TRH immunoreactivity values of healthy subjects were 4.42 +/- 1 ng/l (x +/- SEM); of patients with acute pancreatitis on the 1st day of hospitalization, 23 +/- 7 ng/l; on the 2nd day, 7 +/- 1 ng/l; and on the 3rd day 9 +/- 2 ng/l. Only the urine TRH levels of the pancreatitis patients on day 1 differed significantly (p less than 0.05) from the levels of the healthy subjects. Circulating TRH appears to be derived mostly from the pancreas, where the islets during acute pancreatitis are affected, and TRH is released into circulation and urine.
