Musculoskeletal ultrasound (MSK-US) use for diagnostic purposes is expanding in physical therapy practice. Identifying and describing physical therapy-specific approaches to incorporating MSK-US into the evaluation process is needed. Musculoskeletal ultrasound extends the physical exam to allow clinicians to visualize anatomy and pathophysiology both statically and dynamically. Purpose: To document 1) weekly use of diagnostic MSK-US; and 2) clinical reasoning approach used in challenging patient cases by physical therapists (PTs) registered by Inteleos in musculoskeletal sonography (RMSK-certified).Longitudinal, observational, cohort study using mixed methods for data collection and analysis. All 23 currently RMSK-certified PTs using MSK-US in clinical practice across the United States were contacted, and 16 participated. Data were collected using an online survey created with the Research Electronic Data Capture System. Participants documented MSK-US clinical use and significant cases using weekly, reflective, online journals for three months. Demographic data were summarized using descriptive statistics. Case data were analyzed thematically.Participating RMSK-certified PTs performed 1110 MSK-US examinations over 110 weeks. Clinicians averaged 7 (range 1-25) MSK-US examinations weekly, representing 28% of an average caseload. Examinations contributed significant anatomical/ pathological information 100% of the time. The most common joints scanned were the knee (n = 281), shoulder (n = 254), and wrist (n = 228). Case data revealed three themes: 1) augmenting the clinical evaluation to extend or narrow a diagnosis; 2) outcomes guiding action; and 3) lessons learned from clinical findings.RMSK-certified PTs regularly used MSK-US to validate and refine their clinical diagnoses and treatment. Ultrasound imaging directly influenced patient care by informing the diagnostic process, guiding treatment, and appropriately identifying referrals.

An interesting finding in the literature on vegetarianism and veganism finds that vegetarians and vegans often report that they deviate from their diets from time to time. Work examining this phenomenon finds that these dietary lapses relate to many factors; however, little research examines how these factors collectively influence dietary lapses while also controlling for the relationships that may exist among factors. Here, I fill this gap by drawing from the unified model of vegetarian identity (UMVI) and identity theory (IT) to propose an inclusive model of dietary lapses. Structural equation model results from a sample of 488 vegans reveal differences in how identity and interactional processes relate to dietary lapses across ethical and health motivations. This work is important because it highlights how identities relate to dietary behaviors differently for ethical and health vegans; it also provides fruitful avenues for future work in this area.

Private landowner participation in management initiatives can be encouraged by interventions, which must resonate with the underlying subjective motivations of the landowners. In this study, we use the Theory of Planned Behavior to gauge the relative influences of (1) attitudes; (2) subjective norms; and (3) perceived behavioral control on landowner intentions to harvest trees threatened by invasive insects. We use a survey (n=696) to estimate the effects of these latent factors among family forest owners in New England. Our results suggest that, overall, normative pressures are the dominant influence on landowners' harvest intentions. However, for certain subgroups, such as those with especially high levels of knowledge and experience with forest insects, or those with forestry experience, attitudes are dominant. Perceived behavioral control was not revealed to be dominant among any of our subgroups. These findings can be used to inform landowner interventions that are differentiated by landowner type.

Native plant of marigold (Calendula officinalis L.) synthesizes oleanolic acid saponins classified as glucosides or glucuronides according to the first residue in sugar chain bound to C-3 hydroxyl group. Hairy root culture, obtained by transformation with Agrobacterium rhizogenes strain 15834, exhibit a potent ability of synthesis of oleanolic acid glycosides. The HPLC profile of saponin fraction obtained from C. officinalis hairy roots treated with plant stress hormone, jasmonic acid, showed the 10-times increase of the content of one particular compound, determined by NMR and MALDI TOF as a new bisdesmoside saponin, 3-O-β-d-glucuronopyranosyl-28-O-β-d-galactopyranosyl-oleanolic acid. Such a diglycoside does not occur in native C. officinalis plant. It is a glucuronide, whereas in the native plant glucuronides are mainly accumulated in flowers, while glucosides are the most abundant saponins in roots. Thus, our results revealed that the pathways of saponin biosynthesis, particularly reactions of glycosylation, are altered in C. officinalis hairy root culture.

The objectives of this study were (i) to evaluate moisture loss (ML), solids gain (SG), moisture loss rate (d(ML)/dt), solids gain rate (d(SG)/dt), effective moisture diffusivity (DeM), and effective solids diffusivity (DeS) during osmotic dehydration (OD), ultrasound-assisted osmotic dehydration (UA-OD), and osmotic dehydration of whole cranberries, preceded by microwave-vacuum pretreatment (MV + OD) or freezing/thawing treatment (F + OD ) and (ii) to use a mathematical model to predict moisture content ML∞ and solids content SG∞ in equilibrium state and the distribution of moisture and sucrose in whole cranberries during dehydration. INTRODUCTION: The bromodomain and extra-terminal (BET) family of proteins are epigenetic readers of acetylated histones regulating a vast network of protein expression across many different cancers. Therapeutic targeting of BET is an attractive area of clinical development for metastatic castration-resistant prostate cancer (mCRPC), particularly due to its putative effect on c-MYC expression and its interaction with the androgen receptor (AR). AREAS COVERED: We speculate that a combination approach using inhibitors of BET proteins (BETi) with other targeted therapies may be required to improve the therapeutic index of BET inhibition in the management of prostate cancer. Preclinical data has identified several molecular targets that may enhance the effect of BET inhibition in the clinic. This review will summarize the known preclinical data implicating BET as an important therapeutic target in advanced prostate cancer, highlight the ongoing clinical trials targeting this protein family, and speculate on rationale combination strategies using BETi together with other agents in prostate cancer. A literature search using Pubmed was performed for this review. EXPERT OPINION: Use of BETi in the treatment of mCRPC patients may require the addition of a second novel agent.

In this paper, we use new data on military expenditure (milex) compiled by the Stockholm International Peace Research Institute (SIPRI) to investigate the relationship between military spending and economic growth. We focus on selected countries in Indo-Pacific Asia – an economically diverse but increasingly prosperous region with pockets of strategic competition and growing milex. We confirm the robustness of SIPRI’s milex data by corroborating it with defence budget data published by Australia’s Defence Intelligence Organisation (ADIO). We find no conclusive evidence of an arms race in the region. It is the growing economic prosperity that accounts for most of the growth in Indo-Pacific Asia’s milex. But we also find wide variations in the economic burden imposed by milex at the national level and that milex’ high level of aggregation masks important changes in national military capabilities. Strawberries can improve oxidants-antioxidants balance and reduce some cardiovascular risk factors in obese subjects. Paraoxonase-1 (PON-1) is a high-density lipoprotein-associated enzyme with antioxidant properties that can protect from coronary artery disease in humans. We examined the effect of strawberry consumption on plasma PON-1 activity and lipid profile in healthy nonobese subjects.Thirty-one subjects (body mass index [BMI] 24.4 ± 4.0 kg/m(2)) on their usual diet consumed 500 g of strawberry pulp daily for 30 days (first course) and after a 10-day washout the cycle was repeated (second course). Fasting blood and spot morning urine samples were collected before, during, and after each strawberry course (8 time points) for determination of paraoxonase and arylesterase PON-1 activities and lipid profile. Twenty subjects served as controls with respect to cholesterol and PON-1 activities changes over the study period.Strawberries decreased mean plasma paraoxonase PON-1 activity and this effect was more evident after the second course (by 11.6%, p0.05) than after the first course (5.4%, p = 0.06), whereas arylesterase activity was constant. Strawberries altered total cholesterol levels (p0.05) with a tendency to transiently decrease it (by 5.1%) only after 15 days of the first course. Triglycerides and high- and low-density lipoprotein cholesterol did not change in response to fruit consumption. No changes in PON-1 activities and lipid profile were noted in controls. Paraoxonase correlated with arylesterase activity (ƿ from 0.33 to 0.46 at the first 7 time points, p0.05). This association disappeared at the end of study (ƿ = 0.07) when the strongest inhibition of paraoxonase was noted.Supplementation of the usual diet with strawberries decreased paraoxonase PON-1 activity and did not improve lipid profiles in healthy nonobese subjects. Further studies are necessary to establish the clinical significance of paraoxonase suppression and to define a group of healthy subjects who can benefit from strawberry consumption with respect to cholesterol levels.

Ictal asystole (IA) is a rare phenomenon in patients with seizures with an incidence of 0.27-0.4% and has been proposed as a possible mechanism of sudden unexpected death in epilepsy patients (SUDEP). We present a case of a 53-year-old woman who initially presented with episodes of expressive aphasia and was treated with antiepileptic drugs (AEDs). While on therapy she experienced episodes of syncope. During her hospitalization for tapering of AEDs and 24-hour EEG monitoring, the patient developed a seizure followed by sinus bradycardia and an 18-second sinus pause, resulting in loss of consciousness and slowing of cerebral activity. Ten seconds after the return of cardiac activity, the EEG showed return of cerebral activity. A dual chamber pacemaker was implanted.

The purpose of this essay is twofold. First, I plan to argue that in light of Buddhist epistemology and metaphysics, it would be an inherent contradiction to the Buddhist tradition as whole to defend the cognitivist view that moral knowledge is possible. Quite the contrary, this essay will demonstrate that, in light of Buddhist theories of knowledge and metaphysical philosophies of no-self and emptiness, Buddhist ethics only makes coherent sense from a standpoint of non-cognitivism. Second, from the arguments that support a non-cognitivist reading of Buddhist ethics, I plan to show that such a standpoint does not entail moral nihilism. Rather, what we find in Buddhism is a middle-way ethic of pluralism. Herein I shall argue that the moral life of Buddhism non-cognitively arises within skandha of feelings, yet is conditioned by the cognitive nature of Buddhist wisdom.

Regular strawberry consumption augmented plasma antioxidant activity and decreased lipid peroxidation suggests preventive potential of these fruits against oxidative stress-dependent disorders. Blood phagocytes are important source of oxidants that may contribute to systemic oxidative stress. We examined the effect of strawberry consumption on the luminol enhanced whole blood chemiluminescence (LBCL) reflecting oxidants generation by circulating phagocytes in healthy subjects.Thirty-one healthy subjects (being on their usual diet) consumed 500 g of strawberry pulp daily (between 11.00-14.00) for 30 days (1st strawberry course) and after 10 day wash-out the cycle was repeated (2nd strawberry course). Fasting blood and spot morning urine samples were collected before and after each strawberry course for measuring resting and agonist (fMLP)-induced LBCL, various phenolics and plasma antioxidant activity. Twenty subjects served as a control in respect to LBCL changes over the study period.Strawberry consumption decreased median resting LBCL and this effect was more evident after the 1st course (by 38.2%, p0.05) than after the the 2nd one (18.7%), while fMLP-induced LBCL was constant. No changes in LBCL were noted in controls. Strawberries increased fasting plasma levels of caffeic acid and homovanillic acid as well as urolithin A and 4-hydroxyhippuric acid in spot urine. Plasma antioxidant activity and the number of circulating phagocytes did not change over the study period. Resting LBCL correlated positively with the number of circulating polymorphonuclear leukocytes at all occasions and negative correlation with plasma 4-hydroxyhippuric acid was noted especially after the first strawberry course (r = -0.46, p0.05).The decrease in resting LBCL suggests that regular strawberry consumption may suppress baseline formation of oxidants by circulating phagocytes. This may decrease the risk of systemic imbalance between oxidants and anti-oxidants and be one of mechanisms of health-promoting effect of these fruits consumption.

Percutaneous endoscopic gastrostomy (PEG) is often used for the feeding of patients with malnutrition due to dysphagia, and despite more than 30 years experience, numerous questions on its benefit remain. This was a prospective observational study to assess the safety of PEG.One hundred and nineteen patients mean age 63 years (21-91 years) who were admitted to the Hannover Medical School between November 2010 and March 2012 and had an indication for PEG according to the German guidelines were included. Primary endpoints were the following: reason for indication, date of in-hospital mortality after PEG insertion, death within 3 months after PEG placement, and complications.Most patients (54.6%) received PEG for dysphagia caused by tumors and second (29.4%) for neurologic diseases with a minor proportion of dementia (3%). About 73% of our patients had no complications at all and only 10% suffered severe effects. We saw only 1 case of aspiration, which did not lead to pneumonia. The 30-day mortality was 10%, and no patient died as a result of the PEG procedure. Significantly more patients with neurologic disorders died within 24 weeks of PEG placement than tumor patients (60% versus 27.7%, respectively, p = 0.002, n = 100).It is important to select patients receiving PEG very carefully. The patients' indications, their primary disease, and their capability for mental cooperation are essential. If these aspects are taken into account, PEG is a safe method with few mainly mild complications.

In recent years, significant advances in basic neuroanatomical studies have taken place. Moreover, such classical, clinically-oriented human brain imaging methods such as MRI, PET and DTI have been applied to small laboratory animals allowing improvement in current experimental neuroscience. Contemporary structural neurobiology also uses various technologies based on fluorescent proteins. One of these is optogenetics, which integrates physics, genetics and bioengineering to enable temporal precise control of electrical activity of specific neurons. Another important challenge in the field is the accurate imaging of complicated neural networks. To address this problem, three-dimensional reconstruction techniques and retrograde labeling with modified viruses has been developed. However, a revolutionary step was the invention of the "Brainbow" system, utilizing gene constructs including the sequences of fluorescent proteins and the usage of Cre recombinase to create dozens of colour combinations, enabling visualization of neurons and their connections in extremely high resolution. Furthermore, the newly- introduced CLARITY method should make it possible to visualize three-dimensionally the structure of translucent brain tissue using the hydrogel polymeric network. This original technique is a big advance in neuroscience creating novel viewpoints completely different than standard glass slide immunostaining.

The paper addresses the distinctiveness of migration flows in the post-Soviet space east of the European Union: the Commonwealth of Independent States (CIS) and Georgia. Population mobility within this region is hypothesized to differ from extra-regional population movements due to the common institutional past of these states, all of which were once the USSR republics. It has been claimed that CIS countries form a distinct post-Soviet migration system. Within the framework of the migration systems theory, this paper offers a quantitative analysis of the scale and mechanics of cross-border population mobility in the region. By means of network analysis, it examines the intensity and distinctiveness of intra-regional migration flows relative to those between the region and third countries. We have also used an econometric gravity model to identify the main drivers of migration flows in the region. The resultant findings show that the existence of the post-Soviet migration system is questionable and should not be treated axiomatically. This is because the mobility of people from and to the CIS region is relatively high, and the region is increasingly integrated into the international division of labor through trade and capital mobility. The cross-border mobility of people also reflects this globalization trend.

Reports on the success or failure of enterprise information system (EIS) implementation have been decidedly mixed. In this study, we focus on manufacturing infrastructure preparation prior to EIS implementation and report the results of a survey of 152 US manufacturing companies that have implemented EIS. We have provided four major findings in this study: (1) the requirements from customers and trading partners are more powerful drivers motivating US manufacturing firms to implementing enterprise systems than internal business planning needs; (2) one manufacturing infrastructural issue often has implications for other infrastructural items in implementing technology, so various manufacturing infrastructural issues should be prepared simultaneously; (3) manufacturing infrastructure preparation prior to EIS implementation has significant positive effects on customer-focused performance, production/operations performance, and financial performance; and (4) better customer-focused performance contributes to ...
