Abstract

Patients with coronary artery disease (CAD) are at high risk for secondary CV events. Few studies have been performed concerning the physicians' reactions to high BP in patients with manifest CAD.To compare the prevalence and management of high BP after percutaneous coronary intervention (PCI) in 2011-2012 with that in 2004.We identified 300 consecutive patients from 2011-2012 and 167 from 2004 with a follow-up visit after PCI at a University hospital in Sweden. Their medical records were reviewed to collect BP values and physicians' actions in response to an elevated BP at the follow-up visit.The proportion of patients who had their BP documented increased in 2011-2012 (91%) as compared to 2004 (79%). The proportion of patients with a BP ≥140 mmHg systolic and/or ≥90 mmHg diastolic at the follow-up visit decreased from 55% in 2004 to 42% in 2011-2012 (p = 0.014).BP documentation and control improved from 2004 to 2011-2012 with more patients reaching target BP levels. One of the reasons for the improvements may be the introduction of a secondary preventive unit with cardiovascular specialised nurses who participate in a national quality registry.

Abstract

This prospective, single-blinded study investigated the validity and reliability of cervical auscultation (CA) under two conditions; (1) CA-only, using isolated swallow-sound clips, and (2) CSE + CA, using extra clinical swallow examination (CSE) information such as patient case history, oromotor assessment, and the same swallow-sound clips as condition one. The two CA conditions were compared against a fibre-optic endoscopic evaluation of swallowing (FEES) reference test. Each CA condition consisted of 18 swallows samples compiled from 12 adult patients consecutively referred to the FEES clinic. Patients' swallow sounds were simultaneously recorded during FEES via a Littmann E3200 electronic stethoscope. These 18 swallow samples were sent to 13 experienced dysphagia clinicians recruited from the UK and Australia who were blinded to the FEES results. Samples were rated in terms of (1) if dysphagic, (2) if the patient was safe on consistency trialled, and (3) dysphagia severity. Sensitivity measures ranged from 83-95%, specificity measures from 50-92% across the conditions. Intra-rater agreement ranged from 69-97% total agreement. Inter-rater reliability for dysphagia severity showed substantial agreement (rs = 0.68 and 0.74). Results show good rater reliability for CA-trained speech-language pathologists. Sensitivity and specificity for both CA conditions in this study are comparable to and often better than other well-established CSE components.

Abstract

Dealing with conflicting and target-specific requirements is an important issue in multisensor and multitarget tracking. This paper aims to allocate sensing resources among various targets in reaction to individual information requests. The proposed approach is to introduce agents for every relevant target responsible for its tracking. Such agents are expected to bargain with each other for a division of resources. A bilateral negotiation model is established for resource allocation in two-target tracking. The applications of agent negotiation to target covariance tuning are illustrated together with simulation results presented. Moreover, we suggest a way of organizing simultaneous one-to-one negotiations, making our negotiation model still applicable in scenarios of tracking more than two targets.

Abstract

Increased levels of N-terminal prohormone brain natriuretic peptide (NT-proBNP) are associated with left ventricular dysfunction (LVD) and left ventricular hypertrophy (LVH), but the relation of NT-proBNP to ambulatory blood pressure (ABP) and hypertensive target organ damage in high-risk patients with peripheral arterial disease (PAD) has not been studied. We hypothesized that NT-proBNP levels were increased in patients with PAD in comparison to a matched control group and that levels of NT-proBNP were related to ABP.Blood samples were analysed for NT-proBNP in 103 males with PAD and 96 age- and sex-matched controls. Ninety-eight PAD patients performed ABP monitoring and 99 underwent Tc-99m Sestamibi myocardial perfusion SPECT.NT-proBNP was significantly increased in PAD patients compared with controls [median (interquartiles)] 167(76, 418) vs 68(38, 142) pg/ml, p0.001. Plasma levels of NT-proBNP correlated positively to systolic blood pressure (SBP), pulse pressure (PP), night-day ratio of SBP and showed the strongest correlation to average night PP (r=0.42, p0.001). In multiple regression analysis, night PP remained independently related to NT-proBNP.NT-proBNP levels are markedly increased in PAD patients compared to age-matched controls. Night PP is related to NT-proBNP levels independently of other variables highlighting the importance of ambulatory PP as a cardiovascular risk factor. Measurement of NT-proBNP could be indicated in PAD patients for further risk stratification.

Abstract

The study reports the results of a clinical dysarthria test procedure applied to a group of 59 normal subjects, and two patient groups: 30 individuals with Parkinson's disease (PD) and 30 with multiple sclerosis (MS). The assessment protocol consists of 54 test items divided into 6 subtests—Respiration, Phonation, Oral motor performance, Articulation, Prosody and Intelligibility. The test procedure generated statistically significant differences between a) normal adult subjects and each of the two neurological subject groups; b) normal adult subjects and individuals with dysarthria; and c) normal adult subjects and individuals with MS who did not show any audible signs of abnormality in continuous speech.

Abstract

The main objective of laryngectomee rehabilitation is to restore the ability to communicate orally as well as possible. For a long time electrolarynx speech and oesophageal speech were the only available options for the laryngectomised patient. Since the introduction of surgical prosthetic voice restoration over a decade ago, several new alternatives have become available. The technique, which involves a voice prosthesis in a surgically created tracheo-oesophageal fistula, enables the patient to use pulmonary air for speech. This technique often means considerable improvement to the life quality of the laryngectomised patient.This paper presents a five-year-experience with surgical voice rehabilitation of laryngectomees in terms of patient selection, presurgical evaluation, and two types of low pressure voice prostheses (Blom-Singer, Provox). The importance of the rehabilitation team is stressed.

Abstract

Diagnostic ultrasound was employed to visualize the activity of the tongue, the floor of the mouth, and the hyoid in normal and laryngectomized individuals. Differences in articulatory and masticatory dynamics were noted between groups and among alaryngeal speech modes. Swallowing was seen to be less efficient in some of the laryngectomees. Abnormal activity in the floor of the mouth and the hyoid was observed in all laryngectomized talkers, especially during conventional esophageal speech.
