Abstract

The aim of the present study was to investigate the quantity and quality of hydroxyapatite (HA) in the zone of calcified cartilage of articular cartilage and underlying subchondral bone by Fourier transform infrared microscopy (FTIRM). We hypothesized that the amount of HA deposited in osteoarthritis (OA) might differ from normal cartilage, and that such differences might be significant in the pathology of OA. The tibial epiphysis of the male Hartley strain for the spontaneous OA model and the control Strain 13 guinea pigs (12-month old) were examined. The mineral:matrix ratio was determined by integrating areas of υ1, υ3 bands at 900-1200 cm-1 to Amid 1 band at 1585-1725 cm -1. The CO3:PO4 ratio was determined by integrating areas of υ2 carbonate band at 850-900 cm-1 to υ1, υ3 bands. In Hartley strain, the mean mineral:matrix ratio of the medial side was significantly lower than the lateral side at the zone of the calcified cartilage and subchondral bone. The mean CO3:PO 4 ratio was significantly higher in the medial side than the lateral one at the zone of the calcified cartilage and subchondral bone. In contrast, the results obtained from Strain 13 showed no significant differences between the two sides. Our results showed diminished deposition and increased maturation of HA in Hartley strain. These findings may correlate with the pathological process of knee OA, and indicate that FTIRM is a useful tool for determining the variability of HA in OA.

Abstract

We investigated the histological and immunohistochemical changes within and around the cervical intervertebral discs in symptomatic myeloradiculopathy and myelopathy patients. A total of 163 cervical intervertebral discs, 100 herniated discs and 63 spondylotic discs, were harvested en bloc during anterior decompressive surgeries and histological examination was performed, focusing on nucleus pulposus, annulus fibrosus, cartilaginous endplate, and ligamentous enthesis. There was a close relationship between MRI grade and histological degenerative changes. Histological features of the herniated intervertebral discs were the presence of granulation tissue, newly-developed blood vessels and infiltration of CD68-positive cells, which surrounded the herniated tissue in the outer layer of the annulus fibrosus. Marked vascular invasion was detected in 67% of herniated discs and 54% of spondylotic discs. Immunohistochemically, chondrocytes positive for matrix metalloproteinase (MMP)-3, tumor necrosis factor (TNF)-α, basic fibroblast growth factor (bFGF), and vascular endothelial growth factor (VEGF) were detected in both herniated and spondylotic discs. Our results indicate that the neoangiogenesis process is associated with overexpression of bFGF and VEGF, and suggest that intervertebral disc degradation is enhanced by macrophage infiltration, which is regulated by MMP-3 and TNF-α. The expression of these proteins may be enhanced by the tear of annulus fibrosus.

6. Demonstration of Biological Aggressiveness of Bone Giant Cell Tumor by the Comparative Study of Immunohistochemical Detection of DNA-instability and Cortical Bone Destruction by CT

Abstract

The degree of DNA-instability as revealed by the immunohistochemical staining with anti-single-stranded DNA antibody after acid hydrolysis (DNA-instability test) was used as a marker of malignancy. This was applied to benign (4 osteochondroma and 4 enchondroma cases), border-line (23 bone giant cell tumor, BGCT cases), and malignant (6 chondrosarcoma and 6 osteosarcoma cases) neoplastic lesions. The expression of tumor oncogene, c-myc was detected immunohistochemically. Proliferative activity was evaluated by PCNA-immunohistochemistry, and the quantitative analysis of the number, mean area, mean total area, the largest area, and maximum shape-irregularity of AgNORs in a nucleus were performed for all these cases. The results for 19 BGCT (82.6%) cases, 6 chondrosarcoma cases (100%), and 6 osteosarcoma cases (100%) were positive with the DNA-instability test, indicating their malignancy. All benign tumor cases were negative with the DNA-instability test. Reflecting the malignant character, all chondrosarcoma cases, all osteosarcoma cases and the BGCT cases positive with the DNA-instability test showed statistically highervalues of PCNA-index; and all AgNORsparameters in comparison to those for benign tumor cases, and c-myc was positive for 66.7%, and 26.3%, of them, respectively.But these values for BGCT with positive and negative DNA-instability test results showed no statistically meaningful differences, All the BGCT cases negative with DNA-instability test were negative for c-myc expression. Among the BGCT cases positive with DNA-instability test, 18 cases (94.7%) showed cortical bone destruction by computed tomography (CT), and 5 cases (26.3%) showed extra-osseous expansion. No such radiographic changes were detected among the BGCT cases negative with DNA-instability test.Among 18 BGCT cases with cortical bone destruction, 5 cases (27.8%) showed tumor recurrences, and 2 cases (11.1%) showed lung metastases. These results indicate that the majority of BGCT cases are malignant and the detection of cortical bone destruction by CT is a sensitive clinical marker to detect them.

7. Prognostic Factors and Proliferative Activities in Breast Cancer. A Comparative Study between Recurrent and Non-Recurrent Cases of Human Breast Carcinomas

Abstract

Twenty-one recurrent and twenty-six nonrecurrent cases of breast carcinoma were compared with respect to prognostic factors and proliferative activities. The mean number of lymph nodes involved (lymph node score) and tumor stage in recurrent and non-recurrent cases were 10.0±14.5/2.05±0.59 and 0.96±2.75/1.42±0.50, respectively. Expression of p53/c-erbB-2 was demonstrated immunohistochemically in 28.6/52.4% of recurrent cases, and in 15.4/34.6% of non-recurrent cases, respectively. PCNA indices in the two groups were 47.8±14.2 and 39.5±18.2, respectively. Fluorescent nucleolar organizer regions (FINORs) scores determined by confocal laser scanning microscope in the two groups were 3.03±0.78 and 2.32± 0.54, respectively. There were significant differences between the two groups in lymph node score, tumor stage, and FINORs score, but not in the expression of p53 or c-erbB-2 or in PCNA indices. These findings indicate that lymph node score, tumor stage, and FINORs score may be useful as prognostic factors in patients with breast carcinoma.

Abstract

The cell proliferative activity and capability of cytodifferentiation of cancerous and non-cancerous thyroid tissues were studied using 3H-thymidine (3H-TdR) autoradiography and immunohistochemistry for thyroglobulin. Cancerous lesions were induced by diisopropanolnitrosamine (DIPN) followed by methylthiouracil (MTU). Regenerative thyroid tissues were obtained through wounding. All the rats treated successively with DIPN and MTU were found to have carcinomas that invaded the surrounding tissue or blood vessels. By cumulative labeling with 3H-TdR, the maximal cell cycle times (Tc) of invasive and non-invasive parts of carcinomas were estimated to be 120hr and 180hr, respectively, whereas the Tc of regenerative and hyperplastic thyroid tissues were estimated as 113hr and 519hr, respectively. However, the duration of S phase (Ts) was greater in cancers (15-35hr) than in non-cancerous tissues (3-4hr), and the cancerous tissues showed lower expression of thyroglobulin than the non-cancerous thyroid tissues. These may reflect some injury in the DNA of cancer cells.
