Purpose To assess the effect of varying levels of torsional power on phacoemulsification efficiency using an active-fluidics phacoemulsification platform with a 0.7 mm Sonata tip. Setting John A. Moran Eye Center Laboratories, University of Utah, Salt Lake City, Utah, USA. Design Experimental study. Methods A 0.7 mm tip was used to emulsify 2.0 mm porcine lenses that had been hardened in formalin. The torsional power was varied from 10% to 100% at intervals of 10%, and efficiency and chatter were recorded. A comparison of the 0.7 mm tip with the 0.9 mm Balanced tip was also performed. Using a torsional power setting of 90% and 100%, the 0.7 mm tip and the 0.9 mm tip efficiency times were recorded. Results The study showed an increase in efficiency as torsional power increased. The relationship was linear and more efficient up to 90% ( R 2 = .8025, P = .0005). Chatter was only observed at a torsional power setting of 60%. In addition, a head-to-head comparison of the 0.7 mm tip with the 0.9 mm tip showed that both tips were statistically similar in efficiency times, despite the smaller diameter of the 0.7 mm tip. Conclusions The optimum torsional power setting with the Centurion platform in conjunction with the 0.7 mm tip was 90%. The efficiency of the 0.7 mm tip and the 0.9 mm tip was statistically similar. Study Design: A register study with 2 years of follow up., Objective: To identify predictors of an unfavorable vocational prognosis after hospital contact for herniated lumbar disc (HLD)., Summary of Background Data: There is sparse information about vocational prognosis among HLD patients diagnosed at hospital., Methods: We followed all in- and outpatients diagnosed with HLD at a Danish University Hospital 2001 to 2005 eligible for the labor market in the Danish National Register on Public Transfer Payments (n = 2039). Clinical data were obtained from the Danish National Patient Register. The outcome measure was unfavorable vocational prognosis defined as less than 40 weeks of employment within the second year after hospital contact. Cox proportional hazards models were used., Results: Altogether 41.8% had an unfavorable vocational prognosis. The outcome was associated with unskilled work Hazard Ratio (HR) 2.1 (95% confidence interval [CI] = 1.5-2.8), skilled work HR 1.9 (CI = 1.3-2.7), and semi academic work HR 1.5 (CI = 1.1-2.0) as compared with academic work and less than 40 weeks of employment within year two before hospital contact HR 2.1 (CI = 1.9-2.5). Further negative prognostic factors were lumbar fusion alone HR 1.4 (CI = 1.1-1.8) and in combination with discectomy HR 1.6 (CI = 1.2-2.2) as compared with nonsurgical treatment, ethnicity other than Danish HR 1.55 (CI = 1.2-1.8), and female gender HR 1.2 (CI = 1.1-1.4). Discectomy, age, and year of inclusion were not associated with the outcome., Conclusion: The risk of an unfavorable vocational prognosis after hospital contact for HLD was substantial. Nonacademic work and less than 40 weeks of employment within year two before hospital contact were the strongest prognostic factors, but also lumbar fusion alone and in combination with discectomy, ethnicity, and gender had a negative influence. There seems to be a need for actions addressing these patient categories to avoid long-term sick leave and premature withdrawal from the labor market.

Background: Little is known about international variation in mortality after severe trauma. This study examines variation in mortality, injury severity, and case management among countries from a recent prospective multinational trauma trial., Methods: This trauma trial was a prospective, randomized, double-blinded, multicenter comparison of recombinant activated factor VII versus placebo in severely injured bleeding trauma patients. Differences in baseline patient characteristics, case management, and clinical outcomes were examined for the 11 countries recruiting most patients. Between-country differences in mortality were examined using regression analysis adjusting for case mix and case management differences. Global predictors of mortality were also identified using multivariate regression analysis., Results: Significant differences were observed between countries in unadjusted mortality rates at 24 hours (p = 0.025) and 90 days (p < 0.0001). When adjusting for differences in case mix and case management, the between country differences in mortality at 24 hours and 90 days remained significant. Consistent independent predictors of 24-hour, 24-hour to 90-day, and 90-day mortality were admission lactate >or=5 mmol/L (odds ratio: 9.06, 3.56, and 5.39, respectively) and adherence to clinical management guidelines (odds ratio: 4.92, 5.90, and 3.26, respectively). On average, the damage control surgery guideline was less well adhered to than the RBC transfusion and ventilator guidelines. There was statistically significant variation between countries with respect to adherence to the RBC transfusion guideline., Conclusions: Considering international variation in mortality when designing or interpreting results from multinational trauma studies is important. Significant differences in mortality persisted between patients from different countries after case mix and case management adjustment. Adherence to clinical guidelines was associated with improved survival. Stratification, case mix adjustment, and use of guidelines on damage control surgery, transfusion, and ventilation may mitigate country-driven variation in mortality.

Background: Intraoperative transfusion of red blood cells (RBC) is associated with adverse outcome after orthotopic liver transplantation (OLT). Although experimental studies have shown that platelets contribute to reperfusion injury of the liver, the influence of allogeneic platelet transfusion on outcome has not been studied in detail. In this study, we evaluate the impact of various blood products on outcome after OLT., Methods: Twenty-nine variables, including blood product transfusions, were studied in relation to outcome in 433 adult patients undergoing a first OLT between 1989 and 2004. Data were analyzed using uni- and multivariate stepwise Cox's proportional hazards analyses, as well as propensity score-adjusted analyses for platelet transfusion to control for selection bias in the use of blood products., Results: The proportion of patients receiving transfusion of any blood component decreased from 100% in the period 1989-1996 to 74% in the period 1997-2004. In uni- and multivariate analyses, the indication for transplantation, transfusion of platelets and RBC were highly dominant in predicting 1-yr patient survival. These risk factors were independent from well-accepted indices of disease, such as the Model for End-Stage Liver Disease score and Karnofsky score. The effect on 1-yr survival was dose-related with a hazard ratio of 1.377 per unit of platelets (P = 0.01) and 1.057 per unit of RBC (P = 0.001). The negative impact of platelet transfusion on survival was confirmed by propensity-adjusted analysis., Conclusion: This retrospective study indicates that, in addition to RBC, platelet transfusions are an independent risk factor for survival after OLT. These findings have important implications for transfusion practice in liver transplant recipients.

Study Design: Randomized controlled trial., Objective: To evaluate the effectiveness of an ergonomic and psychosocial intervention in reducing low back pain (LBP) among health care workers., Summary of Background Data: LBP and injuries are reported frequently among health care workers worldwide. Improvement of person-transfer techniques is the preferred tool in the prevention of both. Although popular, to our knowledge, any effect has not been documented in controlled trials., Methods: Study participants were eldercare workers from 19 eldercare groups randomly assigned to the transfer technique, stress management, or reference arm. A total of 163 individuals (79% of the source population) participated in both baseline and follow-up after 2 years. Outcome was intra-individual change in rating of LBP during the past 3 and 12 months., Results: We found no difference in LBP in any of the intervention arms over the study period., Conclusion: The study showed no effect of a transfer technique or stress management program targeting LBP. Thus, there is a need for discussing other priorities in the prevention of LBP among health care workers.
