Group treatment is explored in light of its psychosocial context, treatment effectiveness, and efficiency. A summary of the four group treatment approaches (two for children, two for adults) for communicative disorders are provided: classroom-based elementary school group, multicultural and multilinguistic preschool group, alaryngeal speech rehabilitation and support group, and cooperative group treatment for adults with aphasia. Similarities and differences among the group treatments are presented. Similarities include group processes, communication goals, psychosocial skills, accountability, and naturalistic context. Among the differences are settings for treatment, collaborative versus independent service delivery models, the components of functional communication, group size, group composition, and customized therapy techniques. Role-playing, routines and scripts, themes, authentic vocabulary, and cooperative methods are techniques useful to group therapy. [ABSTRACT FROM AUTHOR]

Four crisis-precipitating events are identified that have the potential to interfere with the individual's overall adjustment following laryngectomy. Important to the success of alaryngeal speech rehabilitation is the clinician's ability to recognize how laryngectomy may influence the individual's physical, social, psychological, and occupational well-being. The speech-language pathologist must be cognizant of and sensitive to the impact that the individual's perception of the quality of life has on alaryngeal speech rehabilitation. The design and implementation of an intervention protocol within the group setting using themes-educational, speech activities, social interaction, and support-counseling-are based on how well the individual is coping, adjusting, and communicating in the process of alaryngeal speech rehabilitation. Hypertension (HTN) and heart failure (HF) can chronically activate the renin-angiotensin-aldosterone system, a mechanism designed to maintain hemodynamic stability by reabsorption of water and electrolytes. Additionally, this system activates the sympathetic nervous system to increase vagal tone. When these patients face acute illness requiring hospitalization, the acute stressor or pathogen also activates the sympathetic nervous system. The combination of activation of both systems puts patients at increased risk of organ failure, specifically renal failure. With early recognition of renal insult, organ damage can be reversed. C-reactive protein (CRP) and D-dimer are commonly used to measure acute inflammation. These biomarkers can alert critical care nurses to excessive inflammation in patients with underlying HTN and HF, enabling nurses to make informed decisions to intervene at the earliest sign of renal failure. This retrospective study of adult SARS-CoV-2 patients in an intensive care unit setting sought to examine the relationship of CRP, D-dimer, and the need for eventual renal support in patients with HF and HTN. Of the sample (n + 189), mean age was 62 (SD = 14.0), and most (70.9%) were male. Thirty-nine patients (20.6%) required renal support. Of the cases requiring renal support, 21 (53.8%) had a history of prior renal disease (P < 0.001, r = 0.351). History of HTN was significantly correlated with requirement for renal support (P = 0.010, r = 0.187). D-dimer (P = 0.038, η = 1.0) and CRP (P = 0.018, η = 0.924) were also significant. Survival was significantly worse in the renal support group (P < 0.001, r = -0.310). D-dimer and CRP were correlated with more severe illness and need for renal support. Purpose: To evaluate Pegasus optical coherence tomography (OCT), a clinical decision support software for the identification of features of retinal disease from macula OCT scans, across heterogenous populations involving varying patient demographics, device manufacturers, acquisition sites, and operators., Methods: Five thousand five hundred and eighty-eight normal and anomalous macular OCT volumes (162,721 B-scans), acquired at independent centers in five countries, were processed using the software. Results were evaluated against ground truth provided by the data set owners., Results: Pegasus-OCT performed with areas under the curve of the receiver operating characteristic of at least 98% for all data sets in the detection of general macular anomalies. For scans of sufficient quality, the areas under the curve of the receiver operating characteristic for general age-related macular degeneration and diabetic macular edema detection were found to be at least 99% and 98%, respectively., Conclusion: The ability of a clinical decision support system to cater for different populations is key to its adoption. Pegasus-OCT was shown to be able to detect age-related macular degeneration, diabetic macular edema, and general anomalies in OCT volumes acquired across multiple independent sites with high performance. Its use thus offers substantial promise, with the potential to alleviate the burden of growing demand in eye care services caused by retinal disease.
