Complex regional pain syndrome (CRPS) is characterized by severe and chronic pain, but the pathophysiology of this disease are not clearly understood. The primary aim of our case-control study was to explore neuroinflammation in patients with CRPS using positron emission tomography (PET), with an 18-kDa translocator protein specific radioligand [C]-(R)-PK11195. [C]-(R)-PK11195 PET scans were acquired for 11 patients with CRPS (30-55 years) and 12 control subjects (30-52 years). Parametric image of distribution volume ratio (DVR) for each participant was generated by applying a relative equilibrium-based graphical analysis. The DVR of [C]-(R)-PK11195 in the caudate nucleus (t(21) = -3.209, P = 0.004), putamen (t(21) = -2.492, P = 0.022), nucleus accumbens (t(21) = -2.218, P = 0.040), and thalamus (t(21) = -2.395, P = 0.026) were significantly higher in CRPS patients than in healthy controls. Those of globus pallidus (t(21) = -2.045, P = 0.054) tended to be higher in CRPS patients than in healthy controls. In patients with CRPS, there was a positive correlation between the DVR of [C]-(R)-PK11195 in the caudate nucleus and the pain score, the visual analog scale (r = 0.661, P = 0.026, R = 0.408) and affective subscales of McGill Pain Questionnaire (r = 0.604, P = 0.049, R = 0.364). We demonstrated that neuroinflammation of CRPS patients in basal ganglia. Our results suggest that microglial pathology can be an important pathophysiology of CRPS. Association between the level of caudate nucleus and pain severity indicated that neuroinflammation in this region might play a key role. Background: Quantification of volume and inflammatory activity in the fat tissue is important because these are closely related to type 2 diabetes mellitus and cardiovascular disease. Fluorine-18 fluorodeoxyglucose (F-FDG) PET/computed tomography (CT) has been utilized to measure the metabolic activity of fat tissue. In this study, we assessed the feasibility of simultaneous PET/magnetic resonance (MR) in metabolic and volumetric measurements of fat tissue and the potential advantage over PET/CT., Methods: Twenty-four healthy individuals were enrolled, who underwent simultaneous F-FDG PET/MRI. Twenty-five F-FDG PET/CT scans were selected. Isocontour volumes of interest (VOIs) were used to segment and separate visceral fat (VF) and abdominal subcutaneous fat (SF) in using the MR image (T1 DIXON VIBE sequence) of PET/MR and the CT image of PET/CT. Volume, mean standardized uptake value of VF, and SF VOIs were calculated., Results: Overlap between F-FDG PET and VF VOI was better in F-FDG PET/MR than PET/CT. The mean standardized uptake value of VF was associated with the degree of intestinal uptake on F-FDG PET/CT, but not on F-FDG PET/MR. Volumetric and metabolic measurements using F-FDG PET/MR showed an excellent reproducibility, with a high intraclass correlation coefficient between different observers (0.951-0.997). The measured metabolic activity was higher in VF than SF., Conclusion: We established a method for the quantitative measurement of volume and metabolic status of abdominal VF and SF using simultaneous F-FDG PET/MR. F-FDG PET/MR has an advantage over F-FDG PET/CT in terms of being less confounded by intestinal uptake. This method could be used to assess the inflammatory activity of fat tissue, which is a major risk factor for type 2 diabetes mellitus and cardiovascular disease.

Objectives: In parathyroid adenoma, uptake of technetium-99m-methoxyisobutylisonitrile ((99m)Tc-MIBI) has been suggested to have a correlation with functional markers. The purpose of this study was to evaluate the feasibility of (99m)Tc-MIBI parathyroid single photon emission computed tomography/computed tomography (SPECT/CT) in evaluating the function and disease severity of parathyroid adenoma., Patients and Methods: Twenty-three patients with surgically confirmed parathyroid adenoma were retrospectively enrolled. A parathyroid planar scan and SPECT/CT were performed before parathyroidectomy. Functional and clinical makers reflecting the disease severity of parathyroid adenoma were also evaluated, including serum intact parathyroid hormone, calcium, bone mineral density, and creatinine clearance. The pathologic volume (V(P)) of the adenoma was measured after parathyroidectomy. On parathyroid SPECT/CT, metabolic volume (V(M)) was measured using an isocontour method. Maximum uptakes of parathyroid adenoma and mean uptakes of contralateral thyroid tissue were measured to calculate the parathyroid adenoma-to-background ratio on parathyroid SPECT/CT (PBR(SCT)) and planar scan (PBR(PL))., Results: V(M) significantly correlated with V(P) (r=0.669, P=0.0005). Serum intact parathyroid hormone level significantly correlated with PBR(PL), PBR(SCT), V(M), and V(P) (P=0.0004, 0.005, 0.003, and 0.025, respectively). However, serum calcium level correlated only with V(M) (P=0.012). Regarding the surgical indication criteria, PBR(SCT) and PBR(PL) were significantly higher in the young-aged group (P=0.0004 and 0.024, respectively) and V(M) was significantly higher in the high calcium level group (P=0.049), whereas V(P) was not different between groups on the basis of any criteria., Conclusion: Quantitative indices of parathyroid SPECT/CT closely correlate with laboratory functional markers and disease severity of parathyroid adenoma. Thus, parathyroid SPECT/CT could be used for evaluation of the underlying functional state and disease severity of parathyroid adenoma, particularly for decision pertaining to surgical treatment.

Objectives: Lymph node involvement in cervical cancer is an indication of poor prognosis and the risk tends to increase according to the level of lymph node involvement. However, the specific prognostic significance of retrocrural lymph node involvement has not been well characterized because of its small size and deep location. The aim of this study was to assess its prognostic value., Patients and Methods: A total of 217 patients with newly diagnosed International Federation of Gynecology and Obstetrics (FIGO) stage IA2-IVA cervical cancer were retrospectively enrolled. All the patients had undergone pretreatment (18)F-fluorodeoxy-D-glucose ((18)F-FDG) PET/CT. Of these patients 145 were treated with concurrent chemoradiotherapy, and in this group we assessed the relationship of retrocrural lymph node involvement with the risk of disease progression., Results: Retrocrural lymph node involvement was seen in 7.4% of patients (16/217). All of them had para-aortic lymph node involvement and 56% of the 16 patients (9/16) had concomitant supraclavicular lymph node involvement. In the patients treated with concurrent chemoradiotherapy it was found that the higher the level of (18)F-FDG-positive lymph nodes detected in them, the worse the progression-free survival experienced by them (none vs. pelvic, pelvic vs. para-aortic, para-aortic vs. retrocrural; P<0.05); however, there was no difference in progression outcome between retrocrural and supraclavicular areas (P=NS). On multivariate Cox proportional hazard analysis, the highest level of (18)F-FDG PET/CT-positive lymph nodes in the para-aortic [hazard ratio (HR) 6.05, 95% confidence interval (CI) 2.18-16.81], retrocrural (HR 17.05, 95% CI 5.34-54.44), and supraclavicular areas (HR 19.56, 95% CI 7.15-53.54) was a significant prognostic factor., Conclusion: The highest level of lymph node involvement in para-aortic, retrocrural, supraclavicular areas was a significant prognostic factor for progression in uterine cervical cancer patients who were treated with concurrent chemoradiotherapy. Retrocrural lymph node involvement shows a similar outcome with supraclavicular involvement, but leads to a worse outcome in terms of progression compared with para-aortic lymph node involvement.
