Genial tubercle fracture is a rare mandibular fracture. It is classified into 2 different types based on the mechanism of injury, including type I, associated with an atrophied edentulous mandible or denture, and type II, associated with mandibular fracture. Although type I usually can be treated by only observation, type II may cause obstruction of the pharyngeal airway due to hematoma and edema of the floor of the mouth. In this report, genial tubercle fracture associated with mandibular fracture caused by a traffic accident was reported. Reconstructed 3-dimensional images of the pharyngeal airway indicated that there was almost no airway space around the intratracheal tube at the naso- and oropharynx.

The number of elderly patients with mandibular fracture is rapidly increasing. To improve outcome, it is important to understand the age-related characteristics of mandibular fracture. Thus, the aim of this study is to analyze the impact of atrophic change on mandibular fracture in elderly patients. The retrospective study was conducted in patients aged ≥65 years old, who underwent surgery for the treatment of mandibular fracture in our hospital from March 2006 until March 2015. Patient characteristics, such as age and gender, causes of injury, anatomic location of fracture, height of mandibular body, extent of atrophy, location of surgical sites, postoperative outcomes, and the follow-up period, were examined. Descriptive statistics were compared between atrophic and nonatrophic mandibles. The patients included 17 males and 12 females and the mean age was 71.9 years old. The average follow-up period was 6.06 months. Regarding occlusion and complications, there were no statistical differences between the atrophic and nonatrophic mandibular fractures. As major complications, nonunion occurred in 2 patients and malunion in 1 patient. There was no mortality associated with anesthesia or surgery. Atrophic and nonatrophic mandibular fractures in elderly patients can be treated successfully with surgery. There was no significant difference with respect to major complications between patients with atrophic and nonatrophic mandibular fractures.

Multiple myeloma (MM) is a neoplasia characterized by the proliferation of plasmacytoid cells, which produce abnormal immunoglobulins. It frequently creates local and systemic complications such as pathologic fractures and renal disease. This article reports the case of a 70-year-old man presenting a pathologic fracture of the right mandibular angle secondary to a plasmocytoma. He had a history of a previous pathologic fracture of the left mandibular angle 13 years before that was treated elsewhere, but no documentation was available. Investigation revealed the disease to be a MM and evolution led to renal failure and death. Differential diagnosis is necessary for this kind of pathologic presentation because MM presents a survival rate of 5.7% in 5 years and mandibular involvement happens in the advanced stage of the disease.

Head and neck cancer most often requires a multimodality approach to treatment that includes the use of radiotherapy. Unfortunately, radiation treatment can cause significant locoregional adverse effects with the particularly debilitating sequelae of late pathologic fracture. We aimed to define comprehensive metrics that will allow us to evaluate the efficacy of therapies aimed at assuaging the damaging effects of radiation on fracture healing.Six male rats (XRT/Fx) received a 36-Gy preoperative radiation dose delivered in 10 fractions over 10 days to their left hemimandible, whereas 6 other rats were not irradiated (Fx). After a recovery period, a unilateral osteotomy was performed with external distractor placement set to a 2.1-mm fracture gap on all the animals. Following a 28-day consolidation interval, mandibles were harvested and scanned via micro-computed tomography. Radiomorphometrics were extracted and analyzed with significance at P < 0.05.A quantifiable decrease in bone volume fraction (73.9% vs 78.6%; P < 0.05) was observed in XRT/Fx compared with Fx, without significant change in bone mineral density. Microarchitectural metrics were significantly altered in XRT/Fx compared with Fx, specifically trabecular thickness (0.37 vs 0.30 µm; P = 0.01), trabecular number (2.18 vs 2.45 N/mm; P = 0.04), and bone surface-bone volume ratio (5.50 vs 6.70; P = 0.01).We found that the irradiated and fractured bone demonstrated a significant diminution in bone quality and strength. We have established predictable and reliable radiographic measures that quantitatively demonstrate the degradative effects of radiation on fracture repair. We can now utilize these comprehensive metrics to evaluate the benefits of therapeutic interventions to remediate the damaging effects of radiation on fracture healing.

Management of advanced bisphosphonate-related osteonecrosis of the jaw, particularly in cases of pathological fractures with extraoral fistulas, is a challenging responsibility for craniomaxillofacial specialists. Although a periosteal reaction is occasionally observed in cases with extensive sequestra, few reports have documented circumferential periosteal osseous formation resulting in bone repair. We present the case of a 78-year-old woman who was treated for breast cancer and received intravenous bisphosphonates. She had bisphosphonate-related osteonecrosis of the jaw with bilateral pathological fractures of the mandible. During the course of minimally invasive treatment, the necrotic segment was separated with circumferential exposure, and new bone spontaneously formed with osseous union to the unilateral fractured stump. The present case typically highlights the potential of periosteal osteogenesis to regain an acceptable state even for patients with severe "bis-phossy jaw," particularly those with pathological fractures, although we do not advocate the universality of the phenomenon presented as "phoenix jaw."

The number of the suicides is increasing all around the world. In this study, the cases autopsied between 2000 and 2007 in The Konya Branch of Forensic Medicine Council were retrospectively investigated. Fifty-seven shotgun suicides were determined. The cases were evaluated in terms of their demographic characters, site of entrance wound, the type of the cartridge used, shooting distance, the place of incident, the place of death, motive for suicide, and the presence of previous suicide attempts. The most preferred site of entrance wound was the head with 34 cases (59.7%). Of 34 cases in which the entrance wound was on the head, there was indirect mandibular fracture in 16 cases (47.1%). In 11 cases (19.3%), there observed small ecchymosed abrasions on the finger surfaces, which are thought to have resulted from the trigger kicking back during triggering or the finger having been stuck between the trigger and the trigger guard. It is concluded that there is a need for a legal regulation that makes obtaining of shotguns more difficult and the people who have them are to be educated not to keep them in easily accessible places.

The aim of this study was to evaluate the effect of zoledronic acid (ZA) in the ovariectomized rat on mandibular fracture. Corpus osteotomies were performed on the mandible of ovariectomized rats. Miniplates were placed on these rats, which were randomly divided into 4 groups. After 7 days, groups 3 and 4 were injected with 0.1 mg/kg ZA. Groups 1 and 2 were control groups, so they were injected 0.1 mg/kg saline solution. Animals in groups 1 and 3 were killed at 4 weeks, whereas groups 2 and 4 were killed 8 weeks after operation. The new bone volume of the fracture gap was examined stereologically. Although the difference was not statistically significant compared with the other groups (groups 1 and 3), more bone formation was found in ZA-treated groups (groups 2 and 4) (P < 0.05).

Pontomedullary lacerations (PMLs) have often been reported in car occupants and pedestrians, are less frequently described in motorcyclists, and are very rarely described in bicyclists. The aim of this study was to determine the frequency of brainstem PMLs among fatally injured motorcyclists and bicyclists as well as the frequency of concomitant cranial, facial, and cervical spine injuries in such cases. A possible underlying mechanism of PML in fatally injured motorcyclists and bicyclists might thus be established. Of 443 cases of fatally injured motorcyclists and bicyclists, a sample of 381 cases of fatally injured motorcyclists and bicyclists with head injury of Abbreviated Injury Scale score of 3 or greater was formed and further analyzed. This group was composed of 345 men and 36 women. The average age was 48.8 ± 20.8 years (range, 15-99 years). In the analyzed sample group, there were 158 motorcyclists and 223 bicyclists. Partial PMLs were present in 44 cases (12%) within the sample of 381 head injuries, which breaks down to 40 men and 4 women. In our study, the impact area on the head and the specific skull base fracture type were good predictors of either PML occurrence or absence (B = -2.036, Wald = 161.312, P < 0.01, for the whole model). Impact to the chin, with or without a skull base fracture, most often led to this fatal injury due to impact force transmission, either through jawbone or vertebral column. Also, lateral head impact, the most frequent in bicyclists, with subsequent hinge fracture, PML, and frontoposterior hyperextension of the head that is associated with upper spine fracture, could be possible mechanisms of brainstem injury in fatally injured motorcyclists or bicyclists. Our study showed that the jawbone, as well as other facial bones, could act as shock absorbers, and their fracture could diminish energy transfer toward the skull and protect the brain and brainstem from injury.

Objective: The objective of the study was to report a 61-year-old man who presented a complication after mandibular follicular cyst removal., Methods: The patient underwent surgery, via intraoral approach: removal of the lesion and the dental follicle with curettage and extraction of the mandibular right third molar. Two weeks after surgery, the patient reported a slight malocclusion. The x-ray showed a fracture near the right mandibular angle. The fracture was treated by application of a long mandibular plate., Results: Two years after surgery, the patient is asymptomatic. Inferior alveolar nerve sensitivity returned completely., Conclusions: In the reported case, a satisfactory result was obtained after the treatment of the complication confirmed by postoperative x-rays.

Background: One of major limitations of applying distraction osteogenesis for craniofacial skeleton hypoplasia is the long duration for bony consolidation. This study investigated whether extracorporeal shock wave therapy (ESWT) could accelerate bony consolidation in distraction osteogenesis of the rat mandible., Methods and Materials: An L-shape osteotomy was performed unilaterally in the hemimandible of Sprague-Dawley rats. An internal distracter was employed to create a 7-mm distraction gap in the mandible. One hundred twenty rats were divided into three groups. The distraction zone of the mandible was received no treatment as controls (group I). Group II received ESWT (500 impulses at 14 kV) at 2 weeks postoperatively. Group III treated with 500 ESW impulses at 21 kV at 2 weeks postoperatively., Results: Dual-energy X-ray absorptiometry and material testing showed that optimal dosage of ESWT in group II significantly increased mineral density and enhanced biomechanical strength of the bone tissue. In histomorphological analysis, the mandibular tissue in group II showed intense osteoblastic cell recruitment, new bone formation, and vascularization. The osteoblasts in the distracted zone in group II indicated significantly strong immunoreactivities for proliferating cell nuclear antigen, vascular endothelial growth factor, and bone morphogenetic protein-2, when compared with other groups., Conclusion: Optimal dosage of ESWT was beneficial for accelerating facial skeleton consolidation and bone regeneration in the distracted rat mandible tissue. The mechanism was presumably associated with the up-regulation of neovascularization, cell proliferation, and osteogenic growth factor expression in bone microenvironment.

The authors report a rare case of mandibular fracture caused by a flying object, discuss the mechanism of the fracture, and review the literature. The patient was a 40-year-old male soldier in the Self Defense Force (SDF). During a mock battle of the SDF Agency, a shell splinter penetrated his mentum and caused a fracture of the mandibular symphysis that resembled horizontal osteotomy for genioplasty. A horizontal fracture of the mandibular symphysis is very rare, and on clinical inspection, the authors found the general course of the clinical fracture line coincided with the weakest portion of the mandible. A powerful blast can cause an object to become airborne and injure persons in the vicinity, as did the metallic fragments that caused the mandibular fracture in patient reported here. Thus, when examining victims of such traumas, the possibility that a foreign object might have penetrated the body should be kept in mind. A thorough inspection of injuries resulting from explosions is mandatory.

The aim of this study was to attempt to establish a correlation between condylar localized fracture, onset of anatomicopathological lesions of the articular structures, and determination of ankylosis so as to define appropriate diagnostic and therapeutic procedures. Clinical, arthroscopic, and histological studies were carried out in two patients with a severe temporomandibular joint disorder after a condylar localized fracture. Treatment included removal of the displaced fragments, condylar surface remodeling, suture of retrodiskal perforations, and diskal repositioning. The histological study of the condylar specimens revealed signs of osteoarthrosis of the articular surface and chondroid metaplasia of the bilaminar zone with early onset of ankylosis. Results of this study confirmed the presence of causal relations between condylar localized fracture and ankylosis with alterations in retrodiskal tissue. These relations are found in 1) tissue damage caused by bony fragments remaining in the articular cavity (underestimated and poorly treated fractures), 2) the lesion that occurs in association with mechanical trauma; and, particularly, c) the disk and retrodiskal impairments caused by catabolic and degenerative osteoarthrotic changes secondary to condylar marrow damage. Thus, timeliness and accuracy of the diagnosis of condylar localized fracture are most important, as are correct diagnosis and treatment before the onset of degenerative anatomicopathological lesions.

Rapid consolidation of osseous fractures requires stable immobilization of the fragments. In the atrophied mandible, the nutrient supply should be ensured by protecting the periosseous soft tissue, i.e., the periosteum. In the conventional placement of osteosynthesis plates, however, the subimplant nutrition is restricted because of the high surface pressure between the osteosynthesis plate and bone. In order to improve the subimplant nutrient supply, we designed a new osteosynthesis plate with a "knobbed" underside enabling multipoint contact. Histomorphologic comparative studies were conducted on 16 Göttingen minipigs with an average weight of 47.5 +/- 8 kg. In each minipig, one side of the mandible was fractured and one side was left intact. One side of each mandible was plated with a six-hole dynamic multipoint contact plate and the other side with a conventional plate. The plates were applied, either epiosseous or epiperiosteal, with bicortical screws under defined torque. The results demonstrate the advantages of multipoint contact plates over the conventional ones and create the clinical basis for a "bio-logical" plate design that allows callus to grow between the knobs and therefore provide adequate nutritional and vascular supply. The plate's knobbed profile also promotes extraosseous venous drainage without impeding intraosseous fluid flow. This plate is the logical choice for rapid fracture consolidation in the high-grade atrophied mandible.

Safety belts reduce the frequency and severity of injuries sustained in motor vehicle crashes. Since the head and face are the most frequently injured anatomic regions in motor vehicle crash victims, the use of active restraints should lessen these injuries. This study was undertaken to examine the ability of safety belts to prevent and alter the pattern of maxillofacial injuries. During a 6-month period beginning February 1, 1987, 613 motor vehicle crash victims presented alive to the Trauma Center at the University of Tennessee Medical Center at Knoxville and were entered into this study. There were 254 restrained victims (R), 290 unrestrained (UR), and 69 who were excluded for lack of sufficient data regarding restraint usage. Unrestrained victims tended to be younger (mean age: UR = 27.9, R = 31.7; p less than 0.03), and more often male (UR = 65% vs. R = 51%; p less than 0.03). Irrespective of position within the vehicle, the mean number of facial lacerations per person was less in the restrained group (R = 0.31, UR = 0.76; p less than 0.03). This injury reduction was more pronounced in front seat occupants (R = 0.27, UR = 0.82; p less than 0.03). Furthermore, a smaller proportion of the lacerations in the restrained group were categorized as complex, i.e., involving more than skin and subcutaneous tissue (R = 5.0%, UR = 15.9%; p less than 0.03).
