The objective was to evaluate the incidence and degree of double-J ureteral stent (DJUS) migration. Additionally, we aimed to investigate the risk factors associated with stent migration in the orthotopic neobladder group. In this retrospective study, 61 consecutive patients were included; 35 patients (45 DJUS placements) underwent radical cystectomy with orthotopic neobladder and 26 patients (35 DJUS placements) underwent urinary bladder without cystectomy between July 2021 and March 2023. All the patients were treated with a DJUS for ureteric strictures. The technical success rate was 100% in each group. The DJUS migration was significantly higher in the orthotopic neobladder group, with 22 of 45 cases (48.9%), compared to the urinary bladder group, which had 4 of 35 cases (11.4%) (P ≤ .001). Among the patients in the orthotopic neobladder group who experienced DJUS migration, stent dysfunction occurred in 18 cases (81.8%), which was statistically significant (P = .003). Multivariate logistic regression analysis revealed that only the size of the DJUS was significantly and positively associated with migration (odds ratio:10.214, P = .010). To evaluate the technical feasibility of the alternative snare technique using a 0.018-inch guide wire and 5-French (Fr) catheter for double-J ureteral stent (DJUS) removal. In this retrospective study, 11 DJUS were removed in 9 consecutive patients between July 2023 and October 2023. We evaluated patient characteristics, DJUS characteristics, and procedure characteristics. Out of 11 cases, 8 (72.7%) were successful in removing the DJUS using the alternative snare technique without major complications. The average time between DJUS insertion and removal was 47.4 ± 50.0 days. The most common DJUS size was an 8-Fr, with proximal tips predominantly in the proximal ureter and renal pelvis. The mean procedure time for successful cases was 15.2 ± 16.8 minutes. Three failed cases, attributed to obstructions like debris, were later successfully addressed using the ALN inferior vena cava filter removal kit, forceps, and modified snare technique. To evaluate the success rates of percutaneous drainage for fluid collection after radical cystectomy, with emphasis on factors affecting the clinical success, including lesion, patient, and procedure characteristics. In this retrospective study, 31 percutaneous drainage catheters were placed in 29 consecutive patients between January 2021 and September 2023. Most fluid collections formed near the uretero-ileal anastomosis site in the right pelvic cavity (80.6%). The technical success rate was 100%. The primary and final clinical success was 80.6% and 96.9%, respectively. Lymphoceles notably increased the primary clinical failure risk (odds ratio and 95% confidential interval: 22.667 (1.839-279.366), P = .015). Significant differences were observed between transabdominal and transgluteal approaches in terms of fluoroscopic time, dose, and location. Leakage indications on computed tomography prompted differing interventions, but all achieved final clinical success. Rationale: Sessile serrated lesions (SSLs) are serrated polyps (SP) with the typical serrated architecture of the crypt lining epithelium. SSL has an important clinical implication because they are recognized as precursor lesion of sporadic colorectal cancer (CRC) through "serrated pathway." SSLs usually appear flat to sessile, and are located in the right colon., Patient Concerns: A 69-year-old man was referred to a tertiary medical center because of intermittent hematochezia for 2 years., Diagnosis: Colonoscopy revealed a large, pedunculated polyp in the rectum. The polyp surface was slightly reddish in color and the elongated stalk was covered with almost normal mucosa. Histopathological examination of the resected specimens revealed the typical features of SSL with low-grade dysplasia., Intervention: Endoscopic mucosal resection using a detachable snare was performed on the tumor for definite diagnosis and treatment., Outcomes: There was no evidence of immediate or delayed bleeding after endoscopic mucosal resection, and the hemoglobin level normalized after a 1-year follow-up., Lessons: We report a rare case of a large pedunculated polyp with typical histological features of SSLs in the rectum. Rationale: Uterine arteriovenous malformation (UVM), which can be congenital or acquired, is a relatively rare disorder that can cause life-threatening hemorrhage. Acquired UVM occurs predominantly after previous uterine procedures; rarely, it may occur after a hysterectomy. Although the best treatment option for UVM remains controversial, transcatheter arterial embolization (TAE) has recently been introduced as a safe and effective treatment., Patient Concerns: A 34-year-old woman who underwent hysterectomy for uncontrolled postpartum bleeding continued to have hemoperitoneum., Diagnosis: Two days after surgery, massive hemoperitoneum was identified on computed tomography scan, and acquired UVM was diagnosed by angiography., Interventions: The patient was successfully treated using TAE with an n-Butyl cyanoacrylate., Outcomes: After embolization, hemodynamic stability was achieved. A day after embolization, hemoglobin was 10.2 g/dL, and the patient was discharged from the hospital 4 days thereafter., Lessons: Although the overall incidence of acquired UVM after hysterectomy is low, bleeding from acquired UVM should be considered as one of the differential diagnoses in the immediate postpartum period, especially when the clinical symptoms do not correlate with the amount of blood loss. Introduction: In blunt traumatic superficial femoral arterial (SFA) injuries, hemorrhage from the branches without injury to the main artery is rare, but can lead to serious complications, such as compartment syndrome affecting the clinical outcomes. Although open surgical repair has been the standard approach to peripheral vascular injuries, endovascular treatment has become more refined and is now an alternative to open surgery, which potentially involves lower morbidity and mortality rates. However, management of arterial injuries, especially when they involve simple bleeding from small branches of the main artery, can be challenging, and the best treatment options for such injuries remains controversial., Patient Concerns: Three cases suffered blunt trauma that resulted in hemorrhage from branches of the SFA., Diagnosis: All patients underwent selective angiography, which demonstrated active extravasation from branches of the SFA., Interventions: All patients were treated using embolization with n-butyl cyanoacrylate (NBCA)., Outcomes: A post-embolization angiography demonstrated successful hemostasis, with no complications., Conclusion: Superselective catheterization using a coaxial technique with a 5-F curved catheter and the smallest caliber microcatheter, and using a permanent liquid embolic agent, such as NBCA, increases the success rate of embolization for cases of hemorrhage from SFA branches.

This study aimed to analyze the dose of radiation to which the physician is exposed during endoscopic retrograde cholangiopancreatography (ERCP) and to identify predictive factors of radiation exposure during the procedure. Furthermore, we evaluated the patient characteristics and procedural factors associated with prolonged fluoroscopy time (FT).A cross-sectional retrospective analysis of 780 ERCPs performed at a tertiary academic hospital over a 2-year period was conducted. The primary outcome was radiation exposure during ERCP as determined by FT; additionally, the association between variables and radiation exposure was determined. Moreover, we evaluated their correlations with age, sex, body mass index (BMI), diagnosis, duration of procedure, procedure name, and procedure complexity.According to the analysis of the 780 ERCPs performed in 2 years, the mean FT was 5.07 minutes (95% confidence interval [CI], 4.87-5.26). The mean radiation durations were as follows: cholelithiasis, 5.76 minutes (95% CI, 4.75-6.80); malignant biliary obstruction, 6.13 minutes (95% CI, 5.91-6.35); pancreatic disease, 5.28 minutes (95% CI, 4.45-6.28); and benign biliary stricture, 5.32 minutes (95% CI, 5.02-5.94). Significant differences affecting fluoroscopy duration between the 2 endoscopists were not observed in the present study. Multivariate analysis revealed that prolonged fluoroscopy duration was related to specific characteristics, including higher BMI (BMI >27.5 kg/m) (+4.1 minutes; 95% CI, 2.56-5.63), mechanical lithotripsy (+4.85 minutes; 95% CI, 0.45-9.25), needle-knife use (+4.5 minutes; 95% CI, 2.15-6.86), and malignant biliary obstruction (+2.34 minutes; 95% CI, 0.15-4.53).ERCPs are associated with significantly higher radiation exposure of patients on the specific procedure. Endoscopists should be aware of the determining factors, including patients with obesity, who underwent mechanical lithotripsy, who had malignant biliary obstruction, and who underwent a procedure using a needle knife, that affect FT during ERCP.

NADPH oxidase (NOX) is a key source of reactive oxygen species (ROS). This study aimed to verify NOX2 and NOX4 expression levels in hepatocellular carcinoma (HCC). A total of 134 matched pairs of HCC cells and non-tumour hepatocytes from 134 patients were examined by immunohistochemical staining, and the association of NOX2 and NOX4 expression with clinicopathological parameters was analysed. Western blotting in four HCC cell lines and reverse transcription digital droplet polymerase chain reaction (RT-ddPCR) in 20 pairs of HCC and non-tumour tissue samples were also performed to detect NOX4. Cytoplasmic NOX2 and nuclear NOX4 expression levels were shown by immunohistochemistry to be higher in HCC cells than in non-tumour hepatocytes (p<0.001 each). The western blotting results for NOX4 in four HCC cell lines were consistent with the immunohistochemical results. Increased cytoplasmic expression of NOX2 and NOX4 in HCC cells was significantly correlated with liver cirrhosis (p<0.001 and p<0.031, respectively). However, decreased cytoplasmic expression of NOX2 and NOX4 was significantly correlated with advanced pathological TNM stage (p<0.029 and p<0.007, respectively). Multivariate analysis with clinicopathological parameters showed that high nuclear and low cytoplasmic NOX4 expression levels are correlated with short overall survival (p=0 .021). Our findings imply that cytoplasmic NOX2 and nuclear NOX4 expression is upregulated during HCC development. In particular, NOX4 translocation into the nucleus may affect the development and progression of HCC. Study Design: A retrospective study., Objective: To analyze adjacent segment degeneration (ASD) in 2-level anterior cervical discectomy, comparing fusion with stand-alone cages [anterior cervical discectomy and fusion (ACDF)-CA] and fusion with cage and plate constructs (ACDF-CPC) with respect to clinical outcomes and radiologic changes., Summary of Background Data: ACDF using a stand-alone cage or a cage and plate construct is a popular procedure. However, there is lack of knowledge concerning ASD between the 2 procedures., Methods: A total of 42 consecutive patients who underwent 2-level ACDF-CA or ACDF-CPC for 2-level cervical disk disease and who completed 2 years of follow-up were included in this study. The patients were divided into 2 groups: ACDF-CA group (n=22) and ACDF-CPC group (n=20). The following parameters were assessed using radiographs: disk space narrowing, anterior osteophyte formation, calcification of the anterior longitudinal ligament, and fusion status. Clinical outcomes were assessed using the Robinson criteria., Results: No difference in clinical outcomes was observed between the 2 groups. Moreover, the ACDF-CPC group showed a similar fusion rate compared with the ACDF-CA group (100% vs. 95%, P=0.335). There was also no statistical significance in anterior osteophyte formation and calcification of the anterior longitudinal ligament. However, mean intervertebral disk height change of an adjacent segment was significantly lower in the ACDF-CA group than the ACDF-CPC group (upper level: 0.08±0.24 vs. 0.49±0.35; lower level: 0.06±0.41 vs. 0.49±0.28; P<0.01)., Conclusions: The use of a cage with or without plate constructs in 2-level ACDF provides similar clinical results and fusion rates. Notwithstanding, ACDF-CPC showed a higher incidence of ASD than ACDF-CA over the 2-year follow-up.

The aim of this study was to see the histological nature of the alar web and to introduce a featheredged rolled-in flap to reduce the alar web.On a cadaver, the perpendicular section of the alar web revealed a thickened dermis portion on both the skin side and the nasal side distal to the alar cartilage. According to histological results, we thought the thinning and rolling in of the distal margin of the end of the open rhinoplasty incision could reduce the alar web. An open rhinoplasty incision was made just distal to the hair-bearing vestibular skin and a V-Y shape incision created at the alar base. After the cartilage work, the skin of the distal end of the flap was featheredged to a 0.5-mm thickness. The distal margin was rolled in and sutured to the nasal lining. A bolster dressing was applied using a 4-0 nylon suture.Thirteen patients (8 males, 5 females) were operated on, and 8 patients were followed up for more than 12 months. Their preoperative and postoperative worm's eye views were compared. Four anthropometric distances were measured preoperatively and postoperatively. The columellar length increased significantly after the operation (P = 0.001 [independent 2-sample t test]) on the cleft side. Preoperatively, the alar width was significantly greater (P = 0.02 [paired-samples t test]) on the cleft side (0.17 ± 0.03 of an intercanthal distance) than the noncleft side (0.14 ± 0.03). After the operations, they became similar (0.16 ± 0.03 on cleft side, 0.16 ± 0.04 on the noncleft side; P = 1.00 [paired-samples t test]).We think this featheredged rolling-in flap might be a good method for the correction of an alar web since this technique increased the columellar length and decreased the alar width on the cleft side.

The aim of this study was to introduce a safe ostectomy method for the correction of prominent mandibular angles via a "burring and holes connecting technique." A subperiosteal dissection was conducted, and the mandibular angle was exposed. Using a 7-mm-diameter burr, 3 to 5 holes were made on the predicted ostectomy line. The consecutive holes were connected using an oscillating saw. A bone fragment was removed, and irregularities were smoothened through light burring. Twenty-three patients (2 men, 21 women; mean age, 28.4 y) were operated on. The mean trace observation period was 11 months. Among the 23 patients, no subcondylar fracture or hematoma occurred. In x-ray comparisons, there was no excessive or insufficient reduction of the mandibular angle. We think this burring and holes connecting ostectomy might be a safe method for the correction of prominent mandible angles even for inexperienced surgeons.

Study Design: A retrospective review series., Objective: To identify vascular anatomy and esophageal deviations that may interfere with a percutaneous anterior approach in cervical spine surgery., Summary of Background Data: The percutaneous anterior approach has been used for minimally invasive interventions of cervical disc diseases. Although the percutaneous anterior approach is frequently performed, reports of obstructing anatomical structures and procedural risks are limited., Methods: Cervical magnetic resonance images obtained from December 2012 to April 2013 from a total of 511 patients at Guro Teun Teun Hospital were evaluated in this study. Each axial T2-weighted MR image from the disc levels of C3-C4 to C6-C7 (total, 3066 images) was reviewed to check for the presence of small vessels along the trajectories of percutaneous cervical procedures on the left and right sides. Esophageal deviation was also measured at level C6-C7., Results: Small vessels in the anterior neck were present, respectively, in 50.5% (trajectory on the left side) and in 49.1% (trajectory on the right side) at disc level C3-C4, in 30.3% and 28.8% at C4-C5, in 24.1% and 7.6% at C5-C6, and in 55.2% and 43.1% at C6-C7. There were no differences in the number of small vessels between the left and right sides at the upper cervical level (C3-C4-C5), but small vessels were less frequently observed on the right side at lower cervical levels (C5-C6-C7). Differences in esophageal deviation were also observed, with less deviation to the right side (0.63 ± 0.35 cm) than the left (1.18 ± 0.52 cm). As well, an esophageal diverticulum was observed in 1 case on the left side., Conclusion: Although surgical approaches are mostly concerned with the location of pathology to be removed, a right-sided percutaneous approach seems preferable because poses less of a risk of encountering small vessels and causing esophageal injury., Level of Evidence: 4.

Study Design: Prospective study comparing the somatometric measurements of young males with normal spinal curves and with adolescent idiopathic scoliosis (AIS) with respect to the severity of AIS., Objective: To assess the somatometric measurements of young males with normal spinal curves and with AIS using the conscription data., Summary of Background Data: The progression of AIS is closely correlated with longitudinal growth during puberty. Although abnormal growth in female AIS patients has been well investigated, the growth of male AIS patients has not been well reported., Methods: During Korean conscription, 409 adolescent males with a normal spinal curvature and 420 adolescent males with AIS were enrolled. Those with AIS were grouped according to the severity of scoliosis using Cobb angles, according to the guidelines issued by the Korean military directorate. Group somatometric measurements, such as body height, corrected body height (corrected using Bjure's equation), body weight, and body mass index (BMI) were compared., Results: Uncorrected heights were insignificantly different (P=0.234), but corrected heights, body weights, and BMIs were all significantly different between the normal and all AIS groups (P<0.001) (in AIS, corrected height was greater and body weight and BMIs corrected or uncorrected for height were lower). Cobb angles were not related to corrected body height or BMI, but was related to weight. Body weight was significantly less in the severe AIS group (Cobb angle >40 degrees) than in the mild or moderate AIS group (P<0.042)., Conclusions: In Korean male AIS patients, significantly different somatometric results were observed: namely, a greater corrected height and a lower body weight and BMIs corrected or uncorrected for height. Furthermore, body weight was significantly lower in the severe group than in the moderate group. This study shows that abnormal growth is observed in male AIS patients and that body weight is closely correlated with AIS severity.

Study Design: Case report with literature review., Objective: To describe a rare case of intradural spinal metastasis from renal cell carcinoma (RCC) spread to the cauda equina, and to discuss the clinical features of metastatic RCC in the cauda equina from the data available in the literature., Summary of Background Data: Intradural spinal metastasis is rare, representing 6% of all spinal metastasis. Indeed, intradural metastasis from a RCC to the cauda equina is extremely rare with previously only 9 case reports., Methods: A 68-year-old male presented with a 2-month history of worsening lower back pain radiating to both legs. The patient had undergone nephrectomy for the treatment of the clear cell RCC 16 years before admission. Magnetic resonance imaging showed a well-defined intradural extramedullary mass in the cauda equina at T12 to L1., Results: The pathological examination displayed metastatic clear cell RCC. Additional imaging studies showed no metastatic in other locations. The patient was discharged without neurological deficit and pain after the operation, and maintained an optimal condition for 2 years., Conclusion: When a lesion of the cauda equina presents, intradural metastasis should be in the differential diagnosis in patients who had been previously treated for RCC although any other metastatic lesion was not observed., Level of Evidence: N/A.

Basal cell carcinoma is the most common of all skin cancers and the most prevalent one among Caucasians. Rarely, these tumors are seen in other races. We report a 77-year-old Korean woman who presented with multiple darkly pigmented enlarging nodules on her scalp, face, trunk, and extremities. The patient had first noted a 6-mm pigmented lesion on her left eyebrow 10 years previously. Since then, other lesions had appeared in many locations on her body. She had been otherwise healthy and without a history of exposure to arsenic or radiation. There was no family history of skin cancer, xeroderma pigmentosum, or basal cell nevus syndrome. On physical examination, multiple darkly pigmented dome-shaped papules and nodules were present on her scalp, face, right forearm, lower abdomen, and inguinal areas. They ranged in size from 0.5 mm to 2 cm. The larger ones showed central ulceration. Multiple biopsy specimens from different sites showed pigmented basal cell carcinomas. Clinically, there was no evidence of nevus sebaceus, xeroderma pigmentosum, basal cell nevus syndrome, or immunodeficiency. Clinical workup including chest radiography, abdominal ultrasound, bone scan, and brain computerized axial tomography scan did not demonstrate primary or secondary tumors. The results of serologic and hematologic tests were also within normal limits. This is an unusual case report of multiple pigmented basal cell carcinomas in an Asian woman without any predisposing risk factors.

A 25-year-old man had a 5-year history of multiple tender lesions of both extremities and trunk. Biopsy found an encapsulated subcutaneous nodule composed of numerous small, well-formed blood vessels, some filled with fibrin thrombi. This lesion was made up > 99.9% of blood vessels with only a minimal amount of adipose tissue. We believe our case is in the spectrum of angiolipoma even though it is made up almost entirely of a vascular component. These tumors may be confused with other multiple or eruptive vascular lesions that have a more ominous clinical course. This case is being reported to stress that multiple or eruptive "vascular" lesions do not always reflect an underlying immunosuppression or malignancy.

We used clinical criteria to study skin biopsy specimens with mild to moderate photoaging taken from the face and dorsal forearms of 74 Caucasian volunteers between the ages of 30 and 50. Facial skin had a greater number of granular cell layers, a higher degree of keratinocytic atypia, and more often showed a compact stratum corneum than arm skin. Furthermore, the dermis of facial skin had a more extensive perivascular and perifollicular lymphocytic infiltrate, more perifollicular fibrosis, a greater number of mast cells and melanophages, and thinner vascular walls than forearm skin. This study demonstrated that the photoaging process is different for face and arm skin. Appreciation of these differences should permit more refined studies of photoaging and the development of more efficient therapies.

Multiecho magnetic resonance imaging of 23Na in the human body could provide valuable information regarding the distribution of the sodium in the intracellular and extracellular spaces in living tissues. Since it is known that more than half the intracellular sodium has a T2 value of less than 3 ms, the conventional spin-echo imaging technique is not appropriate for the quantitative study of sodium in living tissues. A pulse sequence using the three-dimensional planar-integral projection reconstruction algorithm was developed to provide images from the free induction decay (FID) signal and from the spin-echo. Using this imaging sequence, we are able to produce human head images with the FID signal immediately following the 90 degrees radiofrequency pulse thus preserving the signal from the very short T2 components. This technique with a multiple echo imaging scheme may open the possibility of discriminating between intracellular and extracellular sodium compartments in the human brain based on the T2 relaxation properties.
