Understanding how global brain networks are affected in epilepsy may elucidate the pathogenesis of seizures and its accompanying neurobehavioral comorbidities. We investigated functional changes within neural networks in temporal lobe epilepsy (TLE) using graph theory analysis of resting-state connectivity. Twenty-seven TLE presurgical patients (age 41.0 ± 12.3 years) and 85 age, gender, and handedness equivalent healthy controls (HCs; age 39.7 ± 16.9 years) were enrolled. Eyes-closed resting-state functional magnetic resonance image scans were analyzed to compare network properties and functional connectivity (FC) changes. TLE subjects showed significantly higher global efficiency, lower clustering coefficient ratio, and lower shortest path lengths ratio than HCs, as an indication of a more synchronized, yet less segregated network. A trend of functional reorganization with a shift of network hubs to the contralateral hemisphere was noted in TLE subjects. Support vector machine (SVM) with linear kernel was trained to separate between neural networks in TLE and HC subjects based on graph measurements. SVM analysis allowed separation between TLE and HC networks with 80.66% accuracy using eight features of graph measurements. Support vector regression (SVR) was used to predict neurocognitive performance from graph metrics. An SVR linear predictor showed discriminative prediction accuracy for four key neurocognitive variables in TLE (absolute R value range: 0.61–0.75). Despite TLE, our results showed both local and global network topology differences that reflect widespread alterations in FC in TLE. Network differences are discriminative between TLE and HCs using data-driven analysis and predicted severity of neurocognitive sequelae in our cohort.

The National Institutes of Health-sponsored Epilepsy Connectome Project aims to characterize connectivity changes in temporal lobe epilepsy (TLE) patients. The magnetic resonance imaging protocol follows that used in the Human Connectome Project, and includes 20 min of resting-state functional magnetic resonance imaging acquired at 3T using 8-band multiband imaging. Glasser parcellation atlas was combined with the FreeSurfer subcortical regions to generate resting-state functional connectivity (RSFC), amplitude of low-frequency fluctuations (ALFFs), and fractional ALFF measures. Seven different frequency ranges such as Slow-5 (0.01–0.027 Hz) and Slow-4 (0.027–0.073 Hz) were selected to compute these measures. The goal was to train machine learning classification models to discriminate TLE patients from healthy controls, and to determine which combination of the resting state measure and frequency range produced the best classification model. The samples included age- and gender-matched groups of 60 TLE patients and 59 healthy controls. Three traditional machine learning models were trained: support vector machine, linear discriminant analysis, and naive Bayes classifier. The highest classification accuracy was obtained using RSFC measures in the Slow-4 + 5 band (0.01–0.073 Hz) as features. Leave-one-out cross-validation accuracies were ∼83%, with receiver operating characteristic area-under-the-curve reaching close to 90%. Increased connectivity from right area posterior 9-46v in TLE patients contributed to the high accuracies. With increased sample sizes in the near future, better machine learning models will be trained not only to aid the diagnosis of TLE, but also as a tool to understand this brain disorder.

The Epilepsy Connectome Project examines the differences in connectomes between temporal lobe epilepsy (TLE) patients and healthy controls. Using these data, the effective connectivity of the default mode network (DMN) in patients with left TLE compared with healthy controls was investigated using spectral dynamic causal modeling (spDCM) of resting-state functional magnetic resonance imaging data. Group comparisons were made using two parametric empirical Bayes (PEB) models. The first level of each PEB model consisted of each participant's spDCM. Two different second-level models were constructed: the first comparing effective connectivity of the groups directly and the second using the Rey Auditory Verbal Learning Test (RAVLT) delayed free recall index as a covariate at the second level to assess effective connectivity controlling for the poor memory performance of left TLE patients. After an automated search over the nested parameter space and thresholding parameters at 95% posterior probability, both models revealed numerous connections in the DMN, which lead to inhibition of the left hippocampal formation. Left hippocampal formation inhibition may be an inherent result of the left temporal epileptogenic focus as memory differences were controlled for in one model and the same connections remained. An excitatory connection from the posterior cingulate cortex to the medial prefrontal cortex was found to be concomitant with left hippocampal formation inhibition in TLE patients when including RAVLT delayed free recall at the second level.

Post-stroke neuropsychological evaluation is time-intensive in assessing impairments in subjects without overt clinical deficits. We utilized functional connectivity (FC) from ten-minute non-invasive resting-state functional MRI (rs-fMRI) to identify stroke subjects at risk for subclinical language deficit (SLD) using machine learning. Discriminative ability of FC derived from slow-5 (0.01–0.027 Hz), slow-4 (0.027–0.073 Hz) and low frequency oscillations (LFO; 0.01–0.1 Hz) was compared. Sixty clinically non-aphasic right-handed subjects were categorized into three subgroups based on stroke status and normalized verbal fluency (NVF) score: 20 ischemic early-stage stroke subjects at higher risk for SLD (LD+; mean VFS=−1.77), 20 ischemic early-stage stroke subjects with at risk for SLD (LD-; mean VFS=−0.05), 20 healthy controls (HC; mean VFS=0.29). T1-weighted and rs-fMRI were acquired within 30 days of stroke onset. Blood-oxygen-level-dependent signal was extracted within the language network. FC was evaluated and used by a multiclass support vector machine to classify test subject into a subgroup which was assessed by nested leave-one-out cross-validation. FC derived from slow-4 (70%) provided the best accuracy relative to LFO (65%) and slow-5 (50%), reasonably higher than random chance (33.33%). Using subgroup-specific accuracy, classification was best realized within slow-4 for LD+ (81.6%) and LD- (78.3%) and slow-4/LFO for HC (80%), i.e., early-stage stroke subjects showed a slow-4 FC dominance whereas HC also indicated the normalized involvement within LFO. While frontal FC differentiated stroke from healthy, occipital FC differentiated between the two stroke subgroups. Thus, stroke subjects at risk for SLD can be identified using rs-fMRI reasonably in an expedited manner.

The utility and success of resting-state functional connectivity MRI (rs-fcMRI) depend critically on the reliability of this technique and the extent to which it accurately reflects neuronal function. One challenge is that rs-fcMRI is influenced by various sources of noise, particularly cardiac- and respiratory-related signal variations. The goal of the current study was to evaluate the impact of various physiological noise correction techniques, specifically those that use independent cardiac and respiration measures, on the test-retest reliability of rs-fcMRI. A group of 25 subjects were each scanned at three time points--two within the same imaging session and another 2-3 months later. Physiological noise corrections accounted for significant variance, particularly in blood vessels, sagittal sinus, cerebrospinal fluid, and gray matter. The fraction of variance explained by each of these corrections was highly similar within subjects between sessions, but variable between subjects. Physiological corrections generally reduced intrasubject (between-session) variability, but also significantly reduced intersubject variability, and thus reduced the test-retest reliability of estimating individual differences in functional connectivity. However, based on known nonneuronal mechanisms by which cardiac pulsation and respiration can lead to MRI signal changes, and the observation that the physiological noise itself is highly stable within individuals, removal of this noise will likely increase the validity of measured connectivity differences. Furthermore, removal of these fluctuations will lead to better estimates of average or group maps of connectivity. It is therefore recommended that studies apply physiological noise corrections but also be mindful of potential correlations with measures of interest.
