1. Antiviral resistance in influenza viruses-implications for management and pandemic response

Abstract

Studies of community isolates generally revealed low levels of primary resistance, when the incidence increased dramatically in China, possibly due to increased use of over-the-counter amantadine after emergence of severe acute respiratory syndrome. However it is essential to enhance systematic monitoring for the emergence of resistant viruses, rapidly incorporate data on resistance into treatment strategies, and proceed with development of alternative agents.

The drug zanamivir can prevent influenza in people who come in contact with infected individuals and can also shorten the duration of illness in infected people. This was the conclusion of a study of 337 families in which one member contracted the flu.

Abstract

The drug oseltamivir appears to be effective in preventing influenza. Researchers randomly assigned 1,559 healthy people to take one of two dosages of the drug or a placebo for six weeks during the winter of 1997-1998. Based on a laboratory diagnosis consisting of fever, at least one respiratory symptom and at least one other symptom, only 5% of those who took oseltamivir contracted the flu compared to 11% of those who took a placebo. Side effects of the drug included nausea and vomiting.

Abstract

A live but attenuated influenza virus vaccine appears to be effective in children. Researchers tested the vaccine against a placebo in 288 children who received on dose and 1,314 children who received two doses 60 days apart. All doses were administered intranasally. The vaccine contained the same viral strains as are in the regular inactivated influenza vaccine. The vaccine was 93% effective in preventing influenza and was active against viral strains that were most common during the 1996-97 flu season.

Abstract

The drug zanamivir seems to be effective in reducing the symptoms and duration of influenza when given as an intranasal spray. Researchers randomly assigned 262 patients with influenza to self-administer a placebo or zanamivir twice a day for five days via a nasal spray and inhalation therapy. In those who began treatment within 30 hours of the illness, zanamivir reduced the duration of symptoms from seven days to four. Overall, the drug alleviated symptoms one day sooner than the placebo. Zanamivir also reduced the amount of virus shed in nasal secretions.

12. Emergence and apparent transmission of rimantadine-resistant influenza A virus in families

Abstract

The drug rimantadine has been used experimentally to treat and prevent infections with influenza A virus. When the drug is given to subjects for six to eight weeks during the seasons when flus are prevalent, 70 to 90 percent fewer flu cases occur. A trial was conducted to see if rimantadine prevented individuals who had been exposed to the virus yet did not show symptoms from developing the disease. Such intervention could prevent outbreaks in households or communities. Of the 28 families in the study who had been exposed to the virus and treated with rimantadine, 10 of the families had one or more members that developed symptoms of the flu. The same number of families had members who developed the flu, without being treated with the drug. Five of the 28 families developed asymptomatic secondary influenza A infections regardless of whether they were given the drug. Thus, rimantadine is ineffective in preventing the development of the flu following exposure. Furthermore, viruses that are resistant to rimantadine have been isolated from individuals who have been treated with rimantadine. Patients who have been treated with rimantadine should be isolated to prevent the spread of these drug-resistant strains. The use of rimantadine needs further study but should be limited.
