Background: The clinical benefit of mechanical thrombectomy(MT) for stroke patients with tandem occlusion is similar to that of isolated intracranial occlusions. However, the management of cervical internal carotid artery(ICA) occlusion during the MT, particularly in the setting of carotid dissection, remains controversial. We aimed to investigate the clinical impact of cervical ICA patency at day 1 on 3-month functional outcome., Methods: We collected data from the Endovascular Treatment in Ischemic Stroke, a prospective national registry in 30 French centers performing MT between January 2015 and January 2022. Inclusion criteria were consecutive tandem occlusions related to cervical ICA dissection treated with MT. Tandem occlusions of other etiology, isolated cervical ICA occlusions without intracranial thrombus and patients without day-1 ICA imaging were excluded. Primary endpoint was the 3-month functional outcome. Secondary endpoints included intracranial hemorrhage(ICH), excellent outcome, mortality and early neurological improvement. A sensitivity analysis was performed in patients with intracranial favorable recanalization after MT., Results: During the study period, 137 patients were included of which 89(65%) presented ICA patency at day 1. The odds of favorable outcome did not significantly differ between patients with patent and occluded ICA at day 1(68.7 vs 59.1%;aOR=1.30;95%CI 0.56-3.00,p=0.54). Excellent outcome, early neurological improvement, mortality and ICH were also comparable between groups. Sensitivity analysis showed similar results., Conclusion: ICA patency at day 1 in patients with tandem occlusions related to dissection did not seem to influence functional outcome. Background: Weather conditions have been shown to influence the occurrence of cardiovascular events. We tested the hypothesis that weather parameters may be associated with variations of case volume of endovascular treatment (EVT) for acute ischemic stroke., Methods: Individual data from the ETIS (Endovascular Treatment in Ischemic Stroke) French national registry were matched to local weather stations. Meteorological parameters (rainfall, humidity, atmospheric pressure, air temperature) were gathered from national online resources. Weather readings and EVT case volumes were annually standardized per weather station and EVT center, and their associations tested with non-parametric univariable and generalized linear statistical models., Results: Between 2015 and 2021, 9913 EVT procedures addressed by 135 primary stroke units were matched to weather conditions. The mean daily case volume per center was 0.41 [StDev 0.33], and there was a median of 0.84 procedures daily linked to a weather station [StDev 0.47]. We found lower atmospheric pressure (β estimate -0.04; 95%CI[-0.07;-0.03], p<0.001), higher humidity (β estimate 0.07; 95%CI [0.05;0.09], p<0.001) and lower temperatures (β estimate -0.08; 95%CI[-0.10;-0.06], p<0.001) to be associated with higher standardized EVT daily case volumes. These associations were stable when testing them across strata of binned EVT standardized case volumes., Conclusions: Our study suggests that lower ambient temperature, lower atmospheric pressure, and higher air humidity are associated with significantly more daily EVT cases in a European temperate country. These results may provide insight into both system of care optimization at times of climate change and intracranial LVO pathophysiology. Recent studies of the medical literature have revealed numerous and serious problems. Errors in the design, methods and interpretation of studies can frequently be identified. A huge hidden problem is publication bias, the tendency for positive articles to be published, while negative articles are either not written or submitted. This can systematically lead to an overestimation of the value of treatments, of diagnostic or prognostic studies. Even more worrisome is selective reporting: only a subset of a wide array of tested hypotheses are presented (the ones that turned out to be positive with significance testing). This is particularly true for secondary endpoints and subgroup findings, but even the primary endpoints of trials have been modified when publications are compared to protocols. The peer-review process is fallible. Even if it were strengthened, reviewers cannot examine what is not reported. Hence many problems can only be mitigated with better reporting. Background and Purpose: Mechanical thrombectomy (MT) has dramatically changed the landscape of stroke care as well as stroke care organization. Public health institutions are faced with the challenge of swiftly providing equal access to this high technical level procedure with rapidly broadening indications, and constantly developing techniques. The aim of this study was to present a current nationwide overview of technical MT practices in France as well as local organizations., Materials and Methods: Thrombectomy capable French stroke centers, and physicians performing MT were invited to participate to a nationwide survey, disseminated through an existing trainee-led research network (the JENI-RC) under the aegis of the French Society of Neuroradiology. The survey was composed of 64 questions to collect both individual practices and general center-based information., Results: All French centers (100%) answered the survey, and 74% (110/148) of active interventional neuroradiologists (INR) performing MT completed individual questionnaires. The mean number of INR per center performing MT was 3.7±1.85, and 85% of the centers were organized for 24/7 continuity of care. MRI was the most commonly used imaging modality for stroke diagnosis and patients' selection, and perfusion imaging was routinely available in 85% of the centers. Half of centers performed yearly between 100 and 200 MT. Anesthesiologic, and technical considerations are also developed in the manuscript., Conclusions: This nationwide survey highlights the impressive response to the challenge of reorganization of stroke care with regards to mechanical thrombectomy in France. Background: Tandem basilar artery occlusions (tBAO) are defined as concomitant basilar artery and extracranial dominant vertebral artery occlusions. The prognosis of such tBAO treated by mechanical thrombectomy (MT) has been scantly reported. The purpose of our study was to compare the safety and effectiveness of MT for patients with tBAO compared to those with non-tandem basilar artery occlusions (ntBAO)., Patients and Methods: Retrospective analysis of a database of patients who underwent MT at two academic centres. All patients treated for BAO were retrieved. Patients with tBAO, defined as a concomitant BAO and extracranial vertebral artery (VA) occlusion or severe stenosis ≥70% (V1 or proximal V2 segment) were compared with patients with ntBAO., Results: A total of 15 patients with tBAO and 74 patients with ntBAO were enrolled. Successful reperfusion (modified thrombolysis in cerebral infarction score ≥2b) was obtained in 73.3% versus 90.5% (OR = 0.29, 95%CI: 0.07-1.15), good clinical outcome (3-months modified Rankin scale ≤2) was reached by 26.7% versus 32.4% (OR = 0.76; 95% CI: 0.24-2.63) and mortality at 3-months was 46.7% versus 31% (OR = 1.94; 95%CI: 0.63-6) of patients with tBAO versus ntBAO, respectively. Two patients (13.3%) with tBAO and three (4%) with ntBAO had symptomatic intracranial haemorrhage (OR = 3.64; 95% CI: 0.55-24)., Conclusion: Mechanical thrombectomy for patients with tandem basilar artery occlusion tends to be associated with lower rates of successful reperfusion and good clinical outcome, and higher rate of mortality. Cerebral amyloid angiopathy (CAA) is a frequent age-related small vessel disease (SVD) with cardinal magnetic resonance imaging (MRI) signatures that are hemorrhagic in nature, and include the presence of strictly lobar (superficial) cerebral microbleeds and intracerebral hemorrhages as well as cortical superficial siderosis. When investigating a patient with suspected CAA in the context of intracranial hemorrhage (parenchymal or subarachnoid) or cognitive dysfunction, various MRI parameters influence the optimal detection and characterization (and prognostication) of this frequent SVD. The present report describes the influence of imaging techniques on the detection of the key hemorrhagic CAA imaging signatures in clinical practice, in research studies, and the imaging parameters that must be understood when encountering a CAA patient, as well as reviewing CAA literature., (Copyright © 2017 Elsevier Masson SAS. The management of patients with unruptured intracranial aneurysms (UIAs) is a complex clinical challenge and constitutes an immense field of research. While a preponderant proportion of these aneurysms never rupture, the consequences of such an event are severe and represent an important healthcare problem. To date, however, the natural history of UIAs is not completely understood and there is no accurate means to discriminate the UIAs that will rupture from those that will not. Yet, a good understanding of the recent evidence and future perspectives is needed when advising a patient with IA to tailor any information to the given patient's level of risk and psychoaffective status. Background: In anterior acute ischemic stroke (AAIS) treated with endovascular treatment (EVT), the susceptibility vessel sign (SVS+ or SVS-) is related to recanalization results (TICI 2b/3) and clinical outcome. However, a binary qualitative assessment of thrombus using SVS does not reflect its complex composition. Our aim was to assess whether a quantitative MRI marker, Thrombus-T2* relaxation time, may be assessable in clinical routine and may to predict early successful recanalization after EVT, defined as a TICI 2b/3 recanalization obtained in 2 attempts or less., Material and Methods: Thrombus-T2* relaxation time was prospectively obtained from consecutive AAIS patients treated by EVT (concomitant aspiration and stent retriever). Quantitative values were compared between early recanalization and late or unsuccessful recanalization., Results: Thirty patients with AAIS were included and Thrombus-T2* relaxation time was obtained in all patients. Earlier TICI 2b/3 recanalization were obtained in 22 patients (73%) and was significantly associated with SVS+ (1/8 vs. 16/22, P=0.01) and a shorter Thombus-T2* relaxation time (mean SD, range: 257, 18-50ms vs. 45 9, 35-60ms, P<0.001)., Conclusion: A new quantitative MRI biomarker, the Thrombus-T2* relaxation time is assessable in clinical routine. Surgical resection of gliomas involving eloquent brain areas must be maximal in order to improve patients' survival, and safe to prevent postoperative impairments. Therefore, the precise spatial relationship between the lesion and eloquent brain areas needs to be established. Functional magnetic resonance imaging and diffusion tensor imaging are robust methods with increasing indications in neurosurgery for past decade. The aim of this review article is not only to pinpoint the major limitations of these methods in order to avoid erroneous conclusions, but also to detail practical aspects associated with the main paradigms routinely used in functional magnetic resonance imaging, and to discuss recent validation of functional magnetic resonance imaging and diffusion tensor imaging results with direct electrical stimulation during awake surgery., (Copyright © 2017 Elsevier Masson SAS. Background and Purpose: Triage imaging facilitates the timely recognition of acute stroke with prognostic implications. Improvement in MR acquisition speed is needed given the extreme time constraints before treatment. We compared an ultrafast Echo-Planar FLAIR sequence (EPI-FLAIR) and a conventional FLAIR sequence (cFLAIR) for their diagnostic performances and ability to estimate the age of infarction., Material and Methods: Between June and August 2014, 125 consecutive patients (age 69±18 years, 48% men) admitted for a suspicion of acute (≤48-hrs) stroke were explored by both FLAIR sequences at 1.5-Tesla. EPI-FLAIR (15-sec) and cFLAIR (2-min and 15-sec) were compared by two readers, blinded to clinical data., Results: EPI-FLAIR was less prone to kinetic artefacts than cFLAIR (2-3% vs. 23-49% depending on the reader, P<0.001). Diagnostic concordance was excellent for both readers (к>0.9). Amongst 8 hemorrhages, one subarachnoid hemorrhage presenting as a sudden deficit was missed on EPI-FLAIR sequence. Amongst 60 infarctions, cFLAIR and EPI-FLAIR were concordant in 50 (83%), while signal changes were visible on cFLAIR only in the remaining 10 (17%) cases. Amongst the 43 patients with known onset time (n=17 within 4.5hrs), FLAIR-DWI mismatch identified<4.5-hrs infarction with the same sensitivity (65%) using cFLAIR and EPI-FLAIR, but the positive predictive value (PPV) was higher for cFLAIR than for EPI-FLAIR (73% vs. 50%, P=0.008)., Conclusion: EPI-FLAIR allows a drastic reduction of acquisition time devoted to FLAIR sequence and minimizes motion artifacts. Neuroimaging is critical in the evaluation of patients with transient ischemic attack (TIA) and MRI is the recommended modality to image an ischemic lesion. The presence of a diffusion (DWI) lesion in a patient with transient neurological symptoms confirms the vascular origin of the deficit and is predictive of a high risk of stroke. Refinement of MR studies including high resolution DWI and perfusion imaging using either MRI or CT further improve the detection of ischemic lesions. Rapid etiological work-up includes non-invasive imaging of cervical and intracranial arteries to search for symptomatic stenosis/occlusion associated with an increased risk of stroke., (Copyright © 2015. Moya-Moya disease is a rare arterial occlusive disease affecting the internal carotid artery and its branches. It is found in both pediatric and adult populations, and it may lead to severe clinical presentations such as stroke and intracranial hemorrhage. Several surgical procedures have been developed to improve its clinical outcome. Imaging techniques have a key role in management of Moya-Moya disease, as they are necessary for diagnosis, choice of treatment and follow-up. Although catheter angiography remains the diagnostic gold standard, and nuclear-medicine techniques best perform hemodynamic studies, less invasive imaging techniques have become efficient in serving these purposes. Conventional MRI and MR angiography, as well as MR functional and metabolic studies, are now widely used in each stage of disease management, from diagnosis to follow-up. CT scan and Doppler sonography may also help assess severity of disease and effects of treatment. Stroke mimics account for up to a third of suspected strokes. The main causes are epileptic deficit, migraine aura, hypoglycemia, and functional disorders. Accurate recognition of stroke mimics is important for adequate identification of candidates for thrombolysis. This decreases the number of unnecessary treatments and invasive vascular investigations. Correctly identifying the cause of symptoms also avoids delaying proper care. Therefore, this pictorial review focuses on what the radiologist should know about the most common MRI patterns of stroke mimics in the first hours after onset of symptoms. Background and Purpose: Developments in endovascular treatment have opened new promising prospects for treating acute ischemic stroke. In France, EVT is increasingly used, especially when intravenous thrombolysis is contraindicated or has failed. However, it has not been documented how neurointerventional centers are organized practically for the treatment of AIS. The present survey aims to address this., Materials and Methods: The centers in France that are authorized to perform EVT for AIS were invited to participate to an electronic survey. The survey was composed of 33 questions, divided into 6 subheadings: (1) general information, (2) imaging modalities, (3) patient selection, (4) anesthesiology, (5) endovascular procedure and (6) imaging follow-up., Results: The response rate was high at 93.9%. Neuroradiology centers are organized to perform mechanical thrombectomy around the clock in 80.6% of the institutions. MRI was the most commonly used imaging modality to examine acute stroke, alone in 64.5% or in combination with CT in 22.6%. The median number of neurointerventionalists was 3 per center and the median number of procedures performed in 2012 was 925. Since the medical treatment is complex, an anesthesiologist is often required during the procedure (87.1%). Technical issues are also developed in the manuscript., Conclusions: This survey shows that French neuroradiology departments have made important efforts to implement EVT of AIS with a high quality of care for the patients; the majority of the centers used MRI to evaluate the disease and anesthesiologists are involved in order to optimize medical care during EVT., (Copyright © 2014 Elsevier Masson SAS. Background and Purpose: Our aim was to estimate the prevalence of negative diffusion-weighted imaging (DWI) with total perfusion-diffusion mismatch in a large series of anterior circulation stroke patients treated with thrombolysis and to describe the characteristics of these patients., Materials and Methods: From January 2006 to December 2010, a retrospective search was made for total perfusion-diffusion (PWI-DWI) mismatch patterns on pretreatment 1.5-T MRI scans of 166 consecutive thrombolyzed patients taken<4.5 h after onset of anterior stroke. A total mismatch profile corresponded to an absence of initial DWI signal changes with hypoperfusion (T(max)>6 s) on PWI. Clinical and MRI characteristics were compared between DWI+ and DWI- patients., Results: Five (3%) patients had a normal initial DWI. All had stable substantial clinical deficits (NIHSS scores ≥ 6) and large perfusion abnormalities - in other words, 'total mismatch' - and infarcts in the acutely hypoperfused area on follow-up imaging. While DWI- and DWI+ patients did not significantly differ in any of the pretreatment imaging or clinical variables except for the extent of PWI-DWI mismatch, DWI- patients had lower NIHSS scores at 24 h, and were more likely to show early neurological improvement (Δ0-24 h NIHSS ≥ 8) and good outcomes (mRS ≤ 2) at the time of hospital discharge., Conclusion: Total mismatch i.e. failure of DWI to reveal any ischemic tissue despite a large perfusion defect, can be observed before thrombolysis even in stroke patients with stable substantial neurological deficits. Background and Purpose: The best management of unruptured aneurysms remains controversial. Accurate estimates of treatment-related morbidity and efficacy are needed for clinical decision making., Methods: We review recent systematic reviews, case series and population-based studies on clipping and coiling of unruptured aneurysms. A critical analysis of the quality of available studies and of the reliability of results is followed by reappraisal of the logic of clinical research on unruptured aneurysms., Results: Unfavourable outcome, including death, up to 1 year after surgical clipping, was found in 692 of 9845 patients (REW average: 6.7%; (99%)CI: 4.9-9.0). Unfavourable outcome after coiling occurred in 189 of 5044 patients (REW average 4.8%, ((99%)CI: 3.9-6.0). Location and size of aneurysms were risk factors for clipping, while no risk factor reached statistical significance for coiling studies. The quality of reports was in general poor. Estimates from epidemiological studies showed higher morbidity rates than reviews of case series. Treatment efficacy could not be assessed. All studies are attempts to provide answers to questions that were never properly raised., Conclusion: Results of reviews and observational studies should serve as hypotheses for clinical trials., (Copyright © 2012. Imaging of water diffusion or diffusion-weighted MR imaging provides physiological information about brain diseases that cannot be obtained from conventional sequences. This technique is very sensitive for the detection of cerebral ischemia from arterial origin and can distinguish cerebral ischemia from other non-vascular brain pathologies in patients presenting with abrupt onset of focal neurological deficit. Diffusion-weighted imaging is used for the evaluation of non-vascular diseases as well. Combined with conventional sequences, it is helpful to differentiate brain abscesses from necrotic tumors. Quantitative diffusion-weighted imaging provides additional information in the characterization of tumors or inflammatory, degenerative and metabolic lesions. Finally, diffusion-weighted imaging data also has prognostic value.

Few institutions use MRI perfusion without contrast injection called arterial spins labeling (ASL) routinely in clinical setting. After general considerations concerning the different ASL techniques and quantitative issues, we will detail a pulsed sequence that can be used on a clinical 1.5-T MR unit. We will discuss and illustrate the use of ASL in tumoral diseases for diagnosis, gliomas grading, stereotactic biopsy guidance and for follow-up after treatment.

Owing to its rapid acquisition time and high sensitivity, diffusion-weighted imaging has turned into a routine sequence for brain imaging. This is the case not only for stroke, but also for various diseases such as abscesses or tumors. Being aware of the artifacts is important for optimal interpretation. After a brief review of the normal patterns, the most frequent artifacts, inherent to the echoplanar imaging technique, are described and we provide suggestions to avoid them. Most current traps are caused by T2-weighting of the diffusion images; the key for avoiding erroneous interpretation relies on the ADC map.

Intracranial metastases account for up to 35% of intracranial tumors in adult. They can involve any part of the central nervous system: brain, meninges and cranial nerves. Any systemic tumor can metastasize to the brain; the most common primaries include lung, breast and melanoma. Imaging plays a major role in the evaluation and management of patients with metastatic brain tumors. This article discusses optimal CT and MR imaging protocols and describes imaging features and distinguishing characteristics of cerebral and meningeal metastases.

Purpose: To estimate the value of duplex color-coded ultrasonography in the initial diagnosis of acute cervical artery dissection., Material and Method: Retrospective study of 31 patients, mean age 45, sex ratio=1, referred for clinical suspicion of cervical artery dissection, confirmed by MRI. 46 dissected arteries were imaged. Ten patients presented multiple dissections. The evaluated sonographic diagnostic criteria were the direct signs of intra-mural hematoma: localized increased diameter of the artery, narrowed lumen, hypo and/or isoechoic intra-mural hematoma, intimal flap. The associated criteria studied were: the location of intra-mural hematoma and in case of an occlusion, the dissection of an other artery., Results: We analyzed separately the arterial segments visualized in B-mode ultrasound (supra-bulbar internal carotid artery, vertebral artery from V0 to V3) and the arterial segments evaluated only by pulsed Doppler (intrapetrosal carotid artery, V3-V4 of the vertebral artery). With the above criteria, on arterial segments visualized in B-mode ultrasound, the diagnosis of dissection was done in 83% of cases during the initial examination but in only 30% of the arterial segments non visualized on ultrasonography., Conclusion: The direct signs of intra-mural hematoma on internal carotid and vertebral segments imaged by B-mode ultrasonography have a good sensitivity. Because results were compared to MRI, the specificity could not be assessed. In case of occlusion of an artery, multiple dissections are a strong argument for the diagnosis.

Vascular diseases are an important part of orbital pathology. We describe vascular tumours of the orbit and vascular diseases with repercussion on the orbit, from intra or extra orbital origin. The classification of these abnormalities is difficult and several terms are used to describe the same histological entity. The objective of this work is, using the current classification, to illustrate the different imaging aspects of the most frequent vascular diseases of the orbit.

We report a case of a cavernous dural fistula in a 54-year-old man the source of inflammatory ocular symptoms. Poorly adapted treatment with corticosteroids led to iatrogenic complications. We will attempt to show the advantage of color Doppler imaging for the diagnosis of this infrequent lesion. The only indication of digitalized angiography is for endovascular treatment.

Otosclerosis (OS) is a dysplasia of the otic capsule located in most cases on the anterior margin of the oval window or fissula ante fenestrum. Progressive conductive hearing loss is the major clinical symptom, due to stapedovestibular ankylosis. Stapes surgery is the only effective treatment of OS, with excellent functional results in more than 90% of cases. However, failures and complications of the surgery may be observed. In theses cases, the etiologic work-up includes imaging evaluation (CT and MRI). Imaging findings are extremely useful in the therapeutic decision. Surgical failure represents 80% of the causes for surgical revision. The main causes of failure are: displacement of the prosthesis, fibrosis of the oval window, erosion of the long process of the incus, incudo-mallear dislocation, obliterative otosclerosis. CT is essential for diagnosis. MR imaging is rarely indicated in the work-up of surgical failures. Labyrinthine complications account for less than 20% of surgical revisions. Etiologies of labyrinthine complications are: intravestibular penetration of the prosthesis, perilymphatic fistula, intra-vestibular granuloma, labyrinthitis and intravestibular bleeding. CT and MRI are complementary for the work up of these complications.

The advent of new MR techniques such as perfusion and diffusion weighted imaging has revolutionized diagnostic imaging in stroke. In some institutions, MRI is used as the sole screening imaging technique for acute stroke patients. In this document, the authors will review the MR pattern of acute ischemic arterial stroke, highlight the usefulness of MRI for the identification of acute hematomas and stroke like episodes, present the potential use of MRI in the management of acute stroke patients, especially when thrombolysis is contemplated, and discuss the role of MRI for imaging transient ischemic attack.

Diffusion tensor MR imaging is a technique that provides details on tIssue microstructure and organization well beyond the usual image resolution. With diffusion tensor MR imaging, diffusion anisotropy can be quantified and subtle white matter changes not normally seen on conventional MRI can be detected. The purpose of this article is to review the current applications of diffusion tensor MR imaging for the study of the brain, including normal aging, Alzheimer's disease, neuropsychiatric disorders, strokes, multiple sclerosis, brain tumors and intractable seizures.

This article summarizes the role of imaging in the evaluation of cognitive disorders of the elderly. The primary goal of imaging is to exclude potentially reversible dementia due to intracranial lesions such as tumor, subdural hematoma or normal pressure hydrocephalus. The value of neuroimaging in the diagnosis of degenerative disorders is then discussed: Alzheimer's disease, frontotemporal dementias, dementia with Lewy bodies, dementia associated with parkinsonian syndromes, vascular dementias and mild cognitive impairment.
