2. Arthrite septique temporomandibulaire avec résorption condylienne secondaire.

Résumé Introduction Les arthrites septiques sont des infections graves, d’origine majoritairement hématogène ou transmise par contiguïté, peu décrites pour l’articulation temporomandibulaire. Observation Notre patiente présentait le cas d’une infection de l’articulation temporomandibulaire par sinusite maxillaire d’origine dentaire, compliquée initialement d’abcès et empyèmes cérébraux. Le traitement a consisté en un drainage endonasal et endobuccal, une antibiothérapie probabiliste intraveineuse, une surveillance clinicoradiologique rapprochée, ainsi qu’une rééducation au long cours par kinésithérapie active. Secondairement, la patiente a signalé l’apparition d’un trouble de l’articulé dentaire avec contact prématuré gauche et béance latérale droite, correspondant à une importante résorption condylienne homolatérale. Discussion Cette pathologie infectieuse est très rare pour la localisation temporomandibulaire, néanmoins, son pronostic général et fonctionnel est conditionné par la précocité de la prise en charge. Il est donc important de connaître le diagnostic, sa symptomatologie même si celle-ci reste peu spécifique. L’évoquer devant des douleurs de l’articulation temporomandibulaire atypiques associées à un trismus est indispensable. Summary Introduction Septic arthritis are serious infections rarely observed for the temporomandibular joint. They are mainly hematogenous or transmitted by contiguity. Observation Our patient presents the case of an infection of the temporomandibular joint by maxillary sinusitis of dental origin further complicated by cerebral abscess and empyema. Initial treatment consisted of an endonasal and intraoral drainage, intravenous empirical antibiotic therapy, a close clinicoradiological monitoring, and rehabilitation following a long-term active physiotherapy. Furthermore, the patient reported the onset of a dental articulation disorder with a left side premature contact and right lateral open bite, corresponding to a significant left condylar resorption. Discussion This infectious disease is very rare for temporomandibular location; however, its general and functional outcome is determined by the precocity of the treatment. It is important to know the diagnosis and the associated symptoms even if they are not very specifically described. It is essential to consider the diagnosis when facing atypical pain of the temporomandibular joint associated with trismus. [ABSTRACT FROM AUTHOR]

Résumé Le condyle mandibulaire est une structure particulière. Son embryologie, sa physiologie, confèrent à l’ATM un comportement particulier qui explique la survenue de pathologies spécifiques. L’hypercondylie est l’une de ces pathologies. Elle s’explique par une dérégulation des cellules préchondroblastiques de la coiffe cartilagineuse, aboutissant à une augmentation volumique de l’unité condylienne puis de la branche montante et finalement de toute l’hémiface atteinte. La déformation de la mandibule est basale avec des conséquences dento-alvéolaires de rattrapage. Bien que le traitement soit controversé, la condylectomie est réalisée par beaucoup de praticiens. L’intervention permet de stopper l’évolution de la maladie et de rattraper certaines compensations dento-alvéolaires. Dans certains cas, une chirurgie orthognathique complémentaire sera nécessaire. Summary The mandibular condyle is a special structure. Its embryology and physiology provide to the TMJ a particular behavior that explains the occurrence of specific diseases. Condyle hyperplasia is one of these disorders. It can be explained by a dysregulation of the prechondroblast cell layer within the cartilage cap providing an increase in volume first of the condyle, then of the ramus and finally of the entire affected hemiface. Mandible deformation affects the basal bone, leading to dento-alveolar deformations related to compensation attempts. Controversies, condylectomy is performed by many surgeons. This procedure allows for the correction of some of the dento-alveolar compensations. In some cases, an additional orthognathic surgery will be required to achieve a perfect result. 4. Étude anatomique des rapports de l’artère sous-mentonnière pour le prélèvement du lambeau sous-mentonnier.

Résumé Introduction Le lambeau sous-mentonnier est utile pour les reconstructions intra-orales et des deux tiers inférieurs de la face. La dissection est délicate à cause d’une exposition difficile sous le bord basilaire mandibulaire, de nombreuses collatérales dans une couche cellulo-graisseuse et de la proximité du rameau marginal de la mandibule. Le but de notre travail était de proposer des repères anatomiques pour faciliter le prélèvement du lambeau sous-mentonnier. Matériel et méthode Dix cadavres traités par liquide de conservation Biomet ont été disséqués de façon bilatérale. Les rapports anatomiques du rameau marginal du nerf facial avec la mandibule, les rapports de l’artère sous-mentonnière, le nombre de ses collatérales et leurs positions ont été mesurés à l’aide d’un pied à coulisse. Résultats La position la plus haute du rameau marginal était située 0,5 cm au-dessus du bord basilaire alors que la plus basse était située 0,6 cm au-dessous. La longueur mesurée entre le passage de l’artère facial sur le bord basilaire au niveau de l’encoche pré-angulaire et l’origine de l’artère sous-mentonnière a été en moyenne de 1,5 cm. Le nombre moyen de collatérales était de 3,6. Discussion L’incision cutanée réalisée directement sous le bord basilaire, comme décrite par certains auteurs, entraîne un risque de lésion du rameau marginal mandibulaire. Trois positions de l’artère sous-mentonnière par rapport à la glande sub-mandibulaire sont décrites. Les collatérales sont à destinée glandulaire, musculaire, cutanée et osseuse. Il faut accorder un intérêt particulier à l’artère sublinguale, artère de diamètre important qui naît en moyenne à 2,8 cm de son origine et plonge distalement vers le plancher buccal. Il ne faut pas la suivre sous peine de risquer de clamper le grêle pédicule destiné au muscle digastrique. Il est indispensable de respecter l’ambiance cellulo-graisseuse superficielle satellite de l’artère sous-mentonnière pour limiter la congestion veineuse du lambeau. Summary Introduction Submental flap is useful for intra-oral reconstructions and reconstructions of the lower two thirds of the face. Dissection is delicate because of a difficult exposure under the lower rim of the mandible, numerous collateral arterial branches and the proximity of the marginal branch of the facial nerve. The aim of our work was to propose anatomical landmarks in order to facilitate the submental flap raising. Material and method Ten bodies preserved in Biomet liquid were dissected bilaterally. The anatomic relationships between the marginal branch of the facial nerve and the mandible, the relationships of the submental artery, the amount and the location of its collateral branches were measured by means of a caliper. Results The highest marginal branch observed was located 0.5 cm above the mandibular lower rim, while the lower one was located 0.6 cm below this rim. The mean length measured between the facial artery at its crossing over the mandibular rim at the level of the pre-angular notch and the origin of the submental artery was 1.5 cm. The average number of collateral branches was 3.6. Discussion A skin incision made directly under the mandibular lower rim, as mentioned by some authors, may endanger the mandibular marginal branch of the facial nerve. Three positions of the submental artery in relation to the sub-maxillary gland are reported. The collateral branches are intended for gland, muscle, skin and bone. It is necessary to pay particular attention to the sub-lingual artery, an artery of big diameter that arises at 2.8 cm on average from its origin and plunges towards the mouth's floor. It must not be followed at risk of clamping the thin pedicle destined to the digastric muscle. It is important to preserve the fat tissue around the submental pedicle in order to avoid venous congestion of the flap. [ABSTRACT FROM AUTHOR]

Résumé Les fistules médianes de la lèvre supérieure sont une variante extrêmement rare de malformation faciale congénitale. Moins de 30 cas ont été rapportés dans la littérature depuis 1970. Nous rapportons un cas de fistule borgne congénitale médiane de la lèvre supérieure sans communication avec la cavité orale chez une enfant de 2 ans et demi présentant une tuméfaction labiale supérieure récidivante. Le traitement est exclusivement chirurgical. L’exérèse complète du kyste ou du trajet fistuleux doit être obtenue sous peine de récidive. Summary Midline upper-lip fistulas are an extremely rare variant of congenital facial malformations. Less than 30 cases have been reported in the literature since 1970. We report the case of a 2 and a half-year-old girl presenting with a median congenital blind fistula of the upper lip, without any relation with the oral cavity. A recurrent swelling of the upper lip was the main symptom. Complete surgical excision of the cyst or of the fistulous tract must be obtained to avoid recurrence. [ABSTRACT FROM AUTHOR]

Introduction. Third molar extraction is one of the most common surgical procedures in oral and maxillofacial surgery. Infectious complications can be estimated at 3.6% after local anesthesia. Observation. A 39-year-old female patient, presenting with impor-tant swelling of the face and neck after extraction of teeth 28 and 38 after local anesthesia. Clinical and radiographic data led to the diagnosis of facial cellulitis due to a foreign body on the sub angulo-mandibular region. Drainage under general anesthesia was perfor-med and a fragment of anesthesia needle was removed. One month after surgery, the patient was considered cured. Discussion. Breaking an anesthesia needle is a rare accident, which can occur during oral surgery under local anesthesia. Respecting the rules of good practice is essential in preventing this type of compli-cation, especially since forensic rules for dental and oral surgery are becoming stricter. Introduction. L'extractio n de troisième molaire est l'une des inter-ventions chirurgicales les plus fréquentes en chirurgie orale et maxillofaciale. Ses complications infectieuses après anesthésie locale peuvent être estimées à 3,6 %. Observation. Une patiente de 39 ans a présenté une volumineuse tuméfaction cervicofaciale après l'extraction des 28 et 38 sous anesthésie locale. Le bilan clinique et radiologique a mis en évidence une cellulite cervicofaciale sur corps étranger dans la région sous angulo-mandibulaire gauche. Lors du drainage sous anesthésie générale, un fragment d'aiguille d'anesthésie tronculaire a été retiré. Un mois après l'intervention, la patiente était considérée comme guérie. Discussion. La fracture d'aiguille d'anesthésie est un accident rare pouvant survenir lors d'un geste endobuccal sous anesthésie locale. Le respect de règles simples de bonne pratique est primordial pour éviter ce type de complications d'autant plus que l'encadrement médicolégal de la chirurgie dentaire et de la chirurgie orale est de plus en plus strict. The article discusses a research which aims to analyze the odontogenic disorders related to congenital piriform aperture stenosis along with their presentations. It is mentioned that retrospective analysis of twelve patients was done using computed tomography (CT) scan for studying temporal dental germs.
