The health and well-being of retired rugby union and league players, particularly regarding the long-term effects of concussions, are of major concern. Concussion has been identified as a major risk factor for neurodegenerative diseases, such as Alzheimer's and Amyotrophic Lateral Sclerosis (ALS), in athletes engaged in contact sports. This study aimed to assess differences in specific biomarkers between UK-based retired rugby players with a history of concussion and a non-contact sports group, focusing on biomarkers associated with Alzheimer's, ALS, and CTE. We randomly selected a sample of male retired rugby or non-contact sport athletes (n = 56). The mean age was 41.84 ± 6.44, and the mean years since retirement from the sport was 7.76 ± 6.69 for participants with a history of substantial concussions (>5 concussions in their career) (n = 30). The mean age was 45.75 ± 11.52, and the mean years since retirement was 6.75 ± 4.64 for the healthy controls (n = 26). Serum biomarkers (t-tau, RBP-4, SAA, Nf-L, and retinol), plasma cytokines, and biomarkers associated with serum-derived exosomes (Aβ42, p-tau181, p-tau217, and p-tau231) were analyzed using validated commercial ELISA assays. The results of the selected biomarkers were compared between the two groups. Biomarkers including t-tau and p-tau181 were significantly elevated in the history of the substantial concussion group compared to the non-contact sports group (t-tau: p < 0.01; p-tau181: p < 0.05). Although between-group differences in p-tau217, p-tau231, SAA, Nf-L, retinol, and Aβ42 were not significantly different, there was a trend for higher levels of Aβ42, p-tau217, and p-tau231 in the concussed group. Interestingly, the serum-derived exosome sizes were significantly larger ( p < 0.01), and serum RBP-4 levels were significantly reduced ( p < 0.05) in the highly concussed group. These findings indicate that retired athletes with a history of multiple concussions during their careers have altered serum measurements of exosome size, t-tau, p-tau181, and RBP-4. These biomarkers should be explored further for the prediction of future neurodegenerative outcomes, including ALS, in those with a history of concussion.

A Hybrid LiFi and WiFi network (HLWNet) integrates the rapid data transmission capabilities of Light Fidelity (LiFi) with the extensive connectivity provided by Wireless Fidelity (WiFi), resulting in significant benefits for wireless data transmissions in the designated area. However, the challenge of decision-making during the handover process in HLWNet is made more complex due to the specific characteristics of electromagnetic signals' line-of-sight transmission, resulting in a greater level of intricacy compared to previous heterogeneous networks. This research work addresses the problem of handover decisions in the Hybrid LiFi and WiFi networks and treats it as a binary classification problem. Consequently, it proposes a handover method based on a deep neural network (DNN). The comprehensive handover scheme incorporates two sets of neural networks (ANN and DNN) that utilize input factors such as channel quality and the mobility of users to enable informed decisions during handovers. Following training with labeled datasets, the neural-network-based handover approach achieves an accuracy rate exceeding 95%. A comparative analysis of the proposed scheme against the benchmark reveals that the proposed method considerably increases user throughput by approximately 18.58% to 38.5% while reducing the handover rate by approximately 55.21% to 67.15% compared to the benchmark artificial neural network (ANN); moreover, the proposed method demonstrates robustness in the face of variations in user mobility and channel conditions.

Awareness of indoor air quality (IAQ) in crowded places such as schools and offices has increased since 2020 due to the COVID-19 pandemic. In addition, countries' shifting away from containment and towards living with COVID-19 is expected to increase demand for risk mitigation via air-purification devices. In this work, we use Computational Fluid Dynamics (CFD) analysis to investigate the impact of adding an air-purification technology on airflow in an enclosed space. We model a Polyester Filter and UV light (PFUV) dehumidifier in an office with two occupants: one infected with an airborne infectious disease, such as COVID-19; and the other uninfected. We compare three cases where the infected occupant coughs: with no device, and with the device at two different orientations. We construct a CFD model using ANSYS ® 2021 Fluent and the Discrete Phase Model (DPM) for the particle treatment. Thermal comfort is assessed using the Testo 400 IAQ and comfort kit. We find that both the device operation and the placement alter the airflow contours, significantly reducing the potential for the uninfected occupant to inhale the vapour expelled by the infected occupant, potentially impacting the likelihood of disease transmission. The device improved thermal comfort measured by Predicted Mean Vote (PMV), Predicted Percentage Dissatisfied (PPD).

The increasing demand for palliative care in New Zealand presents a potential threat to the quality of service delivery. One strategy to overcome this is through the implementation of valid and reliable patient-reported outcome measures. This mixed-methods study aimed to (1) implement measurement-based palliative care (MBPC) in a community palliative care service in Auckland, New Zealand; (2) evaluate the clinical utility of MBPC perceived by clinicians; (3) describe patient characteristics as measured by the Integrated Palliative Care Outcome Scale (IPOS), the Australasian Modified Karnofsky Performance Scale (AKPS), and Phase of Illness (POI); and (4) evaluate the internal consistency of the IPOS. Participants were over 18 years of age from a community outpatient palliative care service. In a phased approach to implementation, healthcare staff were educated on each instrument used for patient assessment. Uptake and internal consistency were evaluated through descriptive statistics. An interpretive descriptive methodology was used to explore the clinical utility of MBPC through semi-structured interviews with seven clinical staff members. Individual patient assessments ( n = 1507) were undertaken predominantly on admission, with decreasing frequency as patients advanced through to the terminal phase of their care. Mean total IPOS scores were 17.97 (SD = 10.39, α = 0.78). The POI showed that 65% of patients were in the stable phase, 20% were in the unstable phase, 9% were in the deteriorating phase, and 2% were in the terminal phase. Clinicians reported that MBPC facilitated holistic and comprehensive assessments, as well as the development of a common interdisciplinary language. Clinicians expressed discomfort using the psychosocial and spiritual items. Measurement-based palliative care was only partially implemented but it was valued by staff and perceived to increase the quality of service delivery. Future research should determine the optimal timing of assessments, cultural responsivity for Māori and Pacific patients, and the role of MBPC in decision support for clinicians.

The study aim was to determine prevalence and predictors of life satisfaction in New Zealand. In this observational cross-sectional study, a sample of 10,799 participants from NZ were drawn from the Gallup World Poll from 2006 to 2017. Data were analysed using regression analysis and ANOVA. Prevalence of life satisfaction across time varied little from a high of 7.61 ( SD = 1.6) in 2007 to a low of 7.23 ( SD = 1.73) in 2011 (range 0-10). Satisfaction with standards of living predicted life satisfaction regardless of age or gender. For males across all age groups and females up to age 40 years, positive experiences and satisfaction with household income were important predictors. Being married was an important predictor for males over 40 years and feeling satisfied with their current city was important for females across all ages and for men under 40. The levels of life satisfaction changed over time, possibly due to major national events. Satisfaction with standards of living was found to predict life satisfaction regardless of age or gender. These results provide a path for policy focus towards increased life satisfaction.

Background: South Africa, with an estimated annual tuberculosis (TB) incidence of 360,000 cases in 2019, remains one of the countries with the largest burden of TB in the world. The identification of highly burdened TB areas could support public health policy planners to optimally target resources and TB control and prevention interventions. Objective: To investigate the spatial epidemiology and distribution of TB mortality in South Africa in 2010 and its association with area-level poverty and HIV burden. Methods: The study analysed a total of 776,176 TB deaths for the period 2005-2015. Local and global and spatial clustering of TB death rates were investigated by Global and Local Moran's Indices methods (Moran's I). The spatial regression analysis was employed to assess the effect of poverty and HIV on TB mortality rates. Results: There was a significant decrease in TB mortality rate, from 179 per 100,000 population in 2005 to 60 per 100,000 population in 2015. The annual TB mortality rate was higher among males (161.5 per 100,000 male population; (95% confidence interval (CI) 132.9, 190.0) than among females (123.2 per 100,000 female population; (95% CI 95.6, 150.8)). The 35-44 age group experienced higher TB mortality rates, regardless of gender and time. Hot spot clusters of TB mortality were found in the South-Eastern parts of the country, whereas cold spot clusters were largely in the north-eastern parts. Tuberculosis death rates were positively associated with poverty, as measured by the South African Multidimension Poverty Index (SAMPI) as well TB death rates in the neighbouring districts. Conclusion: The findings of this study revealed a statistically significant decrease in TB deaths and a disproportionate distribution of TB deaths among certain areas and population groups in South Africa. The existence of the identified inequalities in the burden of TB deaths calls for targeted public health interventions, policies, and resources to be directed towards the most vulnerable populations in South Africa.

Light Fidelity (LiFi) is a new candidate for wireless networking that utilizes the visible light spectrum and exploits the existing lighting infrastructure in the form of light-emitting diodes (LEDs). It provides point-to-point and point-to-multipoint communication on a bidirectional channel at very high data rates. However, the LiFi has small coverage, and its optical gain is closely related to the receiver's directionality vis-à-vis the transmitter, therefore it can experience frequent service outages. To provide reliable coverage, the LiFi is integrated with other networking technologies such as wireless fidelity (WiFi) thus forming a hybrid system. The hybrid LiFi/WiFi system faces many challenges including but not limited to seamless integration with the WiFi, support for mobility, handover management, resource sharing, and load balancing. The existing literature has addressed one or the other aspect of the issues facing LiFi systems. There are limited free source tools available to holistically address these challenges in a scalable manner. To this end, we have developed an open-source simulation framework based on the network simulator 3 ( ns-3 ), which realizes critical aspects of the LiFi wireless network. Our developed ns-3 LiFi framework provides a fully functional AP equipped with the physical layer and medium access control (MAC), a mobility model for the user device, and integration between LiFi and WiFi with a handover facility. Simulation results are produced to demonstrate the mobility and handover capabilities, and the performance gains from the LiFi-WiFi hybrid system in terms of packet delay, throughput, packet drop ratio (PDR), and fairness between users. The source code of the framework is made available for the use of the research community.

This study investigated how two public spaces for sport and recreation were utilized by different user groups, and how this aligned with the initial design objectives for these spaces. Two newly built urban spaces situated in Copenhagen, Denmark, provided the context for this investigation. The System for Observing Play and Recreation in Communities (SOPARC) was used to examine the physical activity of users in these two urban spaces. The architects responsible for designing each space were interviewed to ascertain the intended target group of each space and to unravel the reasons behind the design decisions. The SOPARC observations revealed that males were more vigorously active than females when using the recreation facilities, and the observed users did not align with the intended target groups. The interviews suggested that design decisions were based on minimal interdisciplinary knowledge, and that expert knowledge was chosen randomly. These findings point to a systematic lack of evidence-based practice when designing sport and recreational facilities. This article has implications for landscape architects and urban planners; a new method must be developed to embed interdisciplinary knowledge in the planning process of future sport and recreation projects. This must be done in a systematic way to make the design process transparent.
