Introduction: Distal tibial fractures make up approximately 3% to 10% of all tibial fractures or about 1% of lower extremity fractures. MIPO is an appropriate procedure and method to achieve stable metal plate fixation and osseointegration by minimizing soft tissue damage and vascular integrity at the fracture site. MIPO to the medial tibia during distal tibial fractures induces skin irritation due to the thickness of the metal plate, which causes discomfort and pain on the medial side of the distal leg, and if severe, complications such as infection and skin defect may occur. The reverse sural flap is a well-researched approach for covering defects in the lower third of the leg, ankle, and foot. Materials and Methods: Among 151 patients with distal tibia fractures who underwent minimally invasive metal plate fixation, soft tissue was injured due to postoperative complications. We treated 13 cases with necrosis and exposed metal plates by retrograde nasogastric artery flap surgery. For these patients, we collected obligatory patient records, radiological data, and wound photographs of the treatment results and complications of reconstructive surgery. Results: In all the cases, flap survival was confirmed at the final outpatient follow-up. The exposed area of the metal plate was well coated, and there was no plate failure due to complete necrosis. Three out of four women complained of aesthetic dissatisfaction because the volume of the tunnel through which the skin mirror passed and the skin plate itself were thick. In two cases, defatting was performed to reduce the thickness of the plate while removing the metal plate. Conclusions: Metal plate exposure after distal tibial fractures have been treated with minimally invasive metal plate fusion and can be successfully treated with retrograde nasogastric artery flaps, and several surgical techniques are used during flap surgery. We aimed to evaluate various aspects of antibiotic therapy as factors associated with candidemia in non-neutropenic patients. A retrospective, matched, case-control study was conducted in two teaching hospitals. Patients with candidemia (cases) were compared to patients without candidemia (controls), matched by age, intensive care unit admission, duration of hospitalization, and type of surgery. Logistic regression analyses were performed to identify factors associated with candidemia. A total of 246 patients were included in the study. Of 123 candidemia patients, 36% had catheter-related bloodstream infections (CRBSIs). Independent factors in the whole population included immunosuppression (adjusted odds ratio [aOR] = 2.195; p = 0.036), total parenteral nutrition (aOR = 3.642; p < 0.001), and anti-methicillin-resistant S. aureus (MRSA) therapy for ≥11 days (aOR = 5.151; p = 0.004). The antibiotic factor in the non-CRBSI population was anti-pseudomonal beta-lactam treatment duration of ≥3 days (aOR = 5.260; p = 0.008). The antibiotic factors in the CRBSI population included anti-MRSA therapy for ≥11 days (aOR = 10.031; p = 0.019). Antimicrobial stewardship that reduces exposure to these antibacterial spectra could help prevent the development of candidemia. [ABSTRACT FROM AUTHOR]

A new allotetraploid species of the genus Asplenium, A. pseudocapillipes, originated from the hybridization between A. capillipes and A. tenuicaule, has been newly discovered in two limestone areas of South Korea. A molecular phylogenetic analysis using one chloroplast region (rbcL) and three single- or low-copy nuclear regions (AK1, gapCp, pgiC) and a cytological analysis, including genome size measurements, were conducted to characterize this new species. From these results, the maternal origin of A. pseudocapillipes was confirmed to be A. capillipes, which has never been reported in Korea. All three nuclear data showed that this new species had genotypes of both A. capillipes and A. tenuicaule. The quantitative characteristics of the leaves showed values intermediate between the two parental species. The absence of gemma accorded with its paternal origin from A. tenuicaule, and 32 spores per sporangium accorded with its maternal origin from A. capillipes. Although A. pseudocapillipes has 32 spores per sporangium, it is considered to be a sexually reproducing, not an apomitic, fern. [ABSTRACT FROM AUTHOR]

In this paper, a numerical investigation was carried out on the performances of a designed axial flow pump for a large cavitation tunnel. From this, the flow characteristics, force, and torque performance of the axial flow pump were investigated, and the rotating speeds of the impeller satisfying the test section speed performances required in the large cavitation tunnel were estimated. The axial flow pump was modeled such that the impeller, stator, and nacelle were located in a cylindrical tunnel. The calculations were carried out for incompressible steady-state turbulent flow considering the impeller rotating. The performance of the pump was confirmed, finding that the head gain was caused by the pressure jump downstream of the pump. The performance of the stator was confirmed to be good enough to refine the tangential flow due to the impeller rotating. To investigate the operating performance of the large cavitation tunnel, the head loss of the entire tunnel without the pump was obtained from a numerical analysis. The operating points were estimated from the specific speed–head coefficient curves, and it was found that the present numerical results were in good agreement with the experiments. Air pollution-related particulate matter (PM) exposure reportedly enhances allergic airway inflammation. Some studies have shown an association between PM exposure and a risk for allergic rhinitis (AR). However, the effect of PM for AR is not fully understood. An AR mouse model was developed by intranasal administration of 100 μg/mouse PM with a less than or equal to 2.5 μm in aerodynamic diameter (PM2.5) solution, and then by intraperitoneal injection of ovalbumin (OVA) with alum and intranasal challenging with 10 mg/mL OVA. The effects of PM2.5 on oxidative stress and inflammatory response via the Nrf2/NF-κB signaling pathway in mice with or without AR indicating by histological, serum, and protein analyses were examined. PM2.5 administration enhanced allergic inflammatory cell expression in the nasal mucosa through increasing the expression of inflammatory cytokine and reducing the release of Treg cytokine in OVA-induced AR mice, although PM2.5 exposure itself induced neither allergic responses nor damage to nasal and lung tissues. Notably, repeated OVA-immunization markedly impaired the nasal mucosa in the septum region. Moreover, AR with PM2.5 exposure reinforced this impairment in OVA-induced AR mice. Long-term PM2.5 exposure strengthened allergic reactions by inducing the oxidative through malondialdehyde production. The present study also provided evidence, for the first time, that activity of the Nrf2 signaling pathway is inhibited in PM2.5 exposed AR mice. Furthermore, PM2.5 exposure increased the histopathological changes of nasal and lung tissues and related the inflammatory cytokine, and clearly enhanced PM2.5 phagocytosis by alveolar macrophages via activating the NF-κB signaling pathway. Although ubiquitination is widely assumed to be the only regulated step in the ubiquitin–proteasome pathway, recent studies have demonstrated several important mechanisms that regulate the activities of the 26S proteasome. Most proteasomes in cells are inactive but, upon binding a ubiquitinated substrate, become activated by a two-step mechanism requiring an association of the ubiquitin chain with Usp14 and then a loosely folded protein domain with the ATPases. The initial activation step is signaled by Usp14's UBL domain, and many UBL-domain-containing proteins (e.g., Rad23, Parkin) also activate the proteasome. ZFAND5 is a distinct type of activator that binds ubiquitin conjugates and the proteasome and stimulates proteolysis during muscle atrophy. The proteasome's activities are also regulated through subunit phosphorylation. Agents that raise cAMP and activate PKA stimulate within minutes Rpn6 phosphorylation and enhance the selective degradation of short-lived proteins. Likewise, hormones, fasting, and exercise, which raise cAMP, activate proteasomes and proteolysis in target tissues. Agents that raise cGMP and activate PKG also stimulate 26S activities but modify different subunit(s) and stimulate also the degradation of long-lived cell proteins. Both kinases enhance the selective degradation of aggregation-prone proteins that cause neurodegenerative diseases. Ferns have conspicuous sporophytes as the dominant phase in their life cycle; however, the gametophytes are completely separated from the sporophytes and supply their own nutrition, unlike in bryophytes and seed plants. Among the gametophytes, some maintain their populations in the gametophyte phase without progressing to sporophyte production and are known as independent gametophytes. Independent gametophytes of Antrophyum obovatum Baker were recently reported in one population on Jeju Island, Korea. In the present study, we surveyed more places to find new independent gametophyte populations of A. obovatum using the rbcL gene sequence-based DNA barcoding technique. We identified two new sites inhabited by independent gametophytes. Archegonia and juvenile sporophytes were independently observed in each location under slightly different environmental conditions. Consequently, in the case of this species, functional sporophyte production is likely suppressed by prezygotic and postzygotic sterility, depending on microenvironmental factors. A series of experiments were performed in the SPARC (spray-aerosol-recombiner-combustion) test facility to simulate a hydrogen mitigation system with the actuation of a PAR (passive auto-catalytic re-combiner) and spray system. In this study, the SPARC-SPRAY-PAR (SSP1) experiment is chosen to benchmark the MELCOR (a lumped-parameter code for severe accident analysis) predictions against test data. For this purpose, firstly we prepared the base input model of the SPARC test vessel, and tested it by a simple verification problem with well-defined boundary conditions. The implementation of a currently used PAR correlation in MELCOR is shown to be appropriate for the simulation of a PAR actuation experiment. In an SSP1 experiment, the PAR is reacting with hydrogen, and the spray actuation starts as soon as hydrogen injection is complete. The MELCOR simulation well predicts the pressure behavior and the gas flow affected by operating both a PAR and spray system. However, the local hydrogen concentration measurement near the inlet nozzle is much higher than the volume average-value by MELCOR, since high jet flow from the nozzle is dispersed in the corresponding cell volume. The experimental reproduction of the phenomena we expect, or, conversely, the identification of phenomena we do not understand, will continue to support the verification of analytical models using experimental data and to analyze the impact of spray on PAR operations in severe accident conditions. In this paper, a method that utilizes the reinforcement learning (RL) technique is proposed to establish an optimal operation plan to obtain maximum power output from a trigen generator. Trigen is a type of combined heat and power system (CHP) that provides chilling, heating, and power generation, and the turbo expander generator (TEG) is a generator that uses the decompression energy of gas to generate electricity. If the two are combined to form a power source, a power generation system with higher efficiency can be created. However, it is very difficult to control the heat and power generation amount of TEG and trigen according to the flow rate of natural gas that changes every moment. Accordingly, a method is proposed to utilize the RL technique to determine the operation process to attain an even higher efficiency. When the TEG and trigen are configured using the RL technique, the power output can be maximized, and the power output variability can be reduced to obtain high-quality power. The genus Lilium L. is widely distributed in the cold and temperate regions of the Northern Hemisphere and is one of the most valuable plant groups in the world. Regarding the classification of the genus Lilium, Comber's sectional classification, based on the natural characteristics, has been primarily used to recognize species and circumscribe the sections within the genus. Although molecular phylogenetic approaches have been attempted using different markers to elucidate their phylogenetic relationships, there still are unresolved clades within the genus. In this study, we constructed the species tree for the genus using 28 Lilium species plastomes, including three currently determined species (L. candidum, L. formosanum, and L. leichtlinii var. maximowiczii). We also sought to verify Comber's classification and to evaluate all loci for phylogenetic molecular markers. Based on the results, the genus was divided into two major lineages, group A and B, consisting of eastern Asia + Europe species and Hengduan Mountains + North America species, respectively. Sectional relationships revealed that the ancestor Martagon diverged from Sinomartagon species and that Pseudolirium and Leucolirion are polyphyletic. Out of all loci in that Lilium plastome, ycf1, trnF-ndhJ, and trnT-psbD regions are suggested as evaluated markers with high coincidence with the species tree. Three-dimensional moderator flow in the calandria tank of CANDU-6 pressurized heavy water reactor (PHWR) is computed with Open Field Operation and Manipulation (OpenFOAM), an open-source computational fluid dynamics (CFD) code. In this study, numerical analysis is performed on the real geometry model including 380 fuel rods in the calandria tank with the heat-source distribution to remove uncertainty of the previous analysis models simplified by the porous media approach. Realizable k-ε turbulence model is applied, and the buoyancy due to temperature variation is considered by Boussinesq approximation for the incompressible single-phase Navier-Stokes equations. The calculation results show that the flow is highly unsteady in the moderator. The computational flow visualization shows a circulation of flow driven by buoyancy and asymmetric oscillation at the pseudo-steady state. There is no region where the local temperature rises continuously due to slow circulating flow and its convection heat transfer. [ABSTRACT FROM AUTHOR]

Valeriana sambucifolia f. dageletiana (Nakai. ex Maekawa) Hara is a broad-leaved valerian endemic to Ulleung Island, a noted hot spot of endemism in Korea. However, despite its widespread pharmacological use, this plant remains comparatively understudied. Plant cells generally contain two types of organellar genomes (the plastome and the mitogenome) that have undergone independent evolution, which accordingly can provide valuable information for elucidating the phylogenetic relationships and evolutionary histories of terrestrial plants. Moreover, the extensive mega-data available for plant genomes, particularly those of plastomes, can enable researchers to gain an in-depth understanding of the transfer of genes between different types of genomes. In this study, we analyzed two organellar genomes (the 155,179 bp plastome and the 1,187,459 bp mitogenome) of V. sambucifolia f. dageletiana and detected extensive changes throughout the plastome sequence, including rapid structural mutations associated with inverted repeat (IR) contraction and genetic variation. We also described features characterizing the first reported mitogenome sequence obtained for a plant in the order Dipsacales and confirmed frequent gene transfer in this mitogenome. We identified eight non-plastome-originated regions (NPRs) distributed within the plastome of this endemic plant, for six of which there were no corresponding sequences in the current nucleotide sequence databases. Indeed, one of these unidentified NPRs unexpectedly showed certain similarities to sequences from bony fish. Although this is ostensibly difficult to explain, we suggest that this surprising association may conceivably reflect the occurrence of gene transfer from a bony fish to the plastome of an ancestor of V. sambucifolia f. dageletiana mediated by either fungi or bacteria.
