In order to reduce greenhouse gas emissions associated with power generation, the replacement of fossil fuels with renewables must be accompanied by the availability of dispatchable sources needed to balance electricity demand and production. Combined cycle (CC) power plants adopting post-combustion capture (PCC) can serve this purpose, ensuring near-zero CO2 emissions at the stack, as well as high efficiency and load flexibility. In particular, the chemical absorption process is the most established approach for industrial-scale applications, although widespread implementation is lacking. In this study, different natural gas combined cycle (NGCC) configurations were modeled to estimate the burden of retrofitting the capture process to existing power plants on thermodynamic performance. Simulations under steady-state conditions covered the widest possible load range, depending on the gas turbine (GT) model. Attention was paid to the net power loss and net efficiency penalty attributable to PCC. The former can be mitigated by lowering the GT air–fuel ratio to increase the CO2 concentration (XCO2) in the exhaust, thus decreasing the regeneration energy. The latter is reduced when the topping cycle is more efficient than the bottoming cycle for a given GT load. This is likely to be the case in the less-complex heat recovery units. Achieving a high participation rate is a common challenge in healthcare research based on web-based surveys. A study on local anesthetic systemic toxicity awareness and usage among medical practitioners at two Swiss university hospitals encountered resistance in obtaining personal email addresses from Heads of Departments. Participants were therefore divided into two groups: those who were directly invited via email (personal invitation group) and those who received a generic link through intermediaries (generic link group). The latter group was eventually excluded from survey data analysis. To determine whether one method of survey administration was more effective than another, we carried out a retrospective analysis of response rates and the proportion of new questionnaires completed after initial invitation and subsequent reminders. The results showed significantly higher response rates in the personal invitation group (40.2%, 313/779) compared to the generic link group (25.3%, 22/87), emphasizing the effectiveness of personal invitations on response rate (+14.9%, p = 0.007). The personal invitation group consistently yielded a higher number of completed questionnaires following the initial invitation and each reminder. The method of survey administration can greatly influence response rates and should be acknowledged as a quality criterion when conducting web-based surveys. [ABSTRACT FROM AUTHOR]

Introduction: Some patients exhibit temporomandibular joint or muscular disorders of the masticatory system before, during, or after orthognathic surgery (OS). These are collectively referred to as temporomandibular disorders (TMDs). This systematic literature review aimed to determine the relationship between orthodontic-surgical treatment and TMDs. Methods: An electronic search of the PubMed database, supplemented by a manual search, was performed; the search included any studies published between 2021 (date of the last search in a systematic review of the literature on the subject) and June 2023 that evaluate the prevalence of TMDs during orthodontic-surgical treatment. The diagnosis of TMDs had to be established using the diagnostic algorithm "diagnostic criteria for temporomandibular disorders (DC/TMDs)", and the diagnosis of disc displacement had to be confirmed using magnetic resonance imaging (MRI). The data were extracted and statistically analyzed. Results: Of the 100 results, seven eligible articles were included, representing a total of 529 cases undergoing orthodontic-surgical treatment. A reduction in joint noises (64.8%), arthralgia (57 to 77%), and myalgia (73 to 100%) was found after orthodontic-surgical treatment despite the fact that a minority of patients exhibited these signs and symptoms even though they were asymptomatic before treatment. The effects of OS on disc position were objectively unpredictable. After surgery, the presence of headaches decreased without significance and the risk of their occurrence was very low (1%). The studies converged toward a reduction in the amplitudes of mouth opening and lateral/protrusion movements. Finally, after the treatment, mandibular function was improved. Conclusion: Under the conditions of this study, OS seems to have a positive impact on the signs and symptoms of TMDs; however, it is not possible to predict the consequential effects on the position of the TMJ disc, whether it is initially in a normal position or displaced. (1) Background: Accurate knowledge of the dynamic anatomy of facial muscles is crucial for the use of functional and aesthetic botulinum toxin injections. We studied the reliability and relevance of facial painting as a pedagogic tool for the dynamic anatomy of facial muscles. (2) Methods: Different facial expressions were performed by a female model after a professional makeup artist applied makeup to the various facial muscles on her left hemiface. A 3D photograph was taken at the beginning and end of each movement using the VECTRA H2 Imaging System device. Cutaneous movements were visualized using displacement vectors. The correlation between the theoretical and dynamic positions of the makeup–muscle was assessed by two facial anatomy experts, thanks to a correlation scale. (3) Results: The overall average score for the 11 analyzed muscles or muscle groups was 3.36 out of 4, indicating a "strong" to "very strong" estimated correlation. There was a moderate agreement between Evaluator 1 and Evaluator 2 (ICC: 0.64; 95%CI: [0.244; 0.852]; p-value: 0.005). (4) Conclusions: The educational model with facial makeup provides an indirect but nonetheless precise and reliable representation of all facial muscles on the skin's surface. A double-lumen tube or bronchial blocker positioning using flexible bronchoscopy for lung isolation and one-lung ventilation requires specific technical competencies. Training to acquire and retain such skills remains a challenge in thoracic anesthesia. Recent technological and innovative developments in the field of simulation have opened up exciting new horizons and possibilities. In this narrative review, we examine the latest development of existing training modalities while investigating, in particular, the use of emergent techniques such as virtual reality bronchoscopy simulation, virtual airway endoscopy, or the preoperative 3D printing of airways. The goal of this article is, therefore, to summarize the role of existing and future applications of training models/simulators and virtual reality simulators for training flexible bronchoscopy and lung isolation for thoracic anesthesia. The significant consumption of resources within the healthcare sector underscores the need to address both efficiency and sustainability concerns. Telemedicine has been identified as one of the most promising pathways for reducing the environmental impacts of the healthcare sector. However, a comprehensive sustainability assessment is still required. The main aim of the present study is to conduct a systematic literature review to explore approaches and methodologies employed for quantifying the environmental, social, and economic impacts of telemedicine. Moreover, the research seeks to determine whether the approaches focus on a single aspect or whether they allow for a comprehensive assessment including all three sustainability pillars. The searching phase was conducted in the Scopus and PubMed databases, considering last 10 years (i.e., 2013–2023). Keywords were related to remote care and sustainability impact fields. Following the PRISMA framework, out of 477 articles, 91 studies were included in the analysis. Primary findings highlighted that studies on telemedicine impacts predominantly focus on transport-related aspects, emphasizing direct emissions and associated costs that are avoided and time savings. Televisit emerged as the most investigated remote care activity. Database and conversion factors were mainly employed for analysis, while other methodologies were sporadically mentioned in the literature. Despite numerous papers addressing these issues, a standardized and comprehensive methodology still appears to be lacking. Future works should consider the entire life cycle process, including more stakeholders. Based on the evidence of a significant communication and connection pathway between the bone and immune systems, a new science has emerged: osteoimmunology. Indeed, the immune system has a considerable impact on bone health and diseases, as well as on bone formation during grafts and its stability over time. Chronic inflammation induces the excessive production of oxidants. An imbalance between the levels of oxidants and antioxidants is called oxidative stress. This physio-pathological state causes both molecular and cellular damage, which leads to DNA alterations, genetic mutations and cell apoptosis, and thus, impaired immunity followed by delayed or compromised wound healing. Oxidative stress levels experienced by the body affect bone regeneration and maintenance around teeth and dental implants. As the immune system and bone remodeling are interconnected, bone loss is a consequence of immune dysregulation. Therefore, oral tissue deficiencies such as periodontitis and peri-implantitis should be regarded as immune diseases. Bone management strategies should include both biological and surgical solutions. These protocols tend to improve immunity through antioxidant production to enhance bone formation and prevent bone loss. This narrative review aims to highlight the relationship between inflammation, oxidation, immunity and bone health in the oral cavity. It intends to help clinicians to detect high-risk situations in oral surgery and to propose biological and clinical solutions that will enhance patients' immune responses and surgical treatment outcomes. Simple Summary: In the present work, we considered areas located in a park of 4000 ha in northern Italy in a suburban area, with an altitude between 190 and 960 m asl. Our aim was to update the list of insect species and to evaluate the influence of habitat and the locality on the beetle fauna present in the park. We therefore monitored the insects using different collection techniques. A total of 409 different species were collected. Species in Coleoptera were the most abundant, followed by Lepidoptera and Hymenoptera, with other orders in smaller numbers. The analysis showed that the habitat influenced the communities more than the locality. Unfortunately, as there is no detailed historical information on the presence of the species in the area, we cannot hypothesize an effect of the area on the communities. This research, therefore, allowed us to list numerous species for the area, among which some are important, as they were never detected in this area, and constitutes a new starting point for future research on the biodiversity in the prealpine areas. Human activities and habitat fragmentation are known to greatly influence biodiversity. The aim of this study was to update an entomological checklist of a prealpine area in Italy, and also to evaluate the influence of different habitats and the proximity to cities on the entomological fauna. This study included different areas of a local park in Northern Italy, covering about 4000 ha, and situated at altitudes between 190 and 960 m asl. The surveys were carried out between 2010 and 2013 using different monitoring techniques (pitfall traps, car mounted nets, light traps, direct catches on soil and vegetation, visual sampling, gall collection). Furthermore, to assess the effect of habitat and locality on the composition of epigeic beetles, pitfall traps were set and inspected from April to September. All captured specimens were classified to species level. A total of 409 species were recorded, belonging to 7 orders and 78 families. A total of 76.1% were represented by Coleoptera, 13% Lepidoptera, 9.4% Hymenoptera, followed by other orders. In particular, some species with peculiar characteristics, or whose presence in the area had not been previously reported, were detected, such as Atheta pseudoelongatula, Ocypus rhaeticus, Tasgius tricinctus, Euplagia quadripunctaria, Scotopteryx angularia, Elachista constitella, Parornix bifurca, Oegoconia huemeri, and Lasius (Lasius) alienus. It seems possible that the habitat affected the community more than the locality. The woods showed a reduced biodiversity, and a simplified community structure. Given the vast scholarly attention paid to multilingualism on traditional media over the years, it seems timely to focus on streaming platforms. This paper sets out to identify potential norms for the rendering of multilingual occurrences in the localised content of Netflix series. It also seeks to explore whether streaming translation practices related to multilingualism differ from the consolidated norms and practices for TV and cinema content. The chosen data sample consists of the Italian dub streams of five TV Netflix-produced shows featuring multilingualism as a main characteristic. The strategies and techniques adopted in each series are singled out, quantified, and labelled according to a combination of taxonomies. These include dubbing, revoicing, subtitling, part-subtitling, diegetic interpreting, unchanged speech transfer, and no translation. A wider analysis is also carried out across all the data sample to draw patterns on a macro level. The findings reveal a strong tendency to mark and preserve multilingualism, in line with Netflix's own policies and dubbing specifications. Transfer unchanged combined with subtitles emerges as the most recurrent strategy, while the dub-over strategy accounts for 13% of the multilingual occurrences in the data sample. Extensive neutralisation is therefore not encountered. That said, a certain degree of overlap between multilingual translation norms on Netflix and conventional Italian dubbing practices (which tend to neutralise) can still be observed. Background: When maxillary transversal expansion is needed, two protocols of treatment can be used: a maxillary orthodontic expansion followed by a classical bimaxillary osteotomy or a bimaxillary osteotomy with maxillary segmentation. The aim of this study was to assess the accuracy of segmented Le Fort I osteotomy using computer-aided orthognathic surgery and patient-specific titanium plates in patients who underwent a bimaxillary osteotomy for occlusal trouble with maxillary transversal insufficiencies. Methods: A virtual simulation of a Le Fort I osteotomy with maxillary segmentation, a sagittal split ramus osteotomy, and genioplasty (if needed) was conducted on a preoperative three-dimensional (3D) model of each patient's skull using ProPlan CMF 3.0 software (Materialise, Leuven, Belgium). Computer-assisted osteotomy saw-and-drill guides and patient-specific implants (PSIs, titanium plates) were produced and used during the surgery. We chose to focus on the maxillary repositioning accuracy by comparing the preoperative virtual surgical planning and the postoperative 3D outcome skulls using surface superimpositions and 13 standard dental and bone landmarks. Errors between these preoperative and postoperative landmarks were calculated and compared to discover if segmental maxillary repositioning using PSIs was accurate enough to be safely used to treat transversal insufficiencies. Results: A total of 22 consecutive patients—15 females and 7 males, with a mean age of 27.4 years—who underwent bimaxillary computer-assisted orthognathic surgery with maxillary segmentation were enrolled in the study. All patients presented with occlusion trouble, 13 with Class III malocclusions (59%) and 9 (41%) with Class II malocclusions. A quantitative analysis revealed that, overall, the mean absolute discrepancies for the x-axis (transversal dimension), y-axis (anterior–posterior dimensions), and z-axis (vertical dimension) were 0.59 mm, 0.74 mm, and 0.56 mm, respectively. The total error rate of maxillary repositioning was 0.62 mm between the postoperative cone-beam computed tomography (CBCT) and the preoperatively planned 3D skull. According to the literature, precision in maxilla repositioning is defined by an error rate (clinically relevant) at each landmark of <2 mm and a total error of <2 mm for each patient. (1) Background: Persistent post-viral olfactory disorders (PPVOD) are estimated at 30% of patients one year after COVID-19 infection. No treatment is, to date, significantly effective on PPVOD with the exception of olfactory training (OT). The main objective of this work was to evaluate OT efficiency on post-COVID-19 PPVOD. (2) Methods: Consecutive patients consulting to the ENT department with post-COVID-19 PPVOD were included after completing clinical examination, the complete Sniffin' Stick Test (TDI), the short version of the Questionnaire of olfactory disorders and the SF-36. Patients were trained to practice a self-olfactory training with a dedicated olfactory training kit twice a day for 6 months before returning to undergo the same assessments. (3) Results: Forty-three patients were included and performed 3.5 months of OT in average. We observed a significant TDI score improvement, increasing from 24.7 (±8.9) before the OT to 30.9 (±9.8) (p < 0.001). Based on normative data, a significant increase in the number of normosmic participants was observed only for the threshold values (p < 0.001). Specific and general olfaction-related quality of life improved after the OT. (4) Conclusions: Olfactory function appeared to improve only in peripheral aspects of post-COVID-19 PPVOD after OT. Simple Summary: Considering the chemical composition of cricket powder, specific nutritional goals (such as high protein content) can be achieved by including cricket powder, even at low levels (i.e., 5%). However, when formulating new food products, it is worth assessing the functional properties of the new ingredient and/or formulation. Thus, the aim of this study was to assess the technological properties of cricket-enriched wheat flours to be used in the development of staple foods, such as bread or pasta, whose features are also discussed. The recent socio-economic situation requires producers to change the composition of basic foods. The aim of this study was to assess the technological properties of wheat flour enriched with cricket powder (CP) (at 5%, 10%, and 20% levels) for the development of bread and pasta. The hydration (i.e., water absorption capacity, oil absorption capacity, water absorption index, water solubility index, and swelling power), foaming (i.e., foaming capacity and stability),emulsifying (emulsifying activity and emulsion stability), and rheological (during gluten aggregation, mixing, extension, and leavening) properties were investigated. Finally, bread and fresh pasta were prepared and characterized. Emulsifying activity, stability, and foaming capacity decreased in the presence of CP, whereas foaming stability and water solubility increased. The results on dough rheology highlighted the need to increase the amount of water, and to decrease the mixing and leavening time, to keep an acceptable bread volume. Indeed, 10% CP enrichment led to a product characterized by a similar volume and crumb hardness to the control (wheat flour). Despite the decrease in extensibility caused by CP, it was possible to produce fresh pasta enriched with CP, with the best cooking behavior obtained at a 5% replacement level. [ABSTRACT FROM AUTHOR]

Prehospital endotracheal intubation (ETI) can be challenging, and the risk of complications is higher than in the operating room. The goal of this study was to compare prehospital ETI rates between anaesthesiologists and non-anaesthesiologists. This retrospective cohort study compared prehospital interventions performed by either physicians from the anaesthesiology department (ADP) or physicians from another department (NADP, for non-anaesthesiology department physicians). The primary outcome was the prehospital ETI rate. Overall, 42,190 interventions were included in the analysis, of whom 68.5% were performed by NADP. Intubation was attempted on 2797 (6.6%) patients, without any difference between NADPs and ADPs (6.5 versus 6.7%, p = 0.555). However, ADPs were more likely to proceed to an intubation when patients were not in cardiac arrest (3.4 versus 3.0%, p = 0.026), whereas no difference was found regarding cardiac arrest patients (65.2 versus 67.7%, p = 0.243) (p for homogeneity = 0.005). In a prehospital physician-staffed emergency medical service, overall ETI rates did not depend on the frontline operator's medical specialty background. ADPs were, however, more likely to proceed with ETI than NADPs when patients were not in cardiac arrest. Further studies should help to understand the reasons for this difference. Local anesthetics systemic toxicity can lead to life-threatening situations. Correct calculation of the maximum safe dose is therefore paramount in preventing such complications. Different solutions have already emerged to support anesthesiologists but are seldom used in clinical practice as they require either access to a computer or specific documents to be at hand. A mobile app could provide an easy and practical solution; however, the few apps already created for this purpose often lack key elements, allowing invalid data to be entered and suggesting doses that might exceed the maximum safe dose. We describe the development of LoAD Calc, a mobile health (mHealth) app developed using a modified version of the Information Systems Research framework, which adds design thinking modes to the original framework. The app was enhanced through multiple iterations and developed with the aid of contextual observations and interviews, brainswarming sessions, prototyping, and continuous feedback. The design process led to the creation of two prototypes which underwent thorough testing by a sample of eight anesthesiologists. The final version of the app, LoAD Calc, was deployed on Apple and Android mobile test platforms and tested again by the same sample until deemed fit for release. [ABSTRACT FROM AUTHOR]

Simple Summary: Insect rearing and processing are widely studied to provide solutions to the increasing demand for proteins caused by a growing human population. It has been demonstrated that insect meal can be introduced into the feed ratio of livestock and its exploitation can limit the environmental impact of animal husbandry. When their ecological footprint is analyzed, insects are considered "biorefineries" able to generate multiple economic outputs for the food industry. Fat and antimicrobial peptides contained in the insect oil can represent important resources to treat animal diseases, e.g., allowing to reduce antibiotic overuse. In this paper we focused on two insects exploitable both for protein and oil production: the black soldier fly larvae (BSFL) and the domestic silkworm. As the diet regimen remarkably affects the fat composition of the insects, both BSFL and silkworm larvae were reared on plant-based diets. Silkworms were fed mulberry leaves, while BSFL were reared on a diet composed of leftover vegetables and fruit, in the perspective of their bioconversion and valorization. The best technique to maximize the oil extraction yield was defined for BSFL and applied to the silkworm pupae. An antimicrobial activity of the oil against some bacterial species was demonstrated in both insects and compared. The aim of this work was to develop processing methods that safeguard the quality and antimicrobial properties of H. illucens and B. mori oils. We adopted a vegetable diet for both insects: leftover vegetables and fruit for H. illucens and mulberry leaves for B. mori. First, alternative techniques to obtain a good oil extraction yield from the dried biomass of H. illucens larvae were tested. Traditional pressing resulted to be the best system to maximize the oil yield and it was successfully applied to B. mori pupae. Oil quality resulted comparable to that obtained with other extraction methods described in the literature. In the case of B. mori pupae, different treatments and preservation periods were investigated to evaluate their influence on the oil composition and quality. Interestingly, agar diffusion assays demonstrated the sensitivity of Gram-positive Bacillus subtilis and Staphylococcus aureus to H. illucens and B. mori derived oils, whereas the growth of Gram-negative Pseudomonas aeruginosa and Escherichia coli was not affected. This study confirms that fat and other active compounds of the oil extracted by hot pressing could represent effective antimicrobials against bacteria, a relevant result if we consider that they are by-products of the protein extraction process in the feed industry. Simple Summary: Natural enemies play a fundamental role in the control of invasive insects and may be particularly important in controlling Xylopagous beetle, which are difficult to contain with traditional chemicals as they are usually concealed within wood. Among the natural enemies that have proven effective against these beetles are parasitoid species in the genus Sclerodermus. The present article furthers knowledge of Sclerodermus brevicornis performance, focusing on the survival capability under different thermal conditions and the longer-term influences of these conditions. Many species of long-horned beetles are invasive pests causing significant economic damage in agro-forestry systems. They spend the majority of their life-cycle concealed inside natural wood or wooden packaging materials and are largely protected from adverse environmental conditions and pesticide sprays. Biological control via parasitoid natural enemies including members of the bethylid genus Sclerodermus, has proven effective against some long-horned beetles that are invasive in China. In Europe, the biocontrol potential of native Sclerodermus species is being evaluated with a view to developing efficient mass-rearing techniques and then actively deploying them against invasive Asian beetles. Here, we continue evaluations of S. brevicornis by establishing that groups of females that have already reared offspring to emergence are capable of reproducing subsequent hosts and by evaluating the lifetime reproductive capacity of individual females provided with successive hosts. Additionally, we assess the laboratory shelf-life of adult females stored for different times at different temperatures including cold storage, and then assess the post-storage reproductive performance of groups of females provided with a single host. We found that adult female longevity declines with increasing storage temperature and that most aspects of subsequent performance are negatively affected by high temperatures. The adaptability to low temperature storage enhances the suitability of S. brevicornis to mass-rearing programs and thus biocontrol deployment. [ABSTRACT FROM AUTHOR]

Simple Summary: Necrobia rufipes (Coleoptera: Cleridae) is an emerging pest of pet food stores. Information on infestation modalities for this pest is absent and specific monitoring tools are missing. In this paper, the adults' and larvae's ability to enter into pet food packaging was evaluated. Furthermore, to set up of a monitoring trap behavioral bioassays were carried out: testing two adhesive surfaces, one generally used in mouse glue trap and the other used in cockroach trap, to evaluate their ability in avoiding insects' escape; screening different molecules, as candidate food attractants: methyl cyclopentenolone, squalene and stearic acid. The results evidenced that N. rufipes enter in packaging through the air vent valves, suggesting that a way to prevent insect infestation would be to modify packaging. Tests showed that the glues have strong differences in the ability to retain the caught insects, with mouse glue more effective than cockroach glue. The behavioral bioassays indicated that methyl cyclopentenolone and squalene are able to attract N. rufipes adults in olfactometer. Finally, the dual-choice arena bioassays showed that a mixture of pet food and methyl cyclopentenolone elicited the strongest attraction in N.rufipes adults, suggesting that this mixture can be used as lure in monitoring traps. Necrobia rufipes (De Geer) (Coleoptera: Cleridae), also known as the red-legged ham beetle, is a newly emerging pest of pet food stores, causing apprehension among producers worldwide. Concerns about this pest are exacerbated by the lack of information about infestation modalities in pet food, while specific monitoring tools are missing. Considering that adequate pet food packaging could limit N. rufipes infestations, information about the penetration modalities in commonly used pet food packaging is needed. Moreover, the development of appropriate monitoring instruments is urgent to detect pest presence early and to reduce chemical treatments for its control. In this paper, the adults' and larvae's ability to enter into pet food packaging was evaluated. Furthermore, to develop monitoring traps, behavioral bioassays were done: (1) testing two different commercial adhesive surfaces, one generally used in mouse glue traps (MG), and the other used in cockroach glue traps (CG), to evaluate their different abilities in avoiding insects' escape; (2) screening different molecules, typical of the substrates attacked by N. rufipes, as candidate food attractants for this pest: methyl cyclopentenolone (MCP), squalene (SQ), and stearic acid (SA). The results show that N. rufipes adults and larvae enter into packaging through the air vent valves on the bottom, suggesting that a way to improve the packaging to prevent insect infestation would be to modify these points of weakness. Laboratory tests show that the different bioassayed glues have strong differences in the ability to retain the caught insects, with MG being more effective than CG. The behavioral bioassay indicated that MCP and SQ attract N. rufipes adults in olfactometer. Finally, the results of dual-choice arena bioassays show that among the candidate attractant tested, a mixture of pet food (PF) and MCP elicited the strongest attraction in N. rufipes adults. These results encourage further experiments with the use of an MG adhesive trap loaded with a mixture of PF+MCP to test the effectiveness of such a tool for monitoring N.rufipes in pet food industries and warehouses. Hermetia illucens is an efficient bioconverter able to grow on various different organic materials, producing larvae, which are a good source of protein and fat with applications in the animal feed and biochemical industries. This fly's capacity to reduce huge amounts of waste presents an interesting opportunity to establish a circular food economy. In this study, we assessed the suitability of using organic wastes from cricket and locust farming to rear H. illucens. Larvae developed until adult emergence on all the wastes, with a mean survival of over 94%. Cricket waste allowed faster development of heavier larvae than locust waste. Substrate reduction was particularly interesting on cricket waste (<72%), while locust waste was only reduced by 33%. The nutritional composition of the larvae reflected that of the growing substrates with a high protein and fat content. These results demonstrate the potential of using H. illucens to reduce and valorise waste generated when farming various insects through the production of a larval biomass for use as a protein meal in animal feeds or industrial applications. Objectives: In the prehospital setting, endotracheal intubation (ETI) is sometimes required to secure a patient's airways. Emergency ETI in the field can be particularly challenging, and success rates differ widely depending on the provider's training, background, and experience. Our aim was to evaluate the ETI success rate in a resident-staffed and specialist-physician-supervised emergency prehospital system. Methods: This retrospective study was conducted on data extracted from the Geneva University Hospitals' institutional database. In this city, the prehospital emergency response system has three levels of expertise: the first is an advanced life-support ambulance staffed by two paramedics, the second is a mobile unit staffed by an advanced paramedic and a resident physician, and the third is a senior emergency physician acting as a supervisor, who can be dispatched either as backup for the resident physician or when a regular Mobile Emergency and Resuscitation unit (Service Mobile d'Urgence et de Réanimation, SMUR) is not available. For this study, records of all adult patients taken care of by a second- and/or third-level prehospital medical team between 2008 and 2018 were screened for intubation attempts. The primary outcome was the success rate of the ETI attempts. The secondary outcomes were the number of ETI attempts, the rate of ETI success at the first attempt, and the rate of ETIs performed by a supervisor. Results: A total of 3275 patients were included in the study, 55.1% of whom were in cardiac arrest. The overall ETI success rate was 96.8%, with 74.4% success at the first attempt. Supervisors oversaw 1167 ETI procedures onsite (35.6%) and performed the ETI themselves in only 488 cases (14.9%). Conclusion: A resident-staffed and specialist-physician-supervised urban emergency prehospital system can reach ETI success rates similar to those reported for a specialist-staffed system. Meningitis, encephalitis, and myelitis are various forms of acute central nervous system (CNS) inflammation, which can coexist and lead to serious sequelae. Known aetiologies include infections and immune-mediated processes. Despite advances in clinical microbiology over the past decades, the cause of acute CNS inflammation remains unknown in approximately 50% of cases. High-throughput sequencing was performed to search for viral sequences in cerebrospinal fluid (CSF) samples collected from 26 patients considered to have acute CNS inflammation of unknown origin, and 10 patients with defined causes of CNS diseases. In order to better grasp the clinical significance of viral sequence data obtained in CSF, 30 patients without CNS disease who had a lumbar puncture performed during elective spinal anaesthesia were also analysed. One case of human astrovirus (HAstV)-MLB2-related meningitis and disseminated infection was identified. No other viral sequences that can easily be linked to CNS inflammation were detected. Viral sequences obtained in all patient groups are discussed. While some of them reflect harmless viral infections, others result from reagent or sample contamination, as well as index hopping. Altogether, this study highlights the potential of high-throughput sequencing in identifying previously unknown viral neuropathogens, as well as the interpretation issues related to its application in clinical microbiology. Simple Summary: In the last few years great efforts have been made to find alternative protein sources to soybean meal and valorize organic by-product, which are produced in large amounts by food industries and are often inedible by humans. In this context, insects could be an alternative protein source. The aims of this study were to evaluate the growth performance, nutrient composition, and environmental impact of Hermetia illucens reared on a control hen diet and by-products diets (okara, maize distillers, and brewer's grains). The rearing substrate affected larvae growth performance, nutritive value, and environmental impacts. Larvae fed on a hen diet and the maize distiller exhibited the higher final weight and required fewer days to reach the prepupal stage. The lipid content of the larvae was positively correlated to that of the insect diets. The environmental impact of larvae production on the hen diet, characterized by a high inclusion of soybean meal, was the most impactful for most of the environmental categories. Feed production activities were the main contributor to environmental impact. The use of by-products for larvae growth allowed us to reduce the environmental impact to produce 1 kg of protein in comparison with the use of soybean meal as a protein source. The aim of the study was to evaluate the use of three by-products as growing substrates for Hermetia illucens (Black Soldier Fly (BSF)) larvae: okara, maize distiller, brewer's grains, and a control hen diet. The study focused on larval growth and bioconversion performance, production of methane by larvae and environmental burden of larvae production, using Life Cycle Assessment (LCA) on a lab scale. Chemical composition of substrates differed: okara had the highest crude protein and ether extract contents, while brewer's grains showed the highest fiber content. Larvae fed on a hen diet and maize distiller exhibited the highest final weights (2.29 and 1.97 g, respectively). Larvae grown on okara showed the highest indexes for waste reduction and efficiency of conversion of the ingested feed. The BSF larvae did not produce any detectable traces of CH4. LCA evaluation showed that larvae production on a hen diet resulted in the most impact for most of environmental categories, for the inclusion of soybean meal in the diet (for climate change, 5.79 kg CO2 eq/kg dry larvae). Feed production activities resulted in the main contributions to environmental impact. In order to compare the larvae production obtained on all substrates, an environmental impact was attributed to okara and brewer's grain through a substitution method, and, by this approach, the best sustainable product resulted from the larvae grown on the maize distiller. Simple Summary: Protein production for animal husbandry is a crucial ecological problem because of its impact on the environment, as it requires water, energy, and land. These resources are limited and not reusable. In this study, we obtained a continuously regenerating system in which by-products of a process constituted rough material for another one. Leftovers from fruit and vegetable markets were employed as rearing substrate for insects (Black Soldier Fly). Insect biomass was transformed into meal and oil for fish feeding and food/pharmaceutical industry, respectively. The residuals from insect rearing were then used as substrate to grow earthworms, which transformed this material into compost for plants. Therefore, we returned to the starting point of our economic and ecological closed loop, i.e., to soil improvers (nutrient material) for fruit and vegetable production. Moreover, earthworms can be conveniently employed as fishing bites. We also studied a series of physiological parameters of the living organisms involved in this system to verify their health conditions (insects), and growth performances (insects and fish). Microbiological analyses of insects, rearing substrate, and insect meal were conducted to assess their safety for fish and humans. Related technological processes, such as insects drying, grinding, and oil extraction, were also tested. The worldwide growing consumption of proteins to feed humans and animals has drawn a considerable amount of attention to insect rearing. Insects reared on organic wastes and used as feed for monogastric animals can reduce the environmental impact and increase the sustainability of meat/fish production. In this study, we designed an environmentally closed loop for food supply in which fruit and vegetable waste from markets became rearing substrate for Hermetia illucens (BSF— black soldier fly). A vegetable and fruit-based substrate was compared to a standard diet for Diptera in terms of larval growth, waste reduction index, and overall substrate degradation. Morphological analysis of insect organs was carried out to obtain indications about insect health. Processing steps such as drying and oil extraction from BSF were investigated. Nutritional and microbiological analyses confirmed the good quality of insects and meal. The meal was then used to produce fish feed and its suitability to this purpose was assessed using trout. Earthworms were grown on leftovers of BSF rearing in comparison to a standard substrate. Chemical analyses of vermicompost were performed. The present research demonstrates that insects can be used to reduce organic waste, increasing at the same time the sustainability of aquaculture and creating interesting by-products through the linked bio-system establishment.
