In the pursuit of advancing COVID-19 diagnosis through imaging, this paper introduces a novel approach utilizing adaptive Fourier neural operators (AFNO) for the analysis of high-resolution computed tomography (HRCT) chest images. The study population comprised 395 patients with 181,106 labeled high-resolution COVID-19 CT images from the HRCTCov19 dataset, categorized into four classes: ground glass opacity (GGO), crazy paving, air space consolidation, and negative for COVID-19. The methods included image preprocessing, involving resizing and normalization, followed by the application of the AFNO model, which enables efficient token mixing in the Fourier domain independent of input resolution. The model was trained using the Adam optimizer with a learning rate of 1 × 10−⁴ and evaluated using metrics such as accuracy, precision, recall, and F1 score. The results demonstrate AFNO’s superior performance in few-shot segmentation tasks over traditional self-attention mechanisms, achieving an overall accuracy of 94%. Specifically, the model showed high precision and recall for the GGO and negative classes, indicating its robustness and effectiveness. This research has significant implications for the development of AI-powered diagnostic tools, particularly in environments with limited access to high-quality imaging data and those where computational efficiency is critical. Our findings suggest that AFNO could serve as a powerful model for analyzing HRCT images, potentially leading to improved diagnosis and understanding of COVID-19, representing a critical step in combating the pandemic.

Aim: Neurological manifestations are common in patients with chronic liver diseases. This study aimed to depict the association between liver cirrhosis and Parkinson’s disease (PD) and propose a clinically relevant diagnostic scheme. Methods: We examined patients’ medical records with PD and chronic liver impairment secondary to cirrhosis or liver metastases for temporal correlations between liver insult and Parkinsonian signs. Results: Thirty-five individuals with PD and chronic liver impairment were included due to either cirrhosis or liver metastases. In all 22 patients with PD and liver metastases, the diagnosis of PD preceded the diagnosis of cancer. Conversely, patients with cirrhosis were often diagnosed with liver impairment before diagnosing PD. Age at diagnosis did not account for this difference. Conclusions: This study reinforces the potential clinical association between cirrhosis and PD. We also provide a diagnostic scheme that may guide therapeutic interventions and prognostic assessments.

The relationship between the inverse Langevin function and the proposed r0-r1-Lambert W function is defined. The derived relationship leads to new approximations for the inverse Langevin function with lower relative error bounds than comparable published approximations. High accuracy approximations, based on Schröder’s root approximations of the first kind, are detailed. Several applications are detailed.

This paper presents the continuation of experimental investigations conducted by the present authors to measure and compare the thermal and fluid dynamic performance of a residential hydronic air coil using nanofluids. The prior experiments were limited to testing only one volumetric concentration (1%) of aluminum oxide (Al2O3) nanofluid. They compared it with the base fluid, a 60% ethylene glycol/40% water mixture by mass (60% EG). The original tests revealed some deficiencies in the experimental setup, which was subsequently revised and improved. This paper summarizes the results of experiments from the improved test bed using three concentrations of Al2O3 nanofluids: 1, 2, and 3% volumetric concentrations prepared with an average particle size of 45 nm in a 60% EG dispersion. The test bed in these experiments simulates a small air handling system typical of heating, ventilation, and air conditioning (HVAC) applications in cold regions. Entering conditions for the air and liquid were selected to emulate typical commercial air handling systems operating in cold climates. Contrary to previous findings, our test results revealed that nanofluids did not perform as well as expected. Prior predictions from many analytical and numerical studies had promised significant performance gain. The performance of the 1% nanofluid was generally equal to that of the base fluid under identical inlet conditions. However, the performance of the 2% and 3% nanofluids was considerably lower than that of the base fluid. The higher concentration nanofluids exhibited heat rates up to 14.6% lower than the 60% EG and up to 44.3% lower heat transfer coefficient. The 1% Al2O3/60% EG exhibited a 100% higher pressure drop across the coil than the base fluid, considering equal heat output. This performance degradation was attributed to the inability to maintain nanofluid dispersion stability, agglomeration, and subsequent decline in the thermophysical properties.

Application of lineament analysis in structural geology gained renewed interest when remote sensing data and technology became available through dedicated Earth observation satellites like Landsat in 1972. Lineament data have since been widely used in general structural investigations and resource and geohazard studies. The present contribution argues that lineament analysis remains a useful tool in structural geology research both at the regional and local scales. However, the traditional “lineament study” is only one of several methods. It is argued here that structural and lineament remote sensing studies can be separated into four distinct strategies or approaches. The general analyzing approach includes general structural analysis and identification of foliation patterns and composite structural units (mega-units). The general approach is routinely used by most geologists in preparation for field work, and it is argued that at least parts of this should be performed manually by staff who will participate in the field activity. We argue that this approach should be a cyclic process so that the lineament database is continuously revised by the integration of data acquired by field data and supplementary data sets, like geophysical geochronological data. To ensure that general geological (field) knowledge is not neglected, it is our experience that at least a part of this type of analysis should be performed manually. The statistical approach conforms with what most geologists would regard as “lineament analysis” and is based on statistical scrutiny of the available lineament data with the aim of identifying zones of an enhanced (or subdued) lineament density. It would commonly predict the general geometric characteristics and classification of individual lineaments or groups of lineaments. Due to efficiency, capacity, consistency of interpretation methods, interpretation and statistical handling, this interpretative approach may most conveniently be performed through the use of automatized methods, namely by applying algorithms for pattern recognition and machine learning. The focused and dynamic approaches focus on specified lineaments or faults and commonly include a full structural geological analysis and data acquired from field work. It is emphasized that geophysical (potential field) data should be utilized in lineament analysis wherever available in all approaches. Furthermore, great care should be taken in the construction of the database, which should be tailored for this kind of study. The database should have a 3D or even 4D capacity and be object-oriented and designed to absorb different (and even unforeseen) data types on all scales. It should also be designed to interface with shifting modeling tools and other databases. Studies of the Norwegian mainland have utilized most of these strategies in lineament studies on different scales. It is concluded that lineament studies have revealed fracture and fault systems and the geometric relations between them, which would have remained unknown without application of remote sensing data and lineament analysis.

Over the past decade, molecular cytopathology has emerged as a relevant area of modern pathology. Notably, in patients with advanced-stage cancer, cytological samples could be the only material available for diagnosis and molecular biomarker testing to identify patients suitable for targeted therapies. As a result, the contemporary cytopathologist’s role extends beyond morphological assessments to include critical skills such as evaluating the adequacy of the cytological samples and managing these specimens for molecular testing. This case collection can be a valuable source of insight, especially for young pathologists, who should learn to combine the opportunities offered by molecular biology with the basis of morphological evaluation.

Paddy (Oryza sativa) is one of the most consumed food grains in the world. The process from its sowing to consumption via harvesting, processing, storage and management require much effort and expertise. The grain quality of the product is heavily affected by the weather conditions, irrigation frequency, and many other factors. However, quality control is of immense importance, and thus, the evaluation of grain quality is necessary. Since it is necessary and arduous, we try to overcome the limitations and shortcomings of grain quality evaluation using image processing and machine learning (ML) techniques. Most existing methods are designed for rice grain quality assessment, noting that the key characteristics of paddy and rice are different. In addition, they have complex and expensive setups and utilize black-box ML models. To handle these issues, in this paper, we propose a reliable ML-based IoT paddy grain quality assessment system utilizing affordable sensors. It involves a specific data collection procedure followed by image processing with an ML-based model to predict the quality. Different explainable features are used for classifying the grain quality of paddy grain, like the shape, size, moisture, and maturity of the grain. The precision of the system was tested in real-world scenarios. To our knowledge, it is the first automated system to precisely provide an overall quality metric. The main feature of our system is its explainability in terms of utilized features and fuzzy rules, which increases the confidence and trustworthiness of the public toward its use. The grain variety used for experiments majorly belonged to the Indian Subcontinent, but it covered a significant variation in the shape and size of the grain.

Epstein–Barr virus-related post-transplantation lymphoproliferative disorder (EBV-PTLD) is a serious complication following hematopoietic stem cell transplantation (HSCT). A pre-emptive strategy using rituximab, which aims to manage patients early at the time of EBV reactivation to avoid PTLD, has been recommended by the most recent ECIL-6 guidelines in 2016. However, there is still a great heterogeneity of viral-load monitoring protocols, targeted patient populations, and pre-emptive treatment characteristics between centers, making precise EBV monitoring recommendations difficult. We conducted a literature review from the most recent publications between 1 January 2015 and 1 August 2023, to summarize the emerging data on EBV-PTLD prevention strategies in HSCT recipients, including the EBV-DNA threshold and use of rituximab. We also present the results of a survey of current practices carried out in 12 of the main HSCT centers across Canada. We confirm that pre-emptive rituximab remains an efficient strategy for EBV-PTLD prevention. However, there is an urgent need to perform prospective, randomized, multicentric trials with larger numbers of patients reflecting current practices to determine the best clinical conduct with regards to rituximab dosing, timing of treatment, and criteria to initiate treatments. Longer follow-ups will also be necessary to assess patients’ long-term outcomes.

Rolling shear in cross-laminated timber (CLT) has been identified as the governing factor influencing design value. Likewise, densification has been found to be an effective method of enhancing the rolling shear strength of lumber and in turn, CLT. In this study, utilizing knowledge of material properties, optimization of the compression ratio for densification has been presented. Three-layered CLT beams made from non-densified lumber, grade #1 loblolly pine (Pinus taeda L.), were subjected to a bending load at a span-to-depth ratio of eight and had a rolling shear failure at the mid-layer with a shear strength of 3 MPa. Assuming the same modulus of rupture (MOR) for both lumber and CLT made from the same species and grade, the MOR of lumber was used to calculate the minimum required shear strength (MRSS) of the transverse mid-layer to change the failure mode of the CLT beam from rolling shear to tensile failure. Using the relationship between the compression ratio and the increase in rolling shear strength, the optimized compression ratio for densification was calculated. This procedure resulted in a compression ratio of 16.67% for densification of the mid-layer to avoid rolling shear in the case of CLT beams with a span-to-depth ratio of eight. To verify this process, CLT beams with mid-layers densified at 16.67% were fabricated and submitted to a bending test. Rolling shear failure was mitigated and densified CLT beams failed in tension with a MOR similar to that of lumber, 47.45 MPa. Likewise, rolling shear strength was observed to increase by 48% for CLT that had a densified mid-layer at 16.67%.

Clinical reasoning is an essential competence of veterinary graduands. It is a complex competence with cognitive, metacognitive, social, and situational activities. The literature on clinical reasoning in veterinary medical education is relatively scarce or focused on theoretical rather than practical applications. In this review, we address the practicality of teaching clinical reasoning to veterinary learners utilizing a practical example of a cow with allergic rhinitis. Learners should be guided through all the domains of clinical reasoning, including concepts, data collection and analysis, take action, and reflection on an encounter. Each of these domains needs to be clearly but concisely explained and practiced repeatedly by learners throughout the veterinary curricula. The teaching of clinical reasoning should start as early in the curriculum as possible, preferably in the pre-clinical years, with a gradual scaffolding and building of complexity before work-based learning begins, with an increase in demanding for advanced clinical reasoning competence. The teaching of clinical reasoning is best performed in specialized sessions and continued as a horizontally and vertically integrated activity.

Background: The practice of self-medication (SM) is common worldwide and is an important component of medical self-care. However, improper practice can be dangerous. This study aimed to estimate the prevalence of SM and the factors associated with it among Bangladeshi adults. Methods: A cross-sectional survey was conducted between April and June 2021 among Bangladeshi adults (aged > 19 years) using convenient sampling. A total of 1320 subjects were collected through face-to-face interviews using a standardized questionnaire. Multivariable logistic regression analysis was used to identify factors associated with the practice of SM. Results: Overall, 41% of adults in our survey reported SMP. The most common illnesses that prompted SM were common cold/flu (66.4%), gastric problems (65%), and headache (64.4%). The most frequent reasons for SM were to get better-perceived quality of care (30.6%), perceiving SM without side effects (23.3%), and saving time with effectiveness (14.56%). Potential risk factors included 10 years (AOR = 1.91; 95% CI: 1.04–3.50) and >12 years of schooling (AOR = 5.03; 95% CI: 2.27–11.15), being a businessman (AOR = 4.64; 95% CI: 1.74–12.37), having ≤6 family members (AOR = 2.13; 95% CI: 1.40–3.24), being a member of a social group (AOR = 1.53; 95% CI: 1.10–2.12), a health status check after every six months (AOR = 1.52; 95% CI: 1.08–2.13), and current ill-health (AOR = 1.41; 95% CI: 1.06–1.87). Protective factors identified included ≤30 years of age (AOR = 0.40; 95% CI: 0.17–0.93), and practice of modern (AOR = 0.39; 95% CI: 0.22–0.69) and herbal (AOR = 0.45; 95% CI: 0.21–0.97) treatment modality. Conclusion: More than one-third of the study participants reported practicing SM. Increasing the community’s awareness of the adverse outcomes of SM and not just the average experience might sway individuals away from SM, and implementing strict jurisdiction could be a way to minimize inappropriate SM.

Internet applications rely on Secure Socket Layer (SSL)/Transport Security Layer (TSL) certifications to establish secure communication. However, the centralized nature of certificate authorities (CAs) poses a risk, as malicious third parties could exploit the CA to issue fake certificates to malicious web servers, potentially compromising the privacy and integrity of user data. In this paper, we demonstrate how the utilization of decentralized certificate verification with blockchain technology can effectively address and mitigate such attacks. We present a decentralized public key infrastructure (PKI) based on a distributed trust model, e.g., Web of Trust (WoT) and blockchain technologies, to overcome vulnerabilities like single points of failure and to prevent tampering with existing certificates. In addition, our infrastructure establishes a trusted key-ring network that decouples the authentication process from CAs in order to enhance secure certificate issuance and accelerate the revocation process. Furthermore, as a proof of concept, we present the implementation of our proposed system in the Ethereum blockchain, confirming that the proposed framework meets the five identified requirements. Our experimental results demonstrate the effectiveness of our proposed system in practice, albeit with additional overhead compared to conventional PKIs.

In this study, we propose how to use objective arguments grounded in statistical mechanics concepts in order to obtain a single number, obtained after aggregation, which would allow for the ranking of “agents”, “opinions”, etc., all defined in a very broad sense. We aim toward any process which should a priori demand or lead to some consensus in order to attain the presumably best choice among many possibilities. In order to specify the framework, we discuss previous attempts, recalling trivial means of scores—weighted or not—Condorcet paradox, TOPSIS (Technique for Order Preference by Similarity to Ideal Solution), etc. We demonstrate, through geometrical arguments on a toy example and with four criteria, that the pre-selected order of criteria in previous attempts makes a difference in the final result. However, it might be unjustified. Thus, we base our “best choice theory” on the linear response theory in statistical physics: we indicate that one should be calculating correlations functions between all possible choice evaluations, thereby avoiding an arbitrarily ordered set of criteria. We justify the point through an example with six possible criteria. Applications in many fields are suggested. Furthermore, two toy models, serving as practical examples and illustrative arguments are discussed.

Variant annotation is an important step in deciphering the functional impact of genomic variants and their association with diseases. In this study, we analyzed 80 pan-cancer cases that underwent comprehensive genomic testing and compared the auto-classified variant tiers among four globally-available software solutions for variant interpretation from Roche, SOPHiA GENETICS, QIAGEN, and Genoox. The results revealed striking differences in tier classifications, which are believed to be a result of several factors, including subjectivity in the AMP/ASCO/CAP guidelines, threshold settings for variant allele frequencies and population allele frequencies, as well as variation in disease ontologies. Although the software tools described here provide a time-saving and repeatable process for interpretation of genomic data, it is crucial to understand the nuances and various settings for these solutions, as they can strongly influence variant tier classifications and downstream management.

The treatment of allergic conditions presents a challenge for both seasonal allergic rhinitis and perennial rhinitis sufferers. The increasing prevalence of both of these types of allergic responses requires the use of a range of treatments which can provide relief. The treatment of allergic rhinitis has been considered under the ARIA (Allergic Rhinitis and its Impact on Asthma) guidelines. Current treatment options include medication and avoidance for those with reduced responses, but more expensive treatments include immunotherapy and the use of monoclonal antibodies (mAb). All treatments target specific parts of the inflammatory response which includes mast cells, eosinophils and basophils. Phototherapy can be a useful addition to these treatments, and combinations of UV-B (5%), UV-A (25%) and visible light (70%) in phototherapy treatments have been shown to reduce the severity of symptoms. Phototherapy consisting of visible wavelengths and infrared light (660 nm 940 nm) was shown to be particularly effective in treating perennial rhinitis. The use of a range of wavelengths in the control of allergic responses is described in this paper. Phototherapy can form part of an effective treatment regime for allergic rhinitis sufferers which can exploit synergies in the control of the condition elicited through several pathways.

Traumatic brain injury (TBI) is a common diagnosis requiring acute hospitalization. Long-term, TBI is a significant source of health and socioeconomic impact in the United States and globally. The goal of clinicians who manage TBI is to prevent secondary brain injury. In this population, post-traumatic cerebral infarction (PTCI) acutely after TBI is an important but under-recognized complication that is associated with negative functional outcomes. In this comprehensive review, we describe the incidence and pathophysiology of PTCI. We then discuss the diagnostic and treatment approaches for the most common etiologies of isolated PTCI, including brain herniation syndromes, cervical artery dissection, venous thrombosis, and post-traumatic vasospasm. In addition to these mechanisms, hypercoagulability and microcirculatory failure can also exacerbate ischemia. We aim to highlight the importance of this condition and future clinical research needs with the goal of improving patient outcomes after TBI.

Secret sharing schemes are widely used to protect data by breaking the secret into pieces and sharing them amongst various members of a party. In this paper, our objective is to produce a repairable ramp scheme that allows for the retrieval of a share through a collection of members in the event of its loss. Repairable Threshold Schemes (RTSs) can be used in cloud storage and General Data Protection Regulation (GDPR) protocols. Secure and energy-efficient data transfer in sensor-based IoTs is built using ramp-type schemes. Protecting personal privacy and reinforcing the security of electronic identification (eID) cards can be achieved using similar schemes. Desmedt et al. introduced the concept of frameproofness in 2021, which motivated us to further improve our construction with respect to this framework. We introduce a graph theoretic approach to the design for a well-rounded and easy presentation of the idea and clarity of our results. We also highlight the importance of secret sharing schemes for IoT applications, as they distribute the secret amongst several devices. Secret sharing schemes offer superior security in lightweight IoT compared to symmetric key encryption or AE schemes because they do not disclose the entire secret to a single device, but rather distribute it among several devices.

An increasing number of dairy farmers in New Zealand (NZ) have adopted a once-a-day (OAD) milking production system, and little is known about the impact of this production system on milk protein composition. The objective of this study was to evaluate the effect of OAD milking on the protein composition in milk from individual cows. Milk was sampled in early, mid-, and late lactation from cows kept at Massey University farms Dairy No. 1 (OAD milking) and Dairy No. 4 (TAD milking) in Palmerston North, NZ. The yields of total milk and milk solids, the proximate composition, and the protein composition were determined. Results showed that OAD milking yielded less milk and milk solids than TAD milking. However, no significant differences in protein, fat, and lactose contents were found. While the proportions of total casein (CN), total whey proteins, αs1-CN, β-CN, and β-lactoglobulin were not affected by the milking frequency, milk from a OAD milking system contained higher proportions of αs2-CN and κ-CN and lower proportions of α-lactalbumin. These proteins also changed differently throughout the milking season in a OAD milking system than in a TAD milking system. These changes in the protein composition of the milk observed in a OAD milking system could have implications for its processing properties and product quality.

Remediation of organic dyes in natural waters is a significant environmental need under active study. This review analyzes bimetallic catalytic degradation systems that are based on the Fenton chemistry concept and that generate reactive oxygen species (ROS) as the agent of dye breakdown. Recently developed advanced oxidation processes (AOPs) take advantage of bimetallic heterogeneous catalysts to facilitate rapid rates and full degradation. Catalysts based on two metals including iron, copper, molybdenum, cobalt and magnesium are discussed mechanistically as examples of effective radical ROS producers. The reactive oxygen species hydroxyl radical, superoxide radical, sulfate radical and singlet oxygen are discussed. System conditions for the best degradation are compared, with implementation techniques mentioned. The outlook for further studies of dye degradation is presented.

Machine learning (ML) has become increasingly prevalent in various domains. However, ML algorithms sometimes give unfair outcomes and discrimination against certain groups. Thereby, bias occurs when our results produce a decision that is systematically incorrect. At various phases of the ML pipeline, such as data collection, pre-processing, model selection, and evaluation, these biases appear. Bias reduction methods for ML have been suggested using a variety of techniques. By changing the data or the model itself, adding more fairness constraints, or both, these methods try to lessen bias. The best technique relies on the particular context and application because each technique has advantages and disadvantages. Therefore, in this paper, we present a comprehensive survey of bias mitigation techniques in machine learning (ML) with a focus on in-depth exploration of methods, including adversarial training. We examine the diverse types of bias that can afflict ML systems, elucidate current research trends, and address future challenges. Our discussion encompasses a detailed analysis of pre-processing, in-processing, and post-processing methods, including their respective pros and cons. Moreover, we go beyond qualitative assessments by quantifying the strategies for bias reduction and providing empirical evidence and performance metrics. This paper serves as an invaluable resource for researchers, practitioners, and policymakers seeking to navigate the intricate landscape of bias in ML, offering both a profound understanding of the issue and actionable insights for responsible and effective bias mitigation.

Background: Multiple randomized controlled trials (RCTs) have investigated the impact of adding checkpoint inhibitors to neoadjuvant chemotherapy for triple-negative breast cancer (TNBC) patients. However, there is a lack of biomarkers that can help identify patients who would benefit from combination therapy. Our research identifies response predictors and assesses the effectiveness of adding immunotherapy to neoadjuvant chemotherapy for TNBC patients. Methods: We identified eligible RCTs by searching PubMed, Cochrane CENTRAL, Embase, and oncological meetings. For this meta-analysis, we obtained odds ratios using the standard random effects model. To assess the heterogeneity of the study outcomes, the I2 statistic was obtained. Potential bias was assessed using a funnel plot and the corresponding Egger’s test. Results: In total, 1637 patients with TNBC were included from five RCTs. Neoadjuvant chemoimmunotherapy significantly improved pCR when compared to neoadjuvant chemotherapy alone. In the subgroup analysis, neoadjuvant chemoimmunotherapy showed higher pCR rates in both Programmed death-ligand 1 (PD-L1)-positive and PD-L1-negative TNBC patients. An Eastern Cooperative Oncology Group (ECOG) performance score (PS) of 0 correlated with increased pCRs (OR = 1.9, p < 0.001) in neoadjuvant chemoimmunotherapy vs. neoadjuvant chemotherapy, but no benefit was observed for patients with ECOG PS 1. Nodal positivity was significantly associated with pCR (OR = 2.52, p < 0.001), while neoadjuvant chemoimmunotherapy did not benefit patients with negative lymph nodes. Conclusions: Checkpoint inhibition and neoadjuvant chemotherapy significantly increased pCRs in TNBC patients, regardless of their PDL-1 status. Additional checkpoint inhibitors improved pCR rates, mainly for patients with ECOG PS 0 and lymph node-positive disease.

Ursula Le Guin’s The Word for World is Forest emerged as a reaction to the Vietnam War, which ravaged human and nonhuman lifeworlds. Le Guin offers two competing discursive systems through which to interpret human and nonhuman alterity—Terran industrial capitalism, grounded in physical and symbolic violence, and Athshean ecosocialism, rooted in an ethics of non-violence and forest-centred nominalism. Le Guin appears to suggest that both “readings” of Athshea are locked in an intractable, adversarial logic, typical of the “paranoid” reading practices that Eve Sedgwick would theorise twenty-five years later. In its sensitivity to the spectrum of negative affect covering anticipatory anxiety about forestalling pain, symmetrical suspicion, and fear of humiliation, the novella offers an uncanny prefiguration of paranoid practices. Le Guin suggests that the way out of the paranoid clash of civilisations can be found in two “reparative” reading stances—Selver’s reinterpretation and rearrangement of components of the oppressor’s culture into new, unexpected wholes (hermeneutic reassemblage) and the alien observers’ valorisation of disinterested curiosity over action as a categorical imperative (cerebral equivocity). Le Guin thus seems to offer a reparative poetics avant la lettre.

Universal education is an elusive goal in many countries, especially for disabled children. Nonetheless, determined efforts around the globe have shown that it can become a reality once existing systems were re-imagined by practitioners who arguably have been to the fore more so than academic researchers. Their efforts have identified new ways of thinking about children’s disabilities, the introduction of new practices in schools, forging partnerships between teachers and parents and mobilising community resources. Societal change is both a consequence of and a support to these local systems. The complexity of creating education for all may be daunting, but it is achievable when driven by committed, creative and inspirational leadership from practitioners, as is evident from the examples provided in this paper, which were further validated by research and evaluation into their efforts.

While immunotherapies, such as CAR T therapy and bi-specific antibodies, have revolutionized the treatment of multiple myeloma (MM), patients with AL amyloidosis have been excluded from trials with these agents due to concerns of underlying autonomic, cardiac, and renal dysfunction, leading to potentially fatal toxicities from these therapies. In this communication, we described the outcomes of two patients with AL amyloidosis and concurrent MM with underlying cardiac and/or renal dysfunction who underwent anti-BCMA CAR T cell therapy with ide-cel or cilta-cel, received cytokine release syndrome prophylaxis, and tolerated therapy well with manageable toxicities and achieved a MRD-negative state. We described the preliminary efficacy and safety of CAR T in patients with AL amyloidosis and highlighted the importance of patient selection and medical optimization of cardiac and renal function prior to CAR T.

In this study, we evaluated gamete quality parameters of mature male koi carp (Cyprinus carpio) exposed to three different concentrations (1, 10, and 100 µg/L) of di-(2-ethylhexyl) phthalate (DEHP). After 60 days of exposure, there was a significant decrease in the gonadosomatic index (GSI) of males exposed to 10 and 100 µg/L of DEHP. Histological analysis of the testes revealed impaired histoarchitecture, including inflammatory cells, intratubular vacuoles, and swollen seminiferous tubules in treatment groups. Gamete quality parameters like sperm production, motility, spermatocrit, and sperm density values were significantly decreased at the 10 and 100 µg/L concentrations. Biochemical compositions, including glucose, cholesterol, and total protein levels, were significantly changed in the treatment groups. Similarly, the ionic compositions of seminal fluid (Na, K, Ca, and Mg) also varied in the treatment groups. Furthermore, the 11-ketotestosterone levels were decreased, and the 17-β estradiol levels were increased in the DEHP-treated groups. The mRNA expression levels of reproduction-related genes, including Fshr, Lhr, Ar, Erα, and Erβ, were significantly changed in the DEHP-treated males in a dose-dependent manner. In conclusion, the findings of this study confirmed that environmentally relevant exposure to DEHP may contribute to a decline in the gamete quality of male fishes.

This study investigates the turbulent flow characteristics and heat transfer performance within a rectangular cooling channel with an aspect ratio of 5:3 and featuring perforated ribs, then explores the effects of the rib perforation directions on its thermal hydraulic performance. Through experimental tests (transient thermographic liquid crystal technique) and numerical simulations, it is demonstrated that horizontal perforated ribs can effectively reduce pressure loss at a high Reynolds number while maintaining notable heat transfer enhancement. Additionally, changing the rib perforation directions results in diverse effects on flow field and heat transfer. Our results show that horizontal perforated ribs can compress the recirculation vortex behind ribs, enhancing heat transfer by flow scouring, whereas upward-tilted perforated ribs increase flow friction and weaken heat transfer due to coupling of the airflow with the separation vortices behind the ribs. Downward-tilted ribs enhance local heat transfer by directing airflow behind the rib, and can also cause detachment of vortices and reduced friction. Our results indicate that introducing horizontal perforated ribs into a rectangular internal cooling channel can decrease pressure loss without significantly compromising heat transfer performance.

Achieving a consistent grain yield while preserving persistent aroma remains a substantial challenge in aromatic rice production in Bangladesh. To address this challenge, a field experiment was conducted at the agronomy research area of Sher-e-Bangla Agricultural University, Dhaka-1207, between 15 June 2022, and 25 November 2022 (Aman season). This study aimed to evaluate the influence of harvesting time on aromatic rice performance. The experiment, following a randomized complete block design with three replications, involved two factors: factor 1 comprised various rice varieties [Bangladesh Rice Research Institute (BRRI) dhan34, BRRI dhan70, BRRI dhan80, and Tulshimala], and factor 2 comprised three harvesting times [3, 4, and 5 weeks after flowering (WAF)]. Results revealed significant impacts of variety and/or harvesting time on grain yield, physicochemical characteristics, and aroma of aromatic rice. Notably, Tulshimala and BRRI dhan80 exhibited superior milling quality, biochemical properties, and aroma characteristics among the aromatic rice types. BRRI dhan70 and BRRI dhan80 displayed higher grain yield when harvested at 5 WAF. However, Tulshimala and BRRI dhan80 showed superiority in grain 2-acetyl-1-pyrroline (2-AP) concentration when harvested 3 or 4 WAF. Earlier harvesting at 3 and 4 WAF resulted in higher percentages of grain 2-AP (60.22% and 53.96%, respectively) compared with later harvesting at 5 WAF (used as check). In conclusion, varying harvesting times markedly impact the yield, physiochemical characteristics, and aroma of aromatic rice varieties, with earlier harvesting beneficial for aroma retention in Tulshimala and BRRI Dhan80 and later harvesting for increased economic yield in BRRI dhan70 and BRRI dhan80.

The present study, carried out in Gujarat (India) between 2005 and 2021, aims to prepare dew and rain maps of Gujarat over a long period (17 years, from 2005 to 2021) in order to evaluate the evolution of the potential for dew and rain in the state. The ratio of dew to precipitation is also determined, which is an important metric that quantifies the contribution of dew to the overall water resources. Global warming leads, in general, to a reduction in precipitation and non-rainfall water contributions such as dew. The study shows, however, a rare increase in the rainfall and dew condensation, with the latter related to an increase in relative humidity and a decrease in wind amplitudes. Rain primarily occurs during the monsoon months, while dew forms during the dry season. Although dew alone cannot resolve water scarcity, it nonetheless may provide an exigent and unignorable contribution to the water balance in time to come. According to the site, the dew–rain ratios, which are also, in general, well correlated with dew yields, can represent between 4.6% (Ahmedabad) and 37.2% (Jamnagar). The positive trend, observed since 2015–2017, is expected to continue into the future.

Antimicrobial peptides (AMPs) are considered a promising alternative to conventional antibiotics to fight against the rapid evolution of antibiotic resistance. Other than their potent antimicrobial properties, AMP-based vesicles can be used as efficient drug-delivery vehicles. In the present study, we synthesized and characterized a new cyclic AMP, consisting of all-hydrophobic cores with antimicrobial activity against S. aureus. Interestingly, CycP undergoes supramolecular self-assembly, and self-assembled CycP (sCycP) vesicles are characterized under an electron microscope; however, these vesicles do not display antimicrobial activity. Next, sCycP vesicles are used in combination with SXT (sulfamethoxazole–trimethoprim) vesicles to check the drug loading and delivery capacity of sCycP vesicles to bacterial cell membranes. Interestingly, sCycP vesicles showed synergistic action with SXT vesicles and resulted in a significant reduction in MIC against S. aureus. Further, electron microscopy confirmed the membrane-specific killing mechanism of SXT-loaded sCycP vesicles. Additionally, CycP showed high binding affinities with the β-lactamase of S. aureus, which was one of its possible antimicrobial mechanisms of action. Overall, the results suggested that CycP is a novel self-assembled dual-action cyclic AMP with non-cytotoxic properties that can be used alone as an AMP or a self-assembled drug delivery vehicle for antibiotics to combat S. aureus infections.

Cold-formed steel (CFS) sections have become popular in construction due to several advantages over structural steel. However, research on the performance of galvanized iron (GI)-based CFS under high temperatures, especially regarding its flexural behavior, has been limited. This study extensively investigates how GI-based CFS beams with varying spans behave under elevated temperatures and subsequent cooling using air and water. This study examines the impact of temperature loading and compares the effectiveness of air- and water-cooling methods. Experimental results are validated and analyzed alongside findings from finite element modeling (FEM) using ABAQUS (2019_09_13-23.19.31) and the Direct Strength Method (DSM). Additionally, this study conducts a parametric investigation to assess how beam span influences flexural capacity. Among beams heated to the same temperature, those cooled with water show slightly lower load capacities compared to those cooled with air. The highest load capacity observed is 64.3 kN for the reference specimen, while the lowest is 26.2 kN for the specimen heated for 90 min and cooled with water, a 59.27% difference between them. Stiffness decreases as heating duration increases, with the reference section exhibiting significantly higher stiffness compared to the section heated for 90 min and cooled with water, with a 92.76% difference in stiffness. As heating duration increases, ductility also increases. Various failure modes are observed based on different heating and cooling conditions across different beam spans. This study provides insights into how GI-based CFS beams perform under temperature stress and different cooling scenarios, contributing valuable data for structural design and safety considerations in construction.

Amyloid plaques and tau tangles are the hallmark pathologic features of Alzheimer’s disease (AD). Traditionally, these changes are identified in vivo via cerebrospinal fluid (CSF) analysis or positron emission tomography (PET) scans. However, these methods are invasive, expensive, and resource-intensive. To address these limitations, there has been ongoing research over the past decade to identify blood-based markers for AD. Despite the challenges posed by their extremely low concentrations, recent advances in mass spectrometry and immunoassay techniques have made it feasible to detect these blood markers of amyloid and tau deposition. Phosphorylated tau (p-tau) has shown greater promise in reflecting amyloid pathology as evidenced by CSF and PET positivity. Various isoforms of p-tau, distinguished by their differential phosphorylation sites, have been recognized for their ability to identify amyloid-positive individuals. Notable examples include p-tau181, p-tau217, and p-tau235. Among these, p-tau217 has emerged as a superior and reliable marker of amyloid positivity and, thus, AD in terms of accuracy of diagnosis and ability for early prognosis. In this narrative review, we aim to elucidate the utility of p-tau217 as an AD marker, exploring its underlying basis, clinical diagnostic potential, and relevance in clinical care and trials.

Three-dimensional (3D) spheroid models are crucial for cancer research, offering more accurate insights into tumour biology and drug responses than traditional 2D cell cultures. However, inconsistent and low-throughput spheroid production has hindered their application in drug screening. Here, we present an automated high-throughput platform for a spheroid selection, fabrication, and sorting system (SFSS) to produce uniform gelatine-encapsulated spheroids (GESs) with high efficiency. SFSS integrates advanced imaging, analysis, photo-triggered fabrication, and microfluidic sorting to precisely control spheroid size, shape, and viability. Our data demonstrate that our SFSS can produce over 50 GESs with consistent size and circularity in 30 min with over 97% sorting accuracy while maintaining cell viability and structural integrity. We demonstrated that the GESs can be used for drug screening and potentially for various assays. Thus, the SFSS could significantly enhance the efficiency of generating uniform spheroids, facilitating their application in drug development to investigate complex biological systems and drug responses in a more physiologically relevant context.

Background: Despite several diagnostic guidelines, Fetal Alcohol Spectrum Disorders (FASDs) remain underdiagnosed or misdiagnosed, delaying the care of these patients and support for families. Objective: This study aims to help professionals caring for these children and their families to suspect this diagnosis earlier and to provide the most appropriate follow-up. Methods: A retrospective chart review with monocentric recruitment was performed at the Genetics Unit of the University Hospital of Reunion Island. A total of 147 children and adolescents with FASDs were included. Results: Prenatal alcohol exposure was associated with paternal alcohol consumption in 42.9%, and a high rate of prematurity (33.3%) was observed. Sixty percent of children or adolescents were placed in foster families. Learning difficulties without cognitive deficits were found in 65.8% of cases (50/76). Postural control and fine motor skills disabilities were described, respectively, in 54.7% (35/64) and 72.5% (50/69) of cases. A systematic genetic assessment was carried out, identifying in these FASD patients an associated Copy Number Variation (CNVs) in 22.6% of cases. Conclusion: Children with FASDs combine significant vulnerabilities, associating exposure to alcohol during the preconception and/or the prenatal period, prematurity, complex familial and sociocultural living conditions, and a genetic anomaly in almost a quarter of cases.

Background: Studies have shown a bidirectional relationship between asthma and obstructive sleep apnea (OSA). However, there is a paucity of national-level data evaluating the impact of OSA on hospital outcomes in pediatric hospitalizations for asthma. Methods: We analyzed the National Inpatient Sample and Kids Inpatient Database to include all pediatric hospitalizations with a primary diagnosis of asthma between 2003–2016. Using ICD codes, the pediatric asthma cohort was divided into two groups: those with and those without a concomitant diagnosis of OSA. The primary outcomes were in-hospital mortality and the need for mechanical ventilation. The secondary outcomes were the lengths of each hospital stay and total hospitalization charges. Results: We analyzed 1,606,248 hospitalizations during the 14-year study period. The overall prevalence rate of OSA was 0.7%. Patients with asthma and OSA were significantly older (8.2 versus 5.9 years) and were more often male, p < 0.001. The OSA group had several increased comorbidities. The overall mortality rate was 0.03%, and multivariate regression analysis showed that OSA was associated with 4.3 times higher odds of in-hospital mortality (95% CI: 2.4 to 7.6, p < 0.001). Furthermore, OSA was associated with a 5.2 times greater need for mechanical ventilation (95% CI: 4.8 to 5.5, p < 0.001). Linear regression analyses demonstrated that OSA independently contributed an additional 0.82 days to the hospital stay length (95% CI: 0.79 to 0.86, p < 0.001) and an extra 10,479 USD (95% CI: 10,110 to 10,848, p < 0.001) in hospitalization charges. Conclusion: OSA in children admitted with asthma is associated with poor hospital outcomes such as increased mortality risk, the need for mechanical ventilation, and increased healthcare utilization.

(1) Background: The Kaiser score is a user-friendly tool that evaluates lesions on breast MRI and has been studied in the general population and a few specific clinical scenarios. We aim to evaluate the performance of the Kaiser score in the characterization of additional lesions identified on staging breast MRI. (2) Methods: The Kaiser score of the biopsied additional lesions identified on staging MRI in recently diagnosed breast cancer patients was retrospectively determined. Statistical analysis was performed to evaluate the diagnostic capability of the Kaiser score and whether it is affected by different imaging and pathological parameters of the additional and the index lesion. (3) Results: Seventy-six patients with ninety-two MRI-detected lesions constitute the studied population. There was a statistically significant difference in the Kaiser score between benign and malignant lesions, irrespective of the pathology of the index cancer (p = 0.221) or the size and the imaging features of the additional lesion. Using a cutoff of 5 and above for suspicious lesions, biopsy could have been avoided in 34/92 lesions. (4) Conclusions: The Kaiser score can assist radiologists in the evaluation of additional MRI lesions identified in recently diagnosed breast cancer patients, thus decreasing the number of unneeded biopsies and delays in definitive surgical management.

This study investigated how Rhus toxicodendron (RT) (6C, 30C, and 200C) can boost the immune system of BALB/c mice that were given cyclophosphamide (CPM), which is an anticancer drug that weakens the immune system. RT, known for its historical use in traditional homeopathic remedies, has demonstrated immunomodulatory and anti-inflammatory effects in various experimental models. To test the immune-boosting effects of RT, CPM (80 mg/kg) was given intraperitoneally to mice on days 4, 8, and 12 of the study but not to the normal control group. CPM-induced immunosuppression led to significant decreases in red blood cell (RBC), white blood cell (WBC), and hemoglobin (Hb) levels, and reduced spleen and thymus indices. Phagocytic activity, cytokine concentrations, and spleen architecture were also adversely affected. RT treatment, particularly at 200C, significantly ameliorated these effects, improving RBC, WBC, and Hb levels. Furthermore, RT partially prevented CPM-induced atrophy of immune organs. Treatment positively influenced cytokine production at both the protein and mRNA levels, restoring immune balance. Histopathological results confirmed that RT stimulated the immune system. The cells were more stable, and the white pulp in the spleen was arranged in a regular pattern. These findings suggest that RT may serve as an adjunctive immunostimulant therapy for conditions characterized by immunosuppression. However, further investigations in other immunocompromised states must validate these results before considering human clinical trials.

The global challenge of sustainable and affordable wastewater treatment technology looms large as water pollution escalates steadily with the rapid pace of industrialization and population growth. The photocatalytic wastewater treatment is a cutting-edge and environmentally friendly technology that uses photons from light source to degrade and remove organic and inorganic contaminants from water. Thus, utilizing solar energy for photocatalytic wastewater treatment holds great promise as a renewable solution to alleviate pressures on the global water crisis. Employing solar concentrators to intensify sunlight for photocatalysis represents a promising avenue for future applications of a low-cost and rapid sustainable wastewater purification process. This groundbreaking approach will unveil fresh technological avenues for a cost-effective, sustainable, and swift wastewater purification process utilizing sunlight. This review article explores diverse solar concentrating systems and their potential applications in the wastewater treatment process.

This article shows that the gas-phase entropy of molecules is proportional to the area of the molecules, with corrections for the different curvatures of the molecular surface. The ability to estimate gas-phase entropy by the area law also allows us to calculate molecular entropy faster and more accurately than currently popular methods of estimating molecular entropy with harmonic oscillator approximation. The speed and accuracy of our method will open up new possibilities for the explicit inclusion of entropy in various computational biology methods.

Background: Although heartburn and reflux are frequently reported in ulcerative colitis [UC], the correlation between UC and gastroesophageal reflux disease [GERD], and its complications, esophageal stricture and Barrett’s esophagus [BE], is not well understood. This study aims to examine the prevalence and associated risk of GERD and its complications within the UC population. Methods: We analyzed the National Inpatient Sample (NIS) dataset, consisting of 7,159,694 patients, comparing GERD patients with and without UC to those without GERD. We assessed the degree of colonic involvement in UC and the occurrence of esophageal complications. Bivariate analyses were conducted using the chi-squared test or Fisher exact test (two-tailed). Results: A higher prevalence of GERD (23.0% vs. 16.5%) and GERD phenotypes, such as non-erosive reflux disease (NERD) (22.3% vs. 16%) and erosive esophagitis (EE) (1.2% vs. 0.6%), was found in UC patients (p < 0.01), including pancolitis, proctitis, proctosigmoiditis, left-sided colitis, and indetermined UC (with undefined colonic involvement). UC patients were more likely to develop GERD (1.421), NERD (1.407), and EE (1.681) (p < 0.01). A higher prevalence of esophageal stricture (16.9 vs. 11.4 per 10,000 patients) and BE without dysplasia (94.5 vs. 39.3 per 10,000 patients) was found in UC (p < 0.05). The odds of developing BE without dysplasia were higher (1.892) in patients with UC (p < 0.01), including ulcerative pancolitis, proctitis, and indeterminate UC (OR of 1.657, 3.328, and 1.996, respectively) (p < 0.05). Conclusions: Our study demonstrates an increased risk of developing GERD and its complications in UC. This highlights the importance of vigilant monitoring and early intervention to minimize associated GERD-related risks in patients with UC.

Background: Pelvic organ prolapse (POP) affects many women and is often managed with pessary treatment, yet predicting the success of fitting remains challenging. This study aims to identify anatomical parameters associated with successful and unsuccessful pessary treatment using dynamic magnetic resonance imaging (dMRI). Methods: A cross-sectional study in Maastricht University Medical Centre (MUMC+), the Netherlands. Sixteen women with a cystocele and/or descensus uteri minimal POP-Q stage 2, using pessary treatment, were included. All women underwent a dynamic MRI of the pelvic floor at rest, during contraction and on Valsalva. The anatomical parameters evaluated included various lengths and angles. The association between the anatomical parameters and pessary fitted is assessed using partial least squares regression. The predictive accuracy was tested using cross-validation based on the partial least squares model with the most important variables. Results: Seven of the sixteen women (43.8%) were in the non-fitting group (due to movement, rotation or expulsion of the pessary), and nine women (56.3%) were in the fitting group. Participants in the non-fitting group had a significantly lower body mass index (BMI). Variables such as total vaginal length (TVL) and certain angles were highly predictive of pessary fitting success, with variable importance of projection (VIP) scores indicating their importance. The prediction models showed accuracies ranging from 53.3% to 80.0%. Conclusions: In this explorative study, TVL, cervical length (CL), sacrococcygeal angle and pubococcygeal angle were key variables associated with pessary fitting success. These findings offer valuable insights for optimizing pessary fitting procedures and the development of new pessaries.

It is difficult to select low-cost filler materials. Specifically, carbon-based filling materials are a matter of concern, and developing a carbon-filled polymer composite with enhanced properties is necessary. In this study, the authors developed a polymer composite using virgin polypropylene (PP) as a matrix and affordable micrographite (µG) as a filler. The developed composite has many potential applications in the automotive, aerospace, and electronic industries. To prepare the test specimens, the composite was prepared using a twin-screw extruder containing 3, 6, 9, 12, or 15 wt.% µG powder (BET surface area ≈ 29 m2/g; particle size > 50 µm) followed by injection molding. Different mechanical properties like the tensile, flexural, and impact strengths were determined. The prepared composites were further characterized by means of XRD, TGA, DSC, FTIR, DMA, FESEM, and PLM tests. The results were analyzed and compared with those for PP. Improved tensile (up to ≈ 34 MPa) and flexural (up to ≈ 40 MPa) strength was observed with an increase in the µG content. However, the impact strength continuously decreased (maximum ≈ 32 J/m for PP) with fractures. These findings underscore that graphite plays a significant role in controlling the deformation behavior and ultimate strength of composites. An XRD analysis revealed that adding graphite restructured the crystalline arrangement of PP and altered the composite’s crystallographic properties. Nonetheless, no induction effect (β-phase formation) was observed. A moderate enhancement in the thermal stability was observed owing to a small increase in the melt (Tm), onset (Tonset), and residual (TR) temperatures. A microstructural analysis showed that the micrographite powder strongly prevented spherulite growth and modified the graphite powder’s rate of dispersion and agglomeration in a polymer matrix. The results show that graphite could be a viable low-cost alternative carbon-based filler material in polypropylene matrices.

The medical device industry is constantly innovating in the search for materials that demonstrate superior performance, better intrinsic characteristics, profitability, and a positive impact on potential patients. The thermoplastic polymer resin Pebax® 5533D is one of the most widely used commercial materials for manufacturing medical device parts due to its easy processability. However, its mechanical and thermal properties require improvements to mitigate identified manufacturing defects, such as a decrease in material flexibility, high susceptibility to moisture, and thermal degradation during processing. Therefore, this study integrated different materials, such as plasticizers and filler additives, to produce a polymer compound prototype formula as a solution technique to enhance the current material’s performance. Modifying mechanical and rheological properties allows to evaluate the impacts on the polymeric material’s flexibility and thermal behavior. This was achieved by processing mixed additives using injector-molding equipment to obtain equal-molded samples of every formula. In addition, material characterization was performed to determine the variations in the samples’ crystallization, flexural strength, and moisture content. Calcium stearate was determined to be the most significant component serving as a mechanical resistance modifier and thermal stabilizer alongside calcium chloride as a moisture content reducer combined with Pebax® 5533D.

Vancomycin-resistant enterococci (VRE) commonly colonize the gut of individuals with hematologic malignancies or undergoing hematopoietic cell transplant (HCT) and may cause bacteremia. In 2012, we identified VRE isolates from patients and patients’ rooms and showed transmission networks of highly genetically related daptomycin-resistant (DR)-VRE strains. This is a follow-up study performing whole-genome sequencing (WGS) and phylogenetic analyses on 82 clinical VRE strains isolated from stools and blood cultures of patients with leukemia and HCT between 2015 and 2019. Here, we observed transmission of highly genetically related strains between rooms on the same or on different floors, including a DR-VRE strain identified in 2012. Eleven of twenty-eight patients with DR-VRE were never exposed to daptomycin, suggesting horizontal transmission. Fifteen of the twenty-eight patients with DR-VRE died within 30 days of positive blood cultures. Amongst those, one DR-VRE strain belonging to ST1471 had the virulence gene bopD responsible for biofilm formation. Additionally, to our knowledge, this is the first report of a DR-VRE strain belonging to ST323 in the United States. In summary, our study demonstrated the emergence and persistence of VRE strains, especially DR-VRE, in our hospital. Adding WGS to routine infection control measures may timely identify potential horizontal VRE transmission including multi-drug-resistant isolates.

(1) Shigella spp. (Shigella) is known for causing dysentery with blood in stool, but most children infected with Shigella have non-dysentery Shigella-associated diarrhea (NDSD). The World Health Organization recommends the use of antibiotics when diarrhea is bloody, leaving most NDSD cases untreated. The absence of dysentery may not indicate a low risk of death and severe morbidity. Rapid diagnosis and treatment of shigellosis in vulnerable, young children may be lifesaving. INSIGHT aims to determine the potential benefit of identifying NDSD cases (n = 296) and their outcomes compared to cases of Shigella dysentery [DS (n = 148)] and non-Shigella watery diarrhea [WD (n = 148)]. (2) Children seeking care at hospitals in Bangladesh will be enrolled and followed for 1 year (NDSD and DS) or 90 days (WD). We will determine the impact of NDSD on morbidity, mortality, gut health, nutritional status, and cognitive development compared to DS and WD in children and whether the simple “Rapid LAMP-based Diagnostic Test (RLDT)” can accelerate the detection and treatment of shigellosis in the clinical settings of rural Bangladesh. (3) INSIGHT will determine the impact of NDSD in children and determine if the treatment guidelines of shigellosis need to be revisited to improve clinical outcomes and the development of these children.

The synthesis of silver nanoparticles (AgNPs) using environmentally friendly methods has become increasingly important due to its sustainability and cost-effectiveness. This study investigates the green synthesis of AgNPs using gall extracts from the plant Ficus recemosa, known for its high phytochemical content. The formation of AgNPs was verified through multiple analytical techniques, including UV-Vis spectroscopy, Fourier-transform infrared (FTIR) spectroscopy, transmission electron microscopy (TEM), zeta potential analysis, and dynamic light scattering (DLS). The UV-Vis spectroscopy results displayed a distinct surface plasmon resonance peak indicative of AgNP formation. FTIR analysis revealed specific interactions between silver ions and phytochemicals in the gall extract, while TEM images confirmed the nanoscale morphology and size of the synthesized particles. Zeta potential and DLS analyses provided insights into the stability and size distribution of the AgNPs, demonstrating good colloidal stability. Biological properties of the AgNPs were assessed through various assays. Antimicrobial activity was tested using the disc diffusion method against Escherichia coli and Staphylococcus aureus, showing significant inhibitory effects. The anticancer potential was evaluated using the trypan blue exclusion assay on Dalton’s Lymphoma Ascites (DLA) cells, revealing considerable cytotoxicity. Additionally, antimitotic activity was studied in the dividing root cells of Allium cepa, where the AgNPs significantly inhibited cell division. This research highlights the effective use of F. recemosa gall extracts for the green synthesis of AgNPs, presenting an eco-friendly approach to producing nanoparticles with strong antimicrobial, anticancer, and antimitotic properties. The promising results suggest potential applications of these biogenic AgNPs in medical and agricultural sectors, paving the way for further exploration and utilization.

Graded-index fibers have been used in recent years to make high-power fiber lasers and amplifiers. Such fibers exhibit self-imaging, a phenomenon in which any optical beam periodically reproduces its original shape in undoped fibers (no gain). In this work, we employed analytic and numerical techniques to study how self-imaging affects the evolution of a signal beam inside a nonlinear graded-index fiber amplifier, doped with a rare-earth element and pumped optically to provide gain all along its length. We also exploited the variational technique to reduce the computing time and to provide physical insights into the amplification process. We compared the variational and fully numerical results for the two pumping schemes (clad pumping and edge pumping) commonly used for high-power fiber amplifiers and show that the variational results are reliable in most cases of practical interest. The stability of the signal beam undergoing amplification is examined numerically by launching a noisy Gaussian beam at the input end of the amplifier. Our results show that the quality of the amplified beam should improve in the case of edge pumping when a narrower pump beam provides an optical gain that varies considerably in the radial direction of the fiber. Such an improvement does not occur for the clad pumping scheme, for which the use of a relatively wide pump beam results in a nearly uniform gain all along the fiber.

When performing remote sensing image segmentation, practitioners often encounter various challenges, such as a strong imbalance in the foreground–background, the presence of tiny objects, high object density, intra-class heterogeneity, and inter-class homogeneity. To overcome these challenges, this paper introduces AerialFormer, a hybrid model that strategically combines the strengths of Transformers and Convolutional Neural Networks (CNNs). AerialFormer features a CNN Stem module integrated to preserve low-level and high-resolution features, enhancing the model’s capability to process details of aerial imagery. The proposed AerialFormer is designed with a hierarchical structure, in which a Transformer encoder generates multi-scale features and a multi-dilated CNN (MDC) decoder aggregates the information from the multi-scale inputs. As a result, information is taken into account in both local and global contexts, so that powerful representations and high-resolution segmentation can be achieved. The proposed AerialFormer was benchmarked on three benchmark datasets, including iSAID, LoveDA, and Potsdam. Comprehensive experiments and extensive ablation studies show that the proposed AerialFormer remarkably outperforms state-of-the-art methods.

Considering environmental concerns, electric vehicles (EVs) are gaining popularity over conventional internal combustion (IC) engine-based vehicles. Hybrid energy-storage systems (HESSs), comprising a combination of batteries and supercapacitors (SCs), are increasingly utilized in EVs. Such HESS-equipped EVs typically outperform standard electric vehicles. However, the effective management of power sources to meet varying power demands remains a major challenge in the hybrid electric vehicles. This study presents the development of a MATLAB Simulink model for a hybrid energy-storage system aimed at alleviating the load on batteries during periods of high power demand. Two parallel combinations are investigated: one integrating the battery with a supercapacitor and the other with a photovoltaic (PV) system. These configurations address challenges encountered in EVs, such as power fluctuations and battery longevity issues. Although batteries are commonly used in conjunction with solar PV systems for energy storage, they incur higher operating costs due to the necessity of converters. The findings suggest that the proposed supercapacitor–battery configuration reduces battery peak power consumption by up to 39%. Consequently, the supercapacitor–battery HESS emerges as a superior option, possibly prolonging battery cycle life by mitigating stress induced by fluctuating power exchanges during the charging and discharging phases.

The global challenge of air pollution’s adverse health effects, particularly highlighted in Dhaka, Bangladesh, underscores the significant impact of particulate matter (PM) exposure. This study aims to assess the current sources of PM2.5 emissions in different microenvironments around Dhaka and explore potential risk factors to assess individual 24 h exposure to PM2.5. A commercially available low-cost sensor was utilized for collecting data for 15 days under various environmental conditions. The average concentrations for PM1.0, PM2.5, and PM10 were 37.05 ± 24.36 µg/m3, 57.22 ± 40.75 µg/m3, and 69.22 ± 48.46 µg/m3, respectively. The highest PM2.5 concentrations were found (78.87 ± 53.69 μg/m3) in restaurants and residences (62.35 ± 41.70 μg/m3), while air-conditioned shopping malls exhibited the lowest concentrations (20.08 ± 15.57 μg/m3). Driving with windows closed and utilizing air conditioning resulted in a 33–52% reduction in PM2.5 concentrations inside the car. The Hazard Quotient (HQ) for PM2.5 varied by location, with a low level observed in the air-conditioned locations and a moderate level observed in restaurants and non-air-conditioned shopping malls. The significance of this study lies in its potential to inform public health strategies and urban planning initiatives aimed at reducing air pollution exposure in highly populated cities like Dhaka.
