Background Iliac crest pain syndrome is a regional pain syndrome that has been identified in many patients with low back pain. Based on anatomical studies, it was suggested that the potential substrate of this syndrome might be the enthesis of the erector spinae muscle at the posterior medial iliac crest. As there have been no imaging studies of this important enthesis, our aim was to assess its characteristics by ultrasound. Methods Erector spinae enthesis was first studied in a cadaver. Then its characteristics were recorded in 25 healthy volunteers (median age: 28.92, SD: 5.31, mean Body Mass Index 22.61, SD: 3.38), with Esaote My Lab 7 machine using linear transducer (4-13 MHz). Results The cadaver study confirmed the attachment of a substantial part of erector spinae to a well-defined region on the medial posterior iliac crest. The US study in the volunteers consistently showed the entheses as typical hyperechoic fibrillar structures, slightly oblique to the skin in the longitudinal plane and attaching to the iliac crest. In the transverse plane, the entheses were seen as oval, densely dotted structures in contact with the superior edge of posterior superior iliac spine. Their mean thickness (4.9 ± 0.6 and 5.2 ± 0.7 mm longitudinally; 4.3 ± 0.6 and 4.4 ± 0.7 mm transversely), maximum width (16.3 ± 2.8 and 15.7 ± 2.3 mm) and depth (10.8 ± 7.3 and 10.6 ± 6.2 mm) on the left and right side, respectively, as well as their echostructure were recorded and described. Conclusions The erector spinae entheses could be assessed in detail by ultrasound, thus their pathological transformation associated with iliac crest pain syndrome could be identified.

Purpose: The normal limits of spleen size must be known in order for pathological changes to be noticed. The aim of this retrospective study is to determine the normal limits of spleen size in healthy children and to reveal their relation to sex, age, body height and weight. Patients and methods: Three hundred and ten children (150 girls and 160 boys) between 0–16 years of age in Eastern Anatolia Region who had normal spleen ultrasound appearances were included in this study. The greatest longitudinal distance of the spleen from the dome to the tip (splenic length) measured at the hilum in the coronal plane was obtained by ultrasonography. Results: There was no significant difference between the sexes in children. Also, no statistically significant differences were found between the two sexes in any age group for splenic length (t-test, p > 0.05). Therefore, all data were rearranged without being separated according to sex. The correlation analysis has shown a positive and significant correlation between splenic length and age, body height, and weight, with high correlation coefficients (r > 0.80). Splenic length showed the strongest correlation with body height. Conclusion: In children clinically suspected of organomegaly, splenic length can be used to assess organ size. The presented data can be applied to routine ultrasonography examinations.

Aim of the study: To evaluate the value of uterine artery Doppler indices and waveform pattern in predicting fetuses at risk for intrauterine growth restriction in hypertensive disorders of pregnancy. Materials and methods: This was a prospective cross-sectional study including 80 pregnant subjects with hypertensive disorders of pregnancy and two control groups. Uterine artery Doppler sonography was performed in all study participants. Uterine artery Doppler indices across the groups were compared using the analysis of variance (ANOVA) while the presence of prediastolic notch was analyzed with the Chi Square test. Results: For the hypertensive disorders of pregnancy group, resistivity index > 0.66 had a sensitivity of 50.0%, specificity of 69.1% and a positive predictive value of 22.2% for predicting intrauterine growth restriction. The odds ratio was 2.2 with a 95% confidence interval of 0.6–7.8. The presence of prediastolic notching had a sensitivity of 100.0%, specificity of 96.0% and a positive predictive value of 80.0% for predicting intrauterine growth restriction. The odds ratio was 22.7 with a 95% confidence interval of 7.5–68.5. Conclusion: Uterine artery Doppler sonography is useful for predicting fetuses at risk for intrauterine growth restriction in hypertensive disorder of pregnancy. Prediastolic notching is more sensitive and more specific than uterine artery resistivity index in predicting fetuses at risk of intrauterine growth restriction in established hypertensive disorder of pregnancy.

AimThe goal of the paper was to evaluate the procedure of percutaneous drainage of symptomatic hepatic cysts under the transabdominal ultrasound control combined with obliteration.Material and methodWithin the period from 2005 to 2015, 70 patients diagnosed with a simple hepatic cyst of symptomatic nature were subject to hospitalization and treated at the 2nd General, Gastroenterological and Cancer Surgery of the Alimentary System Center and Clinics of the Medical University of Lublin. All the patients subject to evaluation were qualified to percutaneous drainage under an ultrasound control. The drainage utilized typical sets of drains with the diameter of at least 9 F, most often of pigtail type. The fluid aspirated form the cyst was dispatched for complex laboratory testing. Further, a 10% sodium chloride solution was administered to the cyst through the drain, in the volume depending on the previous size of the cyst and the patient's reaction.ResultsPatients reported for a re-visit within the period from 3 to 9 months following the procedure. Complete obliteration of the cyst was confirmed only in 8 patients (11%). Cyst recurrence was reported in cases when during the ultrasound evaluation, the diameter of the cyst following aspiration and obliteration enlarged to over 75% of the initial dimension. In this group, in 10 out of 12 examined (83%) there was a relapse of the previously observed ailments. Among patients, who has a cyst imaged within the period of observation, which had the diameter from 50% to 75% of the previous size, only in 6 cases (37.5%) the initial symptoms relapsed.ConclusionsThe utilization of a drainage and obliteration enables one to achieve the acceptable result of the therapy as well as significant decrease in the number of previously reported ailments and symptoms described.Cel pracyCelem niniejszej pracy była ocena procedury przezskórnego drenażu objawowych torbieli wątroby pod kontrolą ultrasonografii przezbrzusznej w powiązaniu z obliteracją.Materiał i metodaW okresie od 2005 do 2015 roku 70 pacjentów z rozpoznaniem prostej torbieli wątroby o objawowym charakterze było hospitalizowanych i leczonych w II Klinice Chirurgii Ogólnej, Gastroenterologicznej i Nowotworów Układu Pokarmowego Uniwersytetu Medycznego w Lublinie. Wszystkich ocenianych pacjentów typozakwalifikowano do drenażu przezskórnego pod kontrolą USG. Do drenażu wykorzystywano typowe zestawy drenów o średnicy co najmniej 9 F, najczęściej typu pigtail, czyli "świński ogonek". Aspirowany płyn z torbieli był wysyłany do kompleksowego badania laboratoryjnego. W dalszej kolejności poprzez założony dren podawano do jamy torbieli 10-procentowy roztwór chlorku sodu w objętości zależnej od wcześniejszej wielkości torbieli oraz od reakcji pacjenta.WynikiPacjenci zgłaszali się na kontrolę w okresie od 3 do 9 miesięcy po zabiegu. Całkowite zarośnięcie jamy torbieli potwierdzono jedynie u 8 leczonych (11%). Za nawrót torbieli uznano przypadki, gdy w ocenie ultrasonograficznej średnica torbieli po aspiracji i obliteracji powiększyła się do ponad 75% początkowego wymiaru. W tej grupie u 10 z 12 badanych (83%) doszło do nawrotu wcześniej stwierdzanych dolegliwości. U pacjentów, u których w okresie obserwacji zobrazowano torbiel o średnicy od 50% do 75% wcześniejszej wielkości, tylko w 6 przypadkach (37,5%) ponowiły się pierwotne objawy.WnioskiZastosowanie drenażu oraz obliteracji pozwala na osiągnięcie akceptowalnego wyniku terapii, a także znacznego spadku wcześniej zgłaszanych dolegliwości i opisywanych objawów.

Introduction: To date, suction drainage has been routinely used after hip joint replacement. Currently, the validity of this practice is questioned in the literature. Hematoma is a risk fac­tor of periprosthetic infection. Post-operative ultrasonography enables precise assessment of hip joint hematoma. Aim: The aim of the study was to evaluate the usefulness of hip joint ultrasonography with respect to the validity of using suction drainage after primary hip arthroplasty. Material: Inclusion criteria: coxarthrosis. Exclusion criteria: primary and secondary coagulopathy, renal or hepatic failure and history of venous or arterial thrombo­sis. In total, 90 patients were enrolled. Methods: The study was prospective. The patients were assigned into groups in accordance with simple randomization. On the third day post-surgery, an ultrasound examination was conducted in all patients. Results: Deep infection was found in two patients with suction drainage. Hematoma was almost twice bigger in the drainage group. There were no statistically significant differences in the Harris Hip Score between the groups. No statistically significant differences were found between the groups in: complete blood count parameters and C-reactive protein values in the first and third day after surgery, the amount of transfused packed red blood cells, duration of hospital stay, cost of hospital stay and the relationship between osteophyte removal and hematoma size. Conclusions: Ultrasonography performed after hip replacement surgeries is useful in the assessment of hematoma. The randomized study did not reveal statistically significant differ­ences between the group with and without drainage, thus suggesting that this practice can be abandoned, except for selected cases. Due to a short hospital stay, it is recommended to conduct an ultrasound scan in addition to routine radiography and laboratory tests in order to reduce the risk of complications.

Objective: The aim of this study was to investigate the usefulness of high-resolution ultra­sonography for the diagnosis of polyneuropathy in diabetes mellitus patients by the exami­nation of the ulnar nerves. Method: We recruited 100 healthy age-matched volunteers (50 women and 50 men) with 200 arms without diabetes or cubital tunnel syndrome as the control group. We assessed the upper limbs of 100 diabetes mellitus patients (45 women and 55 men), 40 of whom had electrophysiologically confirmed diabetic peripheral neu­ropathy and 60 had no diabetic peripheral neuropathy in the upper limbs. Age, sex, height and weight were recorded and the cross-sectional area of the ulnar nerve was measured at every predetermined site. Results: The cross-sectional area of the ulnar nerve was measured at six sites (mid-humerus, inlet of the cubital tunnel, outlet of the cubital tunnel, upon the medial epicondyle, 6 cm upon the wrist crease and Guyon tunnel). The ulnar nerve in two measuring sites (mid-humerus, upon the medial epicondyle) in the control group showed a statistical difference between men and women (p < 0.05). There was no statistical difference in the cross-sectional area in the control group when dominant and non-dominant arms were compared. The cross-sectional area was larger in the diabetic peripheral neuropathy group in three sites (inlet of the cubital tunnel, outlet of the cubital tunnel, Guyon tunnel) compared with those in the control group. Conclusion: High-resolution ultrasonography may be helpful in the early diagnosis of peripheral neuropathy in diabetic patients.

Introduction: The most common hereditary kidney condition is autosomal dominant polycystic kidney disease. It is the cause of 5–10% of end-stage renal disease. Its symptoms are generally late-onset, typically leading to development of hypertension and chronic kidney disease. Ultrasonography is the imaging modality of choice in its diagnosis and management. The aim of this study is to determine the diagnostic value of grayscale ultrasound imaging in evaluating disease severity. Materials and methods: The study group consisted of 81 patients diagnosed with autosomal dominant polycystic kidney disease, 35 adults and 46 children. Inclusion criterion for adults was the presence of at least 10 large cysts in each kidney; children included into the study had developed at least 1 large renal cyst in each kidney. The number of large cysts, echogenicity of kidney parenchyma, cortical thickness and presentation of cortex/medulla boundary were assessed with the use of Logiq E9 apparatus (GE Healthcare, Netherlands). Patients were divided into groups, based on these morphological parameters. Kidney function was assessed according to serum creatinine concentration and creatinine clearance. Statistical analysis was performed, with p-value lower than 0.05 considered as significant. Results: The number of cysts and the degree of parenchymal dysfunction were the determinants of creatinine level and creatinine clearance, with the second predictor proving stronger. Conclusions: We recommend that an ultrasound kidney examination in patients with polycystic kidney disease should include evaluating renal parenchyma and the number of cysts for better assessment of disease severity.

Aim: The goal of the work was comparing gas ultrasound images below the right diaphragm in two groups: in people with intestinal interposition below the diaphragm and ones with pneumoperitoneum and extracting the traits differentiating these two conditions. Material and methods: Retrospectively, the documentation of 22 patients with intestinal interposition below the diaphragm (group 1) was utilized. Clinical material was used for comparison, previously published, composed of 15 cases of pneumoperitoneum following laparotomy and of 14 cases following that symptom as a result of ulcer perforation – group 2 (in total n = 29). Moreover, the distance in millimeters of the gas surface reflecting ultrasounds from the parietal peritoneum was measured, the smoothness of the surface, parietal peritoneum enhancement at the place of gas adherence, gas continuity below the diaphragm with gas in the intestine located below the liver. Results: Direct adherence of the gas surface to the diaphragm was observed in 100% of the cases of emphysema, but in no cases of intestinal interposition. Yet, in the group of patients with colonic interposition (n = 21) there was always a small gap (2–3 mm) and the gas surface among those patients in 100% of the cases was uneven. Conclusions: In differentiation between pneumoperitoneum and liverdiaphragm interposition of the intestine one should take into account – apart from gas movement below the diaphragm at body position changing – the presence of protrusion and section enhancement of the diaphragmatic peritoneum as well as the distance of the gas from the diaphragm, the smoothness of its surface and the continuity with the intestine below the liver. Interpositions of small diaphragm-liver penetration may subside in erect position.

Neuroendocrine tumors make up an interesting pathology of a variable clinical picture, prognosis, localization, endocrine activity and degree of malignancy.The aim of this paper is to assess whether ultrasonography can be helpful in diagnosing neuroendocrine tumors in the small intestine by analyzing changes in the mesentery.From 1996 to 2013, we encountered 17 patients (9 women and 8 men at the mean age of 57) with a neuroendocrine tumor in the small intestine. The diagnosis was confirmed in all patients by pathomorphological examinations. All retrospectively analyzed patients (n =17) had an abdominal US examination conducted in accordance with the previously mentioned protocol.Primary neuroendocrine tumors were visualized ultrasonography in 13 of 17 patients (76.5%). In the remaining 4 patients, tumors in the small bowel were diagnosed in CT enteroclysis (n = 3) and conventional enteroclysis (n = 1). Somatostatin receptor scintigraphy and CT enteroclysis supplemented the ultrasound result by providing information about the stage of the disease in 7 cases. In most of the 17 patients with a neuroendocrine tumor diagnosed by ultrasound (n = 11, 64.7%), changes in the mesentery were prevailing pathological signs that raised suspicion and, consequently, helped localize the primary lesion. The hypoechoic mesenteric lymph nodes were the greatest (9-18 mm), well-circumscribed, and the largest of them showed signs of vascularization. The size of hypoechoic lesions in the mesentery ranged from 25 to 53 mm. These lesions showed moderate blood flow. All of them were single, usually poorly circumscribed; two lesions showed slight calcifications.In an abdominal US examination, 2/3 of patients with neuroendocrine tumors in the small bowel manifest secondary lesions in the mesentery which are easier to visualize than the primary focus. 30% of them are manifested as hypoechoic masses. The detection of such lesions should prompt the search for the primary focus in the small intestine.Nowotwory neuroendokrynne to ciekawy dział patologii o zróżnicowanym obrazie klinicznym, rokowaniu, umiejscowieniu, czynności wydzielniczej i stopniu złośliwości.Celem pracy jest ocena możliwości diagnostyki nowotworów neuroendokrynnych jelita cienkiego przy użyciu ultrasonografii poprzez identyfikację zmian w krezce.W okresie od 1996 do 2013 roku zgromadzono 17 chorych (9 kobiet i 8 mężczyzn w średnim wieku 57 lat) z nowotworem neuroendokrynnym w jelicie cienkim. U wszystkich pacjentów potwierdzono rozpoznanie nowotworu neuroendokrynnego na podstawie badań patomorfologicznych. Wszystkie objęte retrospektywną analizą osoby (W badaniu ultrasonograficznym udało się uwidocznić pierwotny nowotwór neuroendokrynny u 13 z 17 chorych (76,5%). U pozostałych 4 osób guz zlokalizowany w jelicie cienkim został rozpoznany na podstawie enteroklizy TK (W badaniu USG przezbrzusznym u 2/3 chorych z guzem neuroendokrynnym w jelicie cienkim występują wtórne zmiany w krezce, które są łatwiejsze do zobrazowania niż samo ognisko pierwotne. W 30% manifestują się jako masy hipoechogeniczne. Wykrycie takich zmian powinno skłonić do poszukiwania ogniska pierwotnego w jelicie cienkim.

The primary aim of this paper was to assess the relevance of high-frequency ultrasound examination in qualifying patients for either surgical or conservative treatment of postoperative peripheral neuropathies. The study was conducted in a group of 71 patients who in 2009-2011 were referred to ultrasound examinations due to a clinical suspicion of peripheral neuropathies. For the purposes of this analysis, the suspected postoperative neuropathies were divided into three groups: after surgical treatment of the median nerve (1), after surgical treatment of the ulnar nerve (2) and other postoperative neuropathies (3). All patients underwent the interview, physical examination and ultrasound examination. The ultrasound examinations were performed with Esaote MyLab 50 and MyLab 60 systems. Based on the clinical and US examinations, the patients were qualified for either surgical (51 cases) or conservative treatment (20 cases). An EMG examination was also performed in certain patients (60 cases). Mean values of cross-sectional areas and diameters of the nerve trunks were calculated in individual pathology groups. The ultrasound features of the peripheral nerves analyzed in the study, such as echostructure, notch sign, hyperemia and continuity of the transverse ligament, were divided into subgroups. Moreover, the frequency of adhesions between the nerve trunks and adjacent tissues, occurrence of pain on compression with a transducer and instability of the ulnar nerve as well as angulation of the posterior interosseous nerve in a dynamic examination was calculated. The analyses of the collected material were performed by means of descriptive statistics. The results of clinical and surgical verification were consistent with ultrasound findings in 100% of cases. The results indicate that high-frequency ultrasonography is a valuable method in qualifying patients for various types of treatment of peripheral postoperative neuropathies.Podstawowym celem pracy była ocena przydatności badania ultrasonograficznego z zastosowaniem głowic wysokiej częstotliwości w kwalifikowaniu do leczenia operacyjnego albo zachowawczego neuropatii obwodowych o charakterze pooperacyjnym. Materiał pracy stanowiła grupa 71 osób kierowanych w latach 2009–2011 na badanie USG z klinicznym podejrzeniem neuropatii obwodowych. Neuropatie pooperacyjne na potrzeby analizy podzielono na trzy grupy: stan po leczeniu operacyjnym nerwu pośrodkowego (1), stan po leczeniu operacyjnym nerwu łokciowego (2) oraz inne neuropatie pooperacyjne (3). U wszystkich pacjentów przeprowadzono badanie podmiotowe, przedmiotowe oraz ultrasonograficzne. Badania ultrasonograficzne wykonano aparatami Esaote MyLab 50 i MyLab 60. Na podstawie wyniku badania klinicznego i USG pacjentów kwalifikowano do leczenia operacyjnego (51 osób) bądź zachowawczego (20 osób). U niektórych chorych (60 osób) przeprowadzono również badanie EMG. Wyliczono średnie wartości pól powierzchni oraz średnicy pni nerwowych poszczególnych grup patologii nerwów. Analizowane w pracy poszczególne cechy ultrasonograficzne nerwów obwodowych, takie jak echostruktura, objaw wcięcia, przekrwienie oraz ciągłość więzadła poprzecznego, podzielono na podgrupy. Obliczono również częstość występowania zrostów pni nerwowych z przylegającymi tkankami, częstość występowania dolegliwości bólowych w czasie ucisku nerwu głowicą oraz częstość występowania niestabilności nerwu łokciowego oraz zagięcia kątowego pnia nerwu międzykostnego tylnego w badaniu dynamicznym. Analizy zebranego materiału dokonano za pomocą statystyki opisowej. W odniesieniu do weryfikacji klinicznej i operacyjnej zgodność z rozpoznaniem ultrasonograficznym uzyskano w 100% przypadków. Uzyskane wyniki wskazują, że badanie ultrasonograficzne z zastosowaniem głowic wysokiej częstotliwości jest cenną metodą w kwalifikowaniu do rodzaju leczenia neuropatii obwodowych o charakterze pooperacyjnym.

The primary aim of this paper was to assess the relevance of high-frequency ultrasound examination in qualifying patients for either surgical or conservative treatment of posttraumatic peripheral neuropathies. The study was conducted in a group of 47 patients aged 16-65 (mean age 33) who in 2009-2011 were referred to ultrasound examinations due to a clinical suspicion of posttraumatic peripheral neuropathies. The group included 30 females and 17 males. The patients examined presented with neuropathies of the following peripheral nerves: median, ulnar, common peroneal, digital, cutaneous in the deltoid area, mental, PIN and RSNR. In 21 patients, nerve injuries were partial, and in 24 - complete. In 2 cases, the nerve was entrapped between bony fragments. 17 of 21 patients with partial nerve injuries (80.95%) underwent an EMG examination. No functional tests were conducted in the cases of complete injuries when ultrasound imaging had confirmed the result of the clinical examination. All patients underwent the interview, physical examination and ultrasound examination. Ultrasound examinations were performed with Esaote MyLab 50 and MyLab 60 systems using high-frequency broadband linear transducers: 6-18 MHz. The nerves were evaluated in the gray-scale and in the power Doppler mode in longitudinal and transverse sections for localization, morphology and the grade of injury as well as for possible anatomic variants of the nerve trunk and pathologies of the adjacent tissues. Moreover, a dynamic examination was performed, and it was attempted to induce pain or paresthesia by palpation at the site of the visualized pathology. Additionally, the motor and sensory-motor nerves were assessed indirectly based on the images of the skeletal muscles innervated by these nerves. The analyses of the collected material were performed by means of descriptive statistics. The results of clinical and surgical verification were consistent with ultrasound findings in 100% of cases. The results obtained indicate that high-frequency ultrasonography is a valuable method in qualifying patients for various types of treatment of peripheral neuropathies resulting from trauma.Podstawowym celem pracy była ocena przydatności badania ultrasonograficznego z zastosowaniem głowic wysokiej częstotliwości w kwalifikowaniu do leczenia operacyjnego albo zachowawczego neuropatii obwodowych o charakterze pourazowym. Materiał pracy stanowiła grupa 47 osób w wieku 16–65 lat (średnia wieku 33 lata) kierowanych w latach 2009–2011 na badanie USG z klinicznym podejrzeniem neuropatii pourazowych. W grupie tej było 30 mężczyzn i 17 kobiet. Zbadano pacjentów ze zmianami pourazowymi nerwów obwodowych: pośrodkowego, łokciowego, promieniowego, strzałkowego wspólnego, nerwów palcowych, skórnych okolicy naramiennej, nerwu bródkowego, PIN, RSNR. U 21 osób uszkodzenia miały charakter częściowy, u 24 całkowity, w 2 przypadkach doszło do usidlenia nerwu pomiędzy odłamami kostnymi. U 17 spośród 21 chorych z rozpoznaniem częściowego uszkodzenia nerwu (80,95%) wykonano badanie EMG. Nie przeprowadzano badań czynnościowych w przypadku uszkodzeń całkowitych, gdy obraz ultrasonograficzny potwierdził wynik badania klinicznego. U wszystkich pacjentów wykonano badanie podmiotowe, przedmiotowe oraz ultrasonograficzne. Do badań ultrasonograficznych wykorzystano aparaty Esaote MyLab 50 i MyLab 60 z zastosowaniem szerokopasmowych głowic liniowych o wysokich częstotliwościach: 6–18 MHz. Nerwy oceniano w skali szarości oraz w opcji dopplera mocy, w przekroju podłużnym i poprzecznym pod kątem lokalizacji, morfologii i stopnia uszkodzenia, ewentualnych wariantów anatomicznych pnia nerwu i patologii przylegających tkanek. Ponadto wykonywano badanie dynamiczne oraz próbę wywołania objawów bólu lub parestezji poprzez palpację miejsca uwidocznionej patologii, a także dokonywano pośredniej oceny nerwów ruchowych i czuciowo-ruchowych na podstawie obrazu unerwianych przez nie mięśni szkieletowych. Analizę zebranego materiału przeprowadzono za pomocą statystyki opisowej. W odniesieniu do weryfikacji klinicznej i operacyjnej zgodność z rozpoznaniem ultrasonograficznym uzyskano w 100% przypadków. Wyniki wskazują, że badanie ultrasonograficzne z zastosowaniem głowic wysokiej częstotliwości jest cenną metodą w kwalifikowaniu do rodzaju leczenia neuropatii obwodowych o charakterze pourazowym.

The primary aim of this paper was to assess the relevance of high-frequency ultra-sound examination in qualifying patients for either surgical or conservative treatment of peripheral entrapment neuropathies. The study was conducted in a group of 55 patients aged 7-83 (mean age 43.6), including 28 males and 27 females, who in 2009-2011 were referred to an ultrasound examination due to a clinical suspicion of entrapment neuropathies. For the purposes of the analysis, the patients were divided into four groups: carpal tunnel syndrome (1), ulnar nerve entrapment (2) (cubital tunnel syndrome and Guyon's canal syndrome), posterior interosseous nerve syndrome (3) and other entrapment neuropathies (4). The cases of isolated idiopathic carpal tunnel syndrome were excluded from the analysis. All patients underwent the interview, physical examination and ultrasound examination. Ultrasound examinations were performed with Esaote MyLab 50 and MyLab 60 systems using high-frequency broadband linear transducers: 6-18 MHz. Sixty-seven percent of patients (37 persons) underwent a neurophysiological test. Nerve echostructure, its hyperemia as well as nerve cross-sectional area or, in the case of small nerves, diameter were assessed in all patients. Furthermore, the following were assessed in individual groups: notch sign in group 1, nerve instability in a dynamic ultrasound examination in group 2, nerve angulation in a dynamic ultrasound examination and tenderness on nerve compression at the site of the visualized pathology in group 3. The analyses of the collected material were performed by means of descriptive statistics. The results of clinical and surgical verification were consistent with ultrasound findings in 96.4%. The results indicate that high-frequency ultrasonography is a valuable method in qualifying patients for various types of treatment of peripheral neuropathies resulting from compression.Podstawowym celem pracy była ocena przydatności badania ultrasonograficznego z zastosowaniem głowic wysokiej częstotliwości w kwalifikowaniu do leczenia operacyjnego albo zachowawczego neuropatii obwodowych o charakterze uciskowym. Materiał pracy stanowiła grupa 55 osób w wieku 7–83 lata (średnia wieku 43,6 roku), w tym 28 mężczyzn i 27 kobiet, kierowanych w latach 2009–2011 na badanie ultrasonograficzne z klinicznym podejrzeniem neuropatii uciskowych, na potrzeby analizy podzielonych na cztery grupy: zespół kanału nadgarstka (1), zespół ucisku nerwu łokciowego (2) (ucisk na poziomie rowka nerwu łokciowego oraz ucisk na poziomie kanału Guyona), zespół ucisku nerwu międzykostnego tylnego (3), inne neuropatie uciskowe (4). Z analizowanej grupy wyłączono przypadki izolowanego idiopatycznego zespołu kanału nadgarstka. U wszystkich pacjentów przeprowadzono badanie podmiotowe, przedmiotowe oraz badanie ultrasonograficzne. Badania ultrasonograficzne przeprowadzono aparatami Esaote MyLab 50 oraz MyLab 60 z zastosowaniem szerokopasmowych głowic liniowych o wysokich częstotliwościach: 6–18 MHz. U 67% pacjentów (37 osób) wykonano badanie neurofizjologiczne. U wszystkich pacjentów oceniano echostrukturę nerwu i jego przekrwienie oraz określano pole powierzchni przekroju nerwu albo – w przypadku drobnych nerwów – średnicę. Dodatkowo w grupie 1 oceniano objaw wcięcia, w grupie 2 – niestabilność nerwu w dynamicznym badaniu ultrasonograficznym, w grupie 3 – zaginanie kątowe nerwu w dynamicznym badaniu ultrasonograficznym oraz tkliwość w czasie ucisku nerwu w miejscu uwidocznionej patologii. Analizy zebranego materiału dokonano za pomocą statystyki opisowej. W odniesieniu do weryfikacji klinicznej i operacyjnej zgodność z rozpoznaniem ultrasonograficznym osiągnięto w 96,4% przypadków. Uzyskane wyniki wskazują, że badanie ultrasonograficzne z zastosowaniem głowic wysokiej częstotliwości jest cenną metodą w kwalifikowaniu do rodzaju leczenia neuropatii obwodowych o charakterze uciskowym.

BackgroundThree-dimensional (3D) ultrasound of the shoulder is characterized by a comparable accuracy to two-dimensional (2D) ultrasound. No studies investigating 2D versus 3D inter-rater reliability in the detection of supraspinatus tendon tears taking into account the level of experience of the raters have been carried out so far.ObjectivesThe aim of this study was to determine the inter-rater reliability in the analysis of 3D ultrasound image sets of the supraspinatus tendon between sonographer with different levels of experience.Patients and methodsNon-interventional, prospective, observational pilot study of 2309 images of 127 adult patients suffering from unilateral shoulder pain. 3D ultrasound image sets were scored by three raters independently. The intra-and interrater reliabilities were calculated.ResultsThere was an excellent intra-rater reliability of rater A in the overall classification of supraspinatus tendon tears (2D vs 3D κ = 0.892, pairwise reliability 93.81%, 3D scoring round 1 vs 3D scoring round 2 κ = 0.875, pairwise reliability 92.857%). The inter-rater reliability was only moderate compared to rater B on 3D (κ = 0.497, pairwise reliability 70.95%) and fair compared to rater C (κ = 0.238, pairwise reliability 42.38%).ConclusionsThe reliability of 3D ultrasound of the supraspinatus tendon depends on the level of experience of the sonographer. Due to the possibility to examine at the patient bedside or incubator, ultrasound imaging of the central nervous system, particularly through the anterior fontanelle, is the most common diagnostic examination performed in neonates and infants with neurological problems. Despite its common application, this method has certain limitations. These limitations are associated with cross-sections of the cerebral structures that can be obtained when examining through the anterior fontanelle. The aim of the paper was to assess the usefulness of three dimensional sonography of the central nervous system in neonates and infants in the assessment of intracranial bleeding and its consequences when examined through the anterior fontanelle. The study enrolled 283 patients treated at the Pediatric Teaching Hospital at the Medical University of Lublin in whom a transfontanelle cerebral examination was indicated. The two- and three-dimensional ultrasound examinations of the central nervous system were conducted in 283 patients aged from 1 day to 18 months (mean age: 2 months). 254 three-dimensional examinations were of diagnostic value. The number of detected pathological lesions was higher in a statistically significant way (p = 0.007) in the three-dimensional ultrasound examination. In the authors' own studies, the highest sensitivity and statistically significant superiority of the three-dimensional method over the two-dimensional one referred to detecting intraventricular or intracerebral hemorrhages. Novel techniques of ultrasound imaging, including the three-dimensional one, have undoubtedly increased the diagnostic possibilities of sonography and, at the same time, retained all its advantages.Ultrasonografia centralnego układu nerwowego, a szczególnie obrazowanie przez ciemiączko przednie, ze względu na możliwość wykonania badania przy łóżku pacjenta lub też w inkubatorze jest najczęściej stosowaną metodą diagnostyczną u noworodków i niemowląt z problemami neurologicznymi. We present an additional sonographic sign of Hashimoto's thyroiditis (HLT), increasing the specificity of this method in pediatric populations.A total of 98 children (mean age 12.7 years, range 7-17 years) were selected from the registry of the endocrinology outpatient department. All subjects met the diagnostic criteria for HLT. All children underwent a prospective thyroid ultrasound examination with special attention paid to the presence of lymph nodes adjacent to the thyroid gland. In order to form a control group, we analyzed 102 healthy volunteers and 94 children with cervical lymphadenopathy, age- and sex-matched with the main study group.The ultrasound of the thyroid revealed typical sonographic signs of autoimmune thyroid disease in all children with HLT and in none of the individuals in the control groups. In 96 children (98%) from the HLT group, at least 2 lymph nodes adjacent to the lower part of the thyroid gland lobes localized on one or both sides of the thyroid were detected. No lymph nodes adjacent to the lower part of the thyroid lobes were found in healthy children or children with cervical lymphadenopathy.Lymph nodes adjacent to the lower part of the thyroid lobes are an additional ultrasound sign of pediatric Hashimoto's lymphocytic thyroiditis, with 98% sensitivity and 100% specificity.W niniejszej pracy została przedstawiona dodatkowa cecha ultrasonograficzna przewlekłego limfocytowego zapalenia tarczycy, zwiększająca specyficzność tej metody w populacji dzieci.Z rejestru kliniki endokrynologii wybrano 98 dzieci (średnia wieku: 12,7 roku, przedział wiekowy: 7–17 lat). Wszyscy pacjenci spełniali kryteria diagnostyczne choroby Hashimoto. Każde dziecko poddano prospektywnemu badaniu ultrasonograficznemu tarczycy ze szczególnym uwzględnieniem występowania węzłów chłonnych przylegających do tarczycy. W celu utworzenia grupy kontrolnej analizie poddano 102 zdrowych ochotników oraz 94 dzieci z limfadenopatią szyjną, dopasowanych pod względem wieku i płci do badanej grupy.Badanie ultrasonograficzne tarczycy ujawniło typowe cechy sonograficzne autoimmunologicznej choroby tarczycy u wszystkich dzieci z chorobą Hashimoto i u nie ujawniło ich u żadnego pacjenta z grupy kontrolnej. U 96 chorych dzieci (98%) wykryto co najmniej dwa węzły chłonne przylegające do dolnej części płatów tarczycy, umiejscowione z jednej lub z dwóch stron gruczołu. Ultrasound assessment of morphological parameters of the internal jugular veins and vertebral veins in healthy individuals as well as their dependence on the patient's position.The examinations were conducted in 185 healthy individuals (101 females and 84 males) aged 18-89. Ultrasound examinations were conducted with the use of a linear probe with the frequency of 5-9 MHz in the supine (0°) and sitting position (90°).In 154 cases (83.2%) on the left side and in 150 cases (81.1%) on the right side, the jugular veins were completely closed in the sitting position. In 31 cases (16.8%) on the left side and in 35 cases (18.9%) on the right side, they were merely narrowed. By contrast with the jugular veins, the cross-sectional area (CSA) of the vertebral veins was greater in the sitting position than in the supine position in a statistically significant way. The CSA values of the jugular veins in the supine position ranged from 0 cm(2) to 4.3 cm(2). There were no statistically significant differences in the CSA between men and women. The cross-sectional area of the right jugular vein in the supine position was greater in a statistically significant way than that of the left jugular vein. In this study population, the ratio of the cross-sectional areas of the jugular veins on both sides amounted to 8.5:1.The width of the jugular and vertebral veins significantly varies depending on the patient's position. The range of the CSA values for the jugular veins is broad, which should be taken into account when interpreting imaging findings. The internal jugular veins can show considerable asymmetry.Ultrasonograficzna ocena parametrów morfologicznych żył szyjnych wewnętrznych i żył kręgowych u osób zdrowych oraz ich zależności od pozycji badanego.Zbadano 185 zdrowych osób: 101 kobiet i 84 mężczyzn w wieku 18–89 lat. Badanie USG wykonywano za pomocą sondy liniowej o częstotliwości 5–9 MHz w pozycji leżącej (0°) oraz w pozycji siedzącej (90°).W 154 przypadkach (83,2%) po stronie lewej i w 150 (81,1%) po stronie prawej po przyjęciu pozycji siedzącej światło żył szyjnych ulegało całkowitemu zamknięciu; w 31 (16,8%) przypadkach po stronie lewej oraz 35 (18,9%) przypadkach po stronie prawej ulegało jedynie zmniejszeniu. W przeciwieństwie do żył szyjnych pole przekroju poprzecznego (CSA) żył kręgowych w pozycji siedzącej było istotnie statystycznie większe niż w pozycji leżącej. Wartości CSA żył szyjnych w pozycji leżącej mieściły się w zakresie od 0 cmSzerokość żył szyjnych i żył kręgowych zmieniają się w zależności od pozycji ciała osoby badanej. Zakres wartości CSA żył szyjnych jest szeroki, co powinno być brane pod uwagę przy interpretacji wyników badań obrazowych tych naczyń. Żyły szyjne wewnętrzne mogą wykazywać znaczną asymetrię.

This study was designed to evaluate the performance of shear-wave elastography as a diagnostic tool for prostate cancer in a larger cohort of patients than previously reported.Seventy-three patients with suspected prostate carcinoma were investigated by ultrasound elastography followed by directed biopsy. The elastographic and histological results for all biopsies were compared.After exclusion of invalid and non-assessable results, 794 samples were obtained for which both a histological assessment and an elastometric result (tissue stiffness in kPa) were available: according to the histology 589 were benign and 205 were malignant. Tissue elasticity was found to be weakly correlated with patient's age, PSA level and gland volume. ROC analysis showed that, for the set of results acquired, elastometry did not fulfil literature claims that it could identify malignant neoplasia with high sensitivity and specificity. However, it did show promise in distinguishing between Gleason scores ≤6 and6 when malignancy had already been identified. Unexpected observations were the finding of a smaller proportion of tumours in the lateral regions of the prostate than generally expected, and also the observation that the elasticity of benign prostate tissue is region-sensitive, the tissue being stiffest in the basal region and more elastic at the apex.Shear-wave elastography was found to be a poor predictor of malignancy, but for malignant lesions an elasticity cut-off of 80 kPa allowed a fairly reliable distinction between lesions with Gleason ≤6 and those with Gleason6. We demonstrate an increase in elasticity of benign prostate tissue from the basal to the apical region.Badanie przeprowadzono w celu ustalenia wartości diagnostycznej elastografii fali poprzecznej w raku stercza, analizując większą grupę chorych, niż dotychczas przedstawiano w piśmiennictwie.Badaniem objęto 73 pacjentów z podejrzeniem raka stercza. Ocenę gruczołu krokowego wykonywano za pomocą elastografii, po której stosowano biopsję celowaną.Po wykluczeniu nieważnych i trudnych do oceny wyników uzyskano 794 przypadki, które oceniono zarówno histopatologicznie, jak i metodą elastografii (sztywność tkanek mierzona w kPa), stwierdzając w badaniu histopatologicznym 589 zmian łagodnych i 205 złośliwych. Wykazano słabą korelację sprężystości tkanek z wiekiem chorych, poziomem PSA i objętością gruczołu krokowego. Analiza ROC otrzymanych wyników udowodniła, że elastometria w diagnostyce ognisk złośliwego nowotworzenia nie jest tak czułą i swoistą metodą, jak zakładano w piśmiennictwie. W przypadkach, w których zdiagnozowano proces złośliwy, metoda ta umożliwia rozróżnienie ognisk ocenianych w skali Gleasona ≤6 od tych6 punktów. Te nieoczekiwane obserwacje dotyczyły mniejszego odsetka nowotworów, niż wcześniej zakładano, położonych w bocznych częściach prostaty. Ponadto zaobserwowano zależność między poziomem sprężystości tkanek objętych procesem łagodnym w poszczególnych częściach gruczołu krokowego: największą sztywność wykazano w części podstawnej, a większą sprężystość w części wierzchołkowej.Elastografia fali poprzecznej jest słabym predyktorem złośliwości, chociaż w diagnostyce ognisk złośliwych, z punktem odcięcia poziomu sprężystości wynoszącym 80 kPA, umożliwia dość wiarygodne zróżnicowanie zmian o stopniu zaawansowania ≤6 punktów i zmian o stopniu zaawansowania6 punktów w skali Gleasona.

Lung sonography allows rapid diagnosis of lung emergencies such as pulmonary edema, hemothorax or pneumothorax. The ability to timely diagnose an intraoperative pneumothorax is an important skill for the anesthesiologist. However, lung ultrasound exams require an interpretation of not only real images but also complex acoustic artifacts such as A-lines and B-lines. Therefore, appropriate training to gain proficiency is important. Simulated environment using ultrasound phantom models allows controlled, supervised learning. We have developed hybrid models that combine dry or wet polyurethane foams, porcine rib cages and human hand simulating a rib cage. These models simulate fairly accurately pulmonary sonopathology and allow supervised teaching of lung sonography with the immediate feedback. In-vitro models can also facilitate learning of procedural skills, improving transducer and needle positioning and movement, rapid recognition of thoracic anatomy and hand - eye coordination skills. We described a new model to teach an ultrasound guided thoracentesis. This model consists of the experimenter's hand placed on top of the water-filled container with a wet foam. Metacarpal bones of the human hand simulate a rib cage and a wet foam simulates a diseased lung immersed in the pleural fluid. Positive fluid flow offers users feedback when a simulated pleural effusion is accurately assessed.Badanie ultrasonograficzne płuc pozwala na szybkie (co jest bardzo istotne dla anestezjologa) rozpoznanie stanów nagłych płuc, takich jak np. śródoperacyjna odma opłucnowa, obrzęk płuc czy krwiak opłucnej. Jednakże wymaga interpretacji nie tylko rzeczywistych obrazów, lecz także skomplikowanych artefaktów akustycznych, takich jak linie A i linie B. Tym ważniejsze jest właściwe szkolenie w celu uzyskania biegłości diagnostycznej. Środowisko symulowane, wykorzystujące fantomy, umożliwia naukę w kontrolowanych warunkach pod okiem specjalisty. Autor niniejszej pracy opracował modele hybrydowe, które łączą suchą lub mokrą piankę poliuretanową, szkielet świńskiej klatki piersiowej oraz ludzką dłoń symulującą szkielet klaki piersiowej. The aim of the study was to compare the values of the sonographic inferior vena cava/ aorta index obtained with the transducer placed in the median line and right anterior axillary line.The study enrolled 45 volunteers, including 33 women, aged 22.5 ± 1.26 with a negative history of circulatory and renal diseases. The study consisted in a sonographic assessment and measurement of the inferior vena cava and aorta by placing the transducer in the anterior median line and right anterior axillary line. The value of the inferior vena cava/aorta index was obtained by calculating the ratio of the diameters of the inferior vena cava and aorta. The diameter measurements were taken by placing the ultrasound transducer in the anterior median line and right anterior axillary line. Two examiners performed the measurement three times and used convex probes of 3.5-5 MHz. Additionally, the subjects' weight, height and arterial blood pressure were taken.Following a statistical analysis with the use of STATISTICA software, the following values of the inferior vena cava/aorta index were obtained: in the anterior median line - 1.43 ± 0.21, and in the right anterior axillary line - 1.285 ± 0.19. There was no statistically significant difference between the measurements obtained by the two examiners (p = 0.17). A strong correlation was noted between the inferior vena cava/ aorta indices calculated in both sites of transducer placement, which was irrespective of the examiners (the correlation coefficient: r = 0.61 and r = 0.71). The study indicate that the inferior vena cava/aorta index measured in the right anterior axillary line is a simple and reproducible method for determining the body fluid status; the examinations conducted in both sites may be used interchangeably. The aim of this study was to evaluate the usefulness of ultrasonography in the diagnosis of hemorrhagic cystitis following bone marrow transplantation in children.The study involved an analysis of clinical material and the results of imaging tests performed in 334 patients who underwent hematopoietic cell transplantation. Ultrasonographic findings in 42 patients with hemorrhagic cystitis were analyzed in detail. The ultrasound images served to assess the severity of hemorrhagic cystitis and the results were compared with the clinical assessment of the disease on the Droller scale, as well as the laboratory and endoscopic tests.In the studied group of patients hemorrhagic cystitis following allogeneic transplantation was diagnosed in 12.5% cases. 73.8% patients received transplants from unrelated donors, 26.2% - from compatible siblings. The study revealed a higher incidence of hemorrhagic cystitis in children above 10 years of age. Grade 3 according to the Droller was diagnosed in 42.9%, grade 2 - in 30.9%, grade 4 - in 14.3%, and grade 1 - in 11.9% patients. The number of ultrasound examinations depended on the clinical symptoms, severity, duration and co-occurrence of other complications following the transplantation and was within the 1-15 range (average: 4.6). Grades 3 and 4 were related to the poor clinical condition of the patients and to their longer hospitalization. During this period there was an increased risk of renal malfunction and acute renal failure, post-inflammatory narrowing of the ureters, hydronephrosis, and in grade 4 the fibrosis of the bladder with reduced bladder capacity. Analyses demonstrated a significant correlation between the ultrasound image of the bladder wall and the clinical severity.Ultrasound with Doppler options remains the primary diagnostic tool in the evaluation of hemorrhagic cystitis, and is useful in terms of its diagnosis, determination of the severity, and monitoring of the treatment.Celem pracy była ocena przydatności ultrasonografii w diagnostyce krwotocznego zapalenia pęcherza moczowego, dość częstego powikłania po przeszczepach szpiku kostnego u dzieci.W pracy przeanalizowano materiał kliniczny i wyniki badań obrazowych 334 pacjentów po transplantacji komórek hematopoetycznych. Analizie poddano wyniki badań ultrasonograficznych wykonanych u 42 dzieci z krwotocznym zapaleniem pęcherza moczowego. Na podstawie USG oceniano stopień zaawansowania krwotocznego zapalenia pęcherza moczowego, a wyniki porównywano z oceną kliniczną procesu w skali Drollera, badaniami laboratoryjnymi i endoskopowymi.W analizowanej grupie dzieci krwotoczne zapalenie pęcherza moczowego rozpoznano po transplantacjach allogenicznych u 12,5% badanych, u 73,8% był to przeszczep od dawcy niespokrewnionego, u 26,2% – od zgodnego rodzeństwa. Wykazano częstsze występowanie krwotocznego zapalenia pęcherza moczowego u dzieci starszych, powyżej 10. roku życia. Stopień 3. w skali Drollera stwierdzono u 42,9% dzieci, stopień 2. – u 30,9%, stopień 4. – u 14,3%, a stopień 1. – u 11,9% dzieci. Liczba badań ultrasonograficznych i rozpoznanie zależały od objawów klinicznych, stopnia zaawansowania, czasu trwania oraz współwystępowania innych powikłań po transplantacji i wynosiła 1–15 (średnio 4,6). Trzeci i czwarty stopień wiązały się ze złym stanem klinicznym chorych, a także wydłużeniem czasu hospitalizacji. W okresie tym wzrastało ryzyko uszkodzenia czynności nerek i ostrej niewydolności nerek, pozapalnego zwężenia moczowodów, wodonercza, a w końcowym stadium zwłóknienia pęcherza moczowego. Stwierdzono istotną zależność obrazu ultrasonograficznego zmienionej w przebiegu zapalenia ściany pęcherza moczowego, ze stopniem klinicznego zaawansowania.Badanie ultrasonograficzne z opcjami dopplerowskimi pozostaje podstawowym badaniem diagnostycznym w ocenie krwotocznego zapalenia pęcherza moczowego, jest przydatne w rozpoznawaniu, określaniu stopnia zaawansowania choroby i monitorowaniu leczenia.

Sonoelastography is a technique that assesses tissue hardness/compressibility. Utility and sensitivity of the method in prostate cancer diagnostics were assessed compared to the current gold standard in prostate cancer diagnostics i.e. systematic biopsy.The study involved 84 patients suspected of prostate cancer based on elevated PSA levels or abnormal per rectal examination findings. Sonoelastography was used to evaluate the prostate gland. In the case of regions with hardness two-fold greater than that of symmetric prostate area (strain ratio2), targeted biopsy was used; which was followed by an ultrasound-guided 8- or 10-core systematic biopsy (regardless of sonoelastography-indicated sites) as a reference point.The mean age of patients was 69 years. PSA serum levels ranged between 1.02 and 885 ng/dl. The mean prostate volume was 62 ml (19-149 ml). Prostate cancer was found in 39 out of 84 individuals. Statistically significant differences in strain ratios between cancers and benign lesions were shown. Sonoelastography guided biopsy revealed 30 lesions - overall sensitivity 77% (sensitivity of the method - 81%). Sonoelastographic sensitivity increased depending on cancer stage according to the Gleason grading system: 6-60%, 7-75%, 8-83%, 9/10-100%. The estimated sensitivity of systematic biopsy was 92%.Sonoelastography shows higher diagnostic sensitivity in prostate cancer diagnostics compared to conventional imaging techniques, i.e. grey-scale TRUS, Doppler ultrasound. It allows to reduce the number of collected tissue cores, and thus limit the incidence of complications as well as the costs involved. Sonoelastography using the determination of compressibility ratio for symmetrical prostatic regions may prove useful in the detection of clinically significant prostate cancer.Elastosonografia jest techniką oceniającą twardość/ściśliwość tkanek. Badano użyteczność i czułość tej metody w diagnostyce raka stercza w porównaniu z obecnie obowiązującym złotym standardem w diagnostyce raka gruczołu krokowego – biopsją systematyczną.Badaniu poddano 84 chorych z podejrzeniem raka stercza na podstawie podwyższonego poziomu PSA lub nieprawidłowości w badaniuŚredni wiek chorych wynosił 69 lat. Poziom PSA w surowicy krwi mieścił się w zakresie 1,02–885 ng/dl. Średnia objętość gruczołu krokowego wynosiła 62 ml (19–149 ml). Pośród 84 badanych raka prostaty ujawniono u 39 osób. Wykazano statystycznie istotne różnice wartości stosunku odkształcenia między rakami i zmianami łagodnymi. Biopsja pod kontrolą elastosonografii ujawniła 30 zmian – czułość (Elastosonografia ma wyższą czułość w diagnostyce raka stercza niż konwencjonalnie używane techniki obrazowania, tj.: TRUS w skali szarości, USG dopplerowskie. Pozwala zredukować liczbę pobieranych rdzeni tkankowych, a co za tym idzie – zmniejszyć liczbę powikłań i ograniczyć koszty. Elastosonografia z oznaczeniem stosunku ściśliwości symetrycznych obszarów stercza może być pomocna w wykryciu klinicznie istotnego raka gruczołu krokowego.

Vesicoureteral reflux appears in 20-50% of pediatric patients with recurrent urinary tract infections. The most common method of diagnosing this disease is voiding cystourethrography. However, contemporary pediatric radiology does not favor this method due to exposure to X-radiation.The aim of this study was to assess the usefulness of 2D/3D/4D contrast-enhanced voiding urosonography in the diagnosis and treatment monitoring of vesicoureteral reflux in children and the possibility of using contrast-enhanced voiding urosonography to replace voiding cystourethrography.Voiding cystourethrography and contrast-enhanced voiding urosonography were conducted in 80 pediatric patients in order to assess sensitivity, specificity, positive and negative predictive values as well as the number of vesicoureteral refluxes detected by each of the two methods. The second stage of the study involved performing voiding urosonography in an extended protocol in 58 children in order to determine the usefulness of three-dimensional (3D/4D) examinations in the assessment of vesicoureteral reflux and the ability to assess the urethra.The concordance between the two methods was 86.95%. The sensitivity of voiding urosonography was 84.51%, specificity - 90.99%, positive predictive value - 85.71% and negative predictive value - 90.17%. A 3D/4D assessment of the urinary bladder and transperineal 2D morphological assessment of the urethra were possible in all patients (100%). Assessment of the urethra during micturition with the use of 2D/3D/4D techniques was possible in all patients in whom voiding was elicited (95.83%), and 3D/4D assessment of vesicoureteral reflux was possible in all patients with reflux (100%). Although the application of 3D/4D techniques allowed accurate specification of the grade of reflux in all cases (100%), it appeared particularly useful in differentiating between grades II and III (70.97%).Contrast-enhanced voiding urosonography allows the diagnosis and monitoring of treatment of vesicoureteral reflux in pediatric patients as well as assessment of the urethra in both girls and boys. The method is characterized by high sensitivity and specificity. Moreover, it is safe, relatively inexpensive and can replace voiding cystourethrography.Odpływy pęcherzowo-moczowodowe występują u 20–50% dzieci z nawracającymi zakażeniami układu moczowego. Najczęściej wykorzystywaną metodą w diagnostyce tego schorzenia jest cystouretrografia mikcyjna, od której nowoczesna radiologia pediatryczna stara się odchodzić, z powodu narażenia na promieniowanie rentgenowskie.Celem pracy była ocena przydatności sonocystografii mikcyjnej w diagnostyce i monitorowaniu leczenia odpływów pęcherzowo-moczowych u dzieci i możliwości zastąpienia omawianą metodą dotychczas stosowanej cystouretrografii mikcyjnej.U 80 dzieci wykonano cystouretrografię mikcyjną oraz sonocystografię mikcyjną w celu określenia czułości, swoistości, wartości predykcyjnej dodatniej i ujemnej sonocystografii mikcyjnej oraz ilości wykrywanych odpływów pęcherzowo-moczowych przez każdą z metod. W drugim etapie u 58 dzieci wykonano sonocystografię mikcyjną w rozszerzonym protokole w celu określenia przydatności badania trójwymiarowego (3D/4D) w ocenie odpływów pęcherzowo-moczowych oraz możliwości oceny cewki moczowej.Zgodność obu metod wynosiła 86,95%. Czułość sonocystografii mikcyjnej wynosiła 84,51%, swoistość 90,99%, wartość predykcyjna dodatnia 85,71%, ujemna 90,17%. Ocena 3D/4D pęcherza moczowego oraz ocena morfologiczna 2D cewki moczowej z dostępu przezkroczowego możliwa była u wszystkich pacjentów (100%). Ocena cewki moczowej w trakcie mikcji techniką 2D/3D/4D możliwa była u wszystkich pacjentów, u których uzyskano mikcję (95,83%), a ocena 3D/4D OPM – u wszystkich pacjentów z odpływem (100%). Zastosowanie technik 3D/4D pozwoliło na precyzyjne określenie stopnia odpływów we wszystkich przypadkach (100%), jednakże szczególnie przydatne okazało się w różnicowaniu pomiędzy stopniem II a III (70,97%).Sonocystografia mikcyjna jest metodą umożliwiającą diagnostykę i monitorowanie leczenia odpływów pęcherzowo-moczowodowych u dzieci oraz ocenę cewki moczowej u dziewcząt i chłopców. Metodę charakteryzuje wysoka czułość i swoistość – jest bezpieczna, stosunkowo niedroga i może zastąpić cystouretrografię mikcyjną.

Compared to adult patients, ultrasonography in children and adolescents is much more common, due to lack of ionizing radiation, and its wide availability. With the introduction of contrast-media for use in ultrasonography, one major drawback of the method could be overcome. In Europe, SonoVue(®) is the only widely available agent, which due to improved stability makes it possible to image normal and diseased tissue perfusion and vascularization with high accuracy. Inability to hold the breath and voluntary body movement of the patient is less of an obstacle compared to color Doppler techniques and makes the method very attractive for use in children, which, depending on age, may not be very cooperative. Use of intravenous contrast-medium in minors is currently very limited for several reasons: availability, lack of recommendation in national and international guidelines, and lack of official licensing. The article will touch medical indications, technique, safety considerations, and perspective of intravenous use of contrast-media in children and adolescents, including data from a 6-year period in 37 patients.The purpose of the study was to collect data on ultrasonographic examinations, expanded by intravenous administration of the contrast agent SonoVue(®) in children and adolescents. Besides assessing diagnostic yield, data on adverse medication effects was collected.The study includes contrast-enhanced ultrasound examinations in 37 children at a single institution. Indications for the examinations were tumor lesions, infections, traumatic organ injuries, and parenchymal organ ischemia. Parents of the patients and adolescent patients were informed about the off-label use of the contrast agent. Thirty-nine examinations were performed, the average age of the patient was 11.1 years (range 1 to 17 years).All of the examinations yielded additional diagnostic value, always expanding results from B mode and color coded sonography. Overall, most examinations were done to assess the liver (n=16), followed by the kidney in 10 cases. The different etiologies were encountered in the following order: tumor (n=22), infection (n=9), trauma (n=5), ischemia (n=4). Most examinations were performed to evaluate a hepatic lesion (n=12). There was one incident recorded that fit the criteria of a possible adverse effect. In an 8-year-old girl nausea was noted, that started 15 minutes after the end of the examination and resolved spontaneously. In none of the patients medical treatment for adverse effects was necessary.Ultrasonography in children, enhanced by intravenous use of contrast medium is feasible and allows for further evaluating cystic and solid tumors, and organ perfusion. Given that proper medical equipment and correct ultrasound machine settings are used, it is a robust method without diagnostic failures. In this small-sized case series there were no severe adverse effects, however, off-label use in children needs to be addressed.Badania ultrasonograficzne są wykonywane w populacji pediatrycznej znacznie częściej niż u osób dorosłych. Metoda ta jest szeroko dostępna oraz nie naraża pacjenta na promieniowanie jonizujące. Wraz z wprowadzeniem do ultrasonografii środków kontrastujących przezwyciężono jedno z ograniczeń metody, jakim był brak możliwości oceny perfuzji. W Europie szeroko stosowanym ultrasonograficznym środkiem kontrastującym jest SonoVueCelem badania była ocena przydatności badań ultrasonograficznych poszerzonych o zastosowanie dożylnego środka kontrastującego SonoVueBadania ultrasonograficzne z zastosowaniem dożylnego środka kontrastującego wykonano u 37 dzieci w ośrodku autora. Przeprowadzono 39 badań. Średnia wieku pacjentów wynosiła 11,1 roku (zakres od 1 do 17 roku). Wskazaniami do badania były guzy, infekcje, urazy i niedokrwienie narządów wewnętrznych. Rodziców pacjentów oraz młodzież poinformowano o braku rejestracji stosowanego środka kontrastującego.We wszystkich wykonanych badaniach z zastosowaniem dożylnego ultrasonograficznego środka kontrastującego uzyskano dodatkowe, istotne z punktu widzenia klinicznego informacje, uzupełniające badania w opcji B-mode i z wykorzystaniem dopplera kodowanego kolorem. Większość badań dotyczyła oceny zmian w wątrobie (n=16), w 10 przypadkach w nerkach. Stwierdzono następujące nieprawidłowości: guzy (n=22), infekcje (n=9), urazy (n=5) oraz upośledzenie przepływu (n=4). Większość badań przeprowadzono w celu oceny zmiany zlokalizowanej w wątrobie (n=12). Odnotowano jeden przypadek odpowiadający kryteriom możliwej reakcji niepożądanej – u ośmioletniej dziewczynki 15 minut po zakończeniu badania wystąpiły nudności, które samoistnie ustąpiły. U żadnego pacjenta nie było konieczne leczenie reakcji niepożądanych.Badanie ultrasonograficzne z zastosowaniem dożylnego środka kontrastującego u dzieci jest możliwe do przeprowadzenia i pozwalana dokładniejszą ocenę zmian ogniskowych (torbielowatych i litych) oraz perfuzji narządowej. Przy zastosowaniu odpowiedniej aparatury oraz właściwych ustawień stanowi doskonałą metodę, dzięki której można uniknąć pomyłek diagnostycznych. W badanej grupie nie odnotowano reakcji niepożądanych. Problemem pozostaje brak rejestracji ultrasonograficznych środków kontrastujących u dzieci.

The result of therapeutic success is always the effect of medical professionals cooperation. The creation of adequate mechanisms of cooperation of these teams demands time and appropriate examples. In the understanding of differentiated behaviors in the line patient - diagnostician - surgeon, particularly the mechanism of the cascade of errors formation, giving simple examples may help - their awareness will facilitate the formation of an adequate pattern of diagnostic-therapeutic chain. The therapeutic team formed in this way provides optimal forms of cooperation and positive result. One of the elements of the cooperation is the surgical procedure qualification card which is an example of the communication between surgeon and diagnostician. The propagation of proven examples seems to be justified by practical reasons. The introduction of the surgical procedure qualification card enabled maintaining of the preoperative and postoperative diagnoses in the range from 88.4% to 89.29%, the barrier of 90% however is still not achieved. The diagnoses discrepancy is still the most often occurring patient safety incident and our results should head towards its mineralization. In particularly complicated cases we come back to a well-known form of medical consultation, that is the form of examination and treatment establishment basing on simultaneous physical and ultrasound examination - hence the colloquial name of ultrasound consultation. The universality of medical consultation makes out of it an excellent tool, particularly in cases of significant discrepancy between physical and ultrasound examination. This is excellent form of the experience exchange and learning about mutual possibilities. We believe that the mechanisms presented will influence the improvement of patient security.Wynik procesu leczniczego jest zawsze efektem pracy zespołu profesjonalistów medycznych. Zbudowanie prawidłowych mechanizmów pracy takich zespołów wymaga czasu i odpowiednich wzorców. W zrozumieniu zróżnicowanych zachowań na liniach pacjent – diagnosta – chirurg, zwłaszcza mechanizmu tworzenia się kaskad błędów, może pomóc przedstawienie prostych przykładów – ich świadomość ułatwi tworzenie prawidłowego wzorca łańcucha diagnostyczno-leczniczego. Powstały w ten sposób zespół terapeutyczny zapewnia optymalne formy współpracy i korzystny wynik. Jednym z elementów tej współpracy jest karta kwalifikacyjna do zabiegu operacyjnego, będąca przykładem komunikacji chirurga i diagnosty. Propagowanie sprawdzonych wzorców wydaje się w pełni uzasadnione względami praktycznymi. Wprowadzenie karty kwalifikacyjnej do leczenia operacyjnego pozwoliło na utrzymanie zgodności rozpoznań przedoperacyjnych i pooperacyjnych w granicach od 88,44% do 89,29%, wciąż jednak nie udaje się przekroczyć bariery 90%. Rozbieżność diagnoz jest nadal najczęściej występującym zdarzeniem niepożądanym i nasze wysiłki powinny iść w kierunku jego minimalizacji. W przypadkach szczególnie skomplikowanych wracamy do sprawdzonej formuły konsylium lekarskiego, czyli formy badania i ustalania rozpoznania w oparciu o jednoczasowe badanie fizykalne i ultrasonograficzne – stąd potocznie funkcjonująca nazwa konsylium ultrasonograficznego. Uniwersalność konsylium lekarskiego czyni z niego doskonałe narzędzie, zwłaszcza w przypadkach dużej niezgodności badania fizykalnego i ultrasonograficznego. To znakomita forma wymiany doświadczeń i poznania wzajemnych możliwości. Ufamy, że przedstawione mechanizmy wpłyną na poprawę bezpieczeństwa pacjenta.

was to analyze clinical material concerning postoperative atrophy of abdominal integument.The evaluated group consisted of 29 patients with sonographically revealed atrophy of the abdominal wall. Those changes were observed after various surgical procedures: mainly after long, anterolateral laparotomies or several classical operations. Ultrasound examinations up to the year 2000 were performed with analog apparatus, in the latter years only with digital apparatus with linear transducers (7-12 MHz) and sometimes convex type conducers (3-5 MHz). The location, size and intestine stratified wall structure were evaluated. In each case the integument thickness was measured in millimeters in the site of the greatest atrophy and it was compared with the integument thickness from the side that had not been operated which enabled the calculation of the percentage reduction of integument in the area of the scar.In 3 patients who underwent several laparotomies there was a total reduction of muscular mass in the operated area. In these cases we stated only skin and slightly echogenic subcutaneous strand; probably corresponding to fibrous tissue - the thickness of integument in this area was in the range from 3 to 8 mm. In the remaining 26 patients the integument atrophy on the scar level included muscles in a greater extent and covered an extensive area after classical urological procedures on the upper urinary tract: after nephrectomy and even ureter stone evacuation or kidney cyst excision by means of classical anterolateral approach with the integument incision on the length of almost 20 cm. Reduction in the integument thickness was observed on the smaller area after classical cholecystectomies, appendectomies and other surgical procedures with the incision across the integument. The integument atrophy in the operated sites expressed in absolute numbers was in the range of 7-20 mm (average 14 mm). These values are markedly lower than the comparative integument thickness on the not operated side: 17-52 mm (average 25.4 mm). The percentage value of the integument thickness reduction oscillated in the range of 32-67% (average 44.2%). In most cases the atrophy involved all layers of the abdominal wall, what demonstrated as regional prominence of the integument, mimicking the presence of hernia.Ultrasonography allows precise evaluation of the size and extent of atrophy as well as depiction of other lesions simulating that effect. Establishing the correct diagnosis should prevent the unnecessary reconstructions of the abdominal integument.była analiza materiału klinicznego dotyczącego zaniku powłok brzusznych po zabiegach operacyjnych.Zgromadzono grupę 29 chorych z wykazanym ultrasonograficznym zanikiem powłok brzusznych. Zmiany te obserwowano po różnych procedurach operacyjnych, najczęściej po długich nacięciach laparotomijnych przednio-bocznych lub kilkukrotnych operacjach klasycznych. Badania ultrasonograficzne wykonano do 2000 roku na aparatach analogowych, a w następnych latach wyłącznie aparatami cyfrowymi, z głowicami linowymi (7–12 MHz) i niekiedy głowicami typu konweks (3–5 MHz). Określano lokalizację, rozległość, warstwową budowę ściany jelita. W każdym przypadku mierzono grubość powłok w milimetrach w miejscu największego zaniku i porównywano ją z grubością powłok po stronie nieoperowanej, co pozwalało na obliczenie procentowej redukcji powłok w okolicy blizny.U 3 pacjentów po kilku laparotomiach doszło do całkowitej redukcji masy mięśniowej w miejscu operacji. W tych przypadkach stwierdzano jedynie skórę i lekko echogeniczne pasmo podskórne, prawdopodobnie odpowiadające zwłókniałym tkankom – grubość powłok w tym miejscu wahała się od 3 do 8 mm. U pozostałych 26 pacjentów zanik powłok na poziomie blizny pooperacyjnej dotyczył w największym stopniu mięśni i obejmował rozległy obszar po klasycznych zabiegach urologicznych na górnych drogach moczowych: po usunięciu nerki, a nawet po ewakuacji złogu z moczowodu lub wycięciu torbieli nerki z dostępu klasycznego przednio-bocznego z nacięciem powłok na długości prawie 20 cm. Na mniejszym obszarze obserwowano redukcję grubości powłok po klasycznych cholecystektomiach, appendektomiach i innych operacjach z nacięciem powłok na całej grubości. Zanik powłok w miejscach operacji wyrażał się w liczbach bezwzględnych w zakresie 7–20 mm (średnio 14 mm). Wartości te są wyraźnie mniejsze od porównawczej grubości powłok po stronie nieoperowanej: 17–52 mm (średnio 25,4 mm). Procentowa wielkość redukcji grubości powłok wahała się w granicach 32–67% (średnio 44,2%). W większości przypadków zanik obejmował wszystkie warstwy ściany brzucha, co manifestowało się uwypukleniem powłok w tym rejonie pozorującym istnienie przepukliny.Ultrasonografia pozwala z dużą precyzją określić stopień zaniku, jego rozległość oraz zmiany, które mogą pozorować taki efekt. Ustalenie właściwego rozpoznania powinno zapobiec zbędnej rekonstrukcji powłok brzusznych.

Abdominal hernias are usually located in the inguinal canals. Differential diagnosis of hernias should consider other pathologies, which typically occur at this site. The aim of this review paper is to present fat-containing lesions located in the inguinal canal based on both our own experience and literature. The first part of the paper describes the embryogenesis and differences in the structure and content of male and female inguinal canal. The canal is clearly wider in men, which results from the presence of the spermatic cord and its components. The female canal contains only the narrow round ligament of the uterus. The second part of the paper discusses the incorrect use of the term “lipoma” for retroperitoneal fat, which penetrated into the lumen of the patent processus vaginalis – the authors using this term acknowledge its incorrectness. These lesions often coexist with oblique inguinal hernia and should be excised during a surgery as they may cause inguinal pain. As for other fat-containing inguinal pathologies, we discussed lipomatosis of the spermatic cord, liposarcoma and the greater omentum. Differential diagnosis of these lesions considers their echogenicity, echostructure and vascularity. Furthermore, attention was paid to the adipose tissue surrounding the lower epigastric vessels, which may be incorrectly localized in the inguinal canal. Finally, the importance of proper diagnostic methodology and knowledge of the inguinal anatomy for the correct ultrasonographic diagnosis was emphasized.

A 5.5-month-old girl was admitted with non-specific signs and symptoms like dyspnea at rest, tachypnea, fatigue, low body weight and cyanosis on exertion. Physical examination revealed a barrel-shaped chest; no pathological sounds over the heart or both lung areas were detected. The above mentioned symptoms might suggest a circulatory problem. Echocardiography and computed tomography angiography were performed. These examinations revealed supracardiac type total anomalous pulmonary venous drainage. Echocardiographic signs of pulmonary hypertension and severe right ventricle overload were detected. Detailed analysis of both imaging examinations revealed atypical obstruction of the pulmonary venous return: narrowing of the proximal part of superior vena cava. An urgent surgery was performed, with no complications in the postoperative period. A follow-up echocardiography showed normalization of cardiac function and pulmonary pressure as well as normalization of flow profile within the superior vena cava. The paper presents a non-invasive diagnostic process in the described case, and discusses the causes of late diagnosis. A 5.5-month-old girl was admitted with non-specific signs and symptoms like dyspnea at rest, tachypnea, fatigue, low body weight and cyanosis on exertion. Physical examination revealed a barrel-shaped chest; no pathological sounds over the heart or both lung areas were detected. The above mentioned symptoms might suggest a circulatory problem. Echocardiography and computed tomography angiography were performed. These examinations revealed supracardiac type total anomalous pulmonary venous drainage. Echocardiographic signs of pulmonary hypertension and severe right ventricle overload were detected. Rheumatoid arthritis is the most common inflammatory arthritis, affecting approximately 1% of the world's population. Its pathogenesis has not been completely understood. However, there is evidence that the disease may involve synovial joints, subchondral bone marrow as well as intra- and extraarticular fat tissue, and may lead to progressive joint destruction and disability. Over the last two decades, significant improvement in its prognosis has been achieved owing to new strategies for disease management, the emergence of new biologic therapies and better utilization of conventional disease-modifying antirheumatic drugs. Prompt diagnosis and appropriate therapy have been recognized as essential for improving clinical outcomes in patients with early rheumatoid arthritis. Despite the potential of ultrasonography and magnetic resonance imaging to visualize all tissues typically involved in the pathogenesis of rheumatoid arthritis, the diagnosis of early disease remains difficult due to limited specificity of findings. This paper summarizes the pathogenesis phenomena of rheumatoid arthritis and describes rheumatoid arthritis-related features of the disease within the synovium, subchondral bone marrow and articular fat tissue on MRI and ultrasound. Moreover, the paper aims to illustrate the significance of MRI and ultrasound findings in rheumatoid arthritis in the diagnosis of subclinical and early inflammation, and the importance of MRI and US in the follow-up and establishing remission. Finally, we also discuss MRI of the spine in rheumatoid arthritis, which may help assess the presence of active inflammation and complications.

The paper presents a description of essential equipment requirements for scrotal ultrasonography, including current ultrasound techniques, as well as a review of the most common scrotal pathologies. Patient preparation for the examination as well as ultrasound methodology for the assessment of scrotal and inguinal canal structures are discussed. The standard for scrotal ultrasound examination includes a precise B-mode evaluation, including testicular volumetric assessment performed using automatic measurement options based on the formula of a rotating ellipsoid or three measurements perpendicular to one another. Also, criteria for morphological assessment of abnormalities within testicular or epididymal parenchyma, including a precise evaluation of lesion size, delineation, shape and vascular pattern obtained with Doppler US, have been proposed. Standard assessment further includes epididymal evaluation, including epididymal size in the case of enlargement. The paper additionally discusses the method of ultrasonographic examination and describes the most common pathologies occurring within scrotal structures, including a quantitative analysis of hydrocele and other abnormal fluid reservoirs. We have also presented criteria for the assessment of varicocele as well as color and spectral Doppler flows in scrotal pathologies. Furthermore, we have proposed key components of scrotal ultrasound documentation, so that the contained data could be used to establish appropriate diagnosis, allowing for both adequate clinical management and the reproducibility of subsequent US evaluations performed by either the same or a different examiner. The most common causes of diagnostic errors have also been discussed.

The article discusses the principles of the proper performance of the ultrasound examination of the prostate gland. The paper has been divided into two parts: the general one and the detailed one. The first part presents the necessary requirements referring to the ultrasound apparatus for performing transabdominal examinations of the urinary bladder and the prostate gland as well as for transrectal examinations of the prostate gland. The paper also describes the techniques of performing both examinations together with the methods of measuring the capacity of the urinary bladder and the volume of the prostate gland. It also mentions the most frequent indications for performing the examinations as well as diagnostic algorithms applied in case of finding irregularities. The transabdominal ultrasonography is a part of the examination of the abdominal organs and it should be performed in patients complaining of dysuric symptoms. An addition to the examination, especially when the prostate gland is enlarged, should be the measurement of the capacity of the urinary bladder and the assessment of the amount of residual urine after voiding. The indications for the endosonographic examination of the prostate gland are patological changes found in the per rectum examination, elevated concentration of the prostate-specific antigen in the blood serum, cancer and inflammations of the prostate gland if an abscess is suspected, qualification for surgery in the course of benign prostatic hyperplasia and the diagnostics of disorders of ejaculation. A standard procedure performed in case of prostate cancer for the purpose of obtaining specimens for the histopathology examination is biopsy carried out with transrectal ultrasound imaging. The paper presents the indications and techniques of performing prostate biopsy, as well as the types of biopsies together with the necessary preparation and the protection against its side effects for the patient. The paper also lists the necessary elements of the description of the presented procedures. The second part of the paper presents the application of the ultrasound examination in benign prostatic hyperplasia, in cases of inflammation and in prostate cancer.W artykule omówiono zasady prawidłowego wykonania badania ultrasonograficznego gruczołu krokowego. Pracę podzielono na dwie części: ogólną i szczegółową. W części pierwszej przedstawiono niezbędne wymagania dotyczące aparatury ultrasonograficznej do badań przezbrzusznych pęcherza moczowego i gruczołu krokowego, jak również do badań przezodbytniczych gruczołu krokowego. Opisano również techniki wykonania obu badań wraz z metodami pomiaru pojemności pęcherza moczowego i objętości gruczołu krokowego. W pracy omówiono także najczęstsze wskazania do wykonania badań, jak również algorytmy diagnostyczne w przypadku stwierdzenia nieprawidłowości. Badanie przez powłoki jamy brzusznej jest częścią badania narządów jamy brzusznej i powinno być wykonywane u chorych skarżących się na objawy dyzuryczne. Uzupełnieniem badania, szczególnie gdy gruczoł krokowy jest powiększony, powinny być pomiar pojemności pęcherza moczowego i ocena ilości moczu zalegającego po mikcji. Wskazaniami do wykonania badania endosonograficznego gruczołu krokowego są zmiany wyczuwalne palcem w trakcie badania przez odbyt, podwyższone stężenie antygenu sterczowego w surowicy krwi, rak oraz stany zapalne gruczołu krokowego, w tym podejrzenie ropnia, kwalifikacja do zabiegu operacyjnego w przebiegu łagodnego rozrostu stercza oraz diagnostyka zaburzeń wytrysku nasienia. Standardową procedurą wykonywaną w raku stercza w celu uzyskania wycinków do badania histopatologicznego jest biopsja pod kontrolą ultrasonografii przezodbytniczej. Przedstawiono wskazania i techniki wykonania biopsji stercza, jak również rodzaje biopsji, wraz z niezbędnym przygotowaniem oraz zabezpieczeniem przed jej skutkami niepożądanymi dla pacjenta. Wymieniono także niezbędne elementy opisu omawianych procedur. W drugiej części pracy przedstawiono zastosowanie badania ultrasonograficznego w łagodnym rozroście gruczołu krokowego, w stanach zapalnych oraz w raku stercza.

Obesity as a disease of affluence also affects younger children. Numerous observations suggest a link between excessive body weight and thyroid function disorders. Subclinical hypothyroidism has been diagnosed increasingly frequently in patients with obesity. A growing number of papers also point to morphological changes of the thyroid gland in the ultrasound examination in obese children. These reports mainly concern changes in echogenicity. The present paper discusses the most important aspects of this topic on the basis of the literature as well as containing a brief analysis based on own experiences.Otyłość jako choroba cywilizacyjna dotyka również dzieci w młodszych grupach wiekowych. Liczne obserwacje sugerują istnienie związku między nadmierną masą ciała a zaburzeniami funkcji tarczycy. U pacjentów z otyłością coraz częściej rozpoznaje się subkliniczną niedoczynność tego gruczołu. Coraz więcej prac wskazuje też na zmiany morfologiczne tarczycy w badaniu ultrasonograficznym u otyłych dzieci. Doniesienia te dotyczą głównie zmian echogeniczności. Praca zawiera omówienie najważniejszych aspektów tego tematu na podstawie literatury oraz krótką analizę zagadnienia opartą na własnych doświadczeniach.

This is mainly a submaximal exercise stress test, while the maximal exercise stress test is used in the research of ischemia. Echocardiography can be performed at rest and during submaximal exercise stress test. Few papers have proposed exercise echocardiography as a relevant diagnostic tool in heart failure with preserved ejection fraction. The E/e' ratio and the estimated pulmonary artery pressure by maximal tricuspid regurgitation velocity should be measured during standardized exercise. Stroke volume and its change during exercise should be also assessed. In fact, unlike in a normal compliant heart, there is no increase in left ventricular end-diastolic volume during exercise and consequently no increase in cardiac output in heart failure with preserved ejection fraction. The absence of increased cardiac output during exercise is, like E/e' and estimated pulmonary artery pressure, a major parameter to be investigated during submaximal exercise performed to confirm the diagnosis of heart failure with preserved ejection fraction as an etiology of dyspnea. The paper discusses the issue of spreading medical knowledge, connected particularly with ultrasonography, by the social media. Such a way of sharing knowledge and experience results from the needs of recipients - physicians who daily have limited free time. The paper presents the phenomenon of the free open access medical education (FOAM) along with its genesis, an open and global nature as well as the main communication channels. It is emphasized that education via the social media is becoming an element of the mainstream medical didactics. The aforementioned phenomenon is depicted in greater detail in the context of emergency ultrasonography. US imaging is one of the more popular issues in the FOAM community. The paper focuses on the Ultrasound Podcast and the initiative associated with it. Our native (Polish) project, CriticalUSG, is also presented together with its numerous editions. Apart from these two projects, other initiatives, which are equally important not only due to ultrasonography, are also briefly mentioned. The assessment of the body fluid status is one the most challenging tasks in clinical practice. Although there are many methods to assess the body fluid status of patients, none of them is fully satisfactory in contemporary medical sciences. In the article below, we compare the results of measurements performed by experienced and inexperienced examiners based on the inferior vena cava/aorta diameter index in a sonographic hydration assessment. The study enrolled 50 young students at the age of 19–26 (the median age was 22.95) including 27 women and 23 men. The volunteers were examined in the supine position with GE Logiq 7 system and a convex transducer with the frequency of 2–5 MHz. The measurements were performed in the longitudinal and transverse planes by two inexperienced examiners – the authors of this paper, following a four-hour training conducted by an experienced sonographer. The longitudinal values of the inferior vena cava/aorta diameter index obtained in this study were similar to those found in the literature. The reference value for the inferior vena cava/aorta index determined by Kosiak et al., which constituted 1.2 ± 2 SD, for SD = 0.17, was similar to the values obtained by the authors of this paper which equaled 1.2286 ± 2 SD, for SD = 0.2. The article presented below proves that measuring the inferior vena cava/aorta diameter index is not a complex examination and it may be performed by physicians with no sonographic experience. Furthermore, the paper demonstrates that the inferior vena cava/aorta diameter index measured in the transverse plane is similar to the inferior vena cava/aorta diameter index determined in the longitudinal plane. Thus, both measurements may be used interchangeably to assess the hydration status of patients.

Minor procedures with an injection needle are frequently performed in orthopedic clinics. They may be of a diagnostic, diagnostic and therapeutic or purely therapeutic nature. Ultrasound guidance while inserting the needle allows for a safe medicine administration and evacuation of fluid contents. It improves the efficacy of such procedures by ensuring accurate needle insertion in the target site. Ultrasound-guided procedures reduce the duration of treatment (e.g. medicines reach the target site directly; all fluid collections are removed, even multilocular ones) and minimize pain (by the selection of optimal access sites omitting tendons, vessels and peripheral nerves). This paper presents the principles of performing ultrasound-guided interventions. A detailed description of such a technique is provided and the most commonly injected medicines as well as their adverse reactions and contraindications are discussed. Attention is also paid to image optimization and the role of procedure site selection with the exclusion of other pathologies in the puncture site (such as tumors, foreign bodies and vascular or nerve pathologies). What is more, the author also discusses the principles of needle length and thickness selection as well as the manners of its insertion in relation to the transducer. Moreover, the principles of aseptics that are mandatory during such interventions are also presented and the way to protect transducers from the effects of chemical disinfectants is discussed. Furthermore, the paper contains numerous photographs of performed interventions. It is addressed to clinical practitioners and its aim is to facilitate and improve the efficacy of the procedures which are commonly performed in orthopedics. Ultrasound guidance of joint and soft tissue interventions should become a gold standard in all orthopedic clinics.Częstą praktyką w gabinecie ortopedycznym są drobne zabiegi z użyciem igły iniekcyjnej. Mogą mieć charakter diagnostyczny, diagnostyczno-terapeutyczny oraz czysto terapeutyczny. Kontrola ultrasonograficzna prowadzenia igły pozwala na bezpieczne podawanie leków i wykonywanie ewakuacji treści płynnych. Poprawia skuteczność takich zabiegów, zapewniając precyzyjne umieszczenie igły w docelowym miejscu. Wykonywanie zabiegu pod kontrolą obrazu ultrasonograficznego skraca czas leczenia (np. lek zostaje precyzyjnie wprowadzony w miejsce docelowe, ewakuowana zostaje cała zawartość przestrzeni płynowych, nawet wielokomorowych), minimalizuje dolegliwości bólowe (poprzez wybór optymalnego miejsca dostępu, z pominięciem ścięgien, naczyń i nerwów obwodowych). W pracy przedstawiono zasady wykonywania interwencji pod kontrolą ultrasonografii, zaczynając od szczegółowego zreferowania techniki zabiegów, a kończąc na omówieniu najczęściej stosowanych leków, ich skutków ubocznych oraz przeciwwskazań. Uwzględniono zasady optymalizacji obrazu, opisano rolę wyboru miejsca wykonania procedury z wykluczeniem innych patologii okolicy wkłucia (guzów, ciał obcych, patologii naczyniowych lub nerwowych), opisano zasady doboru długości i grubości igieł, sposoby ich prowadzenia względem czoła głowicy. Przedstawiono zasady aseptyki obowiązującej w czasie interwencji. Omówiono również sposoby ochrony głowic przed działaniem odkażających środków chemicznych. Praca zawiera wiele zdjęć z przeprowadzanych interwencji. Jest adresowana do klinicystów praktyków. Ma na celu ułatwienie i poprawę skuteczności tych zabiegów, powszechnie wykonywanych w praktyce ortopedycznej. Monitorowanie nakłuć stawów i tkanek miękkich za pomocą obrazu ultrasonograficznego powinno stać się złotym standardem, obowiązującym we wszystkich gabinetach ortopedycznych.

The role of a standard is to describe examination techniques, recommended norms as well as principles necessary to obtain results and draw appropriate conclusions, rather than a description of pathologies. The paper presents a technique for carotid artery examination as well as currently recommended standards. According to San Francisco Consensus from 2003, a significant stenosis of 70% may be detected when systolic velocity in the internal carotid artery stenosis is230 cm/s, and the diastolic velocity is100 cm/s. The common carotid artery velocity is also taken into account. Hemodynamic evaluation is therefore the primary method for the assessment of the degree of internal carotid artery stenosis. It is important that the examination is performed at an insonation angle of 60° as measurements at higher angles result in an exponential increase in measurement error. Also, an extended version of standards involving measurements performed behind stenosis, which are used in some clinics, is included in the paper. The paper further presents guidelines for the description of the identified pathologies, which ensure that the findings prove unambiguous for clinicians, especially when stenosis eligible for surgical intervention is detected. Morphological measurement of stenosis (according to NASCET criteria) is only of supplementary character (confirming morphological grounds for the increase of velocity) and has no critical importance, especially due to high measurement divergence. Description of atherosclerotic plaques, especially hypoechoic ones, which are considered potentially unstable and may lead to a raised risk of stroke, is a very important element of examination. Ultrasonography is a primary imaging technique in patients with suspected thyroid disease. It allows to assess the location, size and echostructures of the thyroid gland as well as detect focal lesions, along with indication of their size, echogenicity, echostructure and vascularity. Based on these features, ultrasound examination allows to predict abnormal focal lesions for biopsy and monitor the biopsy needle track. This paper presents the standards of thyroid ultrasound examination regarding ultrasound apparatus technical requirements, scanning techniques, readings, measurements, and the description of the examination. It discusses the ultrasound features of increased malignancy risk in focal lesions (nodules) found in the thyroid gland. It presents indications for fine needle aspiration biopsy of the thyroid gland for the visibility of single nodules (focal lesions) and numerous lesions as well as discusses contraindications for thyroid biopsy. It describes the biopsy technique, possible complications and rules for post-biopsy monitoring of benign lesions. The paper is an update of the Standards of the Polish Ultrasound Society issued in 2011. It has been prepared on the basis of current literature, taking into account the information contained in the following publications: Thyroid ultrasound examination and Recommendations of the Polish Ultrasound Society for the performance of the FNAB of the thyroid.Badanie ultrasonograficzne jest podstawowym badaniem obrazowym u osób z podejrzeniem chorób tarczycy. Pozwala na ocenę położenia, wielkości oraz echostruktury gruczołu, w tym wykrywanie zmian ogniskowych, wraz z określeniem ich rozmiaru, echogeniczności, echostruktury i unaczynienia. Na podstawie tych cech badanie ultrasonograficzne umożliwia wytypowanie nieprawidłowej zmiany ogniskowej do biopsji oraz monitorowanie toru igły biopsyjnej. W publikacji przedstawiono standardy badania ultrasonograficznego tarczycy dotyczące wymogów technicznych aparatów ultrasonograficznych, techniki wykonywania badania, wskazań i pomiarów oraz opisu badania. Omówiono cechy ultrasonograficzne zwiększonego ryzyka złośliwości zmian ogniskowych (guzków) w tarczycy. Przedstawiono wskazania do biopsji aspiracyjnej cienkoigłowej tego gruczołu w przypadku uwidocznienia pojedynczego guzka (zmiany ogniskowej) oraz zmian mnogich, a także przeciwwskazania do biopsji tarczycy. Diabetes is a metabolic disease characterized by elevated blood glucose level due to impaired insulin secretion and activity. Chronic hyperglycemia leads to functional disorders of numerous organs and to their damage. Vascular lesions belong to the most common late complications of diabetes. Microangiopathic lesions can be found in the eyeball, kidneys and nervous system. Macroangiopathy is associated with coronary and peripheral vessels. Diabetic retinopathy is the most common microangiopathic complication characterized by closure of slight retinal blood vessels and their permeability. Despite intensive research, the pathomechanism that leads to the development and progression of diabetic retinopathy is not fully understood. The examinations used in assessing diabetic retinopathy usually involve imaging of the vessels in the eyeball and the retina. Therefore, the examinations include: fluorescein angiography, optical coherence tomography of the retina, B-mode ultrasound imaging, perimetry and digital retinal photography. There are many papers that discuss the correlations between retrobulbar circulation alterations and progression of diabetic retinopathy based on Doppler sonography. Color Doppler imaging is a non-invasive method enabling measurements of blood flow velocities in small vessels of the eyeball. The most frequently assessed vessels include: the ophthalmic artery, which is the first branch of the internal carotid artery, as well as the central retinal vein and artery, and the posterior ciliary arteries. The analysis of hemodynamic alterations in the retrobulbar vessels may deliver important information concerning circulation in diabetes and help to answer the question whether there is a relation between the progression of diabetic retinopathy and the changes observed in blood flow in the vessels of the eyeball. This paper presents the overview of literature regarding studies on blood flow in the vessels of the eyeball in patients with diabetic retinopathy.

Central venous access consists in inserting a vascular catheter to the vena cava and placing its tip in the vicinity of the opening to the right atrium. In the patients of the Clinic of Pediatrics, Hematology and Oncology at the Academic Clinical Centre of the Medical University in Gdańsk, such implantation procedures are conducted 40–50 times in a year using Broviac/Hickman catheters that are placed in the subclavian vein. In the Ultrasound and Biopsy Laboratory at the clinic mentioned above, approximately 200–250 examinations have been conducted since 2005 to assess the central venous access. Implantation of a catheter considerably increases the comfort of patients who require a long-term venous access. Nevertheless, it is an invasive procedure, burdened with a risk of numerous, early and late complications. The late complications are associated with implanted catheters and include catheter-related thrombosis. The aim of this paper was to present three patients of the Clinic of Pediatrics, Hematology and Oncology at the Academic Clinical Centre of the Medical University in Gdańsk, in whom thrombotic complications occurred as a result of long-term central venous catheters. The paper also discusses the possibilities of using sonography in the assessment of such complications. In the presented patients, it was possible to determine the size and localization of a thrombus which enabled effective treatment in two cases. The pathomechanism of catheter-related thrombosis was explained and the risk factors of such complications were discussed. The attention was paid to the necessity of conducting ultrasound examinations in pediatric patients with inserted catheters as soon as the first symptoms of thrombosis appear. Based on own observations and despite the lack of validation of ultrasound imaging in the assessment of central catheters, we believe that this method is highly promising and can be recommended for the assessment of thrombotic complications in pediatric patients with central venous catheters.

Totally anomalous pulmonary venous drainage was for the first time described on the basis of a post-mortem examination in 1798(1). In the 21st century such pathologies are diagnosed in a living patient. The echocardiographic detection and diagnosis of totally anomalous pulmonary venous drainage (TAPVD) is a real challenge for every cardiologist. This defect is encountered rarely and it is difficult to be precisely evaluated in young patients. The diagnosis usually requires the application of additional imaging techniques such as 3D CT. With great interest I have read the precise description, actually an echocardiographer's “instruction,” on how to move the ultrasound beam step by step as well as how to interpret the registered 2D or color Doppler images in order to trace the supracardiac type of anomalous pulmonary venous drainage(2). Such a clear and precise description is a genuine pearl in our literature. Particular attention should be drawn to additional (apart from routine) projections and application sites of the ultrasound transducer, i.e. parasternal and high parasternal ones. They are valuable since they extend our sonographic “window” and the classical echocardiographic examinations in parasternal projection belong to the worst tolerated ones by young patients examined both in an outpatient clinic and in the ICU. I wish to congratulate the authors on their beautiful photographic documentation. Seizing the opportunity to share my own experience as an academic teacher with the Authors of the commented paper and the Readers, I wish to remind that the discussed defect appears in the embryonic period and thus, it is present during ultrasound examinations both in 13th–14th and 18th–20th weeks of gestation as well as later. This pathology may be diagnosed in the fetal cardiac center(3–5). So as to transfer this information to the recipients in the clearest way possible (and both medical workers such as obstetricians, neonatologists or pediatric cardiologists as well as parents should be familiar with it), in the center in Łodź, we try to attach a graphic presentation of the defect to each examination result. Anomalous pulmonary venous drainage may occur as the supracardiac type, as discussed in the commented article, but there are also intracardiac and infracardiac types. Each of them may be identified in fetuses. In 2004–2013 in the Polish National Registry of Fetal Cardiac Pathology (www.orpkp.pl), we have registered 19 such cases out of 5,009 fetuses with cardiac defects – they constituted 0.3%. Children with such anomalies are usually referred for a scheduled cardiac surgery. However, in the case of stenosis of the pulmonary veins, the patient's condition may worsen rapidly which might result in their death before relevant treatment is implemented. Pulmonary vein stenosis in a fetus has so far been a lethal cardiac defect(4, 6). Nevertheless, a good prenatal diagnosis and efficient transfer of the neonate to the cardiac catheterisation laboratory might in the future create a possibility to treat the narrowing by means of a stent and to perform further surgical corrections. The second interesting paper(5) is, in my opinion, a real rarity. No similar prenatal diagnoses are mentioned in the Polish National Registry of Fetal Cardiac Pathology and in the MEDLINE database, only several such cases may be found(7–9). It is worth reminding that sling may be present in fetuses with asymmetrically developed lungs. To sum up, the Authors of the commented papers have served a real spiritual feast for the Readers of “Journal of Ultrasonography” who deal with cardiology and echocardiography.

Bacterial meningitis is a major diagnostic and therapeutic problem among children and neonates, with severe, rapidly progressing course and potentially life-threatening complications. Early antibacterial treatment is essential for the patient’s favorable prognosis. Cerebral imaging plays an important role in the diagnostic process alongside physical examination and laboratory tests. Magnetic resonance imaging is the gold standard for diagnosing bacterial meningitis. Because of limited availability of magnetic resonance imaging, cranial ultrasound is the first imaging procedure to be performed (if the anterior fontanelle is preserved providing an adequate acoustic window). The safety and reliability of ultrasound examination, possibility to perform the examination at bedside without the need for sedation make cranial ultrasound a useful tool both for preliminary diagnostic investigation and for the monitoring of both treatment and long-term complications. Sonographic findings in patients with bacterial meningitis and possible complications are diverse. Changes can be seen on the surface of the brain, in the extra-axial space, in the ventricular system and in brain tissue. In some cases they can also be visible in the lumbosacral segment of the spinal cord. This paper presents ultrasound characteristics of lesions associated with bacterial meningitis in neonates and infants, based on the authors’ own material and data from the available literature.

In this paper, the authors attempt to concisely present the anatomical and pathophysiological bases as well as the principles for echocardiographic evaluation of mechanical aspects of cardiac function based on speckle tracking method. This technique uses a phenomenon involving the formation of characteristic image units, referred to as speckles or acoustic markers, which are stable during cardiac cycle, on a two-dimensional echocardiographic picture. Changes in the position of these speckles throughout the cardiac cycle, which are monitored and analyzed semi-automatically by a computer system, reflect deformation of both, cardiac ventricle as a whole as well as its individual anatomical segments. The values of strain and the strain rate, as well as the range and velocity of the movement of these markers, which are in close relationship with multiple hemodynamic parameters, can be visualized as various types of charts – linear, two- and three-dimensional – as well as numerical values, enabling deeper insight into the mechanical and hemodynamic aspects of cardiac function in health and disease. The use of information obtained based on speckle tracking echocardiography allows to understand previously unclear mechanisms of physiological and pathophysiological processes. The first part of the study discusses the formation of a two-dimensional ultrasound image and the speckles, as well as the technical aspects of tracking their movement. The second part presents in more detail the methodology of speckle-tracking echocardiography, the characteristic abnormalities of cardiac mechanics presenting in different clinical entities, and the limitations related to given clinical and technical issues.W niniejszej pracy autorzy podejmują próbę zwięzłego przedstawienia anatomicznych i patofizjologicznych podstaw oraz zasad echokardiograficznej oceny mechanicznych aspektów pracy serca w oparciu o metodę śledzenia markerów akustycznych. AimThe purpose of the study was to improve the ultrasonographic assessment of the anterior cruciate ligament by an inclusion of a dynamic element. The proposed functional modification aims to restore normal posterior cruciate ligament tension, which is associated with a visible change in the ligament shape. This method reduces the risk of an error resulting from subjectively assessing the shape of the posterior cruciate ligament. It should be also emphasized that the method combined with other ultrasound anterior cruciate ligament assessment techniques helps increase diagnostic accuracy.MethodsUltrasonography is used as an adjunctive technique in the diagnosis of anterior cruciate ligament injury. The paper presents a sonographic technique for the assessment of suspected anterior cruciate ligament insufficiency supplemented by the use of a dynamic examination. This technique can be recommended as an additional procedure in routine ultrasound diagnostics of anterior cruciate ligament injuries.ResultsSupplementing routine ultrasonography with the dynamic assessment of posterior cruciate ligament shape changes in patients with suspected anterior cruciate ligament injury reduces the risk of subjective errors and increases diagnostic accuracy. This is important especially in cases of minor anterior knee instability and bilateral anterior knee instability.ConclusionsAn assessment of changes in posterior cruciate ligament using a dynamic ultrasound examination effectively complements routine sonographic diagnostic techniques for anterior cruciate ligament insufficiency.Cel pracyCelem pracy jest ocena możliwości poprawienia ultrasonograficznej oceny niewydolności więzadła krzyżowego przedniego poprzez wprowadzenie elementu badania dynamicznego. The paper is an update of 2011 Standards for Ultrasound Assessment of Salivary Glands, which were developed by the Polish Ultrasound Society. We have described current ultrasound technical requirements, assessment and measurement techniques as well as guidelines for ultrasound description. We have also discussed an ultrasound image of normal salivary glands as well as the most important pathologies, such as inflammation, sialosis, collagenosis, injuries and proliferative processes, with particular emphasis on lesions indicating high risk of malignancy. In acute bacterial inflammation, the salivary glands appear as hypoechoic, enlarged or normal-sized, with increased parenchymal flow. The echogenicity is significantly increased in viral infections. Degenerative lesions may be seen in chronic inflammations. Hyperechoic deposits with acoustic shadowing can be visualized in lithiasis. Parenchymal fibrosis is a dominant feature of sialosis. Sjögren syndrome produces different pictures of salivary gland parenchymal lesions at different stages of the disease. Pleomorphic adenomas are usually hypoechoic, well-defined and polycyclic in most cases. Warthin tumor usually presents as a hypoechoic, oval-shaped lesion with anechoic cystic spaces. Malignancies are characterized by blurred outlines, irregular shape, usually heterogeneous echogenicity and pathological neovascularization. The accompanying metastatic lesions are another indicator of malignancy, however, final diagnosis should be based on biopsy findings.W publikacji przedstawiono aktualizację standardów Polskiego Towarzystwa Ultrasonograficznego dotyczących badania ultrasonograficznego gruczołów ślinowych, wydanych w 2011 roku. Opisano w niej obecne wymogi techniczne dotyczące aparatów ultrasonograficznych, technikę przeprowadzania badania i pomiarów oraz zasady wykonywania opisu badania. Omówiono zarówno prawidłowy obraz ultrasonograficzny ślinianek, jak i najważniejsze patologie w obrębie tych gruczołów, takie jak: stany zapalne, sialozy, kolagenozy, urazy oraz procesy rozrostowe, ze szczególnym podkreśleniem cech zmian o wysokim ryzyku złośliwości. W przypadku ostrych, bakteryjnych zapaleń ślinianki są hipoechogeniczne, powiększone bądź normalnego rozmiaru, ze zwiększonym przepływem miąższowym. W zapaleniach wirusowych echogeniczność jest wyraźnie podwyższona. W zapaleniach przewlekłych widoczne są zmiany degeneracyjne. W kamicy uwidocznić można hiperechogeniczne złogi z towarzyszącym cieniem akustycznym. W sialozach dominują cechy zwłóknienia miąższu. Zespół Sjögrena daje różne obrazy zmian w miąższu ślinianek na różnych etapach choroby. Gruczolaki wielopostaciowe to zwykle zmiany hipoechogeniczne, dobrze odgraniczone i najczęściej policykliczne. Guz Warthina jest zazwyczaj zmianą hipoechogeniczną, owalną, z torbielowatymi przestrzeniami bezechowymi. Zmiany złośliwe cechują się zatartymi granicami, nieregularnym kształtem, zwykle niejednorodną echogenicznością i patologicznym unaczynieniem. Towarzyszące zmianie przerzutowe węzły chłonne są dodatkowym aspektem świadczącym o złośliwym jej charakterze, jednak ostateczne rozpoznanie można postawić na podstawie wyniku biopsji.

This paper presents the case of a very rare capillary hemangioma of the testis in a 23-year-old patient. Physical examination revealed a tumor located in the upper pole of the left testis, which was suspected of being malignant due to its significantly increased density and irregular contours. Blood levels of the following tumor markers were determined: alpha-fetoprotein, human chorionic gonadotropin and lactate dehydrogenase. No abnormalities were found in any of these tests. A gray-scale ultrasound scan of the scrotum revealed a lesion located in the upper pole of the left testis, 24 mm in diameter with slightly decreased echogenicity and irregular contours, which suggested infiltration of the tunica albuginea of the testis. Color and power Doppler scans demonstrated a dense network of blood vessels and increased blood flow in the lesion described. In addition, 3D ultrasound scan images were obtained, which allowed for a thorough determination of the topography of the lesion. No abnormalities were found in the patient's medical history, physical examination or ultrasound scan of the right testis. Taking into account the suspected malignancy with signs of infiltration of the tunica albuginea of the upper pole of the testis a decision was made to remove the left testis together with the spermatic cord using the inguinal approach. A histopathological examination of the whole specimen revealed a multifocal capillary hemangioma of the testis, signs of testicular fibrosis and significant atrophy of the spermatogenic epithelium of the seminiferous tubules. Immunohistochemistry: CD31 (+), CD34 (+), FVIII (−), vimentin (+), CK MNF116 (−), mesothelial cells (−), calretinin (−), MIB-1 = 8.4%. The tumor described is one of the few benign lesions originating from the tissues of the testis for which partial resection of the organ may be considered. Intraoperative histopathological examination and resection of the lesion with the preservation of a healthy tissue margin may be considered in patients with a single testis in whom the tumor is found to have a very rich vascular network and the levels of tumor markers are normal.W niniejszej pracy przedstawiono przypadek bardzo rzadko występującego naczyniaka krwionośnego włośniczkowego jądra u 23-letniego pacjenta. Guz zlokalizowany w badaniu fizykalnym w górnym biegunie jądra lewego ze względu na znacznie wzmożoną spoistość i nieregularne obrysy budził podejrzenie nowotworu złośliwego. Oznaczono stężenia markerów nowotworowych we krwi: alfa-fetoproteiny, gonadotropiny kosmówkowej oraz dehydrogenazy mleczanowej, nie stwierdzając odchyleń od normy w żadnym z tych badań. W badaniu ultrasonograficznym moszny wykonanym w skali szarości w górnym biegunie jądra lewego zaobserwowano ognisko o średnicy 24 mm o nieznacznie obniżonej echogeniczności i nieregularnych obrysach, sugerujących naciekanie błony białawej jądra. W opcji dopplera kolorowego i dopplera mocy stwierdzono gęstą sieć naczyń krwionośnych oraz wzmożone przepływy krwi w opisywanej zmianie. Dokonano również akwizycji obrazów w ultrasonograficznym badaniu trójwymiarowym, które pozwoliło na poznanie dokładnej topografii ogniska. W badaniu podmiotowym, przedmiotowym oraz ultrasonograficznej ocenie jądra prawego nie stwierdzono nieprawidłowości. Biorąc pod uwagę podejrzenie nowotworu złośliwego, o cechach naciekania błony białawej górnego bieguna jądra, pacjenta zakwalifikowano do usunięcia jądra lewego wraz z powrózkiem nasiennym z dostępu przez kanał pachwinowy. W badaniu histopatologicznym całego preparatu stwierdzono wieloogniskowego naczyniaka krwionośnego włośniczkowego jądra, cechy włóknienia jądra oraz znacznego stopnia atrofię nabłonka plemnikotwórczego kanalików nasiennych. Immunohistochemicznie: CD31 (+), CD34 (+), FVIII (−), wimentyna (+), CK MNF116 (−), komórki mezotelialne (−), kalretynina (−), MIB-1 = 8,4%. Opisywany nowotwór jest jedną z nielicznych niezłośliwych zmian wywodzących się z tkanek jądra, w przypadku których można rozważać resekcję częściową narządu. U pacjentów z jądrem jedynym, u których stwierdza się guz o bardzo bogatej sieci naczyń, przy prawidłowych stężeniach markerów nowotworowych można rozważać śródoperacyjne badanie histopatologiczne i resekcję zmiany z zachowaniem marginesu zdrowych tkanek.

Adrenal glands are paired endocrine glands located over the upper renal poles. Adrenal pathologies have various clinical presentations. They can coexist with the hyperfunction of individual cortical zones or the medulla, insufficiency of the adrenal cortex or retained normal hormonal function. The most common adrenal masses are tumors incidentally detected in imaging examinations (ultrasound, tomography, magnetic resonance imaging), referred to as incidentalomas. They include a range of histopathological entities but cortical adenomas without hormonal hyperfunction are the most common. Each abdominal ultrasound scan of a child or adult should include the assessment of the suprarenal areas. If a previously non-reported, incidental solid focal lesion exceeding 1 cm (incidentaloma) is detected in the suprarenal area, computed tomography or magnetic resonance imaging should be conducted to confirm its presence and for differentiation and the tumor functional status should be determined. Ultrasound imaging is also used to monitor adrenal incidentaloma that is not eligible for a surgery. The paper presents recommendations concerning the performance and assessment of ultrasound examinations of the adrenal glands and their pathological lesions. The article includes new ultrasound techniques, such as tissue harmonic imaging, spatial compound imaging, three-dimensional ultrasound, elastography, contrast-enhanced ultrasound and parametric imaging. Choroby nadnerczy prezentują różne obrazy kliniczne. Mogą przebiegać z nadczynnością w zakresie poszczególnych warstw kory nadnercza albo rdzenia, niedoczynnością kory nadnercza lub z zachowaną prawidłową czynnością hormonalną. Najczęściej spotykanymi zmianami nadnerczy są przypadkowo stwierdzane w badaniach obrazowych (ultrasonografia, tomografia, rezonans magnetyczny) guzy, określane terminem incydentaloma. Obejmują one szereg jednostek histopatologicznych, jednak najczęściej są to gruczolaki kory nadnerczy bez nadmiernej czynności hormonalnej. Każde badanie ultrasonograficzne jamy brzusznej dziecka albo osoby dorosłej powinno obejmować ocenę pól nadnerczowych. W przypadku uwidocznienia w polu nadnerczowym wcześniej nieopisywanej, przypadkowej litej zmiany ogniskowej o średnicy1 cm (incydentaloma) konieczne jest jej potwierdzenie i różnicowanie za pomocą badania tomografii komputerowej lub obrazowania rezonansu magnetycznego oraz określenie stanu czynnościowego guza. Badanie ultrasonograficzne wykorzystywane jest również do kontrolowania przypadków incydentaloma nadnerczy niezakwalifikowanych do leczenia operacyjnego. W pracy przedstawiono zalecenia dotyczące wykonywania i oceny badań ultrasonograficznych nadnerczy oraz ich zmian patologicznych. Opracowanie uwzględnia nowe techniki ultrasonograficzne, takie jak tkankowe obrazowanie harmoniczne, obrazowanie złożone przestrzennie, ultrasonografia trójwymiarowa, elastografia oraz obrazowanie z zastosowaniem ultrasonograficznych środków kontrastujących i obrazowanie parametryczne. Zawarte w publikacji wytyczne są przedstawione zgodnie z zaleceniami Polskiego Towarzystwa Ultrasonograficznego.

Pathological lesions within the scrotum are relatively rare in imaging except for ultrasonography. The diseases presented in the paper are usually found in men at the age of 15-45, i.e. men of reproductive age, and therefore they are worth attention. Scrotal ultrasound in infertile individuals should be conducted on a routine basis owing to the fact that pathological scrotal lesions are frequently detected in this population. Malignant testicular cancers are the most common neoplasms in men at the age of 20-40. Ultrasound imaging is the method of choice characterized by the sensitivity of nearly 100% in the differentiation between intratesticular and extratesticular lesions. In the case of doubtful lesions that are not classified for intra-operative verification, nuclear magnetic resonance is applied. Computed tomography, however, is performed to monitor the progression of a neoplastic disease, in pelvic trauma with scrotal injury as well as in rare cases of scrotal hernias involving the ureters or a fragment of the urinary bladder.Patologiczne zmiany w obrębie worka mosznowego są stosunkowo rzadko spotykane w badaniach obrazowych, z wyjątkiem ultrasonografii. Prezentowane choroby dotyczą najczęściej grupy mężczyzn w przedziale wiekowym 15–45 lat, czyli w wieku rozrodczym, dlatego zasługują na szczególną uwagę. U niepłodnych mężczyzn badanie ultrasonograficzne moszny powinno być wykonywane rutynowo, ponieważ w tej populacji częstość wykrywania zmian patologicznych w mosznie jest bardzo wysoka. Nowotwory złośliwe jąder należą do najczęstszych nowotworów występujących u mężczyzn w wieku 20–40 lat. Metodą diagnostyczną z wyboru jest badanie ultrasonograficzne, którego czułość w różnicowaniu zmian wewnątrzjądrowych i zewnątrzjądrowych wynosi prawie 100%. W diagnostyce niejednoznacznych zmian, niekwalifikujących się do weryfikacji operacyjnej, stosowany jest magnetyczny rezonans jądrowy. Tomografię komputerową natomiast wykorzystuje się w monitorowaniu zaawansowania choroby nowotworowej, w stanach pourazowych miednicy z udziałem urazu moszny oraz w rzadkich przypadkach przepuklin mosznowych zawierających moczowody lub fragment pęcherza moczowego.
