Being uncommon neoplasms of elusive origin, epithelioid hemangiomas are benign vascular tumors found in young adults. 1 These epithelioid hemangiomas are mostly found in the head and neck region, presenting as solitary or multiple lesions, and they vary between 2 and 3 cm in diameter, and morphologically manifest as intradermal papules or subcutaneous nodules. 2 More eloquently termed as angiolymphoid hyperplasia with eosinophilia on pathological basis, these tumors are amenable to surgical excision, though rare instances exist where they might regress on their own. 3 A 23-year-old male, who we report, had a bizarre unusual presentation with an extremely large mushroom shaped mass occluding the left external auditory canal. He had considerable facial disfigurement, pulsatile tinnitus and unilateral deafness. Even though it was clinically thought to be a malignant mass, cross sectional imaging gave clue to its highly vascular yet benign nature. Despite its large size, the tumor did not cause osseous destruction and had simply traveled into adjacent anatomical structures and overlying subcutaneous tissues and skin. The characteristic histologic picture was pathognomonic. This case report highlights the unusually large size and atypical clinical presentation of the tumor.

Cardiac epithelioid hemangioma is extremely rare. Currently, there are only a few described cases of intraluminal hemangiomas of the aorta and the aortic valve and no described cases with extraluminal epithelioid hemangiomas of the ascending aorta. - Abstract
Epithelioid hemangioma, otherwise known as angiolymphoid hyperplasia with eosinophilia, is a rare benign vasoproliferative disease with an unknown etiology. We report the case of a 42-year-old man with routine bloods test showing mildly elevated serum bilirubin level. CT scan revealed a lesion in the left liver lobe not typical of FNH. PET/CT scan and a dynamic liver MRI were consistent with a malignant mass. Surgical resection was performed. Histopathology of the 45 mm mass reported well-defined, slightly lobular proliferations of capillary-sized vessels around several central muscular vessels. Although the endothelial cells revealed a "hobnail" appearance, none of them showed pleomorphism or mitotic activity. Endothelial cells showed reactivity for the endothelial markers (CD34, CD31) and smooth muscle was detected in the blood vessel walls via immunohistochemistry. Despite its benign nature, epithelioid hemangioma of the liver can lead to a diagnostic and therapeutic dilemma due to the malignant looking features on imaging modalities., Competing Interests: None

Epithelioid hemangioma (EH) or angiolymphoid hyperplasia is an uncommon benign vasculoproliferative neoplasm. We are reporting a case of EH involving the gingiva in a 12-year-old male patient who reported to the outpatient Department of Oral Medicine and Radiology with the chief complaint of gingival growth in the lower mandibular gingiva involving both buccal and lingual gingiva and facial asymmetry due to enlarged left submandibular lymph nodes. Histopathological features diagnosed the case definitive of EH.

Penile epithelioid hemangioma (EH) is a rare vascular neoplasm with no definite etiology. Herein, we report a case of EH of the penis in a 64-year-old man presenting with painless, bleeding mass on the glans penis. The patient underwent local excision, and on histopathological examination, a diagnosis of EH was made. Immunohistochemistry revealed positivity for CD31, smooth muscle antigen, and negative expression of cytokeratin. The present case highlights the importance of histopathology in conjunction with immunohistochemistry to reach a definitive diagnosis of this rare benign entity and differentiating it from the close malignant mimics, thereby avoiding aggressive management of the patients.

Penile epithelioid hemangioma is a very rare benign vascular neoplasm of unclear etiology. To the best of our knowledge, only 28 cases in the literature have been reported to date. We report a case of epithelioid hemangioma of the penis in a 41-year-old man with a painful penile mass. The presented case exemplifies the importance of establishing a correct diagnosis through appropriate histologic analysis, as epithelioid hemangiomas are often misdiagnosed as two highly malignant tumors, epithelioid hemangioendothelioma and epithelioid angiosarcoma. Establishing benignancy is crucial in order to avoid inappropriate and overly aggressive treatment.

Testicular hemangioma is a very rare benign vascular neoplasm, there are only less than 30 cases of testicular hemangiomas reported in the literature. We report a case of epithelioid hemangioma of the testis in a 53-year-old man who presented with painless mass in the right testis. An orchiectomy was performed, The 1 cm tumor was composed of prominent proliferation of small, capillary-sized vessels lined by plump, epithelioid endothelial cells. Immunohistochemically, the epithelioid endothelial cells reacted with antibodies to factor VIII-related antigen, CD31, CD34 and Vimentin, but not CK, EMA, CEA, S-100 and CD68. Although it is a rare tumor, we should be aware of it and avoid confusion with other vascular neoplasm.

Epithelioid hemangioma (EH) is a benign vascular lesion composed primarily of epithelioid endothelial cells accompanied by a chronic inflammatory infiltrate. This lesion is also known as Histiocytoid hemangioma or Angiolymphoid hyperplasia with eosinophilia (ALHE). This rare tumour arising from vascular structures has uncertain etiology. It often occurs in the skin and subcutaneous fat of the head and neck region, but may also arise in the deep tissues such as muscle, bone and lymph node. The intraoral lesions of EH are uncommon and local recurrence has been observed only in a few cases. We report a case of epithelioid hemangioma involving the alveolar mucosa with local recurrence.
