INTRODUCTION Goiter reoccurs in a substantial number of patients after thyroid resection. OBJECTIVES We aimed to investigate the prevalence and risk factors of recurrent goiters, influence of goiter recurrence on patient quality of life, and the efficacy of therapy with radioiodine (RAI). PATIENTS AND METHODS This was a case‑control study. A total of 481 thyroidectomized patients admitted to the outpatient department within the past year were included in the study and their medical records were analyzed. Also, 30 healthy controls were recruited for comparison. Recurrence was defined as nodular lesions present within the remnant tissue or enlargement of the remaining thyroid tissue that required therapy (reoperation or RAI therapy). Clinical and biochemical data were collected. Randomly selected patients were asked to answer the Polish version of Thyroid‑Related Quality‑of‑Life Patient‑Reported Outcome measure (ThyPROpl). RESULTS A total of 68 patients had recurrent goiter and in 413 patients the recurrence did not occur. Higher thyroid‑stimulating hormone at follow‑up and lobectomy were the strongest risk factors for goiter recurrence, followed by a longer follow-up. Postoperative levothyroxine therapy was associated with a lower risk of recurrence. Efficacy of RAI was similar to secondary thyroidectomy. Scores in all comparable scales for patients with recurrent goiters were significantly worse than those in the general population sample. CONCLUSIONS Lobectomies should be avoided as a primary surgical treatment for patients with benign thyroid diseases, and levothyroxine therapy should be considered individually in each patient. RAI therapy seems to be a safe and effective treatment option for patients with recurrent goiters. Recurrent goiters, even if successfully treated, have a negative impact on the quality of life.

INTRODUCTION    There is growing evidence that obstructive sleep apnea (OSA) influences both bone metabolism and structure. Chitinase‑3‑like protein 1 (YKL‑40) is a novel inflammatory and remodeling marker, the levels of which were shown to increase in OSA. YKL‑40 can probably alter the bone turnover. OBJECTIVES    The aim of the study was to assess a possible interplay between YKL‑40 and bone turnover markers in patients with different stages of OSA, and to evaluate the relation between bone mass, severity of OSA, and YKL‑40 levels. PATIENTS AND METHODS    The study involved 72 male patients with OSA. They were divided into 3 groups according to disease severity, using the apnea-hypopnea index (AHI): group 1 (n = 18; 5≤ AHI <15), group 2 (n = 25; 15≤ AHI <30), and group 3 (n = 29; AHI ≥30). All patients underwent polysomnography and densitometry. Fasting blood samples were collected for YKL‑40, C‑terminal telopeptide of typeI collagen (CTX), procollagen type 1 N‑terminal propeptide (P1NP), and other markers. RESULTS    P1NP differed between groups 1 and 2, as well as groups 1 and 3 (P = 0.02). Group 2 had higher CTX levels than group 1 (borderline significance, P = 0.05). A simple linear regression analysis showed that serum YKL‑40 levels were associated with the levels of CTX (P <0.0001, β = 0.9871) and P1NP (P <0.0001, β = 0.9780). CONCLUSIONS    Our study might suggest that YKL‑40 is associated with bone turnover in OSA. We may assume that this marker influences both bone formation and destruction; thus, OSA could be characterized by preserved bone mineral density.

INTRODUCTION    Graves ophthalmopathy (GO) is an autoimmune disease associated with Graves disease. Its treatment is largely dependent on the severity and activity of ocular lesions. Particular attention should be given to radioiodine (RAI) therapy. Although its use is a valuable therapeutic option for hyperthyroidism, it may be followed by worsening of GO.  OBJECTIVES    The aim of the present study was to analyze how the severity of nicotine addiction affects the response to RAI treatment in patients with GO. PATIENTS AND METHODS    A total of 106 patients (58 smokers and 48 nonsmokers) with mild GO treated with 800 MBq of RAI were included to the study. We assessed the serum levels of thyroid‑stimulating hormone (TSH), thyroid hormones, autoantibodies against thyroperoxidase, thyroglobulin, and TSH receptor (TSHR‑Abs), as well as urinary cotinine levels and severity of ophthalmopathy. Analyses were conducted at baseline (before RAI treatment) and 2 and 6 months after the therapy. RESULTS    Significant differences in serum levels of TSHR‑Abs were found between nonsmokers and smokers at 2 and 6 months after RAI therapy, whereas there were no differences at baseline. In smokers, there were significant differences in the severity of ophthalmopathy and the concentration of serum TSHR‑Abs assessed at baseline and at 6 months of follow‑up. Six months after RAI therapy, 46.2% of smokers and 4.3% of nonsmokers (P <0.001) progressed from mild to moderate GO.  CONCLUSIONS    High urinary cotinine levels in smokers were associated with the deterioration of ocular lesions after RAI treatment. A high dose of RAI did not induce an exacerbation of GO in nonsmokers who were administered oral steroid prophylaxis.
