Abstract

We demonstrate a noninvasive system for real-time monitoring of tumor blood flow and automatic adjustment of treatment light fluence-rate to conserve tumor perfusion during PDT. Improved efficacy was observed using blood-flow informed light delivery.

We used Diffuse Optical Spectroscopy to measure the effect of PDT on hemoglobin concentration changes during intra-operative PDT for mesothelioma. Those with less PDT-created hypoxia had better prognosis than those with severe hypoxia.

Blood flow in mice tumor models was measured with Diffuse Correlation Spectroscopy and compared to concurrent physiology monitoring. Positive correlations were (not) found between flow and heart (breath) rate during anesthesia periods.

Diffuse correlation spectroscopy has been applied for real-time monitoring of tumor blood flow responses to photodynamic therapy using two photosensitizers: Photofrin and Motexafin lutetium (MLu). The different photosensitizers exhibited different blood flow dynamics during PDT.

Photodynamic therapy (PDT) requires oxygen to damage tumor. We demonstrated that the fractional change in tumor oxygenation, measrued by diffuse reflectance spectroscopy, shortly after versus before PDT significantly correlates with time-to-regrowth of the same tumor.

By extending dynamic light scattering from single scattering limit to high-order multiple scattering, we measure the dynamics of biological tissues averaged over some deep scattering volume. Clinical applications including tumor blood flow assessment are presented.
