Abstract

Correlation of virulence factors of Escherichia coli with renal inflammation documented by 99mTc-dimercaptosuccinic acid renal scan was undertaken in 59 children with febrile urinary tract infections to identify more accurately the role of bacterial virulence factors in the development of pyelonephritis. P fimbriae were present in 63% of isolates from the positive scan group and 83% of those from the negative scan group (P = 0.126). Multivariate regression analysis showed no significant role for established E. coli virulence factors in the development of pyelonephritis. The pap genome was independently associated with negative scan (P less than 0.007) and with the absence of reflux (P = 0.031). E. coli pyelonephritogenic clone O16:K1:H6 was isolated from negative scan patients and did not produce hemolysin. We conclude that P fimbriae are important in the development of febrile urinary tract infection regardless of the level of infection. Virulent E. coli clones described in prior Scandinavian urinary tract infection studies were not common causes of pyelonephritis in our patient population.

Abstract

Background. Speckle tracking echocardiography (STE) is a new method that allows to quantitalively measure myocardial deformation in an angle-independent manner. The aim of this study was to evaluate the accuracy of STE-derived estimates of circumferential and longitudinal strains, in patients with variable degrees of LV dysfunction, using tagged cardiac magnetic resonance (cMR) as the reference standard. Methods. A total of 80 patients (66 men, mean age 54 ± 16 years) with a variety of cardiac diseases underwent STE and cMR on the same day. For circumferential strain, data were analyzed on matched short-axis cross-sections (at basal and apical levels) using either the QLab software (for STE) or HARP (for tagged cMR). For longitudinal strain, data were analyzed on similar 4-, 3- and 2-chamber views using either QLab (for STE) or a custom-made program that allows for tracking of cMR tags in long-axis projections (for tagged cMR). Results. 87% of myocardial segments could be analyzed. Correlation and limits of agreement between STE and cMR were good for both global (r = 0.95, p ). Correlation and limits of agreement between STE and cMR were similarly good for global (r =0.96, p ). Conclusions. STE provides accurate measurements of segmental circumferential and longitudinal strains and thus offers the opportunity to routinely assess these parameters in daily clinical practice.

9. Abstract 3179: Intraoperative Transoesophageal Echocardiographic Predictors of Recurrent Aortic Regurgitation after Aortic Valve Repair

Abstract

Background. Surgical valve repair for aortic regurgitation (AR) has significant advantages over valve replacement, but little is known about the mechanisms of its failure. This echocardiographic study examines the intraoperative features associated with “late failure” of valve repair, eventually necessitating reoperation. Method. Intraoperative transesophageal echocardiography (TEE) after cardiopulmonary bypass was performed in 168 consecutive patients (pts) undergoing valve repair for AR over a 10-year period. From this cohort, we carefully reviewed all clinical, pre-operative, intraoperative and follow-up TEE data of 62 pts (group A, 53 ± 13 years) with no or trivial AR at follow-up and 31 pts (group B, 46 ± 15 years) with recurrent severe AR at follow-up of whom 17 needed a re-operation. In these pts, echocardiography identified the cause of repair failure as rupture of a pericardial patch in 2 pts, residual cusp prolapse in 18 pts, restrictive cusp motion in 8 pts, aortic dissection in 2 pts and endocarditis in the remaining pt. Results. Pre-operatively, both groups were similar for aortic root dimensions and incidence of bicuspid valve (39 ± 4 % vs 35 ± 5 %, χ 2 = 0.4). Marfan disease and restrictive cusp motion or endocarditis were more frequent in group B than in group A (16 ± 4% vs 0% and 48 ± 5% vs 14 ± 3%, χ 2 = 0.001). After cardiopulmonary bypass, the coaptation length was shorter (2.2 ± 1.5 vs. 6.8 ± 2.8 mm, p Conclusion. Our results demonstrate that intraoperative TEE can be used to identify pts undergoing AR repair who are at increased risk for late repair failure.

10. Abstract 1769: Multidetector Computed Tomography allows accurate non-invasive Diagnosis of Etiology of Heart failure as compared with Coronary Angiography and contrast-enhanced MRI

Abstract

Background: The distinction of ischemic vs. non-ischemic origin is essential for prognosis and treatment of patients (pts) with congestive heart failure (CHF). To determine etiology, pts typically undergo invasive coronary angiography (CA). Additional information can be obtained by detecting necrosis or fibrosis on delayed enhanced (DE) magnetic resonance (cMR). Recently, we and others have demonstrated that detection of myocardial necrosis is also feasible by multidetector CT (MDCT) at the time of non-invasive coronary imaging. We therefore evaluated whether such a combined and comprehensive evaluation of coronary anatomy and infarct characterization by MDCT might be useful for determining the etiology of CHF. Methods: Sixty-one consecutive pts (44 males, 60 ± 16 years) with CHF (mean ejection fraction: 26 ± 11%) of undetermined etiology underwent MDCT, DE-MR and CA. Presence of coronary artery disease (CAD) and patterns of DE on MDCT were compared to CA and DE-MR. Results: According to CA, 27 pts (44%) had significant CAD (>50% stenosis in at least one major coronary artery). DE-cMR demonstrated transmural or subendocardial DE in 21 of these 27 pts. These 21 pts were considered to have definite ischemic CHF (group 1). Among the 34 pts without CAD on CA, 23 had no DE on cMR, while 7 (11%) presented midventricular or sub-epicardial DE. These 30 pts were considered to have definite non-ischemic CHF (group 2). Four pts had transmural DE but no CAD and were considered to have probable ischemic CHF (group 3). Finally, 6 pts with CAD on CA, but without DE on cMR were considered to have probable non-ischemic CHF (group 4). Pts were also classified into the same 4 groups using coronary and DE-MDCT. On a per patient basis, agreement between coronary and DE-MDCT and CA/DE-cMR for the diagnosis of patients was excellent (κ=0.87; p Conclusion: Combined coronary and DE-MDCT allows for the accurate differentiation of ischemic vs non ischemic etiology of CHF as compared to CA and DE-cMR. Using this single test is however cheaper and faster than combined use of DE-cMR and CA for establishing etiology of CHF.

Abstract

Purpose . Whole-heart magnetic resonance coronary angiography (WH-MRCA) has been recently proposed for non-invasive coronary imaging. Early studies have suggested that WH-MRCA might have similarly high diagnostic accuracy for detection of coronary disease as multidetector CT (MDCT). Yet, no direct comparison between both techniques has been performed. The aim of the present study was therefore to perform a head-to-head comparison of both techniques for detection of significant coronary stenoses using invasive cardiac catheterization as reference standard. Methods. Seventy-seven consecutive patients (56 M, 61±14 years) prospectively underwent free-breathing 3-dimensional WH-MRCA and 40/64-slice MDCT before cardiac catheterization. WH-MRCA and MDCT images were visually graded by 2 blinded observers and the diagnostic accuracy of both methods for detecting >50% luminal diameter stenoses (DS) in segments and vessels >1.5 mm size was compared using quantitative angiography (QCA) as reference method. Results. MDCT was successfully completed in all 77 patients in < 5 minutes. By contrast, WH-MRCA failed in 9 patients (12%) because of poor navigator performance and lasted 20±4 minutes (p 1.5 mm diameter, 49 presented >50% DS. If all segments including non interpretable segments were considered, WH-MRCA had lower sensibility (35/49 or 71% vs. 45/49 or 92%, p50% DS was similar. This was also the case on per-vessel basis. Conclusion. In the present study, MDCT had higher success rate than WH-MRCA. Therefore on an intention to diagnose basis, MDCT was superior to WH-MRCA. However, the diagnostic accuracy of WH-MRCA on per-segment and per-vessel basis was not statistically different from MDCT if only interpretable segments were considered.
