Objectives: Studies on the relationship between serum sclerostin, a Wnt/β-catenin pathway inhibitor, and atherosclerosis have yielded inconsistent results. We aim to longitudinally investigate the relationship between serum sclerostin levels and the risk of increased arterial stiffness in Japanese community-dwelling women., Methods: Of 1044 women aged ≥50 years whose brachial-ankle pulse wave velocity (baPWV) value was available in a baseline survey in 2011-2012, we excluded 374 whose baPWVs were ≥1800 cm/s, set as the cutoff for increased arterial stiffness, and eight with missing data. Of the remaining 662 women, we included in the analysis 556 followed in the 4- to 5-year follow-up study. The coefficient of variation of the sclerostin measurement was 3.45%. We obtained odds ratios (ORs) for sclerostin at baseline categorized by tertiles, with the high tertile as reference for increased arterial stiffness., Results: Ninety-four women showed increased arterial stiffness during a mean follow-up of 4.0 years. The increased arterial stiffness rates in the low, medium, and high tertiles were 22.2%, 16.1%, and 12.4%, respectively (trend test p = 0.013). The ORs for the medium and low tertiles for increased arterial stiffness were 1.58 (p = 0.205) and 2.16 (p = 0.027), respectively, after adjusting for age and baseline baPWV. After further adjustment for baseline BMI, hypertension, hyperlipidemia, diabetes mellitus, eGFR, and BMC at whole body, the ORs for the medium and low tertiles were 1.65 (p = 0.181) and 2.50 (p = 0.014), respectively., Conclusion: Lower serum sclerostin levels were associated with elevated risks for increased arterial stiffness in Japanese community-dwelling women., (© The Author(s) 2024. The Japanese Population-based Osteoporosis (JPOS) Cohort Study was launched in 1996 to produce a reference database of areal bone mineral density (aBMD) by dual energy X-ray absorptiometry (DXA) and bone turnover markers in the Japanese female population and to determine risk factors for osteoporotic fractures. At baseline, 3984 women aged 15 to 79 years were randomly selected to provide representative bone status data and aBMD values for the diagnosis of osteoporosis. Follow-up surveys were conducted in 1999, 2002, 2006 and 2011/12 to determine changes in aBMD and identify incident morphometry-confirmed vertebral fractures and clinical fractures. These outcomes were obtained from 2174 women who participated in at least one follow-up survey. JPOS is a unique resource of individual-level bone health information with radiological and biological archives that include DXA images, and serum, plasma and DNA for future analyses with emerging radiological and biological techniques. The JPOS dataset is not freely available, but new collaborations are encouraged. Background: Individuals of low socioeconomic status (SES) are likely to have poor sleep and poor health. This study aims to evaluate whether and how much of the socioeconomic differences in health are explained by sleep., Methods: The subjects were 3684 (2471 males and 1213 females) employees aged 20-65 working in local government in Japan. A questionnaire survey was conducted in January 2003. Analysis of covariance (ANCOVA) was performed to examine the association of employment-grade with sleep, measured by the Pittsburgh Sleep Quality Index (PSQI), and with health, measured by the Physical and Mental Component Summary Scales (PCS and MCS) of the Short Form-36 (SF-36)., Results: In men, higher grade employees had better sleep and better health. The age-adjusted difference between the highest and the lowest grade employees was 1.9 points (95% confidence interval = 1.0-3.0) in the PCS and 3.4 points (1.8-4.9) in the MCS. The grade difference in health reduced to 1.5 points (0.5-2.5) in the PCS (21.1% reduction) and 2.0 points (0.6-3.4) in the MCS (41.2% reduction), when the PSQI global score was adjusted for. The grade differences in sleep quality contributed more to the health inequalities than sleep quantity. Among women, no significant grade differences were observed in the PSQI global score. The grade differences in the PCS and MCS were weaker and less consistent than those of men, and the differences hardly changed when the PSQI global score was adjusted for., Conclusion: Sleep quality may mediate the relationship between SES and physical and, in particular, mental health in men.

Study Objective: To clarify whether socioeconomic and sex inequalities in poor sleep quality are explained by socioeconomic and sex differences in work and family characteristics., Design: A cross-sectional study., Participants: Three thousand five hundred fifty-six employees (2397 men and 1159 women) aged 20 to 65 years in local government in Japan., Measurements: Respondents completed a self-administered questionnaire that asked about sleep quality, as measured by the Pittsburgh Sleep Quality Index; work characteristics, as measured by the job-demand-control-support model, work hours, and shift work; and family characteristics, such as family structure and family-work conflicts., Results: Lower control at work, higher work demands, lower social support, shorter and longer working hours, shift work, being single, higher family-to-work conflict, and higher work-to-family conflict were independently associated with poorer sleep quality in both men and women. In men, the age-adjusted odds ratio (OR) of low-grade employees for poor sleep quality was 1.64 (95% confidence interval: 1.14-2.36) in comparison with high-grade employees. The difference in sleep was attenuated when work and family characteristics were adjusted for (OR=1.25 [0.84-1.86]). Among women, there was no significant grade difference in sleep. Women tended to have poorer sleep quality than men (the age-adjusted OR=1.75 [1.49-2.06]). The sex difference was attenuated and no longer significant when adjustments were made for work and family characteristics (OR=1.04 [0.85-1.27])., Conclusions: The results of this study suggest that work and family characteristics may be important for reducing socioeconomic and sex inequalities in sleep. Sex differences in the pattern of socioeconomic inequalities in sleep deserve further research.

Study Objective: To determine gender and weekly differences in sleep patterns in Japanese children in junior high school., Design: A school-based study., Setting: N/A., Participants: Ninety-one children, aged 13 to 14 years, from junior high schools in Japan., Interventions: N/A., Measurements: Sleep patterns were measured using actigraphy (Actiwatch) for 7 consecutive days, in parallel with daily logs and a questionnaire., Results: Girls showed better indicators of sleep and higher degrees of motionless sleep during the week. Actual sleep values and sleep efficiency were higher and the fragmentation index was lower in girls compared to in boys. On weekends, bedtime was delayed, especially for boys--11:17 PM versus 10:49 PM for girls. A higher level of activity during sleep was more evident among boys, especially during weekends. Boys also showed a higher number of nighttime awakenings. Subjective complaints such as drowsiness and desire for a longer sleep time were significantly higher for the boys., Conclusion: Our findings support the hypothesis that, during the junior high school period, the majority of sleep-quality indicators in Japanese schoolchildren were better for girls than for boys. Boys' sleep was less efficient and more fragmented during the entire week in comparison to that of the girls. These objective findings are supported by subjective assessments. Additional studies should clarify whether these disparities are culturally determined or represent a universal gender-specific characteristic.

Background: Vitamin D receptor (VDR) gene polymorphisms have been inconsistently associated with bone mineral density (BMD). To precisely evaluate the associations between three VDR gene polymorphisms and BMD, we performed a large-scale representative study of the Japanese female population., Methods: Fifty women were randomly selected from each of the 5-year age stratified populations (15-79 years) in each of the three municipalities examined, as a part of the Japanese population-based osteoporosis (JPOS) baseline study in 1996. In the study, BMD at the lumbar spine, hip, and distal forearm were measured using dual energy X-ray absorptiometry (DXA). Two polymorphisms were determined in the VDR gene locus identified by the restriction endonucleases ApaI and TaqI through a novel allele discrimination method using two different allele-specific fluorescence probes. The other VDR gene polymorphism was identified by the restriction endonuclease FokI using the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method. Changes in BMD were determined in a follow-up study 3 years after the baseline study., Results: After the exclusion of women who had any medical or menstrual history affecting BMD, 1434 women were analysed. The annual per cent changes in BMD at the lumbar spine over 3 years in subjects with tt genotype in the TaqI polymorphism were different from other genotypes, both in the women who were premenopausal at the follow-up survey (F-premenopausal women) and in the women who were postmenopausal at the baseline survey (B-postmenopausal women). However, the effects of tt genotype on change in BMD were opposite in the two groups. In addition, these associations or tendencies were not observed at the different skeletal sites., Conclusions: This study revealed that none of the individual VDR gene polymorphisms displayed consistent association with baseline BMD or BMD change. Therefore, the effect of the VDR genotype on bone mass is negligible in Japanese women.

Background: Although there have been a number of studies on the effects of multiple roles on health and how a combination of work and family roles may be either advantageous (role enhancement) or disadvantageous (role strain) for health, there has been relatively little investigation on the psychosocial content of such roles. Work-to-family conflict and family-to-work conflict could arise from inability to combine multiple roles and result in stress and ill health. The question of whether both types of conflict mediate between the association of multiple roles with health has not been analysed before. This paper sets out to investigate whether: (1) work-to-family conflict or family-to-work conflict contributes towards explaining the association of multiple roles with mental health; (2) the effect of work-to-family conflict and family-to-work conflict on mental health varies by gender; (3) the effect of work-to-family and family-to-work conflict on mental health vary between countries with different welfare state arrangements and social norms., Methods: Cross-sectional data of economically active male and female public sector employees aged 35-60 in London (UK), Helsinki (Finland), and the West Coast of Japan. Linear regression models (separate for each gender and cohort) of SF-36 mental component scores were analysed with role combinations, family-to-work and work-to-family conflict as explanatory variables., Results: Single fathers in all three cohorts and of single mothers in the Helsinki cohort had poor mental health, and this was partly explained by their higher levels family-to-work conflict. Both types of conflict affect the mental health of men and women independently of each other. Japanese women had the greatest conflict and poorest mental health while Helsinki women had the lowest conflict and best mental health., Conclusion: Both work-to-family and family-to-work conflict affect the mental health of men and women in three different countries. Work and family roles and the balance between the two may be important for the mental health of men and women in industrialized societies. Any analysis of the effect of multiple roles on health needs to take into account the psychosocial content of such roles.

Background: To compare socioeconomic differences in behavioural and biological risk factors in Japanese and English 39-59-year-old employed men., Methods: We measured systolic and diastolic blood pressure, total and high density lipoprotein (HDL) cholesterol, body mass index (BMI), waist-to-hip ratio and fibrinogen at medical screenings and smoking and alcohol consumption in health questionnaires among Japanese non-manual and manual employees of a steel products company and among English non-manual employees working in Civil Service departments., Results: In England, higher employment grades were advantaged with regard to most risk factors studied. In Japan the picture was different in that higher grades had higher BMI, waist-to-hip-ratio and lower HDL cholesterol. In Japan employment grade differences in these three risk factors are clearly larger among younger men than among older men, while in England age differentials in the grade differences are small. Similar results were obtained for education., Conclusions: Important differences in the social patterning of risk factors were observed in our cohorts of employed Japanese and English men. The contribution that these risk factors make in explaining social differences in health may vary accordingly. Studies that identify the common and unique determinants of socioeconomic health differences in different populations are needed.

To determine whether birth weight and childhood growth, especially rate of height increase, are independently related to major cardiovascular disease risk factors in adult life, the authors conducted a 20-year follow-up study in a Japanese population, using the record-linkage method. From medical checkup data for babies and for residents aged 20 years in Ishikawa, Japan, the authors obtained 20-year follow-up data (1985-1994) on 4,626 participants (2,198 men and 2,428 women) born in 1965-1974. Using multiple linear regression analysis, the authors estimated that a 1-standard-deviation higher birth weight was significantly associated with systolic blood pressure that was lower by 1.6 mmHg in men and by 1.0 mmHg in women, and with a serum cholesterol level that was lower by 0.07 mmol/liter in men and by 0.04 mmol/liter in women, after adjustment for current weight and rate of height increase. Moreover, after adjustment for birth weight and current weight, a 1-standard-deviation higher rate of height increase from age 3 years to age 20 years was significantly associated with systolic blood pressure that was lower by 0.7 mmHg in men and by 0.5 mmHg in women, and with serum cholesterol that was lower by 0.09 mmol/liter in men and by 0.05 mmol/liter in women. The results suggest that lower birth weight and lower rate of height increase during childhood are independently associated with increases in blood pressure and serum cholesterol in adult life.

Parameters of humoral and cellular immunity were investigated in patients with chronic cadmium (Cd) disease and compared to values in an age matched control group. In the patient group, there was a slight reduction in WBC although T and B lymphocyte ratios were normal. RBC counts were significantly reduced (P less than 0.05). No significant differences in serum immunoglobulin (Ig) concentrations were found. Blood Cd was significantly raised (P less than 0.001) and urine Cd was elevated. In lymphocyte transformation tests, responses to Staphylococcus aureus, PHA and PPD were not impaired and no evidence of Cd hypersensitivity was found. Responses in PHA and ADCC cytotoxicity tests were normal and nitro-blue tetrazolium (NBT) reduction was only slightly decreased in the patient group. beta 2-Microglobulin (beta 2M) was significantly raised in serum (P less than 0.001) and urine (P less than 0.001) of patients. Patient 3 day lymphocyte cultures produced significantly more beta 2M than cultures from the control group (P less than 0.01). beta 2M was increased in lymphocyte cultures, from patient and control groups, containing PHA (P less than 0.01), low Cd concentrations (P less than 0.01) or PPD. The results indicate that chronic Cd disease is not associated with significant alterations in immunological parameters in vitro. The high beta 2M levels associated with this disease may not result entirely from kidney damage but may in part be due to an increase in the release of beta 2M from cells, induced by Cd.

The rate of lithium, sodium, and potassium transport in red blood cells is thought to be associated with essential hypertension. In order to investigate the contribution of genetic and environmental factors to cation transport, their correlation was analyzed in 60 parent-offspring pairs and 17 husband-wife pairs in Toyama City, Japan. Lithium-sodium countertransport and sodium-potassium cotransport rates were significantly correlated in parent-offspring pairs (r = 0.52, p less than 0.01 and r = 0.46, p less than 0.01, respectively) but not in husband-wife pairs. Sodium pump rates were significantly correlated in both pairs (r = 0.48, p less than 0.01 in parent-offspring pairs, r = 0.46, p less than 0.05 in husband-wife pairs). Therefore, lithium-sodium countertransport and sodium-potassium cotransport were found to have a substantial genetic component and the sodium pump to have a substantial environmental component. Sodium pump rates were significantly correlated with sodium/creatinine (r = 0.21, p less than 0.05) and sodium/potassium (r = 0.32, p less than 0.01) in casual urine.
