Debate continues in many European countries about both equality of opportunity and the continuing wastage of talent, and the ways in which differing systems of secondary schooling contribute to these. Drawing on Turner’s concepts of sponsored and contest mobility and on Allmendinger’s classification along the dimensions of stratification and selection, we describe the amount of flexibility currently in the German secondary school system which, despite ongoing reforms, is still stratified and selective. Earlier research suggests that the sorting process is socially, not just academically, selective. Building on this, we analyse factors influencing whether individuals make use of the available opportunities for changing track. We find that, rather than alleviating the early social inequality, these opportunities reinforce it since young people from more privileged backgrounds are more likely to benefit from flexibility, whereas disadvantaged individuals are more likely to drop out of the academic track. Most earlier relevant work has used regression-based methods, but we use an alternative configurational method, Ragin’s Qualitative Comparative Analysis (QCA). Using this set theoretic approach allows us, when exploring what conjunctions of factors are sufficient conditions for the types of mobility we focus upon, to move beyond the limitations of a ‘net effects’ approach. Purpose. To determine the efficacy and safety of clofarabine and cytarabine (Ara-C) in adult patients with relapsed or refractory acute myeloid leukemia(AML)and in elderly patients with untreated AML and heart disease. Patients and Methods. Patients with relapsed/refractory AML and older patients for whom there was a concern over toxicity from additional anthracyclines received 5 days of clofarabine, 40 mg/m2 per day i.v. over 1 hour, followed 4 hours later by Ara-C, 1,000 mg/m2 per day i.v. over 2 hours. Results. Thirty patients were enrolled. The median age was 67 years (range, 38-82 years) and 18 (60%) had received at least one prior therapy. Eleven (37%) patients had a history of cardiovascular disease and were considered to be at high risk for anthracycline toxicity. High-risk cytogenetic abnormalities were present in 14 (47%) patients. The overall response rate (complete remission [CR] plus partial remission) was 53%, including a CR in 14 patients (47%). Responses were observed in all cytogenetic risk groups and in patients who had received up to five prior therapies. The median disease-free survival interval was 9.5 months. The 30-day mortality rate was 20% (de novo AML, 8%; relapsed/ refractory AML, 28%). Of the 14 patients achieving a CR, half were able to proceed to curative hematopoietic stem cell transplantation. Conclusions. Clofarabine in combination with Ara-C is effective in both untreated and previously treated patients with AML. The design and location of double bars in Beethoven's autograph scores is very different from what one might deduce from recent scholarly editions of his music, which normally disregard this feature of his notation. The study of double bars has in fact generally neglected in musicology as a whole, yet the signs present several problems. These are particularly acute in Beethoven's music, especially in works that consist movements or sections not wholly independent of each other. A survey of a large number of his autograph scores reveals that his use of double bars is very complex largely systematic, and has significant implications for how he envisaged the boundaries between sections of his works. These implications offer fresh clues to the most appropriate interpretation in performance at relevant points. Beethoven's double bars also provide startling evidence that the Third Piano Concerto was composed mainly 1800 rather than 1803, and that the Violin Romance Op. 50 was originally the slow movement of a concerto--most likely WoO 5.
