We report the discovery of two mini-Neptunes in near 2:1 resonance orbits (P = 7.610303 d for HIP 113103 b and P  = 14.245651 d for HIP 113103 c) around the adolescent K-star HIP 113103 (TIC 121490076). The planet system was first identified from the TESS mission, and was confirmed via additional photometric and spectroscopic observations, including a ∼17.5 h observation for the transits of both planets using ESA CHEOPS. We place ≤4.5 min and ≤2.5 min limits on the absence of transit timing variations over the 3 yr photometric baseline, allowing further constraints on the orbital eccentricities of the system beyond that available from the photometric transit duration alone. With a planetary radius of R p  =   |$1.829_{-0.067}^{+0.096}$| R⊕, HIP 113103 b resides within the radius gap, and this might provide invaluable information on the formation disparities between super-Earths and mini-Neptunes. Given the larger radius R p  =  |$2.40_{-0.08}^{+0.10}$| R⊕ for HIP 113103 c, and close proximity of both planets to HIP 113103, it is likely that HIP 113103 b might have lost (or is still losing) its primordial atmosphere. We therefore present simulated atmospheric transmission spectra of both planets using JWST, HST , and Twinkle. It demonstrates a potential metallicity difference (due to differences in their evolution) would be a challenge to detect if the atmospheres are in chemical equilibrium. As one of the brightest multi sub-Neptune planet systems suitable for atmosphere follow up, HIP 113103 b and HIP 113103 c could provide insight on planetary evolution for the sub-Neptune K-star population. We report on the discovery and validation of a two-planet system around a bright (V  = 8.85 mag) early G dwarf (1.43  R⊙, 1.15  M⊙, TOI 2319) using data from NASA's Transiting Exoplanet Survey Satellite (TESS). Three transit events from two planets were detected by citizen scientists in the month-long TESS light curve (sector 25), as part of the Planet Hunters TESS project. Modelling of the transits yields an orbital period of |$11.6264 _{ - 0.0025 } ^ { + 0.0022 }$|  d and radius of |$3.41 _{ - 0.12 } ^ { + 0.14 }$|  R⊕ for the inner planet, and a period in the range 19.26–35 d and a radius of |$5.83 _{ - 0.14 } ^ { + 0.14 }$|  R⊕ for the outer planet, which was only seen to transit once. Each signal was independently statistically validated, taking into consideration the TESS light curve as well as the ground-based spectroscopic follow-up observations. Radial velocities from HARPS-N and EXPRES yield a tentative detection of planet b, whose mass we estimate to be |$11.56 _{ - 6.14 } ^ { + 6.58 }$|  M⊕, and allow us to place an upper limit of 27.5 M⊕ (99 per cent confidence) on the mass of planet c. Due to the brightness of the host star and the strong likelihood of an extended H/He atmosphere on both planets, this system offers excellent prospects for atmospheric characterization and comparative planetology. We report on the discovery and validation of TOI 813 b (TIC 55525572 b), a transiting exoplanet identified by citizen scientists in data from NASA's Transiting Exoplanet Survey Satellite (TESS) and the first planet discovered by the Planet Hunters TESS project. The host star is a bright (V  = 10.3 mag) subgiant (⁠|$R_\star =1.94\, R_\odot$|⁠ , |$M_\star =1.32\, M_\odot$|⁠). It was observed almost continuously by TESS during its first year of operations, during which time four individual transit events were detected. The candidate passed all the standard light curve-based vetting checks, and ground-based follow-up spectroscopy and speckle imaging enabled us to place an upper limit of |$2\, M_{\rm Jup}$| (99 per cent confidence) on the mass of the companion, and to statistically validate its planetary nature. Detailed modelling of the transits yields a period of |$83.8911 _{ - 0.0031 } ^ { + 0.0027 }$|  d, a planet radius of 6.71 ± 0.38  R ⊕ and a semimajor axis of |$0.423 _{ - 0.037 } ^ { + 0.031 }$|  AU. The planet's orbital period combined with the evolved nature of the host star places this object in a relatively underexplored region of parameter space. We estimate that TOI 813 b induces a reflex motion in its host star with a semi-amplitude of ∼6 m s−1, making this a promising system to measure the mass of a relatively long-period transiting planet. We report detection of quasi-periodic (1.5-d) dimming of HD 240779, the solar-mass primary in a 5 arcsec visual binary (also TIC 284730577), by the Transiting Exoplanet Survey Satellite. This dimming, as has been shown for other 'dipper' stars, is likely due to occultation by circumstellar dust. The barycentric space motion, lithium abundance, rotation, and chromospheric emission of the stars in this system point to an age of ≈125 Myr, and possible membership in the AB Doradus moving group. As such it occupies an important but poorly explored intermediate regime of stars with transient dimming between young stellar objects in star-forming regions and main-sequence stars, and between UX Orionis-type Ae/Be stars and M-type 'dippers'. HD 240779, but not its companion BD+10 714B, has Wide-field Infrared Survey Explorer (WISE) -detected excess infrared emission at 12 and 22 μ m indicative of circumstellar dust. We propose that infrared emission is produced by collisions of planetesimals during clearing of a residual disc at the end of rocky planet formation, and that quasi-periodic dimming is produced by the rapid disintegration of a ≳100 km planetesimal near the silicate evaporation radius. Further studies of this and similar systems will illuminate a poorly understood final phase of rocky planet formation like that which produced the inner Solar system. [ABSTRACT FROM AUTHOR]

We have identified a star, EPIC 249706694 (HD 139139), that was observed during K2 Campaign 15 with the Kepler extended mission that appears to exhibit 28 transit-like events over the course of the 87-d observation. The unusual aspect of these dips, all but two of which have depths of 200 ± 80 ppm, is that they exhibit no periodicity, and their arrival times could just as well have been produced by a random number generator. We show that no more than four of the events can be part of a periodic sequence. We have done a number of data quality tests to ascertain that these dips are of astrophysical origin, and while we cannot be absolutely certain that this is so, they have all the hallmarks of astrophysical variability on one of two possible host stars (a likely bound pair) in the photometric aperture. We explore a number of ideas for the origin of these dips, including actual planet transits due to multiple or dust emitting planets, anomalously large TTVs, S- and P-type transits in binary systems, a collection of dust-emitting asteroids, 'dipper-star' activity, and short-lived starspots. The Kepler mission is providing photometric data of exquisite quality for the asteroseismic study of different classes of pulsating stars. These analyses place particular demands on the pre-processing of the data, over a range of time-scales from minutes to months. Here, we describe processing procedures developed by the Kepler Asteroseismic Science Consortium to prepare light curves that are optimized for the asteroseismic study of solar-like oscillating stars in which outliers, jumps and drifts are corrected. We present our analysis of the long-cadence Kepler data for the well-studied Blazhko star RR Lyr, gathered during the first two quarters of the satellite's observations and covering a total of 127 d. Besides being of great importance for our understanding of RR Lyrae stars in general, these RR Lyr data can be regarded as a case study for observations of bright stars with Kepler. Kepler can perform high-precision photometry on targets like RR Lyr, as the saturated flux is conserved to a very high degree. The Kepler data on RR Lyr are revolutionary in several respects. Even with long-cadence sampling (one measurement per 29.4 min), the unprecedented precision ( mmag) of the Kepler photometry allows the study of the star's extreme light-curve variations in detail. The multiplet structures at the main frequency and its harmonics, typical for Blazhko stars, are clearly detected up to the quintuplets. For the first time, photometric data of RR Lyr reveal the presence of half-integer frequencies, linked to a period-doubling effect. This phenomenon may be connected to the still unexplained Blazhko modulation. Moreover, with three observed Blazhko cycles at our disposal, we observe that there is no exact repetition in the light-curve changes from one modulation cycle to the next for RR Lyr. We present our analysis of Kepler observations of 29 RR Lyrae stars, based on 138 d of observation. We report precise pulsation periods for all stars. Nine of these stars had incorrect or unknown periods in the literature. 14 of the stars exhibit both amplitude and phase Blazhko modulations, with Blazhko periods ranging from 27.7 to more than 200 d. For V445 Lyr, a longer secondary variation is also observed in addition to its 53.2-d Blazhko period. The unprecedented precision of the Kepler photometry has led to the discovery of the the smallest modulations detected so far. Moreover, additional frequencies beyond the well-known harmonics and Blazhko multiplets have been found. These frequencies are located around the half-integer multiples of the main pulsation frequency for at least three stars. In four stars, these frequencies are close to the first and/or second overtone modes. The amplitudes of these periodicities seem to vary over the Blazhko cycle. V350 Lyr, a non-Blazhko star in our sample, is the first example of a double-mode RR Lyrae star that pulsates in its fundamental and second overtone modes. [ABSTRACT FROM AUTHOR]

Background: Conception following gonadotrophin-stimulated IVF and embryo transfer has been associated with a higher intrafollicular cortisol:cortisone ratio and decreased metabolism of cortisol to cortisone. The role of glucocorticoids in human oocyte maturation is not fully understood, but active glucocorticoid (cortisol) may be important. This study relates intrafollicular cortisol and cortisone concentrations to oocyte fertilization and embryo implantation in unstimulated cycles.Methods: Patients aged <40 years with favourable sperm underwent unstimulated IVF-embryo transfer. Study 1 related intrafollicular cortisol levels to oocyte and IVF outcome: (i) fertilized, pregnant (n = 9); (ii) fertilized, not pregnant (n = 21); and (iii) unfertilized (n = 12). Study 2 was a case-control study of 27 patients (same outcome groups of equal size) which measured intrafollicular cortisol, cortisone and the cortisol:cortisone ratio.Results: Conception cycles demonstrated higher cortisol concentrations compared with the fertilized group (study 1) [median (95% confidence interval): 299 (249-330) versus 227 nmol/l (185-261); P < 0.05] and higher cortisol:cortisone ratios when compared with the unfertilized group (study 2) [7.38 (5.23-9.19) versus 3.56 (1.75-7.46) respectively; P = 0.02]. Of the women with cortisol:cortisone ratios greater than the outcome independent mean of 5.90, 58% conceived compared with only 13% with ratios <5.90 (P < 0.02).Conclusion: Higher cortisol:cortisone ratios in conception cycles suggest that active glucocorticoid may be important for final oocyte maturation and embryo implantation in unstimulated cycles. Background: Cancellation of assisted conception cycles because of poor ovarian response to gonadotrophins is a significant problem in assisted reproduction. Various adjuvant treatments have been suggested to improve responsiveness. This study reports on the potential benefits of low dose dexamethasone.Methods: Patients <40 years of age were invited to participate in a twin centre prospective double blind randomized placebo controlled study. A total of 290 patients were recruited and computer randomized using sealed envelopes to receive either 1 mg dexamethasone (n = 145) or placebo tablets (n = 145) in addition to a standard long protocol gonadotrophin-releasing hormone analogue with gonadotrophin stimulation regime.Results: A significantly lower cancellation rate for poor ovarian response was observed in the dexamethasone group compared with controls (2.8 versus 12.4% respectively, P < 0.002). Further comparisons between the dexamethasone group and controls were made of median fertilization rates (60 versus 61% respectively, NS), implantation rates (16.3 versus 11.6% respectively, NS) and pregnancy rate per cycle started (26.9 versus 17.2%, NS). The benefit was apparent in patients both with polycystic and normal ovaries.Conclusion: Low dose dexamethasone co-treatment reduces the incidence of poor ovarian response. It may increase clinical pregnancy rates and should be considered for inclusion in stimulation regimes to optimize ovarian response. [ABSTRACT FROM AUTHOR]

There is growing evidence that the pathogenic effects of bacterial vaginosis may not be confined to the lower genital tract. Possible associations with infertility and effects on fertilization and implantation were studied in patients undergoing in-vitro fertilization (IVF) treatment. High vaginal swabs taken at the time of oocyte collection were assessed by Gram staining. The prevalence of bacterial vaginosis and of intermediate and normal flora in 301 patients was 25.6, 14.0 and 60.4% respectively. Bacterial vaginosis was more prevalent in patients with tubal (31.5%, n = 149) compared with non-tubal (19.7%, n = 152) infertility (odds ratio (OR) 1.87, CI 1.11-3.18, P = 0.02). Bacterial vaginosis did not have an adverse effect on fertilization rate. Further, no significant difference in implantation rates was seen when comparing bacterial vaginosis (15. 8%, OR 1.03, CI 0.66-1.61) and intermediate flora (13.1%, OR 0.82, CI 0.45-1.52) with normal flora (15.5%). Though confidence intervals around the observations were relatively wide, the findings suggest that routine screening for bacterial vaginosis in the hope of improving the success of IVF treatment is not justified. The prevention of complications in pregnancy associated with bacterial vaginosis might be a more relevant indication for screening at the time of IVF treatment, in particular patients with tubal disease, if treatment were shown to be effective for that particular purpose. However, antibiotic treatment before IVF has been shown to be positively disadvantageous for IVF by encouraging other organisms. [ABSTRACT FROM AUTHOR]

This study investigated the development of functional ovarian cysts during pituitary down-regulation prior to in-vitro fertilization (IVF), and identified 16 cases of cysts in 288 IVF cycles studied. Comparing the patients with functional ovarian cysts to the other 272 IVF cycles, there was no significant difference in age or incidence of endometriosis but significantly (P < 0.01) more patients with cysts had ovulatory dysfunction. The serum progesterone was < 5.7 nmol/l in all 16 patients with cysts on day 4 of the IVF cycle, and in eight of these patients the serum progesterone was < 5.7 nmol/l on the day buserelin was commenced. In 10 of the 16 patients with cysts, serum oestradiol concentrations remained elevated despite the prolonged use of buserelin, and the cysts were aspirated. The aspirate in all cases was clear without any suggestion of endometriosis. The cyst aspirates had significantly lower progesterone (P < 0.001), higher androstenedione (P < 0.01) and similar oestradiol concentrations to 10 follicular fluid samples collected at the time of oocyte retrieval. This study suggests that functional ovarian cysts may develop during pituitary down-regulation, and these cysts are follicular cysts rather than persistent corpora lutea or endometriomata. This study identifies a group of 87 patients, who demonstrated a ‘poor’ response to a standard buserelin/human menopausal gonadotrophin (HMG) regime. The subsequent outcome in 61 of these ‘poor’ responders when treated with a higher dose of HMG to achieve a satisfactory response was compared with 250 patients, who showed a ‘good’ response to the standard regime. ‘Poor’ responders were significantly older than ‘good’ responders ( < 0.001), but no significant difference was demonstrated in the indication for in-vitro fertilization (IVF). Even on higher doses of HMG, ‘poor’ responders took longer for their follicles to achieve maturity than the ‘good’ responders ( < 0.01). ‘Poor’ responders produced 8.9 oocytes per oocyte collection compared to 11.8 in the ‘good’ responders ( < 0.01). The fertilization rate was significantly lower in the ‘poor’ responders compared to the ‘good’ responders ( < 0.01). Although there was no significant difference in morphometric grading between ‘poor’ responder embryos and ‘good’ responder embryos, the rate of cell division was significantly slower in embryos of the ‘poor’ responders than the ‘good’ responders ( < 0.01). The pregnancy rate per oocyte retrieval was 9% in the ‘poor’ responders compared to 29% in the ‘good’ responders ( < 0.01). This review examines recent evidence suggesting a role for the immune system, in particular cytokines, in the pathogenesis of ovarian hyperstimulation syndrome (OHSS). Ovarian tissue is known to contain cells capable of producing a range of immunological mediators and the concentrations of these have been shown to be elevated in serum and ascitic fluid from women with established OHSS. Available evidence points to a role for vascular endothelial growth factor and interleukin-2, possibly acting through other intermediary cytokines, in the pathogenesis of OHSS. However, each individual has a unique cytokine profile and several cytokines may share biological actions, making it difficult to interpret data on isolated cytokine concentrations from relatively small numbers of patients. Improved understanding of the role of the immune system in the development of OHSS may have implications for the prediction, prevention and management of this iatrogenic condition.
