While the invasive spotted lanternfly, Lycorma delicatula (White) [Hemiptera: Fulgoridae], continues to expand its range in the United States, there remains a knowledge gap regarding the economic threat that this pest presents to forest ecosystems and production nurseries. L. delicatula uses several common hardwood trees as hosts and a previous study found that short-term feeding can reduce growth of young maple saplings. Herein, long-term feeding over 4 consecutive seasons significantly reduced diameter growth and below-ground starch storage in roots of young silver maples (Acer saccharinum L.), weeping willows (Salix babylonica L.), river birches (Betula nigra L.), and trees of heaven (Ailanthus altissima [Mill.] Swingle) in response to L. delicatula feeding pressure in a density-dependent manner. In Year 3 when feeding pressure was the lowest, silver maple and willow recovered with greater diameter growth than in Year 2. Nutrients essential for photosynthesis and growth (iron, sulfur, and phosphorus) were reduced in leaves of all tree species compared to controls in the second year. This 4-yr study represents a worst-case scenario in which L. delicatula fed on the same trees for 4 consecutive growing seasons. In the wild, population numbers can vary greatly from year to year on individual trees and they move frequently among hosts (until autumn when they settle on A. altissima or other late-season hosts that have not yet senesced). Management to control the spotted lanternfly, Lycorma delicatula (White), would ideally achieve managers' goals while limiting impacts on nontarget organisms. In a large-scale field study with 45 plots at least 711 m2, we tested foliar applications of dinotefuran and 2 formulations of Beauveria bassiana (Balsamo) Vuillemin, each applied from the ground and separately by helicopter. Applications targeted early instar nymphs. For both application methods, a single treatment with dinotefuran significantly reduced L. delicatula numbers, as measured by catch on sticky bands (91% reduction by air and 84% reduction by ground 19 days after application) and by timed counts (89% reduction by air and 72% reduction by ground 17 days after application). None of the B. bassiana treatments significantly reduced L. delicatula numbers, even after 3 applications. Beauveria bassiana infection in field-collected nymphs ranged from 0.4% to 39.7%, with higher mortality and infection among nymphs collected from ground application plots. Beauveria bassiana conidia did not persist for long on foliage which probably contributed to low population reduction. Nontarget effects were not observed among arthropods captured in blue vane flight intercept traps, San Jose Scale pheromone sticky traps or pitfall traps, but power analysis revealed that small reductions of less than 40% may not be detected despite extensive sampling of 48,804 specimens. These results demonstrate that dinotefuran can markedly reduce local abundance of L. delicatula with little apparent effect on nontarget insects when applied shortly after hatch, and that aerial applications can match or exceed the effectiveness of applications from the ground. Transdisciplinary research approaches are being applied to today's complex health problems, including the climate crisis and widening inequalities. Diverse forms of disciplinary and experiential knowledge are required to understand these challenges and develop workable solutions. We aimed to create an updated model reflective of the strengths and challenges of current transdisciplinary health research that can be a guide for future studies. We searched Medline using terms related to transdisciplinary, health and research. We coded data deductively and inductively using thematic analysis to develop a preliminary model of transdisciplinary research. The model was tested and improved through: (i) a workshop with 27 participants at an international conference in Xiamen, China and (ii) online questionnaire feedback from included study authors. Our revised model recommends the following approach: (i) co-learning , an ongoing phase that recognizes the distributed nature of knowledge generation and learning across partners; (ii) (pre-)development , activities that occur before and during project initiation to establish a shared mission and ways of working; (iii) reflection and refinement to evaluate and improve processes and results, responding to emergent information and priorities as an ongoing phase; (iv) conceptualization to develop goals and the study approach by combining diverse knowledge; (v) investigation to conduct the research; (vi) implementation to use new knowledge to solve societal problems. The model includes linear and cyclical processes that may cycle back to project development. Our new model will support transdisciplinary research teams and their partners by detailing the necessary ingredients to conduct such research and achieve health impact. [ABSTRACT FROM AUTHOR]

Spotted lanternfly (Lycorma delicatula) is a recently introduced pest in the United States, where it threatens the wine, timber, and ornamentals industries. Knowledge of the dispersal ability of L. delicatula is key to developing effective management strategies for this invasive pest. We conducted a mark, release, re-sight study, marking nymphs with fluorescent powders and observing dispersal distances from a central release point at three time points over 7 d following release. To examine how dispersal patterns changed over the course of nymphal development, we repeated this process for each of L. delicatula 's four instars. All releases were conducted in contiguous, deciduous forest, which is a widespread habitat type within L. delicatula 's invaded range and a habitat where this pest may have negative ecological and economic impacts. We found that nymphs displayed clear directionality in their movement following release, apparently preferring to move uphill on the modest 6° grade at our release site. Most nymphs remained near the release location, while some moved tens of meters. The maximum displacement we observed was 65 m from the release point, 10 d after release. Nymphs were re-sighted singly and in small groups on a variety of trees, shrubs, and understory vegetation. All four instars had similar dispersal distances over time, though third instar nymphs moved farthest on average, with estimated median displacement of 16.9 m 7 d after release. Despite its broad host range, the spotted lanternfly Lycorma delicatula (White), is known to have a marked preference for Ailanthus altissima. However, whether this polyphagous phloem feeder can complete its life cycle in the absence of A. altissima is unknown. We examined the performance of L. delicatula with and without access to A. altissima by tracking development, survival, host tree species association, and oviposition in large enclosures planted with Salix babylonica and Acer saccharinum along with either A. altissima or Betula nigra. We monitored enclosures from late May 2019 through June 2020. Lycorma delicatula survival was slightly higher in enclosures with A. altissima and 50% of individuals in A. altissima enclosures reached the adult stage ~6.5 d earlier than in enclosures without A. altissima. In the presence of A. altissima , nymphs were most frequently observed on this host while adults were found at similar frequencies on A. altissima and A. saccharinum. In the absence of A. altissima , nymphs were most frequently associated with S. babylonica and A. saccharinum , while adults were most often found on A. saccharinum. Females laid a total of 46 and 6 egg masses in enclosures with and without A. altissima , respectively, before freezing temperatures killed the remaining adults. The proportion of eggs that hatched per egg mass did not differ between treatments. Although L. delicatula can complete development and reproduce on other host species without access to A. altissima , fitness was reduced. With a high surface to volume ratio, small organisms must carefully regulate their internal water status. Spotted-wing drosophila, Drosophila suzukii (Matsumura), is an invasive frugivorous insect distributed across a wide range of geographical regions that can have periods of dry and hot weather, suggesting that this species has strategies to avoid stressful environments and reduce water loss. It also survives winter as an adult fly, indicating that it has adaptations to the low air humidity of this season. To determine the importance of water stress to D. suzukii, we studied their survival in environments of low humidity, which was manipulated using Drierite, and their survival and water loss in response to desiccation. Survival of both sexes was lower in drier conditions, and while female winter morph D. suzukii had higher mortality early on, remaining flies were able to survive longer in the drier conditions than the summer morphs. A bioassay method was adapted from Enjin et al. (2016) using 48-well plates to videotape the location of flies and quantify their behavioral responses to humidity. Male and female D. suzukii avoided dry conditions within the bioassay system, but only when there was at least 25% differential between humidity extremes. This response was observed for both summer and winter morphs of D. suzukii and our results provide guidance for attempts to manipulate crop environments to reduce the economic impact of this pest. Background: The extent to which individuals are similar to their sexual partners influences STI-transmission probabilities, yet there is a dearth of empirical data, especially those representative of the population.Methods: Analyses of data reported by 10 759 sexually active people aged 16-74 y interviewed for a British national probability survey undertaken in 2010-12. Computer-assisted self-interviews asked about partner numbers and characteristics of participants' three most recent partnerships (MRPs). Opposite-sex MRPs were weighted to represent all such partnerships in the past year (N = 16 451). Estimates of disassortative age mixing (≥±5-y difference), ethnic mixing (partner of a different ethnic group) and geographical mixing (partner from a different region/country when they first met) were calculated, stratified by gender, age group and partnership status (casual/steady). Multivariable regression examined how these disassortative mixing measures were associated with STI-risk measures: condom use at first sex together at the partnership-level and, at the participant-level, STI-risk perception and reporting STI diagnoses.Results: Disassortative age mixing occurred in around one-third of opposite-sex partnerships, with men ≥5 y older in most cases, although this proportion varied by participant's gender and age group. Ethnic mixing occurred less frequently (11.3% of men's and 8.6% of women's partnerships) as did geographical mixing (14.1 and 16.3%, respectively). Disassortative mixing was more common among casual vs steady partnerships. Condom use at first sex was less likely in women's partnerships that were age-disassortative [adjusted odds ratio (AOR): 0.79, 95% confidence interval (CI): 0.69-0.95], whereas men reporting disassortative ethnic mixing were more likely to perceive themselves at STI risk (AOR: 1.76, 95% CI: 1.23-2.52) and report STI diagnoses (AOR: 2.37, 95% CI: 1.22-4.59).Conclusions: Disassortative mixing, although uncommon among opposite-sex partnerships in Britain, is independently associated with STI risk, warranting consideration in STI-prevention efforts. Objective. The mechanisms that determine the efficacy or inefficacy of MTX in JIA are ill-defined. The objective of this study was to identify a gene expression transcriptional signature associated with poor response to MTX in patients with JIA. Methods. RNA sequencing was used to measure gene expression in peripheral blood mononuclear cells collected from 47 patients with JIA prior to MTX treatment and 14 age-matched controls. Differentially expressed baseline genes between responders and non-responders were evaluated. Biological differences between all JIA patients and controls were explored by constructing a signature of differentially expressed genes. Unsupervised clustering and pathway analysis was performed. Results. A signature of 99 differentially expressed genes (Bonferroni-corrected P<0.05) capturing the biological differences between all JIA patients and controls was identified. Unsupervised clustering of samples based on this list of 99 genes produced subgroups enriched for MTX response status. Comparing this gene signature with reference signatures from sorted cell populations revealed high concordance between the expression signatures of monocytes and of MTX non-responders. CXCL8 (IL-8) was the most significantly differentially expressed gene transcript comparing all JIA patients with controls (Bonferroni-corrected P = 4.12 x 10-10 ). Conclusion. Variability in clinical response to MTX in JIA patients is associated with differences in gene transcripts modulated in monocytes. Background: There are currently no large general population epidemiological studies of Mycoplasma genitalium (MG), which include prevalence, risk factors, symptoms and co-infection in men and women across a broad age range.Methods: In 2010-–12, we conducted the third National Survey of Sexual Attitudes and Lifestyles (Natsal-3), a probability sample survey in Britain. Urine from 4507 sexually-experienced participants, aged 16–44 years, was tested for MG.Results: MG prevalence was 1.2% [95% confidence interval (CI): 0.7–1.8%] in men and 1.3% (0.9–1.9%) in women. There were no positive MG tests in men aged 16–19, and prevalence peaked at 2.1% (1.2–3.7%) in men aged 25–34 years. In women, prevalence was highest in 16–19 year olds, at 2.4% (1.2–4.8%), and decreased with age. Men of Black ethnicity were more likely to test positive for MG [adjusted odds ratio (AOR) 12.1; 95% CI: 3.7–39.4). For both men and women, MG was strongly associated with reporting sexual risk behaviours (increasing number of total and new partners, and unsafe sex, in the past year). Women with MG were more likely to report post-coital bleeding (AOR 5.8; 95%CI 1.4–23.3). However, the majority of men (94.4%), and over half of women (56.2%) with MG did not report any sexually transmitted infection (STI) symptoms. Men with MG were more likely to report previously diagnosed gonorrhoea, syphilis or non-specific urethritis, and women previous trichomoniasis.Conclusions: This study strengthens evidence that MG is an STI. Background. We examined differences by geographical origin (GO) in time from HIV seroconversion (SC) to AIDS, death, and initiation of antiretroviral therapy (cART). Methods. Data from HIV seroconverter cohorts in Europe, Australia and Canada (CASCADE) was used; GO was classified as: western countries (WE), North Africa and Middle East (NAME), sub-Saharan Africa (SSA), Latin America (LA), and Asia (ASIA). Differences by GO were assessed using Cox models. Administrative censoring date was 30 June 2008. Results. Of 16 941 seroconverters, 15 548 were fromWE, 158 NAME, 762 SSA, 349 LA, and 124 ASIA. We found no differences by GO in risks of AIDS (P = .99) and death (P = .12), although seroconverters from NAME (adjusted hazard ratio [aHR]: 0.57; 95% CI: 0.33-.94) and SSA (aHR: 0.74; 95% CI: 0.50-1.10) appeared to have lower mortality than WE. Chances of initiating cART differed by GO (P < .001): seroconverters from SSA were more likely to initiate cART than WE (aHR: 1.48; 95% CI: 1.26-1.74), but not after adjustment for CD4 at SC (aHR: 1.11; 95% CI: 0.88-1.40). Background. Recent updates of human immunodeficiency virus (HIV) treatment guidelines have raised the CD4+ cell count thresholds for antiretroviral therapy initiation from 350 to 500 cells/mm3 in the United States and from 200 to 350 cells/mm3 in mid- and low-income countries. Robust data of time from HIV seroconversion to CD4+ cell counts of 200, 350, and 500 cells/mm3 are lacking but are needed to inform health care planners of the likely impact and cost effectiveness of these and possible future changes in CD4+ cell count initiation threshold. Methods. Using Concerted Action on Seroconversion to AIDS and Death in Europe data from individuals with well-estimated dates of HIV seroconversion, we fitted mixed models on the square root of CD+ cell counts measured before combined antiretroviral therapy (cART) initiation. Restricting analyses to adults (age >16 years), we predicted time between seroconversion and CD4+ cell count <200,<350, and<500 cells/mm3 as well as CD4+ cell count distribution and proportions reaching these thresholds at 1, 2, and 5 years after seroconversion. Results. Median (interquartile range [IQR]) follow-up for the 18 495 eligible individuals from seroconversion while cART-free was 3.7 years (1.5, 7). Most of the subjects were male (78%), had a median age at seroconversion of 30 years (IQR, 25-37 years), and were infected through sex between men (55%). Estimated median times (95% confidence interval [CI]) from seroconversion to CD4+ cell count <500, <350, and <200 cells/mm3 were 1.19 (95% CI, 1.12-1.26), 4.19 (95% CI, 4.09-4.28), and 7.93 (95% CI, 7.76-8.09) years, respectively. Almost half of infected individuals would require treatment within 1 year of seroconversion for guidelines recommending its initiation at 500 cells/mm3, compared with 26% and 9% for guidelines recommending initiation at 350 and 200 cells/mm3, respectively. Background: Hard-to-reach population subgroups are typically investigated using convenience sampling, which may give biased estimates. Combining information from such surveys, a probability survey and clinic surveillance, can potentially minimize the bias. We developed a methodology to estimate the prevalence of undiagnosed HIV infection among men who have sex with men (MSM) in England and Wales aged 16-44 years in 2003, making fuller use of the available data than earlier work.Methods: We performed a synthesis of three data sources: genitourinary medicine clinic surveillance (11 380 tests), a venue-based convenience survey including anonymous HIV testing (3702 MSM) and a general population sexual behaviour survey (134 MSM). A logistic regression model to predict undiagnosed infection was fitted to the convenience survey data and then applied to the MSMs in the population survey to estimate the prevalence of undiagnosed infection in the general MSM population. This estimate was corrected for selection biases in the convenience survey using clinic surveillance data. A sensitivity analysis addressed uncertainty in our assumptions.Results: The estimated prevalence of undiagnosed HIV in MSM was 2.4% [95% confidence interval (95% CI 1.7-3.0%)], and between 1.6% (95% CI 1.1-2.0%) and 3.3% (95% CI 2.4-4.1%) depending on assumptions; corresponding to 5500 (3390-7180), 3610 (2180-4740) and 7570 (4790-9840) men, and undiagnosed fractions of 33, 24 and 40%, respectively.Conclusions: Our estimates are consistent with earlier work that did not make full use of data sources. Reconciling data from multiple sources, including probability-, clinic- and venue-based convenience samples can reduce bias in estimates. Phylogenetic reconstructions of transmission events from individuals with acute human immunodeficiency virus (HIV) infection are conducted to illustrate this group's heightened infectivity. Varied definitions of acute infection and assumptions about observed phylogenetic clusters may produce misleading results. We conducted a phylogenetic analysis of HIV pol sequences from 165 European patients with estimated infection dates and calculated the difference between dates within clusters. Nine phylogenetic clusters were observed. Comparison of dates within clusters revealed that only 2 could have been generated during acute infection. Previous analyses may have incorrectly assigned transmission events to the acutely HIV infected when they were more likely to have occurred during chronic infection. [ABSTRACT FROM AUTHOR]

Background. We examined attendance at sexually transmitted disease (STD) clinics and the prevalence, distribution, and associated demographic and behavioral factors of self-reported sexually transmitted infections (STIs) in a population survey of sexual attitudes and lifestyles. Methods. We analyzed data from stratified probability sample surveys obtained through the British National Surveys of Sexual Attitudes and Lifestyles (Natsal), which was undertaken in 1990 (n=13,765) and 2000 (n=11,161) among men and women aged 16–44 years. National STD surveillance data for 1999 were used to determine disease- and risk factor-specific proportionate population burden (PPB). Results. Between 1990 and 2000, the number of subjects who reported having attended an STD clinic during the past 5 years increased from 4.3% to 7.6% among men and from 3.3% to 6.6% among women. In 2000, 3.0% of men and 4.0% of women reported having received a diagnosis of an STI during the past 5 years; 77.6% of men and 60.3% of women with an STI had attended an STD clinic. Reported STI acquisition was independently associated with age, increasing numbers of sex partners, male homosexual partners, and partners from abroad (for women only). Of all reported STIs during the past 5 years (PPB, 10.2%), 10.2% were reported by the 2.9% of men who reported having had homosexual partners during the past 5 years. Of all reported STIs in the past 5 years (PPB, 41.6%), 41.6% were reported by the 4.0% of women who reported having ≥10 sex partners during that time. Analysis of national STI surveillance data showed that the PPB for new episodes of Chlamydia trachomatis diagnosed among homosexual men was 2.8%, that for gonorrhea was 17.4%, and that for syphilis was 32.1%. Conclusions. Numbers and types of sexual partnerships remain the dominant individual and population risk factors for STI acquisition. Combined population behavior and surveillance data demonstrate the high PPB for STIs attributable to key risk factors. This paper draws on a review of the literature about the types of health promotion activities conducted by health promoting hospitals and an observation of how some Australian hospitals have structured the organizational arrangements to be more health promoting. This paper also draws on the experiences of one of the authors (A.J) in managing and evaluating an organizational change process a: a major specialist hospital in Adelaide, South Australia, that sought to re-orientate the hospital towards placing more emphasis on health promotion. From these three sources, a typology of four approaches of organizational arrangement to health promotion is presented. These approaches are. doing a health promotion project, delegating it to the role of a specific division, department or staff'; being a health promotion setting'; and 'being a health promotion setting and improving the health of the community'. For the re-orientation of the specialist hospital to occur and be sustainable, the research indicated that over the case study period of 1994-1998 there had to be strong organizational commitment to change, supported at multiple levels of the organization, and reflected in policy and practice change. The objective was to determine the current use of their general practitioner (GP) by patients with human immunodeficiency virus (HIV) infection and whether such patients would be interested in having ‘shared care’ between a specialist HIV clinic and their GP. A questionnaire was administered to 203 HIV-positive men attending the HIV outpatient clinic of a central London teaching hospital. The main outcome measures were patient characteristics, numbers of patients registered with a GP, numbers of patients with a GP aware of their diagnosis, contacts with the GP in the last year and level of interest and shared care. Eighty-five per cent of patients were registered with a GP of whom 67% knew of the diagnosis. Those diagnosed for more than 2 years were significantly more likely to have an informed GP. A total of 73% of those registered had visited their GP in the previous year although only 27% had visited for an HIV-related problem. Only 19% had a GP actively involved in their HIV care. In all 51% of the patients indicated an interest in having shared care between the clinic and their GP. A high proportion of HIV patients are registered with and attend a GP although they rarely consult for HIV-related problems. A significant proportion of patients expressed interest in having shared care suggesting that there is the potential for increased GP involvement in the care of patients with HIV infection. To determine the relationship between antibodies to herpes simplex virus (HSV) types 1 and 2 and diagnosis of orolabial and genital herpes, a cross-sectional survey was done among 869 sexually transmitted disease clinic attenders and 1594 blood donors in London. Among clinic attenders, the prevalence of HSV-1 infection was 59.5% and that of HSV-2 infection was 22.7%, and among blood donors the prevalence was 44.6% and 7.6%, respectively. The sensitivity and specificity of a diagnosis of oral herpes for the presence of HSV-1 antibody was almost identical in the 2 groups (clinic attenders: sensitivity, 33.1%, and specificity, 91.4%; blood donors: sensitivity, 32.3%, and specificity, 94.3%). A diagnosis of genital herpes was less sensitive for antibody for HSV-2 among donors than among clinic attenders (P < .001); however, the specificity was similar in the 2 populations (clinic attenders: sensitivity, 32.1%, and specificity, 96.6%; blood donors: sensitivity, 17.5%, and specificity, 99.5%). False-positive clinical histories were also relatively common (clinic attenders, 12%; donors, 6%). The sensitivity of the diagnosis of genital herpes would be improved if accurate serologic assays for detection of HSV type-specific antibodies were more widely available. [ABSTRACT FROM PUBLISHER]

Introduction. Lack of sexual interest is a common sexual difficulty. Estimates of the prevalence of lack of sexual interest vary widely, and the evidence with regard to factors associated with lack of interest is not always consistent. Aims. The aims of this study were to identify factors associated with reporting lack of interest in sex among women, and to explore whether these factors differ according to whether or not help was sought. Methods. Our data came from the second National Survey of Sexual Attitudes and Lifestyles, a probability survey of 12,110 men and women aged 16–44 years and resident in Britain between 1999 and 2001 (N = 6,942 women). Computer-assisted personal interviewing was used to collect sociodemographic, behavioral, and attitudinal data. Main Outcome Measures. (i) Persistent lack of interest in sex (≥6 months or longer in the past year); and (ii) seeking help for persistent lack of interest in sex. We examined data for all women, regardless of their partnership status. Results. In this study, 10.7% of women reported lacking interest in sex for a period of 6 months or longer, and of these, 27.9% sought help for this difficulty. Reporting persistent low desire per se (outcome 1), and reporting seeking help for low desire (outcome 2) were associated with not enjoying sex, wanting sex more often, not being “competent” at first intercourse, poor communication about sex with partner, frequency of sex, and attitudes according sex low priority. Increasing age, reporting a birth in the last year, having children under 5 in the house, and reporting no sexual partner in the past year were associated with outcome 1 only. Being married and self-perceived health status were associated with outcome 2 only. Conclusion. Identifying the factors associated with seeking help for low sexual interest is useful in understanding risk markers for problematic sexual interest, and in providing useful avenues for therapeutic discussion.
