Study Objectives: Multiple sclerosis (MS) is an autoimmune disease impacting the central nervous system. A hallmark symptom of MS is fatigue, which impairs daytime function and quality of life (QOL). Sleep disorders and disturbances are common in persons with MS and exacerbate fatigue. We evaluated relationships between sleep-disordered breathing (SDB), insomnia symptoms, sleep quality, and daytime functioning in veterans with MS participating in a larger study. Methods: Twenty-five veterans with clinically diagnosed MS were included (average age = 57 ± 11, 80% male). One had a co-occurring thoracic spinal cord injury. Twenty-four participants completed in-laboratory polysomnography (PSG) to measure apnea-hypopnea index (AHI) and sleep efficiency (PSG-SE). Insomnia Severity Index (ISI) and Pittsburg Sleep Quality Index (PSQI) were used to measure sleep subjectively. The Flinders Fatigue Scale (FFS), Epworth Sleepiness Scale (ESS), PHQ-9 depression scale, and GAD-7 anxiety scale assessed daytime symptoms. The World Health Organization Quality of Life (WHOQOL) was used to assess quality of life. Relationships between sleep (AHI, PSG-SE, ISI, PSQI), daytime symptoms (ESS, FFS, PHQ-9, and GAD-7), and quality of life (WHOQOL) were evaluated with bivariate correlations. Results: Higher ISI (r = 0.78, 95% CI = [0.54, 0.90], p < .001), higher PSQI (r = 0.51, 95% CI = [0.10, 0.77], p = .017), and lower PSG-SE (r = -0.45, 95% CI = [-0.74, -0.02], p = .041) were associated with worse fatigue (FFS). Higher ISI was also associated with worse WHOQOL (Physical Domain; r = -0.64, 95% CI = [-0.82, -0.32], p = .001). There were no other significant relationships. Conclusion: In veterans with MS, more severe insomnia and worse sleep quality may be associated with more fatigue and lower quality of life. Background: Cognitive behavioral therapy for insomnia (CBTI) targets changing dysfunctional sleep-related beliefs. The impact of these changes on daytime functioning in older adults is unknown.Purpose: We examined whether changes in sleep-related beliefs from pre- to post-CBTI predicted changes in sleep and other outcomes in older adults.Method: Data included 144 older veterans with insomnia from a randomized controlled trial testing CBTI. Sleep-related beliefs were assessed with the Dysfunctional Beliefs and Attitudes about Sleep-16 (DBAS-16, subscales: Consequences, Worry/Helplessness, Sleep Expectations, Medication). Outcomes included sleep diary variables, actigraphy-measured sleep efficiency, Pittsburgh Sleep Quality Index (PSQI), Insomnia Severity Index (ISI), Epworth Sleepiness Scale (ESS), Flinders Fatigue Scale (FFS), Patient Health Questionnaire-9, and health-related quality of life. Analyses compared slope of change in DBAS subscales from baseline to posttreatment between CBTI and control, and assessed the relationship between DBAS change and the slope of change in outcomes from baseline to 6 months.Results: Compared to controls, the CBTI group demonstrated stronger associations between improvement in DBAS-Consequences and subsequent improvement in PSQI, ISI, ESS, and FFS. The CBTI group also demonstrated stronger associations between improvement in DBAS-Worry/Helplessness and subsequent improvements in PSQI, ISI, and FFS; improvements in DBAS-Medication and PSQI; and improvements in DBAS-Sleep Expectations and wake after sleep onset (sleep diary) and FFS (all p < .05).Conclusions: Significant reduction in dysfunctional sleep-related beliefs following CBTI in older adults predicted improvement in several outcomes of sleep and daytime functioning. GS-9695 and GS-9822 are next-generation noncatalytic site integrase inhibitors (NCINIs) with significantly improved potency against human immunodeficiency virus compared with previous drugs such as BI-224436. Development stopped due to vacuolation of the bladder urothelium seen in cynomolgus monkey but not in rat; this lesion was absent in equivalent preclinical studies with BI-224436 (tested in dog and rat). Lesions were unlikely to be attributable to target because NCINIs specifically target viral integrase protein and no mammalian homologue is known. Secondary pharmacology studies, mitochondrial toxicity studies, immunophenotyping, and analysis of proteins implicated in cell-cell interactions and/or bladder integrity (E-cadherin, pan-cytokeratin, uroplakins) failed to offer any plausible explanation for the species specificity of the lesion. Because it was characterized by inflammation and disruption of urothelial morphology, we investigated physicochemical changes in the bladder of cynomolgus monkey (urinary pH 5.5–7.4) that might not occur in the bladder of rats (urinary pH 7.3–8.5). In measurements of surface activity, GS-9822 showed an unusual transition from a monolayer to a bilayer at the air/water interface with decreasing pH, attributed to the strong association between drug molecules in adjacent bilayer leaflets and expected to be highly disruptive to the urothelium. Structural analysis of GS-9822 and GS-9695 showed zwitterionic characteristics over the range of pH expected in cynomolgus monkey but not rat urine. This exotic surface behavior is unlikely with BI-224436 since it would transition from neutral to cationic (never zwitterionic) with decreasing pH. Kidney cancer is one of the 10 most common cancers both in the United States and worldwide. Until this year, there had not previously been a conference focused on translational studies in the broad and heterogeneous group of kidney cancers. Therefore, a group of researchers, clinicians, and patient advocates dedicated to renal cell carcinoma launched the Kidney Cancer Research Summit (KCRS) to spur collaboration and further therapeutic advances in these tumors. This commentary aims to summarize the oral presentations and serve as a record for future iterations of this meeting. The KCRS sessions addressed the tumor microenvironment, novel methods of drug delivery, single cell sequencing strategies, novel immune checkpoint blockade and cellular therapies, predictive biomarkers, and rare variants of kidney cancers. In addition, the meeting included 2 sessions to promote scientific mentoring and kidney cancer research collaborations. A subsequent KCRS will be planned for the fall of 2020. [ABSTRACT FROM AUTHOR]

Long-term antibiotics are not effective for the therapy of patients with persistent symptoms and a history of Lyme disease. However, some clinicians still prescribe these therapies. We present a case of peripherally inserted central catheter-associated Nocardia nova endocarditis in a patient who had been receiving intravenous antibiotics for the management of chronic Lyme disease. Partial migration is a common life-history strategy among ungulates living in seasonal environments. The decision to migrate or remain on a seasonal range may be influenced strongly by access to high-quality habitat. We evaluated the influence of access to winter habitat of high quality on the probability of a female white-tailed deer (Odocoileus virginianus) migrating to a separate summer range and the effects of this decision on survival. We hypothesized that deer with home ranges of low quality in winter would have a high probability of migrating, and that survival of an individual in winter would be influenced by the quality of their home range in winter. We radiocollared 67 female white-tailed deer in 2012 and 2013 in eastern Washington, United States. We estimated home range size in winter using a kernel density estimator; we assumed the size of the home range was inversely proportional to its quality and the proportion of crop land within the home range was proportional to its quality. Odds of migrating from winter ranges increased by 3.1 per unit increase in home range size and decreased by 0.29 per unit increase in the proportion of crop land within a home range. Annual survival rate for migrants was 0.85 (SD = 0.05) and 0.84 (SD = 0.09) for residents. Our finding that an individual with a low-quality home range in winter is likely to migrate to a separate summer range accords with the hypothesis that competition for a limited amount of home ranges of high quality should result in residents having home ranges of higher quality than migrants in populations experiencing density dependence. We hypothesize that density-dependent competition for high-quality home ranges in winter may play a leading role in the selection of migration strategy by female white-tailed deer. For animals that forage widely, protecting young from predation can span relatively long time periods due to the inability of young to travel with and be protected by their parents. Moving relatively immobile young to improve access to important resources, limit detection of concentrated scent by predators, and decrease infestations by ectoparasites can be advantageous. Moving young, however, can also expose them to increased mortality risks (e.g., accidents, getting lost, predation). For group-living animals that live in variable environments and care for young over extended time periods, the influence of biotic factors (e.g., group size, predation risk) and abiotic factors (e.g., temperature and precipitation) on the decision to move young is unknown. We used data from 25 satellite-collared wolves (Canis lupus) in Idaho, Montana, and Yellowstone National Park to evaluate how these factors could influence the decision to move pups during the pup-rearing season. We hypothesized that litter size, the number of adults in a group, and perceived predation risk would positively affect the number of times gray wolves moved pups. We further hypothesized that wolves would move their pups more often when it was hot and dry to ensure sufficient access to water. Contrary to our hypothesis, monthly temperature above the 30-year average was negatively related to the number of times wolves moved their pups. Monthly precipitation above the 30-year average, however, was positively related to the amount of time wolves spent at pup-rearing sites after leaving the natal den. We found little relationship between risk of predation (by grizzly bears, humans, or conspecifics) or group and litter sizes and number of times wolves moved their pups. Our findings suggest that abiotic factors most strongly influence the decision of wolves to move pups, although responses to unpredictable biotic events (e.g., a predator encountering pups) cannot be ruled out. "Home range" is a standard concept in animal ecology and behavior but few people try to understand what home ranges mean to the animals that have them and often assume that a home-range estimate, quantified using some method, is the home range. This leads to 2 problems. First, researchers put much energy into discerning and using the "best" methods for estimating home ranges while no one understands, really, what a home range is. Second, maps delineating home-range estimates may have little connection with what home ranges are and what they mean to the animals that have them. To gain insight into these problems, Roger Powell (hereafter, Roger) documented his own use of space for 65 days, obtaining complete data on where he went, what he did, and how much energy and money he expended and gained in each place. Roger's use of space is consistent with how other mammals use space and, therefore, examination of his data provides insight into what a home range is and how ecologists should approach quantifying other animals' home ranges. We present estimates of Roger's home range in 5 different metrics, or currencies, that provide important and different insights. Home-range estimators that combine different types of information to estimate the spatial distribution and qualities of resources that structure animal behavior (i.e., fitness surfaces) will probably provide the most insight into animals' home ranges. To make reasonable estimates of home ranges, researchers must collect data on habitat, resources, and other attributes of the landscape, so that they can understand basic behaviors of animals and understand how animals may view their environment. Economic models predict behavior of animals based on the presumption that natural selection has shaped behaviors important to an animal's fitness to maximize benefits over costs. Economic analyses have shown that territories of animals are structured by trade-offs between benefits gained from resources and costs of defending them. Intuitively, home ranges should be similarly structured, but trade-offs are difficult to assess because there are no costs of defense, thus economic models of home-range behavior are rare. We present economic models that predict how home ranges can be efficient with respect to spatially distributed resources, discounted for travel costs, under 2 strategies of optimization, resource maximization and area minimization. We show how constraints such as competitors can influence structure of homes ranges through resource depression, ultimately structuring density of animals within a population and their distribution on a landscape. We present simulations based on these models to show how they can be generally predictive of home-range behavior and the mechanisms that structure the spatial distribution of animals. We also show how contiguous home ranges estimated statistically from location data can be misleading for animals that optimize home ranges on landscapes with patchily distributed resources. We conclude with a summary of how we applied our models to nonterritorial black bears (Ursits americanus) living in the mountains of North Carolina, where we found their home ranges were best predicted by an area-minimization strategy constrained by intraspecific competition within a social hierarchy. Economic models can provide strong inference about home-range behavior and the resources that structure home ranges by offering falsifiable, a priori hypotheses that can be tested with field observations. We studied gray wolf (Canis lupus) homesite attendance rates using global positioning system locations of 17 GPS-radiocollared wolves from 7 packs in Idaho. Nonbreeding wolves attended homesites more once pups were weaned and we hypothesize this is a behavior that benefits subsequent pup-rearing. The breeding status and sex of the wolf was the strongest predictor of homesite attendance in the preweaning period but the dominant predictor postweaning was the number of helpers in the pack. We estimated that each additional helper in a pack decreased an individual's attendance rate by 7.5%. Because helpers can either attend or provision pups, our results suggest that small packs invest in protecting pups at the expense of having additional adults foraging. [ABSTRACT FROM AUTHOR]

AZFc deletions of the Y chromosome are the major known genetic cause of spermatogenetic failure. Meiotic studies have shown a prevalence of synaptonemal complex fragmentation and an excess of early-stage sperm cells, suggesting that the maturation block could involve apoptosis. We present a prospective and observational study of apoptotic markers in the sperm of four AZFc-deleted patients and two non-obstructive azoospermic controls without an AZFc deletion. Polycaspases assays and terminal deoxynucleotidyl transferase dUDP nick-end labelling (TUNEL) assays were combined to evaluate the incidence of apoptosis in pre-meiotic, meiotic and post-meiotic germs cells identified, respectively, using anti-melanoma-associated antigen A4 (MAGE-A4), anti-synaptonemal complex protein 3 (SCP3) and anti-sperm acrosome membrane-associated protein 1 (SPACA1) antibodies. We detected apoptosis at all stages of AZFc-deletion spermatogenesis. Using the caspase assay, the incidence of positive cells was found to be heterogeneous for pre-meiotic (from 4.8 to 84.5%) and meiotic stages (from 7.9 to 57.6%), while for post-meiotic cells, the mean incidence was 6% in AZFc-deleted patients compared with 26.5% in controls (P < 0.05). Using the TUNEL assay, the mean percentage with DNA fragmentation for meiotic cells was 54.0% in AZFc-deleted patients compared with 20.3% in controls (P < 0.05), while the percentage of TUNEL-positive post-meiotic cells ranged from 5.3 to 44.7%. Spermatocyte loss in AZFc-deleted patients occurs via the apoptotic pathway. In post-meiotic cells, the lower incidence of apoptosis in testis from three of the four AZFc-deleted patients, compared with controls, is consistent with AZFc deletions having little negative impact on sperm quality. Territoriality in animals is of both theoretical and conservation interest. Animals are territorial when benefits of exclusive access to a limiting resource outweigh costs of maintaining and defending it. The size of territories can be considered a function of ecological factors that affect this benefit-cost ratio. Previous research has shown that territory sizes for wolves (Canis lupus) are largely determined by available biomass of prey, and possibly pack size and density of neighboring wolf packs, but has not been interpreted in a benefit-cost framework. Such a framework is relevant for wolves living in the Northern Rocky Mountains where conflicts with humans increase mortality, thereby potentially increasing costs of being territorial and using prey resources located near humans. We estimated territory sizes for 38 wolf packs in Montana from 2008 to 2009 using 90% adaptive kernels. We then created generalized linear models (GLMs) representing combinations of ecological factors hypothesized to affect the territory sizes of wolf packs. Our top GLM, which had good model fit (R2 = 0.68, P < 0.0005), suggested that territory sizes of wolves in Montana were positively related to terrain ruggedness, lethal controls, and human density and negatively related to number of surrounding packs relative to the size of the territory. We found that the top GLM successfully predicted territory sizes (R2 = 0.53, P < 0.0005) using a jackknife approach. Our study shows that territory sizes of group-living carnivores are influenced by not only intraspecific competition and availability of limiting resources, but also by anthropogenic threats to the group's survival, which could have important consequences where these territorial carnivores come into conflict with humans. BACKGROUND Recurrent AZFb deletions on the human Y chromosome are associated with an absence of ejaculated spermatozoa consequent to a meiotic maturation arrest that prevents the progression of germ cells to haploid stages. The extreme rarity of partial deletions has hampered the identification of the AZFb genes required for normal meiotic stages. The critical interval, refined by two overlapping deletions associated with full spermatogenesis (AZFc and b1/b3), measures over 4 Mb and contains 13 coding genes: CDY2, XKRY, HSFY1, HSFY2, CYORF15A, CYORF15B, KDM5D, EIF1AY, RPS4Y2 and four copies of RBMY. METHODS AND RESULTS We screened 1186 men from infertile couples for Y chromosome deletions, and identified three unrelated oligozoospermic men and one azoospermic man who carry an identical 768 kb deletion resulting in loss of the entire P4 palindrome, including both HSFY genes, the only coding genes within the deletion interval. This 768 kb deletion was not found in 1179 control men. The deletion breakpoints share only 4 bp of nucleotide identity, revealing that the deletions are not recurrent, but are descendants of a founding deletion. Confirming this, we find that all four men carry a Y chromosome of the same highly defined haplogroup (R1b1b1a1b) (incidence 30% in Southern France), although further haplotype analyses showed that they were not closely related. CONCLUSIONS Although the HSFY deletion is restricted to our infertile group, it has been transmitted naturally over many generations, indicating that HSFY genes make only a slight contribution to male fertility. BACKGROUND: Deletion of the entire AZFb interval from the Y chromosome is strictly associated with azoospermia arising from maturation arrest during meiosis. Here, we describe the exceptional case of an oligozoospermic man, 13-1217, with an AZFb + c (P5/distal-P1) deletion. Through the characterization of this patient, and two AZFb (P5/proximal-P1) patients with maturation arrest, we have explored three possible explanations for his exceptionally progressive spermatogenesis. Herpetofauna populations are decreasing worldwide, and the range of wild pigs (Sus scrofa) is expanding. Depredation of threatened reptile and amphibian populations by wild pigs could be substantial. By understanding depredation characteristics and rates, more resources can be directed toward controlling populations of wild pigs coincident with threatened or endangered herpetofauna populations. From April 2005 to March 2006 we used firearms to collect wild pigs (n 5 68) and examined stomach content for reptiles and amphibians. We found 64 individual reptiles and amphibians, composed of 5 different species, that were consumed by wild pigs during an estimated 254 hours of foraging. Primarily arboreal species (e.g., Anolis carolinensis) became more vulnerable to depredation when temperatures were low and they sought thermal shelter. Other species (e.g., Scaphiopus holbrookii) that exhibit mass terrestrial migrations during the breeding season also faced increased vulnerability to depredation by wild pigs. Results suggest that wild pigs are opportunistic consumers that can exploit and potentially have a negative impact on species with particular life-history characteristics. [ABSTRACT FROM AUTHOR]

The Fire and Fire Surrogate study--a replicated, manipulative experiment--sought the most economically and ecologically efficient way to restore the nation's fire-maintained ecosystems. As part of this study, we conducted a 3-year mark--recapture study, comprising 105,000 trap-nights, to assess demographic responses of cotton mice (Peromyscus gossypinus) to Fire and Fire Surrogate treatments at the Gulf Coastal Plain site, where longleaf pine was the ecosystem to be restored. We compared competing models to evaluate restoration effects on variation in apparent survival and recruitment over time, space, and treatment, and incorporated measures of available source habitat for cotton mice with reverse-time modeling to infer immigration from outside the study area. The top-ranked survival model contained only variation over time, but the closely ranked 2nd and 3rd models included variation over space and treatment, respectively. The top 4 recruitment models all included effects for availability of source habitat and treatments. Burning appeared to degrade habitat quality for cotton mice, showing demographic characteristics of a sink, but treatments combining fire with thinning of trees or application of herbicide to the understory appeared to improve habitat quality, possibly creating sources. Bottomland hardwoods outside the study also acted as sources by providing immigrants to experimental units. Models suggested that population dynamics operated over multiple spatial scales. We evaluated selection of den sites by American black bears (Ursus americanus) in the Pisgah Bear Sanctuary, western North Carolina, by comparing characteristics of dens at 53 den sites with availability of habitat characteristics in annual home ranges of bears and in the study area. We also tested whether den-site selection differed by sex, age. and reproductive status of bears. In addition, we evaluated whether the den component of an existing habitat model for black bears predicted where bears would select den sites. We found bears selected den sites far from gravel roads, on steep slopes, and at high elevations relative to what was available in both annual home ranges and in the study area. Den-site selection did not differ by sex or age, but it differed by reproductive status. Adult females with cubs preferred to den in areas that were relatively far from gravel roads, but adult females without cubs did not. The habitat model overestimated the value of areas near gravel roads, underestimated the value of moderately steep areas, and did not include elevation as a predictor variable. We tested the hypothesis that gravel roads, not paved roads, had the largest negative effect on habitat quality for a population of American black bears (Ursus americanus) that lived in a protected area, where vehicle collision was a relatively minimal source of mortality. We also evaluated whether road use by bears differed by sex or age and whether annual variation in hard mast productivity affected the way bears used areas near roads. In addition, we tested previous findings regarding the spatial extent to which roads affected bear behavior negatively. Using summer and fall home ranges for 118 black bears living in the Pisgah Bear Sanctuary in western North Carolina during 1981-2001, we estimated both home-range-scale (2nd-order) and within-home-range scale (3rd-order) selection for areas within 250, 500, 800, and 1,600 m of paved and gravel roads. All bears avoided areas near gravel roads more than they avoided areas near paved roads during summer and fall for 2nd-order selection and during summer for 3rd-order selection. During fall, only adult females avoided areas near gravel roads more than they avoided areas near paved roads for 3rd-order selection. We |found a positive relationship between use of roads by adults and annual variability in hard mast productivity. Overall, bears avoided ureas within 800 m of gravel roads. We estimated survival, fertility, and realized and asymptotic population growth rates from 1981 to 2002 for a protected population of black bears (Ursus americanus) in the southern Appalachian Mountains. We used Akaike's information criterion to assess the time interval for averaging observations that was best for estimating vital rates for our study, given our yearly sample sizes. The temporal symmetry approach allowed us to directly assess population growth and to address all losses and gains to the population by using only capture data, offering an alternative to the logistically intensive collection of reproductive data. Models that averaged survival and fertility across 5- and 7-year time intervals were best supported by our data. Studies of black bear populations with annual sample sizes similar to ours should be of at least 5 years in duration to estimate vital rates reliably, and at least 10 years in duration to evaluate changes in population growth rate (λ). We also hypothesized that survival would not track changes in λ because λ is influenced by both survival and fertility. The 5-year model supported our hypothesis, but the 7-year model did not. Where long-term dynamics of large, relatively stable bear populations are of interest, monitoring survival is likely to be sufficient for evaluating trends in λ. Despite the transition from authoritarian rule to a democratically elected government in 1985, there remains in Brazil a persistent gap between the formal principles and the actual practices of democracy. The gap is particularly manifest in the daily contacts between citizens and representatives of state authority, especially regarding the treatment of Afro-Brazilians. Analyses of the "regulated" and "relational" character of citizenship in Brazil, as well as observations about the attitudinal dispositions of the members of the criminal justice system, suggest that Afro-Brazilians are likely to benefit from fewer protections compared to whites and are more likely to suffer discrimination at the hands of the police. Analyses of the 1988 National Household Survey (PNAD-88) support both hypotheses: Net of statistical controls for key socioeconomic indicators, Afro- Brazilians are more likely than whites to be the victim of assault, and they are more likely to be assaulted by the polices The findings show how the perceptions of class, color, and criminality produce differential protections and treatments inconsistent with the attributes of universal citizenship. Our analysis points more generally to the formidable institutional and cultural challenges that confront the attempt to fully consolidate a democratic regime in Brazil. [ABSTRACT FROM AUTHOR]

Recombination is believed to prevent genetic deterioration in sexual populations because it allows conservation of functional genotypes by removing deleterious mutations. Moreover, evidence that non-recombining segments of a genome deteriorate is provided by genetic experiments in Drosophila and yeast. Y chromosomes generally do not recombine along most of their length, and thus Y chromosome genes, despite having been selectively maintained for their function, could be lost from the genome. Here we present definitive evidence that functional Y genes can be lost from the mammalian genome. TSPY genes must have been selectively maintained on the mammalian Y chromosome since before the radiation of eutheria, 80 million years ago, as they are found conserved on the Y chromosome in two mammalian orders: primate and artiodactyl. We have now identified TSPY on the rodent Y chromosome, in mouse and rat. The gene structure and expression of rat TSPY suggest that it is a functional, testis-specific gene, but the closely related mouse gene, Tspy , has clearly become non-functional, producing only low levels of aberrantly spliced transcripts. Thus TSPY lost its function in the mouse lineage after its divergence from the rat lineage. So, in the case of Tspy at least, the absence of recombination does appear to have led to the loss of a functional gene. [ABSTRACT FROM AUTHOR]

Mucoid strains of Pseudomonas aeruginosa, isolated from patients with cystic fibrosis, were studied for the prevalence of each of the seven Fisher immunotype antigens and were compared with their nonmucoid transformants, obtained by repeated subculturing, for suceptibility to opsonic antibody. Of the 30 strains tested—one from each of 30 patients—16 were typable and were tested in the opsonophagocytic assay with use of immunotype-specific rabbit antiserum; eight had significant opsonization by complement without antiserum. Of the eight strains requiring antiserum, seven strains required a higher minimum concentration of antiserum for a 1.0 log., reduction of viable P. aeruginosa than the paired nonmucoid derivative. These end-point titers were significantly greater for nonmucoid Pseudomonas (P = 0.0007). A mucoid strain not requiring antibody for opsonization was shown to use primarily the alternative complement pathway. These results are consistent with the hypothesis that the immunodeterminant for opsonic antibody in nonmucoid strains is blocked in the mucoid strain. [ABSTRACT FROM PUBLISHER]

Background The risk of Cushing syndrome (CS) patients experiencing a thrombotic event (TE) is significantly higher (odds ratio; OR 18%) than that of the general population. However, there are currently no anticoagulation guidelines. Methods A retrospective, single-center, longitudinal study of patients undergoing all types of treatment—surgical (pituitary, unilateral, and bilateral adrenalectomy) and medical treatment—was undertaken. TEs were recorded at any point up until last patient follow-up; myocardial infarction (MI), deep venous thrombosis (DVT), and pulmonary embolism (PE) or stroke. Patients' doses and complications of anticoagulation were recorded. Results Included were 208 patients; a total of 165 (79.3%) were women, and mean age at presentation was 44 ± 14.7 years. Thirty-nine (18.2%) patients had a TE; extremity DVT (38%), cerebrovascular accident (27%), MI (21%), and PE (14%). Of 56 TEs, 27 (48%) were arterial and 29 (52%) were venous. Patients who underwent bilateral adrenalectomy (BLA) had an odds ratio of 3.74 (95% CI 1.69-8.27) of developing a TE. Of patients with TEs, 40.5% experienced the event within the first 60 days after surgery. Baseline 24-hour urinary free cortisol levels did not differ in patients with or without TE after BLA. Of 197 patients who underwent surgery, 50 (25.38%) received anticoagulation after surgery, with 2% having bleeding complications. Conclusions The risk of TEs in patients with CS was approximately 20%. Many patients had more than 1 event, with higher risk 30 to 60 days postoperatively. The optimal prophylactic anticoagulation duration is unknown, but most likely needs to continue up to 60 days postoperatively, particularly after BLA. [ABSTRACT FROM AUTHOR]

Abstract: Objective: Hypersensitivity to aprotinin is low (1–3%) but more likely with re-exposure. The manufacturer issued a black box warning which lists aprotinin re-exposure within 1 year of prior exposure as a contraindication. We investigated the temporal relationship between re-exposure interval and hypersensitivity in children. Methods: With Human Research Review Board approval, charts of all patients exposed to aprotinin during cardiac surgery were reviewed. We extracted data for re-exposure interval and hypersensitivity to skin tests, intravenous test dosing or infusion of the loading dose. We defined systemic hypersensitivity as haemodynamic instability, respiratory symptoms or diffuse skin reaction temporally related to exposure. Results: From March 1994 to June 2007, there were a total of 2333 aprotinin exposures in 1824 patients. A total of 509 re-exposures occurred in 381 patients: 280 in 244 patients with early (within 1 year) re-exposure and 229 in 222 patients with late (after 1 year) re-exposure. Thirteen systemic hypersensitivity reactions occurred in the 509 re-exposures (2.6%): two during skin testing and 11 during the loading dose. Although the incidence of local hypersensitivity was increased with early re-exposure (6/280 or 2.1% vs 0/229, p =0.019), the incidence of the systemic reaction was not different between early and late re-exposures (6/280 or 2.1% (CI 0.8–4.6%) vs 7/229 or 3.1% (CI 1.2–6.2%), p =0.6). Six patients with a previous hypersensitivity reaction had an additional re-exposure to aprotinin; one of these patients had a systemic reaction during the third exposure. Conclusion: The incidence and type of hypersensitivity to aprotinin re-exposure in our cohort is consistent with previous reports. Repeat exposure within 1 year did not increase the risk of systemic hypersensitivity. [Copyright &y& Elsevier]
