Objectives The South Peace Polypharmacy Reduction Project is a quality improvement project in three communities in rural Canada that aimed to reduce polypharmacy and inappropriate prescribing practices in older adults. This study aims to evaluate the impact of a multifaceted intervention consisting of online team-based medication reviews and educational workshops on the number of chronic medications. Methods A controlled before-after design was used to compare if a decrease in the number of chronic medications was associated with the intervention comprising of online team-based medication reviews and educational workshops, compared with two matched control groups that received either a standard medication review or no medication review. Logistic regression models fit with generalized estimated equations were used to identify the impact of the interventions on decreasing the number of chronic medications. Key findings Following a medication review, the percentage of individuals that had deprescribed at least one medication was highest in the intervention group (52%), followed by the medication review controls at 45%, and 36% in non-medication review controls. Individuals in the intervention group were 20% more likely to have at least one medication deprescribed than individuals in the medication review control group (adjusted odds ratio: 1.20; 95% CI: 1.03 to 1.39), whereas they were 42% more likely to deprescribe at least one medication compared with non-medication review controls (adjusted odds ratio: 1.42; 95% CI: 1.25 to 1.61). Conclusions Online team-based medication reviews had a significant impact on decreasing the number of chronic medications in older adults. Furthermore, providing healthcare providers with education can complement the role of other healthcare interventions. [ABSTRACT FROM AUTHOR]

Objective Use of specific medications may accelerate the progression of radiographic knee OA (RKOA). Our aim was to examine the effect of medication use on the progression of RKOA. Methods We used longitudinal data from the Osteoarthritis Initiative (OAI), an observational study of risk factors for knee OA. At baseline, we selected participants with RKOA (Kellgren–Lawrence grade ≥2) and excluded those with a history of knee-related injury/surgery and other musculoskeletal disorders. Current medication use (use/non-use in the previous 30 days) and radiographic medial minimum joint space width (mJSW) data were available at baseline and annually up to 96 months follow-up. We used random effects, panel regression to assess the association between current medication use (non-users as reference group) and change in mJSW. Results Of 2054 eligible participants, 2003 participants with baseline mJSW data were included [55.7% female, mean age 63.3 (s. d. 8.98) years]. Of seven medication classes, at baseline NSAIDs were the most frequently used analgesia (14.7%), anti-histamine (10.4%) use was frequent and the following comorbidity medications were used most frequently: statins (27.4%), anti-hypertensives (up to 15.0%), anti-depressant/anxiolytics/psychotropics (14.0%), osteoporosis-related medication (10.9%) and diabetes-related medication (6.9%). Compared with current non-users, current use of NSAIDs was associated with a loss of mJSW (b  = −0.042, 95% CI −0.08, −0.0004). No other associations were observed. Conclusions In current users of NSAIDs, mJSW loss was increased compared with current non-users in participants with RKOA. Clinical trials are required to assess the potential disease-modifying effects of these medications. [ABSTRACT FROM AUTHOR]

Objectives Synovitis in symptomatic knee OA (KOA) is common and is associated with joint symptoms. Optimal synovial measurement on MRI is, however, unclear. Our aims were to examine the relationship between MRI measures of synovitis and knee symptoms in symptomatic KOA. Methods Data from a randomized, multicentre, placebo-controlled trial (UK-VIDEO) of vitamin-D therapy in symptomatic KOA were utilized. Participants reported knee symptoms using WOMAC at baseline and annually. On contrast-enhanced (CE) MRI, synovial thickness was measured using established, semi-quantitative methods whilst synovial tissue volume (STV) was assessed as absolute STV (aSTV) and relative to the width of femoral condyle (rSTV). STV of the infrapatellar region was also assessed. Associations between synovial measures and symptoms were analysed using multiple linear regression modelling. Results No linear association was observed between knee symptoms and synovitis thickness scores. Whole-joint aSTV (0.88, 95% CI: 0.17, 1.59) and infrapatellar aSTV (5.96, 95% CI: 1.22, 10.7) were positively associated with knee pain. Whole-joint rSTV had a stronger association with pain (7.96, 95% CI: 2.60, 13.33) and total scores (5.63, 95% CI: 0.32, 10.94). Even stronger associations were found for infrapatellar rSTV with pain (55.47, 95% CI: 19.99, 90.96), function (38.59, 95% CI: 2.1, 75.07) and total scores (41.64, 95% CI: 6.56, 76.72). Conclusions Whole-joint and site-specific infrapatellar STV measures on CE-MRI were associated with knee pain, respectively. Volumes relative to the size of the femoral condyle may be promising outcome measures in KOA trials. [ABSTRACT FROM AUTHOR]

Objective Pulsed electromagnetic field (PEMF) therapy is a potentially useful treatment for osteoarthritis (OA), but its effectiveness is still controversial. This study aimed to examine the effects of PEMF therapy and PEMF parameters on symptoms and quality of life (QOL) in patients with OA. Methods Cochrane Central Register of Controlled Trials, PubMed, CINAHL, EMBASE, PEDro, clinical trial registers, and reference lists were searched until April 2019. This study examined randomized, placebo-controlled trials, patients with OA, symptom and/or QOL related outcomes, and articles published in English. Two authors extracted data and completed quality assessment. Results Sixteen studies were included in our systematic review, while 15 studies with complete data were included in the meta-analysis. Our primary outcome was the standardized mean difference, which was equal to the treatment effect in the PEMF group minus the treatment effect in the placebo group divided by the pooled standard deviation. For pain, the standardized mean difference was 1.06 (95% CI = 0.61 to 1.51), for stiffness 0.37 (95% CI = 0.07 to 0.67), for function 0.46 (95% CI = 0.14 to 0.78), and for QOL 1.49 (95% CI = −0.06 to 3.04). PEMF parameters did not influence symptoms. Conclusions Compared with placebo, there was a beneficial effect of PEMF therapy on pain, stiffness, and physical function in patients with OA. Duration of treatment may not be a critical factor in pain management. Further studies are required to confirm the effects of PEMF therapy on QOL. Impact Our study suggests that PEMF therapy has clinically significant effects on pain in patients with OA. The current evidence was limited to the short-term effects of PEMF therapy. Bark beetle-induced mortality in forested landscapes of structurally uniform, even-aged lodgepole pine stands has inspired a growing interest in the potential of silvicultural treatments to enhance resilience by increasing spatial and vertical complexity. Silvicultural treatments can simulate mixed-severity disturbances that create multiaged lodgepole pine stands, which, along with heterogeneous forest landscapes, can play a role in mitigating susceptibility to primary disturbance agents (bark beetles and wildfire). With this article, we review multiaged lodgepole pine stand dynamics and discuss variable-retention harvesting as a silvicultural option for lodgepole pine. We describe the establishment and initial outcomes of an experimental variable-retention harvesting project established at the Tenderfoot Creek Experimental Forest (Montana) in 1999 -2003 and the objectives of a collaborative multiagency effort that is presently revisiting and analyzing that experiment. Objective Electronic health records possess critical predictive information for machine-learning-based diagnostic aids. However, many traditional machine learning methods fail to simultaneously integrate textual data into the prediction process because of its high dimensionality. In this paper, we present a supervised method using Laplacian Eigenmaps to enable existing machine learning methods to estimate both lowdimensional representations of textual data and accurate predictors based on these low-dimensional representations at the same time. Materials and methods We present a supervised Laplacian Eigenmap method to enhance predictive models by embedding textual predictors into a lowdimensional latent space, which preserves the local similarities among textual data in high-dimensional space. The proposed implementation performs alternating optimization using gradient descent. For the evaluation, we applied our method to over 2000 patient records from a large single-center pediatric cardiology practice to predict if patients were diagnosed with cardiac disease. In our experiments, we consider relatively short textual descriptions because of data availability. We compared our method with latent semantic indexing, latent Dirichlet allocation, and local Fisher discriminant analysis. The results were assessed using four metrics: the area under the receiver operating characteristic curve (AUC), Matthews correlation coefficient (MCC), specificity, and sensitivity. Results and discussion The results indicate that supervised Laplacian Eigenmaps was the highest performing method in our study, achieving 0.782 and 0.374 for AUC and MCC, respectively. Supervised Laplacian Eigenmaps showed an increase of 8.16% in AUC and 20.6% in MCC over the baseline that excluded textual data and a 2.69% and 5.35% increase in AUC and MCC, respectively, over unsupervised Laplacian Eigenmaps. Conclusions As a solution, we present a supervised Laplacian Eigenmap method to embed textual predictors into a low-dimensional Euclidean space. This method allows many existing machine learning predictors to effectively and efficiently capture the potential of textua predictors, especially those based on short texts. [ABSTRACT FROM AUTHOR]
