Purpose: To investigate the psychologic morbidities associated with tooth loss despite treatment with technically successful dentures., Materials and Methods: This cross-sectional questionnaire-based study compared the psychologic disturbance and functional difficulties in two groups: individuals with tooth loss who wore optimal-quality removable dentures (test group), and individuals with tooth loss who did not wear dentures (control group). The questionnaire used was developed and validated previously. The short-form revised Eysenck personality questionnaire was also used to assess the relationship with personality traits. A total of 138 participants were recruited (denture group = 70; control group = 68)., Results: There was a significant difference in body image dissatisfaction between the groups (χ 2 = 7.72, P value = .005). The denture group had 5.75-times higher probability than the control group of suffering from body image disturbance. Older patients were predicted to have 75% less probability of body image disturbance (OR = 0.25), and men were predicted to have up to 70% less disturbance (OR = 0.3). As for psychologic morbidities, participants in both groups presented with somatic symptoms related to depression or anxiety, which were nearly double that expected in the general population (15.7% and 7.8%, respectively). Furthermore, participants who complained about body image impairment were more likely to have higher scores on the neuroticism scale (OR = 3.64)., Conclusion: Tooth loss and dentures could be associated with body image dissatisfaction and psychologic morbidity. Planning for patient-centered care is paramount prior to extracting any teeth and providing replacement options.

Patients presenting both severe maxillary atrophy and dental malposition require a multidisciplinary treatment approach to achieve optimal esthetic and functional results. This case history report demonstrates how digital treatment planning and teeth set-up can serve as a reference for surgical, orthodontic, and prosthodontic procedures, leading to an all-ceramic full-arch implant-supported fixed prosthesis.

Purpose: The partial removable dental prosthesis (PRDP) is used widely to replace missing teeth, but it can disturb the ecology of the mouth. In this retrospective cohort study, the null hypothesis was that there was no difference between tooth loss and the type of edentulous spaces in participants wearing PRDPs., Materials and Methods: Participants were selected from patients who were treated consecutively with PRDPs between January 2003 and December 2006 by undergraduate dental students at the University of British Columbia and attended the clinic for at least 5 years following placement of the PRDP. Clinical records were analyzed for 50 men and 52 women aged 19 to 85 years at baseline, before placement of the PRDP (mean age: 63.2 years). Baseline data were obtained from the records on missing teeth and edentulous spaces, and thereafter data were collected on teeth lost over the following 5-year period. Baseline data on age, sex, occlusal contacts, number of dental rests for the dentures, carious teeth, gingival attachment loss, and teeth with endodontic fillings were also obtained., Results: The presence at baseline of teeth with endodontic fillings significantly (P < .05) increased the prediction of tooth loss over the 5 years, but the type of edentulous space did not., Conclusion: The risk of tooth loss over a 5-year period was higher in the presence of endodontically treated teeth, but no higher in participants wearing PRDPs with distally extended edentulous spaces rather than tooth-bounded edentulous spaces.

Purpose: The aim of this long-term cohort study was to evaluate the efficacy and complications of fixed partial dentures in a convenience sample of 100 patients with periodontal disease who were treated and maintained periodontal patients after 20 years., Materials and Methods: After active treatment, including periodontal surgery and endodontic and prosthetic treatment, patients were enrolled in a supportive periodontal care (SPC) program with 3- to 6-month recalls. All patients showed clinical data recorded at (1) the original consultation (T0), (2) the first SPC visit following the completion of prosthetic treatment (T1), and (3) at the latest SPC clinical session 20 years after T1 (T2). Multivariate analyses were performed to investigate the influence of clinical variables on the risk of prosthetic abutment (PA) loss after 20 years' visits., Results: The final sample comprised 100 patients. At T1, a total of 948 PAs represented the original sample of experimental teeth. At the 20-year follow-up, a total of 854 PAs (90.1%) were still in function, while 94 (9.9%) PAs in 41 patients (41%) were lost during SPC; 98% of lost PA were endodontically treated. Vertical root fracture (48%) was the major cause of PA loss, while progression of periodontitis caused 31% of PA loss. Age (P = .002), Full-Mouth Plaque Score (P < .0001), Full-Mouth Bleeding Score (P = .0002), and oral parafunctions (P = .0083) were associated with increased probability of PA failure. Among clinical-related factors, endodontic treatment (P = .0082), root resection/ amputation (P < .0001), multi-rooted teeth (P = .0005), and abutment associated with parafunction (P < .0001) were associated with increased risk of abutment loss after 20 years., Conclusions: Perioprosthetic treatment in compliant patients is highly successful after 20 years of SPC.

Purpose: This study aimed to investigate the impact of preradiation tooth loss in patients with head and neck cancer., Materials and Methods: Records of 397 (partially) dentate patients who were referred for preradiation oral screening were included. Number and location of teeth lost and occluding pairs lost were determined for different tumor locations., Results: The majority of patients (54%) were affected by tooth loss. Proportion of teeth lost, their location, and proportion of occluding pairs lost were not evenly distributed across tumor locations. The highest proportions of teeth were removed with oral tumors (maxilla: 25%; mandible: 47%). For preradiation preventive extractions only, ie, not taking into account teeth that were lost due to ablative surgery, tooth loss in the mandible was still not evenly distributed across tumor locations, but tooth loss in the maxilla and occluding pairs lost were., Conclusions: Tumor location affects preradiation tooth loss, though this is primarily a consequence of ablative surgery rather than a consequence of preradiation dental extraction decisions. Since patients with oral cavity tumors are affected most by preradiation tooth loss, treatment planning with regard to functional rehabilitation is desirable for this patient group in particular.

The aim of this study was to investigate the loss of teeth in positions mesially adjacent to implant-supported fixed dentures (IFDs) after insertion of IFDs in unilateral free-end edentulous spaces in the mandible at dental clinics. There were a total of 157 adjacent teeth. Nine adjacent teeth (5.73%, 9 of 157), 9 opposing teeth (2.59%, 9 of 348), 38 posterior teeth (1.93%, 38 of 1,964), and 3 anterior teeth (0.22%, 3 of 1,380) were lost during the observation period. The percentage of lost adjacent teeth was higher than that of posterior teeth (P = .002). Teeth in positions adjacent to the IFDs require more attention than other remaining teeth.

Purpose: To evaluate gingival margin changes in the esthetic zone after immediate implant placement and provisionalization over 5 years with a custom anatomic provisional abutment., Materials and Methods: Single maxillary incisor immediate implant placement and provisionalization procedures, completed and followed between February 2006 and August 2006, were analyzed retrospectively. During clinical recalls at 3 months, 1 year, and 5 years, changes in gingival margins were recorded with clinical photographs and recorded in millimeters., Results: Forty-seven patients each received a single implant (19 central incisors, 28 lateral incisors) and were followed for 5 years. Implants and restorations had a 100% survival rate during the study. The mean gingival margin changes (recession) were 0.17 mm at the time of definitive restoration, 0.27 mm at 3 months, 0.30 mm at 1 year, and 0.30 mm at 5 years. After 5 years, 24 of the 47 implant crowns had no significant gingival recession. All central incisor sites received 4.3-mm-diameter implants and had a mean change in tooth length of 0.03 mm at 5 years. Lateral incisor sites (n=28) received either a 3.5-mm-diameter implant (n=20) or a 4.3-mm-diameter implant (n=8). Lateral sites with the 3.5-mm implant had a mean change in gingival margin height of 0.08 mm of tooth length at 5 years; lateral sites with a 4.3-mm platform displayed a mean change of 0.82 mm of tooth length., Conclusion: This study suggests that implant diameter, gingival biotype, surgical technique, and/or the reason for tooth loss can influence the amount of gingival recession occurring over 5 years. Most recession occurred within the first 3 months, between implant placement/provisionalization and definitive restoration. The use of a customized anatomic provisional abutment can reduce the amount and frequency of recession.

Purpose: To compare the complication rate of natural teeth adjacent to implant supported dentures (IFDs) with that of teeth serving as abutments for fixed partial dentures (FPDs). The second goal was to assess the risk factors for complications in teeth adjacent to bounded edentulous spaces., Materials and Methods: The study subjects were selected from patients who received prosthodontic treatment for their bounded edentulous space not exceeding two missing teeth between February 1990 and March 2007. Sixty-one patients were included in the IFD group and 66 patients were included in the FPD group. Tooth complications were defined as tooth extraction, periodontal lesion, periapical lesion, and loss of prosthesis and were assessed by one examiner based on dental records., Results: The 8-year cumulative complication rate for the IFD group (7.9%) was significantly lower than for the FPD group (40.7%). Additionally, the 8-year cumulative complication rate of vital teeth (6%) was significantly lower than that of nonvital teeth (45.9%). A cox proportional hazard analysis revealed that nonvitality of dental pulp was a significant risk factor for tooth complications, whereas treatment modality was not., Conclusions: Teeth adjacent to IFD-treated edentulous spaces presented fewer complications than natural teeth serving as abutments for FPDs. Conservation of teeth adjacent to edentulous spaces as vital teeth was the key finding to limit further tooth loss.

A healthy Caucasian male lost all of his teeth because of aggressive cervical, apical, and lateral root resorption over a period of 11 years. Microscopically, there was no sign of reparative cementum apposition, but cell-rich fibrous connective tissue filling the deep resorptions was seen. The resorbed teeth were surrounded by fibro-osseous tissue of osteolytic appearance, as noted during radiologic examination. Radiography showed the osteolytic processes to regress dramatically with the removal or loss of teeth. No underlying systemic or local factor was identified. In spite of endodontic and surgical treatment, none of the resorptions went into remission.

In a German dental practice, 299 patients were treated with 1011 ceramic fillings between June 1987 and October 1990. The restorations were produced according to the Cerec method and in each case inserted adhesively at the same appointment. The clinical success was continuously observed and documented. The period in place was between 15 and 18 years. Event criteria were loss of the tooth or of the restoration, fracture with partial loss of tooth and/or restoration, marginal caries, marginal repair and endodontic complication. The probability of success according to the Kaplan-Meier method was reduced after 16.7 years to a value of 84.4%. No further event was observed up to the final time of 18.3 years. If "restoration still in place" is made the basis for the survival analysis, ie, therapeutic measures such as trepanation and marginal correction with composite remain unconsidered, then the Kaplan-Meier value for the observation period is 89%. The size of the filling had no influence on the success prognosis. Premolars achieved a better result than molars. Vital teeth prove to be better than non-vital teeth. The use of dentin adhesives increased the probability of success. A total of 121 events, 86 of these losses of restoration, occurred in the observation interval. The most frequent causes were ceramic and tooth fractures.

Aim: Significant delay in tooth eruption may be observed in many orthodontic patients. This delay may lead to complications, such as devitalization, ankylosis, external root resorption, injury to the adjacent teeth, and mucogingival problems. These complications may result in prolonged treatment time, esthetic deformities, damage to the periodontal tissues, and tooth loss. This report investigates the special considerations needed in treating such cases., Methods: Combined orthodontic and surgical treatment can be used in managing teeth exhibiting significant delay in eruption., Results: A high rate of success can be achieved in terms of esthetics, occlusion, and periodontal health., Conclusion: Nontraumatic surgery, adequate control of gingival inflammation, and use of minimal orthodontic forces may ensure a higher percentage of success and prevention of problems associated with teeth exhibiting significant delay in eruption.

Purpose: This study assessed decayed/missing/filled teeth (DMFT), presence of occlusal units, and prevalence of shortened dental arches in a Tanzanian adult population., Materials and Methods: The dental state of samples of the Tanzanian population was studied. Oral examinations were conducted on 5,532 adults from rural and urban cluster samples. DMFT was related to age, gender, and residence. The pattern of tooth loss within dental arches was mapped, and the prevalence of shortened dental arches was estimated., Results: Mean DMFT scores increased gradually from 1.8 (20- to 29-year-olds) to 3.8 (50- to 59-year-olds). A steep increase was observed in the > or = 60-year-olds (DMFT 8.1). "Missing" was the dominant component of DMFT. The mean number of present teeth ranged from 27 in the youngest to 20 in the oldest age group. Under 60 years of age, DMFT was significantly higher for women than for men. No differences were found between residence categories. Molars were more frequently decayed, missing, and mobile than premolars. Of all subjects, 41% had complete dental arches, 44% had interruptions (of these, 73% were in posterior regions only), and 15% had shortened dental arches; 0.5% were edentulous. Of the subjects with shortened dental arches, about 65% had at minimum three pairs of occluding premolars., Conclusion: An initial low DMFT rate increased after the age of 60 years. Molars had the highest risk of dental decay and were most frequently absent. Shortened dental arches develop as a consequence of the pattern of tooth decay and tooth loss, although interruptions were frequently seen in the posterior regions.

The goal of this study was to provide data relative to the use of a new implant system (Mac System, Cabon, Milan, Italy) with a Morse taper implant-abutment connection for single implant restorations. The implant system is composed of an endosseous screw made of commercially pure titanium grade 2, while the abutment is titanium alloy (Ti-6Al-4V). A total of 80 single implants were placed in 69 patients (36 women and 33 men, mean age 42 years, range 16 to 61). All patients gave their informed consent and received a thorough clinical and radiographic examination. Smokers and diabetics were excluded from the study. Three implants were placed in areas of previous tooth impaction, 5 were placed in posttraumatic edentulous areas, 2 were used in situations involving tooth agenesis, and 60 replaced teeth lost because of caries or periodontal disease. All patients were edentulous for at least 1 year prior to treatment. The implants received a definitive prosthesis and had been in function for a mean period of 3.5 years. At second-stage surgery, 2 implants were removed because of lack of osseointegration. After 2 years of loading, 1 implant showed evidence of peri-implantitis and was removed. In addition, 2 fractured abutments and 1 loosened abutment were observed. Few mechanical or infectious complications were seen, and this may have been the result of high stability of the conical connection.

The longevity of teeth depends directly on the state of the periodontal tissues. Many etiologic factors can lead to the loss of a tooth. Tooth loss is frequently associated with bone resorption. The diagnosis of a condition and knowledge of its etiology are essential to assess the prognosis of the remaining teeth and to formulate the correct treatment plan. Many parameters must be investigated to ascertain an accurate diagnosis. An understanding of the patient's needs and the length and likely success of treatment guides the decision of whether to preserve teeth or extract them and place implants. Advanced periodontitis poses a major therapeutic dilemma. Judicious, strategic extractions may permit the placement of long implants in ideal positions.

Purpose: This article describes the concept of the Marburg double crown system (MDC system) in the treatment of partially edentulous patients. Long-term success is assessed by a review of patient records., Materials and Methods: Double crowns with clearance fit are used to retain tooth-, mucosa-, and implant-supported removable partial dentures (RPD). To achieve retention, an additional attachment, the TC-SNAP system, is used. All metal components are fabricated in a single cobalt-chromium-molybdenum alloy; the framework (including outer crowns) is cast in one piece. Because of the framework's rigidity, the RPD can be constructed without major and minor connectors. The denture base adjacent to the abutments is fabricated using a perioprotective design that is similar to fixed partial dentures. One hundred eleven dentures, of which 49 (44%) were fabricated for patients with intraoral defects, were evaluated by reviewing patient records., Results: The mean age of the patients at the time of insertion was 57.5 years (+/- 12.3). The mean number of double crowns per denture was 3.5 +/- 2.1 (range, 1 to 9). The probability that a patient will have lost all abutment teeth 10 years after insertion of the denture is 4%. The probability that a patient will have kept all teeth that abut the denture 5 years after insertion is 87%; 10 years after insertion the probability is 80%., Conclusion: The Marburg double crown system is a versatile and successful means of achieving the long-term restoration of the partially edentulous jaw. Insertion and removal of the denture and routine oral hygiene are easy to perform, even for patients with limited manual dexterity. As a full-arch reconstruction, the MDC system enables easy adjustment, modification, and relining with low follow-up costs.

Osteoporosis has been suggested to contribute to the edentulous condition. However, studies specifically designed to examine the relationship between tooth loss and osteoporosis are scarce, and reported results have been mixed. This study investigated the correlation between tooth loss and spinal bone density in a nonHispanic white female population. Forty-four subjects were recruited and divided into high and low spinal bone density groups based on whether the bone density was higher or lower than the age-adjusted population mean bone density. Missing teeth and periodontal status in these groups were assessed and analyzed. Tooth loss was not significantly different between subjects with low spinal bone density (4.90 +/- 0.89 teeth) and high spinal bone density (3.81 +/- 0.90 teeth). Tooth loss did not differ after age and periodontal adjustment. However, the loss of attachment levels (relative to the cementoenamel junction) was greater in the low spinal bone density subjects (3.42 +/- 0.30 mm) than in the high spinal bone density subjects (2.37 +/- 0.26 mm) (P < .05). The difference may have resulted from gingival recession (P < .05), because the gingival pocket depths were not significantly different between the two groups. In this study, total tooth loss was not directly associated with systemic bone density. However, periodontal disease was negatively associated with spinal bone density. Whether periodontal disease will ultimately lead to greater tooth loss in low spinal bone density group merits further investigation.

Patients receiving artificial crowns and fixed partial dentures at a prosthodontic clinic were examined 18 to 23 years after treatment. The long-term results were good, especially for the metal ceramic restorations: 79% remained unchanged and only 3% had been removed. The success rate for the gold-resin restorations was less favorable: 43% remained unchanged and 33% had been removed. The use of one cantilever extension unit had not negatively influenced the results for the fixed partial dentures. Generally, those abutment teeth that had been removed during the observation period had received previous endodontic treatment before restorations had been placed. Endodontic complications on abutment teeth were more frequent in the mandible than in the maxillae.
