Surgical simulation has become a commonly utilized and well-researched training adjunct in nearly all surgical specialties. Balancing high-quality orthopaedic surgical training in the face of work hour restrictions and efficiency pressures has become a challenge to educators and trainees alike. Surgical simulation is an opportunity to enhance such training and potentially permit trainees to be better equipped for the operating room. In orthopaedics, various low-fidelity, high-fidelity, and virtual reality simulation platforms are readily available to almost all trainees and permit simulation of a wide array of arthroscopic surgeries. In this review, we seek to highlight the potential utility of simulation-based training in orthopaedic surgery, the various types of available simulators, and review the evidence for simulator use. INTRODUCTION: The purpose of this study was to compare the hospital course and disposition of COVID-19 positive versus negative patients following an operatively managed hip fracture. MATERIALS AND METHODS: This retrospective cohort study evaluated patients presenting to a university medical center with a hip fracture who underwent surgical management between February 1, 2020 and April 1, 2021. COVID-19 diagnosis was obtained using PCR testing. Hospital length of stay, disposition, readmission, and mortality were compared between patients with and without COVID-19. RESULTS: 399 total patients were identified who met inclusion criteria, with 14 patients who were COVID-positive (3.1%). There was a 6.1 day increase in length of hospital stay for COVID-19 positive patients compared to those who were COVID negative (p = 0.002), without significant changes in disposition, readmission rates, or mortality. INTRODUCTION: The purpose of this study was to evaluate the accuracy of visual joint angle assessments by orthopaedic surgery trainees amongst various levels of training. METHODS: Sagittal plane photographs of several joints at various angles were distributed to trainees within an orthopaedic residency program. Joint angles were estimated and compared to those obtained with a goniometer. Inter-and intra-rater reliability and ANOVA were conducted to assess differences between groups. RESULTS: Twenty trainees were studied. The percent error for knee measurements differed at 23.1%, 26.2% and 11.1% for the PGY 2-3, PGY 4-5 and PGY 6 groups, respectively (P=0.024). Percentage error for ankles showed the greatest variability at 69.7-96.3%. Intra-rat-er reliabilities for all visual joint angle assessment were similar amongst groups. CONCLUSIONS: Visual joint angle assessments vary amongst trainees, with PGY 6s most accurately identifying knee joint angles. Visual assessment is inaccurate compared to goniometric measurements, thus limiting visual measurements during patient encounters. [ABSTRACT FROM AUTHOR]

Osteoporosis and fragility fractures (FFs) are closely intertwined as the former is a common predisposing factor to the latter. This causal relationship is due to low bone density of osteoporosis and compromised bone microarchitecture, leading to structural failure, decreased ability to withstand applied stresses, and increased propensity to fracture. Osteoporosis can be idiopathic or due to a variety of secondary causes, and numerous treatment strategies are available. FFs are common injuries among the elderly and are caused by factors both intrinsic and extrinsic to the patient. The clinical and economic significances of osteoporosis and FFs are substantial, with considerable associated morbidity and mortality, and billions spent on healthcare expenditures in the US annually. Osteoporosis and FFs are two of the most important topics related to fracture liaison services (FLSs), and their understanding is integral to appreciating the benefits an FLS can provide for patients and providers. The article presents a review of nonsurgical, surgical, and alternative treatment approaches for osteoarthritic (OA) pain. Topics discussed include traditional treatments for painful OA, including nonsurgical and procedural or surgical options, as well as nontraditional methods; controlled studies demonstrated that pain was correlated with joint space narrowing and other structural abnormalities; and nerve growth factor (NGF), which promotes axon growth and survival of peripheral neurons,.

Introduction: Femoral neck fractures in the young patient require prompt anatomic reduction and stabilization to preserve the vascular supply to the femoral head and minimize future need for arthroplasty. Secondary to unique biomechanical and vascular considerations, these injuries are prone to nonunion. Case Report: A 29-year-old male with a chronic femoral neck fracture nonunion who experienced successful fracture healing and symptom resolution following Bone Marrow Aspirate Concentrate (BMAC) administration. Discussion: Femoral neck nonunion in young patients is a challenging problem with treatment strategies aimed at improving the biological and biomechanical fracture environment. While the use of both vascularized and nonvascularized bone grafting has shown promising results, they have high complication rates and substantial donor site morbidity. BMAC has demonstrated multiple uses throughout orthopedic surgery and may result in an improved fracture healing environment with minimal patient morbidity. Conclusion: The success of the BMAC procedure for this patient is promising and may be considered in similar patients, with or without revision internal fixation methods. [ABSTRACT FROM AUTHOR]

Introduction: The prevalence of amputation and post-amputation pain (PAP) is rising. There are two main types of PAP: residual limb pain (RLP) and phantom limb pain (PLP), with an estimated 95% of people with amputations experiencing one or both. Medical Management: The majority of chronic PAP is due to phantom limb pain, which is neurogenic in nature. Common medications used include tricyclic antidepressants, gabapentin, and opioids. Newer studies are evaluating alternative drugs such as ketamine and local anesthetics. Rehabilitation Management: Mirror visual feedback and cognitive behavioral therapy are often effective adjunct therapies and have minimal adverse effects. Surgical Management: Neuromodulatory treatment and surgery for neuromas have been found to help select patients with PAP. Conclusion: PAP is a complex condition with mechanisms that can be located at the residual limb, spinal cord, and brain -- or a combination. This complex pain can be difficult to treat.
