This article explores the impact of the 'hostile environment' on racialised migrant women's experiences of pregnancy and childbirth in England, arguing that the 'hostile environment' functions as a technology of 'stratified reproduction.' First coined by Shellee Colen, the concept of stratified reproduction describes the dynamic by which some individuals and groups may be supported in their reproductive activities, while others are disempowered and discouraged. This paper locates the stratified reproduction produced by the 'hostile environment' as intertwined with wider gendered and racialised discourses around British citizenship which have been 'designed to fail' racialised residents of the UK. Drawing on interviews with racialised migrant mothers in the north of England, this paper analyses how the proliferation and intensification of immigration controls interacts with gender, race, class, and other social regimes to differentially allocate the resources necessary for a safe and healthy pregnancy and childbirth, and how this is experienced materially by pregnant migrants. This article draws on Incident Reporting System data from the National Offender Management Service over a ten-year period (2004–2014) and limited, small-scale interviews with four custodial managers. Pat Carlen's work (2008) on imaginary penalities provides the theoretical framework for an assessment of the reporting, recording and initial response to sexual assaults in prisons in England and Wales. The article argues that the recording of sexual assaults became part of a response to new management systems that emphasised compliance, process and audit rather than realising safety in custody. Although the data shows substantial levels of initial activity among staff it is, in essence, practice without prospect. The article suggests that outcomes generally for sexual assaults in prisons in England and Wales are uncertain. Incident reporting has become a bureaucratic process 'or paper shadow', which Goffman described as showing 'what has been done by whom, what is to be done, and who last had responsibility for it' (Goffman 1961: 73). The Scottish Law Commission has recently confirmed in its Eleventh Programme of Reform, published in May 2023, the continuation of its medium-term project to examine the law of homicide. This includes a review of sexual infidelity killings and provocation as a partial defence to murder, or otherwise. It embraces a critique of whether and, if so, how any necessary proposals for modernising the law in this sensitive and important area should be made. A Discussion Paper has previously been published for examination. Against this backdrop, we analyse whether sexual infidelity ought to be retained in extant Scots law, as one of only two relationally relevant qualifying triggers. In November 2023, the Law Commission in England and Wales announced its forthcoming review of domestic homicide given contemporary understandings of the impact of domestic abuse. We provide novel insights into Anglo-Scottish law to seek to recalibrate the understanding of coercive/controlling behaviour in the contextualisation of sexual infidelity killings. Our review highlights the ways in which allegations of (actual or perceived) sexual infidelity may be symptomatic of coercive control through consideration of recent case law, and via the lens of Monckton-Smith's eight stages to femicide. A risk assessment is utilised to demonstrate the extent to which victims of coercive controlling behaviour have their capacity for action delimited, and how this should inform deliberations in cases where the coercively controlled individual kills in response. A novel and original new template is promulgated for provocation reform predicated de novo on qualifying triggers of domestic abuse and gross breach of trust and with a part-justificatory – part excusatory standardisation. It is suggested that a new nomenclature and typology is required as extant Scottish law on provocation is out of step with contemporary social mores, and the law in England and Wales does not adequately address contextual factors, such as, ethnicity, culture, religion, sexuality, disability, and migrant status, which impact on a domestic abuse victim's capacity for action. It is further propounded that a social entrapment lens, drawing on initiatives crystallised from New Zealand and Australian academician perspectives, ought to be enacted as part of urgently needed effective remedial legislation. The 21st century has seen the rise of a new phenomenon – the creation of statues and monuments celebrating the lives of entertainers. Drawing on debates about popular culture, placemaking and heritage, and in the context of recent controversies about the politics of statues and memorials, this paper examines a manifestation of this phenomenon as represented by statues of comedians erected in the towns of Northern England. The paper begins by sketching the characteristics of the statues and their subjects. It proceeds by reflecting on their emergence in the context of debates about the contested place of nostalgia in social and cultural theory. The paper uses a strategically selected sub-set of the statues to reflect on the emerging cultural and political imaginary of towns as sites of contemporary political struggles. It concludes by asserting that the significant affective investment into statues projects and the progressive potential of popular cultural nostalgia offer an important corrective to crude assumptions about the cultural lives of towns. The project on which this paper is grounded is the first in-depth empirical study of the impacts of COVID-19 on each stage of the English and Welsh Youth Justice System. We take the notion of a child's right to a fair trial as the lens by which we detail the findings from our research. The paper documents the experiences of professionals working in the courts and children who had contact with the courts during the pandemic. While we concentrate on processes in England and Wales as an exemplar of the impact of COVID-19, recognising that globally, courts were experiencing similar challenges, initiates a discourse about how to re-envision their role in wider criminal justice systems in a COVID-19 world. Our research demonstrates an urgent need for renewed consideration of what support children need to effectively participate in court, and where and how children's cases should be heard. The pandemic demonstrated that creativity is possible and creates a timely opportunity to review the evidence and think more radically about a welfare-based, trauma-informed court process for children. Background: In recent years, a significant proportion of inpatient facilities for people with intellectual disabilities and/or autism has been de-commissioned in England, This has resulted in individuals with intellectual disabilities being sent to distant hospitals far away from their families and carers leading to challenges in follow-up, community care and interventions. The impact of de-institutionalisation, has often caused patient trauma, family distress and subsequent discharge difficulties. Not every individual with intellectual disabilities and/or autism requires inpatient care but inpatient care when needed has to be local, adequate and appropriate. Aims: To evaluate current evidence of utility of inpatient models for people with intellectual disabilities and outline best clinical practice. Method: PubMed, CINAHL, EMBASE, Cochrane Library, Scopus, Web of Science were searched with key search terms. The search was conducted by the information specialist and identified abstracts screened further for inclusion criteria, methodological issues, and other appropriate characteristics. Twenty-three papers were included in the rapid review. Papers shortlisted had the inclusion criteria applied against the full text version independently by two reviewers. Disagreements regarding eligibility of studies was resolved by discussion and consensus within the project team. Key data related to in-patient models of care was extracted from the included papers, which included year of study, design, study objectives, target population, method/s tested, outcomes reported, country of study/studies, and results. Data extraction was performed by two reviewers and reviewed by the project team. Results: From the review of services for people with intellectual disabilities, we came across four broad models/frameworks/approaches. Evidence on what worked for inpatient service provision tended to be based on models developed and implemented locally. Conclusions: We make recommendations for the best clinical practice and standards. The onset of prehistoric farming brought unprecedented changes to landscapes and their biodiversity. Past biodiversity patterns are broadly understood for different parts of Europe, and demonstrate trajectories that have been linked to prehistoric and historic demographic transitions, and associated land-use practices. To our knowledge, this paper is the first attempt to directly link evidence of agricultural practice from the archaeological record to biodiversity patterns. Records of fossil pollen are used to estimate plant and landscape diversity patterns, and novel approaches are employed to analyse 1194 harmonised archaeobotanical samples (plant macrofossil remains) spanning the prehistoric and Roman periods, from southern England. We demonstrate changes in the use of crops and gathered edible plants and non-linear trends in cultivation practices. Whilst, overall, cereal production is characterised by ever larger and extensive regimes, different trajectories are evident for most of early prehistory, the Middle Iron Age and the Late Roman period. Comparisons with the Shannon diversity of fossil pollen records from the same region suggest a positive relationship between developing agricultural regimes and landscape scale biodiversity during the prehistoric period. The Roman period represents a tipping point in the relationship between expanding agriculture and pollen diversity, with declining pollen diversity evident in the records from the region. Background: The Health of the Nation Outcomes Scales for people with Learning Disabilities (HoNOS-LD) is an 18-item measure which provides a structured and standardized approach to rating various clinical and psychosocial outcomes and has been in use nationally since 2002. Aims: To revise and improve the HoNOS-LD's utility in contemporary intellectual disability (ID) services whilst retaining its original objectives and five-point severity ratings. Method: ID clinicians were invited to complete an online survey, rating each item on the existing measure for being fit for purpose, identifying issues and suggesting improvements based on their experience of using the HoNOS-LD in practice. Scales were then assessed and revised sequentially; survey responses were used to inform discussion and revisions to the HoNOS-LD by the Advisory Board. Results: A total of 75 individuals replied. Respondents had used HoNOS-LD for an average of 8.0 years (S.D. 5.28 years) and 88% found the scale to be useful in their practice. On average, respondents used HoNOS-LD ratings to inform care 42.4% of the time (S.D. 33.5%). For each scale there was a significant negative correlation between the percentage of positive/very positive respondent ratings and the number of changes proposed. Common changes included simplifying terms, reducing ambiguity and replacing anachronistic language. Conclusion: The changes outlined in this paper are based on the advisory group's expert consensus. Radicalisation has become a highly influential idea in British policy making. It underpins and justifies Prevent, a core part of the UK's counter-terrorism strategy. Experts have theorised the radicalisation process, often beset by a weak evidence base and mired in fundamental contestation on definitions and explanatory factors. Experiential experts have been active contributors to these debates, presenting a challenge to the low-ranking role often given to experiential knowledge in evidence hierarchies and a contrast to policy areas in which it remains poorly valued. This paper draws on interviews with radicalisation experts to examine the dynamics of this pluralisation in practice. With a focus on credibility contests, it explains how experiential experts can claim authoritative knowledge and the challenges they face from those who prioritise theory-driven empirical data as the basis for contributions to knowledge. Age is an important known driver of residential sorting, yet little is understood about how age segregation is affected by housing unaffordability. This relationship is particularly pertinent given trends of increasing housing inequalities and population ageing, in Europe and elsewhere. Using harmonised population data for small areas linked with local house price statistics and household incomes in England and Wales, this paper examines the scale of, and links between, residential age segregation and housing unaffordability. The results reveal a strong association between increasing housing unaffordability (for sales and rentals) and increasing residential age segregation (beyond other local characteristics). This association is particularly marked in urban and rich (least deprived) areas. This points to increasing spatial polarisation along the intersections of wealth and age: not only are the wealthiest parts of the country, where housing is particularly unaffordable, becoming increasingly demarcated socio-economically but also by age. This implies that age-related life course processes are integral to the trends observed more broadly of increasing socio-spatial polarisation. [ABSTRACT FROM AUTHOR]

Introduction: Chronic Pain is increasingly affecting young people, their quality of life and wellbeing including education, social life and mental health. Current Western approaches to psychological support for Chronic Pain often focus on 'pain management' and learning skills and strategies from professionals, making use of Cognitive Behavioural Therapy (CBT), Acceptance and Commitment Therapy (ACT) and psychoeducation approaches in individual and group interventions. As a Paediatric Psychology service, we have trialled groups running over multiple weeks informed by these approaches and experienced low attendance rates. Method: This paper describes a feasibility study of an alternative group intervention; the 'Living with Pain group', which combines the 'Journey of Life' (Denborough, 2014) with collective narrative and liberation psychology approaches. Results: Thirty three young people and eight parents / carers attended the intervention. Attendance rates, quantitative and qualitative feedback from one face-to-face group for young people and parents and two online groups for young people showed increased access and acceptability of the group. Conclusion: The findings from this study have shown that a 1-day collective narrative group intervention is feasible, suitable and acceptable and has increased access to our group intervention. Future directions include co-creation with young people and families, community engagement to increase accessibility of our groups and developing partnerships to respond to young people's hopes to raise awareness. In this commentary, I examine the role of institutional investors in affordable housing production in England, reflecting and expanding on the papers in this special issue on the governance of residential investment. Drawing on my research on the politics of municipal debt and local authority housebuilding in London, I provide a snapshot of the key regulatory changes that have enabled insurance companies, pension funds, and other institutional investors to extract profits from social and affordable housing. I also explore the politics and relations of power that underpin this transformed environment through a discussion of investors' lobbying activities, and through an analysis of 'income strip deals', long-term leasing agreements between investors and local authorities that have gained popularity in recent years. In line with the authors in this issue, I argue that to grasp the recent wave of institutional investment in public housing, we need to pay attention to the narrative framings through which the promise of patient capital is enacted and legitimised, and to the range of regulatory actions mobilised to support and maintain the flow of value to rentiers. This paper explores the neglected topic of the organization and use of household space for work (paid and unpaid) during the early modern period. It engages critically with the concept of 'the open house' in relation to recent arguments about processes of spatial and social closure of houses and households in the early modern period associated with the development of a capitalist socio-economic system. Drawing on a variety of sources including diaries, autobiographies, inventories and court records the analysis considers how work was organized in the domestic context and how experience varied according to gender, age, status and location as well as time of day, week or year. The paper argues that a perceived dichotomy between an open house and a closed home secluded from the social and business world of community and commerce is inadequate in the consideration of space and attitudes in this period. Individuals in larger houses could control access to certain spaces at specific times and work could be concentrated in certain rooms. But in practice in most households the complicated demands of the household economy required openness to outsiders that rendered the boundaries of houses more permeable and potentially more unstable than they are today. The analysis considers the case of England and compares rural and urban homes in a particular regional context, but it opens up several ways for historians to think about the relationship between work and home in other European regions. This paper investigates intonation in the urban dialect of Liverpool, Scouse. Scouse is reported to be part of a group of dialects in the north of the UK where rising contours in declaratives are a traditional aspect of the dialect. This intonation is typologically unusual and has not been the subject of detailed previous research. Here, we present such an analysis in comparison with Manchester, a city less than 40 miles from Liverpool but with a noticeably different prosody. Our analysis confirms reports that rising contours are the most common realization for declaratives in Liverpool, specifically a low rise where final high pitch is not reached until the end of the phrase. Secondly, we consider the origin of declarative rises in Scouse with reference to the literature on new dialect formation. Our demographic analysis and review of previous work on relevant dialects suggests that declarative rises were not the majority variant when Scouse was formed but may have been adopted for facilitating communication in a diverse new community. We highlight this contribution of intonational data to research on phonological aspects of new dialect formation, which have largely considered segmental phonology or timing previously. E. P. Thompson's classic article "Time and Work-Discipline in Industrial Capitalism," gives an incomplete picture of the transition to the time consciousness in industrial capitalism. This is for two reasons. First, by not understanding time logics of pre-industrial societies and viewing such logics as "irregular," Thompson was unable to understand how wages were paid, and workers disciplined in a culture that used seasonally variable temporal hours in pre-industrial England. Second, with regard to industrialism, Thompson did not recognize the effects on labor of credit and a huge capital influx from the sugar trade on the emergence of manufacturing in England in the 18th century. By documenting the issues of changing ways of relating time to wage, seasonality, place, and finance, this paper argues that the increased year-round availability of capital combined with the alienation of workers from the seasonality of time in specific locations drove the shift from daywork to clock-measured wages during the Industrial Revolution, and that the adoption of clock time was a response to confusion over how day wages could be made uniform over different latitudes and during different times of year. This paper examines the relationship between religious diversity, religious and national identity, and neighbourhood trust. Using data from 6,089 individuals in England matched to census-based statistical estimates for 300 local areas, we find that religious diversity is negatively associated with neighbourhood trust. Further analyses tested indirect relationships between religious diversity and neighbourhood trust to examine whether higher levels of religious diversity are associated with a stronger sense of religious identity, and whether a stronger sense of religious identity is associated with lower levels of neighbourhood trust. We simultaneously tested whether higher levels of religious diversity are associated with a weaker sense of national identity, and whether a weaker sense of national identity is associated with lower neighbourhood trust. Multigroup analyses were conducted to assess patterns across religious groups. Results indicate that, for Christians, religious diversity is associated with a stronger subordinate religious identification, which is in turn associated with lower neighbourhood trust. There were no associations between religious diversity, national identification, and neighbourhood trust. For the other religious groups, no significant associations were found between our variables of interest. This paper addresses the complex issue of the embodiment of grief. It explores how a theoretical shift to the body has influenced scholarly literature about grief and bereavement. Despite this shift, we argue that bodily interpretations and experiences are undertheorised in western psychological literature on bereavement. Specifically, we argue that linear stage models of grief have encouraged the view that grief needs 'working through' in the mind, and not necessarily the body. We draw on empirical data from interviews with bereaved people undertaken in England to illustrate aspects of the embodied experience of grief that differ from how psychological grief theories conceive of the bereaved person's body. Findings highlight the role of the bereaved person's body in managing grief and how the absence and continuing presence of the deceased person is managed through embodied practices. We conclude that understanding grief as an embodied experience can enable the development of grief theories that better capture the complex negotiation between the psychological processes of grief and the materiality of bodies. Traditionally, jurists have distinguished between voluntary/involuntary behaviour via the theory of volition. Though perceived as the conventional approach, this paper argues that the volitional understanding of voluntariness is an inadequate instrument for assessing complex behaviours which seemingly portray a striking level of intelligence and purposiveness on the part of the accused. In particular, the phenomenon known as hypnotically-induced behaviour, which forms the focus of this paper, is one such troublesome case. To this end, the version of the volitional theory most staunchly advocated by Professor Michael Moore is singled out for scrutiny, due to his strong sentiments supporting the application of his philosophy to these aforementioned behaviours. In contrast to Moore, this paper suggests that the position most recently proposed by the Law Commission of England and Wales within their discussion paper on the defences of insanity and automatism is to be preferred. Specifically, the Commission recommend substituting the theory of volition for that of 'control' as a means for assessing the voluntariness of any given behaviour. This paper submits that a theory of control has two major advantages over the traditional theory of volitionalism. First, the possession/absence of control more accurately reflects the contemporary system of criminal law in England and Wales. Second, a theory of control is more conceptually defensible as an explanation for why behaviours performed under hypnosis are typically perceived as involuntary. [ABSTRACT FROM AUTHOR]

Occupation orders are the dedicated legal remedy through which victims of domestic abuse can be supported to remain in the family home following a relationship breakdown. Case law indicates, however, that victims experience barriers to securing orders due to the high threshold criteria and because concerns about protecting the rights of perpetrators has led to judicial reluctance to grant extensive protection to victims. The options for providing protection to victims of abuse in respect of the family home are shortly set to be reformed by the Domestic Abuse Act 2021, which creates a new Domestic Abuse Protection Order (DAPO). It is anticipated that DAPOs will be easier to secure because they will have a lower threshold criteria, they will be available in family, civil and criminal proceedings, and both victims and third parties will be able to make an application thereby alleviating the burden on victims who feel unable to take any action. Whilst there is no intention at this point to repeal occupation orders, the Home Office has acknowledged that 'DAPOs will become the 'go to' protective order in cases of domestic abuse' suggesting that occupation orders will be replaced by DAPOs in most cases. By drawing on data obtained from an analysis of court statistics, a questionnaire of legal practitioners and domestic abuse specialists, and in-depth interviews with victims of domestic abuse, this paper offers original empirical insights into where the current law fails victims of domestic abuse. The analysis reveals three key barriers to securing occupation orders. Firstly, despite the Legal Aid, Sentencing and Punishment of Offenders Act 2012 making efforts to preserve legal aid for victims of domestic abuse, the means test is difficult for victims to satisfy, resulting in increases both to the number of victims taking no action to pursue protection and who act as litigants in person in occupation order proceedings. Secondly, the prospects of a victim securing protection can be adversely affected by their unrepresented status. Thirdly, despite case law indicating a less restrictive approach to granting occupation orders, many victims continue to struggle to satisfy the strict threshold criteria. Some judges are seemingly willing to bypass this by granting alternative remedies which may offer victims a weaker form of protection in respect of the family home. Where orders are granted, the data suggest this is on restricted terms and for limited durations which reduce their effectiveness at preventing post-separation abuse and supporting victims to regulate their short and longer-term housing situation. These empirical findings are then situated within a discussion of the Domestic Abuse Act 2021. The authors analyse whether forthcoming DAPOs are likely to offer a more accessible and effective form of protection than occupation orders. The analysis suggests that by increasing the scope of applicants, the breadth and flexibility of available protection and the sanctions for breach, DAPOs have the potential to remedy many of the existing barriers to securing protection over the family home. As is always the case with new legislation however, the key will be in its implementation, to ensure that existing issues are not simply transferred across to the new regime. The findings are novel because academic commentaries on protective injunctions typically focus on 'personal protection' offered by non-molestation orders, domestic violence protection orders, and restraining orders, meaning that both occupation orders and protection for victims in respect of the family home are under-researched areas of domestic abuse. It has been well established at a global level that sex workers are often victims of direct violence in the course of their work, targeted by their 'perceived vulnerability' as a marginalised group. In one police force in England (Merseyside) since 2006 they have addressed this victimisation through adopting a 'hate crime' approach to policing crimes against sex workers. The aims of this paper are first, to review the implementation of the hate crime model applied to crimes against sex workers; second, to explore how police forces are adopting the policy across the UK, and the operational barriers to doing so; and third to explore the legal, theoretical and critical issues raised by treating crimes against sex workers as a type of hate crime through policing models. The conclusion weighs up some of the challenges to expanding the current law to include sex workers as a target group for increased protection, assessing that a fully funded policing approach may well be sufficient alongside law reform around decriminalisation of sex work. This article will draw on data from police forces in England. This paper focuses on the development, delivery and preliminary impact analysis of an engineering Work Experience Week (WEW) programme for KS4 students in the School of Civil, Aerospace and Mechanical Engineering (CAME) at the University of Bristol, UK. Key stage 4, is the legal term for the two years of school education which incorporate GCSEs in England, age 15–16. The programme aims to promote the engineering profession among secondary school pupils. During the WEW, participants worked as engineering researchers: working in teams, they had to tackle a challenging engineering design problem. The experience included hands-on activities and the use of state-of-the-art rapid prototyping and advanced testing equipment. The students were supervised by a group of team leaders, a diverse group of undergraduate and postgraduate engineering students, technical staff, and academics at the School of CAME. The vision of the WEW programme is to transmit the message that everybody can be an engineer, that there are plenty of different routes into engineering that can be taken depending on pupils' strengths and interests and that there are a vast amount of different engineering careers and challenges to be tackled by the engineers of the future. This paper develops an understanding of the structural and spatial characteristics of regulated forms of temporary use across England's core cities. The paper's contribution lies in its adoption of an extensive research design that goes beyond the intensive qualitative approaches that predominate in the temporary use literature. We employ a novel data set of 5890 temporary use interventions that have been recorded over a 15-year period (2000–2015). Informed by the temporary use literature, we distinguish between 'extraordinary' (e.g. urban beaches) and 'ordinary' (e.g. car parks) forms of temporary use alongside other characteristics that include the time of occurrence, the function of space appropriated, decisions taken and whether instances were isolated or reoccurring. Logistic regression is used to test whether the odds that a temporary use was defined as 'ordinary' or 'extraordinary' increased or decreased owing to their underlying structural characteristics. The analysis revealed that applications for extraordinary temporary uses increased in the period following the 2007/2008 financial crisis but that ordinary forms of temporary uses remained much more common before and after the recession. It also revealed differences between ordinary and extraordinary uses in relation to the functions of the spaces appropriated and decisions taken by the planning authority in processing the application. Geospatial approaches were then applied to two case study cities – Bristol and Liverpool. The analysis revealed a tendency towards the clustering of temporary uses that was spatially and temporally uneven, with extraordinary uses in particular concentrated in the cores/downtowns of the two cities. Drawing on the politicised history of Public Conveniences in England since the 19th century, this paper traces the socio-political motives for their provision and for their gradual withdrawal in recent decades. It discusses the effects these developments have had on public mobility, and the socio-political complexity these infrastructures pose to city-making agendas. In particular, the essay highlights the notions of stigma associated with these spaces in relation to gender, body-politics and control, which led to a lack of political interest in their provision and a pattern of closures that began in the Thatcher era and has continued through later times of economic austerity. To unfold these arguments, the essay examines a series of initiatives put forward to reclaim for public use a derelict toilet in the centre of London: from the concept of an interactive site-specific intervention to raise awareness of its closure, to a campaign for its listing as an Asset of Community Value, to contest its privatisation. This case study is used to address the spatial stigma that public toilets carry as a contested locus of public sanitation and, furthermore, to highlight important questions surrounding their provision in the context of contemporary citizen-driven urban agendas. To articulate this argument, the case study exemplifies how critical spatial practices can operate as a form of pedagogical urban praxis for awareness-raising and citizen engagement, advancing a Lefebvrian 'right to the city' against hegemonic neoliberal agendas. This paper discusses how intersecting identities, stigma and health-based infrastructures are spatially affiliated and territorialised in the South East of England through the findings of three research projects aimed at understanding health inequalities among urban Black, Asian and Ethnic Minorities including Gypsies and Travellers (BAME and GT) groups. It problematises Wacquant's approach to territorial stigma by explaining how Butler's notion of vulnerability and Castoriadis' notion of autonomous agency help to expand our understanding of the interplay between stigma and health infrastructures. Moreover, it suggests that such interplay requires an intersectional approach to identity as performative and embodied practice using illustrative examples. We propose that these health settings and infrastructures can be characterised as 'transbordering assemblages', following Irazábal who describes its embedded notions of pluri-locality (here and there: '[T]Here'), pluri-identity and practices of bordering (being in or out/in and out/in between) when experiencing health needs. Objective: It is increasingly recognized that patient safety requires heterogeneous insights from a range of stakeholders, yet incident reporting systems in health care still primarily rely on staff perspectives. This paper examines the potential of combining insights from patient complaints and staff incident reports for a more comprehensive understanding of the causes and severity of harm. Methods: Using five years of patient complaints and staff incident reporting data at a large multi-site hospital in London (in the United Kingdom), this study conducted retrospective patient-level data linkage to identify overlapping reports. Using a combination of quantitative coding and in-depth qualitative analysis, we then compared level of harm reported, identified descriptions of adjacent events missed by the other party and examined combined narratives of mutually identified events. Results: Incidents where complaints and incident reports overlapped (n = 446, reported in 7.6%' of all complaints and 0.6% of all incident reports) represented a small but critical area of investigation, with significantly higher rates of Serious Incidents and severe harm. Linked complaints described greater harm from safety incidents in 60% of cases, reported many surrounding safety events missed by staff (n = 582), and provided contesting stories of why problems occurred in 46% cases, and complementary accounts in 26% cases. Conclusions: This study demonstrates the value of using patient complaints to supplement, test, and challenge staff reports, including to provide greater insight on the many potential factors that may give rise to unsafe care. Accordingly, we propose that a more holistic analysis of critical safety incidents can be achieved through combining heterogeneous data from different viewpoints, such as through the integration of patient complaints and staff incident reporting data. European countries have developed a range of long-term care (LTC) policy responses to support the increasing share of older people. However, little is known about the effectiveness of LTC services and benefits, particularly their impact on older peoples' quality of life (QoL). This paper investigates the role of personal, care service and environmental characteristics on the effects of home care services on QoL across England, Finland and Austria. We used data from surveys conducted in England, Finland and Austria. In total, 811 older adults were included in the analysis. OLS regression including main effects and country-specific interactions was used to explore variation in gains in long-term care service-related quality-of-life (LTC-QoL). Explanatory variables were derived from the production of welfare framework and comprised home care service user socio-demographics, needs indicators, social support and environmental variables and characteristics of home care service provision. In all three countries, LTC-QoL gains increased with needs, indicating that home care services perform well, with additional gains declining the higher the needs. Also, better process quality contributed to LTC-QoL improvements in all three countries. In addition, the availability of informal care, social contact, financial household situation and living alone, were associated with changes in LTC-QoL only in one or two of the countries. Home care services increased service users' QoL in all three European countries. The increase in QoL, however, varied across the countries. This paper examines the use of narrative verdicts in the coronial system in England and Wales to record findings in cases of death after police contact. It uses a dataset of 68 verdicts into such cases in the period 2004-2015. The paper considers how regulation is constructed in a way that makes complex cases comprehensible through narrative. The construction of these narratives is affected by legal structures, institutional structures, but also the structures imposed by narrative convention. The paper argues that the relationships between these structures affect what type of narrative is constructed in the aftermath of a death after police contact. Mentally vulnerable defendants who struggle to effectively participate in their trial in the magistrates' courts are not receiving the same protection as those who stand trial in the Crown Court. The Law Commission for England and Wales recognised this lacuna and suggested that the law relating to effective participation should be equally applicable in the magistrates' courts. On closer examination of the law, the legal aid system and perspectives of legal professionals on the 'front line', it is clear that improvements in policy are of greater importance than legal reform and are more likely to meet the needs of these vulnerable individuals. The aim of this paper will be to demonstrate that reform of the law will be insufficient to adequately protect mentally vulnerable defendants in the magistrates' courts and that changes in policy are needed in place of, or alongside, legal reforms. Objectives: To explore the extent to which national policy in end of life care in England influences and guides local practice, to ensure that care for patients over the age 75 years is of a consistently good quality. Method: This paper reports on phase one of a larger study and focuses its discussion on the high-level (macro) determinants emerging from the analysis. Fifteen in-depth interviews were conducted with professionals involved in the development of English policy in end of life care. Results: Factors influencing the quality of end of life care were stratified into three system levels: meso, macro and micro. English national policy was reported to be an important macro-level determinant of effective outcomes, and examples were provided to demonstrate how policy was influencing practice. Yet, the complexity of the area and the range of interacting contributory factors mean the value of policy alone is hard to assess. At the macro-level, concern was voiced around: whether policy was effective in tackling rising inequity; lack of mandatory leverage to exert change relating to end of life outcomes; the impact of ongoing infrastructural change on statutory services; workforce pressures; over-reliance on acute services and continued abdication of responsibility for end of life care to medical professionals supported by the continued dominance of the medical model of care. Conclusions: The links between the existence of policy at the macro-level of the system and the effective enactment of good practice remain unclear, although strategies are suggested to help achieve greater national consistency in end of life care outcomes. Policymakers must pay attention to the following: controlling the rise in localism and its contribution to regional inequalities; the impact of continuous infrastructural change together with increasing workforce pressures; encouraging broader professional and public responsibility for recognition and care of those at the end of life. Specialization serves an important purpose in health services, ensuring resources are used efficiently and patients can access specialized skills and interventions. However, specialization also results in services being concentrated in fewer locations, with less patient choice. Focussing upon the example of gender-identity services for trans people in England, this paper outlines contemporary debates regarding hormone prescription pathways and argues that concepts of 'specialization' in health care may at times disadvantage minority populations who have needs that are uncommon but not clinically complex. Supplying gender-identity services in specialized clinics has sometimes been presented as avoiding discrimination that may occur in wider health services, but may reinforce perceptions that other health providers do not need to engage with trans populations. Primary care-led models for providing trans health care operate internationally and are being explored in the UK. The paper reports a study that examines how to determine if a road traffic collision took place in daylight or in the dark. An innovative method was developed, based on solar altitude, to establish cut-off points of daylight and darkness determined from a study of daylight availability in England, Scotland and Wales. This approach provides a rigorous method to differentiate daytime and night-time collisions. The criteria were used in a study of the collisions reported in the STATS19 data set for the weeks either side of the clock changes that are necessary between Greenwich Mean Time and British Summer Time. By comparing periods with the same clock time either side of the time change, using the aforementioned method, it was possible to isolate collisions within the same time period that during one week occurred in darkness and in the other week in daylight. Objectives: The paper reports on an empirical study of Health and Wellbeing Boards (HWBs) in England. Established by the Health and Social Care Act 2012, HWBs act as place-based hubs for leaders in health, social care, local government and other sectors to come together to address health improvement and the wider determinants of health. Methods: We conducted a three-year study of HWBs (2015–2017) in five localities across England. This involved collecting qualitative data from semi-structured interviews with key actors in the HWBs at strategic and operational levels, and focus group sessions with voluntary-sector participants at each HWB. Results: HWBs have largely followed the path of previous partnerships in terms of a lack of clear aims and objectives, lack of ownership and accountability by partners, and an absence of any significant impact on health outcomes. Conclusions: Many of the features of unsuccessful partnership working were largely displayed by HWBs. Objectives: The introduction of medical revalidation in 2012 has been a controversial and radical change to medical regulation in the UK. It involved changes to the way organizations manage medical performance, and to the relationships between doctors, their employers and the professional regulatory body. In this paper, we explore the implementation of medical revalidation, analysing the change process and its consequences for doctors and organizations. Methods: We conducted a qualitative investigation of the implementation of revalidation in 15 case study organizations in 2016–2017, collecting documents and undertaking a total of 80 interviews with medical and non-medical staff. We used Normalization Process Theory to frame and structure the analysis. Results: Revalidation reforms were largely implemented successfully within and across our case study organizations, with evidence of growing acceptance of the purpose and processes of revalidation. There was an emergent shift from securing doctors' compliance towards the use of revalidation to strengthen clinical governance, and towards evaluating revalidation processes and seeking to make them more effective. However, there was substantial variation in the implementation and impact of revalidation; it was still not fully understood by many doctors, and revalidation processes were highly reliant on a few key individuals in each organization. The changes brought about by revalidation have had consequences for the way in which doctors construct their identity and the way they relate to the organizations in which they work. Conclusion: Despite considerable early scepticism and overt opposition in the medical profession, revalidation has become gradually accepted, embedded and even valued over time. Its impact and effectiveness are still questioned by many stakeholders, and the focus of attention has now shifted towards revising and improving the way revalidation works in practice. Objective: This paper aims to explore the nature of clinical commissioning groups (CCGs) in England as membership organizations. Utilizing the concept of meta-organization as a lens, we discuss the impact that this organizational form might have on CCGs' ability to become 'strategic commissioners'. Methods: We used a longitudinal qualitative approach to explore the adoption and implementation of primary care co-commissioning. The study was undertaken between May 2015 and June 2017 and included interviews with senior policy makers, analysis of policy documents, two telephone surveys, and case studies in four CCGs nationally. Results: CCGs operate as membership organizations with closed boundary and low stratification, whereby a consensus or majority needs to be reached by members when activities impact on membership or the CCG's constitution. While CCGs should move towards a more strategic commissioning role that is focused on local priorities agreed by their members, they are faced with a complex system of accountabilities and responsibilities, which makes this difficult to achieve. Conclusions: The nature of CCGs as membership-based meta-organizations has the potential to both help and hinder CCGs in becoming strategic commissioners. The complexities in accountability and governance that the membership approach introduces, and the potential difficulties that CCGs face with competing meta-organizations, raises questions about the future of CCGs as membership organizations. The Australian Association of Social Workers has campaigned for many years for the registration of social workers in Australia, but with little success to date. This paper critically examines the key arguments for and against registration with reference to academic and government policy literature from international jurisdictions (primarily New Zealand and England) where government regulation of social work already exists. This review of existing literature found that arguments in favour of registration broadly centre on two main assumptions. They are improved protection of clients and amplification of professional standing. The findings indicate that there is only limited empirical evidence as to whether registration does in fact lift the professional standing of social workers or protect consumers from substandard practice. We conclude that further research is required to examine whether registration of social workers is justified in Australia, and particularly to ascertain whether the regulation of social workers generally or in specific practice areas such as child protection can realistically take place independently of the large number of human service practitioners who are not social work trained. Background: Globally, smoking remains a significant issue for mental health populations. Many mental health trusts in England are facing challenges of implementing the National Institute for Health and Care Excellence guidance according to which all mental health settings, no matter the type, should be entirely smoke-free and provide comprehensive smoking cessation support. Aim: The aim of this paper was to determine if unit type and unit manager smoking status influence mental health smoke-free policy implementation. Method: This paper reports on the secondary analysis of data from a cross-sectional survey of 147 mental health inpatient settings in England, in 2010. The original study’s main aim was to understand unit managers’ perceived reasons for success or failure of smoke-free policy. Results: Unit managers (n = 131) held a positive stance towards supporting smoke-free policy and most perceived that the policy was successful. Non-smoker unit managers were more likely to adopt complete bans than smoker unit managers, whereas smoker unit managers were more likely than non-smoker unit managers to think that stopping smoking aggravated patients’ mental illness. Smoking rates for staff and patients remain high, as perceived by unit managers, regardless of unit type. Proportion of units offering nicotine replacement therapy and peer support to patients was significantly higher in locked units compared to semi-locked or residential rehabilitation. Applied strategies significantly vary by type of unit, whereas unit managers’ knowledge, attitude and practices vary by their smoking status. Discussion: There are nuanced differences in how smoke-free policy is enacted which vary by unit type. These variations recognise the differing contexts of care provision in different types of units serving different patient groups. Addressing staff smoking rates, promoting consistency of staff response to patients’ smoking and providing staff education and support continue to be key strategies to successful smoke-free policy. Conclusions: Our results demonstrate the importance of taking into account the type of unit and acuity of patients when enacting smoke-free policy and addressing staff smoking. The aim of this paper is to reflect critically on the current health promotion initiatives targeting overweight individuals in Western countries. The paper’s methodological approach is to draw on analytical findings from my and other sociologists’ empirical work on how the problems of overweight people are being defined in various settings in Denmark, England, Australia and the US. I try to illustrate how health promotion targeting overweight individuals can not only be seen as a project aimed at securing longer lives and fewer illnesses for people carrying excess fat but also a moral project that, in a more general sense, aims to tell people how they ought to live their lives. I link this moral aspect of health promotion to a) the medicalization tendency in current Western society (e.g. a growing pharmaceutical industry and its economic interest in transforming the human condition of being overweight into a treatable disorder) and b) the strong focus on individual risk today. One of the main arguments in the paper is that health in relation to overweight is primarily defined from a biomedical perspective that praises certain physical measurements of the body, as well as dominant societal values such as  self-responsibility and self-control, and that a combination of biomedicine and these dominating values can lead to health promotion becoming a problematic moral endeavour. The paper considers the notion that innovation in territorial governance is associated with a set of core neoliberal ideas about local economic development which have come to constitute a new and pervasive consensus. Through a case study of attempts to construct city-regional institutions in Manchester, England, over a period of 25 years, it attempts to track the themes that have underpinned the development of a local politics of economic development. The paper considers the extent to which evolution of city-regional institutions and policy accord to wider ideas about post-political forms of governance and the erosion of democracy in cities. Objectives: Calls for major reconfigurations of health services have been accompanied by recommendations that wide ranging stakeholders be involved. In particular, patients and the wider public are seen as critical contributors as both funders and beneficiaries of public health care. But public involvement is fraught with challenges, and little research has focused on involvement in the health service transformation initiatives. This paper examines the design and function of public involvement in reconfiguration of health services within the English NHS. Methods: Qualitative data including interviews, observation and documents were collected in two health service 'transformation' programmes; interviews include involved public and professional participants. Data were analysed using parallel deductive and inductive approaches. Results: Public involvement in the programmes was extensive but its terms of reference, and the individuals involved, were restricted by policy pressures and programme objectives. The degree to which participants descriptively or substantively represented the wider public was limited; participants sought to 'speak for' this public but their views on what was 'acceptable' and likely to influence decision-making led them to constrain their contributions. Conclusions: Public involvement in two major service reconfiguration programmes in England was seen as important and functional, and could not be characterized as tokenistic. Yet involvement in these programmes fell short of normative ideals, and could inadvertently reduce, rather than enlarge, public influence on health service reconfiguration decisions. Objectives: The Health and Social Care Act 2012 introduced Clinical Commissioning Groups to take responsibility for commissioning (i.e. planning and purchasing) the majority of services for local populations in the English NHS. Constituted as 'membership organizations', with membership compulsory for all GP practices, Clinical Commissioning Groups are overseen by, and are accountable to, a new arm's-length body, NHS England. This paper critically engages with the content and policy narrative of the 2012 Act and explores this in relation to the reality of local policy enactment. Methods: Set against a careful review of the 2012 Act, a case study of a nascent Clinical Commissioning Groups was conducted. The research included observations of Clinical Commissioning Group meetings and events (87 h), and indepth interviews (16) with clinical commissioners, GPs, and managers. Results: The 2012 Act was presented as part of a broader government agenda of decentralization and localism. Clinical Commissioning Group membership organizations were framed as a means of better meeting the needs and preferences of local patients and realizing a desirable increase in localism. The policy delineated the 'governing body' and 'the membership', with the former elected from/by the latter to oversee the organization. 'The membership' was duty bound to be 'good', engaged members and to represent their patients' interests. Fieldwork with Notchcroft Clinical Commissioning Group revealed that Clinical Commissioning Groups' statutory duty to NHS England to 'ensure the continuous improvement' of GP practice members involved performance scrutiny of GP practices. These governance processes were carried out by a varied cast of individuals, many of whom did not fit into the binary categorization of membership and governing body constructed in the policy. A concept, the governing assemblage, was developed to describe the dynamic cast of people involved in shaping the work and direction of the Clinical Commissioning Group, many of whom were unelected and of uncertain status. This was of particular significance in Notchcroft Clinical Commissioning Group because the organization explicitly pursued a governance system based on developing positions of consensus. The governing assemblage concept is valuable in articulating the actual practices of Clinical Commissioning Group governance, how these relate to the normative content of the 2012 Act, and the tensions that emerge. Conclusions: The governing assemblage concept provided clarity in discussion of the dynamics of organizational governance in Notchcroft Clinical Commissioning Group, which did not follow the simple template articulated in the 2012 Act. The concept merits application in the study of other Clinical Commissioning Groups and may prove valuable in illuminating governance processes within a range of other health care organizations in different contexts. The governing assemblage holds promise for the analysis of ongoing changes to NHS organization, as well as international health care organizations such as accountable care organizations in the US. The proliferation of statutory and common law powers that can be utilised to manage and control public protest gives the police a wide range of options to in any given scenario and requires a careful balance to be struck between the maintenance of order and facilitation of convention rights. This paper discusses a novel use of the criminal law of conspiracy and considers the potential benefits of this approach and the ramifications for the protection of convention rights. It is submitted that the controversial use of the criminal law against Chinese dissidents in the UK was perhaps a result of the law of unintended consequences arising from the development of a body of law that has been piecemeal and reactive. Despite an emerging literature on guerrilla gardening as a political practice in public spaces, with few exceptions, these accounts theorise it as working against many corporate and bureaucratic forms of power. Using the example of ‘F Troop’—a group of gardeners operating on a site in an English midland city—this paper focuses on the practices of urban guerrilla gardening in order to illustrate that these are perhaps not as ‘resistant’ or ‘celebratory’ as previous accounts have suggested. Rather, this paper draws on ethnographic data to focus attention on the micro politics of garden activism, arguing that the social backgrounds and motivations of those involved in guerrilla gardening and their relationship with other users of the space surrounding the dig site are also important—but largely underacknowledged—aspects of guerrilla gardening. This paper examines three recent high-profile cases involving gifts and benefits to police. The cases, two from Australia and one from England, involved both frontline officers and senior managers. The analyses track the unfolding scandals, and how they were investigated and evaluated by official inquiries. In two of the cases, gifts and hospitality were enmeshed with wider forms of corruption. The official inquiries identified how gratuities undermined public confidence in the impartiality of police, and how inappropriate gifts and benefits were facilitated by liberal policies and deficient leadership. Starting in 2002, the UK Labour Government of 1997–2010 introduced a series of changes to the National Health Service (NHS) in England designed to increase patients’ choices of the place of elective hospital care and encourage competition among public and private providers of elective hospital services for NHS-funded patients. In 2006, the Department of Health initiated the Health Reform Evaluation Programme (HREP) to assess the impact of the changes. In June 2008, the White Paper, High quality care for all, was published. It represented the government’s desire to focus the next phase of health care system reform in England as much on the quality of care as on improving its responsiveness and efficiency.The 2008 White Paper led to the commissioning of a further wave of evaluative research under the auspices of HREP, as follows:an evaluation of the implementation and outcomes of care planning for people with long-term conditions;an evaluation of the personal health budget pilots;an evaluation of the implementation and outcomes of the Commissioning for Quality and Innovation (CQUIN) framework; andan evaluation of cultural and behavioural change in the NHS focused on ensuring high quality care for all.This Supplement includes papers from each project.The evaluations present a mixed picture of the impact and success of the 2008 reforms. All the studies identify some limitations of the policies in the White Paper. The introduction of personal health budgets appears to have been the least problematic and, depending on assumptions, likely to be cost-effective for the sorts of patients involved in the pilot. For the rest of the changes, impacts ranged from little or none (CQUIN and care planning for people with chronic conditions) to patchy and highly variable (instilling a culture of quality in acute hospitals) in the three years following the publication of the White Paper. On the other hand, each of the studies identifies important insights relevant to modifying and improving the policies. These findings have continuing relevance since both the 2008 White Paper’s policies, and the issues they were focused on remedying, remain central to the current Coalition Government’s reform agenda. This paper considers the origins and traces the history of the Social Fund, an ideologically-motivated social policy departure of the Thatcher era. It identifies the controversy, in research communities amongst others, which surrounded the establishment of the Fund, and considers the evidence of recurring difficulties in its practical operation over a thirty-year period. It then turns to the Coalition administration in Westminster’s decision to divest itself of responsibility for key parts of the Fund, devolving such obligations to local authorities in England, the Parliament in Scotland and the Assembly in Wales. The paper sets out a comparison of the approaches being developed to discharge the Fund’s prior responsibilities in the three nations. It concludes that the essential policy thrust lies in a determination to roll back central state obligations in poverty relief and income maintenance which had hitherto been accepted by all post-war administrations. New relationships between service users and the welfare state have emerged as a result of governmental strategies of public service reform in which participation has appeared as the cure for a putative welfare dependency. A new public has been invoked in technologies of governance which have conflated responsible citizenship with participation in the marketplace and have aimed to change the behaviour of welfare service users accordingly. This paper investigates the ability of welfare service users to resist, or amend, the disciplinary intentions of these discourses, to constitute ‘counter-publics’, and to formulate their own visions of public services. Drawing on research with English social housing tenants engaged in participation with their quasi-public landlords, and applying a theoretical framework based on the work of feminist and queer theorist Judith Butler, the paper explores the behavioural effects of participation on tenants and evidences their use of consumerist and communitarian discourses to construct alternative perceptions of a ‘public’, and re-imagine their relationship with public services. This paper examines the ways in which young women articulated strategies of resistance to internalised abortion stigma. It does so through secondary analysis of young women’s narratives from two qualitative studies in England and Wales. Whilst participants felt stigmatised by their abortion[s] in different ways, many also resisted stigmatisation. They did this through different stigma resistance strategies that were shaped by a number of different interactions: their socio-economic situations, family and relationships contexts, the circumstances in which they became pregnant, and their beliefs and values with respect to abortion and motherhood. Being able to construct their abortion decision as morally sound was an important element of stigma resistance. Although socio-cultural norms and values on abortion, reproduction, and motherhood were shown to constrain women’s reproductive choices, these norms were all open to challenge. In this paper, I highlight key differences between a discourse analytic approach to women’s accounts of abortion and that taken by the growing body of research that seeks to explore and measure women’s experiences of abortion stigma. Drawing on critical analyses of the conceptualisation of stigma in other fields of healthcare, I suggest that research on abortion stigma often risks reifying it by failing to consider how identities are continually re-negotiated through language-use. In contrast, by attending to language as a form of social action, discursive psychology makes it possible to emphasise speakers’ capacity to construct “untroubled” (i.e. non-stigmatised) identities, while acknowledging that this process is constrained by the contexts in which talk takes place. My analysis applies these insights to interviews with women concerning their experiences of having an abortion in England. I highlight three forms of discursive work through which women navigate “trouble” in their accounts of abortion, and critically consider the resources available for meaning-making within this particular context of talk. A central element in the privatization of council housing has been the development of stock transfer policy. A variety of perspectives on this process have been explored including the impact on accountability relations; however, the tenants’ experience is almost completely absent from this literature. The paper develops a case study that draws on the experience of the tenants involved in a stock transfer. In the process stock transfers, and related accountability relations, are shown to be contested with tenant-led campaigns challenging this neoliberal inspired policy. The case study illustrates the power and financial resource asymmetries in transfer campaigns with a range of anti-democratic tactics employed by those pursuing the transfer. On the basis of a critique of neoliberalism, the stock transfer process is seen as an attack on the previous democratic control of council housing, which is replaced with ‘governance by experts and elites’ and private sector inspired corporate governance forms of accountability. Thus the paper seeks to answer two questions; how democratic is the transfer process and what are the long-term implications for democratic accountability in the social housing sector.
