Forty-three children were treated for peripheral arterial injuries. Single injuries of the radial, ulnar, anterior or posterior tibial arteries with good collateral supply were best treated by ligation. Injuries about the elbow or knee, associated with fractures, carried the highest morbidity. Early arteriography, prompt exploration in doubtful cases, and a high level of expertise are essential for good results in these two areas.

Phenobarbital has infrequently been reported to cause hepatitis. The previously reported cases of barbiturate-induced jaundice are reviewed and data from two previously unreported cases are presented. A discussion of mechanisms of iatrogenic hepatotoxicity is presented with emphasis on idiosyncratic mechanisms. Hepatitis secondary to phenobarbital administration appears to be due to a hypersensitivity reaction. This mechanism seems most likely because of the commonly seen clinical picture of rash, fever and eosinophilia. Speculation is made that phenobarbital hepatitis is possibly more common than the small number of reported cases would indicate. Criteria for the assessment and treatment of this iatrogenic disease are presented.

Basic concepts and formulation of the speech interference level (SIL) measure are discussed and the implications of the use of SIL to measure aircraft cabin environment are analyzed. Intelligibility tests with both words and phrases indicate that serious interference with speech can be demonstrated by adding supposedly unimportant frequencies to the SIL criterion masking band. Innocuous effects are also demonstrated by masking with low frequencies alone. Conclusions are drawn that because of the complex nature of a propeller-driven aircraft acoustic environment, SIL criteria, without qualification, are unacceptable for measuring either intelligibility or comfort.

ConclusionsHydroxypyruvic aldehyde in the presence of disodium phosphate acted, within the limits set forth in these experiments, as an effective antidote against mercuric chloride poisoning in rabbits and cats.

A case of acute renal failure, hepatic dysfunction and skin and mucous membrane eruption following a course of sulfamethoxazole therapy is reported. Although several cases of multisystem reactions to long acting sulfonamides have been reported, this case describes the first such reaction to sulfamethoxazole. The subsequent fatal outcome of this case demonstrates the potential significance of sulfamethoxazole hypersensitivity reactions.

SummaryPropenyl ethyl ether produces prompt surgical anesthesia in many species of animals upon inhalation. The anesthetic index is higher than ethyl ether and the compound appears to be without deleterious action upon the myocardium, the normal histology of the liver or kidney, or upon the red blood cell in anesthetic concentrations. However, this ether in common with its isomer, cypreth ether, produces irritation of the upper respiratory tract in animals and especially in man. For this reason we are of the opinion that the compound is unsuited as a volatile anesthetic in man.

Specialty credentialing has been discussed extensively and debated within pharmacy for more than ten years. Within the profession, there now appears to be a consensus on the need for and appropriateness of acknowledging professional practice areas as unique, defined entities. However, there remain substantive differences on how the definition of specialty practice should be constructed. One approach is to identify practice areas by functional activity (e.g., nuclear pharmacy, drug information) and/or therapeutic focus (e.g., psychopharmacy, clinical pharmacokinetics). A second approach is to define clinical pharmacy as a specialty practice that would, at least initially, coalesce clinical practitioners with the common denominator of an active role in the therapeutic decision-making process. The following document was developed by the Committee on Clinical Pharmacy as a Specialty (CCPS). The CCPS is independent of any formal organizational affiliations. It will serve as the preface to a petition that will be submitted to the Board of Pharmaceutical Specialties requesting recognition of clinical pharmacy as a specialty. The intention of the committee is to resolve the question of specialty status for clinical pharmacy through the Board of Pharmaceutical Specialties' review process. It is then hoped that pharmacy can move forward on the important issue of specialty credentialing.
