■ Nepal has an isolated population where diarrhoeal disease is one of the main causes of infant death.■ The distinctions made about causes and consequences of abdominal problems are considered.■ Cognitive mapping was used to elicit mothers' ideas of diarrhoea, constipation and vomiting.■ Literacy programmes have been introduced.■ Diagrammatic research can be useful in a culture which is predominantly oral in tradition.
