目的 观察老年帕金森病(PD)患者认知功能的变化情况,并探讨其相关影响因素. 方法 选取老年(年龄≥60岁) PD患者80例. 门诊随访2年,由专职人员详细记录患者一般资料与量表结果. 量表评估包括统一帕金森病评定量表(UPDRS)、简易智能精神状态量表(MMSE)、中国版韦氏成人智力量表(WAIS-RC)、中国版韦氏记忆量表(WMS)、汉密尔顿抑郁量表(HAMD)和蒙特利尔认知评估量表(MoCA). 结果 随访结束时,认知功能正常老年PD患者比例显著降低(P<0.05),轻度认知功能障碍老年PD患者比例显著升高(P<0.05),认知功能障碍率显著升高(P<0.05);MMSE评分和MoCA评分均显著降低(P均<0.05);MoCA指标中,除视空间、执行功能和定向外,余各项指标均显著降低(P均<0.05);MMSE指标中,除阅读理解、书写能力和图形描画外,余各项指标均显著降低(P均<0.05). Logistic回归分析结果显示,病程和运动亚型对老年PD患者认知功能影响显著(P均<0.05). 结论 老年PD患者认知功能下降明显,病程和运动亚型对老年PD患者认知功能影响较大. [ABSTRACT FROM AUTHOR]
