Purpose: To compare attempted versus achieved corneal stromal thickness reduction in a consecutive case series of patients undergoing laser in situ keratomileusis (LASIK) in one eye and small incision lenticule extraction (SMILE) in the other eye., Methods: This prospective, randomized, contralateral eye study included 22 consecutive patients (44 eyes), one eye randomized to have myopic LASIK and the contralateral eye to have SMILE. Anterior segment optical coherence tomography was performed preoperatively and at 3 months postoperatively. For each of the treatment groups, the achieved maximum stromal thickness reduction was compared to the planned/attempted thickness. The deviation of planned versus achieved stromal thickness reduction was then compared between the two groups., Results: At 3 months postoperatively, LASIK had a lower difference between planned versus attempted stromal thickness reduction compared to SMILE (13.72 ± 14.45 vs 24.00 ± 19.45 µm, P = .03). Graphical analysis revealed this deviation to be exaggerated in higher myopic errors, when a higher maximum stromal reduction was planned. The mean stromal reduction thickness was 83.40 ± 29.52 µm achieved versus 97.13 ± 25.69 µm mean planned ablation depth in the LASIK group ( P < .001) compared to 76.45 ± 29.69 µm achieved versus 100.45 ± 26.56 µm planned ablation depth in the SMILE group ( P < .001)., Conclusions: LASIK had a significantly lower difference between planned versus achieved stromal thickness reduction when compared to SMILE ( P = .03). Purpose: To investigate possible refractive changes in pregnant women who have previously undergone myopic laser in situ keratomileusis (LASIK)., Methods: This prospective study included 64 pregnant women (128 eyes) who had previously undergone bilateral myopic LASIK. Uncorrected distance visual acuity (UDVA), spherical equivalent, corneal keratometry, topography and tomography, and epithelial mapping were evaluated before LASIK, 12 months after LASIK, during the third trimester of pregnancy, and 1 year postpartum., Results: The average age at the time of recruitment was 32.5 ± 5 years (range: 24.5 to 39.5 years). The evaluation during pregnancy was at an average 55 months (range: 12 to 108 months) from the initial LASIK procedure. The preoperative mean refractive error was -6.72 ± 2.96 diopters (D) (range: -1.00 to -11.00 D). Comparing 12-month refractive data after the original LASIK procedure to the those within the third trimester of the subsequent pregnancy, the average UDVA was 20/18, the residual refractive error was -0.63 ± 1.00 to -0.51 ± 0.82 D, the steepest keratometry value was 40.85 ± 1.89 to 40.88 ± 2.84 D, and the central epithelial thickness was 56.4 ± 3.89 to 57.38 ± 5.04 µm, respectively., Conclusions: In this study, pregnancy did not affect the refractive stability of LASIK. Pregnancy-related changes in refractive error, corneal stability, and total corneal and epithelial thickness in women after LASIK were not found to be statistically significant. Purpose: To report the safety and long-term efficacy of topography-guided partial-refraction PRK combined with corneal cross-linking (CXL) (the Athens Protocol), refractive, topographic, and pachymetric changes of keratoconic eyes treated were studied., Methods: Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), refraction, keratometry, qualitative and quantitative assessment of corneal keratometric, and pachymetric properties recorded by topography and tomography were evaluated for 10 years postoperatively., Results: A total of 144 eyes were followed up for a mean 128 ± 4 months (range: 120 to 146 months). Mean UDVA markedly improved at 1 year (0.19 ± 0.17 to 0.53 ± 0.21 decimal) with further improvement to 0.55 ± 0.19 decimal at 10 years. CDVA increased from 0.59 ± 0.21 to 0.80 ± 0.17 decimal at 1 year and further to 0.81 ± 0.19 decimal at 10 years. Corneal thickness decreased from 468.74 ± 35.05 to 391.14 ± 40.07 μm at 1 year (P < .01) and up to 395.42 ± 32.21 μm at 10 years. Steep keratometry decreased from 50.57 ± 2.80 to 45.87 ± 2.70 diopters (D) (P < .01) at 1 year and to 44.00 ± 3.22 D at 10 years. Maximum keratometry decreased from 53.43 ± 2.97 to 46.17 ± 1.18 D at 1 year and 44.75 ± 2.14 D at 10 years (P < .01). A total of 94.4% demonstrated ectasia stabilization and 3.5% showed progressive "overcorrection" or "hyperopic" shift., Conclusions: The Athens Protocol confirms long-term safety and efficacy for corneal ectasia and visual function. Purpose: To compare safety and efficacy of topography-guided LASIK and contralateral eye SMILE for myopia and myopic astigmatism correction., Methods: This prospective, randomized contralateral eye study included 44 eyes of 22 patients with bilateral myopia or myopic astigmatism. Treated eyes were divided into two groups: 22 eyes were treated with topography-guided LASIK and the fellow eye of each patient was treated with SMILE. The following parameters were evaluated preoperatively and up to 3 months postoperatively: uncorrected distance vision acuity (UDVA), corrected distance vision acuity (CDVA), refractive error, corneal keratometry, contrast sensitivity, and Objective Scatter Index., Results: At 3 months, 86.4% of the LASIK group and 68.2% of the SMILE group had UDVA of 20/20 (P < .002) and 59.1% and 31.8%, respectively, had UDVA of 20/16 (P < .002). Spherical equivalent refraction (±0.50 D) was 95.5% for the LASIK group and 77.3% for the SMILE group (P < .002). Residual refraction cylinder (≤ 0.25 D) was 81.8% for the LASIK group and 50% for the SMILE group (P < .001). Contrast sensitivity (6 cycles/degree) was 7.2 ± 1.01 in the LASIK group and 6.20 ± 1.52 in the SMILE group. Objective Scatter Index measurements at 3 months were 1.35 in the LASIK group and 1.42 in the SMILE group., Conclusions: Topography-guided LASIK was superior in all visual performance parameters studied, both subjective and objective. The main difference between the two techniques likely derives from the eye tracking, cyclorotation compensation, and active centration control in the LASIK technology studied in contrast to the current technology available with SMILE-like procedures. Purpose: To comparatively investigate the efficacy of the enhanced Athens Protocol procedure guided by novel Placido-derived topography with cyclorotation compensation (the cyclorotation adjusted group) to similar cases guided by Scheimpflug-derived tomography without cyclorotation compensation (the non-cyclorotation adjusted group)., Methods: Two groups were evaluated: the cyclorotation adjusted group (n = 110 eyes) and the non-cyclorotation adjusted group (n = 110 eyes). Analysis was based on digital processing of Scheimpflug imaging derived curvature difference maps preoperatively and 3 months postoperatively. The vector (r, ϑ) corresponding to the steepest corneal point (cone) on the preoperative surgical planning map (rp, ϑp) and on the curvature difference map (rd, ϑd) were computed. The differences between the peak topographic angular data (Δϑ = |ϑp - ϑd|) and weighted angular difference (WΔϑ = Δϑ × Δr) were calculated., Results: For the cyclorotation adjusted group, Δϑ was 7.18° ± 7.53° (range: 0° to 34) and WΔϑ was 3.43 ± 4.76 mm (range: 0.00 to 21.41 mm). For the non-cyclorotation adjusted group, Δϑ was 14.50° ± 12.65° (range: 0° to 49°) and WΔϑ was 10.23 ± 15.15 mm (range: 0.00 to 80.56 mm). The cyclorotation adjusted group appeared superior to the non-cyclorotation adjusted group, in both the smaller average angular difference between attempted to achieved irregular curvature normalization and in weighted angular difference, by a statistically significant margin (Δϑ: P = .0058; WΔϑ: P = .015)., Conclusions: This study suggests that employment of the novel Placido-derived topographic data of highly irregular corneas, such as in keratoconus, treated with topography-guided profile with cyclorotation compensation leads to markedly improved cornea normalization., (Copyright 2015, SLACK Incorporated.)

Purpose: To evaluate corneal stromal thickness reduction and compare to attempted and achieved ablation depth in a consecutive case series study of myopic LASIK., Methods: Stromal thickness reduction was retrospectively evaluated in 205 consecutive eyes of 205 patients undergoing myopic and myopic astigmatic LASIK. Anterior segment optical coherence tomography was performed preoperatively and 3 months postoperatively. Epithelial thickness remodeling was also accounted for to achieve objective stromal thickness reduction. The derived maximum stromal thickness reduction was then compared to the programmed (planned) maximum ablation depth. Deviation of planned versus achieved maximum stromal thickness changes was correlated with residual refractive error., Results: The 3-month stromal reduction was 86.01 ± 28.28 µm, compared to the average programmed maximum ablation depth of 88.48 ± 26.05 µm. Purpose: To objectively define the effective centration of myopic femtosecond laser-assisted LASIK ablation pattern, evaluate the difference between achieved versus planned excimer laser ablation centration, and compare these results from two different generations of an excimer laser system., Methods: The study retrospectively evaluated 280 eyes subjected to myopic LASIK. Digital image analysis was performed on Scheimpflug sagittal curvature maps (difference of preoperative to postoperative). Centration was assessed via proprietary software digital analysis of the coordinate displacement between the achieved ablation geometric center and the planned ablation center, which was the corneal vertex. Results from two different excimer laser generations (Eye-Q 400 [140 eyes] and EX500 [140 eyes]; Alcon/WaveLight, Fort Worth, TX) were compared., Results: Radial displacement was on average 360 ± 220 µm (range: 0 to 1,030 µm) in the Eye-Q 400 laser group and 120 ± 110 µm (range: 0 to 580 µm) in the EX500 laser group (P < .01). The percentage of eyes with displacement of greater than 300 µm was 52% in the Eye-Q 400 laser group and 4% in the EX500 laser group., Conclusions: Displacement of ablation pattern may depend on the laser platform used. The improvement in the efficiency of centration indicates that newer generation excimer lasers with faster eye tracking and active centration control appear to achieve a significantly more accurate centration of myopic ablation patterns. Purpose: To investigate preoperative and postoperative anterior and posterior keratometry and simulated corneal astigmatism in keratoconic eyes treated with collagen cross-linking combined with anterior surface normalization by partial topography-guided excimer ablation (the Athens Protocol)., Methods: Anterior and posterior corneal keratometry were measured by Scheimpflug imaging for 267 untreated keratoconic eyes. Following treatment, they were assessed 1 year postoperatively., Results: Before treatment, average anterior keratometric value was 47.06 ± 6.02 diopters (D) for flat and 51.24 ± 6.75 D for steep. The posterior keratometric values were -6.70 ± 0.99 D (flat) and -7.67 ± 1.15 D (steep). Anterior astigmatism was on average with-the-rule (-1.97 ± 6.21 D), whereas posterior astigmatism was against-the-rule (+0.53 ± 1.02 D). The posterior and anterior astigmatism were highly correlated (r(2) = 0.839). After treatment, anterior keratometric values were 43.97 ± 5.81 D (flat) and 46.55 ± 6.82 D (steep). Posterior keratometric values were -6.58 ± 1.05 D (flat) and -7.69 ± 1.22 D (steep). Anterior astigmatism was on average with-the-rule (-1.56 ± 3.80 D), whereas posterior astigmatism was against-the-rule (+0.45 ± 1.29 D). The statistically significant (P < .05) keratometric changes indicated anterior surface flattening -3.09 ± 2.67 D (flat) and -4.19 ± 2.96 D (steep). The posterior keratometric changes were not statistically significant (P > .05)., Conclusions: Before treatment, there was a strong correlation between posterior and anterior corneal astigmatism. After treatment, statistically significant anterior keratometric values flattened. Purpose: To map corneal and epithelial layer thickness changes following cataract removal surgery employing a spectral-domain anterior-segment optical coherence tomography system., Methods: Corneal and epithelial thickness three-dimensional profile distribution was clinically imaged preoperatively and up to 3 months postoperatively with anterior-segment optical coherence tomography in 116 consecutive cases. Descriptive statistics investigated central corneal thickness, minimum corneal thickness, and epithelial thickness at the central 2-mm area, the mean over the 6-mm area, and mid-peripherally at 5-mm ring., Results: In comparison to preoperative, the center, mean, and mid-peripheral epithelial thickness at the first postoperative day increased by +2.84, +2.35, and +2.25 μm, respectively (P < .001, < .001, and = .0014). One week postoperatively, the epithelial thickness differences were -1.91, -2.62, and -2.76 μm, respectively (P < .001, < .001, and < .001). Four weeks postoperatively, the differences of -0.20, -0.59, and -0.66 μm for the center, mean, and mid-periphery were not statistically significant (P = .6449, .1512, and .11097). Three months postoperatively, the differences were -0.05, -0.28, and -0.09 μm, respectively (P = .8722, .2341, and .6431)., Conclusions: Qualitative and quantitative assessment of epithelial remodeling following cataract removal indicated that the early (1 day and 1 week) corneal and epithelial thickness returned to the preoperative baseline 4 weeks postoperatively. Purpose: To evaluate epithelial thickness profile changes following myopic femtosecond laser-assisted LASIK in relation to the degree of myopia corrected, evaluated with a spectral-domain anterior-segment optical coherence tomography system., Methods: Sixty-one consecutive cases were observed for corneal epithelial thickness distribution preoperatively and at 1 day, 1 week, 1 month, and 1 year postoperatively. Epithelial thickness mapping was obtained with a spectral-domain optical coherence tomography system (Optovue Inc., Fremont, CA). Descriptive statistics investigated epithelial thickness at the central 2-mm area, the mean over the central 6-mm area, and mid-peripherally at the 5-mm ring area., Results: Preoperatively, the pupil center epithelial thickness was 51.67 ± 2.57 μm (range: 45 to 56 μm), mean was 51.76 ± 2.66 μm (range: 45 to 57 μm), and mid-periphery was 51.78 ± 2.71 μm (range: 46 to 57 μm). Compared to the preoperative values, the epithelial thickness for the center, mean, and mid-periphery was −0.30, +1.07, and +1.35 μm at 1 week, +1.58, +2.88, and +3.31 μm at 1 month (P = .0036, < .001, and < .001), and +1.42, +2.90, and +3.19 μm at 1 year postoperatively (P = 0.146, < .001, and < .001), respectively. The correlation analysis between the epithelial thickness increase and the spherical equivalent of myopic correction showed a trend toward epithelial thickness increase with the amount of myopic ablation, particularly at the mid-peripheral 5-mm area., Conclusions: In this comprehensive study of postoperative corneal epithelial thickness remodeling following femtosecond laser-assisted myopic LASIK correction, an increase at the 1-month and up to 1-year postoperative interval suggested postoperative epithelial activity in connection to the extent of ablation.

Purpose: To investigate refractive, topometric, pachymetric, and visual rehabilitation changes induced by anterior surface normalization for keratoconus by partial topography-guided excimer laser ablation in conjunction with accelerated, high-fluence cross-linking., Methods: Two hundred thirty-one keratoconic cases subjected to the Athens Protocol procedure were studied for visual acuity, keratometry, pachymetry, and anterior surface irregularity indices up to 3 years postoperatively by Scheimpflug imaging (Oculus Optikgeräte GmbH, Wetzlar, Germany)., Results: Mean visual acuity changes at 3 years postoperatively were +0.38 ± 0.31 (range: -0.34 to +1.10) for uncorrected distance visual acuity and +0.20 ± 0.21 (range: -0.32 to +0.90) for corrected distance visual acuity. Mean K1 (flat meridian) keratometric values were 46.56 ± 3.83 diopters (D) (range: 39.75 to 58.30 D) preoperatively, 44.44 ± 3.97 D (range: 36.10 to 55.50 D) 1 month postoperatively, and 43.22 ± 3.80 D (range: 36.00 to 53.70 D) up to 3 years postoperatively. The average Index of Surface Variance was 98.48 ± 43.47 (range: 17 to 208) pre-operatively and 76.80 ± 38.41 (range: 7 to 190) up to 3 years postoperatively. The average Index of Height Decentration was 0.091 ± 0.053 μm (range: 0.006 to 0.275 μm) preoperatively and 0.057 ± 0.040 μm (range: 0.001 to 0.208 μm) up to 3 years postoperatively. Mean thinnest corneal thickness was 451.91 ± 40.02 μm (range: 297 to 547 μm) preoperatively, 353.95 ± 53.90 μm (range: 196 to 480 μm) 1 month postoperatively, and 370.52 ± 58.21 μm (range: 218 to 500 μm) up to 3 years postoperatively., Conclusions: The Athens Protocol to arrest keratectasia progression and improve corneal regularity demonstrates safe and effective results as a keratoconus management option. Purpose: To evaluate safety, efficacy, ergonomy, and refractive and keratometric stability in high myopia LASIK procedures using a novel femtosecond and excimer laser surgery platform., Methods: One hundred sixteen eyes in consecutive cases of high myopic LASIK ( ≥ -6.00 diopters [D]) with the Alcon-WaveLight FS200 femtosecond and EX500 excimer lasers (Alcon Laboratories, Fort Worth, TX) were evaluated preoperatively and postoperatively for the following parameters: refractive error, corrected distance visual acuity, uncorrected distance visual acuity, spherical equivalent correction, keratometry (with Placido topography and Scheimpflug tomography), and refractive astigmatism. Average follow-up time was 6.2 months (range: 3 to 12 months)., Results: Postoperative average refractive error was -0.37, -0.43, and -0.25 D for the 3-, 6-, and 12-month period, compared to -7.67 ± 1.55 D preoperatively. At 3, 6, and 12 months postoperatively 94%, 96.3%, and 100% of eyes, respectively, were within 1.0 D defocus equivalent. Postoperative refractive astigmatism was -0.21, -0.21, -0.13 D for the 3-, 6-, and 12-month period compared to -1.07 ± 1.91 D preoperatively. The proportion of eyes with postoperative astigmatism within 0.25 D was 85.3%, 81.5%, and 100%, for the 3-, 6-, and 12-month visit, respectively. Keratometric stability was within 0.22 D after the 12-month visit. There was no epithelial ingrowth or diffuse lamellar keratitis in any case., Conclusions: Clinical outcomes with this technique and technology appear to be promising in high level uncorrected visual rehabilitation of high myopia. Purpose: To evaluate the potential influence of corneal cross-linking (CXL) with ultraviolet-A light and riboflavin in keratoconic eyes on several objective parameters of dry eye syndrome., Methods: This prospective single center study included 30 consecutive eyes of 16 patients that underwent CXL with riboflavin and ultraviolet-A treatment (epithelial removal, 30 minutes soaking with riboflavin, 30 minutes of illumination with 365 nm, 3 mW/cm(2), 5 cm distance). Several dry eye syndrome parameters were evaluated preoperatively and 3 and 6 months after the procedure: intra-individual comparison of fluorescein and Rose bengal staining, height of tear film meniscus, and tear film break-up time., Results: Pathologic staining (more than 10 point-shaped areas or diffuse staining) with fluorescein was evident before CXL in 1 eye, 3 months after CXL in 1 eye, and 6 months after CXL in 1 eye. Rose bengal staining 3 and 6 months postoperatively was comparable to preoperative staining. Tear film height was reduced in 2 eyes before CXL and in 4 eyes 3 months postoperatively, and was normal in all eyes after 6 months. Purpose: To report a novel application of collagen cross-linking (CXL) in refractive astigmatic enhancement of previously performed astigmatic keratotomy., Methods: A 28-year-old woman with prior history of bioptics correction of high myopic astigmatism with femtosecond laser-assisted astigmatic keratotomy followed by topography-guided LASIK showed long-term regression of the astigmatism 4 years later. A novel CXL application was employed in an attempt to reverse the regression of the astigmatic keratotomy., Results: The high fluence CXL intervention resulted in correction of 2 diopters of topographic and refractive cylinder. Uncorrected distance visual acuity changed from 20/50 to 20/20 and refraction from -0.50 -2.00 @ 90 to +0.25 -0.25 @ 90 at the 7-month follow-up., Conclusions: A possible novel application of high fluence CXL with refractive cornea effect is introduced. Purpose: To compare and correlate central corneal thickness (CCT), anterior chamber depth (ACD), and keratometric (flat and steep K) measurements using two anterior segment imaging methods, a Scheimpflug camera system (Oculyzer II [Oculus Optikgeräte GmbH]) and a partial coherence biometry system (WaveLight OB820 [Alcon Laboratories Inc]) in eyes undergoing cataract surgery., Methods: Ninety patients (mean age: 66±13 years [range: 32 to 88 years]) underwent preoperative measurement of CCT, ACD, and keratometric measurements by Scheimpflug tomography (Oculyzer II) and optical low coherence reflectometry (WaveLight OB820). Interdevice agreement and correlation between the two techniques were assessed., Results: All measurements were highly correlated and showed no clinically significant difference between methods. Mean CCT was 554.21±39.07 μm and 546.59±37.75 μm for the Oculyzer II and WaveLight OB820, respectively (R²=0.9268). Mean ACD was 2.63±0.44 mm and 2.63±0.43 mm for the Oculyzer II and WaveLight OB820, respectively (R(2)=0.9488). The principal meridian keratometric values were also highly correlated. Mean flat K was 42.88±1.50 diopters (D) and 42.96±1.40 D for the Oculyzer II and WaveLight OB820, respectively (R²=0.8741). Mean steep K was 44.08±1.79 D and 44.26±1.95 D for the Oculyzer II and WaveLight OB820, respectively (R(2)=0.9159)., Conclusions: Our data show that the Oculyzer II and WaveLight OB820 provide measurements that are in agreement with published values for CCT and ACD in patients. Excellent agreement for CCT and ACD was found between the two devices, as demonstrated by a high degree of correlation and linearity, in addition to minimal bias. Thus, CCT, ACD, and K measurements by these instruments can both be used in clinical preparation, and their agreement is an ensuring precision factor for cataract and refractive surgeons., (Copyright 2012, SLACK Incorporated.)

Purpose: To evaluate the safety and efficacy of intrastromally applied collagen cross-linking (CXL) in a comparative contralateral eye study of topography-guided femtosecond laser-assisted hyperopic LASIK., Methods: Thirty-four consecutive patients with hyperopia and hyperopic astigmatism elected to have bilateral topography-guided LASIK and were randomized to receive a single drop of 0.1% sodium phosphate riboflavin solution under the flap followed by 3-minute exposure of 10 mW/cm2 ultraviolet A (UVA) light with the flap realigned in one eye (CXL group) and no intrastromal CXL in the contralateral eye (no CXL group). All eyes were treated with the WaveLight FS200 femtosecond laser and WaveLight EX500 excimer laser (Alcon Laboratories Inc). Refractive error and keratometric, topographic, and tomographic measurements were evaluated over mean follow-up of 23 months., Results: Preoperatively, mean spherical equivalent refraction was +3.15 +/- 1.46 diopters (D) and +3.40 +/- 1.78 D with a mean cylinder of 1.20 +/- 1.18 D and 1.40 +/- 1.80 D and mean uncorrected distance visual acuity (UDVA) (decimal) of 0.1 +/- 0.26 and 0.1 +/-0.25 in the CXL and no CXL groups, respectively. At 2 years postoperatively, mean spherical equivalent refraction was -0.20 +/- 0.56 D and +0.20 +/- 0.40 D with mean cylinder of 0.65 +/- 0.56 D and 0.76 +/- 0.72 D and mean UDVA of 0.95 +/- 0.15 and 0.85 +/- 0.23 in the CXL and no CXL groups, respectively. Eyes with CXL demonstrated a mean regression from treatment of +0.22 +/- 0.31 D, whereas eyes without CXL showed a statistically significant greater regression of +0.72 +/- 0.19 D (P = .0001)., Conclusions: Topography-guided hyperopic LASIK with or without intrastromal CXL is safe and effective, with greater long-term efficacy (less regression) in eyes with CXL. Our data suggest that the regression seen with hyperopic LASIK may be related to biomechanical changes in corneal shape over time.

Purpose: To evaluate a series of patients with corneal ectasia after LASIK that underwent the Athens Protocol: combined topography-guided photorefractive keratectomy (PRK) to reduce or eliminate induced myopia and astigmatism followed by sequential, same-day ultraviolet A (UVA) corneal collagen cross-linking (CXL)., Methods: Thirty-two consecutive corneal ectasia cases underwent transepithelial PRK (WaveLight ALLEGRETTO) immediately followed by CXL (3 mW/cm(2)) for 30 minutes using 0.1% topical riboflavin sodium phosphate. Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), manifest refraction spherical equivalent, keratometry, central ultrasonic pachymetry, corneal tomography (Oculus Pentacam), and endothelial cell counts were analyzed. Mean follow-up was 27 months (range: 6 to 59 months)., Results: Twenty-seven of 32 eyes had an improvement in UDVA and CDVA of 20/45 or better (2.25 logMAR) at last follow-up. Four eyes showed some topographic improvement but no improvement in CDVA. One of the treated eyes required a subsequent penetrating keratoplasty. Corneal haze grade 2 was present in 2 eyes., Conclusions: Combined, same-day, topography-guided PRK and CXL appeared to offer tomographic stability, even after long-term follow-up. Only 2 of 32 eyes had corneal ectasia progression after the intervention. Seventeen of 32 eyes appeared to have improvement in UDVA and CDVA with follow-up >1.5 years. Purpose: To follow the stability of a simultaneously delivered therapy that corrects aberrations and stiffens the corneal collagen of eyes with progressive keratoconus., Methods: Two patients with progressive keratoconus underwent partial treatment (70% cylinder and sphere up to 50-μm central depth) with topographic customized photorefractive keratectomy (PRK) using the T-CAT module of the ALLEGRETTO WAVE Eye-Q excimer laser (Alcon Laboratories Inc), and then immediate corneal collagen cross-linking (CXL) with riboflavin 0.1% drops every 2 minutes while exposed to mean 365-nm ultraviolet A (UVA) light at 3.0 mW/cm² for 30 minutes (the Athens Protocol). Pre- and postoperative evaluations included manifest and cycloplegic refraction, Scheimpflug corneal tomography and pachymetry, and slit-lamp examination of corneal clarity with a minimum follow-up of 30 months., Results: Both treated eyes experienced rapid healing of the epithelial surface within 5 days and progressive improvement of vision. In the first case, partial treatment reduced the astigmatism and aberrations, allowing for successful soft contact lens wear at 3 months. Follow-up at 13, 19, 30, and 36 months showed progressive reduction of refractive myopia and keratometric power. In the second case, laser treatment led to a near emmetropic refraction with an uncorrected visual acuity of 20/20 at 3 months, which remained unchanged at 21 and 30 months postoperative., Conclusions: Partial topography-guided PRK followed by riboflavin/UVA CXL is a safe and effective therapy that halts the progression of keratoectasia and reduces the spherocylindrical refraction and aberrations to improve the visual function of patients with progressive keratoconus. Purpose: To evaluate the safety and efficacy of a novel femtosecond laser-assisted technique for intrastromal administration of riboflavin and higher fluence ultraviolet a (UVA) light in collagen cross-linking (CXL) for keratoconus., Methods: Ten eyes with early keratoconus were treated with CXL and followed for a mean of 26 months (range: 18 to 36 months). Using a femtosecond laser, a 100-microm deep, 7-mm diameter intrastromal pocket was created. Two 0.1-mL doses of 0.1% riboflavin solution were infused into the pocket and the cornea was irradiated with 7 mW/cm2 UVA light of mean 370 nm wavelength for 15 minutes., Results: Mean uncorrected visual acuity improved from 20/40.5 to 20/32.5 best spectacle-corrected visual acuity was unchanged at 20/20, mean sphere was reduced by 0.50 diopters (D), mean cylinder was reduced by 0.90 D, and maximum mean keratometry (K) reduced from 48.7 to 47.90 D. No ectasia progression (defined as increase in K over 3-month follow-up) and no statistically significant change in endothelial cell count was noted during follow-up. The mean thinnest corneal thickness appeared to initially reduce but the mean returned to at least the preoperative level by 18 months. All patients returned to full activities within 1 day postoperative. No adverse effects were noted in any of the cases studies., Conclusions: This novel epithelium-sparing, rapid soak-and-treat method of intrastromal riboflavin instillation and higher fluence UVA light for CXL appears to be safe and effective. No negative biomechanical effect (ectasia/epithelial ingrowth) was noted due to the femtosecond laser-created pocket. Because minimal epithelial injury occurs using this technique, postoperative pain appears to be significantly reduced.

Purpose: The safety and efficacy of corneal collagen cross-linking (CXL) and topography-guided photorefractive keratectomy (PRK) using a different sequence and timing were evaluated in consecutive keratoconus cases., Methods: This study included a total of 325 eyes with keratoconus. Eyes were divided into two groups. The first group (n=127 eyes) underwent CXL with subsequent topography-guided PRK performed 6 months later (sequential group) and the second group (n=198 eyes) underwent CXL and PRK in a combined procedure on the same day (simultaneous group). Statistical differences were examined for pre- to postoperative changes in uncorrected (UCVA, logMAR) and best-spectacle-corrected visual acuity (BSCVA, logMAR), manifest refraction spherical equivalent (MRSE), keratometry (K), topography, central corneal thickness, endothelial cell count, corneal haze, and ectatic progression. Mean follow-up was 36+/-18 months (range: 24 to 68 months)., Results: At last follow-up in the sequential group, the mean UCVA improved from 0.9+/-0.3 logMAR to 0.49+/-0.25 logMAR, and mean BSCVA from 0.41+/-0.25 logMAR to 0.16+/-0.22 logMAR. Mean reduction in spherical equivalent refraction was 2.50+/-1.20 diopters (D), mean haze score was 1.2+/-0.5, and mean reduction in K was 2.75+/-1.30 D. In the simultaneous group, mean UCVA improved from 0.96+/-0.2 logMAR to 0.3+/-0.2 logMAR, and mean BSCVA from 0.39+/-0.3 logMAR to 0.11+/-0.16 logMAR. Mean reduction in spherical equivalent refraction was 3.20+/-1.40 D, mean haze score was 0.5+/-0.3, and mean reduction in K was 3.50+/-1.3 D. Endothelial cell count preoperatively and at last follow-up was unchanged (P<.05) in both groups. Statistically, the simultaneous group did better (P<.05) in all fields evaluated, with improvement in UCVA and BSCVA, a greater mean reduction in spherical equivalent refraction and keratometry, and less corneal haze., Conclusions: Same-day simultaneous topography-guided PRK and CXL appears to be superior to sequential CXL with later PRK in the visual rehabilitation of progressing keratoconus., (Copyright 2009, SLACK Incorporated.)

Purpose: To evaluate the safety and efficacy of staged ultraviolet A (UVA) cross-linking following intrastromal 0.1% riboflavin administration in eyes with advanced corneal edema., Methods: Ten eye bank corneas divided in two groups (n = 5) were placed on a pressurized artificial anterior chamber following Descemet's membrane stripping. Two consecutive corneal pockets (350- and 150-microm depth) were sequentially created using a femtosecond laser. Sequential intrastromal injections of 0.1% riboflavin (0.2 mL) followed by either UVA irradiation (15 mW/cm2) for 7 minutes or exposure to air were performed for each pocket. Corneal clarity and central thickness were measured before and after the two UVA cross-linking steps. The same steps were clinically applied in an 84-year-old woman with bullous keratopathy prior to corneal transplantation and followed for 6 months., Results: The corneal clarity improved in the treated but not the control eyes. The mean central corneal thickness was significantly reduced by 256 microm (ultrasound, P = .0002) and 273 miccrom (Scheimpflug, P = .0004) in treated eyes, but only 100 microm (ultrasound, P = .048) and 107 microm (Scheimpflug, P = .075) in the control eyes. The clinical treatment of corneal edema showed improved clarity and reduced central corneal thickness from 675 to 550 microm (ultrasound) and 696 to 571 microm (Scheimpflug) at 1 month. Best spectacle-corrected visual acuity improved from finger counting to 20/80 at 1 week and beyond, postponing corneal transplantation for > 6 months., Conclusions: Staged UVA cross-linking (15 mW/cm2) with femtosecond laser facilitated intrastromal 0.1% riboflavin administration may be a safe (no corneal scarring) and effective (marked reduction of edema) temporizing alternative method for managing bullous keratopathy.

Purpose: To obtain normative values of angle kappa in a normal population by synoptophore and Orbscan II and to compare the reliability of these devices., Methods: Three hundred consecutive healthy individuals were enrolled in the study. A complete orthoptic and ophthalmologic examination was performed. Synoptophore and Orbscan II corneal topography were used to measure angle kappa. To evaluate the association of the angle kappa and refraction measures, individuals were further classified according to the degree of myopia and hyperopia. The spherical equivalent error measures were grouped into six categories: > or = -3.00 diopters (D); -2.75 to -1.50 D; -1.25 to -0.50 D; +0.50 to +1.25 D; +1.50 to +2.75 D; and > or = +3.00 D. Paired t test and Pearson's correlation test were used for statistical analysis., Results: The mean age of the individuals was 28.74 +/- 1.63 years (range: 20 to 40 years). The angle kappa values obtained by synoptophore and Orbscan II were normally distributed. In the myopic group, angle kappa values decreased significantly towards negative refractive errors. In contrast, a correlation existed between large positive angles and positive refractive errors in the hyperopic group. Angle kappa values obtained by Orbscan II were significantly higher in all groups when compared to synoptophore (P < .0001). A significant correlation was noted between synoptophore and Orbscan II measurements (r = 0.932, P < .0001)., Conclusions: A significant correlation exists between positive refractive errors and large positive angle kappa values. Refractive surgeons must take into account angle kappa, especially in hyperopic patients, to avoid complications related to decentration of the ablation zone.

Purpose: To describe our clinical experience in wavefront-guided LASIK enhancements using the WaveLight ALLEGRETTO system (WaveLight Technologie AG, Erlangen, Germany) for symptomatic eyes previously treated with standard LASIK., Methods: Twenty-six eyes of 20 patients with residual myopia, hyperopia, or mixed astigmatism and/or night vision symptoms after primary standard LASIK were considered for wavefront-guided customized retreatment using the WaveLight ALLEGRETTO WAVE 200 Hz excimer laser system (model 106). Preoperative best spectacle-corrected visual acuity (BSCVA), uncorrected visual acuity, topography with the ALLEGRETTO Topolyzer, wavefront analysis using the ALLEGRETTO WAVE Tscherning Analyzer, and contrast sensitivity were compared to postoperative (enhancement) measurements., Results: Twenty-two of the original 26 eyes underwent wavefront-guided enhancement, 4 were excluded because they did not meet wavefront-guided treatment inclusion guidelines of this study. Mean follow-up was 8 months (range: 6 to 13 months, standard deviation [SD] 2). All patients were within +/- 0.50 diopters (manifest refraction) of intended postoperative refraction. The mean preoperative BSCVA improved from 20/25 (SD +/- 0.12) to 20/18 (SD +/- 0.1) postoperatively. All patients gained at least one line of BSCVA, and a maximum of three lines. There was no loss of BSCVA in any patient. The total amount of high order aberrations (RMSH) decreased from an average of 1.04 (SD +/- 0.22) to 0.46 (SD +/- 0.14) microm. Patients also had a mean improvement in low contrast sensitivity of 59%., Conclusions: Based on this small series, customized wavefront-guided enhancements using the WaveLight ALLEGRETTO system in patients who underwent previous LASIK appear to be safe and effective in correcting residual refractive error, reducing high order aberrations, and improving visual symptoms when reliable and reproducible measurements are achieved.

Purpose: To evaluate the safety and efficacy of the ALLEGRETTO WAVE excimer laser system (WaveLight Laser Technologie AG, Erlangen, Germany) in LASIK for hyperopia and hyperopic astigmatism., Methods: One hundred twenty consecutive LASIK cases for hyperopia with or without astigmatism treated with the ALLEGRETTO WAVE excimer laser were prospectively evaluated up to 12 months postoperatively. Patients were allocated into three groups according to their refractive sphere and cylinder: a low hyperopia group, with up to +3.00 diopters (D) sphere and astigmatism < or = +1.00 D (n = 52); a moderate hyperopia group with +3.25 to +5.00 D sphere and astigmatism of < or = +1.00 D (n = 45); and a high hyperopia/toric group with sphere > or = +5.25 D or cylinder > or = +1.25.D (n = 23). Flaps were created with the Moria M2 microkeratome (Moria, Antony, France). Parameters evaluated were pre- and postoperative refractive error, uncorrected visual acuity, best spectacle-corrected visual acuity (BSCVA), higher order aberration change, and contrast sensitivity., Results: One hundred twelve eyes (93%) were available for follow-up at 12 months. Of the eyes in the low hyperopia group, 92% were within +/- 0.50 D of the refractive goal. For the moderate sphere group and the high hyperopia/toric group, 79% and 71% of eyes, respectively, were within +/- 0.50 D of the refractive goal. No eye lost > or = 2 lines of BSCVA. An increase in higher order aberrations was noted in the high hyperopia/toric group from 0.47 microm (+/- 0.096) to 0.94 microm (+/- 0.167) (P < .001). No significant changes in higher order aberrations were noted in the low and moderate hyperopia groups., Conclusions: Hyperopic LASIK using the WaveLight ALLEGRETTO WAVE excimer laser appears to be safe and effective in the correction of low, moderate, and high hyperopia and hyperopic astigmatism.

Purpose: To evaluate the safety and efficacy of the Moria M2 single use 130 microkeratome head in consecutive LASIK procedures for correction of myopia and myopic astigmatism., Methods: One hundred eyes of 55 patients underwent LASIK in which the flaps were created with the Moria M2 microkeratome using the single use 130 head and excimer laser ablation was done with the Allegretto Wave-light laser. Flap parameters measured were: thickness, diameter, hinge length, and overall quality. Preoperative uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), refraction, wavefront aberrations, and low contrast sensitivity were compared to postoperative values at 6-month follow-up., Results: Mean flap thickness was 145 +/- 17.5 microm, mean flap diameter was 8.5 +/- 0.40 mm, and mean hinge cord length was 4.05 +/- 0.35 mm. At 6-month follow-up, UCVA improved from 20/200 (+/-0.24) to 20/18.5 (+/-0.12) and BSCVA improved from 20/20.5 (+/-0.18) to 20/17.5 (+/-0.11)., Conclusions: The Moria M2 single use 130 microkeratome head appears to be safe and effective in performing LASIK procedures.

Purpose: To evaluate the use of topography-guided ablations for refractive irregularities induced by previous surgery., Methods: This prospective, non-comparative trial comprised 27 symptomatic eyes with a history of LASIK for myopia that underwent topography-guided treatment with the ALLEGRETTO WAVE system. Pre- and postoperative refraction, uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), corneal asphericity (Q value), low contrast sensitivity, and patient's subjective assessment of improvement were measured., Results: Preoperative data were sphere -0.84 +/- 1.37 diopters (D), cylinder -1.55 +/- 0.78 D, UCVA 20/49 +/- 0.22, BSCVA 20/32 +/- 0.15, and Q value 1.46 +/- 0.79. Postoperative data at mean 6-month follow-up were: sphere -0.61 +/- 0.81 D, cylinder -0.53 +/- 0.58 D, UCVA 20/25 +/- 0.21 (P < .01), BSCVA 20/21 +/- 0.14 (P < .001), and Q value 1.07 +/- 0.89. Contrast sensitivity scores improved by 70%. No loss of BSCVA occurred in any patient., Conclusions: Topography-guided treatments may be effective in correcting the quality of vision. It should be viewed as a possible two-step procedure due to spherical adjustment that may change refraction unpredictably.
